Tiivistelmä. Voimistettu ääni on tärkeä työkalu esimerkiksi poliisin ja jalkapallovalmentajien työtehtävissä. Pitkään jatkunut tai toistuva äänen voimistaminen aiheuttaa kuitenkin äänihäiriöitä, jotka vaikuttavat työkykyyn. Äänenkäyttöön liittyvällä informoinnilla ja koulutuksella kyetään vaikuttamaan näihin ongelmiin. Ammattiäänenkäyttäjien äänihäiriöiden tutkimus on tärkeää, jotta ongelmat tunnistettaisiin ajoissa ja niiden aiheuttajiin pystyttäisiin puuttumaan mahdollisimman aikaisessa vaiheessa. Tutkimustulosten perusteella myös ennaltaehkäisevän koulutuksen kehittäminen ja tarjonnan parantaminen on mahdollista. Tämän kandidaatintutkielman tarkoituksena on tarkastella, millaisia äänihäiriöitä ja seurauksia voimakas äänenkäyttö työtehtävissä aiheuttaa. Lisäksi tarkoituksena on selvittää, ovatko työssään voimistettua ääntä käyttävät henkilöt saaneet äänenkäyttöön liittyvää koulutusta. Tutkielma perustuu kahdeksaan vuosina 2001–2021 julkaistuun tieteelliseen tutkimusartikkeliin. Kaikki artikkelit käsittelivät ammatteja, joissa toistuvasta äänen voimistamisesta johtuen esiintyy äänihäiriöitä. Artikkeleista neljässä käsiteltiin lisäksi äänenkäytöstä annettua koulutusta. Tulokset osoittivat, että ammattiäänenkäyttäjät kohtaavat usein työssään erimittaisia jaksoja, jolloin äänihäiriöt vaikuttavat heidän työhyvinvointiinsa. Äänihäiriöt muun muassa vaikeuttivat työtehtävissä pärjäämistä ja aiheuttivat työpoissaoloja. Koulutus ei lähtökohtaisesti kuulunut ammattiäänenkäyttäjien koulutusohjelmiin ja sen saatavuus muuta kautta oli myös hyvin vaihtelevaa. Tämän tutkielman ja aiempien tutkimusten perusteella äänihäiriöt vaikuttavat laajasti eri ammattilaisten työhyvinvointiin. Myös ennaltaehkäisevän äänenkäytön koulutukselle koetaan tarvetta. Jatkotutkimuksia tarvitaan niin ammattiäänenkäyttäjäryhmien äänihäiriöiden vaikutuksista yksilö- ja yhteiskuntatasolla, kuin myös koulutusten sisällöistä ja niiden vaikuttavuudesta.

Rubaga Division, Kampala, Uganda.To use polymerase chain reaction (PCR) based regions of difference (RD) analysis to study the species diversity of Mycobacterium tuberculosis complex isolates from a community-based sample of tuberculosis (TB) patients from Rubaga and to perform long sequence polymorphism (LSP) analysis to further characterise the M. tuberculosis Uganda genotype, a group of strains previously recognised by their characteristic spoligotype patterns.For the present study, 344 consecutive TB patients attending clinics in Rubaga Division were enrolled. Sample processing and culture were performed at the National Tuberculosis and Reference Laboratory and molecular assays at Makerere Medical School. Species identification was achieved by determining the RDs, while spoligotyping and LSP analysis were performed to characterise the M. tuberculosis Uganda genotype.Of the 344 isolates, 343 (99.7%) were M. tuberculosis sensu stricto, while one was classical M. bovis. The Uganda genotype strains characteristically lacked RD724, a locus that defines one of the major sub-lineages of M. tuberculosis, which suggested that this geographically constrained lineage is specifically adapted to a central African human host population.M. tuberculosis is the most prevalent species of the M. tuberculosis complex in Kampala, and the Uganda genotype is the predominant strain.

Serum triglycerides, high density lipoprotein (HDL) cholesterol, and total cholesterol were measured in 698 patients examined by angiography. The ratio of HDL cholesterol to total cholesterol was significantly lower in patients with single, double, and triple vessel disease than in patients without disease. The serum concentration of triglyceride was significantly higher in patients with single, double, and triple vessel disease than in those without coronary artery disease. Similar proportion of patients with coronary artery disease and without had serum cholesterol concentrations of greater than or equal to 6.5 mmol/l, but total cholesterol was significantly higher in patients with single, double, and triple vessel disease than in those without. HDL cholesterol (less than 1.0 mmol/l), triglycerides (greater than 2.0 mmol/l), and the ratio of HDL cholesterol to total cholesterol (less than 0.20) were significantly better than total cholesterol as indicators of coronary risk.

This paper presents a new industrial web-enabled competence management and networking solution and its implementation and piloting in a complex big-science environment of globally distributed competences.

DNA flow cytometric analysis was done from 100 paraffin-embedded non-small cell (NSCLC) and 29 small cell lung carcinomas (SCLC). Most of the patients had locally advanced disease and all were considered inoperable and treated by radiotherapy. DNA aneuploidy was detected in 62 (62%) of the NSCLCs and in 20 (69%) of the SCLCs. Radiotherapy induced a slightly more extensive and prolonged tumour regression in DNA-aneuploid NSCLC with high S-phase fraction (SPF) than in those with low SPF and no evidence of aneuploidy. However, neither in NSCLC nor in SCLC were significant survival differences detected between DNA-diploid and DNA-aneuploid cases or between those with low ( 12%) S-phase fraction (SPF). Neither did the degree of differentiation have prognostic significance in this material.

Expression of epidermal growth factor receptor (EGFR) and ERBB2 oncoprotein were studied in paraffin-embedded normal (n = 2), hyperplastic (n = 17), and malignant (n = 147) prostatic tissues by immunohistochemistry. Strong immunoreactivity was detected in the epithelial cells of all normal and hyperplastic prostates with a new EGFR antibody (Mab31G7). In prostatic carcinomas, the EGFR immunoreactivity was variable with 47% showing uniform, 39% partial, and 14% no staining. Tumors with partial or uniform EGFR immunoreactivity were locally more advanced and of higher histological grade than the EGFR-negative tumors. EGFR-positive tumors also had two to three times higher S-phase fraction, suggesting that EGFR expression conferred proliferative advantage. Patients who had either partially or uniformly EGFR-positive carcinomas had a worse 10-yr progression-free (p = 0.05), overall (p = 0.03), and prostatic carcinoma-specific (p = 0.007) survival than those with EGFR-negative carcinomas. However, according to a multivariate analysis, EGFR did not have independent prognostic value. None of the normal, hyperplastic, or malignant prostate tissues showed clearly positive ERBB2 immunoreactivity with MAb1 antibody.
