Bu çalışmanın temel amacı, terör eylemlerinin Türk turizm endüstrisini ne derece etkilediğini incelemektir. Terörizm; küresel dünya da ülkelerin toprak bütünlüğünü veya vatandaşların can güvenliğini tehdit etmektedir. Az gelişmiş ya da gelişmekte olan ülkeler açısından turizm endüstrisi hızlı kalkınmanın anahtarı olarak görülmektedir. Türkiye'de de turizm endüstrisi ekonomik kalkınma açısından büyük önem teşkil etmektedir. Az gelişmiş ya da gelişmekte olan ülkelerin ekonomik refaha kavuşmalarını istemeyen terör örgütlerinin destekçileri, yaptıkları ekonomik yardımlar ile terör örgütlerinin gelişmesine ve büyümesine destek vermektedirler. Türkiye'de yaşanan terör eylemleri, turizm endüstrisini ciddi anlamda etkilemektedir.Çalışma kapsamında, 2015 ve 2017 yılları arasında Türkiye'de yaşanan terör olayları ve aynı yıllarda Türkiye'ye giriş yapan turist sayıları temel alınarak zaman serisi analizi yapılmıştır. Ayrıca çalışmanın kapsamına, Diyarbakır ve Mardin'de bulunan 15 otel işletmesinin orta ve üst kademe yöneticileri dâhil edilmiştir. Araştırmada nitel araştırma yöntemi kullanılmış, literatür taraması yapılmıştır. Araştırmanın verileri Mardin ve Diyarbakır'da bulunan üç yıldız ve üzeri otel işletmelerinin orta ve üst kademe yöneticileri ile görüşmeler yapılarak elde edilmiştir. Elde edilen veriler betimsel analiz tekniği ile analiz edilmiştir. Araştırma sonucunda terör eylemlerinin; Dünya turizmini, Türk turizmini ve Güneydoğu Anadolu Bölgesi turizmini ekonomik, sosyal ve turizm çeşitliliği gibi yönlerden olumsuz bir şekilde etkilediği anlaşılmıştır.Anahtar Kelimeler: Terörizm, Turizm, Türkiye, Güneydoğu Anadolu Bölgesi, Mardin, Diyarbakır. The main purpose of this study is to examine what extent the terrorist activities affected the Turkish tourism industry. Terrorism; threatens the territorial integrity of the countries or the safety of citizens globally. For the less developed or developing countries, the tourism industry is seen as the key to rapid development. The tourism industry in Turkey is of great importance in terms of economic development. The supporters of the terrorist organizations who do not want underdeveloped or developing countries to reach economic prosperity, support the development and growth of the terrorist organizations with their economic assistance. Terrorist acts taking place in Turkey, seriously affect the tourism industry.In the study, the terrorist attacks taking place in Turkey between 2015 and 2017 and the number of tourists in the same years arriving in Turkey was conducted on the basis of time-series analysis. In addition,15 middle and upper level managers of hotel business located in Diyarbakır and Mardin are included to the scope of the study. Qualitative research method was used in the research and literature review was done. The data of the study was obtained by interviews with middle and upper level managers of three star and above hotel enterprises in Mardin and Diyarbakır. The data obtained were analyzed qualitatively. ‘’Maggot Debridement Therapy" (MDT), known as larval therapy, is one of the traditional and complementary medicine practices with a history of application from ancient times to the present day. It is used in the treatment of chronic wounds that do not heal with conventional treatments. There are many case histories of successful treatment of stubborn wounds such as diabetic foot ulcers with MDT. Since it reduces the need for antibiotics and does not have any known significant side effects, it is an FDA approved treatment method that is now frequently used. In this review study, it is aimed to explain the development steps of MDT from past to present and its application methods and to discuss its medical importance in the light of the literature.

Geleneksel ve tamamlayıcı tıp alanında tıbbi olarak faydalı olduğu bilinen bitkilerin farklı oranlarda karışım olarak kullanılması oldukça yaygın bir kullanım alanı bulmuştur. İlaç üretimindeki maddi ve zamansal kısıtlamalar, tedavi yerine destekleyici bitkisel bileşenlerin kullanımı açısından önemli bir fayda sağlamaktadır. Bununla birlikte, bu bileşenleri oluştururken, bitkilerin kökeninden aktif maddeye ve kullanılabilecek doz aralığına kadar birçok alanda toksik olmayan dozu belirlemek gerekmektedir. Bu çalışmada Remember Regeneration Tedavide (RTM) kullanıldığı bilinen DVD-REG® bitkisel takviyelerinin akut ve subakut toksisite analizleri yapılmıştır. Klinik gözlem bulguları ISO 10993-11:2017 standartlarına göre değerlendirilmiş olup kontrol ve uygulama grupları arasında herhangi bir değişikliğe ratlanılmamıştır. Hematolojik ve biyokimyasal kan parametreleri 0., 1., 7. ve 14. günlerinde değerlendirilmiş kontrol ve uygulama grupları arasında herhangi bir değişiklik tespit edilmemiştir. Histolojik açıdan toksisitenin araştırıldığı karaciğer, kalp, böbrek ve dalak dokularında gruplar arasında patolojik bir bulguya rastlanılmamıştır. Bu çalışma ile Türkiye’de kullanımakta olan gıda takviyesi DVD-REG®’in akut ve subakut etkileri araştırılmış olup elde edilen veriler kronik toksisite testleri için fayda sağlayacaktır., The use of herbs that are known to be medically useful in the field of traditional and complementary medicine has been widely used as a mixture in different proportions. Material and temporal restrictions in drug production provide a significant benefit in terms of using supportive herbal ingredients instead of treatment. However, when creating these components, it is necessary to determine the nontoxic dose in many areas, from the origin of plants to the active substance and the dose range that can be used. In this study, acute and subacute toxicity analyzes of DVD-REG® herbal supplements known to be used in Remember Regeneration Therapy Method (RTM) were performed. Clinical observation findings were evaluated according to ISO 10993-11:2017 standards and no changes were made between the control and application groups. Hematological and biochemical blood parameters were evaluated on days 0, 1, 7, and 14, and no changes were found between the control and administration groups. There was no pathological finding among the groups in liver, heart, kidney and spleen tissues where histological toxicity was investigated. In this study, which are now used in Turkey food supplement data obtained from the DVD-REG® was researched it will benefit for chronic effects of acute and subacute toxicity tests.

Objective: In this review study, it is aimed to summarize the information cited about medical and cosmetic applications of the date persimmon (Diospyros kaki) and accordingly the toxicity assessment. Material-Method: In this study, it was summarized the information cited about medical and cosmetic applications of the date persimmon (Diospyros kaki) and accordingly the toxicity assessment. Results: Persimmon (Diospyros kaki), which is cultivated in tropical/subtropic regions such as China, Korea, Japan and Brazil, especially in the Far Eastern countries with more hot climate conditions, has been named as Trabzon Persimmon because it entered Turkey through the Black Sea region. This fruit type has a very important role on the immune system thanks to vitamins and some active ingredients. Although it is mostly consumed as fresh fruit and dried in our country, there are also formulations developed as medical support products (dematological and cosmetic applications etc.) in different countries and reagions. However, toxicity assessment studies on natural plants/herbal products are also very few. Conclusion: Since there is scientific evidence on the phytotherapeutic effects of Diospyros kaki, and the product scale on the market is very narrow, this is highly promising for future healthcare products.

