PURPOSE: To investigate the effects of preoperative fractioned irradiation using an electron beam on the healing process of colocolonic anastomoses in rats that underwent early and late surgical intervention. METHODS: Thirty Wistar rats, distributed as follows: group A (surgery only), group B (fractionated irradiation for 30 days (if), surgery seven days after the end of it), group C (if for 30 days, and surgery after 30 days of termination). On the seventh postoperative day the anastomotic segment analysis was taken, using tension tests, histology and collagen deposition evaluation by computerized analysis. RESULTS: Regarding the tension resistance of the anastomosis, there were no statistical differences (p=0.42). Aims: The prognosis on treatment of the cancer of the rectum has not changed in the last fifty years. Survival rates of 50 to 55% seems immutable in several published series. The main cause for those results is the high incidence of recurrence, either local or widespread. Local recurrence is directly related to the number of undifferentiated cells and to the grade of wall invasion. Widespread recurrence depends specifically on the lymphatic and vascular spreading. So any kind of treatment that would diminish the number of undifferentiated cells and the size or the tumor wall penetration would certainly decrease the local recurrence rate, lengthening the interval free from cancer and, perhaps, modifying the long term survival rate. Between 1978 and 2009, a total of 538 patients with adenocarcinoma of the lower rectum (from the pectinate line to 10 cm above) were treated by preoperative radiotherapy. Methodology: The same protocol was used in all the patients – 400 cGy, 200 cGy/day, during 4 consecutive weeks (anterior and posterior pelvic fields) by means of a Linear Megavoltage Accelerator (25 MeV). Surgery was performed 2 months after completion of the radiotherapy. Results: Statistical analysis of the whole group showed that preoperative radiotherapy does decrease frequency of undifferentiated cells. Moreover, the incidence of local recurrence diminished after irradiation by 3.4%. Preoperative radiotherapy reduces tumor volume (ERUS) and wall invasion, as well as the mortality rate due to local recurrence (2.4%) and alters long-term survival rate (80.1%). Conclusion: Preoperative radiotherapy is really effective in reducing the number of undifferentiated cells and in diminishing the tumor volume and the carcinomatous infiltration of the rectal wall.

O retardo da cicatrização de feridas em pacientes diabéticos eleva os custos hospitalares e a morbidade dos procedimentos operatórios. O objetivo deste trabalho é avaliar a influência da suplementação dietética de óxido de zinco e cromo (ZC) na reparação tecidual de feridas cutâneas. Foram estudados 69 ratos Wistar, pesando entre 200-270 gramas, separados em cinco grupos (G). Na primeira semana do experimento induziu-se diabetes com streptozotocin em 53 animais (GIII, GIV, GV). A partir do 8º dia os ratos normais (GI, GII) e os diabéticos receberam, além de ração e água: GI, nenhum tratamento (NT); GII, ZC adicionados à água; GIII, NT; GIV, insulina; GV, ZC e insulina. No 15º dia foi retirado fragmento elíptico de pele e subcutâneo do dorso dos animais. Nos 15º, 22º, 29º e 36º dias, as feridas operatórias foram fotografadas. Essas fotografias foram transferidas para computador, onde foram calculadas as suas áreas. Os valores obtidos foram submetidos à análise estatística. No 22º dia da avaliação, não houve diferença significante (NS) na redução das áreas das feridas operatórias entre os grupos. Nas datas seguintes, os ratos diabéticos não tratados (GIII) também não apresentaram diferença significante na redução das áreas das feridas operatórias (RA) em relação aos normais (GI). Mas os tratados com zinco e cromo e com insulina (GII, GIV e GV) apresentaram RA significativamente maior do que os do GIII. O GV apresentou maior RA do que o GIV, porém NS. Conclui-se que o tratamento de ratos diabéticos com insulina e a suplementação dietética de ZC favorecem a cicatrização de feridas cutâneas. The delay in tissue repair of cutaneous wounds in diabetic patients increase hospital costs and morbidity in surgical procedures. The purpose of this study is to assess the influence of zinc oxide and chromium as diet supplements (ZC) in tissue repair of cutaneous wounds. This study was carried out in 69 Wistar rats with weight ranging from 200 to 270 grams divided in 5 (five) groups (G). Diabetes was induced in 53 animals with streptozotocin in the first week of the study (GIII, GIV, and GV). After the 8th day, normal (GI, GII) and diabetic rats (GIII, GIV, GV) received diet supplements together with food and water: GI, no treatment (NT); GII, ZC added to the water; GIII, NT; GIV, insulin; GV, ZC and insulin. On the 15th day an elliptical fragment was taken from the skin and from the dorsum of the animals. The operative wounds were then photographed on days 15, 22, 29 and 36. These pictures were sent to a computer and wound areas were calculated. The scores obtained were then subjected to statistical analysis. On the 22nd day of assessment there was no significant difference (NS) in the reduction of the operative wound site among the groups (RA). In the following dates the diabetic rats that did not receive the treatment (GII) also did not have any significant difference in RA in comparison to normal rats (GI). On the other hand, those rats treated with zinc, chromium and insulin (GII, GIV. and GV) had a higher RA level than those of the GIII did. The GV had a higher level of RA than GIV, but it was not significant. (NS). The conclusion was that the treatment of diabetic rats with insulin and diet supplement with ZC improved the healing of cutaneous wounds.

