Emergency care in Germany is in transition. Emergency departments (EDs) treat their patients based on symptoms and acuity. However, this perspective is not reflected in claims data. The aim of the AKTIN project was to establish an Emergency Department Data Registry as a data privacy-compliant infrastructure for the use of routine medical data.Data from the respective documentation systems are continuously transmitted to local data warehouses using a standardized interface. They are available for several applications such as internal reports but also multicentre studies, in compliance with data privacy regulations. Based on a 12-months period we evaluate the population with focus on acuity assessment (triage) and vital parameters in combination with presenting complaints.For the period April 2018 to March 2019, 436,149 cases from 15 EDs were available. A triage level is documented in 86.0% of cases, and 70.5% were triaged within 10 min of arrival. Ten EDs collected a presenting complaint regularly (82.3%). The respective documentation of vital signs shows plausible patterns.The AKTIN registry provides an almost real-time insight into German EDs, regardless of the primary documentation system and health insurance claims data. The Federal Joint Committee's requirements are largely met. Standardized presenting complaints allow for symptom-based analyses as well as health surveillance.HINTERGRUND: Die Notfallversorgung befindet sich im Umbruch. In Notaufnahmen werden Patienten ausgehend von Symptomen und Dringlichkeit versorgt; dies bildet sich jedoch in den etablierten Routinedaten der gesetzlichen Sozialversicherung nicht ab. Ziel des AKTIN-Projekts war der Aufbau einer datenschutzkonformen Registerinfrastruktur zur Nutzung von klinischen Routinedaten aus Notaufnahmen.Über eine standardisierte Schnittstelle werden aus den verschiedenen Dokumentationssystemen kontinuierlich Daten der Notaufnahmebehandlung in ein lokales Data Warehouse exportiert. Dort stehen sie sowohl für lokale Nutzungen, wie interne Berichte und Qualitätsmanagement, als auch gleichzeitig datenschutzkonform für multizentrische Auswertungen zur Verfügung. Anhand der Registerpopulation wird die Ersteinschätzung sowie die Erhebung von Vitalparametern in Abhängigkeit von Vorstellungsgründen für einen 12-Monats-Zeitraum analysiert.Für den Zeitraum 04/2018 bis 03/2019 wurden 436.149 gültige Fälle aus 15 Notaufnahmen übermittelt. In 86,0 % der Fälle ist eine Ersteinschätzung dokumentiert. Diese fand in 70,5 % innerhalb von 10 min nach Ankunft des Patienten statt. In 10 Kliniken wird regelhaft (82,3 %) ein Vorstellungsgrund erfasst. Die Erfassung von Vitalparametern variiert plausibel zwischen den Vorstellungsgründen.Das AKTIN-Notaufnahmeregister bietet einen zeitnahen Einblick in das Versorgungsgeschehen der Notaufnahmen ohne zusätzlichen Dokumentationsaufwand und unabhängig vom primären IT-System, Kostenträger, Fallart und Abrechnungsmodus. Die Vorgaben des Gemeinsamen Bundesausschusses zur Ersteinschätzung werden weitgehend umgesetzt. Durch die Etablierung von standardisierten Vorstellungsgründen werden symptombasierte Analysen und Gesundheitssurveillance ermöglicht.

Hintergrund: Notaufnahmen sind in Deutschland zentraler Anlaufpunkte für akute medizinische Notfälle. Mass measurements of fission and projectile fragments, produced via $^{238}$U and $^{124}$Xe primary beams, have been performed with the multiple-reflection time-of-flight mass spectrometer (MR-TOF-MS) of the FRS Ion Catcher with a mass resolving powers (FWHM) up to 410,000 and an uncertainty of $6\cdot 10^{-8}$. The nuclides were produced and separated in-flight with the fragment separator FRS at 300 to 1000 MeV/u and thermalized in a cryogenic stopping cell. The data-analysis procedure was developed to determine with highest accuracy the mass values and the corresponding uncertainties for the most challenging conditions: down to a few events in a spectrum and overlapping distributions, characterized only by a broader common peak shape. With this procedure, the resolution of low-lying isomers is increased by a factor of up to three compared to standard data analysis. The ground-state masses of 31 short-lived nuclides of 15 different elements with half-lives down to 17.9~ms and count rates as low as 11 events per nuclide were determined. This is the first direct mass measurement for seven nuclides. The excitation energies and the isomer-to-ground state ratios of six isomeric states with excitation energies down to about 280~keV were measured. For nuclides with known mass values, the average relative deviation from the literature values is $(2.9 \pm 6.2) \cdot 10^{-8}$. The measured two-neutron separation energies and their slopes near and at the N=126 and Z=82 shell closures indicate a strong element-dependent binding energy of the first neutron above the closed proton shell Z=82. The experimental results deviate strongly from the theoretical predictions, especially for N=126 and N=127.

12th International Congress on Plasma Physics, 25-29 October 2004, Nice (France); The structure and scaling properties of drift-wave turbulence has been studied using a $8\times$8 Langmuir-probe matrix. Quasi-coherent structures drift into the electron-diamagnetic direction. The scalings of correlation lengths and times with the drift scale parameter $\rho_s$ and other dimensionless parameters are investigated. Using five different gases from hydrogen to argon, $\rho_s$ could be varied by a factor of 10. A scaling weaker than Bohm-like has been found for the correlation length. The study of $\rho_s$ scaling on the diffusivity turns out to be misleading since residual dependencies on other parameters are present and the cross-phase between poloidal electric field and density fluctuations is also a function of $\rho_s$. The measured turbulent diffusivity is consistent with typical values reported for the edge of fusion plasmas.

12th International Congress on Plasma Physics, 25-29 October 2004, Nice (France); The three-dimensional structure of drift wave turbulence is studied in the low temperature plasma of the torsatron TJ-K.Previous 2D investigations of plasma density and floating potential fluctuations in the poloidal plane have shown that drift waves play the dominant role for the formation of coherent structures and radial transport [U. Stroth, Phys. Plasma, 11, 2558(2004)]. The observed structures are quasi-coherent and show a scaling with the drift scale rho_s=c_s/w_ci. The distinguished characteristic of drift waves is the dynamic parallel to the magnetic field, i.e. k_||=/=0. Therefore a detailed understanding requires fully three dimensional investigations. A combination of a 2D-movableprobe and a poloidal 8x8 matrix of electrostatic probes is used to investigate the parallel dynamics in TJ-K. A heuristic model is proposed, which shows that the parallel dynamic in TJ-K is described by the occurrence of large, spatially localized coherent structures with drift wave dynamics.
