In the Surveillance Tool for Outcome-based Comparison of FREEdom from infection (STOC free) project (https://www.stocfree.eu), a data collection tool was constructed to facilitate standardised collection of input data, and a model was developed to allow a standardised and harmonised comparison of the outputs of different control programmes (CPs) for cattle diseases. The STOC free model can be used to evaluate the probability of freedom from infection for herds in CPs and to determine whether these CPs comply with the European Union's pre-defined output-based standards. Bovine viral diarrhoea virus (BVDV) was chosen as the case disease for this project because of the diversity in CPs in the six participating countries. Detailed BVDV CP and risk factor information was collected using the data collection tool. For inclusion of the data in the STOC free model, key aspects and default values were quantified. A Bayesian hidden Markov model was deemed appropriate, and a model was developed for BVDV CPs. The model was tested and validated using real BVDV CP data from partner countries, and corresponding computer code was made publicly available. The STOC free model focuses on herd-level data, although that animal-level data can be included after aggregation to herd level. The STOC free model is applicable to diseases that are endemic, given that it needs the presence of some infection to estimate parameters and enable convergence. In countries where infection-free status has been achieved, a scenario tree model could be a better suited tool. Further work is recommended to generalise the STOC free model to other diseases.

Countries have implemented control programmes (CPs) for cattle diseases such as bovine viral diarrhoea virus (BVDV) that are tailored to each country-specific situation. Practical methods are needed to assess the output of these CPs in terms of the confidence of freedom from infection that is achieved. As part of the STOC free project, a Bayesian Hidden Markov model was developed, called STOC free model, to estimate the probability of infection at herd-level. In the current study, the STOC free model was applied to BVDV field data in four study regions, from CPs based on ear notch samples. The aim of this study was to estimate the probability of herd-level freedom from BVDV in regions that are not (yet) free. We additionally evaluated the sensitivity of the parameter estimates and predicted probabilities of freedom to the prior distributions for the different model parameters. First, default priors were used in the model to enable comparison of model outputs between study regions. Thereafter, country-specific priors based on expert opinion or historical data were used in the model, to study the influence of the priors on the results and to obtain country-specific estimates. The STOC free model calculates a posterior value for the model parameters (e.g. herd-level test sensitivity and specificity, probability of introduction of infection) and a predicted probability of infection. The probability of freedom from infection was computed as one minus the probability of infection. For dairy herds that were considered free from infection within their own CP, the predicted probabilities of freedom were very high for all study regions ranging from 0.98 to 1.00, regardless of the use of default or country-specific priors. The priors did have more influence on two of the model parameters, herd-level sensitivity and the probability of remaining infected, due to the low prevalence and incidence of BVDV in the study regions. The advantage of STOC free model compared to scenario tree modelling, the reference method, is that actual data from the CP can be used and estimates are easily updated when new data becomes available.

The existence, stage of eradication and design of control programmes (CPs) for diseases that are not regulated by the EU differ between Member States. When freedom from infection is reached (or being pursued), safe trade is essential to protect (or reach) that status. The aim of STOC free, a collaborative project between six countries, is to develop and validate a framework that enables a transparent and standardized comparison of confidence of freedom for CPs across herds, regions or countries. The framework consists of a model combined with a tool to facilitate the collection of the necessary parameters. All relevant actions taken in a CP are included in a Bayesian network model, which allows prior distributions for most parameters. In addition, frequency of occurrence and risk estimates for factors that influence either the probability of introduction or temporary misclassification leading to delayed detection of the infection are included in the model. Bovine viral diarrhoea virus (BVDV) is used as an example disease. Many countries have CPs in place for BVDV and although elements of the CPs are similar, biosecurity measures and testing protocols, including types of tests and testing frequency, as well as target groups, differ widely. Although the initially developed framework is based on BVDV, the aim is to make it sufficiently generic to be adaptable to CPs for other diseases and possibly other species. Thus, STOC free will result in a single general framework, adaptable to multiple disease CPs, which aims to enhance the safety of trade. European Food Safety Authority