Objective: Honey samples were collected from 82 different pieces during the season when rhododendron flowers bloomed in Duzce areas in 2017. Since the majority of these honeys collected are sold as mad honey by local people, has been determined that it is the most suitable period to make their analysis.Material-Method: To apply our sample preparation method, honey samples were prepared to be analyzed on the LC-MS/MS. The accuracy, repeatability and reliability of the analysis method were provided by us. Results: The obtained results were calculated with the calibration plot drawn at ppb (ng/ml) level in LC-MS/MS. The grayanotoxin -III levels in mad honey were found to be % 36 for 0 ng/ml, %43 for 0.5-10 ng/ml, %15 for 10-50 ng/ml and %6 for 50≥ ng/ml.Conclusion: Most of honey samples do not contain toxins. Some of them are safe to use in middle proportions and less of them dangerous to consume.

Modern people have increased their living standards due to the development of technology. However, health problems have increased with increasing living standards. Efforts to return to nature and natural products such as herbal drugs, medicinal mushrooms and traditional medicine practices have become popular. Medical mushrooms, which have an important place for human beings at every stage of history; in recent years, has succeeded to take its place from traditional medicine applications by separating itself from phytotherapy applications. The aim of our research is to raise awareness about this value, which is present in our country, which has a rich mycota, and to make determinations regarding its usage areas in the world and its importance in the history. Our study could be regarded as a resource for the practices and future studies in our country.

Objective: Cancer is one of the leading causes of death worldwide and is characterized by the proliferation of abnormal cells. In recent years, as in the treatment of many diseases, there is also a return to nature in the treatment of cancer. In developing societies, the concepts such as prophylaxis of disease prevention rise to prominence in order to reduce the material and moral losses caused by disease treatment. Consequently, the use of supplementary food products occupies more and more space in daily life. In this study, it was aimed to investigate the antiproliferative properties of IST-GLIO® food supplement, which is one of these products and used in the Remember Regeneration Therapy Method (RTM), on C6 (rat glioma) cells.Material-Method: WST-1 cell proliferation test protocol was applied to examine the antiproliferative effect of IST-GLIO® and the results were evaluated according to ELISA microplate reading data. The product was tested at concentrations ranging from 1 ng/mL to 100 ng/mL. Results: When the results were evaluated, it was appeared to be effective in the concentration range of 6-20 ng/mL on the C6 glioma cell culture; it was found that the efficacy didn’t occur through general toxicity, specifically reducing the reproduction of the cancer cells in question, since food supplement did not inhibit cell division further at higher doses.Conclusion: As a result, non-specific toxicity is observed in many cultures that are similarly investigated in cell cultures, depending on concentration as high doses are obtained. The IST-GLIO® product does not show this feature is considered as promising.

The use of herbs for therapeutic purposes has been widespread since ancient times. In traditional treatments, plants are applied in a variety of forms, either directly or prepared by isolation and / or enrichment of biologically active ingredients. Momordica charantia L. (bitter melon) is a popular bush that has traditional use as a supplement in the treatment of various diseases with its rich bioactive ingredient content. The plant has common pharmacological potential, including antimicrobial, antimutagenic, antifertility, antidiabetic, antioxidant, antilipolytic, hypoglycemic, anticancer, antimicrobial, antiviral and hepatoprotective activities. This review provides a short overview of the in vitro, in vivo and clinical studies conducted to evaluate the therapeutic efficacy and safety of Momordica charantia.

Objective: Hot spring medicine and hot spring tourism has been increasingly popular in the world and our country for the last 50 years. People in our country have been in search of natural and traditional methods to relieve their discomfort and contribute to their health during their holidays as in the rest of the world. Awareness and knowledge levels of physicians on this subject is important to suggest hot springs individually within the scope of other alternative and complementary medicine and develop internal tourism (health tourism) movements. The aim of this study is the determination of the opinions, knowledge and awareness levels of the physicians working on Duzce University Faculty of Medicine.Material-Method: The data were collected by survey technique. In the research, a questionnaire form was distributed to all physicians working in the faculty and 72 physicians agreed to fill out the questionnaire. 69 of them were found suitable for evaluation. The survey includes a total of 22 statements apart from demographic questions. The expressions used are five-point Likert scale (I agree-I don’t agree). The statements are generally aimed at determining the awareness of physicians about hot spring treatments.Results: Approximately one-third of the physicians have never been to a hot spring before; almost all of them declared that they do not recommend hot spring treatment to any of their patients. Almost all of them wish to be educated on this subject and consciously recommend and prescribe their patients. It is understood that the majority of physicians lack information about hot spring medicine, use, and awareness.Conclusion: Almost all physicians have the demand to have information on this subject. There is consensus on increasing scientific research on the subject and ensuring its integration in modern medicine.

Hilyetu’l-Muluk, which was translated in the first half of the 17th century is an example of Ottoman Turkish in terms of writing and language usage which has not yet fully seen a standardization since more than two usages have been identified at the word and annex levels. The introductory part of our study contains detailed information about the manuscript and its writer. Our language analysis exemplifies the language features of the text rather than general language features of the period. The large part of our study consists of transcription and index. In the appendix, some sample prints of the manuscripts we used in our study are added.

Günümüzde görüntü işlemeye tıp, askeri, eğitim, güvenlik gibi birçok alanda ihtiyaç duyulmaktadır. Segmentasyon işlemi de görüntü işlemenin en önemli adımlarından birini oluşturmaktadır. Literatür araştırmalarında, genellikle segmentasyon işleminin 2D olarak yapıldığı ve 3D segmentasyon çalışmalarının daha az olduğu görülmektedir. 3D segmentasyonda derinlik bilgisi de bulunduğundan, görüntünün daha doğru analiz edilmesi işlemine katkı sağlamaktadır. Bu durum 3D segmentasyon işleminin önemini daha da artırmaktadır. Birçok alanda kullanılan ELM (Extreme Learning Machine – Aşırı Öğrenme Makinesi) tek gizli katmana sahip ileri beslemeli bir yapay sinir ağı modelidir. 3D segmentasyon işlemi için yapılan bu çalışmada ELM kullanılmıştır. Önerilen yöntemde ilk olarak ELM'nin eğitimi yapılmaktadır. Ardından test aşamasında, segmentasyonu istenen 3D görüntü kullanıcıya yardımcı olması için GUI (Graphical Unit Interface) kullanılarak seçtirilmektedir. Seçilen görüntüde daha önce eğitimi yapılan nesneler var ise, tanınan nesnenin 3D olarak segmentasyonu gerçekleştirilmektedir. Buna ek olarak eğitim aşamasında nesnelerin isimleri de öğretildiğinden, tanınan nesnelerin yan taraflarına adları da yazdırılmaktadır. Segmentasyon için seçilen görüntüde eğitimi yapılmamış nesneler var ise nesneyi ayrıştırarak yanına tanınmayan nesne ifadesini yazmaktadır. Önerilen yöntemle yapılan segmentasyon işleminin sonucu YSA ve FCM ile karşılaştırılmış olup elde edilen sonuçlar önerilen yöntemin başarısını açıkça ortaya koymuştur. Today, image processing is needed in many areas such as medicine, military, education and security. Segmentation is also one of the most important steps in image processing. In literature research, it is generally seen that the segmentation process is done in 2D and 3D segmentation studies are less. As 3D segmentation has depth information, it contributes to more accurate analysis of the image. This further increases the importance of 3D segmentation.The ELM (Extreme Learning Machine), which is used in many areas, is a feed-forward neural network model with a single hidden layer. ELM is used in this study for 3D segmentation. In the proposed method, ELM is trained first. Then, during the testing phase, the segmented 3D image is selected using the GUI (Graphical Unit Interface) to assist the user. If there are objects previously trained in the selected image, 3D segmentation of the recognized object is performed. In addition, since the names of objects are also taught during the training phase, the names of the recognized objects are printed on their side. If there are objects that have not been trained in the image selected for segmentation, it parses the object and writes the unrecognized object expression next to it. The result of the segmentation process with the proposed method was compared with ANN and FCM, and the results clearly demonstrated the success of the proposed method. 76

