Local management of adult soft tissue sarcoma of the extremities has evolved over the past decades. Until the 1970s, radical surgery (amputations) was the standard therapeutic procedure resulting in significant physical and psychological morbidity for the patients. In the present era, limb sparing surgery combined with radiotherapy represents the current standard of care for high grade and > 5 cm STSs. This approach guarantees high local control rate and function preservation. The aim of this paper is to summarize the current evidence for RT in STSs of the extremities. Outcomes, technical details (techniques, timing, dose, volumes of treatment) and the emerging role of RT in the management of oligometastatic disease will be analysed. Finally, results of the recent clinical trials testing new scenarios in RT of STSs will be described.

Since its discovery in mid 19th century, various papers regarding waterjet processes have been published, making this technology mature from the research standpoint. The goal of this review is to highlight advantages and issues of waterjet and abrasive waterjet basing on previous studies, as well as to provide statistical information about the quoted papers. This research is focused on three main material classes (metallic alloys, composite materials, soft materials), with particular regard to aerospace- and automotive- related applications. One of the most interesting researches about cutting optimization and performances in milling complex 3D features is here summarized, considering also advanced and promising materials for the aerospace field such as CFRP/Ti stacks. An insight regarding sustainability-related processes as metallic alloys remanufacturing and tyres recycling is also provided. Finally, the paper also presents an overview on the current industrial employment of the aforementioned technologies, listing some companies and their most attracting solutions.

Paramutation involves the transfer of a repressive epigenetic mark from a silent allele to an active homolog and, consequently, non-Mendelian inheritance. In tomato, the sulfurea (sulf) paramutation is associated with a high level of CHG hypermethylation in a region overlapping with the transcription start site (TSS) of the SlTAB2 gene that affects chlorophyll synthesis. The CCG subcontext hypermethylation is under-represented at this region relative to CTG or CAG, implicating the CHROMOMETHYLTRANSFERASE3a (CMT3) in paramutation at this locus. Consistent with this interpretation, loss of CMT3 function leads to loss of the sulf chlorosis, the associated CHG hypermethylation, and paramutation. Loss of KRYPTONITE (KYP) histone methyltransferase function has a similar effect linked to reduced H3K9me2 at the promoter region of SlTAB2 and a shift in higher order chromatin structure at this locus. Mutation of the largest subunit of RNA polymerase V (PolV) in contrast does not affect sulf paramutation. These findings indicate the involvement of a CMT3/KYP–dependent feedback loop rather than the PolV-dependent pathway leading to RNA-directed DNA methylation (RdDM) in the maintenance of paramutation.

Language production has often been described as impaired in psychiatric diseases such as in psychosis. Nevertheless, little is known about the characteristics of linguistic difficulties and their relation with other cognitive domains in patients with a first episode of psychosis (FEP), either affective or non-affective. To deepen our comprehension of linguistic profile in FEP, 133 patients with FEP (95 non-affective, FEP-NA; 38 affective, FEP-A) and 133 healthy controls (HC) were assessed with a narrative discourse task. Speech samples were systematically analyzed with a well-established multilevel procedure investigating both micro- (lexicon, morphology, syntax) and macro-linguistic (discourse coherence, pragmatics) levels of linguistic processing. Executive functioning and IQ were also evaluated. Both linguistic and neuropsychological measures were secondarily implemented with a machine learning approach in order to explore their predictive accuracy in classifying participants as FEP or HC. Compared to HC, FEP patients showed language production difficulty at both micro- and macro-linguistic levels. As for the former, FEP produced shorter and simpler sentences and fewer words per minute, along with a reduced number of lexical fillers, compared to HC. At the macro-linguistic level, FEP performance was impaired in local coherence, which was paired with a higher percentage of utterances with semantic errors. Linguistic measures were not correlated with any neuropsychological variables. No significant differences emerged between FEP-NA and FEP-A (p≥0.02, after Bonferroni correction). Machine learning analysis showed an accuracy of group prediction of 76.36% using language features only, with semantic variables being the most impactful. Such a percentage was enhanced when paired with clinical and neuropsychological variables. Results confirm the presence of language production deficits already at the first episode of the illness, being such impairment not related to other cognitive domains. The high accuracy obtained by the linguistic set of features in classifying groups support the use of machine learning methods in neuroscience investigations.

