Abstract: Background: The European Organisation for Research and Treatment of Cancer (EORTC) 1325-MG/KEYNOTE-054 trial in patients with resected, high-risk stage III melanoma demonstrated improved recurrence-free survival with adjuvant pembrolizumab compared with placebo (hazard ratio 0·57 [98·4% CI 0·43-0·74]; pMethods: This double-blind, randomised, controlled, phase 3 trial was done at 123 academic centres and community hospitals across 23 countries. Patients aged 18 years or older with previously untreated histologically confirmed stage IIIA, IIIB, or IIIC resected cutaneous melanoma, and an Eastern Cooperative Oncology Group performance status score of 1 or 0 were eligible. Patients were randomly assigned (1:1) using a central interactive voice-response system on the basis of a minimisation technique stratified for stage and geographic region to receive intravenously 200 mg pembrolizumab or placebo. Treatment was administered every 3 weeks for 1 year, or until disease recurrence, unacceptable toxicity, or death. The primary endpoint of the trial was recurrence-free survival (reported elsewhere). HRQOL was a prespecified exploratory endpoint, with global health/quality of life (GHQ) over 2 years measured by the EORTC QLQ-C30 as the primary analysis. Analyses were done in the intention-to-treat population. This study is registered with ClinicalTrials.gov, NCT02362594, and EudraCT, 2014-004944-37, and long-term follow-up is ongoing.Findings: Between Aug 26, 2015, and Nov 14, 2016, 1019 patients were assigned to pembrolizumab (n=514) or placebo (n=505). Median follow-up was 15·1 months (IQR 12·8-16·9) at the time of this analysis. HRQOL compliance was greater than 90% at baseline, greater than 70% during the first year, and greater than 60% thereafter for both groups. Because of low absolute compliance numbers at later follow-up, the analysis was truncated to week 84. Baseline GHQ scores were similar between groups (77·55 [SD 18·20] in the pembrolizumab group and 76·54 [17·81] in the placebo group) and remained stable over time. The difference in average GHQ score between the two groups over the 2 years was -2·2 points (95% CI -4·3 to -0·2). The difference in average score during treatment was -1·1 points (95% CI -3·2 to 0·9) and the difference in average score after treatment was -2·2 points (-4·8 to 0·4). These differences are within the 5-point clinical relevance threshold for the QLQ-C30 and are therefore clinically non-significant.Interpretation: Pembrolizumab does not result in a clinically significant decrease in HRQOL compared with placebo when given as adjuvant therapy for patients with resected, high-risk stage III melanoma. Abstract: International audience; Ustekinumab, a monoclonal antibody that targets interleukin (IL)-12/23, is used to treat psoriasis, psoriatic arthritis and Crohn disease. In 2011, a meta-analysis of randomized trials alerted on a potential risk of major adverse cardiovascular events (MACE) within the first months after the initiation of anti-IL-12/23 antibodies. Our objective was to assess if ustekinumab initiation may trigger MACE. Using the French National Health Insurance database, covering 66 million subjects, we included all patients exposed to ustekinumab between 2010 and 2016, classified according to their cardiovascular risk level (high vs. low risk). We conducted a case-time-control study. We defined the ‘risk’ period as the 6 months before MACE, defined as myocardial infarctions and strokes, and the ‘reference’ period as the 6 months before the risk period. The initiation of ustekinumab was screened in both periods, enabling to assess the odds-ratio (OR) between the initiation of ustekinumab and MACE. Among the 9290 patients exposed to ustekinumab, 179 displayed MACE: 65 myocardial infarctions, 68 unstable anginas and 46 strokes. Among patients at high-level cardiovascular risk, a significant association between ustekinumab initiation and MACE occurrence was identified (OR, 4.17; 95% CI, 1.19-14.59). Conversely, no association was found in patients at low-level cardiovascular risk (OR, 0.30; 95% CI, 0.03-3.13). From real-world data, we suggest that ustekinumab initiation could trigger MACE in patients at high cardiovascular risk, in line with current immunologic models of atherosclerotic disease. These results should call for caution regarding the prescription of ustekinumab in patients at high cardiovascular risk.