Uzun süreli tedavi gerektirmeyen kemik travmalarında kullanılacak vidave diğer bağlantı elemanlarının biyobozunur özelliğe sahip magnezyumalaşımlardan yapılması, özellikle büyüme çağındaki çocuklarda uygulanan,metalik implantların zamanla neden olduğu olumsuz etkileri ortadankaldırmaktadır. Bu çalışmanın amacı; saf magnezyumun yapay vücut sıvısı (SBF)içerisindeki korozyon mekanizmasını 30 saat süren açık devre potansiyeliölçümleri (OCP) ve bu esnada yapılan in-situ pH ölçümleriyle araştırmakve iki yöntem arasındaki korelasyonu ortaya koymaktır. Elde edilen verilerçalışma elektrotunun yüzeyinde de zamanla oluşan Mg(OH)2 tabakasınınkorozyon hızını etkilediğini göstermektedir.

Aim: The thoracoabdominal aorticaneurysm surgery may cause splanchnic ischemia and may result in severepostoperative complications caused by liver and/or kidney injury. This studyinvestigated the effect of temporary occlusion of the aorta on the developmentof I/R injury of liver and kidney and the protective effect of montelukast, aselective reverse CysLT1 receptor antagosit. Material and Methods: Twenty-one male Sprague-Dawleyrats were randomly assigned to three groups (n=7 per group) as G1 (no aorticocclusion and montelukast administration), G2 (45 min. aortic occlusion; nomontelukast administration) and G3 (45 min. aortic occlusion, 10 mg/kg montelukastadministration). At the 48th hour of reperfusion, kidney and liversamples were dissected for histopathological evaluation and immunohistochemicalstaining for HSP70, interleukin-6 and myeloperoxidase (MPO). Results: Tissuesamples taken from the kidney showed significant intergroup differences interms of tubular cell degeneration, cytoplasmic vacuole formation, hemorrhageand tubular dilatation (P = 0.001, P = 0.017, P = 0.001, P = 0.001respectively). There was a significant difference with regards to HSP-70 stain(P = 0.01) in favor of G1 and G3 with respect to G2. Hepatocyte degeneration inthe liver samples of G3 was significantly lower than that of G1 and G2 (P = 0.027). Conclusion: Thefindings of the current study demonstrated that montelukast has protectiveeffects on both kidney and liver; and were confirmed histopathologically andimmunohistochemically. The protective effects of montelukast can be attributedto its potential of anti-oxidative and anti-inflammatory actions., Amaç:Torakoabdominal aort anevrizması cerrahisinin sebep olabildiği splanknikiskemi, karaciğer ve/veya böbrek hasarının neden olduğu ciddi postoperatifkomplikasyonlarla sonuçlanabilir. Bu çalışmada seçici tersinir bir CysLT1reseptör antagonisti olan montelukastın, aortanın geçici oklüzyonuyla yaratılanböbrek ve karaciğer iskemi-reperfüzyon (I/R) hasarı üzerindeki koruyucuetkisini incelemek için tasarlandı. Gereç ve Yöntemler: Yirmi birerkek Sprague-Dawley tipi sıçan, rasgele üç gruba (her grupta n=7) ayrıldı; G1(aort oklüzyonu ve montelukast uygulaması yok), G2 (45 dakika aort oklüzyonu;montelukast uygulması yok) ve G3 (45 dakika aort oklüzyonu, 10 mg/kgmontelukast uygulaması). Reperfüzyonun 48. saatinde böbrek ve karaciğerdenalınan doku örneklerinden histopatolojik değerlendirme ve HSP 70, interlökin-6ve miyeloperoksidaz (MPO) ile immünohistokimyasal boyama yapıldı. Bulgular: Böbrektenalınan doku örnekleri tübüler hücre dejenerasyonu, sitoplazmik vakuol formasyonu,hemoraji ve tübüler dilatasyon açısındna incelendi ve gruplar arasındaistatistiksel olarak anlamlı fark bulundu (P = 0.001, P = 0.017, P = 0.001). G1ve G3 gruplarında HSP-70 ile boyanmada G2 ye göre anlamlı fark saptandı (P = 0.001).G3 karaciğer örneklerinde hepatosit dejenerasyonu G1 ve G2 den anlamlı olarakdüşüktü (P = 0.027). Sonuçlar:Yapılan çalışmanın bulguları, montelukastın böbrek ve karaciğer üzerindekoruyucu etkileri olduğunu histopatolojik ve immünohistokimyasal olarakgöstermiştir. Montelukastın koruyucu etkileri, ilacın antioksidan veantiinflamatuar etkilerine bağlanabilir.

Deaths from mushroom poisoning, due to the ingestionofmushrooms containing amatoxins, seem to be increasingboth in Turkey andworldwide. The amatoxinsfound in Amanita phalloides (also called thedeath capmushroom) are most toxic agents, and they are responsible for more than 95% ofthe cases of deadlymushroom poisoning. Alpha amanitin is best known toxin ofthis group. Alpha amanitin analysis can be carried out with special methodssuch as chromatography and ELISA. ELISA method is suitable in order to analyseurine and blood containingfor alpha amanitin. Gas chromatography system is notsuitable to analyse for alpha amanitin. HPLC and LC-MS/MS systems are bestanalytic methods for all materials containing alpha amanitin.Some analytical machines, supplies and experienced personalare required to perform these analytical methods. There is a need for moresimple analytical method. The lateral flow immunoassay method may be an easysolution for this purpose. The lateral flow immunoassays are commonly usedworldwide for different purposes such as pregnancy test and drug screeningtest. This test works based on antigen-antibody complexes that is coupled withvisible molecules. These tests are theoretically possible for every analytewhose antibody is producible, and by changing the antibody concentrations, thesensitivity of these tests can be adjusted to the desired thresholds.Analysis of the amanitins and application of alpha amanitinto lateral flow immunoassay will be declarate in this speech.