BACKGROUND Chemotherapy and radiotherapy followed by durvalumab is currently the standard treatment for locally advanced node-positive non-small-cell lung cancer (NSCLC). We describe the case of a patient with locally advanced node-positive NSCLC (LA-NSCLC) treated in a phase II prospective protocol with chemotherapy, accelerated hypofractionated radiotherapy (AHRT) and surgery in the pre-immunotherapy era. CASE SUMMARY A 69-year-old male, ex-smoker (20 PY), with a Karnofsky performance status of 90, was diagnosed with locally advanced squamous cell lung carcinoma. He was staged by total body computed tomography (CT) scanning, and integrated 18F-fluorodeoxyglucose positron emission tomography/CT scan [cT4 cN3 cM0, stage IIIC according to TNM (tumor-node-metastasis) 8th edition] and received AHRT between chemotherapy cycles, in accordance with the study protocol (EudractCT registration 2008-006525-14). At the end of the study the patient underwent surgery, which was not part of the protocol, and showed a complete pathological response. CONCLUSION This case report confirms that AHRT can be used successfully to treat primary LA-NSCLC with bilateral mediastinal lymph node involvement. Our case is of particular interest because of the pathological response after AHRT and the lack of surgical complications. We hypothesize that this radiotherapeutic approach, with its proven efficacy, could be delivered as a short course reducing treatment costs, increasing patient compliance and reducing toxicity. We are currently investigating the possibility of combining hypofractionation, chemotherapy and immunotherapy for patients with LA-NSCLC.

The Coleman-Oort conjecture says that for large $g$ there are no positive-dimensional Shimura subvarieties of $\mathsf{A}_g$ generically contained in the Jacobian locus. Counterexamples are known for $g\leq 7$. They can all be constructed using families of Galois coverings of curves satisfying a numerical condition. These families are already classified in cases where: a) the Galois group is cyclic, b) it is abelian and the family is 1-dimensional, and c) $g\leq 9$. Osteoporosis is a systemic skeletal disorder characterized by increased bone fragility, which is associated with an enhanced fracture risk. The first fracture often represents indeed the clinical manifestation of this condition. In the present document we provided an overview of the economic and clinical impact of a not-adequate therapeutic appropriateness and suboptimal adherence to osteoporosis therapy, that are both widely reported in literature despite osteoporotic treatments have proved their efficacy in reducing fracture risk. Adequate treatment and adherence were reported to be associated with a lower risk of re-fracture and all-cause mortality. Moreover, healthcare costs in osteoporotic patients with previous fractures were significantly lower in those receiving osteoporosis treatment rather than among untreated patients. Nevertheless, these two key-factors are not improving over time. The measurement of indicators of adherence and therapeutic appropriateness allows to analyse the utilization profile of the drugs indicated for the treatment of osteoporosis and to evaluate the presence of possible deviation between the prescriptive behaviours observed in clinical practice and the recommendations reported in the guidelines. The periodic monitoring of such indicators together with prescribing audit activity could represent a useful tool for the optimization of osteoporosis management and to achieve a correct resource allocation. Although there is still no standard treatment for recurrent glioblastoma multiforme (rGBM), re-irradiation could be a therapeutic option. We retrospectively evaluated the efficacy and safety of re-irradiation using helical TomoTherapy (HT) with a simultaneous integrated boost (SIB) technique in patients with rGBM. 24 patients with rGBM underwent HT-SIB. A total dose of 20 Gy was prescribed to the Flair (fluid-attenuated inversion recovery) planning tumor volume (PTV) and 25 Gy to the PTV-boost (T1 MRI contrast enhanced area) in 5 daily fractions to the isodose of 67% (maximum dose within the PTV-boost was 37.5 Gy). Toxicity was evaluated by converting the 3D-dose distribution to the equivalent dose in 2 Gy fractions on a voxel-by-voxel basis. Median follow-up after re-irradiation was 27.8 months (range 1.6–88.5 months). Median progression-free survival (PFS) was 4 months (95% CI 2.0–7.9 months), while 6-month PFS was 41.7% (95% CI 22.2–60.1 months). Median overall survival following re-irradiation was 10.7 months (95% CI 7.4–16.1 months). There were no cases of re-operation due to early or late toxicity. Our preliminary results suggest that helical TomoTherapy with the proposed SIB technique is a safe and feasible treatment option for patients with rGBM, including those large disease volumes, reducing toxicity.