Animal diseases are global issues affecting the productivity and financial profitability of affected farms. Johne’s disease is distributed on farms worldwide and is an endemic contagious bacterial infection in ruminants caused by Mycobacterium avium subspecies paratuberculosis. In cattle, the clinical disease manifests itself as chronic enteritis resulting in reduced production, weight loss, and eventually death. Johne’s disease is prevalent in the UK, including Scotland. Direct costs and losses associated with Johne’s disease have been estimated in previous research, confirming an important economic impact of the disease in UK herds. Despite this, the distributional impact of Johne’s disease among milk consumers and producers in Scotland has not been estimated. In this paper, we evaluate the change in society’s economic welfare, namely to dairy producers (i.e. infected and uninfected herds) and milk consumers in Scotland induced by the introduction of Johne’s disease in the national Scottish dairy herd. At the national-level, we conclude that the economic burden falls mainly on producers of infected herds and, to a lesser extent, milk consumers, while producers of uninfected herds benefit from the presence of Johne’s. An infected producer’s loss per cow is approximately two times larger in magnitude than that of an uninfected producer’s gain. Such economic welfare estimates are an important comparison of the relative costs of national herd prevalence and the wider economic welfare implications for both producers and consumers. This is particularly important from a policy, public good, cost sharing, and human health perspective. The economic welfare framework presented in this paper can be applied to other diseases to examine the relative burden of society’s economic welfare of alternative livestock disease scenarios. In addition, the sensitivity analysis evaluates uncertainty in economic welfare given limited data and uncertainty in the national herd prevalence, and other input parameters, associated with Johne’s disease in Scotland. Therefore, until the prevalence of Johne’s is better understood, the full economic cost to Scottish dairy herds remains uncertain but in the meantime the sensitivity analysis evaluates the robustness of economic welfare to such uncertainties.

Small-scale keepers are less likely to engage with production organisations and may therefore be less aware of legislation, rules and biosecurity practices which are implemented in the livestock sector. Their role in the transmission of endemic and exotic diseases is not well studied, but is believed to be important. The authors use small-scale pig keepers in Scotland as an example of how important small-scale livestock keepers might be for national biosecurity. In Scotland more than two thirds of pig producers report that they keep less than 10 pigs, meaning that biosecurity practices and pig health status on a substantial number of holdings are largely unknown; it is considered important to fill this knowledge gap. A questionnaire was designed and implemented in order to gather some of this information. The questionnaire comprised a total of 37 questions divided into seven sections (location of the enterprise, interest in pigs, details about the pig enterprise, marketing of pigs, transport of pigs, pig husbandry, and pig health/biosecurity). Over 610 questionnaires were sent through the post and the questionnaire was also available online. The questionnaire was implemented from June to October 2013 and 135 questionnaires were returned by target respondents. The responses for each question are discussed in detail in this paper. Overall, our results suggest that the level of disease identified by small-scale pig keepers is low but the majority of the small-scale pig keepers are mixed farms, with associated increased risk for disease transmission between species. Almost all respondents implemented at least one biosecurity measure, although the measures taken were not comprehensive in the majority of cases. Overall as interaction between small-scale keepers and commercial producers exists in Scotland the former can pose a risk for commercial production. This investigation fills gaps in knowledge which will allow industry stakeholders and policy makers to adapt their current disease programmes and contingency plans to the reality of small-scale pig-keeping enterprises’ health and biosecurity status. We predict that some conclusions from this work will be relevant to countries with similar pig production systems and importantly some of these findings will relate to small-scale producers in other livestock sectors.

Classical swine fever (CSF) is a notifiable, highly contagious viral disease of swine which results in severe welfare and economic consequences in affected countries. To improve preparedness, it is critical to have some understanding of how CSF would spread should it be introduced. Based on the data recorded during the 2000 epidemic of CSF in Great Britain (GB), a spatially explicit, premises-based model was developed to explore the risk of CSF spread in GB. We found that large outbreaks of CSF would be rare and generated from a limited number of areas in GB. Despite the consistently low vulnerability of the British swine industry to large CSF outbreaks, we identified concerns with respect to the role played by the non-commercial sector of the industry. The model further revealed how various epidemiological features may influence the spread of CSF in GB, highlighting the importance of between-farm biosecurity in preventing widespread dissemination of the virus. Knowledge of factors affecting the risk of spread are key components for surveillance planning and resource allocation, and this work provides a valuable stepping stone in guiding policy on CSF surveillance and control in GB.

