The Cosmology Large Angular Scale Surveyor (CLASS) is a polarization-sensitive telescope array located at an altitude of 5,200 m in the Chilean Atacama Desert and designed to measure the polarized Cosmic Microwave Background (CMB) over large angular scales. The CLASS array is currently observing with three telescopes covering four frequency bands: one at 40 GHz (Q); one at 90 GHz (W1); and one dichroic system at 150/220 GHz (HF). During the austral winter of 2022, we upgraded the first 90 GHz telescope (W1) by replacing four of the seven focal plane modules. These new modules contain detector wafers with an updated design, aimed at improving the optical efficiency and detector stability. Low-risk branch duct intraductal papillary mucinous neoplasms (BD-IPMNs) lacking worrisome features (WF) and high-risk stigmata (HRS) warrant surveillance. However, their optimal duration, especially among cysts with initial 5 years of size stability, warrants further investigation. We systematically reviewed the surveillance of low-risk BD-IPMNs and investigated the incidence of WF/HRS and advanced neoplasia, high-grade dysplasia, and pancreatic cancer during the initial (5 years) and extended surveillance period (5-years).A systematic search (CRD42020117120) identified studies investigating long-term IPMN surveillance outcomes of low-risk IPMN among the Cochrane Library, Embase, Google Scholar, Ovid Medline, PubMed, Scopus, and Web of Science, from inception until July 9, 2021. The outcomes included the incidence of WF/HRS and advanced neoplasia, disease-specific mortality, and surveillance-related harm (expressed as percentage per patient-years). The meta-analysis relied on time-to-event plots and used a random-effects model.Forty-one eligible studies underwent systematic review, and 18 studies were meta-analyzed. The pooled incidence of WF/HRS among low-risk BD-IPMNs during initial and extended surveillance was 2.2% (95% CI, 1.0%-3.7%) and 2.9% (95% CI, 1.0%-5.7%) patient-years, respectively, whereas the incidence of advanced neoplasia was 0.6% (95% CI, 0.2%-1.00%) and 1.0% (95% CI, 0.6%-1.5%) patient-years, respectively. The pooled incidence of disease-specific mortality during initial and extended surveillance was 0.3% (95% CI, 0.1%-0.6%) and 0.6% (95% CI, 0.0%-1.6%) patient-years, respectively. Among BD-IPMNs with initial size stability, extended surveillance had a WF/HRS and advanced neoplasia incidence of 1.9% (95% CI, 1.2%-2.8%) and 0.2% (95% CI, 0.1%-0.5%) patient-years, respectively.A lower incidence of advanced neoplasia during extended surveillance among low-risk, stable-sized BD-IPMNs was a key finding of this study. However, the survival benefit of surveillance among this population warrants further exploration through high-quality studies before recommending surveillance cessation with certainty.

To determine factors associated with localized anterior arthrofibrosis (cyclops lesion), such as graft size, warranting early reoperation for lysis of adhesions after anterior cruciate ligament reconstruction (ACLR) with all-soft tissue quadriceps tendon (ASTQT) autograft.All primary ASTQT autograft ACLRs within a single surgeon's prospectively collected registry with minimum 6-month follow-up were included. Patients who underwent multiligament knee reconstruction or cartilage restoration procedures were excluded. Localized anterior arthrofibrosis was defined as the requirement for a second procedure to achieve debridement and lysis of adhesions owing to the inability to regain terminal extension within 6 months of ACLR. The sex-specific incidence of arthrofibrosis was evaluated relative to age, weight, femoral and tibial tunnel sizes, meniscal repair, and meniscectomy by a binary logistic regression.This study included 721 patients (46% female patients). There were 52 cases of localized anterior arthrofibrosis (7.2%). Female patients had a greater incidence of arthrofibrosis than male patients. Male patients with a femoral tunnel diameter of 9.25 mm or greater had an increased incidence of arthrofibrosis compared with those with a diameter of less than 9.25 mm, whereas a similar cutoff was not found to be statistically significant for female patients. Concomitant meniscal repair was associated with an increased risk of arthrofibrosis.Female sex and concomitant meniscal repair were associated with an increased localized anterior arthrofibrosis incidence. Furthermore, ASTQT with a femoral tunnel diameter of 9.25 mm or greater in male patients was associated with an increased incidence of arthrofibrosis. Silicon carbide (SiC) metal-oxide semiconductor (MOS) power devices such as metal-oxide semiconductor field-effect transistors (MOSFETs) require a stable and low defect-density interface, and a high-quality dielectric, for good device performance and reliability. Notably, the interface and dielectric properties determine the threshold voltage stability, the field-effect channel mobility, and the device lifetime as limited by dielectric breakdown in both the forward on-state and reverse blocking conditions. Here we discuss the present state of SiC MOS processing and properties and point to directions for future development. Important items to address are: 1) interface passivation approaches; 2) dielectrics; 3) device design; and 4) in-depth measurements of the interface quality and reliability.