In locally advanced pancreatic cancer (LAPC), the combination of chemotherapy and radiotherapy is a widely used treatment option. We performed a pooled analysis, including an exploratory analysis for prognostic and predictive factors, of two phase 2 trials including 73 patients with LAPC, treated with gemcitabine and oxaliplatin (GEMOX) and hypofractionated tomotherapy. With a median follow-up of 36 months (range 1–65), median progression-free (PFS) and overall survival (OS) were 10.2 (95% confidence interval [CI] 7.8–13.2) and 14.3 (95% CI 12.0–18.1) months, respectively. The overall resectability rate was 23.3% (95% CI 13.6–33.0), and the R0 resection rate was 13.7% (95% CI 5.8–21.6). In the multivariate analysis, ECOG performance status (PS) 0 and low levels of CA 19–9 were associated with improved OS and PFS. Concerning OS, log(CA19–9) resulted in a hazard ratio (HR) of 1.20 (95% CI 1.02–1.42), p = 0.027. For ECOG PS 0, HR was 1.00, for PS 1, HR was 2.69 (95% CI 1.46–4.96), for PS 2, HR was 4.18 (95% CI 0.90–19.46), p = 0.003. Low CA19–9 levels were also predictive for resection, with an odds ratio of 0.71 (95% CI 0.52–0.97), p = 0.034. In conclusion, GEMOX and hypofractionated radiotherapy is a treatment option in LAPC. Further studies are needed to identify differences in tumor biology, which may help to predict resectability and prognosis.

As for any symmetric space the tangent space to Siegel upper-half space is endowed with an operation coming from the Lie bracket on the Lie algebra. We consider the pull-back of this operation to the moduli space of curves via the Torelli map. We characterize it in terms of the geometry of the curve, using the Bergman kernel form associated to the curve. It is known that the second fundamental form of the Torelli map outside the hyperelliptic locus can be seen as the multiplication by a certain meromorphic form. Our second result says that the Bergman kernel form is the harmonic representative - in a suitable sense - of this meromorphic form.

We describe a new construction of families of Galois coverings of the line using basic properties of configuration spaces, covering theory, and the Grauert-Remmert Extension Theorem. Our construction provides an alternative to a previous construction due to Gonz\'alez-D\'{i}ez and Harvey (which uses Teichm\"uller theory and Fuchsian groups) and, in the case the Galois group is non-abelian, corrects an inaccuracy therein., Comment: Final version accepted for publication on Algebraic and Geometric Topology

Malignant Pleural Mesothelioma (MPM) is a rare malignancy with an overall poor prognosis. The standard therapeutic strategy in early-stage disease is trimodality therapy. In this publication, we report the preliminary toxicity results of the first 20 patients treated with accelerated hypofractionated radiotherapy. Between July 2017 to June 2019, 20 MPM patients were enrolled and treated with accelerated hypofractionated radiotherapy using helical tomotherapy and intensity-modulated arc therapy. The prescription dose was 30 Gy in five daily fractions, while an inhomogeneous dose escalation to 40 Gy was prescribed based solely upon the presence of gross residual tumor. Only one case of G3 toxicity was reported, which was a bilateral pneumonitis that occurred two years after treatment probably due to superinfection. Median Time to Progression reached 18.2 months while one- and three-year Overall Survival rates were 85% (95% CI:60.4–94.9) and 49.5% (95% CI:26.5–68.9), respectively. Treatment of the intact lung with pleural intensity-modulated arc irradiation is a novel treatment strategy that appears to be safe, feasible, and without a high grade of lung toxicity. Survival rates and Time to Progression are encouraging.

