Abstract: Background: Current management practices and outcomes in weaning from invasive mechanical ventilation are poorly understood. We aimed to describe the epidemiology, management, timings, risk for failure, and outcomes of weaning in patients requiring at least 2 days of invasive mechanical ventilation. Methods: WEAN SAFE was an international, multicentre, prospective, observational cohort study done in 481 intensive care units in 50 countries. Eligible participants were older than 16 years, admitted to a participating intensive care unit, and receiving mechanical ventilation for 2 calendar days or longer. We defined weaning initiation as the first attempt to separate a patient from the ventilator, successful weaning as no reintubation or death within 7 days of extubation, and weaning eligibility criteria based on positive end-expiratory pressure, fractional concentration of oxygen in inspired air, and vasopressors. The primary outcome was the proportion of patients successfully weaned at 90 days. Key secondary outcomes included weaning duration, timing of weaning events, factors associated with weaning delay and weaning failure, and hospital outcomes. This study is registered with ClinicalTrials.gov, NCT03255109. Findings: Between Oct 4, 2017, and June 25, 2018, 10 232 patients were screened for eligibility, of whom 5869 were enrolled. 4523 (77·1%) patients underwent at least one separation attempt and 3817 (65·0%) patients were successfully weaned from ventilation at day 90. 237 (4·0%) patients were transferred before any separation attempt, 153 (2·6%) were transferred after at least one separation attempt and not successfully weaned, and 1662 (28·3%) died while invasively ventilated. The median time from fulfilling weaning eligibility criteria to first separation attempt was 1 day (IQR 0–4), and 1013 (22·4%) patients had a delay in initiating first separation of 5 or more days. Of the 4523 (77·1%) patients with separation attempts, 2927 (64·7%) had a short wean (≤1 day), 457 (10·1%) had intermediate weaning (2–6 days), 433 (9·6%) required prolonged weaning (≥7 days), and 706 (15·6%) had weaning failure. Higher sedation scores were independently associated with delayed initiation of weaning. Delayed initiation of weaning and higher sedation scores were independently associated with weaning failure. 1742 (31·8%) of 5479 patients died in the intensive care unit and 2095 (38·3%) of 5465 patients died in hospital. Interpretation: In critically ill patients receiving at least 2 days of invasive mechanical ventilation, only 65% were weaned at 90 days. A better understanding of factors that delay the weaning process, such as delays in weaning initiation or excessive sedation levels, might improve weaning success rates. Abstract: Background: Dietary prevention of cardiovascular risk factors is seldom implemented. Aim: We assessed the dietary changes made by subjects at high risk of cardiovascular disease (CVD). Design and setting: Cross-sectional, multicentre observational study (European Society of Cardiology-ESC EORP-EUROASPIRE V Primary Care) including 78 centres from 16 ESC countries. Methods: Participants aged 18-79 years, devoid of CVD but treated with antihypertensive and/or lipid-lowering and/or antidiabetic therapy were interviewed >6 months and = 60 years, 43.5% with obesity, 71.1% on antihypertensive, 29.2% on lipid-lowering and 31.5% on antidiabetic treatment). Among participants with obesity, 47.7% reported having received dietary advice to lose weight [range: 24.7% (Greece) to 71.8% (Lithuania)]. Among participants on antihypertensive drug therapy, 53.9% reported being on a blood pressure lowering diet [range: 5.6% (UK) to 90.4% (Greece)]; a reduction of salt intake in the last three years was reported by 71.4% [range: 12.5% (Sweden) to 89.7% (Egypt)]. Among participants on lipid-lowering therapy, 56.0% reported being on a lipid lowering diet [range: 7.1% (Sweden) to 90.3% (Egypt)]. Among participants with diabetes, 57.2% reported being on a diet [range: 21.6% (Romania) to 95.1% (Bosnia & Herzegovina)]; a reduction in sugar intake was reported by 80.8% [range: 56.5% (Sweden) to 96.7% (Russian Federation)]. Conclusions: In ESC countries, fewer than 60% of participants at high CVD risk report being on a specific diet, with wide differences between countries. (c) 2023 The Author(s). 5. Post-Traumatic Cognitions Inventory (PTCI): psychometric properties in clients with serious mental illness and co-occurring PTSD