Foram estudados e analisados de forma prospectiva os resultados obtidos em 24 pacientes portadores de câncer do reto baixo (tumores situados entre a linha pectínea e os 4cm acima da mesma) operados por via laparoscópica. Somente foram avaliados pacientes portadores de adenocarcinoma, independentemente da idade, do sexo ou da raça do paciente. A observação macroscópica e anatômica dos espécimens extirpados por via laparoscópica permitiu concluir que é possível realizar-se uma excisão total do mesorreto (excisão total perirretal) com as mesmas definições descritas por Heald:l.2 a excisão total perirretal foi obtida em 91,3% dos pacientes operados por via laparoscópica. Em paciente algum registrou-se a ruptura da massa tumoral ou contaminação celular pélvica intra-operatória. A extensão de ressecção linfonodal foi similar à obtida por via laparotômica, com uma média de 12 linfonodos dissecados por espécimen, com um grau de positividade no tecido perirretal de 33,3 % dos pacientes, proporção esta que chega a 72,7% se analisados os pacientes portadores de tumores Dukes C e a 100% para os tumores com baixo grau de diferenciação celular. A proporção de positividade (número de linfonodos com metástases em relação ao número de linfonodos extirpados) variou de um mínimo de 11,11% a um máximo de 75%. A radioterapia pré-operatória não foi óbice para a execução da cirurgia por via laparoscópica: em paciente algum ocorreu qualquer tipo de complicação peroperatória em decorrência da irradiação prévia. Da mesma forma, a irradiação não foi causa de conversão de um método cirúrgico para outro. O índice de recidiva local observado após amputação abdômino-perineal do reto por via laparoscópica com excisão total perirretal em pacientes previamente irradiados foi de 4,2%. Não foram registradas, até o momento, metástases ao nível da introdução dos trocartes. Twenty-four patients (1993-1997) with cancer of the lower rectum (tumors situated between the pectinate line and four cm above it) underwent abdominoperineal excision by laparoscopic approach and its results were studied and analyzed. Only adenocarcinoma tumors were evaluated regardless of age and gender. All patients were submitted to preoperative radiotherapy (4.000 cGy) and operated on ten days after the end of the irradiation. The anatomic observation of the specimens excised by laparoscopy led to the conclusion that a total mesorrectum excision (total peri-rectal excision) was achieved in 91.3% of the patients. Preoperative radiotherapy did not create intra-operative hazard or change the surgical procedure. None of the patients had intra-operative rupture of the tumor or pelvic spillage of carcinomatous cells. The extension of linfonodal excision was similar to the one obtained by open surgery, with an average of 12 lymphnodes dissected by specimen, with positive perirrectal lymphnodes in 33.3% of the patients, or in 72.7% of the patients with Dukes C tumors and in 100% of the tumors with low grade of cellular differentiation. The proportion of positivity (number of metastatic lymphnodes in relation to the number of extirpated lymphnodes) varied from a minimum of 11.11% to a maximum of 75%. The rate of local recurrence observed after abdominoperineal excision of the rectum by laparoscopic approach with total excision of the perirretum in patients previously irradiated was of 4.2%, patient with Dukes C tumor and low grade of cellular differentiation. There were no portal implantation in the series.

