Human cell lines (the T-cell lines H9, Jurkat, and HUT102, the myeloid lines U937 and HL60, and the Raji B cell line) were infected with HIV-1. HIV-1 antigen could be detected by immunofluorescence analysis in more than 50% of T cells and myeloid cells 15 days after infection. Infection of Raji cells took more than 2-3 months. Studies of cell surface marker expression revealed remarkable changes after HIV-1 infection of Raji cells: expression of CR2 (C3d/EBV receptor, CD19, CD20, CD22, CD23, CD10, and surface IgM) were highly reduced, in the case of CR2 and membrane-IgM from 100 to 0%, whereas levels of CD37 and CD38 remained unaltered by HIV-1 infection. U937 cells showed a reduction of CD4 expression from 14 to 5% after HIV-1 infection; the CR3 expression slightly increased from 25 to 30%. In contrast, HLA-DR was only expressed (21%) after HIV-1 infection but not in uninfected U937 cells. Expression of HLA-DR could be detected also in HL60 cells (33%) after HIV-1 infection. In H9 cells, CD4 was reduced from 60 to 30% after HIV-1 infection, whereas HLA-DR and CD25/IL-2 receptor expression increased from 16 to 90% and from 0 to 50%, respectively. CD4 was reduced from 70 to 0% from Jurkat cells after HIV-1 infection, whereas expression of CR2 was only slightly diminished from 8 to 4%. The sera of four patients with amoebic liver abscess and two patients with diarrhea caused by Entamoeba histolytica were tested on "Western blots" prepared from E. histolytica strains A3, SFL-3 and HK-9 separated by SDS-PAGE. IgG antibodies in the sera from patients with liver abscess were found to react with several "major" (i.e. strongly staining) band of approximate molecular weights 150-250 kD, 100 kD, 80 kD, 60 kD, 35 kD, 30 kD, 20 kD, and 5-15 kD and the 150-250 kD group of bands. A comparison of the patterns of reactivity obtained with these sera and strains A3, SFL-3 and HK-9 would indicate that some of these bands, especially the 5-15 kD groups of bands might carry antigenic determinants specific for individual strains of amoebae. This 5-15 kD group of bands is completely pelleted at 30.000 g and could therefore be associated with the cytoplasmic membrane. The 150-250 kD group of bands consists of smaller subunits of 40-65 kD molecular weight linked by disulfide bridges; the respective proteins are sensitive to digestion with elastase but not with trypsin, and seem to carry antigenic determinants containing sugar residues as evidenced by their sensitivity to treatment with NaJO4.

This study followed 184 drug abusers. Examined in all of them were urinary neopterin levels, HBV, SGOT, and Luestest. Seventy-three percent of IV drug addicts showed elevated neopterin levels reflecting activated cellular immunity. Statistically, no correlation of neopterin levels with, eg, excessive alcohol consumption, duration of drug abuse, or studied laboratory parameters was found. Individuals using cocaine revealed higher neopterin levels than those not doing so. Twenty-one of twenty-two patients with no parenteral drug use had normal neopterin excretion. In 34 drug detoxification patients, we examined in addition: T-lymphocyte subsets (T4/T8 ratio) and serum neopterin levels. Thirty-eight of ninety-four parenteral drug addicts presented with anti-LAV/HTLV-III antibodies (ELISA + Western blot + IFT). Our data demonstrate an activated cellular immune status in parenteral drug addicts that cannot be attributed to LAV/HTLV-III infection in all cases. The development of AIDS seems to depend not only on the exposure to LAV/HTLV-III but also on activated cellular immunity, which is easily assessed by neopterin measurement.

The effect of D-penicillamine on the replication of the Human Immunodeficiency Virus (HIV) in H9 T-lymphoblastoid cells was evaluated. HIV-infected and uninfected H9 cells were incubated for a period of 6 weeks with various concentrations of D-penicillamine. During the first 8 days, D-penicillamine (at concentrations of 0-150 micrograms/ml) did not influence growth and viability of H9/H9-IIIB cells. However, long term cultures and higher doses of the drug caused a decay in cell number and viability of the cultured cells. Up to the 8th day of culture, D-penicillamine did not exert an inhibiting effect on the number of HIV-infected cells but did suppress HIV-antigen production by up to 29%. After 6 weeks, in addition to a decrease of the relative viral antigen concentration from 390-500 ng/ml in the medium control to 210-320 ng/ml in cultures containing 150 micrograms D-penicillamine/ml, D-penicillamine also reduced the percentage of HIV-positive cells. Thus our results demonstrate an antiviral effect of D-penicillamine in H9 cultures.

In a longitudinal study, we examined 183 parenteral drug addicts who were imprisoned in Innsbruck, Austria, from March 1985 to March 1987. Thirty-seven percent of the patients had anti-HIV-antibodies. A 2-year follow-up of 43 seropositive patients revealed some evidence that urinary neopterin levels are of predictive value. Neopterin is produced from human macrophages upon stimulation with interferon gamma and, therefore, represents a sensitive parameter of activated T-cells. Eight patients who developed AIDS-related complex and one of them who contracted AIDS had significantly higher neopterin levels during the previous follow-up than the other groups of patients. Our data support the view that T-cell activation might be a crucial event in the induction of HIV reproduction in vivo.

Urinary neopterin levels are raised with a high incidence in all risk groups for AIDS. Neopterin elevations reflect activated cellular immunity in risk group members, in some cases independently of LAV/HTLV-III infection. Moreover, we are able to show that in patients receiving multiple blood transfusions at least a transient challenge of cell-mediated immunity occurs, which is indicated in part by increasing neopterin levels. We conclude that neopterin levels are a reliable index for assessment of susceptibility for AIDS when infection with LAV/HTLV-III occurs. Activated status of cell-mediated immunity might predispose infected persons to an overwhelming infection and secondary spreading of LAV/HTLV-III, thus leading to the development of full-blown AIDS or ARC. As a consequence of these observations, T-cell-stimulatory actions and agents should intentionally be avoided. Treatment of AIDS patients with immunosuppressants should be examined. The success of therapeutic regimens should be monitored by measurement of neopterin levels.

Aims/hypothesis: Islet transplantation is a minimally invasive approach to curing Type I (insulin-dependent) diabetes mellitus. Success has recently been reported in patients receiving solitary islet transplants but the outcome in patients receiving islets together with, or after, kidney transplants has been limited and unpredictable.