Abstract: Background: People with post-traumatic stress disorder (PTSD) exhibit negative cognitions, predictive of PTSD severity. The Post-Traumatic Cognitions Inventory (PTCI) is a widely used instrument measuring trauma-related cognitions and beliefs with three subscales: negative thoughts of self (SELF), negative cognitions about the world (WORLD), and self-blame (BLAME). Aims: The current study attempted to validate the use of the PTCI in people with serious mental illness (SMI), who have greater exposure to trauma and elevated rates of PTSD, using confirmatory factor analysis (CFA) and examining convergent and divergent correlations with relevant constructs. Method: Participants were 432 individuals with SMI and co-occurring PTSD diagnosis based on the Clinician Administered PTSD Scale, who completed PTCI and other clinical ratings. Results: CFAs provided adequate support for Foa’s three-factor model (SELF, WORLD, BLAME), and adequate support for Sexton’s four-factor model that also included a COPE subscale. Both models achieved measurement invariance at configural, metric and scalar levels for three diagnostic groups: schizophrenia, bipolar and major depression, as well as for ethnicity (White vs Black), and gender (male vs female). Validity of both models was supported by significant correlations between PTCI subscales, and self-reported and clinician assessed PTSD symptoms and associated symptoms. Conclusions: Findings provide support for the psychometric properties of the PTCI and the conceptualization of Sexton’s four-factor and Foa’s three-factor models of PTCI among individuals diagnosed with SMI (Foa et al., 1999).

8. Acute health effects of Heated Tobacco Products - comparative analysis with traditional cigarettes and electronic cigarettes in young adults

Abstract: BackgroundHeated Tobacco Products (HTPs) were designed to deliver nicotine by heating the tobacco instead of burning it.AimThe study aimed to examine the acute health effects on the respiratory and cardiovascular systems during the use of HTPs and compare these effects with acute health effects evoked by cigarette smoking or electronic cigarettes.MethodsThe study group comprised 160 healthy young adults (age 23 [Q1 21; Q3 26] years), both sexes divided into groups according to their smoking status: 40 HTPs users (H-group), 40 traditional cigarette smokers (T-group), 40 e-cigarette users (E-group), 40 non-smokers (C-group). The measurements of heart rate, blood pressure, O2 saturation, the concentration of FeNO, CO, temperature of exhaled air, and spirometry were performed thrice: initially, immediately after the exposure, and after 30 min. The exposure differed depending on smoking status: heating HTP, smoking a cigarette, using an e-cigarette, and simulation of smoking.ResultsAfter 5 min of exposure, a significant decrease in FeNO was observed in H and E groups, from 12.8±5.5ppb to 11.2±5.3ppb and from 16.9±6.5 to 14.2±6.8, pConclusionsThe use of HTPs elicits acute respiratory and cardiovascular health effects.

Abstract: The identification of mediators for physiologic processes, correlation of molecular processes, or even pathophysiological processes within a single organ such as the kidney or heart has been extensively studied to answer specific research questions using organ-centered approaches in the past 50 years. However, it has become evident that these approaches do not adequately complement each other and display a distorted single-disease progression, lacking holistic multilevel/multidimensional correlations. Holistic approaches have become increasingly significant in understanding and uncovering high dimensional interactions and molecular overlaps between different organ systems in the pathophysiology of multimorbid and systemic diseases like cardiorenal syndrome because of pathological heart-kidney crosstalk. Holistic approaches to unraveling multimorbid diseases are based on the integration, merging, and correlation of extensive, heterogeneous, and multidimensional data from different data sources, both -omics and nonomics databases. These approaches aimed at generating viable and translatable disease models using mathematical, statistical, and computational tools, thereby creating first computational ecosystems. As part of these computational ecosystems, systems medicine solutions focus on the analysis of -omics data in single-organ diseases. However, the data–scientific requirements to address the complexity of multimodality and multimorbidity reach far beyond what is currently available and require multiphased and cross-sectional approaches. These approaches break down complexity into small and comprehensible challenges. Such holistic computational ecosystems encompass data, methods, processes, and interdisciplinary knowledge to manage the complexity of multiorgan crosstalk. Therefore, this review summarizes the current knowledge of kidney-heart crosstalk, along with methods and opportunities that arise from the novel application of computational ecosystems providing a holistic analysis on the example of kidney-heart crosstalk.

