La recurrència postquirúrgica en els pacients amb malaltia de Crohn intervinguts és molt freqüent i, al seu torn, té implicacions terapèutiques importants. No s'han caracteritzat fins ara marcadors biològics que, pel seu valor predictiu, poguessin ser d'utilitat en el maneig postquirúrgic d'aquests pacients. En aquest estudi s'analitza el perfil evolutiu després de la cirurgia de diverses citocines (IL-2, IL-6, IL-10, IL1B, IFN-g i TNF-a), marcadors fecals (calprotectina) i paràmetres serològics (proteïna C reactiva, fibrinogen), per determinar la seva possible utilitat com a marcadors predictius de recurrència després de la cirurgia resectiva. La recurrencia postquirúrgica en los pacientes con enfermedad de Crohn intervenidos es muy frecuente y, a su vez, tiene implicaciones terapéuticas importantes. No se han caracterizado hasta la fecha marcadores biológicos que, por su valor predictivo, pudieran ser de utilidad en el manejo postquirúrgico de estos pacientes. En el presente estudio se analiza el perfil evolutivo tras la cirugía de diversas citoquinas (IL-2, IL-6, IL-10, IL1B, IFN-g y TNF-a), marcadores fecales (calprotectina) y parámetros serológicos (proteína C reactiva, fibrinógeno), para determinar su posible utilidad como marcadores predictivos de recurrencia tras la cirugía resectiva.

La malaltia de Crohn és una malaltia inflamatòria intestinal en la qual s'ha observat que la densitat i la diversitat bacteriana difereixen dels subjectes sans. En els últims anys s'estan emprant tractaments biològics com els anti-TNFα (Adalimumab) de manera alternativa a tractaments ja existents. Els resultats obtinguts després de l'anàlisi de la composició bacteriana de la mucosa intestinal de pacients amb Malaltia de Crohn tractats amb Adalimumab, són esperançadors ja que apunten a una recuperació del patró normal a través de la recuperació d'espècies pròpies de persones sanes i la desaparició d'aquelles més prevalents en malalts de Crohn.

La Enfermedad Inflamatoria Intestinal (Enfermedad de Crohn,EC; Colitis Ulcerosa,CU; Colitis inclasificable, CI) ha sufrido variaciones de incidencia, su estudio permite conocer los factores fisiopatológicos implicados y tratamientos específicos. Se ha realizado un estudio observacional retrospectivo de los casos diagnosticados de EII en el Departamento de Salud 6 de Valencia relacionando resultados con los obtenidos en España y otros países. La incidencia estandarizada a la población europea, ajustada a sexo y edad fue: 3.89(EC),6.635(CU) y de 2.845(CI). La incidencia de CU fue más elevada que la de EC, excepto en los más jóvenes, superior para CU y CI en hombres.

This paper is the second in a series of three publications relating to the European evidence-based consensus on the diagnosis and management of Crohn's disease and concerns the management of active disease, maintenance of medically induced remission and surgery. The aims and methods of the ECCO Consensus, as well as sections on diagnosis and classification are covered in the first paper [van Assche et al. JCC 2009a]. The final paper covers post-operative recurrence, fistulating disease, the management of paediatric and adolescent IBD, pregnancy, psychosomatics, extraintestinal manifestations and complementary or alternative therapy for Crohn's disease [Van Assche et al JCC 2009b]. #### Principal changes with respect to the 2006 ECCO guidelines The early use of azathioprine/mercaptopurine or methotrexate in combination with steroids is an appropriate option in moderately active localised ileocaecal CD. Anti-TNF therapy should be considered as an alternative for patients with objective evidence of active disease who have previously been steroid-refractory, steroid-dependent, or steroid-intolerant (based on Statement 5B). For those patients with severely active localised ileocaecal Crohn's disease and objective evidence of active disease who have relapsed, anti-TNF therapy with or without an immunomodulator is an appropriate option [EL1a, RG B for infliximab]. For some patients who have infrequently relapsing disease, restarting steroids with an immunomodulator may be appropriate (based on Statement 5C). All currently available anti-TNF therapies appear to have generally similar efficacy and adverse-event profiles for inflammatory (‘luminal’) Crohn's disease, so the choice depends on availability, route of delivery, patient preference, cost and national guidelines \[EL5, RG D\] (Statement 5I). Patients receiving azathioprine or mercaptopurine who relapse should be evaluated for adherence to therapy and have their dose optimised. Changing their maintenance therapy to methotrexate [EL1b RG B] or anti-TNF therapy [EL1a RGB] should be considered. Surgery should always be considered as an option in localised disease [EL4, …

