Study Design: Descriptive study. Place and Duration of Study: Third-level Newborn Intensive Care Units in Kahramanmaras Sutçu Imam University Health Practice and Research Hospital and Megapark Private Hospital, Turkey, between July 2017 and May 2019. Methodology: The data of 132 and 81 patients who underwent tunnelled femoral vein (FV) and percutaneous internal jugular vein (IJV) access procedures, respectively, for advanced medical management were reviewed. Planned procedures performed in an operating room under general anaesthesia were included in the study. Demographic data of infants, kind and the number of complications, and findings in clinical follow-up were recorded. Results: There were similar success rates between techniques. No complication requiring intervention occurred during the placement of the FV catheters. Four procedures were interrupted due to periprocedural complications requiring intervention in the placement of IJV catheters. To evaluate the treatment results of patients who were operated with the diagnosis of esophageal atresia and tracheoesophageal fistula (EA and TEF) in our clinic. The results of patients who were operated with the diagnosis of esophageal atresia and tracheaesophageal fistula between April 2017 and November 2019 were retrospectively evaluated. The patients were evaluated in terms of gestational age, birth weight, gender, atresia type, surgical approach, and duration of treatment. Postoperative mechanical ventilator follow-up was examined in terms of the transition time to nasogastric tube and oral feeding, and complications. A total of 24 patients, 20 (83%) boys and 4 (17%) girls, were included in the study. The mean age at surgery was 3.9 days. The gestational age of the patients was 35 weeks and the mean birth weight was 2391 grams. Distal tracheaesophageal fistula + proximal atresia were detected in 22 (92%) patients. Congenital heart disease was detected in 14 (58%) patients. The follow-up time in the ventilator was 27 hours, and the average oral feeding time was 11 days. Three (12.5%) patients died. Hospital stay was 16 days. During their follow-up, anastomotic stenosis was observed in 5 (21%) patients, and anastomotic leakage was observed in one (4%) patient. EA and TEF are anomalies that can be highly cured with surgical treatment. The follow-up of patients with EA by the neonatal team is important for the follow-up and treatment of additional anomalies. Therefore, these patients should be followed up with a multidisciplinary approach. Families should be informed about the frequent postoperative anastomotic stenosis., Kliniğimizde özefagus atrezisi ve trakeaözefageal fistül (ÖA ve TÖF) tanısı ile ameliyat edilen hastaların tedavi sonuçlarını değerlendirmek. Nisan 2017 - Kasım 2019 tarihleri arasında özefagus atrezisi ve trakeaözefageal fistül tanısıyla ameliyat edilen hastaların sonuçları retrospektif olarak değerlendirildi. Hastaların gestasyonel yaş, doğum ağırlığı, cinsiyet, atrezi tipi, cerrahi yaklaşım, tedavi süresi açısından değerlendirildi. Postoperatif mekanik ventilatör takibi, nazogastrik tüp ve oral beslenmeye geçiş süresi ve komplikasyonlar açısından incelendi. Çalışmaya 20 (% 83) erkek ve 4 (% 17) kız toplam 24 hasta dahil edildi. Ortalama ameliyat yaşı 3,9 gün idi. Hastaların gestasyonel yaşı 35 hafta ve ortalama doğum ağırlığı 2391 gramdı. Distal trakeaözefageal fistül + proksimal atrezi 22 (% 92) hastada tespit edildi. Ondört (% 58) hastada konjenital kalp hastalığı tespit edildi. Ventilatörde takip süresi 27 saat, ortalama oral beslenme süresi 11 gündü. Üç (%12.5) hasta öldü. Hastanede kalış süresi 16 gündü. Takiplerinde 5 (% 21) hastada anastomoz darlığı, bir (% 4) hastada anastomoz kaçağı görüldü. ÖA ve TÖF cerrahi tedavi ile yüksek oranda iyileştirilebilen anomalilerdir. ÖA hastalarının yenidoğan ekibi tarafından izlenmesi, ek anomalilerin takibi ve tedavisi için önemlidir. Bu nedenle bu hastalar multidisipliner bir yaklaşımla takip edilmelidir. Ameliyat sonrası sık karşılaşılan anastamoz darlığı açısından aileler bilgilendirilmelidir.