La pandemia da Coronavirus ha evidenziato le disuguaglianze che caratterizzano il tessuto sociale del Paese, ma anche le fragilità della scuola italiana: molti nodi, già noti prima dell’emergenza, sono venuti al pettine durante il primo lockdown e altre debolezze sono destinate a emergere nei prossimi mesi, quando saranno disponibili i dati sugli apprendimenti. In questo contributo forniamo una fotografia del contesto sociale e organizzativo in cui si trovavano la scuola italiana e la sua popolazione prima della pandemia, per poi vedere come il periodo del lockdown e della Dad abbia acuito le difficoltà e rivelato debolezze e inefficienze.

The aim of the study was to evaluate the safety and efficacy of a new chemo-radiotherapy regimen for patients with locally advanced pancreatic cancer (LAPC). Patients were treated as follows: gemcitabine 1000 mg/m2 on day 1, and oxaliplatin 100 mg/m2 on day 2, every two weeks (GEMOX regimen) for 4 cycles, 15 days off, hypofractionated radiotherapy (35 Gy in 7 fractions in 9 consecutive days), 15 days off, 4 additional cycles of GEMOX, restaging. From April 2011 to August 2016, a total of 42 patients with non resectable LAPC were enrolled. Median age was 67 years (range 41&ndash, 75). Radiotherapy was well tolerated and the most frequently encountered adverse events were mild to moderate nausea and vomiting, abdominal pain and fatigue. In total, 9 patients underwent surgical laparotomy (5 radical pancreatic resection 1 thermoablation and 3 explorative laparotomy), 1 patient became operable but refused surgery. The overall resectability rate was 25%, while the R0 resection rate was 12.5%. At a median follow-up of 50 months, the median progression-free survival and overall survival were 9.3 (95% CI 6.2&ndash, 14.9) and 15.8 (95% CI 8.2&ndash, 23.4) months, respectively. The results demonstrate the feasibility of a new chemo-radiotherapy regimen as a potential treatment for unresectable LAPC.