Abstract: Oxidative stress plays an important role in the development of vitiligo. The aim of our study was to assess the effect of oral vitamin supplementation with antioxidant activity on the effectiveness of the vitiligo treatment. The study group consisted of 46 patients suffering from nonsegmental vitiligo for over a year. Before and after the therapy, the clinical advancement of the disease (VASI) and impairment of quality of life (DLQI) were assessed in all patients, and the activities of selected antioxidant enzymes (SOD, CAT, and GPx), lipid peroxidation products (MDA), and vitamins A and E were determined. Each patient was randomly assigned to one of three therapeutic groups: FOTO (UVB therapy at 311 nm, 3x a week for 4 months), WIT (oral antioxidant supplementation-vitamin A + E at a dose of 5000 IU of retinol and 400 mg of tocopherol), and FOTO + WIT (combination therapy). After the treatment, an increase in all antioxidant enzymes and a decrease in the concentration of malondialdehyde in patients were noted, regardless of the therapeutic method used. The greatest improvement in the repigmentation of skin lesions was achieved in patients treated with combined therapy (VASI −6.95 ± 4.69, p < 0.01 , DLQI −1.90 ± 2.77, p = 0.011 ). The burden of patients with risk factors for oxidative stress turned out to be associated with a greater severity of the disease process, and a longer period of disease was positively correlated with a reduction in the quality of life in patients.

15. Unterstützung für Kampagnen zum strategischen Wählen: Evidenz aus einem Umfrageexperiment zur Bundestagswahl 2021

Abstract: Although strategic voting is common in the first tier of the German electoral system, large-scale campaigns designed to foster strategic voting have rarely been observed in Germany. This changed in the General Election of 2021 when the left-wing organization Campact ran campaigns in several electoral districts supporting the most promising left-wing candidate in order to impede the election of a more right-wing candidate. Especially, Campact’s campaign in favor of the SPD candidate Frank Ullrich, with the goal of impeding the election of the controversial CDU candidate Hans-Georg Maaßen, received much public attention. Drawing on this case as an example, I address the question of how Green voters evaluate such campaigns. To do so, I conducted a survey experiment in which different information about the campaign of Campact is provided to Green voters. I find that providing more information about the German electoral system does not increase support for the campaign. However, Green voters react to a party cue, i.e., they become more favorable towards the campaign when they know that the Green Party supported the campaign.

17. Experimental research on the thermal properties of innovative insulation boards made of polyurethane-polyisocyanurate (PUR/PIR)

Abstract: In this work, the results of investigations of polyurethane materials were presented. Innovative materials based on polyurethane-polyisocyanurate (PUR/PIR) foam were obtained. Different types of additives (flame retardants, aerogels – additives that decrease thermal conductivity) are used in the composition of PUR/PIR foam. Foams are a type of composite composed of two phases: continuous (polyurethane polymers) and dispersed (composed of gases). All samples have been tested for thermal parameters: thermal conductivity, specific heat, and thermal diffusivity. Then they have been compared with each other and with a reference sample (RS) without additives. Based on the research, it was shown that innovative insulation materials were characterized by thermal conductivity λ in the range of 0.0254–0.0294 W/(m · K). The thermal properties of foams depending on the type and chemical composition of the material. Depending on the used substrates, their molar ratio, type, synthesis conditions, modifying agents and catalysts, a different polyurethane material is obtained.

In the Georges-François Leclerc Cancer Center (Dijon, France), we suspended the use of textured Biocell© implants from November 2018 onwards, in favour of smooth surface, round implants. Objective: To assess the impact of textured implant withdrawal on surgical practices in breast reconstruction (BR) in a single French cancer center. Methods: This is a retrospective, single-centre study in the Georges-François Leclerc Cancer Center (Dijon, France). Surgical practices of BR and complications were compared between 2 periods: before the withdrawal of textured implants from the market (Period 1: September 2017-November 2017) and after (Period 2: December 2018-March 2020). Patients were included if they were operated on for the first time for immediate or secondary BR in either period. Univariate analysis was performed to compare before vs after groups, and to analyse surgical practices (rates of autologous techniques and breast implant) of BR and complications ((defined as any complication grade IIIB or higher according to the Clavien Dindo classification) between the 2 periods. Results: Data from 168 and 159 BR were collected from Periods 1 and 2 respectively. Characteristics were comparable between groups.The rate of BR with implants was significantly lower in period 2 compared with period 1 (23.9% vs 53.6%; p< 0.001). Similarly, autologous BR increased significantly in period 2 for muscle sparing latissimus dorsi (2.4% vs 12.6%; p< 0.001) and exclusive fat grafting (9.5% vs 21.5%; p< 0.01). Others autologous techniques was comparable between the two periods (DIEP and latissimus dorsi). The number of complications decreased in period 2 (10% vs 18.4% in period 1, p=0.04). Conclusion: After the withdrawal of textured implants, we observed a significant change in surgical practices for BR in our center. The rate of reconstruction with implants decreased in favour of autologous techniques. This change in practice was accompanied by a decrease in complications. 20. Abstract P1-09-11: Impact of breast surgical procedure on survival in BRCA mutated patients with invasive breast cancer: mastectomy versus conservative treatment