Abstract: National audience; Face to the insufficient fiber intake in the French diet, promoting the evolution of food behaviors is not enough; improving the food offer is also needed. The Céréfibres project was set up to evaluate the potential of fiber enrichment in cereal food products, without affecting consumer satisfaction. Indeed, palatability is the major limitation to consumption of fiber enriched foods. The project focused on four types of cereal products: baguette, crispbread, breakfast biscuit and biscuit with chocolate filling (for children). For each type of product, two types of fibers were used, each at three different concentrations in order to determine rejection thresholds for consumers. The results highlighted that fiber enrichment is possible in some products. Thus, in crispbreads it is possible to double the fiber content of the standard product. For the baguette, the potential increase is around 30% and at least 67% for the biscuit with chocolate filling. Nevertheless, these potential increases only considered the sensory acceptability, but some technological limits may be considered as well.; Face au déficit de l’apport en fibres des régimes alimentaires des Français, il ne suffit pas d’oeuvrer pour l’évolution des habitudes alimentaires, mais il faut aussi faire évoluer l’offre des produits. For each type of product, two types of fibers were used, each at three different concentrations in order to determine rejection thresholds for consumers. The results highlighted that fiber enrichment is possible in some products. Thus, in crispbreads it is possible to double the fiber content of the standard product. For the baguette, the potential increase is around 30% and at least 67% for the biscuit with chocolate filling. Nevertheless, these potential increases only considered the sensory acceptability, but some technological limits may be considered as well.

Abstract: International audience; Rituximab (RTX), currently recommended as first-line treatment in moderate to severe pemphigus vulgaris (PV) and superficial pemphigus (PS) along with initial systemic steroids, may also be used as second-line or subsequent treatment, and this therapeutic strategy was investigated in a real-life monocentre retrospective survey.Material and methods: all patients treated between January 2010 and March 2018 with RTX as second-line or subsequent treatment for moderate to severe PV or PS and followed for at least one year were included. The main objective was to evaluate rates and times of complete clinical remission (CCR) after a first course of RTX. The secondary objectives consisted mainly of treatment safety, and frequency and time to relapse after the initial CCR.Results: the 24 patients selected received on average 2 cycles of RTX (i.e. 24 initial cycles and 24 additional cycles in all) over a mean follow-up period of 45 months. 18/24 (75%) patients achieved initial CCR within a mean 7.7 months. Despite at least one relapse in 13/18 initially responding patients regardless of relapse time, 59% (14/24) and 33% (8/24) were either in CCR and off treatment, or in partial remission, whether treated or untreated, according to the latest patient news, with an overall response rate of 92%. Safety was fair in these fragile patients.Discussion and conclusion: this survey of the practical use of RTX confirms its interest in moderate to severe pemphigus as a second-line or subsequent treatment, a situation that probably remains relevant even if this molecule is increasingly used as first-line therapy.; Le rituximab (RTX), actuellement recommandé en première ligne dans les formes modérées à sévères de pemphigus vulgaire (PV) ou superficiel (PS) en association à une corticothérapie initiale, peut également être utilisé en 2nde ligne ou plus. Ce schéma d’utilisation a été l’objet d’une analyse rétrospective monocentrique.Matériel et méthodes : tous les patients de notre centre traités entre janvier 2010 et mars 2018 pour PV ou PS modéré à sévère par du RTX en 2nde ligne ou plus et suivis au moins un an ont été inclus. L’objectif principal était évaluer le taux et le délai d’obtention d’une rémission clinique complète (RCC) après une première cure. Les objectifs secondaires étaient d’évaluer la tolérance du traitement, la fréquence et le délai de rechute après RCC initiale.Résultats : les 24 patients inclus ont reçu en moyenne 2 cycles de RTX (24 cycles initiaux et 24 cycles additionnels au total) au cours d’un suivi moyen de 45 mois. Dix huit sur 24 (75 %) ont obtenu une RCC initiale dans un délai moyen de 7,7 mois. Malgré l’apparition d’au moins une rechute chez 13/18 patients répondeurs tous cycles confondus, 59 % (14/24) étaient en RCC sans traitement aux dernières nouvelles pour un taux global de réponse de 92 %. La tolérance était satisfaisante chez ces patients fragiles.Discussion et conclusion : cette étude confirme l’intérêt du RTX dans le traitement du pemphigus modéré à sévère en 2nde ligne, une situation qui reste probablement d’actualité même si son emploi en première ligne est actuellement recommandé.