Background: COVID-19 infections have presented with various cutaneous manifestations in COVID-19 positive patients, including reports of rising cases of herpes zoster (HZ). Objective: Our investigation sought to investigate the proposed association between a positive COVID-19 test result and herpes zoster infection as well as determine whether the COVID-19 pandemic has affected the overall incidence of HZ. Methods: In this cross- sectional study, de-identified patient data was collected from the University of California COVID Research Data Set (UC CORDS), a centralized, rapidly accumulating database of COVID-19 tested patients. Patients were divided into those diagnosed with HZ before COVID test and those with HZ after COVID testing. The University of California Health Data Warehouse was then used to collect the total number of HZ cases from March to August in 2019 and 2020 to calculate and compare the HZ condition rate. Results: A total of 608 patients were diagnosed with HZ from March 1 to August 31, 2020; of which, 2.1% (n=13) tested positive for COVID-19. Background Concurrent chemotherapy and radiation using conventional fractionation is the standard treatment for inoperable, locally advanced non-small-cell lung cancer (NSCLC). We tested accelerated hypofractionated radiotherapy (AHR) and chemotherapy for the treatment of locally advanced NSCLC. Methods Eligible patients with locally advanced NSCLC were treated with induction chemotherapy (cisplatin and docetaxel), followed by AHR using tomotherapy and consolidation chemotherapy. The prescribed doses were 30 Gy/5 daily fractions at the reference isodose (60–70%) to the tumor, and 25 Gy/5 daily fractions to the clinically involved lymph nodes. The primary end-point was response rate (RR); the secondary end-points were acute and late side-effects, local progression-free survival (PFS), metastasis-free survival (MFS) and overall survival (OS). This trial closed before the first planned interim analysis due to poor accrual. Results From January 2009 to January 2012, 17 of the 23 enrolled patients were evaluable. Treatment yielded an overall RR of 82%. Median follow-up was 87 months (range: 6–87), local PFS was 19.8 months (95% CI 9.7 - not reached), MFS was 9.7 months (95% CI 5.8–46.0) and OS was 23 months (95% CI 8.4–48.4). 70% of patients experienced acute G4 neutropenia, 24% G4 leukopenia, 24% G3 paresthesia, 4% G3 cardiac arrythmia, 4% underwent death after chemotherapy. Late toxicity was represented by 24% dyspnea G3. Conclusions AHR combined with chemotherapy is feasible with no severe side-effects, and it appears highly acceptable by patients. Background MRI-based differential diagnosis of glioma recurrence (GR) and treatment-induced changes (TICs) remain elusive in up to 30% of treated glioma patients. We aimed to determine 18F-FET PET diagnostic performance in this clinical scenario, its outcome dependency on established prognostic factors, optimal 18F-FET semi-quantitative thresholds, and whether 18F-FET parameters may instantly predict progression-free survival (PFS) and overall survival (OS). Methods We retrospectively analyzed 45 glioma patients treated with chemoradiation therapy (32 males; mean age: 51 years, glioma grade: n=26 WHO4; n=15 WHO3; n=4 WHO2) who underwent 18F-FET PET to resolve differential diagnosis of GR and TICs raised by MRI performed in the preceding 2 weeks and depicting any of the following changes in their radiation field: volumetric increase of contrast-enhancing lesions; new contrast-enhancing lesion; significant increase in T2/FLAIR non-enhancing lesion without reducing corticosteroids. 18F-FET PET outcome relied on evaluation of maximum tumor-to-brain ratio (TBRmax), time-to-peak (TTP), and time-activity curve pattern (TAC). Metabolic tumor volume (MTV) and total tumor metabolism (TTM) were calculated for prognostic purposes. Standard of reference was repeat MRI performed 4–6 weeks after the previous MRI. Non-parametric statistics tested 18F-FET-based parameters for dependency on established prognostic markers. ROC curve analysis determined optimal cutoff values for 18F-FET semi-quantitative parameters. 18F-FET parameters and prognostic factors were evaluated for PFS and OS by Kaplan-Meier, univariate, and multivariate analyses. Results 18F-FET PET sensitivity, specificity, positive predictive value, negative predictive value were 86.2, 81.3, 89.3, 76.5%, respectively; higher diagnostic accuracy was yielded in IDH-wild-type glioma patients compared to IDH-mutant glioma patients (sensitivity: 81.8 versus 88.9%; specificity: 80.8 versus 81.8%). KPS was the only prognostic factor differing according to 18F-FET PET outcome (negative versus positive). Optimal 18F-FET cutoff values for GR were TBRmax ≥ 2.1, SUVmax ≥ 3.5, and TTP ≤ 29 min. PFS differed based on 18F-FET outcome and related metrics and according to KPS; a different OS was observed according to KPS only. On multivariate analysis, 18F-FET PET outcome was the only significant PFS factor; KPS and age the only significant OS factors. Conclusion 18F-FET PET demonstrated good diagnostic performance. 18F-FET PET outcome and metrics were significantly predictive only for PFS.

BackgroundThe presence of hypoxic cells in high-grade glioma (HGG) is one of major reasons for failure of local tumour control with radiotherapy (RT). The use of hyperbaric oxygen therapy (HBO) could help to overcome the problem of oxygen deficiency in poorly oxygenated regions of the tumour. We propose an innovative approach to improve the efficacy of hypofractionated stereotactic radiotherapy (HSRT) after HBO (HBO-RT) for the treatment of recurrent HGG (rHGG) and herein report the results of an ad interim analysis.MethodsWe enrolled a preliminary cohort of 9 adult patients (aged >18 years) with a diagnosis of rHGG. HSRT was administered in daily 5-Gy fractions for 3-5 consecutive days a week. Each fraction was delivered up to maximum of 60 minutes after HBO.ResultsMedian follow-up from re-irradiation was 11.6 months (range: 3.2-11.6 months). The disease control rate (DCR) 3 months after HBO-RT was 55.5% (5 patients). Median progression-free survival (mPFS) for all patients was 5.2 months (95%CI: 1.34-NE), while 3-month and 6-month PFS was 55.5% (95%CI: 20.4-80.4) and 27.7% (95%CI: 4.4-59.1), respectively. Median overall survival (mOS) of HBO-RT was 10.7 months (95% CI: 7.7-NE). No acute or late neurologic toxicity >grade (G)2 was observed in 88.88% of patients. One patient developed G3 radionecrosis.ConclusionsHSRT delivered after HBO appears to be effective for the treatment of rHGG, it could represent an alternative, with low toxicity, to systemic therapies for patients who cannot or refuse to undergo such treatments.Clinical Trial Registrationwww.ClinicalTrials.gov, identifier NCT 03411408.

