Aims: \ud Despite the common occurrence of coronary artery disease (CAD) and heart failure (HF) with preserved ejection fraction (HFpEF), there is limited evidence to guide revascularization.\ud \ud Methods and Results: \ud We investigated the long-term outcomes of coronary artery bypass grafting (CABG) in patients with HF and significant CAD across the spectrum of ejection fraction, using a large national cohort of patients from the Veteran Affairs (VA) Medical Centers in the US. Patients with HF were stratified into groups, HFpEF, HF with mid-range ejection fraction (HFmrEF), and HF with reduced ejection fraction (HFrEF) and compared to patients with no preoperative HF. We analysed 10 396 patients. Despite an increased hazard in the first year following revascularization, the long-term survival (median follow-up 6.6 years; interquartile range 3.7–10.1) of HFpEF post-CABG was similar to controls (hazard ratio 0.85, 95% confidence interval 0.68-1.06), but survival progressively declined with HFmrEF and HFrEF. Similar trends were seen with recurrent HF hospitalization with lower risk with baseline HFpEF (43.9 ± 6.9/100 patient-years) compared to HFmrEF (65.9 ± 3.8/100 patient-years) and HFrEF (93.4 ± 4.8/100 patient-years). Although HFpEF patients had lower mortality and HF hospitalization post-CABG compared to patients with a lower ejection fraction, they experienced the highest rates of future myocardial infarction.\ud \ud Conclusion: \ud Although HFpEF patients with CAD have greater short-term risk post-CABG, their long-term survival is comparable to controls. However, they are at increased risk for HF hospitalizations and myocardial infarction. These data support the safety of CABG in HFpEF patients and suggest continuum of mortality risk for ischaemic HF when stratified by baseline ejection fraction before revascularization.

We report autosomal recessive deficiencies of OAS1, OAS2, or RNASEL in five unrelated children with MIS-C. The cytosolic double-stranded RNA (dsRNA)-sensing OAS1 and OAS2 generate 2'-5'-linked oligoadenylates (2-5A) that activate the single-stranded RNA-degrading ribonuclease L (RNase L). Monocytic cell lines and primary myeloid cells with OAS1, OAS2, or RNase L deficiencies produce excessive amounts of inflammatory cytokines upon dsRNA or severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) stimulation. Exogenous 2-5A suppresses cytokine production in OAS1-deficient but not RNase L-deficient cells. Cytokine production in RNase L-deficient cells is impaired by MDA5 or RIG-I deficiency and abolished by mitochondrial antiviral-signaling protein (MAVS) deficiency. OBJECTIVES We evaluated the impact of preoperative liver function on early and 1-year postoperative outcomes in patients supported with a left ventricular assist device (LVAD) and subsequent evolution of liver function markers. METHODS A retrospective multicentre cohort study was conducted, including all patients undergoing continuous-flow LVAD implantation. The Model for End-stage Liver Disease (MELD) score was used to define liver dysfunction. RESULTS Overall, 290 patients with an LVAD [78% HeartMate II, 15% HVAD and 7% HeartMate 3, mean age 55 (18), 76% men] were included. Over 40 000 measurements of liver function markers were collected over a 1-year period. A receiver operating characteristic curve analysis for the 1-year mortality rate identified the optimal cut-off value of 12.6 for the MELD score. Therefore, the cohort was dichotomized into patients with an MELD score of less than or greater than 12.6. The early (90-day) survival rates in patients with and without liver dysfunction were 76% and 91% (P = 0.002) and 65% and 90% at 1 year, respectively (P CONCLUSIONS Preoperative liver dysfunction is associated with higher early 90-day and 1-year mortality rates after LVAD implantation. Furthermore, liver function improved in both patient groups. It has become imperative to optimize the selection criteria for possible LVAD candidates, since those who survive the first year show excellent recovery of their liver markers.

SARS-CoV-2 is the trigger of MIS-C, which suggests that other viruses may trigger different forms of Kawasaki disease. The discovery of inborn errors of immunity underlying MIS-C would facilitate that of inborn errors of immunity to viruses underlying Kawasaki disease., Multisystem inflammatory syndrome in children (MIS-C) emerged in April 2020 in communities with high COVID-19 rates. This new condition is heterogenous but resembles Kawasaki disease (KD), a well-known but poorly understood and clinically heterogenous pediatric inflammatory condition for which weak associations have been found with a myriad of viral illnesses. Epidemiological data clearly indicate that SARS-CoV-2 is the trigger for MIS-C, which typically occurs about 1 mo after infection. These findings support the hypothesis of viral triggers for the various forms of classic KD. We further suggest that rare inborn errors of immunity (IEIs) altering the immune response to SARS-CoV-2 may underlie the pathogenesis of MIS-C in some children. The discovery of monogenic IEIs underlying MIS-C would shed light on its pathogenesis, paving the way for a new genetic approach to classic KD, revisited as a heterogeneous collection of IEIs to viruses.