Com o objetivo de avaliar a influência de bloqueadores H2 no processo de cicatrização de lesões gástricas suturadas foram estudadas vinte cobaias machos. Após anestesia geral, laparotomia, gastrotomia de 0,5cm de extensão na parede anterior do corpo gástrico e gastrorrafia, os animais foram separados em dois grupos (GI e GII) com dez cobaias cada. As do GI receberam injeções de 0,1ml de água destilada e as do GIl 0,75mg de ranitidina, por via intramuscular, de 12/12 horas, iniciadas imediatamente após a operação e mantidas até as datas de sacrifício, realizado em número igual de cobaias, no segundo e no décimo dias de pós-operatório (PO). As peças operatórias ressecadas foram avaliadas detalhadamente. À macroscopia observou-se deiscência de sutura gástrica em um animal do GI no décimo dia de PO. A face mucosa de todas as peças apresentava enantema na linha de sutura no segundo dia de PO, porém, no décimo dia de PO o aspecto era normal, com pequena cicatriz branca no local. À microscopia, no segundo dia de PO, observou-se processo inflamatório agudo grave em ambos os grupos. No décimo dia de PO evidenciou-se inflamação moderada no GI e leve no Gil. À morfometria não houve diferença significante entre os grupos, em relação ao número de células inflamatórias, mas a densidade volumétrica de fibroblastos e de fibras colágenas foi significativamente maior no GII no décimo dia de PO. Concluiu-se que o tratamento de cobaias com bloqueadores H2 não provocou retardo da cicatrização de feridas gástricas suturadas. In order to evaluate the influence of H2-blockers in the healing process of sutured gastric wounds, twenty male guinea pigs were studied. After general anesthesia, followed by a laparotomy, a 0.5 cm-long gastrostomy in the anterior wall of the gastric body and final suturing. The guinea pigs were separated into two groups (GI and GII), with ten animais each. Group I received injections of 0,1 ml of distilled water and GII received 0,75 mg of ranitidine by a intramuscular injection every 12 hours, starting immediately after surgery and maintained until sacrifice, which was conducted in equal numbers of animais on the 2nd and 10th postoperative day (PO). Operative specimens were evaluated in detail. In macroscopic evaluation, dehiscence of gastric suture was observed in one GI animal on the 10th PO day. The mucosal membrane of all specimens presented erythema on the suture on the 2nd PO day; however, on the 10th PO day the aspect was normal, with a small white scar on the site. In microscopic evaluation, there was a severe acute inflammatory process in both groups on the 2nd PO day. III the morphological evaluation and cellular counting, there were no significant differences between groups regarding the number of polymorphis neutrophils, lymphocites and plasma cells, but the volumetric density of fibroblasts and collagen fibers was significantly larger in GII on the 10th PO day. Therefore, we conclude that H2-blockers caused no delay in healing of sutured gastric wounds in guinea pigs.