When bilinguals switch between their two languages, they often alternate between words whose formation rules in one language are different from the other (e.g., a noun-verb compound in one language may be a verb-noun compound in another language). In this study, we analyze behavioral performance and electrophysiological activity to examine the effects of morphological configuration on language control during production and comprehension. Chinese–English bilinguals completed a joint naming-listening task involving cued language switching. The findings showed differential effects of morphological configuration on language production and comprehension. In production, morphological configuration was processed sequentially, suggesting that bilingual production may be a combination of sequential processing and inhibition of morphological levels and language interference. In comprehension, however, bottom-up control processes appear to mask the influence of sequential processing on language switching. Together, these findings underscore differential functionalities of language control in speaking and listening.

The study examined the integrated intervention of psychosocial stimulation, nutrition support and water sanitation hygiene (WASH) on the developmental status of malnourished children in Kanam, Plateau State within a time frame of four months. The study had one (1) Objective, one (1) Research question and one hypothesis. Experimental design was employed with a sample size of 16 malnourished children, age 6–59 months, which were purposively sampled based on the consent of the parents/caregivers. The instruments for data collection were Anthropometric assessment proforma, communication and play tracker, and WASH checklist. A focus group discussion with the caregivers/parents on their knowledge and attitudes in childcare practices were also conducted with weekly and monthly facilitation/support of mother-to-mother counseling visitation as part of intervention strategies for the study. Food supplements formulated from flours of cereal which are commonly used for making complementary food in the study area (millet), a protein-rich ingredient (soybean Glycine max)) and a fat-rich ingredient (sesame). Selective Androgen Receptor Modulators (SARMs) are not FDA approved, and obtaining SARMs for personal use is illegal. Nevertheless, SARM use is increasingly popular amongst recreational athletes. Recent case reports of drug-induced liver injury (DILI) and tendon rupture raise serious concerns for the safety of recreational SARM users. On 10 November 2022 PubMed, Scopus, Web of Science, and ClinicalTrials.gov were searched for studies that reported safety data of SARMs. A multi-tiered screening approach was utilized, and any study or case report of generally healthy individuals exposed to any SARM was included. Thirty-three studies were included in the review with 15 case reports or case series and 18 clinical trials (total patients N = 2136 patients, exposed to SARM N = 1447). There were case reports of drug-induced liver injury (DILI) (N = 15), Achilles tendon rupture (N = 1), rhabdomyolysis (N = 1), and mild reversible liver enzyme elevation (N = 1). Elevated alanine aminotransferase (ALT) was commonly reported in clinical trials in patients exposed to SARM (mean 7.1% across trials). Two individuals exposed to GSK2881078 in a clinical trial were reported to have rhabdomyolysis. Recreational SARM use should be strongly discouraged, and the risks of DILI, rhabdomyolysis, and tendon rupture should be emphasized. However, despite warnings, if a patient refuses to discontinue SARM use, ALT monitoring or dose reduction may improve early detection and prevention of DILI.