Amaç: Deli bal zehirlenmelerin büyük bir kısmı ülkemizde görülmektedir. Deli bal içinde bulunan toksinlerin temel kaynağı Rhododendron türü bitkiler olup, bu bitkilerdeki toksinlerden en fazla grayanotoksin-III bulunur. Deli bal zehirlenmelerinde klinikte bradikardi, atriyoventriküler blok, asistoli, hipotansiyon, salivasyon gibi semptomlar görülür. Bu çalışmada, sıçanlara farklı dozlarda intravenöz (i.v.) yolla verilen grayanotoksinIII'ün doza bağımlı kardiyovasküler etkilerinin değerlendirilmesi amaçlanmıştır.Yöntem: Çalışmada, 42 Wistar albino sıçan kullanılmış ve hayvanlar Kontrol, Grup 1 (5 µg/kg), Grup 2 (10 µg/kg), Grup 3 (20 µg/kg), Grup 4 (50 µg/kg) ve Grup 5 (100 µg/kg) olmak üzere altı gruba ayrılmıştır. Elektrokardiyogram (EKG), tansiyon arteriyel ve solunum sayısı için bazal değerler elde edildikten sonra i.v. grayanotoksin uygulanmasının ardından 60 dk. boyunca bu parametreler kaydedilmiştir.Bulgular: 5 µg/kg grayanotoksin-III verilen grupta, tüm zamanlarda anlamlı bir değişiklik olmazken, 10 ve 20 µg/kg toksin verilen gruplarda 2. ve 5. dk.'dan sonra anlamlı biçimde solunum sayıları ve kalp tepe atım sayıları azalmış, arteryel basınçlarda düşme görülmüştür. Yirmi beşinci dk.'dan sonra ise tüm parametreler bazal değerlere yeniden geri dönmüştür. Elli ve 100 µg/kg grayanotoksin-III verilen gruplarda, 2. dk.'dan itibaren tüm parametrelerde hızla düşme olmuş ve 5-10 dk. içinde sıçanlar ölmüştür. Sonuç: Sıçanlar için i.v. grayanotoksin-III'ün 50 µg/kg ve üzerindeki yüksek dozlarda lethal olabildiği, ölümün kardiyak arrest nedeniyle görüldüğü, 5 µg/kg ve altındaki düşük dozlarda toksik etkilerin görülmediği, i.v. uygulamada toksik etkilerin yaklaşık olarak 25 dk. sürdüğü saptanmıştır. Objective: The mad honey poisonings are commonly seen in our country. The source of the toxin in mad honey is plants of Rhododendron species. Grayanotoxin-III type toxin is commonly found in these plants. Clinical effects of this toxin are bradycardia, atrioventricular block, asystole, hypotension and symptoms such as salivation. In this study; different doses of grayanotoxin-III were intravenously administered to rats (i.v) and the effect of grayanotoxin on cardiovascular system was evaluated.Methods: This study was performed on 42 Wistar albino rats and the animals were divided into six groups. Control, Group 1 (5 µg/kg), Group 2 (10 µg/kg), Group 3 (20 µg/kg), Group 4 (50 µg/kg) and Group 5 (100 µg/kg) groups were formed. Baseline values for electrocardiography (ECG), arterial blood pressure and respiratory rate were obtained and recorded for 60 minutes after administration of i.v. grayanotoxin-III.Results: Group1 (5 µg/kg) was not significantly changed in all time periods. In 10 and 20 µg/kg groups, respiratory rates, peak heart rates decreased significantly and drop in arterial blood pressures was observed after 2nd and 5th minutes. While 25 minutes after administration of i.v. grayanotoxin-III all parameters returned to baseline values again. In 50 and 100 µg/kg grayanotoxin-III groups all parameters fell sharply from 2 minutes on, and rats died within 5-10 minutes.Conclusion: In conclusion, it was found that the lethal dose of grayanotoxin-III for Kabul tarihi: 07.06.2017 rats is 50 µg/kg. In addition the death was developed due to cardiac arrest. In case of the lower level of 5 µg/kg grayanotoxin-III, any toxic effect was not seen. The effects of toxin apparently lasted nearly 25 minutes.

AmAÇ: Çalışma, deneysel bir modelde, seçici tersinir bir CysLT1 reseptör antagonisti olan montelukastın, spinal kord iskemi-reperfüzyon (I/R) hasarı üzerindeki rolünü incelemek için tasarlandı. yÖntem ve GereÇler: Yirmi bir erkek Sprague-Dawley tipi sıçan, rasgele üç gruba (her grupta n 7) ayrıldı; G1 (aort oklüzyonu ve montelukast uygulaması yok), G2 (45 dakika aort oklüzyonu; montelukast uygulaması yok) ve G3 (45 dk aort oklüzyonu, 10 mg / kg montelukast uygulaması).Reperfüzyonun 48. saatindeki Motor Defisit Indeksi (MID) skorlaması kullanılarak yapılan nörolojik değerlendirmeden sonra, lomber spinal kordlar, histopatolojik değerlendirme ve HSP70, interlökin-6 ve miyeloperoksidaz (MPO) için immünhistokimyasal boyama amacıyla çıkartıldı. BulGulAr: G1 grubundaki tüm sıçanların nörolojik durumu normaldi ve MDI skorları 0\'dı (p 0.05). G3\'ün MDI skoru, G2 grubuna göre anlamlı derecede düşük bulundu (2.8 vs 5.5; p 0.05 ). G1'de vaküoler konjesyon, diğer gruplara göre anlamlı derecede düşük bulundu (p 0.0001). The degradation of aggrecan by upregulated disintegrin and metalloproteinase with trombospondin motifs (ADAMTSs) is the key event in the development of both rheumatoid arthritis (RA) and osteoarthritis (OA). Increased levels of leptin in both RA and OA have been demonstrated, thus linking leptin to arthritic diseases, but the mechanism has not been clarified. This study investigated the putative role of signaling pathways (p38, JNK, MEK1, NF-?B, and PI3) involved in leptin-induced cartilage destruction. Normal human articular chondrocytes were cultured with recombinant human leptin at 100, 250, 500, and 1000ng/mL doses for 6, 12, 24, and 48h, after which ADAMTS-4, -5, and -9 genes expression were determined by real time-polymerase chain reaction (RT-PCR) and Western Blot methods. The signaling pathways involved in leptin-induced ADAMTSs upregulation were also investigated by using inhibitors of signaling pathways. It was demonstrated that ADAMTSs expression level was peaked at 1000ng/mL doses for 48hours, and MAPKs (p38, JNK, and MEK) and NF-?B signaling pathways involving in leptin triggered ADAMTSs upregulation. Conclusion: The level of alpha amanitin in Amanita phalloides from Duzce Province is differ from different countries. Higher and lower levels of toxin than our data obtained investigations are present in the literature. The reason of this differences might be several factors like extraction methods, analysis methods and environmental conditions.

Extracorporeal shockwave (ESW) have demonstrated significant bactericidal effectiveness in vitro and effectiveness and safety were evaluated in an animal model of osteomyelitis. In this experimental study, we aimed to test our hypothesis that the use of ESW together with systemic antibiotic treatment will provide synergy for the treatment of implant-related chronic osteomyelitis caused by methicillin-susceptible Staphylococcus aureus (MSSA). The proximal tibia of 32 rats was contaminated with (10)8CFU/ml methicillin-sensitive S. aureus (MSSA-ATCC 29213) and Kirschner-wires were placed into the medulla of the tibia. After 4 weeks, Kirschner-wires were removed and the rats were randomly divided into four groups: group I, untreated contaminated control group; group II, receiving only ESW therapy; group III, receiving only systemic teicoplanin; group IV, treated with a combination of ESW and systemic teicoplanin. ESW was applied twice to the infected limbs and all rats were sacrificed at the end of 8th week. The degree of tibial osteomyelitis was assessed by quantitative culture analysis. Bacterial counts in groups III and IV were significantly reduced relative to the control (p=0.002 and 0.001, respectively). The decrease in bacterial counts was more pronounced and significant in group IV compared to group III (p=0.024). In group II, bacterial counts also decreased, but the differences were in significant (p=0.068). Our experimental model suggests that ESW provides significant synergy for systemic antibiotic treatment. However, further clinical trials are required in order to use this treatment modality safely in patients, even though our study demonstrated successful results in the treatment of implant-related chronic osteomyelitis in rats. Method: Sedoanalgesia has been used to sedate patients for a variety of pediatric procedures in our department between 2007 and 2010. This is a retrospective review of 2720 pediatric patients given ketamine for sedation with midazolam premedication. Ketamine was given intravenously (1-2 mg/kg) together with atropine (0.02 mg/kg) and midazolam (0.1 mg/kg) + a local infiltration anesthetic 2 mg/kg 0.5% bupivacaine hydrochloride. Result: Median age of the patients included in the study was 5.76 +/- 2.12 (0-16 years). The main indications for ketamine include circumcision (69%), inguinal pathologies (inguinal hernia (17%), orchidopexy (2.68%), hydrocele (3.38%), hypospadias (1.94%), urethral fistula repair (0.33%), urethral dilatation (0.25%), and other conditions. All of our patients were discharged home well. In this regard, we have the largest group of patients ever given ketamine. Conclusion: Sedoanalgesia might be used as a quite effective method for daily surgical procedures in children.