Com o objetivo de padronizar normas técnicas para obtenção de modelo animal com tumor de Walker 256 e de estabelecer o número de células tumorais necessárias para que esse tumor tenha grande porcentagem de pega e longevidade, possibilitando o desenvolvimento de pesquisas em várias áreas da saúde, foi realizado trabalho em duas etapas. Na primeira foram utilizados 120 ratos para treinamento e definição da técnica. Na segunda etapa foram utilizados 84 ratos, sendo estes separados em 7 grupos (G) de 12 animais cada. O tumor, na forma ascítica, foi inoculado no tecido celular subcutâneo do dorso dos ratos com os seguintes números de células: GI, 1 x 10(7); GII, 5 x 10(6); GIII, 2,5 x 10(6); GIV, 1 x 10(6); GV, 5 x 10(5); GVI, 3 x 10(5) e GVII, 2 x 10(5). Foram avaliadas a porcentagem de pega e a longevidade nos grupos. Os animais dos GI, GII, GIII e GIV obtiveram 100% de desenvolvimento tumoral, porém baixa longevidade. Os dos GV e GVI obtiveram desenvolvimento tumoral em frequência maior que 90% e longevidade satisfatória. Os do GVII não apresentaram desenvolvimento tumoral. Concluiu-se que todos os procedimentos devem ser exaustivamente treinados e que o número de células tumorais viáveis para inoculação, em tecido celular subcutâneo de ratos, deve estar na faixa entre 5 x 10(5) e 3 x 10(5). The work was realized in two stages. In first were used 120 rats to crew’s training and technicals definitions. In second stage were used 84 rats, these separated in 7 groups (G) with 12 animals each one. The tumor, in ascitic form was inoculated on subcutaneous cellular tissue on dorsal of rats with the follow number of cells : GI, 1 x 10(7); GII, 5 x 10(6); GIII, 2,5 x 10(6); GIV, 1 x 10(6); GV, 5 x 10(5); GVI, 3 x 10(5) e GVII, 2 x 10(5). Were evaluate the join percentage and longevity in groups. The animals of GI, GII, GIII e GIV obtained 100% of tumorous development, however low longevity. The two GV and GVI had tumorous development often high that 90% and satisfactory longevity. The GVII didn’t show tumorous development . The conclusion was made all procedures must be exhaustivelly trained and the number of tumorous cells passable to inoculation, in subcutaneous cellular tissue of rats, must be in the range of 5x10(5) and 3x10(5) .

A ressecção do esôfago sem toracotomia vem sendo utilizada com maior freqüência, nos últimos anos, para as afecções benignas, sobretudo no megaesôfago avançado. A vantagem dessa via de acesso é a de evitar o comprometimento da dinâmica pulmonar, mas, entretanto, podendo haver abertura da pleura, com o conseqüente hemopneumotórax, além da potencial agressão a outros órgãos mediastinais com morbidade pós-operatória muitas vezes expressiva. Por sua vez, no megaesôfago avançado, a esofagite de estase predispõe à instalação de carcinoma. Com base nessas considerações, foi proposta, previamente em animais e cadáver humano, a retirada da mucosa-submucosa do esôfago, mediante sua invaginação completa, sem toracotomia. Os resultados satisfatórios estimularam a continuação nessa linha de pesquisa, iniciando-se a experiência na área clínica. Assim, o presente trabalho teve por objetivo demonstrar o resultado do pós-operatório imediato, a técnica de retirada da mucosa do esôfago pelo descolamento submucoso, conservando a túnica muscular intacta no mediastino. O procedimento foi realizado pela via cervicoabdominal em 60 pacientes portadores de megaesôfago graus III ou IV. Efetuou-se a reconstrução do trânsito grastrintestinal transpondo o estômago pelo mediastino posterior, por dentro da túnica muscular esofágica ou pela via retroesternal. O estudo permitiu concluir: 1) a ressecção da mucosa pelo plano submucoso, mediante a invaginação, mostrou ser de execução simples e viável em 98,4% dos casos; 2) ausência de sangramento, no intra ou no pós-operatório imediato, cuja origem fosse do leito da túnica muscular esofágica remanescente ao nível mediastinal; 3) baixa incidência de complicações pleuropulmonares - 5,0%. Partial ou total esophagectomy without thoracotomy has been used with greater frequency for the last few years to treat benign affections, specially advanced megaesophagus. Although this procedure presents the advantages of avoiding compromise of lung dynamics, it is not free complications. Among these, we have to emphasize the opening of pleura with hemopneumothorax, together with the potential aggression to other organs in the mediastinum, with a significant postoperative morbidity. On the other hand, in advanced megaesophagus, stasis esophagitis may lead to carcinoma. Based on these considerations, it was proposed, previously in animals and human beings, mucosal and submucosal removal by complete invagination, without thoracotomy. These results were satisfactory in experimental surgery and encouraged the beginning of the clinical experience area. Thus, the aim of the present work is to show, by a detailed analysis, the tecnique of esophageal mucosal and submucosa removal from the muscular layer, main taining it complete at the mediastinum. This procedure was performed by a cervicoabdominal approach in 60 patients with grade III and IV megaesophagus. The reconstruction of the gastrointestinal tract by the stomach transposition was performed through the posterior mediastinum inside the muscular layer or by retrosternal route. The study allowed us the following conclusions: 1) the mucosa removal by the submucosa, through invagination proved to be simple and seasible in 98,4% of the cases; 2) the absence of bleeding from the bed layer muscular esophagus, during or at immediate postoperative period; 3) The incidence of pleura and lung complications - (5,0%.) is low.