Mycobacterium haemophilum is a nontuberculous mycobacterium with fastidious in vitro growth requirements and an increasingly reported cause of extrapulmonary disease. Timely diagnosis and management of M. haemophilum infections and the immune reconstitution inflammatory syndromes (IRIS) observed in a subset of patients during treatment remain challenging. We conducted a retrospective chart review between January 1, 2010, and January 1, 2022 and identified 26 patients diagnosed with M. haemophilum infection at our institution. We describe their clinical presentation, diagnostic results, management, and outcomes. The majority of patients in our cohort had upper and/or lower extremity skin involvement, were immunosuppressed, and had generally favourable treatment outcomes. All tested M. haemophilum isolates were susceptible in vitro to clarithromycin and trimethoprim-sulfamethoxazole. Moreover, high rates of susceptibility were noted for ciprofloxacin (95%), linezolid (90%), and rifampin (85%). IRIS was identified in 31% of cases and should be considered in patients who develop worsening skin lesions or systemic symptoms following the initiation of effective antimicrobial therapy. Visualisation of acid-fast bacilli on initial tissue stains, a positive mycobacterial blood culture, and rapid de-escalation of tumour necrosis factor-α inhibitors and/or corticosteroids were more frequently encountered among patients in our cohort who developed IRIS. M. haemophilum infection should be considered among patients receiving immunomodulatory therapy who develop discoloured or nodular skin lesions involving the extremities, worsening focal arthritis, tenosynovitis, or isolated adenopathy. A heightened awareness of this pathogen’s clinical and laboratory characteristics can lead to a timely diagnosis and favourable outcome.

PURPOSE: Interpersonal continuity has been shown to play an essential role in primary care’s salutary effects. Amid 2 decades of rapid evolution in the health care payment model, we sought to summarize the range of peer-reviewed literature relating continuity to health care costs and use, information critical to assessing the need for continuity measurement in value-based payment design. METHODS: After comprehensively reviewing prior continuity literature, we used a combination of established medical subject headings (MeSH) and key words to search PubMed, Embase, and Scopus for articles published between 2002 and 2022 on “continuity of care” and “continuity of patient care,” and payor-relevant outcomes, including cost of care, health care costs, cost of health care, total cost of care, utilization, ambulatory care–sensitive conditions, and hospitalizations for these conditions. We limited our search to primary care key words, MeSH terms, and other controlled vocabulary, including primary care, primary health care, family medicine, family practice, pediatrics, and internal medicine. RESULTS: Our search yielded 83 articles describing studies that were published between 2002 and 2022. Of these, 18 studies having a total of 18 unique outcomes examined the association between continuity and health care costs, and 79 studies having a total of 142 unique outcomes assessed the association between continuity and health care use. Interpersonal continuity was associated with significantly lower costs or more favorable use for 109 of the 160 outcomes. CONCLUSIONS: Interpersonal continuity today remains significantly associated with lower health care costs and more appropriate use. Further research is needed to disaggregate these associations at the clinician, team, practice, and system levels, but continuity assessment is clearly important to designing value-based payment for primary care.

In the present study we determined the effect of endothelial microvesicles (EMVs) isolated from obese/hypertensive adults on mediators of cardiomyocyte hypertrophy [cardiac troponin T (cTnT), α-actinin, nuclear factor-kB (NF-kB)] and fibrosis [transforming growth factor (TGF-β), collagen1-α1] as well as endothelial nitric oxide synthase (eNOS) expression and NO production. EMVs from obese/hypertensive induced significantly higher expression of hypertrophic (cTnT, α-actinin, NF-kB) and fibrotic (TGF-β) proteins as well as significantly lower eNOS activation and NO production in cardiomyocytes than EMVs from normal weight/normotensive adults. EMVs are a potential mediating factor in the increased risk of cardiomyopathy and heart failure with obesity/hypertension.

Switch-like motifs are among the basic building blocks of biochemical networks. A common motif that can serve as an ultrasensitive switch consists of two enzymes acting antagonistically on a substrate, one making and the other removing a covalent modification. To work as a switch, such covalent modification cycles must be held out of thermodynamic equilibrium by continuous expenditure of energy. Here, we exploit the linear framework for timescale separation to establish tight bounds on the performance of any covalent-modification switch, in terms of the chemical potential difference driving the cycle. The bounds apply to arbitrary enzyme mechanisms, not just Michaelis-Menten, with arbitrary rate constants, and thereby reflect fundamental physical constraints on covalent switching., 29 pages, 6 figures