Disgerminom, dev boyutlara ulaşabilen overin en sık görülen maligntümörüdür. Genelde hormon salgılamamakla beraber beta-HCG salgılayabilirler. Bizdekarında kitle ve karın ağrısı şikâyetleri olan radyolojik görüntüleme iletespit edilen beta-HCG salgılayan dev boyutlara ulaşmış disgerminom tanılı 3olguyu inceledik. Üç olguya da salpingo-ooferektomiameliyatı yapıldı. Bir olguda komsu lenf nodlarından, peritondan ve karşıoverden örnekleme yapıldı. Bir olgu da üç haftada bir dört kür BEP (Bleomisin, Etoposid, Sisplatin)kemoterapi protokolü ile fertilite de korunarak tedavi sağlanmış oldu. Birolguda da 3 kür BEP kemoterpi protokülü verilmesi planlandı ve 2 kür BEPkemoterapi protokolü verildi., Disgerminomais the most common malign tumor of the ovary that can reach giant sizes. Ingeneral, although they do not secrete hormones, they may secrete hormones, theymay secrete beta-HCG. We have studied 3 cases of disgerminoma that have reachedgiant dimensions that secrete beta-HCG detected by radiological imaging, whichcaused a complaint of abdominal pain in the abdomen. Salpingo-oophorectomywas performed in all three cases. In one case, neighboringlymph nodes, peritoneum and counter-ovary were sampled. In one case, once inthree weeks, four times treatment of BEP (Bleomycn, Etoposide, Cisplatin)chemotherapy protocol was administered and treatment was provided withoutharming fertility. In another case, three treatment BEP chemotherapy protocolwas planned to administer and two treatment BEP chemotherapy protocol wasperformed.

Objectives: In our study, the assessment of age, gender, incidence and seasonal differences in the regions related to appendicitis were investigated. Methods: 676 patients that received an operation following a diagnosis of appendicitis in 3 hospitals from 3 different regions of Turkey occurring at different dates were examined retrospectively after being categorized by region. The differences among groups were compared to each. Results: The mean age of male (63%) (n = 426) and female (37%) (n = 250) patients (total = 676) was 10.8 years (range, 1–18 years). The percentage of female patients in the The Black Sea Region (BR) group was significantly higher (48%) than that in the other two groups (%33 for SR, 30% for MR) (P = 0.001). The frequency of appendicitis was higher (36%) in the spring and winter (25%) than that during summer or fall. The number of cases increased the most in the BR group in spring (47%) (P < 0.001). When the types of appendicitis in the different age groups were compared with season, acute appendicitis (P < 0.02) was more frequently (93%) seen in the child-adolesan (CA) group in the spring; however, perfore apandicitis (45%) was greater in the Infant-Preschool (IPS) group (P < 0.02). Acute appendicitis comprised 55% and perforated appendicitis comprised 45% of cases in the IPS group, whereas acute appendicitis comprised 78% and perforated appendicitis comprised 22% in the CA group. Conclusion: According to the results of our study, appendicitis was most frequently seen in the spring and winter seasons and more frequent in boys between the ages of 10-13 years. As age decreases, the frequency of perforated appendicitis increases. Appendicitis is affected by environmental factors. Reducing the incidence of appendicitis may be possible by establishing regional and specific studies related to this subject and the etiologies of the disease. Amaç: Çalışmamızda apandisitin yaşı, cinsiyeti, insidansı ve mevsimsel olarak bölgesel farklılıkların değerlendirilmesi amaçlanmıştır. Yöntemler: Bu çalışmada Türkiye’nin 3 farklı bölgesindeki, 3 hastanede çeşitli tarihlerde apandisit tanısı ile ameliyat edilen 676 hasta bölgelerine göre gruplandırılarak gruplar arasındaki farklar birbirleri ile karşılaştırıldı. Bulgular: Toplamda (n = 676) hastaların %63’ü (n = 426) erkek, %37’si (n = 250) kız, yaş ortalaması 10.8 yıl (range, 1-18 years) idi. The Black Sea Region (karadeniz bölgesi) (BR) grubundaki kız hastaların yüzdesi diğer iki grupla karşılaştırıldığında istatistiksel olarak anlamlı derece yüksek (%45, p=0.001) idi. Mevsimsel olarak değerlendirildiğinde en sık (%36) ilkbahar ve kış (%25) aylarında apandisite rastlandı. İlkbaharda en çok BR grubunda hasta sayısı artmıştır (P < 0.001). Farklı yaş gruplarında apandisitin tipi mevsimler olarak karşılaştırıldığı zaman akut apandisit (%93) ilkbaharda child-adolesan (çoçuk-erişkin) (CA) grubunda daha sık idi (P < 0.02). Ancak perforasyon (%45) Infant-Preschool(bebek-okul öncesi) (IPS) grubunda daha fazla idi (P < 0.02). IPS grubunda Akut apandisit %55, perfore apandisit %45 buna karşın CA grubunda akut apandisit %78, perfore apandisit %22 oranında görüldü. Sonuç: Çalışmamızın sonuçlarına göre apandisit en sık ilkbahar ve kış mevsimlerinde, 10-13 yaşlar arasındaki erkek çocuklarda daha çok görülmüştür. Yaş küçüldükçe perfore apandisit sıklığı artmıştır. Apandisit çevresel faktörlerden etkilenmektedir. Bu konuda yapılacak bölgesel ve spesifik çalışmalar ile etiyolojiler tespit edilerek apandisitin azaltılması gelecekte mümkün olabilir.

