1. Renal pelvisteki idrarın üreter taşının impaktasyonunu belirlemedeki rolü

Abstract

Renal pelvisteki idrarın üreter taşının impaktasyonunu belirlemedeki rolüÖzetAmaç: Üreter taşının olduğu taraftaki renal pelvisin Hounsfield Ünitesi değerinin taşın impaktasyonu hakkında fikir verip veremeyeceğini değerlendirmek.Materyal ve method: Üreter taşı nedeniyle opere edilen hastaların, demografik verileri ile operasyon öncesi çekilen kontrastsız tomografi filmine göre hesaplanan, taşın transvers ve en uzun boyutu, üst üreterin çapı, üst üreterin alt üreter çapına oranı, taş dansitesi, renal pelvisin ön arka çapı ile renal pelvisteki idrarın ve mesanedeki idrarın HU değeri, taşın impakte olduğu ve olmadığı iki grup arasında karşılaştırıldı.Bulgular: Hastaların ortanca yaşı 44 (37 - 53) yıl olup, %86.4’ü erkekti. Yaş, cinsiyet, komorbidite, taraf ve taş lokalizasyonu bakımından gruplar arasında fark yoktu (p=0.067, p=0.073, p=0.093, p=0.0141 ve p=0.074, sırasıyla). Renal pelvisteki idrarın HU değeri ile mesanedeki idrarın HU değeri gruplar arasında karşılaştırıldığında anlamlı farklılık görülmedi (p=0.511, p=0.376, sırasıyla). Mesanedeki idrar dansitesinin renal pelvis dansitesine oranına bakıldığında gruplar arasında anlamlı fark yoktu (p=0.461). 2. Hemoroidektomi Sonrası Üriner Retansiyon: Riski Azaltabilir miyiz?

Abstract

Objectives: Hemorrhoidectomy is a common surgery and urinary retention (UR) after anorectal surgeries is an unwelcome complication. In this study we aimed to investigate risk of UR after hemorrhoidectomy and predict the factors associated with UR.Methods: Between 2016-2018 years, 180 patients who undergone hemorrhoidectomy by general anesthesia were included study. The patients with urological symptoms and urological surgery before were excluded. Results: Median age of patients was 45.5 (IQR: 40.0-55.0) years. There was 116 (64.4%) of male and 64 (35.6%) of woman. Urinary retention was seen significantly higher in men (p=0.03) but, visual pain scale (VPS) and the analgesic requirement were similar between men and women (p=0.39 and p=0.86, respectively). Regression analysis showed that male gender, operation time and analgesic requirement was not associated UR and the VPS is only predictive factor for UR (OR:0.224, p, Amaç: Hemoroidektomi sık uygulanan bir ameliyattır ve anorektal ameliyatlar sonrası üriner retansiyon (ÜR) istenmeyen bir komplikasyondur. Bu çalışmada hemoroidektomi sonrası ÜR riskini araştırmayı ve ÜR ile ilişkili faktörleri tahmin etmeyi amaçladık.Yöntemler: 2016-2018 yılları arasında genel anestezi ile hemoroidektomi yapılan 180 hasta çalışmaya dahil edildi. Abstract

Objective: In this study, we aimed to investigate the early and late effect of percutaneous nephrolithotomy on renal function based on preoperative glomerular filtration rate levels.Material and Method: Between January 2012 and November 2016, 351 patients who underwent percutaneous nephrolithotomy were included. 4. Patient Selection Criteria For Off-Pump and On-Pump Coronary Artery Bypass Grafting