Background: Perioperative atrial fibrillation (AF) and myocardial injury after noncardiac surgery (MINS) are common complications after noncardiac surgery. Inflammation has been implicated in the pathogenesis of both disorders. The COP-AF trial tests the hypothesis that colchicine reduces the incidence of perioperative AF and MINS in patients undergoing major noncardiac thoracic surgery. Design: The 'COlchicine for the Prevention of Perioperative Atrial Fibrillation' (COP-AF) trial is an international, blinded, randomized trial that compares colchicine to placebo in patients aged at least 55 years and undergoing major noncardiac thoracic surgery with general anesthesia. Exclusion criteria include a history of AF and a contraindication to colchicine (e.g., severe renal dysfunction). Oral colchicine at a dose of 0.5 mg or matching placebo is given within 4 hours before surgery. Thereafter, patients receive colchicine 0.5 mg or placebo twice daily for a total of 10 days. The two independent co-primary outcomes are clinically important perioperative AF (including atrial flutter) and MINS during 14 days of follow-up. The main safety outcomes are sepsis or infection and non-infectious diarrhea. We aim to enroll 3,200 patients from approximately 40 sites across 11 countries to have at least 80% power for the independent evaluation of the two co-primary outcomes. Summary: COP-AF is a large randomized and blinded trial designed to determine whether colchicine reduces the risk of perioperative AF or MINS in patients who have major noncardiac thoracic surgery. ExteNET showed that neratinib, an irreversible pan-HER tyrosine ki- nase inhibitor, given for 1 year after trastuzumab-based therapy significantly improved inva- sive disease-free survival in women with early-stage HER2-positive breast cancer. We report the final analysis of overall survival in ExteNET.

SUMMARY Seismic tomography models reveal differences in the geographic distribution and magnitude of P- and S-wave velocity variations (VP and VS, respectively) below ∼2200 km depth in the Earth’s mantle. In particular, large low shear velocity provinces (LLSVPs) beneath the Pacific and Africa exhibit a distinct low velocity population in the distribution of VS that does not stand out in VP models, carrying important implications for the origin of these features. However, it is possible that the absence of a distinct low velocity feature in VP models is an artefact of VP models having lower resolution compared to VS models owing to differences in coverage. Here, we use ‘tomographic filters’ computed from the singular value decomposition of the sensitivity matrices for a pair of VP and VS models in order to test whether such low velocity features are suppressed in VP models. Our ‘cross-filtered’ results show that resolution alone cannot explain the absence of a corresponding low VP population. We additionally apply the joint VP and VS tomographic filter technique to thermochemical mantle convection models to show that cases with distinct phase and/or composition may be differentiated from cases where only temperature varies. We then develop a new proxy for exploring uncorrelated VP and VS more broadly using the difference between the observed VP model and the filtered VS model input. Our results show that ‘large uncorrelated modulus provinces’ (LUMPs) extend beyond the boundaries of LLSVPs, and exhibit anomalies in both fast and slow regions.

This article presents a dataset comprising Scottish agricultural prices for the period 1826–1974. The data are derived from an archived source in which prices set at meetings of courts held across each of Scotland’s 32 historic counties were recorded. The courts, convened by county sheriffs, met once a year to gather evidence relating to county-wide prices for crops grown within each locality. Once set, the prices were used, primarily, to determine the annual stipends of the clergy of the established Church of Scotland. Prices varied according to crop variety and quality, with some counties setting up to three prices in any one year for one type of grain. The most widely grown crop was oatmeal, with wheat, peas and barley also extensively farmed. The system operated without interruption until the abolition of the ‘fiars’ courts in 1974. The dataset opens two key areas of research. First it enables the further development of analyses of the integration and operation of Scottish regional grain markets in the nineteenth and early twentieth centuries. Second it facilitates empirical analyses of the operation of the clerical labour market in Scotland for which long runs of stipend (wage) data collected on a consistent basis are required.

The Medical Library Association (MLA) has defined 7 domain hubs aligning to different areas of information professional practice. To assess the extent to which content in the Journal of the Medical Library Association (JMLA) is reflective of these domains, we analyzed the magnitude of JMLA articles aligning to each domain hub over the last 10 years. Bibliographic records for 453 articles published in JMLA from 2010 to 2019 were downloaded from Web of Science and screened using Covidence software. Thirteen articles were excluded during the title and abstract review because they failed to meet the inclusion criteria, resulting in articles included in this review. The title and abstract of each article were screened by two reviewers, each of whom assigned the article up to two tags corresponding to MLA domain hubs (i.e., information services, information management, education, professionalism and leadership, innovation and research practice, clinical support, and health equity & global health). These results inform the MLA community about our strengths in health information professional practice as reflected by articles published in JMLA.