Methods: Six hundred fifty-five DM patients were enrolled. Patients receiving diabetes therapies for at least one year were retrospectively assessed with physical examination, detailed medical, habits, demographic characteristics and laboratory tests, and divided into two groups according to the diabetes treatment type (using oral agent or insulin and using metformin or not). Insulin users were grouped according to insulin regimens (intensive, basal and mixed) and to the insulin type (glargine, detemir, human insulin, biphasic analogue). Cancer cases were identified at the first visit. Results: Among 655 DM patients 13 (2%) were Type 1 DM, 642 (98%) were Type 2 DM; 379 (58%) were female, 276 (42%) were male. Thirty-sir cancers and 76 benign tumors were observed. Patients with and without a cancer were compared for diabetes treatment regimens and types of agents. No significant differences were observed between groups using oral agent or insulin (p=0.429) and using metformin or not (p=0.119). There were no significant differences between groups according to insulin regimens (p=0.059) and the insulin type (p=0.418). Conclusion: There was no significant difference in terms of cancer risk between different types of pharmacological treatments of diabetes (including analog insulins).

İlk defa 1940ların başında Condrad Hal Waddington tarafından ortaya atılan epigenetik kavramı, DNA dizilimini değiştirmeden gen ifadesini etkileyen kalıtılabilir değişiklikler olarak tanımlanabilir. Yapılan araştırmalarla epigenetik mekanizmaların birçok biyolojik olayda ve kanser gibi hastalıklarda önemli görevler ifa ettiği bulunmuştur. Bu derlemede literatürdeki epigenetik mekanizmalar ve bunların kanserle olan ilişkileri hakkında bilgiler özetlenmiştir. Epigenetics, proposed by Condrad Hal Waddington at the begining of the 1940 describes heritable changes that affect gene expression without changing DNA sequencing. After researches, it has been found that the epigenetics modifications perform important task in many biological processes and diseases like cancer. In this review we summarize the information in literature related with epigenetic mechanisms and their relationship with cancer.

Amaç: Dünya genelinde mide kanserinin insidansı düşmesine rağmen hala önemli bir sağlık problemidir. Türkiye’de ise yılda 30.000 yeni mide kanseri vakasıyla 2. en sık görülen kanserdir. Mide kanseri genetik ve çevresel faktörlerin etkileşimiyle ortaya çıkan çok faktörlü karmaşık bir hastalıktır. Kanserli dokuda aşırı ifade edilen survivin, apoptozis inhibe edici proteinlerdendir. Bu çalışmada Survivin -31 G/C polimorfizmi ile mide kanseri arasındaki ilişki araştırıldı. Gereç ve yöntem: Çalışma Düzce Üniversitesi Araştırma ve Uygulama Hastanesi Patoloji Laboratuvarına gelen mide kanseri tanısı konmuş 46 hasta ve sağlıklı bireylerin oluşturduğu 42 kişilik kontrol grubu ile gerçekleştirildi. Bu bireylerin genotipi Düzce Üniversitesi, Tıp Fakültesi, Tıbbi Genetik Anabilim Dalı Laboratuvarlarında PCR-RFLP yöntemiyle tayin edildi. Bulgular: Hasta grubunda, GG genotipi 16 (% 34,8), GC genotipi 21 (% 45,7) ve CC genotipi ise 9 (% 19,6) olguda saptandı. Kontrol grubunda ise, genotip dağılımı sırasıyla 13 (% 31), 26 (% 61,9) ve 3 (% 7,1) bulundu. Hasta ve kontrol grubu karşılaştırıldığında istatistiksel olarak anlamlı bir fark saptanamadı. Fakat CC genotipine sahip bireylerin mide kanserine yakalanma riskinin GG (OR=1,52) daha fazla risk oluşturduğu bulundu. Sonuç: Bu çalışma bildiğimiz kadarıyla Türk toplumunda mide kanseri ile Survivin -31G/C polimorfizmini araştıran ilk çalışmadır. Elde ettiğimiz sonuçlar hasta ve kontrol gruplarımızın temsil ettiği toplum kesitinde mide kanseri ile Survivin -31 G/C polimorfizmi arasında anlamlı bir ilişki olmadığını göstermekle birlikte CC genotipinin mide kanserine yatkınlık oluşturduğu düşünülebilir. Objectives: Despite decreasing incidence of gastric cancer in worldwide, it is still a major health problem. Every year, 30.000 new gastric cancer cases emerging, and it is the second most common cancer in Turkey. Gastric cancer is a complex multifactorial disease, emerging by interaction between genetic and environmental factors. Survivin, apoptosis inhibitory protein is over-expressed in cancer tissue. In this study, association between Survivin -31G/C polymorphism and gastric carcinoma was investigated. Materials and Methods: 46 gastric carcinoma patients who had been admitted at Düzce University Research and Practice Hospital, Laboratory of Pathology and 42 healthy individuals have been included in the study. Samples have been subjected to genetic analysis by PCRRFLP method in Medical Genetics Department laboratory at Düzce University. Results: GG genotype was found in 16 (34.8%), GC genotype in 21 (45.7%), CC genotype in 9 (19.6%) in patient group. In control group, genotype distribution were found 13 (31%), 26 (61.9%) and 3 (7.1%) respectively. The statistically significant difference was not found when compared between patient and control groups. However, we observed the increased occurrence of gastric cancer associated with CC genotype (OR=1.52). Conclusions: In our knowledge, this study is the first to evaluate the relationship between gastric carcinoma and Survivin -31G/C polymorphism in Turkish population. Our results show that there is no any association between gastric carcinoma and Survivin -31G/C polymorphism in the community which is represented by our study and control groups. However, it was concluded that CC genotype may create the susceptibility to gastric cancer.