A 46-year-old man was admitted with non-ST elevation myocardial infarction and newly diagnosed acutely decompensated heart failure. Echocardiogram demonstrated left ventricular ejection fraction of 30% with basal inferior and inferolateral akinesis. Coronary angiography showed mild diffuse coronary artery disease and an anomalous right coronary artery arising from the left coronary cusp. Further imaging was consistent with ischemia in the right coronary distribution. Etiology of ischemia was thought to be the anomalous right coronary artery, and surgical unroofing of the right coronary ostium was performed. Here, we report a multimodality imaging approach, including cardiac magnetic resonance, cardiac computed tomographic angiography, and single-photon emission computed tomography, to support the diagnosis and management of a patient with anomalous right coronary artery arising from the left coronary cusp.

The amine-bis(phenolate) ligands was prepared through a Mannich reaction utilizing two equivalents of 2,4-di-tert-butylphenol or 4-tert-butylphenol, two equivalents of formaldehyde and a single equivalent of 2-aminoethyl diphenylborinate as primary amine. This work deals with the synthesis and evaluation of new (B←N) and (B-O) units containing amine-bis(phenolate) boron complexes designed by combination of amine-bis(phenolate) ligands and various boronic acids in toluene reflux using a Dean-Stark apparatus to remove water formed as a by-product. The newly synthesized amine-bis(phenolate) ligands and their boron complexes were characterized using elemental analysis, and their probable structures were pro- posed based on 1H and 13C NMR, FT-IR, UV-Vis spectroscopy and LC-MS/MS spectrometry. The inhibition effects of the synthesized compounds on acetylcholinesterase (AChE) activity in vitro and their radical scavenging activities were evaluated. The AChE was effectively inhibited by compounds, with KI values in the range from 5.48 ± 0.65 to 40.56 ± 4.42 μM. The (L1B4) has more inhibition effect on AChE with KI value (6.37 ± 1.50 μM) in (L2) series, while (L2B4) has an inhibition effect with KI (5.48 ± 0.65 μM) in (L2) series. Also, the compounds showed about 18-45% DPPH and 26-85% ABTS radical scavenging ac- tivity. On the other hand, butylated hydroxy anisole (BHA), butylated hydroxytoluene (BHT) and Trolox showed about 72-96% DPPH and 94-96% ABTS radical scavenging activity, respectively in the same con- centration (25-30 μg/mL). Besides, the (L1B4), (L1B5), and (L2B4) determined as the most active inhibitors were docked into the binding site of AChE to find the binding interactions of the boron complexes with the protein.

Aims The time course of changes in pulmonary artery (PA) pressure due to left ventricular assist devices (LVADs) is not well understood. Here, we describe longitudinal haemodynamic trends during the peri‐LVAD implantation period in patients previously implanted with a remote monitoring PA pressure sensor. Methods and results We retrospectively studied PA pressure trends in patients implanted with CardioMEMS™ PA pressure sensor between October 2007 and March 2017 who subsequently had an LVAD procedure. Data are presented as mean ± standard deviation, and P‐values are calculated using standard t‐test with equal variance. Background and Objectives: The unfortunate complication of a colotomy resulting from a colonoscopic polypectomy can be disastrous. Using the versatility of laparoscopic surgery, we aim to provide a treatment algorithm for these colonoscopic perforations. Methods: We report a case of cecal perforation, in a 70 year old female, following colonoscopic polypectomy that was treated successfully with laparoscopic application of an Endo-GIA linear stapler. Results: Four months following staple resection of her cecal perforation, the patient is asymptomatic and has undergone a repeat colonoscopy without any sequelae. Conclusions: Laparoscopic techniques can be safely applied in the treatment of colonic perforations following therapeutic polypectomies. A suggested management algorithm is provided highlighting the role laparoscopy may provide in selected patients.