Abstract: Resume Introduction Le syndrome d’activation macrophagique (SAM) est une pathologie rare de pronostic severe definie par des criteres cliniques, biologiques et cyto-histologiques. Les infections representent une cause classique de SAM. Les SAM secondaires a une fievre boutonneuse mediterraneenne sont rares ; seuls quelques cas sont rapportes dans la litterature. Observations Nous rapportons les cas de deux hommes de 77 et 63 ans qui avaient une eruption maculo-purpurique febrile avec escarre d’inoculation associee a des anomalies biologiques comprenant cytopenie, hyperferritinemie et hypertriglyceridemie. Une hemophagocytose etait presente dans les deux cas. La serologie et la PCR Rickettsia conorii etaient positives en faveur d’une infection recente, confirmant le diagnostic de fievre boutonneuse mediterraneenne (FBM). Ces elements permettaient dans les deux cas de poser le diagnostic de SAM secondaire a une FBM. Discussion Le premier cas de SAM a ete decrit en 1979. Seize cas de SAM secondaires a une FBM sont decrits dans la litterature. La presence d’une cytopenie associee a une hyperferritinemie et une hypertriglyceridemie au cours d’une FBM est fortement evocatrice du diagnostic de SAM. Le pronostic depend du delai diagnostique et des facteurs propres a l’hote. L’association d’immunosuppresseurs a l’antibiotherapie peut etre necessaire pour obtenir la guerison. Conclusion Les rickettsioses peuvent induire un syndrome d’hemophagocytose ; la survenue possible de cette complication de mauvais pronostic merite d’etre connue.

14. Supplementary Material for: Efficacy and Tolerance of Cetuximab Alone or Combined with Chemotherapy in Locally Advanced or Metastatic Cutaneous Squamous Cell Carcinoma: An Open Study of 14 Patients

Abstract: Background/Aims: Previous reports highlighted the potential interest of cetuximab alone or in combination with chemotherapy in locally advanced or metastatic cutaneous squamous cell carcinomas (cSCC) care. Material and Methods: To further evaluate the efficiency and safety of cetuximab in advanced cSCC, a single-center retrospective study including all patients treated with cetuximab alone or combined with carboplatin for locally advanced or metastatic cSCC was conducted in a tertiary referral center. The primary end point was the overall response rate (ORR) after 2 cycles of treatment. Secondary end points were best overall disease control rate (DCR), overall survival (OS), best response duration, progression-free survival (PFS), and toxicity profile. Results: Of the 14 enrolled patients, no complete response was obtained after 2 cycles of treatment, but 3 partial responses and 6 stable diseases were observed. ORR and DCR were 21.4 and 64.3%, respectively. Median OS and PFS were 9.25 and 2.65 months, respectively. Median PFS was longer with combined treatment compared with cetuximab monotherapy (9.03 vs. 3.55 months). The safety profile was acceptable with a trend toward a relationship between acne-like rash and longer response (median PFS 5.2 vs. 2.2 months). Discussion/Conclusion: In all series including ours, disease control is usually rapidly obtained with cetuximab alone or combined with conventional chemotherapy, although with a minority of partial responses and no complete response. However, this control is of short duration in most cases. The safety profile is acceptable. A randomized phase III trial is warranted to better assess the benefit/risk ratio.

Abstract: International audience; As knowledge continues to develop, regular updates are necessary concerning recommendations for practice. The recommendations for the management of melanoma stages I to III were drawn up in 2005. At the request of the Société Française de Dermatologie, they have now been updated using the methodology for recommendations proposed by the Haute Autorité de Santé. In practice, the principal recommendations are as follows: for staging, it is recommended that the 7th edition of AJCC be used. The maximum excision margins have been reduced to 2cm. Regarding adjuvant therapy, the place of interferon has been reduced and no validated emerging medication has yet been identified. Radiotherapy may be considered for patients in stage III at high risk of relapse. The sentinel lymph node technique remains an option. Initial examination includes routine ultrasound as of stage II, with other examinations being optional in stages IIC and III. A shorter strict follow-up period (3years) is recommended for patients, but with greater emphasis on imaging.

Abstract: International audience; Melanomas have a high rate of brain metastases. Both the functional prognosis and the overall survival are poor in these patients. Until now, surgery and radiotherapy represented the two main modalities of treatment. Nevertheless, due to the improvement in the management of the extracerebral melanoma, the systemic treatment may be an option in patients with brain metastases. Immunotherapy with anti-CTLA4 (cytotoxic T-lymphocyte-associated protein 4) - ipilimumab - or BRAF (serine/threonine-protein kinase B-raf) inhibitors - vemurafenib, dabrafenib - has shown efficacy in the management of brain metastases in a- or pauci-symptomatic patients. Studies are ongoing with anti-PD1 (programmed cell death 1) and combinations of targeted therapies associating anti-RAF (raf proto-oncogene, serine/threonine kinase) and anti-MEK (mitogen-activated protein kinase kinase).