Background & aim: Immunosenescence can increase morbi-mortality. Lactic acid producing bacteria may improve immunity and reduce morbidity and mortality in the elderly. We aimed to investigate the effects of a mixture of two new probiotic strains of Lactobacillus plantarum-CECT 7315 and 7316- on systemic immunity in elderly. Methods: 50 institutionalized elderly subjects were randomized, in a double-blind fashion, to receive for 12 weeks 1) 5·10(8) cfu/day of L. plantarum CECT7315/7316 ("low probiotic dose") (n = 13), 2) 5·10(9) cfu/day of the probiotic mixture ("high probiotic dose") (n = 19), or 3) placebo (n = 15). Leukocyte subpopulations, and cytokine levels (IL-1 , IL-10, TGF-β1) were measured in venous blood at baseline, end of treatment (week 12), and end of follow-up (week 24). Infection and survival rates were recorded. Results: After treatment, high probiotic dose resulted in significant increases in the percentages of activated potentially T-suppressor (CD8+CD25+) and NK (CD56+ CD16+) cells, while low probiotic dose increased activated T-helper lymphocytes (CD4+CD25+), B lymphocytes (CD19+), and antigen presenting cells (HLA-DR+). Also, plasma TGF-β1 concentration significantly decreased after treatment with both probiotic doses. Most of these changes remained 12 weeks after probiotic discontinuation. Incidence of infections during treatment showed a significant trend to be lower in the high probiotic dose group. In addition, there was a significant trend for mortality to be greater in the placebo group vs. both probiotic groups. Conclusions: Depending on the dose, L. plantarum CECT7315/7316 have different immune-enhancing effects in elderly subjects. These effects might result in a better clinical outcome. Las bacterias productoras de ácido láctico pueden mejorar la inmunidad y disminuir la morbilidad y mortalidad en los ancianos. Nuestro objetivo fue investigar los efectos de una mezcla de dos nuevas cepas probióticas de Lactobacillus plantarum -CECT 7315 y 7316- sobre la inmunidad sistémica en ancianos. Métodos: 50 ancianos institucionalizados se aleatorizaron, en un diseño a doble-ciego, para recibir durante 12 semanas 1) 5·10(8) ufc/día de L. plantarum CECT7315/ 7316 ("dosis baja de probiótico") (n = 13), 2) 5·10(9) ufc/día de la mezcla probiótica ("dosis alta de probiótico") (n = 19), o 3) placebo (n = 15). Se determinaron las subpoblaciones leucocitarias y los niveles de citokinas (IL-1 , IL-10, TGF-β1) en sangre venosa periférica basalmente, al final del tratamiento (sem. 12) y al final del seguimiento (sem. 24). Se registró la tasa de infecciones y la mortalidad. Resultados: Tras el tratamiento, la dosis alta de probiótico indujo aumentos significativos en los porcentajes de células potencialmente T-supresoras (CD8+CD25+) y NK (CD56+CD16+) activadas, en tanto que la dosis baja aumento los linfocitos T-colaboradores activados (CD4+CD25+), los linfocitos B (CD19+), y las células presentadoras de antígeno (HLA-DR+). Asimismo, la concentración plasmática de TGF-β1 disminuyó tras el tratamiento con ambas dosis de probiótico. La mayor parte de estos cambios se mantuvieron 12 semanas después de suspender el tratamiento. La incidencia de infecciones durante el tratamiento mostró una tendencia significativa a ser menor con la dosis alta de probiótico, mientras que se observó una tendencia significativa a que la mortalidad fuera mayor el grupo placebo vs. ambos grupos tratados con probiótico. Conclusiones: Dependiendo de la dosis, L. plantarum CECT7315/7316 tiene distintos efectos inmunoestimulantes en ancianos. Dichos efectos podrían contribuir a una mejor evolución clínica.

Consensus on the European level is brought in order to promote early prevention, diagnosis and optimal management of opportunistic infections in inflammatory bowel disease on behalf of the European Crohn's and Colitis Organisation (ECCO).

To investigate the relationship of sugar malabsorption to the development of clinical symptoms in functional bowel disease.Twenty-five consecutive outpatients [five men, 20 women; mean age 38.7 +/- 2.6 (SEM) yr] with functional bowel disease and symptoms suggestive of carbohydrate malabsorption were studied. Twelve healthy subjects [six men, six women; mean age 35.7 +/- 3.7 (SEM) yr] acted as the control group. Sugar malabsorption was assessed by breath-hydrogen test after an oral load of various solutions containing lactose (50 g), fructose (25 g), sorbitol (5 g), fructose plus sorbitol (25 + 5 g), and sucrose (50 g). The severity of symptoms developing after sugar challenge was studied. In addition, the effect on clinical symptoms of a diet free of the offending sugars, compared to a low-fat diet, was assessed.Frequency of sugar malabsorption was high in both patients and controls, with malabsorption of at least one sugar in more than 90% of the subjects. Median symptom scores after both lactose [median 6; interquartile (IQ) range 3-7] and fructose plus sorbitol (median 2; IQ range 0-4) malabsorption were significantly higher than after sucrose load (median 1; IQ range 0-1.5) in functional bowel disease patients (p = 0.001 and p = 0.007, respectively). However, there were no differences in healthy controls. In addition, symptoms score after both lactose and fructose plus sorbitol malabsorption was significantly higher in patients than in control subjects (p = 0.02 and p = 0.008, respectively). On the other hand, H2 production capacity, as measured following lactulose load, was significantly higher in patients than in controls. The clinical symptoms improved in 40% of the evaluated patients after restriction of the offending sugars.These results suggest that sugar malabsorption may be implicated in the development of abdominal distress in at least a subset of patients with functional bowel disease.