Drawing on the debate about how feminist issues have increasingly been invoked in neoliberal and conservative discourse (Stacey 1983; Eisenstein 2005; Fraser 2013; McRobbie 2013; Gill 2007; Rottenberg 2014; Schreiber, 2008; Banet-Weiser 2018; Banet-Weiser et al. 2019), this paper addresses how women-related themes are mobilized in the discourses of Italian right-wing female politicians. By combining content analysis and feminist political theory, we provide an in-depth reading of the public statements by three current and former right-wing MPs at the forefront of their parties: Giorgia Meloni, Daniela Santanchè and Flavia Perina. Despite some clear differences, the discourses of these three MPs regarding Italian women’s conditions are representative of how feminist issues can be addressed in conservative neoliberal and nationalistic terms, especially in a context characterized by the overt role of the Catholic Church in political matters and widespread agreement on natural sexual difference (Garbagnoli 2017). Our sources are their discourses on «women» as reported in: 1. the LexNex full-text newspaper dataset during two parliamentary terms (29 April 2008 – 22 March 2018); 2. their authored books on women and/or gender issues. Our inductive analysis of their discourses on three topics related to gender issues (interpretations of sexual difference and second-wave feminism, women’s role in the family and in the labour market, and reproductive rights) will shed light on both their «righting» of feminism (Farris, Rottenberg 2017) and their misrepresentations of it. Our discussion of these data will show that the lack of real critique of the sexual division of labour and of essentialism within neoliberal and social democratic traditions in Italy has allowed for certain ostensibly liberal elements to be added into the right’s discursive field. In particular, we will show that one pervasive theme in their arguments is that a low fertility rate is the main problem of Italian women. This induces all of the female MPs to uphold what we term emancipatory complementarism, a mix of liberal and conservative elements that generates incongruities and internal contradictions. Ultimately, we argue that emancipatory complementarism, in its being a conservative political strategy, is at odds with feminist projects of structural transformation.

The aim of this study is to investigate the role of body dissatisfaction (BD) - conceived here as predicting, predicted and mediating variable - in relation to media influence, body mass index (BMI) and body modification strategies among adolescents, with particular attention to gender differences. Specifically, we investigate, through a multivariated model, the direct and mediated effect of internalization and pressure to conform to aesthetic ideals and BMI on engagement in body modification behaviours, namely eating problems, attitudes toward cosmetic surgery, body art and physical activity. A questionnaire designed to assess all these dimensions was administered to 843 Italian adolescents aged between 15 and 18years. The findings confirm the available evidence on BD as predicted by a number of sociocultural and personal variables, and as predictive of adolescents' bodily attitudes and behaviours. Gendered differences in behaviours considered as 'ways out' from discomforting feelings about physical appearance are discussed.

Commento all'iniziativa "Il giusto mezzo" riguardo alla spesa del Recovery Fund per infrastrutture sociali e gender mainstreaming.

Nell’ultimo decennio, molti paesi in tutta Europa, tra cui l’Italia, hanno registrato crescenti dibattiti pubblici sull’educazione di genere nelle scuole, nonostante il sempre maggiore riconoscimento istituzionale dei benefici di questa attività e una crescente esperienza in questo campo. Il contributo propone una revisione critica della ricerca su questo argomento, mostrando sia le criticità sia i potenziali vantaggi dell’educazione di genere a scuola. Dopo aver presentato alcuni approcci classici alla socializzazione, il contributo si concentra sulla socializzazione di genere, specie alla luce della nuova sociologia dell’infanzia e delle teorie del genere come pratica. Quindi descrive a grandi linee le metodologie e gli obiettivi dell’educazione di genere in Italia e ne discute criticamente i risultati.

In questo contributo viene chiarito il legame tra violenza e stereotipi di genere nelle relazioni di intimità contemporanee, a partire da alcune ricerche empiriche. Viene poi trattato il tema dell’educazione di genere come strumento imprescindibile per mettere in discussione il fondamento di questo legame.