Abstract: Twenty experts, members of a French medical network devoted to neurofibromatosis 1 have elaborated recommendations for the management of the disease. Bibliography was obtained through a Medline of articles from 1966 to 1999 for the terms neurofibromatosis, NF1, neurofibroma and from textbooks. A consensual document was written taking into account extracted data. An annual careful clinical examination is recommended except in cases with complications. Screening investigations are not recommended due to the rarity of complications, generally symptomatic and easily detected during the clinical follow-up. The only controversial exception might be magnetic resonance imaging for early detection of optic pathway gliomas in young children. A co-ordinated follow-up in specialised multidisciplinary centres, providing patients with a rational management, is recommended.

From the nutritional point of view, even though technical solutions exist for the substitution of meat and bone meals and fats of animal origin, one must expect difficulties in the use of some raw materials (soybean meal at high level, palm oil, recycled oils...), altered consumption of the feeds, a possible decrease in performance (in particular with poultry) and an over-excretion of water (turkeys, laying liens) with sanitary consequences. The impact on the quality of the animal products comes essentially from the replacement of animal fats by vegetable oils which are the most often unsaturated and may cause faulty presentation of the carcasses and a poorer conservation, since the products are more sensitive to oxidation. For the consumer and in terms of food safety point, there may be higher potential chemical (contaminants, mycotoxins and other toxic substances produced by plants) and biological (pathogenic bacteria) risks associated with importing raw materials from different origins which are often poorly inspected and are not traceable. Some of these risks are covered by specific regulations of the European Union. There may also be traceability difficulties for those sectors which use official signs of quality or certifications, concerning the importing of non-differentiated raw materials (standard or GMO). There are not, however, identified risks associated with the GMO that have currently been authorised for commercialisation. Finally, concerning the environment, recycling must be stopped ('hidden phosphates' in meat and bone meal) and there must be an increased sampling of the non-renewable resources (mineral phosphates). In addition, there may be an increased rejection of phosphorus (lower bioavailability of plant phosphorus) and traces of metals (not present in meat meals) which come from mineral phosphates.; Différents points d’impact des mesures de suspension des farines et graisses animales ont été identifiés. En matière de technologie des aliments, les répercussions prévisibles sont liées au retrait des graisses animales avec un risque de plus grande friabilité des granulés et des difficultés d’incorporation de quantités importantes de graisses. Au plan nutritionnel, s’il existe des solutions techniques pour remplacer les farines et les graisses animales, on peut cependant prévoir des difficultés d’utilisation de certaines matières premières (tourteau de soja à taux plus élevés, huile de palme, huiles de récupération…), des perturbations de la consommation d’aliments, voire des baisses de performances (volailles surtout) et une surexcrétion d’eau (dinde, poule pondeuse) avec ses conséquences sanitaires. L’impact sur la qualité des produits animaux provient essentiellement du remplacement des graisses animales par des matières grasses d’origine végétale le plus souvent insaturées qui peut se traduire par des défauts de présentation des carcasses et une moins bonne conservation des produits animaux plus sensibles à l’oxydation. Concernant la sécurité alimentaire pour le consommateur, on peut s’attendre à un accroissement des risques potentiels d’ordre chimique (contaminants, mycotoxines et autres substances toxiques produites par les plantes) et biologique (bactéries pathogènes) associés à l’importation de matières premières de provenances multiples, souvent mal contrôlées et dépourvues de traçabilité. Certains de ces risques sont cependant encadrés par des réglementations spécifiques de l’Union Européenne. On peut s’attendre également à des difficultés de traçabilité pour les filières fonctionnant avec des signes officiels de qualité ou des certifications, en regard de l’importation de matières premières standard et OGM non différenciées. En revanche, il n’y a pas de risques identifiés associés aux plantes OGM ayant à ce jour reçu une autorisation de commercialisation. Enfin en matière d’environnement, il faut prévoir un arrêt du recyclage (phosphates " cachés " des farines de viande et d’os) et des prélèvements accrus sur des ressources non renouvelables (phosphates minéraux), ainsi que des risques de rejets plus importants de phosphore (biodisponibilité plus faible du phosphore végétal) et d’éléments traces métalliques (absents des farines animales) provenant des phosphates minéraux.