A proctocolectomia total com reservatório enteral é a principal técnica utilizada para o tratamento operatório de algumas doenças do intestino grosso. Com o objetivo de estudar as características da mucosa enteral de reservatórios em J, foram operados 36 cães. Após a ressecção de 5 cm do jejunum caudalis, para estudo-controle, os cães foram separados em dois grupos (GI e GII). No GI, foi realizada anastomose êntero-enteral término-terminal; no GII, foi feito um reservatório na forma de "J" no segmento cranialis, que foi anastomosado ao segmento caudalis da mesma forma que no GI. Aos 21, 42 e 63 dias de pós-operatório, foi realizada eutanásia de 6 cães de cada grupo para estudos macroscópico e microscópico, incluindo-se contagens celulares e medidas da lâmina própria da mucosa enteral. Os resultados revelaram, aos 21 dias, edema e enantema, à macroscopia, e necrose à microscopia, em freqüências semelhantes nos dois grupos. Aos 42 e 63 dias, essas alterações diminuiram ou desapareceram no GI e persistiram no GII. Somente neste último foram observadas úlceras à macroscopia aos 21 dias (2 cães) e 42 dias (1 cão). Não houve diferença significante à presença de abscesso, à microscopia, nos dois grupos. Na contagem celular, já aos 21 dias houve aumento da densidade volumétrica de neutrófilos, macrófagos, eosinófilos, plasmócitos e linfócitos nos dois grupos. Todas as células, exceto eosinófilos, apresentavam-se com densidade volumétrica significantemente maior no GII, aos 63 dias. O diâmetro transversal da lâmina própria, excluindo a altura dos vilos, apresentou-se significantemente maior no GII aos 63 dias de pós-operatório. Concluiu-se que a mucosa enteral de reservatórios em J, em cães, apresenta modificações do padrão de normalidade macroscópica e microscópica, entre as quais o aumento da densidade volumétrica de células inflamatórias.Total proctocolectomy with enteral reservoir is, at present, the best technique used for the surgical treatment of some large intestine diseases involving the whole colon. In order to study the characteristics of the "J" reservoir enteral mucosa, thirty-six dogs underwent surgery. After resection of 5cm of the jejunum caudalis the animals were divided into two equal groups (GI and GII) for study-control. In GI, an entero-enteral end-to-end anastomosis was performed; in GII, a "J" reservoir was performed in the cranialis segment of the jejunum and this pouch was anastomosed end-to-end to the jejunun caudalis (similar to GI). On the 21st, 42nd and 63rd postoperative days, six animals of each group were sacrificed for macro and microscopic studies, including cellular counting and the measuring of the enteral mucosa lamina propria. On the 21st day, both groups showed oedema and erythema on macroscopic examination, and necrosis under microscopy in similar numbers. On the 42nd and 63rd days, these alterations decreased or disappeared in GI and persisted in GII. Only in the latter group, on macroscopy, ulcers were observed on the 21st day (2 dogs) and on the 42nd day (1 dog). In both groups there was no significant difference as to the incidence of abscess under microscopy. On the 21st day there was an increase of the volumetric density of neutrophils polymorph, macrophages, eosinophils, plasma cells and lymphocites in both groups. All the cells, except the eosinophils, showed a significant enlargement of the volumetric density in GII on the 63rd day. The transversal diameter of lamina propria, except for the villi height, was significantly larger in GII on the 63rd postoperative day. It was concluded that the enteral mucosa of the "J" reservoir in dogs shows alterations of the normal pattern of macroscopy and microscopy, including the volumetric density enlargement of the inflammatory cells.