Abstract: Introduction: Patients with BRCA1/2 mutations have a higher risk of developing breast cancer compared to the wild-type population. For patients with a BRCA mutation, there are no specific recommendations for surgical management. The aim of this study was therefore to retrospectively investigate overall survival (OS) and recurrence-free survival (RFS) of BRCA mutated patients with localized invasive breast cancer, by comparing conservative surgery versus mastectomy. Methods: This study was based on data from the Côte d’Or breast and gynecological cancer registry. Data from patients with a constitutional BRCA1/2 mutation who presented with invasive breast cancer were collected retrospectively from 1998 to 2018. The Kaplan-Meier method was used to describe RFS and OS. Results:A total of 104 patients were included in the analysis, of whom 69 had conservative surgery and 35 underwent mastectomy. Regarding survival, there was no significant difference in OS (HR =1.49; 95% confidence interval (CI) [0.76-2.93], p=0.25). Similarly, there was no significant difference in RFS (HR =1.40; 95% CI [0.81-2.40], p=0.22), survival without homolateral recurrence (HR =0.88; 95% CI [0.30-2.61], p=0.89), without contralateral recurrence (HR =1.50; 95% CI [0.55-4.09], p=0.42), or without distant metastatic recurrence (HR =1.42, 95% CI [0.69-2.90], p=0.33). Conclusion: In invasive breast cancer in a patient with a germline BRCA1/2 mutation, conservative surgery, when possible, appears to be a feasible option over total mastectomy, with no difference in overall survival. However, the patient should be informed of the aggressive nature of recurrence in this population requiring chemotherapy in most cases. Abstract: Confronting stigma early in life could enhance treatment seeking. In two randomized controlled trials (RCTs), one focused on psychosis and the other on adolescent depression, the efficacy and equivalence of brief social contact-based videos were evaluated and compared with a control condition. The outcomes of interest were changes in illness-related stigma and treatment-seeking intention. The hypotheses were that the intervention videos would show greater efficacy than control conditions and that traditional and selfie videos would demonstrate similar efficacy.Young adults (study 1, N=895) and adolescents (study 2, N=637) were randomly assigned to view intervention videos (in traditional or selfie styles) or to a control condition. In short videos (58-102 seconds), young presenters humanized their illness by emotionally describing their struggles and discussing themes of recovery and hope.Repeated-measures analyses of variance and paired t tests showed significant differences in stigma and treatment seeking between the intervention and control groups and similar efficacy of the traditional and selfie videos. Cohen's d effect sizes ranged from 0.31 to 0.76 for changes in stigma from baseline to 30-day follow-up in study 1 and from 0.13 to 0.47 for changes from baseline to postintervention in study 2.The RCTs demonstrated the efficacy of brief videos, both traditional and selfie, in reducing illness-related stigma among young adults and adolescents and in increasing treatment-seeking intention among adolescents. Future studies should explore the effects of brief videos presented by social media influencers on mental health stigma and treatment engagement.