Nav channels are essential for metazoan membrane depolarization, and Nav channel dysfunction is directly linked with epilepsy, ataxia, pain, arrhythmia, myotonia, and irritable bowel syndrome. Human Nav channelopathies are primarily caused by variants that directly affect Nav channel permeability or gating. However, a new class of human Nav channelopathies has emerged based on channel variants that alter regulation by intracellular signaling or cytoskeletal proteins. Fibroblast growth factor homologous factors (FHFs) are a family of intracellular signaling proteins linked with Nav channel regulation in neurons and myocytes. However, to date, there is surprisingly little evidence linking Nav channel gene variants with FHFs and human disease. Here, we provide, to our knowledge, the first evidence that mutations in SCN5A (encodes primary cardiac Nav channel Nav1.5) that alter FHF binding result in human cardiovascular disease. We describe a five*generation kindred with a history of atrial and ventricular arrhythmias, cardiac arrest, and sudden cardiac death. Affected family members harbor a novel SCN5A variant resulting in p.H1849R. p.H1849R is localized in the central binding core on Nav1.5 for FHFs. Consistent with these data, Nav1.5 p.H1849R affected interaction with FHFs. Further, electrophysiological analysis identified Nav1.5 p.H1849R as a gain-of-function for INa by altering steady-state inactivation and slowing the rate of Nav1.5 inactivation. In line with these data and consistent with human cardiac phenotypes, myocytes expressing Nav1.5 p.H1849R displayed prolonged action potential duration and arrhythmogenic afterdepolarizations. Together, these findings identify a previously unexplored mechanism for human Nav channelopathy based on altered Nav1.5 association with FHF proteins.

Septic pulmonary emboli (SPE) can be a difficult clinical entity to distinguish from thromboembolic pulmonary embolism (TPE) in a patient with history of IV drug abuse (IVDA). We present a case of a patient who presented with failure to thrive and presumed diagnosis of recurrent PE that ultimately was discovered to have fungal pulmonary valve endocarditis resulting in a right ventricular outflow obstruction. This required replacement of the pulmonary valve and repair of the right ventricular outflow tract. This case highlights difficulty in differentiating pulmonary valve endocarditis with septic emboli from chronic PE in a patient with a complex medical history.

WOS: 000300746300022, PubMed ID: 22399186, The role of various microorganisms including Chlamydophila (formerly Chlamydia) pneumoniae, have been frequently investigated in the pathogenesis of atherosclerosis. In our study, the relationship between C.pneumoniae seropositivity and risk factors for atherosclerosis have been evaluated. A total of 90 atherosclerotic patients (71 of them were male; age range: 45-87 years; mean age: 65.3 +/- 8.7 years) and 90 control subjects without coronary diseases (41 of them were male; age range: 42-84 years; mean age: 61.6 +/- 9.6 years) were included in the study. Both groups were also evaluated for the presence of risk factors such as age, gender, smoking, hypertension, diabetes, obesity, dyslipidemia, familial history and the high levels of ferritin, cholesterol (total, HDL and LDL) and HS (high sensitive)-CRP. The presence of C.pneumoniae IgG, IgM and IgA antibodies were investigated by micro-immunofluorescence (MIF) and ELISA methods using commercial kits (Euroimmun, Germany). The total antibody seropositivity rate was found 100% (90/90) in patient group by both MIF and ELISA methods, while this rate in control group was 94% (85/90) by MIF and 92% (83/90) by ELISA. When MIF test results were taken into consideration (since it is accepted as the reference method for C.pneumoniae serology), IgG, IgM and IgA positivity rates in patient and control groups were found as 100% (90/90) and 89% (80/90); 70% (63/90) and 59% (53/90); 3% (3/90) and 2% (2/90), respectively. Statistically (s)ignificant difference between patient and control groups was detected only for IgG positivity (p < 0.05) and for total antibody positivities (100% and 94%, respectively) (p < 0.05). The evaluation of the risk factors revealed that age, hypertension, dyslipidemia and HS-CRP levels exhibited statistically significant differences between patient and control groups (p < 0.05 for each parameter tested). Statistically significant relation was detected only between high HS-CRP levels and C.pneumoniae seropositivity (p < 0.05). It was concluded that in areas with high C.pneumoniae infection prevalence, early diagnosis and specific treatment of C.pneumoniae infections, may prevent establishment of chronic infection and eliminate a risk factor for the development of atherosclerosis.

There is increased scrutiny on the quality in health care with particular emphasis on institutional heart transplant survival outcomes. An important aspect of successful transplantation is appropriate donor selection. We review the current guidelines as well as areas of controversy in the selection of appropriate hearts as donor organs to ensure optimal outcomes. This decision is paramount to the success of a transplant program as well as recipient survival and graft function post-transplant.

This case report describes the repair of a complete sternal dehiscence of the lower right sternum using sternal wires, manubrial plates, and a Talon closure device for rigid, multidimensional sternal fixation. Sternal dehiscence is a rare but significant cause of morbidity for patients undergoing median sternotomy. The risk factors for this complication are well described and although sternal wires have traditionally been used for primary closure, rigid fixation with sternal plates is a viable alternative to avoid dehiscence in this high-risk cohort.