Öncelikle bu mantarlar toplanmış, ekstrakte edilmiş, 2 defa preparatif HPLC ile saflaştırma işlemi uygulanmıştır. Toksinin karşılaştırması, analitik HPLC sisteminde tutulma zamanı ve ultraviyole spektrumu karşılaştırılması yöntemleriyle yapılmıştır. Bulgular: İlk saflaştırma sonucunda elde edilen beta amanitin saflık oranı %91(±2,36) olarak ölçülmüştür. İkinci saflaştırma sonucunda elde edilen beta amanitin saflık oranı %99,2(±0,38) olarak ölçülmüştür. Saflaştırma sonucu elde ettiğimiz toksin ile beta amanitin standardın UV spektrumlarında her ikisinde de 303 nm’de maksimum, 263 nm’de minimum absorbans verdiği ve spektrum yapısının aynı olduğu görülmüştür. Sonuç: Tanımladığımız bu yöntemle, >%99 saflıkta beta amanitinin üretilmesi mümkündür. Bu çalışmada, son yıllarda önemli bir kurumsal yönetim aracı olarak kabul edilen iç denetim ve günümüzde en geniş kabul gören iç denetim yaklaşımı olan risk odaklı iç denetim yaklaşımı ele alınmış, risk odaklı iç denetim yaklaşımına ilişkin metodoloji, araç ve teknikler belirli bir sistematik dahilinde ortaya koyulmuştur.Çalışmada öncelikle iç denetimin varlık sebebini açıklayan ve kavramsal çerçevesini ortaya koyan kuramlar tespit edilerek, iç denetimin bu kuramlar ile ilişkisi değerlendirilmiştir. In this study, internal auditing, acknowledged as one of the most important corporate governance tools and risk based internal auditing as the most widely accepted approach of internal auditing is discussed in detail. The methodologies, tools and techniques of risk based internal auditing is examined in detail.Firstly in the study, the theoretical framework explaining the conceptual basis of internal auditing is identified, then the relationship between these theories and internal auditing is evaluated. After this evaluation, the functioning of internal auditing as a solution for some of the corporate problems referred within the context of these theories is discussed.In the study, the effectiveness of risk based internal auditing as an objective and independent corporate governance tool is evaluated in terms of services provided towards achievement of organizational goals, preseving the economic continuity of the organization and protecting the interests of the stakeholders. Purpose: Nilvadipine is a dihydropyridine-type calcium channel blocker with emanatingneuroprotective properties in various models of neuronal diseases. This study aimed atinvestigating the prophylactic effect of nilvadipine on memory impairment and hippocampalmalodialdehyde (MDA) levels in global brain ischemia model induced by 4-vessel occlusionischemia (4-VO) in rat.Materials and methods: The 4-VO ischemia was induced in Wistar rats by occluding thevertebral arteries permanently by cauterization. The common carotid arteries were twiceoccluded bilaterally for 10 minute at 60 minute interval. One week after 4-VO the memory wasevaluated measuring the correct and error choices in 8-armed radial maze. The ischemiareperfusion-induced damage was evaluated measuring level of MDA in the hippocampus usingthiobarbituric acid method. The study involved three groups: sham, ischemia-control andischemia nilvadipin. Aim: To evaluate teriparatide and alendronate sodium on bone mineral density ofpostmenopausal women.Materials and methods: Thirty-three postmenopausal women who had been treated withteriparatide and alendronate sodium were evaluated, retrospectively. Patients with a secondaryosteoporosis cause were excluded from the study. Aim: Caffeine has a stimulating effect on the central nervous system functions and are includednaturally in beverages such as tea and coffee while it is added later some beverages such ascola. Different amounts of caffeine are consumed with these drinks in many countries. Inparticular, the amount of caffeine in Turkish coffee is not clear. In our study, caffeine levelswere measured at beverages and Turkish coffee from different markets in Turkey.Method: Three samples were collected from each of teas, coffees, Turkish coffee, soft drinksand energy drinks at beverages sold different brands and forms. After maintainingstandardization in the preparation of beverages, the amounts of caffeine were analyzed withultraviolet determination method by HPLC.Results:The highest concentration of caffeine in drinks in the measurement was found to bethe Turkish coffee (858mg/L). Caffeine levels were measured in normal tea 840 mg/L ,in solublecoffee 509 mg/L, in tea bags 218 mg/L, in coke 148 mg/L and in energy drinks 169 mg/L.Discussion and Conclusion: Caffeine containing drinks are consumed through the entire lifefrom childhood to senescence. ÇalışmamızdaTürkiye’de marketlerde satılan farklı markalardan elde edilen içeceklerdeki kafein miktarıölçüldü.Yöntem: Farklı marka ve farklı formlarda satılan kahveler, çaylar, diyet içecekleri, enerjiiçecekleri ve Türk kahvesinin her birinden 3 örnek alındı. İçeceklerin hazırlanmasındastandardizasyon sağlandıktan sonra, içeceklerdeki kafein miktarı HPLC yöntemiyle analiz edildi.Bulgular: Ölçüm yapılan içecekler içerisinde en yüksek kafein konsantrasyonunun Türkkahvesinde olduğu saptandı (858 mg/L). The long QT syndrome (LQTS) is characterized with arrhythmic attacks, repeating syncopes,seizures, and ventricular arrhythmias leading to sudden death. Arrhythmias with a potential ofmortal course develop due to elongation of ventricular repolarization, either acquired or inborn.Herein the present paper, a case of congenital LQTS, admitted after a sudden cardiac arrest andtotally recovered with implantable cardiac defibrillator (ICD), was focused on, Uzun QT sendromu (LQTS); çarpıntı atakları, tekrarlayan senkop, konvulsiyonlar ve ani ölümeneden olabilen, ventriküler aritmilerle karakterize bir hastalıktır. Mortal seyredebilen aritmiler,ventriküler repolarizasyon fazındaki uzama sonucu oluşur ve bu uzamalar edinsel yadakonjenital olabilir. Kaya, Ertugrul/0000-0003-0081-682X WOS: 000317245800051 Phosalone (6-chloro-3-[diethoxyphosphinothioylsulfanylmethyl]-1,3-benzoxazol-2-one) is one of the most commonly used organophosphorus pesticides in the peat control of crops. Subchronic phosalone exposure was evaluated for its effects on the serum activities of some enzymes concerning hepatic and pancreatic damage including aspartate aminotransferase (AST), alanine aminotransferase (ALT), glutamyltransferase (GGT), lactate dehydrogenase (LDH), alkaline phosphatase (ALP), cholinesterase (ChE); and finally protective effects of combination of vitamins E and C in 24 wistar-albino rats. Experimental groups were as follows: control group (n = 8); a group treated with 120 mg/kg body weight phosalone (P group, n = 8); and a group treated with 120 mg/kg body weight phosalone + vitamin E + vitamin C (P+V group, n = 8). The P and P+V groups were treated orally with phosalone on 5 days a week for 4 weeks. The serum activities of the above mentioned enzymes were analyzed. In the samples phosalone significantly increased the activities of ALT, LDH and decreased ChE (p < 0.05). However no significant change was detected for the remainder enzymes (p > 0.05). In the P+V group, ALT and LDH activities were significantly increased and ChE decreased (p < 0.05). It is concluded that subchronic phosalone causes rat liver damage to an extent, which is somewhat reflected on the liver enzymes. Furthermore, a combination of vitamins E and C can reduce the toxic effects of phosalone on liver tissue of rats.

En zehirli mantar türlerinden olan Amanita phalloides isimli şapkalı mantar türünün kloroform, aseton, metanol ve distile su ile hazırlanan ekstraklarının, Candida parapsilosis, Candida albicans, Metisilin duyarlı Staphylococcus aureus (MSSA), Enterococcus faecalis, Escherichia coli, Pseudomonas aeruginosa ve Metisilin dirençli Staphylococcus aureus(MRSA) türlerine karşı antimikrobiyal etkisi araştırıldı. Yöntem olarak in vitro disk difüzyon yöntemi kullanıldı. Bulgulara go¨re; en yüksek antibakteriyel aktivitesi MRSA ve MSSA tu¨rlerine kars¸ı bulunmakla birlikte digˆer bakterilere kars¸ı da farklı seviyelerde antibakteriyel aktivite go¨sterdiği saptandı. Sonuç olarak; sadece Amanita phalloides'in distile su ile hazırlanan ekstraktının bakterilere karşı etkisi olduğu fakat antifungal etki göstermediği saptandı. The antimicrobial effect of Amanita phalloides extracts, one of the most poisonous mushroom species, prepared with chloroform, acetone, methanol and distilled water were investigated against Candida parapsilosis, Candida albicans, Methicillin-susceptible Staphylococcus aureus (MSSA), Enterococcus faecalis, Escherichia coli, Pseudomonas aeruginosa and Methicillin-resistant Staphylococcus aureus (MRSA) species. In vitro disc diffusion method was used as a method. According to the findings; While the highest antibacterial activity was against MRSA and MSSA species, it was determined that it showed different levels of antibacterial activity against other bacteria. As a result; It was determined that only the extract of Amanita phalloides prepared with distilled water had an effect against bacteria but did not show an antifungal effect.