Abstract: The relation between digitalization and environmental sustainability is ambiguous. There is potential of various digital technologies to slow down the transgression of planetary boundaries. Yet resource and energy demand for digital hardware production and use of data-intensive applications is of substantial size. The world over, there is no comprehensive regulation that addresses opportunities and risks of digital technology for sustainability. In this perspective article, we call for a Digital Green Deal that includes strong, cross-sectoral green digitalization policies on all levels of governance. Abstract: This paper uses accounting concepts—particularly the concept of Return on Investment (ROI)—to reveal the quantitative value of scientific research pertaining to a major US cyberinfrastructure project (XSEDE—the eXtreme Science and Engineering Discovery Environment). XSEDE provides operational and support services for advanced information technology systems, cloud systems, and supercomputers supporting non-classified US research, with an average budget for XSEDE of US$20M+ per year over the period studied (2014–2021). To assess the financial effectiveness of these services, we calculated a proxy for ROI, and converted quantitative measures of XSEDE service delivery into financial values using costs for service from the US marketplace. We calculated two estimates of ROI: a Conservative Estimate, functioning as a lower bound and using publicly available data for a lower valuation of XSEDE services; and a Best Available Estimate, functioning as a more accurate estimate, but using some unpublished valuation data. Using the largest dataset assembled for analysis of ROI for a cyberinfrastructure project, we found a Conservative Estimate of ROI of 1.87, and a Best Available Estimate of ROI of 3.24. Through accounting methods, we show that XSEDE services offer excellent value to the US government, that the services offered uniquely by XSEDE (that is, not otherwise available for purchase) were the most valuable to the facilitation of US research activities, and that accounting-based concepts hold great value for understanding the mechanisms of scientific research generally.

The aim of the research (N=238) is to check whether religiosity affects the perception of the importance of religious principles in everyday life and religion in political life. In the presented approach, religiosity was a multidimensional variable and was conceptualized using a modified five-point Religiosity Centrality Scale (Huber 2003). It was confirmed that all dimensions of religiosity are significantly related to the assessment of the importance of religious principles. The dimension that showed the strongest correlation with the created variables was the dimension of religious cult, and the dimension that correlated the least was the dimension of reading the Catholic press.

Abstract: Thermal insulation may be influenced by the size of clothing and thus the volume of air gaps. The aim of this study was to determine the relationship between the size of outer wear clothing, and thus the indirect fit (the volume and size of air gaps), and thermal insulation in static and dynamic conditions. A set of underwear and two types of outerwear for workers of the energy sector and the chemical industry were selected for the study. Results showed that the value of thermal insulation (regardless of the type of outerwear) first increased with increasing clothing size.

Abstract: BACKGROUND: Given the physical and economic burden of complications in spine surgery, reducing the prevalence of perioperative adverse events is a primary concern of both patients and health care professionals. This study aims to identify specific perioperative factors predictive of developing varying grades of postoperative complications in adult spinal deformity (ASD) patients, as assessed by the Clavien-Dindo complication classification (Cc) system. METHODS: Surgical ASD patients ≥18 years were identified in the American College of Surgeons’ National Surgical Quality Improvement Program from 2005 to 2015. Postoperative complications were stratified by Cc grade severity: minor (I, II, and III) and severe (IV and V). Stepwise regression models generated dataset-specific predictive models for Cc groups. Model internal validation was achieved by bootstrapping and calculating the area under the curve (AUC) of the model. Significance was set at P < 0.05. RESULTS: Included were 3936 patients (59 ± 16 years, 63% women, 29 ± 7 kg/m(2)) undergoing surgery for ASD (4.4 ± 4.7 levels, 71% posterior approach, 11% anterior, and 18% combined). Overall, 1% of cases were revisions, 39% of procedures involved decompression, 27% osteotomy, and 15% iliac fixation. Additionally, 66% of patients experienced at least 1 complication, 0% of which were Cc grade I, 51% II, 5% III, 43% IV, and 1% V. The final model predicting severe Cc (IV–V) complications yielded an AUC of 75.6% and included male sex, diabetes, increased operative time, central nervous system tumor, osteotomy, cigarette pack-years, anterior decompression, and anterior lumbar interbody fusion. Final models predicting specific Cc grades were created. CONCLUSIONS: Specific predictors of adverse events following ASD-corrective surgery varied for complications of different severities. Multivariate modeling showed smoking rate, osteotomy, diabetes, anterior lumbar interbody fusion, and higher operative time, among other factors, as predictive of severe complications, as classified by the Clavien-Dindo Cc system. These factors can help in the identification of high-risk patients and, consequently, improve preoperative patient counseling. CLINICAL RELEVANCE: The findings of this study provide a foundation for identifying ASD patients at high risk of postoperative complications . LEVEL OF EVIDENCE: 4.