Il capitolo presenta i termini di un dibattito sulla materialità dell'esperienza di genere che si è acceso in reazione alla svolta costruttivista degli anni Novanta. Dalla epistemologia della scienza ai contributi sociologici ispirati all'opera di Berger e Luckmann fino alla svolta fenomenologica femminista, il capitolo si propone di illustrare la varietà di posizioni che occupano un campo che si vuole esterno al riduzionismo biologico ma anche critico verso la fallacia culturalista.

BackgroundMRI-based differential diagnosis of glioma recurrence (GR) and treatment-induced changes (TICs) remain elusive in up to 30% of treated glioma patients. We aimed to determine 18F-FET PET diagnostic performance in this clinical scenario, its outcome dependency on established prognostic factors, optimal 18F-FET semi-quantitative thresholds, and whether 18F-FET parameters may instantly predict progression-free survival (PFS) and overall survival (OS).MethodsWe retrospectively analyzed 45 glioma patients treated with chemoradiation therapy (32 males; mean age: 51 years, glioma grade: n=26 WHO4; n=15 WHO3; n=4 WHO2) who underwent 18F-FET PET to resolve differential diagnosis of GR and TICs raised by MRI performed in the preceding 2 weeks and depicting any of the following changes in their radiation field: volumetric increase of contrast-enhancing lesions; new contrast-enhancing lesion; significant increase in T2/FLAIR non-enhancing lesion without reducing corticosteroids. 18F-FET PET outcome relied on evaluation of maximum tumor-to-brain ratio (TBRmax), time-to-peak (TTP), and time-activity curve pattern (TAC). Metabolic tumor volume (MTV) and total tumor metabolism (TTM) were calculated for prognostic purposes. Standard of reference was repeat MRI performed 4–6 weeks after the previous MRI. Non-parametric statistics tested 18F-FET-based parameters for dependency on established prognostic markers. ROC curve analysis determined optimal cutoff values for 18F-FET semi-quantitative parameters. 18F-FET parameters and prognostic factors were evaluated for PFS and OS by Kaplan-Meier, univariate, and multivariate analyses.Results18F-FET PET sensitivity, specificity, positive predictive value, negative predictive value were 86.2, 81.3, 89.3, 76.5%, respectively; higher diagnostic accuracy was yielded in IDH-wild-type glioma patients compared to IDH-mutant glioma patients (sensitivity: 81.8 versus 88.9%; specificity: 80.8 versus 81.8%). KPS was the only prognostic factor differing according to 18F-FET PET outcome (negative versus positive). Optimal 18F-FET cutoff values for GR were TBRmax ≥ 2.1, SUVmax ≥ 3.5, and TTP ≤ 29 min. PFS differed based on 18F-FET outcome and related metrics and according to KPS; a different OS was observed according to KPS only. On multivariate analysis, 18F-FET PET outcome was the only significant PFS factor; KPS and age the only significant OS factors.Conclusion18F-FET PET demonstrated good diagnostic performance. 18F-FET PET outcome and metrics were significantly predictive only for PFS.

Muovendo dall’analisi del pensiero galileiano che Husserl svolge nella celebre conferenza di Praga, edita successivamente con il titolo di La crisi delle scienze europee, in questo lavoro ci si prefigge di mettere il luce come, nonostante i meriti che il padre della fenomenologia riconosca al filosofo scienziato Galileo Galilei, alcuni grandi errori filosofici si sono perpetuati nella storia proprio a seguito della divulgazione e applicazione del suo metodo alle scienze umane. Il concetto di quantificazione, ad esempio, nato dall’idea della matematizzazione della natura, ha condotto la riflessione scientifica della psicologia ad un vicolo cieco e ad una impossibilità reale di poter comprendere a fondo la complessa struttura della natura spirituale dell’essere umano. La riflessione (la “presa di coscienza storica”) sulla perpetuazione di una forma mentis che nasce dall’assunzione di tale metodo, può dunque, secondo Husserl, salvarci dalla profonda crisi in cui a conseguenza di ciò sono scivolate le scienze. Tanto più importante oggi, in un momento storico così articolato e delicato, in cui la psicologia ha assolutamente bisogno di tornare a dialogare con la dottrina dell’anima e con una considerazione della natura non quantificabile dello spirito umano e del mondo-della-vita.