Bağımlılık yapıcı madde kullanımı tüm dünya başta olmak üzere ülkemizde de toplum sağlığını tehdit eden en önemli sağlık sorunların başında gelmektedir. Son yıllarda uyuşturucu ve uyarıcı bağımlılık yapıcı madde kullanımlarında artış görülmektedir. Son birkaç yılda ülkemizde uyuşturucu kullanımı ve bağımlılık yapıcı maddelerin dağıtımı ile mücadele başlamıştır. Toplumda yükselen bağımlılık yapıcı madde kullanımı oranlarının nedenleri arasında; yeni, ucuz, kolay ulaşılabilen, zor tespit edilebilen bağımlılık yapıcı madde satışının artması gösterilmektedir. Özellikle de sentetik kannabinoid maddesi kullanımlarında artış görülmektedir. Bu çalışmada amacımız laboratuvarımızda yapılan uyuşturucu analizlerinde kullanılan iki farklı yöntem arasında sentetik kannabinoid analizleri üzerinde herhangi bir farklılık olup olmadığını ortaya koymaktır. Her iki yöntem olan LC-MS/MS (sıvı kromatografisi, kütle spektrofotometresi) ve enzim immünassay yöntemi ile analiz yapan otoanalizör cihazları günümüzde uyuşturucu madde ve ilaç analizlerinde kullanılan güvenli ve hassas yöntemlerdir. ALFA AMANİTİNİN SIÇANLARDA TOKSİKOKİNETİĞİAmanita phalloides dünyadaki görülen ölümcül mantar zehirlenmelerinin %95'in üzerinde sorumlu olan mantar türüdür. Bu mantarın 2 grup toksini tanımlanmıştır. Bunlar amanitin grubu ve falloidin grubu toksinler olarak bilinmektedir. Amanitin grubu toksinlerden mantar içinde en fazla bulunan ve hakkında en fazla araştırma yapılan toksin ?-amanitindir. Bu grup toksinler RNA polimeraz II enziminin spesifik inhibitörüdür. Mantarlarla olan zehirlenmelerde bu toksinin toksikokinetiği çok önemlidir. Ancak bu konuda yapılan araştırmalar yetersizdir.Yaptığımız saha çalışmalarında Ekim-Kasım 2010 aylarında Düzce çevresindeki ormanlardan yeterli miktarda Amanita phalloides mantarı toplanmıştır. Mantar içeriğindeki toksin analizi ve mantarlardan ?-amanitin saflaştırılması HPLC cihazıyla gerçekleştirilmiştir. Çalışmamızda, anestezi altındaki sıçanlara oral ve intravenöz yoldan 1 mg/kg ?-amanitin verildikten sonra farklı zaman dilimlerinde (2.-5.-10.-15.-20.-45.-60.-90. dakika ve 2.-3.-4.5.-6.-9.-12.-24 saat) alınan kanların serumlarında ?-amanitin toksininin HPLC yöntemiyle C18 kolon, amonyum asetat: asetonitril: metanol (80:10:10,v/v/v) mobil faz ve 1 mL/dak izokratik akış hızı kullanılarak analizi yapıldı.Yapmış olduğumuz analizler sonucunda, ?-amanitin toksikokinetiğinin ?iki kompartmanlı dışarıya açık model?e uygunluk gösterdiği, oral biyoyararlanımının F=0.936, tdoruk: 2-3 saat (2.64), Cdoruk-oral = 2.86 ± 0.08 µg/ml, Cdoruk-i.v. = 12.01 ± 0.07 µg/ml, klirensinin CL = 75.91 ± 9.11 ml/st/kg ve eliminasyon yarılanma ömrü (t1/2ß) 110.4 ± 0.30 dak. olduğu saptandı.Çalışmamızda oral biyoyararlanım oranı, eliminasyon yarılanma ömrü, tdoruk, Cdoruk, klirensin belirlenmesi ve literatüre kazandırılması önemli bir aşamadır. There have been determined two groups of toxin in these mushrooms. These toxins are known as amanitin group and phalloidin group. Among toxins, the most frequent toxin is ?-amanitin type toxin and there are a lot of distributions in the mushroom. ?-amanitin is specific inhibitor of RNA polymerase II. Toxicokinetics of this toxin is very important about poisoning caused mushrooms but pharmacokinetics of this toxin has not been sufficiently determined.In recent field study at October-December in 2010, Amanita phalloides was adequately collected in forest around the Düzce. Toxins in the mushrooms were analyzed with HPLC method. In our study, ?-amanitin (1 mg/kg) was administrated via oral and intravenously in rats at different times ((2.-5.-10.-15.-20.-45.-60.-90. minutes and 2.-3.-4.5.-6.-9.-12.-24 hours) under anaesthesia. After that, we will give ?-amanitin to rats. In the different times, blood specimens of the taking from rats will be analyzed for ?-amanitin by using HPLC methods (C18 column, amonium asetate: acetonitrile: methanol (80:10:10,v/v/v) mobile phase, and 1 mL/min. isoacratic flows).Results from analyzes that we were done, toxicokinetics of ?-amanitin is demonstrate compliance with ?two compartment-open model?, oral bioavailability F = 0.936, tmax: 2-3 hours (2.64), Cmax-oral = 2.86 ± 0.08 µg/ml, Cmax-i.v. = 12.01 ± 0.07 µg/ml, clearance CL = 75.91 ± 9.11 ml/h/kg, elimination half life (t1/2ß) 110.4 ± 0.30 min.We explain parameters of toxicokinetics of ?-amanitin which oral bioavailability, elimination half life, tmax, Cmax, clearance and earned to literatures in rats. ÖZETFARELERDE AMANİTA PHALLOİDES TOKSİNİ ALFA AMANİTİNİNDERİDEN EMİLİMİMustafa Gani SÜRMENYüksek Lisans Tezi, Farmakoloji Anabilim DalıTez danışmanı Yrd. Doç. Amanitin grubu toksinler ökaryot hücrelerde bulunan RNA polimeraz II enzimini bloke ederek etki gösterir. Alfa amanitin birçok ökaryot hücre tipinde letal olduğundan, antiparaziter ve antifungal ilaç olarak terapotik değeri olabilir. Alfa amanitinin ökaryotik ektoparazitlere karşı harici olarak kullanımı mümkün olabilir. Alfa amanitinin söz konusu terapotik etkinliğinde kritik nokta, perkutan uygulamada konağa toksisitesinin olmamasıdır. Bu araştırmada alfa amanitinin perkutan yolla deriye uygulanması durumunda, emiliminin ve toksisitesinin varlığı araştırılmıştır. Bu amaçla farelere perkutan olarak 1 mg/kg alfa amanitin uygulanmış, aynı dozda intraperitoneal uygulamadaki toksisite ile karşılaştırılmıştır. Deri, karaciğer ve böbrek toksisitesi patolojik inceleme yoluyla; kan, karaciğer ve böbreklerdeki toksin miktarı ise HPLC analizleri yoluyla incelenmiştir. Kontrol grubunda deride ve organlarda toksisiteye ve toksine rastlanmamıştır. İlginç olarak perkutan toksin uygulanan grupta da deride ve organlarda toksisiteye ve toksin varlığına rastlanmamıştır. İntraperitoneal uygulamada 24 saat sonunda karaciğerde apoptotik konsülman cisimcikleri ve piknotik hücreler görülmüş ve böylece toksisite varlığı gösterilmiştir. Aynı uygulama yönteminde, karaciğer, böbrek ve kanda maksimum alfa amanitin düzeyleri sırasıyla 3.3(6 saat), 0.2(6 saat), 1.2(1 saat) µg/mL olarak ölçülmüştür. Sonuç olarak; alfa amanitinin perkutan uygulama ile kullanımı durumunda, deride ve organlarda toksisitesinin olmadığı ve deriden emilmediği gösterilmiştir. Those are amanitin and falloidin. Amnitin group becomes effective with blocking RNA polymerase enzyme II found in eukaryotic cells. Since Alpha amanitin has a lethal effect on the majority of eukaryotic cells, it can be valuable as antiparasitic drug or antifungal drug. It can be used externally against ectoparasites. The critical point is that toxin must not be toxic for host via percutaneous application in terms of the mentioned therapeutic effectiveness of alpha amanitin. In this study, the likely absorption and toxicity has been researched for application percutaneous of alpha amanitinin. 