36. Relationships between Changes in Hematological Adaptations and Exercise Capacity in Olympic Rowers after a Period of Reduced Training Loads

Abstract: Endurance performance is positively associated with hematological adaptations; therefore, high total hemoglobin mass and intravascular volumes are commonly observed in high-level endurance athletes. However, it is still unclear whether the fluctuations in exercise capacity that typically occur in endurance athletes during the annual training cycle are directly associated with changes in hematological adaptations, which appear to be relatively stable during this time. To better understand this issue, a study was conducted with 10 Olympic rowers, who followed the same training program. Athletes underwent laboratory testing in the competitive and the general preparation phase of an annual training cycle (a 34% reduction in training volume). This included a graded exercise test on a rowing ergometer (GXT) and blood measurements of hemoglobin concentration (Hb), total hemoglobin mass (tHb-mass), plasma volume (PV), and blood volume (BV). Decreases in maximal values of power relative to body mass (p = 0.028), lactate concentration (p = 0.005), and heart rate (p = 0.017) in the GXT were registered. At the same time, absolute (p = 0.017) and relative (p = 0.005) PV decreased. Changes in PV (rS = 0.842, p = 0.002) and BV (rS = 0.818, p = 0.004), but not in tHb-mass (rS = 0.588, p = 0.074) and Hb (rS = −0.188, p = 0.602), were significantly correlated with changes in maximal power in the GXT. Our results indicate a close relationship between changes in intravascular volumes and maximal exercise capacity after a period of reduced training loads in elite endurance athletes.

Abstract: Background: Cannabis use suppresses the endocannabinoid system in healthy individuals. However, the association between cannabis use with the endocannabinoid system is understudied in individuals with psychosis despite the high rate of cannabis use in these individuals. Methods: We enrolled 83 individuals who were admitted to an inpatient psychiatric unit with psychotic presentations, and measured their plasma levels of main endocannabinoids, Anandamide (AEA) and 2-Acylglycerol (2-AG), and endocannabinoid related compounds, Palmitoylethanolamine, and N-oleoylethanolamine. Cannabis use was assessed with urine toxicology and frequency of cannabis use was assessed using self-reported questionnaires. The Positive and Negative Syndrome Scale was used to assess the severity of psychotic symptoms. Results: Overall, we had 38 individuals in cannabis positive group (CN+) and 45 individuals in cannabis negative group (CN−). Compared to CN−, CN+ group had lower plasma levels of AEA, which remained significant after controlling for age, gender, race/ethnicity, and use of other drugs. Conclusion: Cannabis use is associated with low plasma AEA levels in individuals with psychosis, which is in the same line with reported suppressive effects of cannabis on the endocannabinoid system in healthy individuals. Further studies are needed to investigate the clinical significance of this finding.

42. Tied-array beam localization of radio transients and pulsars

Abstract: Multi-element interferometers such as MeerKAT, which observe with high time resolution and have a wide field of view, provide an ideal opportunity to perform real-time, untargeted transient and pulsar searches. However, because of data storage limitations, it is not always feasible to store the baseband data required to image the field of a discovered transient or pulsar. This limits the ability of surveys to effectively localize their discoveries and may restrict opportunities for follow-up science, especially of one-off events like some fast radio bursts. Here, we present a novel maximum-likelihood estimation approach to localizing transients and pulsars detected in multiple MeerKAT tied-array beams at once, which we call tied-array beam localization, as well as a Python implementation of the method named SeeKAT. We provide real-world examples of SeeKAT’s use as well as a Monte Carlo analysis to show that it is capable of localizing single pulses detected in beamformed MeerKAT data to (sub)arcsec precision.

43. Exploring the Sport–Alcohol Relationship: A Longitudinal Qualitative Study of Student-Athlete Drinking Following the Transition out of University

Abstract: The purpose of this study was to provide new knowledge about the temporal and contextual aspects of the alcohol–sport relationship. Eight U.K. student-athletes completed the Alcohol Use Disorders Identification Test in their final year at university, 18 months, and 30 months after graduation. They also completed semistructured interviews about their drinking motives, behaviors, and life circumstances. Results showed that participants reduced their alcohol consumption after leaving university, but despite the onset of some adult responsibilities, most were still drinking at hazardous levels. After university, drinking took place with old friends, new colleagues, and new sporting teammates. At all time points, social drinking motives were the most prevalent. Findings demonstrate a relationship between alcohol and sport that is cemented at university but continues beyond it. Targeted interventions to reduce the role of alcohol in the social experience of sport are needed to support long-term athlete health.

