Purpose The aim of this study was to assess the safety and effectiveness of percutaneous vulvoperineal varices sclerotherapy using 2% polidocanol foam under fluoroscopic guidance. Subjects and Methods This is a retrospective review of prospectively collected data of 70 patients treated between June 2020 and November 2021, with a mean age of 31 years (range: 24–42 years). All patients were not pregnant with residual vulvoperineal varices after embolization of their refluxing tributaries of the ovarian vein and/or internal iliac veins presenting with vulvoperineal heaviness/pain with or without lower limb varicosities. Technical and clinical successes together with procedural complications were assessed. Results The technical success rate was 100%. No minor or major complications have been reported. Follow-up was at 1 week, 2 weeks, 6 months, and 12 months post sclerotherapy. Clinical success was observed in all patients with improvement in their symptoms. Conclusion Direct fluoroscopic-guided sclerotherapy for vulvoperineal varices offers a safe and effective treatment option with the ability of visualization and embolization of the pelvic leak points to avoid recurrence.

Purpose: The purpose of this study was to assess the safety and efficacy of using particles only in the embolization of the hemorrhoidal arteries for the management of hemorrhoids. Subjects and Methods: This is a retrospective study for patients treated between March 2015 and December 2018. We treated 33 patients, 13 men and 20 women with a mean age of 37 years (range: 18–70 years), in which 11 patients had Grade II hemorrhoids and 22 had Grade III hemorrhoids. Technical and clinical successes together with procedural complications were assessed. Results: The technical success rate was 100%. No minor or major complications have been reported. No cases of anorectal ischemia, anal incontinence, hemorrhoidal thrombosis, or complications related to femoral arterial puncture have occurred. Follow-up was at 3 months and 12 months postembolization. Clinical success was observed in 32 patients (96.9%) with improvement by at least 2 points of the French bleeding score at 3 months postembolization. Conclusion: The use of particles alone in the embolization of hemorrhoidal arteries whether from the superior rectal artery and/or from the middle and inferior rectal arteries can offer a safe and effective treatment option.

Background Chronic renal failure is one of the main health problems in Egypt. Arterio-venous fistulas, grafts, as well as conventional sites for placement of the dialysis catheters are liable to thrombosis; stenosis, and occlusions, so alternative routes are considered as life-saving ways in such cases. Transhepatic permcath is one of the new and up to date methods for inferior vena cava and right atrium catheterization. Few studies with rather limited nomber are available to evaluate transhepatic permcath. The aim of our study is to emphasize the technique, complications, and efficacy of application of percutaneous transhepatic hemodialysis catheters. Results Two hundred-ninety six chronic renal dialysis patients were included in this prospective interventional study. They include 180 males and 116 females with mean age of 53.2 years ± 11.7 years ranging from 38 to 65 years. Percutaneous transhepatic hemodialysis catheters were inserted for all patients. Technical success of the procedure was achieved in all cases (100%). Post-procedure patency and function of the catheters were followed up with mean follow-up period 750 days. Mean of primary and secondary devices service intervals were 290 and 270 days respectively. Mean time catheter in situ was 280 days. Mean cumulative duration of catheter in situ was 557 days. Catheters migration, sepsis, thrombosis, and exit site infection rates were 0.14, 0.15, 0.18, and 0.32 per 100 catheter-days respectively. Three patients had hepatic subcapsular hematoma (1%). No mortality or other complications were related to the procedure. Conclusions Combined ultrasound and fluoroscopic-guided transhepatic permanent dialysis catheter application for patients with exhausted classic venous access routes and non-functioning/thrombosed AV fistulas or grafts showed excellent technical success with good short and mid-term patency rates and low complications rates. Thus, this study encourages us to expand this promising technique for application of dialysis catheter in indicated cases.