1 mg/kg alfa amanitin has been administered percutaneously in mice and the results of percutaneous application has been compared with intraperitoneal application of 1 mg/kg alfa amanitin on account of toxicity. The toxicities of skin, liver and kidney have been researched by pathological examination. As for the amount of toxin, it has been studied by HPLC analysis. The toxicity and the toxin haven?t been found in the skin and organs of mice in the control group. Interestingly enough, the toxicity and the toxin a could not be found in percutaneous application group. After 24 hours councilman bodies and pyknotic cells were observed on intraperitoneal application and so toksisty existence was proved. In liver, kidney and blood maximum levels of alpha-amanitin were measured by same application method as 3.3(6 hours), 0.2(6 hours), 1.2(1 hours) µg/mL in turn. Eventually, it has been demonstrated that the toxin is not absorbed from mouse skin and the toxin doesn?t have any toxic effect via the percutaneous alpha amanitin application. Amaç: Bu çalışmada sisplatin nefrotoksisitesi oluşturulmuş sıçanlarda N-asetilsistein proflaksi ve tedavisinin ve bu tedaviye C vitamini eklenmesinin böbrek fonksiyonları üzerine olan koruyucu etkisi 99mTc-MAG3 birlikteliğinde araştırıldı.Yöntem: Erişkin Wistar sıçanlar 4 gruba ayrıldı(n=8). Tüm hayvanların 99mTc-MAG3 ile böbrek bazal sintigrafik görüntüleri alındı. Kontrol grubuna serum fizyolojik verildi. Nefrotoksisite grubuna, üçüncü ve dördüncü gruplara deneyin dördüncü günü tek doz intraperitoneal 12 mg/kg sisplatin enjeksiyonu yapıldı. Üçüncü ve dördüncü gruplara yedi gün boyunca her gün intraperitoneal 500 mg/kg N-aetilsistein verildi. Dördüncü gruba yedi gün boyunca her gün ek olarak intraperitoneal 250 mg/kg C vitamini de uygulandı. Deneyin sonunda tüm hayvanların anestezi altında 99mTc-MAG3 ile böbrek sintigrafileri çekildi, biyokimyasal ve histopatolojik incelemeler için intrakardiyak kan örnekleri alınıp böbrek dokuları çıkartıldı.Bulgular: Sisplatinin böbreğin konsantrasyon ve ekskresyon fonksiyonlarını bozduğu, perfüzyonu etkilemediği, N-asetilsisteinin bunu düzelttiği, N-asetilsistein + C vitamininin bu konuda daha etkili olduğu, konsantrasyon fonksiyonlarını tama yakın düzelttiği gözlenmiştir. Oksidatif hasar göstergesi serum ve doku MDA düzeylerine ve serum BUN, üre, kreatinin düzeylerine baktığımızda sisplatin grubunda arttığı, N-asetilsistein ve N-asetilsistein + C vitamini gruplarında azaldığı ancak kontrol düzeylerine indirilemediği görülmüştür. Total antioksidan kapasite düzeyleri sisplatin grubunda azalırken, N-asetilsistein ve N-asetilsistein + C vitamini gruplarında düzelme gözlenmiştir. Histopatolojik incelemelerde tübüler nekrozun N-asetilsistein ve N-asetilsistein + C vitamini gruplarında sisplatin grubundan daha az olduğu gözlenmiştir.Sonuç: Sisplatin nefrotoksisitesinde N-asetilsisteinin oksidatif hasarı azalttığı fakat böbrek fonksiyonlarını korumada yeterli olmadığı, bu tedaviye C vitamininin eklenmesi ile oksidatif hasarın daha da azaldığı ve böbrek fonksiyonlarının oldukça düzeldiği görülmüştür.Anahtar kelimeler: N-asetilsistein, C vitamini, sisplatin, nefrotoksisite, 99mTc-MAG3 Background and Aim: This study investigated the protective and curative effects of N-acetylcysteine treatment alone and together with ascorbic acid on renal functions by using 99mTc-MAG3 in cisplatin-induced nephrotoxicity.Methods: Adult Wistar rats were divided into four groups (n=8). All the rats? baseline scintigraphic images were taken by 99mTc-MAG3 under anesthesia. Control group received normal saline. Nephrotoxicity group, third and fourth groups received a single intraperitoneal injection of 12 mg/kg cisplatin on the fourth day of experiment. The third and fourth groups were given intraperitoneal N-acetylcysteine (500 mg/kg per day for 7 days). In addition the fourth group was administered intraperitoneal ascorbic acid (250 mg/kg per day for 7 days). At the end of the experiment all animals? renal scintigraphic images were taken by 99mTc-MAG3 under anesthesia. Intracardiac blood samples were taken, kidneys were removed for biochemical and histopathological evaluation.Results: It was observed that cisplatin impaired concentration and excretion functions of kidney and not affected perfusion, N-acetylcysteine improved this, N-acetylcysteine + ascorbic acid was more effective, it almost completely ameliorated concentration functions. Serum BUN, urea, creatinine levels, serum and tissue MDA levels which indicates oxidative injury increased in cisplatin group, decreased in . Total antioxidant capacity levels decreased in cisplatin group, increased in N-acetylcysteine and N-acetylcysteine + ascorbic acid groups. Tubular necrosis was less severe in N-acetylcysteine and N-acetylcysteine + ascorbic acid groups than cisplatin group in histopathologic analysis.Conclusion: N-acetylcysteine decreased oxidative damage in cisplatin-induced nephrotoxicity, but it was not sufficient to maintain kidney functions. It was observed that adding ascorbic acid to the treatment supported antioxidative effects, kidney protective effects, so it can be a suitable alternative.Key words: N-acetylcysteine, ascorbic acid, cisplatin, nephrotoxicity, 99mTc-MAG3 96