ÖZETGiriş ve Amaç: Hirschsprung hastalığı için yapılan ameliyatlarda uygulananpelvik diseksiyonlarda, anorektal fonksiyonlar yanında alt üriner sistemfonksiyonları da etkilenebilmektedir. Bu çalışmada Swenson ve transanalendorektal pull through (TAP) yöntemleriyle ameliyat edilen Hirschsprunghastalarının klinik, anorektal manometrik ve ürodinamik olarak karşılaştırılmasıamaçlandı.Hastalar ve Yöntem: Bu çalışma Swenson ameliyatı (Grup I) ve TAP yapılan(Grup II) ve her birinde 10'ar hasta olan iki grup üzerinde yapıldı. Grup I'dekihastaların sekizi erkek, ikisi kız, ameliyat yaşı ortalamaları 2.5 ay idi. Grup II'dekihastaların yedisi erkek, üçü kız, ameliyat yaşı ortalamaları 1.7 ay idi. Klinikbulgular, anorektal ve üriner fonksiyonlar hakkında bilgi yüzyüze görüşmeyöntemiyle elde edildi. Ürodinami ve anorektal manometri işlem yaşı ortalamalarıGrup I'de 5.3 yaş, Grup II'de 3.7 yaş idi. Her iki gruptaki hastalara standartanorektal manometri ve ürodinami işlemleri yapıldı. statistiksel çalışmada Mann-Whitney U ve Ki-Kare testleri kullanıldı.Bulgular: Her iki grup arasında yaş, cinsiyet, ameliyat ile ürodinami ve anorektalmanometri işlem yaşları, anorektal ve üriner fonksiyonlar, rektoanal inhibitörrefleks (RA R), mesane kompliyansı, mesane kapasitesi ve artık idrar kalmasıaçısından anlamlı fark saptanmadı. Pelvik taban kaslarının elektromyografik(EMG) aktiviteleri, Grup II lehine iki grup arasında farklı bulundu.Sonuçlar: Pelvik taban kaslarının EMG aktivitelerinin, TAP yöntemiyle ameliyatedilen hastalarda, Swenson ameliyatı yapılanlara göre daha iyi olması, TAPyönteminin daha güvenilir bir yöntem olduğunu ortaya çıkarmıştır.Anahtar Kelimeler : Hirschsprung hastalığı, Swenson, endorektal pull through,manometri, ürodinami THE CLINICAL, ANORECTAL MANOMETR C ANDURODYNAM C COMPARISON OF PATIENTS WITHHIRSCHSPRUNG?S DISEASE WHO UNDERWENTSWENSON AND TRANSANAL ENDORECTAL PULL-THROUGH OPERATIONSABSTRACTBackground and aim: During pelvic dissections for the Hirschsprung?s disease, inaddition to the anorectal functions, low urinary system can also be affected. In thisstudy, we aimed to compare the clinical, anorectal manometric and urodynamicfindings of Swenson?s operation and transanal endorectal pull through (TAP)performed for Hirschsprung?s disease.Patients and method: This study was performed on two groups with each havingten patients who underwent Swenson?s operation (Group I) and TAP (Group II).Group I was consisted of eight males and two females with 2.5 months of averageage. Group II consisted of seven male and three female patients with 1.7 months ofaverage age. The clinical symptoms about anorectal and urinary functions wereobtained by face-to-face interviews. Standard anorectal manometric and urodynamicstudies were performed on all patients in both groups. The average age for theurodynamic and anorectal manometric study was 5.3 and 3.7 years for Group I andGroup II, respectively. Mann-Whitney U and Chi-Squared tests were used for thestatistical analysis.Results: There was no statistical difference in the age, sex, operation ages,urodynamic and anorectal manometry ages, anorectal and urinary functions,rectoanal inhibitory reflex (RAIR), bladder compliance, bladder capacity, andresidual urine between the two groups. The electromyographic (EMG) activities ofthe pelvic floor muscles were statistically different between two groups, which is infavor of Group II.Conclusion: Since the EMG activities of pelvic floor muscles were better in thepatients who underwent TAP method than Swenson?s procedure, TAP seems to bemore reliable than Swenson?s method.Key words: Hirschsprung?s disease, Swenson, endorectal pull through, manometry,urodynamics 54