Background\ud Escherichia coli (E. coli) O157 is a virulent zoonotic strain of enterohaemorrhagic E. coli. In Scotland (1998-2008) the annual reported rate of human infection is 4.4 per 100,000 population which is consistently higher than other regions of the UK and abroad. Cattle are the primary reservoir. Thus understanding infection dynamics in cattle is paramount to reducing human infections.\ud \ud A large database was created for farms sampled in two cross-sectional surveys carried out in Scotland (1998 - 2004). A statistical model was generated to identify risk factors for the presence of E. coli O157 on farms. Specific hypotheses were tested regarding the presence of E. coli O157 on local farms and the farms previous status. Pulsed-field gel electrophoresis (PFGE) profiles were further examined to ascertain whether local spread or persistence of strains could be inferred.\ud \ud Results\ud The presence of an E. coli O157 positive local farm (average distance: 5.96km) in the Highlands, North East and South West, farm size and the number of cattle moved onto the farm 8 weeks prior to sampling were significant risk factors for the presence of E. coli O157 on farms. Previous status of a farm was not a significant predictor of current status (p = 0.398). Farms within the same sampling cluster were significantly more likely to be the same PFGE type (p < 0.001), implicating spread of strains between local farms. Isolates with identical PFGE types were observed to persist across the two surveys, including 3 that were identified on the same farm, suggesting an environmental reservoir. PFGE types that were persistent were more likely to have been observed in human clinical infections in Scotland (p < 0.001) from the same time frame.\ud \ud Conclusions\ud The results of this study demonstrate the spread of E. coli O157 between local farms and highlight the potential link between persistent cattle strains and human clinical infections in Scotland. This novel insight into the epidemiology of Scottish E. coli O157 paves the way for future research into the mechanisms of transmission which should help with the design of control measures to reduce E. coli O157 from livestock-related sources.

When facing incursion of a major livestock infectious disease, the decision to implement a vaccination programme is made at the national level. To make this decision, governments must consider whether the benefits of vaccination are sufficient to outweigh potential additional costs, including further trade restrictions that may be imposed due to the implementation of vaccination. However, little consensus exists on the factors triggering its implementation on the field. This work explores the effect of several triggers in the implementation of a reactive vaccination-to-live policy when facing epidemics of foot-and-mouth disease. In particular, we tested whether changes in the location of the incursion and the delay of implementation would affect the epidemiological benefit of such a policy in the context of Scotland. To reach this goal, we used a spatial, premises-based model that has been extensively used to investigate the effectiveness of mitigation procedures in Great Britain. The results show that the decision to vaccinate, or not, is not straightforward and strongly depends on the underlying local structure of the population-at-risk. With regards to disease incursion preparedness, simply identifying areas of highest population density may not capture all complexities that may influence the spread of disease as well as the benefit of implementing vaccination. However, if a decision to vaccinate is made, we show that delaying its implementation in the field may markedly reduce its benefit. This work provides guidelines to support policy makers in their decision to implement, or not, a vaccination-to-live policy when facing epidemics of infectious livestock disease.