The sterically hindered salicylaldimine ligands N,N'-(1,5-diaminonaphthalene)-3,5-bu(2)(t)-salicylaldimine (L-1), N,N'-(2,7-diaminofluaren)-3,5-bu(2)(t)-salicylaldimine (L-2) and N,N'-(1,8-diaminonaphthaline)-3,5-bu(2)(t)-salicylaldimine (L-3) have been synthesised by the condensation of 1,5-diaminonaphthalene, 2,7-diaminofluarene, and 1,8-diaminonaphthaline with 3,5-di-tert-butylsalicylaldehyde, respectively. Dinuclear M(II) complexes of L-1 and L-2 and mononuclear M(II) complexes of L-3 have been prepared using Cu(II), Ni(II), Co(II), and Mn(II) salts and characterised. The synthesised sterically-hindered, salen-type complexes were tested as catalysts for the formation of cyclic carbonates from CO2 and liquid epoxides (propylene oxide, epichlorohydrine, 1,2-epoxybutane and styrene oxide), which served as both reactant and solvent. Ligand structure and the type of metal centre have a marked influence on the catalytic activity. Mn(II) complexes showed the highest catalytic activity.

A 21-year-old male presented to the emergency department with the complaint of muscle weakness. The patient had used a powderized over-the-counter product named 'Tekumut' for 2 weeks to quit smoking. The granulated product was studied and determined to contain 'licorice' containing glycyrrhizic acid., Licorice (a plant which contain glycyrrhizic acid) -induced hypokalemia usually has a mild progression. However, it may cause a critical failure in physical action by means of weakness followed by paralysis and may cause rhabdomyolysis or ventricular fibrillation, leading to death, when left untreated., This report has presented the first case with hypocalemia due to licorice consumption in granulated form from Turkey. In addition, the report has aimed to emphasize the importance of obtaining the detailed history of a patient in diagnosis.

