Objective: To explore and characterize the effect of the discrepancy between crestal bone height (CB) and pulp chamber floor (PCF) in the fatigue performance of endodontically-treated teeth rehabilitated with an endocrown restoration. Materials and methods: A total of 75 human molars free of defects, caries history or cracks were selected, then endodontically treated and randomly allocated into 5 groups (N = 15) according to the difference between PCF and CB, as follows: PCF 2 mm above, PCF 1 mm above, PCF leveled, PCF 1 mm below and PCF 2 mm below. Endocrown restorations were made with composite resin (Tetric N-Ceram, shade B3, Ivoclar) in 1.5 mm thickness and luted with a resin cement (Multilink N, Ivoclar) onto the dental elements. Monotonic testing was performed to define the fatigue parameters, and a cyclic fatigue test was used until failure of the assembly. The collected data were submitted to statistical survival analysis (Kaplan-Meier followed by Mantel-Cox and Weibull), fractographic analysis and finite element analysis (FEA) were performed as complementary analyzes. Results: The PCF 2 mm below and PCF 1 mm below groups presented the best results regarding fatigue failure load (FFL) and number of cycles for failure (CFF) (p < 0.05), but presented no difference between each other (p > 0.05). The PCF leveled and PCF 1 mm above groups presented no statistical difference between them (p > 0.05), but performed better than the PCF 2 mm above group (p < 0.05). The rate of favorable failures of PCF 2 mm above, PCF 1 mm above, PCF leveled, PCF 1 mm below and PCF 2 mm below groups were 91.7%, 100%, 75%, 66.7% and 41.7%, respectively. FEA showed different stress magnitudes according to the pulp-chamber design. Conclusion: The insertion level of the dental element to be rehabilitated with an endocrown interferes in the mechanical fatigue performance of the set. The discrepancy between the CB height and the PCF has a direct effect, where the higher the PCF in relation to the CB, the greater the risk of mechanical failure of the restored dental element.

Statement of problem Various strategies have been proposed to reduce the cement space of foundation restorations for endodontically treated teeth. However, they may add more operative steps, or the dentist must keep different sizes of drills and posts in stock. A 2-piece universal adjustable post system has been developed to overcome this problem, but whether the system has acceptable fatigue survival performance is unclear. Purpose The purpose of this in vitro study was to evaluate the fatigue survival and stress distribution of endodontically treated teeth without a ferrule and restored with different glass fiber post strategies versus a recently introduced universal 2-piece fiber post system. Material and methods Bovine incisor roots were randomly assigned to 3 groups as per the post used (n=13): adapted glass fiber post with post space preparation of the same size, composite resin-custom glass fiber post (CTM), and universal 2-piece glass fiber-reinforced composite resin post (UNI). The posts were adhesively luted, the composite resin core was added, and a composite resin crown was produced with computer-aided design and computer-aided manufacturing (CAD-CAM), and then adhesively luted to each core. A fatigue test was performed with the stepwise stress method (10 000 cycles/step; 20 Hz; load=100 N to 750 N; step=50 N) until fracture, and the failure mode analyzed. The stress distribution was evaluated by finite element analysis with the maximum principal stress criteria by following the parameters of the in vitro test. The solids were considered homogeneous, linear, and isotropic, except for the glass fiber post (orthotropic), and a load of 450 N at 30 degrees was applied. The fatigue failure load and the number of cycles for failure were analyzed with Kaplan-Meier and Mantel-Cox (log rank test) (α=.05). The finite element analysis results were analyzed with colorimetric graphs. Results The highest fatigue failure load and the number of cycles for failure were found in the UNI system, whereas the lowest results were found in the CTM group. All groups exhibited repairable failures. The finite element analysis showed the lowest stress in root dentin in the UNI system. The CTM system had the largest stress regions at the dentin and dentin-core interface. Conclusions The use of a 2-piece universal glass fiber post system resulted in more fatigue behavior compared with composite resin-custom glass fiber posts.

To map the existing evidence regarding the masking ability of resin composites.The literature search was conducted electronically, based on the PRISMA Extension for Scoping Reviews-online protocol at https://osf.io/m2h67/ with no language or time restrictions. Two independent reviewers conducted the screening, and a third reviewer was consulted in case of disagreement. Studies that evaluated resin composite masking ability regardless of background, application technique, thickness, or number of layers were selected. The search found 2995 potentially eligible studies. After removal of duplicates (657), irrelevant articles (2323), 15 citations met the eligibility criteria based on title and abstract, and eight studies were included based on full text analysis (seven in vitro, and one case report). Acceptable masking ability is obtained by one layer of opaque shade resin composite or by the layering technique. A black background is masked with 1.0- to 2.0 mm-thick layers of opaque shade resin composites. Masking of the C4 background is achieved with one layer of 0.5- to 1.5 mm-thick opaque shade resin composite or by the layering technique using different combinations of enamel body and dentin shades with a final thickness of 1.5 mm.Acceptable masking of C4 shade background is achieved with one layer of opaque shade composite at least 0.5 mm-thick or by different combinations of the layering technique, with a final thickness of 1.5 mm. Acceptable masking of the black background of the oral cavity is achieved with a resin composite of at least 1.0 mm opaque shade.The thickness of the resin composite layer required to achieve adequate masking is variable and depends on the translucency/opacity of the tested resin composites and the background shade.