Context: Percutaneous placement of gastrostomy tube has replaced surgical placement as the most accepted method of gastrostomy tube insertion. It can be done by an alternative nonendoscopic fluoroscopy-guided technique that combines the advantages of fluoroscopic guidance and the pull technique. Aims: This study aimed to describe a percutaneous fluoroscopy-guided technique for applying mushroom-retained large-bore gastrostomy advanced through the nose without endoscopy. Settings and Design: This retrospective study was conducted at the Interventional Radiology Unit, Ain Shams University Hospitals, Cairo, Egypt. Subjects and Methods: Between January 2015 and November 2017, 86 neurologically compromised patients underwent placement of 24F mushroom gastrostomy tubes. There were 55 males and 31 females, with the mean age of 61 years (58–87 years). Technical success and procedural complications were assessed. Follow-up data were collected retrospectively by reviewing the medical records at the neurology clinic to evaluate tube function and monitor complications. Results: Technical success rate was 100%. Procedure time varied between 10 and 13 min. No major procedure-related complications occurred. Twenty-two patients (25.5%) died during the study period with no procedure-related deaths. Nearly 34.8% of the patients (30/86 patients) could not be followed up due to loss of contact. Follow-up time ranged between 200 and 230 days in the remaining 34 patients with no evidence of tube dysfunction. Conclusions: Fluoroscopy-guided percutaneous placement of large-bore pull gastrostomy tubes inserted via nasal route showed a high rate of technical success and long-term patency with low risk of complications.

Background: Trans-catheter renal arterial embolization (RAE) has emerged as a possible alternative to surgery in the management of iatrogenic renal arterial injuries. Objective: To discuss the role of renal arterial embolization in patients with iatrogenically-induced renal arterial injuries. Methodology: All cases were done at the Interventional Radiology Unit, Ain Shams University Hospital. Results: Technical and Clinical success rates of the RAE reached 90% for each, with post procedural complications that only amounted up to 30%, half of which was the post embolization syndrome that presents as fever, leukocytosis and pain and is treated conservatively. Conclusion: Renal artery embolization has proven to be a safe, minimally invasive option in the treatment of iatrogenic renal arterial injuries achieving high technical and clinical success rates.

Background: Trans-catheter renal artery embolization has a role in the management of iatrogenic and traumatic renal vascular injuries. Objective: To assess the technical success and short term clinical efficacy of renal artery Embolization in patients with iatrogenic and traumatic renal vascular injuries. Methodology: All cases were done at interventional radiology unit Ain Shams University. Conclusion: Renal artery embolization plays a great role in the management and treatment of renal vascular injuries either traumatic or iatrogenic.

We report a case of an 11-year-old child with end-stage renal disease and extensive systemic venous thrombosis including the inferior and superior vena cava. Transhepatic portal vein hemodialysis tunneled catheter was inserted after exhausting all other possible venous access through internal jugular, subclavian, femoral, and hepatic veins.

We report our assessment of the experience of female interventional radiologists in different Arab countries based on an online questionnaire that was sent to 57 female interventional radiologists in 5 Arab countries. These interventional radiologists were identified from the Pan Arab Interventional Radiology Society database and through personal communications. The survey included items related to demographics, social status, and the professional challenges of interventional radiologists, with a response rate of 93%. Forty-five percent of respondents were between 31 and 45 years of age. Interventional radiology practice represented 25%-50% of work time for 51% of respondents, and the main challenges reported were related to radiation exposure, balancing family/work life, and the male-dominated interventional radiology community.

Objectives To assess the feasibility and efficacy of prostatic artery embolization in relieving symptoms of benign prostatic hyperplasia. Materials and methods In a prospective study 28 patients with symptomatic benign prostatic hyperplasia were presented for prostatic artery embolization between June 2012 and June 2014. Patients age was 48–85 years with mean age 68.5 years ± 10.6 SD. International Prostate Symptoms Score (IPSS) before intervention measured 20–35 with mean score 26.3 ± 6.8 SD. Prostatic volume before intervention ranged between 48 and 166 cc 3 with mean of 82.6 ± 11.2 SD. Results Technical success was achieved in all cases (100%). All patients were followed for 6 months after the procedure. IPSS improved at 6 months in all patients with post embolization mean of 12.2 ± 3.4 SD with significant P value of 0.0006. Mean post-procedure prostatic volume at 6 months was 49.8 cc 3 ± 16.9 SD with 39.7% mean volume reduction. No major complications were recorded. We achieved clinical success in 27 patients (96.4%) with only one non responding patient (3.6%). Conclusion Prostatic artery embolization is a feasible technique and preliminary short-term results show promising high technical and clinical success rates in symptomatic patients with benign prostatic hyperplasia.