Aims Patients with coronary artery disease (CAD) and patients with peripheral artery disease (PAD) are at risk for major adverse cardiovascular events (MACE) and major adverse limb events (MALE). There are limited data regarding dietary patterns and the risk of recurrent MACE and MALE in CAD and PAD patients. We aimed to identify dietary patterns associated with MACE and MALE in patients with CAD and/or PAD. Methods and results We analysed data collected from patients enrolled into the Cardiovascular Outcomes for People Using Anticoagulation Strategies (COMPASS) trial, in which diet was assessed by a short food frequency questionnaire (FFQ) at baseline. Two dietary pattern scores, the modified Alternate Healthy Eating Index (mAHEI) and Mediterranean Diet Score (mMDS), were calculated. We tested the association between mAHEI and mMDS and the incidence of MACE and/or MALE. The mean mAHEI score was 23.0 ± 7.7 (out of 70) overall and was similar comparing CAD and PAD patients. The incidence of MACE or MALE was 6.3% in the lowest diet quality quartile (as assessed by mAHEI) compared with 4.2% in the highest quartile over 30 months. In the fully adjusted model, the hazard ratio of a low diet quality (Quartile 1) compared with the highest (Quartile 4) for MACE or MALE was 1.27 (95% CI: 1.08–1.49; P = 0.004, Q1 vs. Q4). This excess hazard was primarily driven by higher MACE in both the CAD and PAD cohorts. Conclusions Poor diet quality as assessed by the mAHEI is independently associated with a higher risk of recurrent MACE and MALE in patients with chronic CAD and/or PAD.

Multiprincipal-element alloys are an enabling class of materials owing to their impressive mechanical and oxidation-resistant properties, especially in extreme environments1,2. Here we develop a new oxide-dispersion-strengthened NiCoCr-based alloy using a model-driven alloy design approach and laser-based additive manufacturing. This oxide-dispersion-strengthened alloy, called GRX-810, uses laser powder bed fusion to disperse nanoscale Y2O3 particles throughout the microstructure without the use of resource-intensive processing steps such as mechanical or in situ alloying3,4. We show the successful incorporation and dispersion of nanoscale oxides throughout the GRX-810 build volume via high-resolution characterization of its microstructure. The mechanical results of GRX-810 show a twofold improvement in strength, over 1,000-fold better creep performance and twofold improvement in oxidation resistance compared with the traditional polycrystalline wrought Ni-based alloys used extensively in additive manufacturing at 1,093 °C5,6. The success of this alloy highlights how model-driven alloy designs can provide superior compositions using far fewer resources compared with the ‘trial-and-error’ methods of the past. These results showcase how future alloy development that leverages dispersion strengthening combined with additive manufacturing processing can accelerate the discovery of revolutionary materials.

Context Peatlands in the Australian Alps are important ecologically and recognised in national environmental legislation. Increasing fire frequency over the past 40 years has put the community at greater risk of degradation. Aims Using published studies of fire effects in peatlands and new data, we investigate general responses of peatlands to fire so that risk can be evaluated and appropriate management adopted. Methods We present four case studies that employ repeated measures of floristic composition or chronosequences to assess fire-related changes. Key results Cover of frequently-occurring species declined after fire but most had returned to pre-fire cover after 10 years. Recovery of the obligate seeder shrub Dracophyllum continentis (B.L.Burtt) S.Venter and the dominant moss Sphagnum cristatum Hampe was much slower, but variable for the latter, apparently depending on fire intensity and post-fire moisture availability; both species favoured less frequently burnt peatlands with high soil moisture. Some herbs (including non-native species) benefitted from fire, briefly becoming abundant soon afterwards. Overall species richness and diversity were unaffected by fire. Conclusions Peatlands in the Australian Alps tend to be resilient to single fires with effects on species composition being short-lived. However, species cover (especially Sphagnum cristatum) requires perhaps 20 years between fires for full recovery. Fire can cause localised community extinction and this is likely to be exacerbated by other pressures such as trampling and drought. Implications Fire will be difficult to manage in peatlands but resilience might be built by removing other pressures such as trampling by feral animals.