The present study aimed to determine the influence of adhesive luting on the fatigue mechanical properties of simplified lithium disilicate crowns luted to dentin analogues preparations, and to compare two different approaches of simulation of a non-adhesive luting procedure. To this end, 30 prosthetic preparations of glass fiber-filled epoxy resin used as dentin analogues were milled and lithium disilicate crowns were pressed (1.5 mm thickness), which were allocated into three different groups: resin cement (RC), resin cement isolated by paraffin oil (RCI) and zinc phosphate cement (ZP). For luting procedures, the ceramic crowns of the RC and RCI groups had their internal surface treated with 5% hydrofluoric acid and universal primer. Afterwards, a thin layer of paraffin oil was applied onto the ceramic crown intaglio surface in the RCI group. The dentin analogues were treated with 10% hydrofluoric acid, followed by primer application and luting as recommended by the manufacturer. No previous surface treatments were performed for the ZP group, and the cement was applied as recommended. The fatigue test was run under cyclic fatigue (load 400–2000 N, step-size 100 N, 15,000 cycles/step, frequency 20 Hz) until failure detection (radial crack) by transillumination. The fatigue failure load (FFL), number of cycles until failure (CFF), and survival rate in each testing step were recorded. The RC group presented a higher FFL, CFF and survival rate compared to the other groups (p < 0.001), which were statistically equal to each other (p > 0.05). Weibull analysis detected no difference between groups for the Weibull module. Therefore, loss of adhesion between the evaluated substrates induces a significant reduction in load-bearing capacity under fatigue of the lithium disilicate crowns, strengthening the strong-importance of long-lasting adhesion for crown's survival. In addition, different methods for the simulation of non-adhesive conditions induce the same performance in the explored outcomes, confirming that both are valid approaches for laboratory studies.

Objectives: To evaluate the effect of in-lab simulation of CAD/CAM grinding and intaglio surface treatments on the surface characteristics (topography and roughness) and fatigue behavior of adhesively luted 4YSZ simplified restorations. Methods: Ceramic discs (Ø = 10 mm, thickness = 1 mm) were randomly allocated into 6 groups considering: “In-lab simulation of CAD/CAM grinding” (ground or polished) and “intaglio surface treatments”: Ctrl (without surface treatment), AlOx (aluminum oxide air abrasion) or GLZ (glaze spray application). The surface roughness of all samples was measured, the treated discs received a ceramic primer, were luted with resin cement onto a dentin analogue material (woven glass-reinforced epoxy resin) and tested under a cyclic fatigue test (step-stress approach, n = 15; 1.4 Hz, 10,000 cycles/step, step-size of 100N starting at 200N until failure). A complementary analysis was performed to corroborate the findings in the fatigue test that the glaze fill defects increase the mechanical properties of the ceramic. To do so, bars (n= 10; 1.0 × 1.0 × 12 mm; considering the groups: N-ID: non-indented; ID: indented; ID-GLZ: indented plus glaze spray application) were indented in a vickers hardness tester to produce a crack pattern, treated with glaze or not, and then submitted to flexural strength tests (FS). Fractographic and topographic analysis were performed. Results: In-lab simulation of CAD/CAM grinding decreased the fatigue failure load of the 4YSZ ceramic when comparing polished and ground groups, regardless of surface treatment. GLZ induced better fatigue performance compared to the air abrasion, regardless of the grinding condition (ground or polished surface). The results of the flexural strength test corroborated the findings in the fatigue test, as the ID-GLZ group presented superior FS than the ID group, however both had inferior FS than N-ID. There is an inverse association between roughness and fatigue failure load, as the higher the surface roughness, the lower the fatigue failure load. Failures in the fatigue and flexural strength tests started from the face subjected to tensile stresses. Conclusion: In-lab simulation of CAD/CAM grinding had a detrimental effect on the fatigue behavior of 4YSZ and glaze spray induced better 4YSZ performance compared to the air abrasion. The intaglio surface treatments differently influenced the 4YSZ fatigue performance, however, only glaze spray can reverse the damage caused by the grinding.