Abstract

Amaç: Bu çalışmada, koroner arter hastalıklarının tedavisinde off-pump ve on-pump cerrahi metodlarının, güvenilirliği, etkinliği ve teknik seçiminin nedenleri karşılaştırılmıştır. Gereç ve Yöntemler: Mart 2003-Ocak 2013 tarihleri arasında izole KABG( Koroner Arter Baypas Greftleme) operasyonu uygulanan ardışık 540 hasta, retrospektif olarak karşılaştırıldı. On-pump metod ile tedavi edilen 270 hasta(Grup 1) ile off-pump metod ile tedavi edilen 270 hasta (Grup 2) karşılaştırıldı. On-pump metod (Grup 1) uygulanan hastaların, 215'i erkek ve 55'i kadındı ve ortalama yaş 56,3±9,3 idi. 27(%10) hastada sol ana koroner arter lezyonu tanısı kondu. Off-pump metod (Grup 2) uygulanan hastaların, 209'u erkek ve 61'i kadındı ve ortalama yaş 57,8±9,8 idi. Bulgular: Koroner arter baypas yapılan damar sayısı grup 1'de 3,1±0,6; grup 2'de ise 2,1±0,7 idi. Bu farklılık anlamlı bulundu (p=0,016). Mekanik ventilatör desteğinde geçen süre grup 2'de daha kısa idi(p=0,0001). Grup 1'de üç ve/veya daha çok hedef damara baypas yapılan hasta sayısı, grup 2'ye göre daha fazla iken; bir ve/veya iki hedef damara baypas yapılan hasta sayısı ise, grup 2'ye göre grup 1'de daha azdı. CK-MB ve troponin I düzeyleri, grup 2'de daha düşüktü. Grup 2'deki hastaların grup 1'dekilere göre daha kısa hastanede kalış süreleri olduğu gözlendi (p=0,0001). Sonuç: Off-pump operasyon tekniği, daha az sayıda hedef damar sayısı olan hastalarda tercih edilirken; On-pump operasyon tekniği ise diabetes mellitus(DM) tanılı, çoklu damar lezyonu olan ve sol ana koroner arter lezyonu tanısı olan hastalarda tercih edildi. Anahtar kelimeler: Off-Pump Koroner Baypas; On-Pump Koroner Bypass; Miyokardial İskemi., Background: This study compares safety, efficacy, and reasons of selection criteria between off-pump coronary bypass grafting (OPCAB) and conventional on-pump coronary bypass grafting (CCAB) in patients with coronary artery disease. Material and Methods: The study feeds on a retrospective, observational cohort study of data collected from 540 consecutive patients with coronary artery disease undergoing isolated CABG (coronary artery bypass grafting) (270 OPCAB , 270 CCAB) between March 2003 and January 2013. Two hundred and seventy patients who underwent surgery with on-pump method (group 1) were compared to the group of 270 patients who underwent surgery with off-pump method (group 2). Results: The mean number of anastomoses was 3.1±0.6 in group 1 and 2.1±0.7 in group 2; the difference was significant (p=0.016). The duration of mechanical ventilation support time was shorter for patients in group 2 (p=0.0001). The number of patients with three and/or four target vessel coronary artery bypasses in group 1 was substantially higher compared with the patients in group 2, while the number of patients undergoing one and/or two target vessel bypasses was substantially lower. The levels of CK-MB and troponin I were substantially lower in group 2. Group 2 patients demonstrated shorter hospital stay period than group 1 patients (p=0.0001). Conclusion: The off-pump operation technique is preferred for patients with less target vessel anastomoses, while the on-pump operation technique is preferred for the patients with diabetes mellitus, multiple coronary artery lesions, and left main coronary artery (LMCA) lesions. Abstract

PURPOSE: Sperm DNA fragmentation (SDF) testing was recently added to the sixth edition of the World Health Organization laboratory manual for the examination and processing of human semen. Many conditions and risk factors have been associated with elevated SDF; therefore, it is important to identify the population of infertile men who might benefit from this test. The purpose of this study was to investigate global practices related to indications for SDF testing, compare the relevant professional society guideline recommendations, and provide expert recommendations. MATERIALS AND METHODS: Clinicians managing male infertility were invited to take part in a global online survey on SDF clinical practices. This was conducted following the CHERRIES checklist criteria. The responses were compared to professional society guideline recommendations related to SDF and the appropriate available evidence. Expert recommendations on indications for SDF testing were then formulated, and the Delphi method was used to reach consensus. RESULTS: The survey was completed by 436 experts from 55 countries. Almost 75% of respondents test for SDF in all or some men with unexplained or idiopathic infertility, 39% order it routinely in the work-up of recurrent pregnancy loss (RPL), and 62.2% investigate SDF in smokers. While 47% of reproductive urologists test SDF to support the decision for varicocele repair surgery when conventional semen parameters are normal, significantly fewer general urologists (23%; p=0.008) do the same. Nearly 70% would assess SDF before assisted reproductive technologies (ART), either always or for certain conditions. Recurrent ART failure is a common indication for SDF testing. Very few society recommendations were found regarding SDF testing. CONCLUSIONS: This article presents the largest global survey on the indications for SDF testing in infertile men, and demonstrates diverse practices. Furthermore, it highlights the paucity of professional society guideline recommendations. Expert recommendations are proposed to help guide clinicians.