WOS: 000448672400013, Organic matters in raw water have a potential to generate harmful disinfection byproducts such as trihalomethanes (THMs) and haloacetic acid (HAA) during the chlorination process. The aim of this study was to investigate the trihalomethane formation potential (THMFP) and haloacetic acid formation potential (HAAFP) in Melendiz River and to determine the effect of several factors including water quality parameters, distance, and seasonal variation. Water samples (n= 93) were collected at Melendiz River during December 2015-November 2016. The range of THMFP and HAAFP was 47.28-3348.73 mu g/L and 41.42-4952.68 mu g/L, respectively. ANOVA analysis was performed to understand the impact of the seasonal and/or distance changes on DBPs formation. It was found that the seasonal variations (sig. 0.80) at Melendiz River. Finally, a non-linear statistical model was developed based on Multiple Stepwise Regression Analysis using highly correlated water quality parameters with TTHM and THAA 5 in Pearson Correlation Analysis. According to the results, among all factors, flow, dissolved organic carbon (DOC), UV254, UV272 , bromide, temperature, and additional chlorine dose have a remarkable effect on THMFP and HAAFP. Especially, the Pearson correlation (P) values for the DOC and the Chlorine dosages are 0.583, 0.786 and 0.994 and 0.865 for THMFP and HAAFP models, respectively. WOS: 000343613400042, The electron paramagnetic resonance (EPR) of gamma-irradiated single crystals of cholesteryl methyl carbonate (C29H48O3) has been studied for different orientations of the crystals in a magnetic field. EPR spectra of cholesteryl methyl carbonate (C29H48O3 irradiated by Co-60-gamma were recorded between 125 K and 300 K for different orientations of the crystal in the magnetic field. The spectra were found to be temperature independent. In C29H48O3 single crystal,. the radiation damage centers caused by a Co-60-gamma source were determined as CH3CH2CH radical. However, these molecules also have potent antiangiogenic effects. Whereas, angiogenesis may be the most crucial determinant of the prognosis of cardiovascular diseases, and except some special situation, antiangiogenic effect is not desirable in the most of the cardiovascular disease. In this study, we aimed to compare the antiangiogenic potency of UFH, enoxaparin, and tinzaparin. The antiangiogenic efficacies of UFH, enoxaparin, and tinzaparin were examined in vivo by using the chick chorioallantoic membrane (CAM) model. Twenty fertilized eggs were used for each studied drug. Drug solutions were prepared in 10 and 1 IU/10 mu l concentrations. Decreases in the density of the capillaries were assessed and scored. All three drugs showed antiangiogenic effects on the chick CAM at the 10 IU/10 mu l concentration. However, the antiangiogenic score of the UFH was significantly higher than that of enoxaparin and tinzaparin at 1 and 10 IU/10 mu l concentrations. UFH had stronger and antiangiogenic potential than enoxaparin and tinzaparin. However, tinzaparin showed dose-dependent antiangiogenic effects. We think that an anticoagulant molecule with a less and dose-dependent antiangiogenic effect, as in the case of tinzaparin, may be more desirable in case of cardiovascular disease related with insufficient angiogenesis. WOS: 000366076700031, In this study, recalcitrant total phenol (TPh) and organic matter removal were investigated at olive mill wastewater (OMW) in sequential Coagulation and Fenton system. This study focused on different operational parameters such as pH, H2O2, and Fe2+ dosages, and [Fe2+]/[H2O2] ratios. The optimum conditions were determined as; pH = 3; [Fe2+]= 2.5 g/L; [Fe2+]/[H2O2] = 2.5. A higher treatment efficiency was achieved at sequential Coagulation and Fenton system (COD, 65.5%) and TPh, 87.2%), compared to coagulation process (COD, 51.4%; total organic carbon (TOC), 38.6% and total nitrogen (TN) 52.1%). WOS: 000343772500002, The single crystals of triphenylphosphinselenid [C18H15PSe] were produced by slow evaporation of concentrated ethyl acetate solutions. These single crystals were exposed to Co-60 gamma (gamma) rays with a dose speed of 0.980kGy/h at the room temperature for 72h. The free radical over the sample was observed using electron paramagnetic resonance (EPR)-X band spectrometer. The EPR spectra were recorded between 120 and 400K. Furthermore, the sample irradiated was rotated in steps of 10 degrees and analyzed for different orientations of the crystal in the magnetic field. Only one radical structure was determined on the molecule. The hyperfine constants of the sample were found to be anisotropic. WOS: 000306480900013, Prosthetic vascular graft infection remains one of the most serious complications seen after vascular surgery. Recently, fibrin glue has gained attention as a possible means to deliver drug therapies. In this study, the efficacy of vancomycin incorporated fibrin glue for preventing methicillin-resistant Staphylococcus aureus (MRSA) infection of prosthetic grafts was investigated. The vascular grafts were implanted into subcutaneous pockets in the backs of 32 rats. Group 1: no graft contamination; Group 2: MRSA contamination; Group 3: vancomycin incorporated fibrin glue graft and MRSA contamination; and Group 4: vancomycin soaked graft and MRSA contamination. The grafts were removed after 7 days and evaluated by a quantitative culture analysis. The quantitative culture values for Groups 2, 3, and 4 were 1.8x10(11) +/- 1.4x10(11), 1.1x10(7) +/- 2.4x10(7), and 2.6x10(8) +/- 3.9x10(8), respectively. The culture values of the Group 2 was significantly higher than those of the Group 3 and Group 4 (p=0.014 and p=0.016, respectively), however, Groups 3 and 4 were comparable (p=0.161). In our study, efficacies of vancomycin-incorporated fibrin and vancomycin alone were comparable. The finding of the current study indicated that a fibrin-based delivery system might not be as effective an option as a vancomycin delivery.

In many symptomatic patients with peripheral artery disease, coronary artery disease, and ischemic chronic wounds, there is a pathological insufficiency of angiogenesis. Therefore, determining the angiogenic or antiangiogenic effects of molecules currently used in cardiovascular treatment is crucial. Although levosimendan is the most well studied calcium sensitizer in preclinical and clinical practice, to the best of our knowledge, there are no previous studies investigating its angiogenic or antiangiogenic effects. In the present study, we aimed to investigate the effects of levosimendan on angiogenesis. Methods: The antiangiogenic efficacy of levosimendan was examined in vivo in the chick chorioallantoic membrane (CAM) model by using 20 fertilized eggs and drug solutions of 1 and 10 mu mol/L. concentrations. Decreases in the density of the capillaries were assessed and scored. Results: Significant antiangiogenic effects were observed at 1 and 10 mu mol/L concentrations of levosimendan. The antiangiogenic scores of levosimendan at 1 and 10 mu mol/L concentrations were 0.6 and 1.10, respectively. The antiangiogenic score of bevacizumab, used as a positive control, was 0.95 at 1.0 mu mol/L concentration. No significant difference was found between the antiangiogenic scores of levosimendan and bevacizumab (p=0.54). Conclusions: Our results indicate that levosimendan has antiangiogenic effects on the chorioallantoic membrane. However, these findings must be confirmed in future studies on humans.