Background Escherichia coli O157 is an important cause of acute diarrhoea, haemorrhagic colitis and, especially in children, haemolytic uraemic syndrome (HUS). Incidence rates for human E. coli O157 infection in Scotland are higher than most other United Kingdom, European and North American countries. Cattle are considered the main reservoir for E. coli O157. Significant associations between livestock related exposures and human infection have been identified in a number of studies. Results Animal Studies: There were no statistically significant differences (P = 0.831) in the mean farm-level prevalence between the two studies (SEERAD: 0.218 (95%CI: 0.141-0.32); IPRAVE: 0.205 (95%CI: 0.135-0.296)). However, the mean pat-level prevalence decreased from 0.089 (95%CI: 0.075-0.105) to 0.040 (95%CI: 0.028-0.053) between the SEERAD and IPRAVE studies respectively (P < 0.001). Highly significant (P < 0.001) reductions in mean pat-level prevalence were also observed in the spring, in the North East and Central Scotland, and in the shedding of phage type (PT) 21/28. Human Cases: Contrasting the same time periods, there was a decline in the overall comparative annual reported incidence of human cases as well as in all the major PT groups except 'Other' PTs. For both cattle and humans, the predominant phage type between 1998 and 2004 was PT21/28 comprising over 50% of the positive cattle isolates and reported human cases respectively. The proportion of PT32, however, was represented by few (P = 0.002). Conclusion There was no significant decrease in the mean farm-level prevalence of E. coli O157 between 1998 and 2004 in Scotland, despite significant declines in mean pat-level prevalence. Although there were declines in the number of human cases between the two study periods, there is no statistically significant evidence that the overall rate (per 100,000 population) of human E. coli O157 infections in Scotland over the last 10 years has altered. Comparable patterns in the distribution of PTs 21/28 and 32 between cattle and humans support a hypothesized link between the bovine reservoir and human infections. This emphasizes the need to apply and improve methods to reduce bovine shedding of E. coli O157 in Scotland where rates appear higher in both cattle and human populations, than in other countries.

A methodology is described to establish the relative financial benefit of farm animal disease prevention (biosecurity). This methodology is demonstrated using the example of bovine viral diarrhoea virus (BVDV) incursion on beef suckler farms in Scotland. A random sample of 276 herds was taken and a proportion of young stock on each farm tested for previous exposure to BVDV. There was evidence that 0.4 of herds had been exposed over one year prior to sampling. All herds completed a questionnaire about their biosecurity practices. The influence of these practices on relative risk of BVDV was subjected to a Chi squared test and practices ranked accordingly. Most important risk factors were animal buying in strategy, farm size and a single farm boundary. The economic benefit/disbenefit of these strategies was assessed using a bio-economic model of a 10-year BVDV epidemic. Expected output losses due to BVDV infection and risk were affected by biosecurity strategy and by level of biosecurity threat. The implications of these findings for animal health strategy at farm and regional level are discussed.

This paper provides an ex-ante economic analysis, comparing six alternative control strategies for the eradication of Bluetongue virus 8 against five incursion scenarios in cattle and sheep populations. The economic analysis assumes a common baseline unavoidable cost of public and private measures that together contribute to prevention of incursion of BTV8 into Scotland. These costs continue over the five year horizon of this analysis regardless of whether a BTV8 epidemic ensues in Scotland and their total present value was found to be approximately £141m over the 5year period. The benefit of this investment is the costs of a BTV8 outbreak avoided; which depends on the time, location and nature of the incursion, on the control strategies adopted to counter each incursion, on the persistence of the incursion and on the opportunities to mitigate the damage. Specific variations in all these aspects were explored. The benefit-cost ratios were ranked within each incursion scenario to evaluate the efficiency of control outlays. Although the economic model found that benefit-cost ratios were greater than 1 for all interventions strategies examined, the control strategy option with 100% vaccination and protection zone set at Scottish Borders were economically preferable. This implies that if avoided this control option would deliver the greatest benefit from investment in baseline prevention costs. However, in terms of outbreak losses, this vaccination strategy was always most costly. On the other hand, the control strategy with 50% vaccination and all Scotland as a protection zone often provides the lowest benefits in all control options examined

In this paper, we evaluated the welfare effects of a hypothetical programme of Johne's disease eradication from the Scottish dairy herd on different stakeholders in the domestic milk market. We undertook the evaluation using a Markov-Chain simulation and applying an economic welfare analysis which takes into consideration the effects of an eradication programme on product price, on output quantity, on cost and on milk yield for given levels of supply and demand elasticities. We found that, following the eradication of the disease, milk yield per cow increased for all herd sizes in Scotland whereas price and unit cost of milk production fell. Consequently, milk consumers gained around £14.3 million in discounted economic surplus and producers with infected herds around £13.4 million whereas producers with uninfected herds lost around £10.7 million in discounted surplus. The gain in surplus made by consumers and owners of infected herds, however, more than made up for the loss in surplus made by owners of un-infected herds. Therefore, on balance, Scotland gained a net economic surplus of £17 million from the programme.