Abstract

Purpose: Varicocele is a common problem among infertile men. Varicocele repair (VR) is frequently performed to improve semen parameters and the chances of pregnancy. However, there is a lack of consensus about the diagnosis, indications for VR and its outcomes. The aim of this study was to explore global practice patterns on the management of varicocele in the context of male infertility. Materials and Methods: Sixty practicing urologists/andrologists from 23 countries contributed 382 multiple-choice-questions pertaining to varicocele management. These were condensed into an online questionnaire that was forwarded to clinicians involved in male infertility management through direct invitation. The results were analyzed for disagreement and agreement in practice patterns and, compared with the latest guidelines of international professional societies (American Urological As- sociation [AUA], American Society for Reproductive Medicine [ASRM], and European Association of Urology [EAU]), and with evidence emerging from recent systematic reviews and meta-analyses. Additionally, an expert opinion on each topic was provided based on the consensus of 16 experts in the field. Results: The questionnaire was answered by 574 clinicians from 59 countries. The majority of respondents were urologists/ uro-andrologists. A wide diversity of opinion was seen in every aspect of varicocele diagnosis, indications for repair, choice of technique, management of sub-clinical varicocele and the role of VR in azoospermia. A significant proportion of the re- sponses were at odds with the recommendations of AUA, ASRM, and EAU. A large number of clinical situations were identi- fied where no guidelines are available. Conclusions: This study is the largest global survey performed to date on the clinical management of varicocele for male in- fertility. It demonstrates: 1) a wide disagreement in the approach to varicocele management, 2) large gaps in the clinical prac- tice guidelines from professional societies, and 3) the need for further studies on several aspects of varicocele management in infertile men.

Abstract

Ongun, Şakir (Balikesir Author), Introduction Following the COVID-19 pandemic, the face-to-face meetings are delayed to a future date, which is still not clear. However, seminars, meetings and conferences are necessary for updating our knowledge and skills. Web-based seminars (webinars) are the solutions to this issue. This study aimed to show the participant behaviour when webinars present at the COVID-19 pandemic era. Methods From December 2017 to July 2020, 58 webinars were broadcasted via the Uropedia, electronic library of SUST. Data of all webinars were collected with the YouTube analytics and application of the Uropedia. Data of streaming webinars included participant behaviours such as content views, engagement time, total unique attendees, average engagement time and the number of audience to leads. Data were split into two groups; group-1 is webinars before COVID-19 (before March 2020) and group-2 is the webinars during COVID-19. Results Total broadcast time and total page view number were found to be 112.6 hours (6761 minutes) and 15 919, respectively. The median participant age was 40.1 y. Median content view and median engagement time were found to be 261.0 and 12.2 minutes, respectively. Comparison of two groups revealed a significant increment in the content views (group-1; 134.0 range = 86.0-87.0 and group-2; 414.0 range = 296.0-602.0, P < .001) and the number of the unique attendees (group 1; 18.0 range = 10.0-26.0 and group-2; 57.0 range = 27.0-100.0, P < .001) following COVID-19. However, the median engagement time of the audience did not seem to change with the COVID-19 pandemic (group-1; 11.5 range = 10.0-13.3 minutes and group-2; 13.2 range = 9.4-18.1 minutes, P = .12). Conclusion The webinars are effective ways to share information and have many advantages, including low cost, reaching a high number of audiences. Audience number and page visits seemed to increase following the COVID-19 pandemic. However, this era did not seem to affect the critical attitude of the audience, which is engagement time.