Abstract: Background Beta-blockers, used as an adjunctive to diuretics, digoxin and angiotensin converting enzyme inhibitors, improve survival in chronic heart failure. We report a prospectively planned economic analysis of the cost of adjunctive beta-blocker therapy in the second Cardiac Insufficiency BIsoprolol Study (CIBIS II). Methods Resource utilization data (drug therapy, number of hospital admissions, length of hospital stay, ward type) were collected prospectively in all patients in CIBIS . These data were used to determine the additional direct costs incurred, and savings made, with bisoprolol therapy. As well as the cost of the drug, additional costs related to bisoprolol therapy were added to cover the supervision of treatment initiation and titration (four outpatient clinic/office visits). Per them (hospital bed day) costings were carried out for France, Germany and the U.K. Diagnosis related group costings were performed for France and the U.K. Our analyses took the perspective of a third party payer in France and Germany and the National Health Service in the U.K. Results Overall, fewer patients were hospitalized in the bisoprolol group, there were fewer hospital admissions perpatient hospitalized, fewer hospital admissions overall, fewer days spent in hospital and fewer days spent in the most expensive type of ward. As a consequence the cost of care in the bisoprolol group was 5-10% less in all three countries, in the per them analysis, even taking into account the cost of bisoprolol and the extra initiation/up-titration visits. The cost per patient treated in the placebo and bisoprolol groups was FF35 009 vs FF31 762 in France, DM11 563 vs DM10 784 in Germany and pound 4987 vs pound 4722 in the U.K. The diagnosis related group analysis gave similar results. Interpretation Not only did bisoprolol increase survival and reduce hospital admissions in CIBIS II, it also cut the cost of care in so doing. This `win-win' situation of positive health benefits associated with cost savings is Favourable from the point of view of both the patient and health care systems. These findings add further support for the use of beta-blockers in chronic heart failure.

Abstract: National audience; Successes encountered in some northern European poultry farms raise interest for the use of whole cereals grains distributed with a protein concentrate in broiler chicken feeding. The three main types of distribution of whole grains are compared: splitted diets, loose mix and sequential feeding. Loose-mix and sequential feeding techniques are efficient. These feeding systems allow to control partly the whole cereal and protein concentrate proportions actually ingested by chickens. On the other hand, splitted diets favour the expression of individual factors of food selection, and does not always allow to get growth and body composition as expected by poultry farmers. Broilers digest whole grain as efficiently as ground cereals. Broilers are able to quickly adapt to a diet including a whole cereal grain, by adjustment of their feeding behaviour. The proportion of cereal grain ingested depends on the physical form and composition of the protein concentrate. Whole cereal grains induce a larger development of gizzard whose effects on digestion and on natural resistance of the birds towards coccidiosis require further research. The system of loose-mix feeding is mastered and applied in some northern European poultry farms by computer monitored distribution. Sequential feeding is less expensive than loose-mix feeding but remains to be validated in practise. Economical interest of this feeding method for French farmers requires further evaluation.; L’utilisation des céréales entières distribuées avec un aliment complémentaire dans l’alimentation du poulet de chair présente un regain d’intérêt avec les succès rencontrés dans certains élevages d’Europe du nord. Les résultats de la recherche permettent de comparer les trois principaux modes de distribution des deux aliments : l’alimentation séparée dans l’espace, le mélange et l’alimentation séquentielle. Mélange et alimentation séquentielle sont adaptés à l’élevage : ces techniques permettent de mieux contrôler les proportions de céréale et d’aliment complémentaire consommées par les poulets. L’alimentation séparée laisse en revanche s’exprimer les facteurs individuels de la sélection alimentaire et ne permet pas toujours d’obtenir la croissance et la composition corporelle escomptées par l’éleveur. Le poulet digère aussi bien les céréales entières que broyées. Il est capable de s’adapter rapidement à un régime comportant une céréale entière, en modifiant son comportement alimentaire. La présentation physique et la composition de l’aliment complémentaire affectent la proportion de céréales consommée. Les céréales entières induisent un développement du gésier, dont les effets sur la digestion et la prévention de la coccidiose méritent d’être approfondis. La technique du mélange est maîtrisée et appliquée dans les élevages d’Europe du nord, avec des systèmes de distribution informatisés. Il reste à valider en pratique le mode de distribution séquentiel, moins coûteux que le mélange, et à étudier l’intérêt économique réel de cette méthode d’alimentation pour l’éleveur de poulet en France.

Abstract: 16 cases of acute methotrexate (MTX) poisoning were reported to the Paris Poison Control Centre and 62 others were published between 1974 and 1995. Until 1992, MTX was mainly prescribed for neoplastic diseases. Clinical features involve acute renal failure, pancytopenia, and cutaneous or mucous injury. These cases emphasise major risk factors responsible for misuse and overdosage. Since 1992, MTX has been increasingly used in rheumatological or pulmonary diseases (rheumatoid arthritis, steroid-dependent asthma). Most of the overdosages were due to a medical misuse (in the dosage regimen or a misunderstanding of the prescription by nurse, pharmacist or patient). All patients had a bone marrow suppression. The treatment is based especially on early administration of folinic acid rescue.