The predicted mean value of suckled calves produced was £237/cow (SE 1.8). The regression coefficients of the predictors was 0.74 (0.05), 15 (3.9) and 248 (16.5) for date of first calving, average number of 21-day calving periods (calving spread) and proportion of cows barren respectively. Improving each predictor by 1 SD had the combined potential to improve predicted calf value by £64/cow, over 80% of the current typical gross margin for this type of enterprise. Bull care had a significant influence on calving spread and number of biosecurity measures taken had a positive influence on the proportion of cows barren. However, length of breeding season had no significant impact on the above predictors. Reproductive performance was highly variable and indicators were considerably poorer than published targets. It was concluded that farmers in this sample were not managing their herds for high reproductive performance and output as often advocated. This result suggests considerable scope for improved private and public benefits from the management of reproduction in this type of beef production system.

Consequently, milk consumers gained around £14.3 million in discounted economic surplus and producers with infected herds around £13.4 million whereas producers with uninfected herds lost around £10.7 million in discounted surplus. The gain in surplus made by consumers and owners of infected herds, however, more than made up for the loss in surplus made by owners of un-infected herds. Therefore, on balance, Scotland gained a net economic surplus of £17 million from the programme.

Pulsed-field gel electrophoresis (PFGE) was used to investigate the dissemination and diversity of ampicillin-resistant (Amp(r)) and nalidixic acid-resistant (Nal(r)) commensal Escherichia coli strains in a cohort of 48 newborn calves. Calves were sampled weekly from birth for up to 21 weeks and a single resistant isolate selected from positive samples for genotyping and further phenotypic characterization. The Amp(r) population showed the greatest diversity, with a total of 56 different genotype patterns identified, of which 5 predominated, while the Nal(r) population appeared to be largely clonal, with over 97% of isolates belonging to just two different PFGE patterns. Distinct temporal trends were identified in the distribution of several Amp(r) genotypes across the cohort, with certain patterns predominating at different points in the study. Cumulative recognition of new Amp(r) genotypes within the cohort was biphasic, with a turning point coinciding with the housing of the cohort midway through the study, suggesting that colonizing strains were from an environmental source on the farm. Multiply resistant isolates dominated the collection, with >95% of isolates showing resistance to at least two additional antimicrobials. Carriage of resistance to streptomycin, sulfamethoxazole, and tetracycline was the most common combination, found across several different genotypes, suggesting the possible spread of a common resistance element across multiple strains. The proportion of Amp(r) isolates carrying sulfamethoxazole resistance increased significantly over the study period (P < 0.05), coinciding with a decline in the most common genotype pattern. These data indicate that calves were colonized by a succession of multiply resistant strains, with a probable environmental source, that disseminated through the cohort over time.

Background E. coli O157 is a bacterial pathogen that is shed by cattle and can cause severe disease in humans. Phage type (PT) 21/28 is a subtype of E. coli O157 that is found across Scotland and is associated with particularly severe human morbidity. Methods A cross-sectional survey of Scottish cattle farms was conducted in the period Feb 2002-Feb 2004 to determine the prevalence of E. coli O157 in cattle herds. Data from 88 farms on which E. coli O157 was present were analysed using generalised linear mixed models to identify risk factors for the presence of PT 21/28 specifically. Results The analysis identified private water supply, and northerly farm location as risk factors for PT 21/28 presence. There was a significant association between the presence of PT 21/28 and an increased number of E. coli O157 positive pat samples from a farm, and PT 21/28 was significantly associated with larger E. coli O157 counts than non-PT 21/28 E. coli O157. Conclusion PT 21/28 has significant risk factors that distinguish it from other phage types of E. coli O157. This finding has implications for the control of E. coli O157 as a whole and suggests that control could be tailored to target the locally dominant PT.