Aims The prognosis of patients with atrial fibrillation (AF) and ischemic stroke while taking oral anticoagulation is poorly understood. This study aimed to characterize the outcomes of patients following a stroke event while on oral anticoagulation. Methods and results Individual participant data from five pivotal randomized trials of antithrombotic therapy in AF were used to assess the outcomes of patients with a post-randomization ischemic stroke while on study medication (warfarin, standard-, or lower-dose direct oral anticoagulant regimen) during trial follow-up. The primary outcome was recurrent ischemic stroke after the first post-randomization ischemic stroke. The primary analysis included 1163 patients with a first post-randomization ischemic stroke while on study medication (median age 73 years, 39.3% female, 35.4% history of stroke before trial enrollment). During a median continued follow-up of 337 days, 74 patients had a recurrent ischemic stroke [cumulative incidence at 1 year: 7.0%, 95% confidence interval (CI) 5.2%–8.7%]. The cumulative incidence of mortality at 3 months after stroke was 12.4% (95% CI 10.5%–14.4%). Consistent results for the incidence of recurrent ischemic stroke at 1 year were obtained in an analysis accounting for the competing risk of death (6.2%, 95% CI 4.8%–7.9%) and in a landmark analysis excluding the first 2 weeks after the index stroke and only including patients without permanent study drug discontinuation since then (6.8%, 95% CI 4.6%–8.9%). Conclusion Patients with AF and ischemic stroke while on oral anticoagulation are at increased risk of recurrent ischemic stroke and death. These patients currently have an unmet medical need.

In this article, we report on the synthesis of chiral bimesogens that possess silyloxy central linking groups. On first examination of the mesomorphic properties of the materials by X-ray diffraction and electrical field studies we came to the conclusion that the materials exhibit ferroelectric and antiferroelectric smectic C* phase behaviour. However, detailed investigations of the bimesogens indicated that the phases were not conventional smectics, but in addition strange phases that exhibited natural helical filaments on cooling from the isotropic liquid were formed. Indications are that these phases are possibly related to twist-bend structures or in-plane, modulated bent-core phases.

Nyström and Hammarström (2022) found 7 segments in the bio-active SARS-CoV-2 spike protein that can produce abnormal proteinaceous (fibrinaloid) clots according to the Waltz algorithm. In vitro results confirmed the Waltz predictions. If the spike coding sequence was captured in the BNT162b2, Moderna, and other injectables, as claimed by the manufacturers, the clot producing segments are present in them too. Mainstream medical publications claim that SARS-CoV-2 infection can cause abnormal clotting, especially in “long COVID”. Telling evidence from Medicare data shows a decreasing life expectancy with each dose of COVID-19 “vaccine” — 1 dose is worse than 0, and 2 worse than 1, etc. In Connecticut, 26,091 Medicare participants who died before December 31, 2022, but never took a COVID injection, on the average, survived 428 days after the middle of the pandemic period (July 27, 2020). By then nearly all of them must have been exposed to and/or infected by some SARS-CoV-2 variant — hence, the CDC urging to take the “vaccines”. By contrast, 108,156 Medicare patients across the US who died before January 1, 2023, after just 1 dose of COVID-19 “vaccine”, survived only 308 days — a loss of 119.9 days on the average. Connecticut participants, 23,248 of them, who received 2 to 5 doses, on the average, lost an additional 62 days of life-expectancy with each booster. It follows that 5 boosters times 62 days reduces the average remaining 308 days left-to-live after dose 1 by 310 days. So, nearly all the Medicare participants will have been dead for 2 days by booster 4 (dose 5). The upshot is that 5 doses, on the average, will kill all the Medicare participants who accept the advice of the CDC.[1] For 157,495 of the 65 and older Medicare population studied here — people supposedly most apt to benefit from COVID-19 injectables — days-left-to-live shrinks by 74 days, on the average, with each dose.