44. Multicenter study to assess surgical treatments of 452 sinonasal intestinal-type adenocarcinomas: A REFCOR study

Abstract: The objective was to assess the local oncological outcomes of endoscopic versus external surgical treatment of sinonasal intestinal-type adenocarcinomas (ITAC) and the factors of recurrence.a retrospective non-randomized case-control multicenter study was carried out, including 452 untreated sinonasal ITACs recruited from 10 tertiary referral centers. The tumors were re-classified according to the UICC 2017 (pT). Survival curves were obtained using the Kaplan-Meier method. Univariate analysis was done with the log-rank test. Multivariate analysis was performed with a Cox model adjusted for age, T stage, and radiotherapy. A binary logistic regression compared surgical complications and performed two supplementary analyses on positive margins.We compared 195 and 257 patients operated by the external and endoscopic approach, respectively. The mean follow-up was 59.2 ± 48.7 months. Post-operative margins were invaded in 30.6 versus 18.9% of patients, respectively (p = 0.007). The overall recurrence rate was 33.8 versus 24.6%, respectively (p = 0.034). There was a significant difference in favor of the endoscopic approach regarding local recurrence-free survival thanks to better surgical margins in univariate and multivariate analysis (Odd Ratio = 2.01 (1.2-3.36) p = 0.0087). The complication rate (Odds Ratio = 3.4 (1.79-6.32) p 0.001) was significantly lower in the endoscopic group. The histological positivity of signet-ring cells shows a statistically significant difference in recurrence-free survival (p = 0.0028).the oncological control of ITAC is better through the endoscopic approach, with negative margins and the absence of signet-ring-cells, two independent factors of recurrence.

45. Usefulness of determining some markers of myocardial damage in sports horses subjected to physical exertion

Abstract: The aim of the study was to evaluate the usefulness of determining markers of myocardial damage, such as: aspartate aminotransferase, total creatine kinase, cardiac creatine kinase isoenzyme, total lactate dehydrogenase, cardiac lactate dehydrogenase isoenzyme and peripheral blood lactic acid in sport horses subjected to physical exertion. The research was carried out on 50 horses of three breeds: Ukrainian Riding, Hanoverian and Westphalian, aged 4-16 years. In all horses, the activity of: aspartate aminotransferase, total creatine kinase, cardiac creatine kinase isoenzyme, total lactate dehydrogenase, cardiac isoenzyme lactate dehydrogenase and lactic acid concentration were marked before and after exercise. It was found that after physical exercise in the tested horses, there was an increase in markers of myocardial damage. On the basis of the conducted research, it was demonstrated that the highest increase concerned the concentration of lactic acid and the activity of the cardiac isoenzyme of creatine kinase. The conducted studies additionally indicate that the evaluation markers of myocardial damage in horses may be useful as a screening test in detecting cardiac muscle disorders associated with excessive exercise. However, it should be emphasized that it must be considered together with the results of a clinical examination and additional tests such as cardiac ultrasound and electrocardiography.

Abstract: Introduction: Urinalysis is one of the most frequently performed laboratory tests. An importantfeature of urinalysis standardization is unifying laboratory report forms to enable the analysisof the results obtained in different laboratories and/or at different times.Aim: The aim of the study was to analyze the urinalysis laboratory report forms in medicallaboratories and to compare them with the Polish Society of Laboratory Diagnostics (PTDL)urinalysis guidelines published in 2019 (Bil-Lula et al. Diagn Lab 2019; 55: 145198).Material and methods: In total, 52 urinalysis laboratory reports were analyzed. The majority(85%) included dipstick and urine particle analyses. The reports were obtained from 26laboratories from 8 voivodeships in Poland. The highest percentages of laboratories werefrom Pomeranian Voivodeship (35%) and from large cities with more than 100.000 inhabitants(58%).Results: The reports showed high variability in the names of the parameters, the referenceranges used, and the manner of expressing the patients results. For dipstick erythrocytes,eight different expressions were used including erythrocytes, blood, red blood cells, blood(free hemoglobin). For specific gravity, eleven different reference ranges were used, and formost parameters, a significant percentage of the laboratory report forms (up to 70% for colorand clarity) had no reference ranges. There was also no consistency in the presentation of thepatients results in relation to the reference ranges. In some reports, colloquial expressions,such as sugar, transparency, nonstandard abbreviations, such as Ery/Hb, poj, andtypographical errors were present.Conclusions: Urinalysis reports in laboratories are not standardized, thus, it is advisable toimplement corrective actions aimed at standardizing the presentation of data. The PTDLguidelines published in 2019 can help in this regard.