Aim: Foreign body aspirations FDA are one of the major problems in pediatric emergency services. The objective of this study is to explore the relations between the preferred anesthesia method and common complications emerging under rigid bronchoscope RB in children exposed to FDA along with their preoperative history retrospectively. Methods: RB based upon the diagnosis or any suspicion of FDA between October 2013 and January 2015. Result: Gender distributions were 68.6% male and 31.4% female. Mean age was 2.72 ± 2.51 years. Most common symptoms were cough and dyspnea 86.2% and 58.8% respectively . Distributions of radiological findings were as follows: normal view 29.4%, obstructive emphysema 23.5% and atelectasia 23.5 %. Most of the substances achieved were composed of organic materials during bronchoscope. Placements of the foreign bodies were on the left bronchial and righ bronchial trees 31.3% and 33.4% respectively. Spontaneous ventilation SV in 6 cases where the proximal localization of the FB, in other 45 cases Manual intermittent positive pressure ventilation using a manual neuromuscular blocking MIPPV was prefered. On the both ventilation methods, desaturation, arrhythmia and broncholaryngeal spasm were complications occurred during anesthesia. None of the patients were found mortality. Conclusion: FDA is known as an important life-threatening situation. Therefore, it is substantial to initiate bronchoscope process immediately by assigning the convenient anesthesia method referring to the placement of the foreign body and general situation of the patient in collaboration with anesthesiologist and bronchoscopy performer that will eventually lead future promising results in decreasing mortality., Amaç: Çocuklarda yabancı cisim aspirasyonu YCA sık olarak görülen acillerdendir. Bu çalışmada, YCA nedeniyle rijit bronkoskopi RB yapılan çocukların demografik bilgileri, bronkoskopide seçilen anestezi yöntemi ve oluşan komplikasyonların incelemesi 41 amaçlanmıştır. Metod: Ekim 2013 ile Ocak 2015 tarihleri arasında YCA şüphesi veya tanısı nedeniyle RB 43 yapılan 51 hasta incelenmiştir. Bulgular: Hastaların %68.6'sı erkek, %31.4'ü kız, yaş ortalaması 2,72±2.51 yıldı. En sık görülen semptomlar; %86.2 öksürük ve %58.8 dispne idi. Radyolojik bulguların dağılımı ise %29.4 normal görünüm, %23.5 obstrüktif amfizem ve %23.5 atelektazi idi. Bronkoskopi ile çıkarılan YC'lerin çoğunu organik materyaller oluşturmaktaydı. YC'ler sırasıyla %31.3 sol bronşial ağaç, %33.4 de sağ bronşial ağaçta idi. YC'in proksimal yerleşimli olduğu 6 olguda spontan ventilasyo n SV yöntemi, diğer 45 olguda nöromuskuler bloker kullanılarak Manuel intermittant pozitif pressure ventilasyon MIPPV tercih edildi. Her iki ventilasyon yönteminde desatürasyon, aritmi ve bronşiolaringeal spazm, anestezi sırasında meydana gelen komplikasyonlardı. Hastaların hiçbirisinde mortaliteye rastlanmadı. Sonuç: Çocuklarda YCA'larının hayatı tehdit eden bir durum olduğunu, bronkoskopinin derhal yapılması gerektiğini, anestezistin; hastanın genel durumu, YC'in yerleşimine göre uygun anestezi yöntemini seçmesi gerektiğini ve bu konuda anestezist ile bronkoskopistin işbirliği içinde olmasının mortalite oranını azaltacağı düşünülmüştür.