To evaluate the longevity of metal-ceramic single crowns cemented onto resin composite prosthetic cores using a self-adhesive resin cement in a prospective clinical descriptive study.A total of 152 teeth were endodontically treated and received resin composite prosthetic cores and metal-ceramic crowns cemented with a self-adhesive resin cement. The patients included in the sample were recalled for clinical and radiography evaluation in an up-to-106-month period after the final cementation procedures, with an average of 62 months of follow-up. 91.5% of the sample (142 teeth) were evaluated regarding the treatment survival rate, analyzed considering the loss of crown retention (crown debonding) and tooth loss as the primary outcome. In addition, post debonding, and root fracture occurrences were also recorded as secondary outcomes to evaluate the success rate of the prosthetic treatment. The aesthetic parameters were also evaluated according to the FDI criteria. The Kaplan-Meier method and Cox regression with 95% confidence interval were applied for the statistical analysis.Regarding the primary outcome, the metal-ceramic crowns cemented with self-adhesive resin cement presented a high survival rate (91.5%), with 8 crown debondings and 3 tooth losses (1 due to caries and 2 due to periodontal disease) occurring after the evaluation period. For secondary outcomes, 9 root fractures and 4 post debondings occurred, generating a success rate of 72%. All crowns had a score 1 on the FDI criteria, indicating that they were clinically excellent or very good regarding the aesthetic parameters.The metal-ceramic crowns luted with a self-adhesive resin cement presented a survival rate of 91.5% after an average of 62 months of follow-up. Furthermore, the restorations remained aesthetically satisfactory over time, without changes that would indicate prosthetic retreatment. A success rate of 72% was obtained considering the secondary outcome, mainly related to intraradicular retainer failures (root fractures or post debonding).The self-adhesive resin cement is clinically indicated for cementation of metal-ceramic crowns onto resin composite prosthetic cores.

To assess the effects of a resin cement in high and low viscosity and distinct conditioning of the intaglio surface of lithium disilicate glass-ceramic crowns on fatigue performance of the crowns.Prosthetic preparations (full-crown) in resin epoxy and crowns in lithium disilicate glass-ceramic were machined and allocated considering 2 factors (n = 10): "surface treatment" (HF - 5% hydrofluoric acid etching, followed by silane application; or EP-self-etching ceramic primer) and "resin cement" (high or low viscosity). The preparations were etched with 10% hydrofluoric acid and an adhesive was applied. The intaglio surfaces of the ceramic crowns were treated as aforementioned (HF or EP) and luted with high or low viscosity. The bonded sets were subjected to fatigue testing (step-stress approach: initial load of 200 N, step-size of 50 N, 10,000 cycles/step, 20 Hz) and complementary analyses (fractographic, topographic, and cross-sectional bonded interfacial zone analyses) were performed.Treatment with HF and silane with high viscosity resin cement (955 N/156,000 cycles) and EP with low viscosity resin cement (1090 N/183,000 cycles) showed the best fatigue performance (statistical similarity between them). The failures originated from defects of the cement-ceramic interface, and the HF treatment induced a more pronounced topographical alteration.Distinct topographical patterns from the HF and EP treatments induced better fatigue results for the specific viscosity of the resin cement. Therefore, the fatigue performance depended on the existing topography, type of intaglio surface's defects/irregularities after surface treatment, and how the luting agent filled the irregularities.

This study aimed to investigate the effect of surface roughness (polished vs. CAD/CAM milling simulation) on impact strength of five dental ceramics for manufacturing CAD/CAM monolithic restorations. Specimens of five ceramics (FC- feldspathic glass-ceramic; PICN- polymer-infiltrated ceramic-network; ZLS- zirconia-reinforced lithium silicate glass-ceramic; LD- lithium disilicate glass-ceramic; YZ- yttria-stabilized tetragonal zirconia polycrystal ceramic) to be tested under impact (15×10×2mm3; n= 15) were divided into two groups, according to surface treatment: polishing (pol) and grinding (gri) as CAD/CAM milling simulation. Impact strength was tested using the Dynstat method. Roughness, topographic, fractographic and finite element analyses were performed. © 2021 Wiley Periodicals LLC.Objective: To evaluate the effect of surface treatments on optical, colorimetric, and surface characteristics of lithium disilicate glass–ceramic. Materials and Methods: Specimens (n = 5, IPS e.max CAD) were randomly allocated to the following treatments: mirror-polished: SiC papers; as-cut: mimicking CAD-CAM milling; ground: 90-120 μm-grit diamond bur; ground polished: ground, finished (46-30 μm-grit diamond bur), polished (diamond cups, brush and diamond paste); ground glazed: ground, glazed; ground polished glazed: association of methods. CIELAB color coordinates were obtained by a spectrophotometer. CIEDE2000 color differences (ΔE00) and the translucency parameter (TP00) were calculated. Light transmittance was assessed with a colorimeter. Surface characteristics (topography and roughness) were analyzed. Statistical differences for each condition and outcome were detected using one-way ANOVA with Tukey's post-hoc test (α = 0.05). Results: TP00 data show statistical reduction after grinding (p < 0.05), which was only restored with polishing (solely or with glazing). ΔE00 shows that grinding results in perceptible variations in color (above 0.81), which were restored after all post-processing protocols (exception to only glaze application in contact with a black background). Light transmittance data corroborated such performance. Polishing and glazing reduced roughness and improved surface topography. Conclusion: Grinding statistically increased roughness, reduced translucency, light transmittance through the ceramic, and resulted on color differences. On contrary, polishing (followed or not by glazing) reduced roughness and enhanced ceramic translucency and light transmittance. Glaze also reduced roughness, but it still presented reduced translucency. The positioning (facing up or down) of the ceramic treated surface influenced the considered outcomes. Clinical Significance: Grinding with diamond burs results in a deleterious impact to the optical, colorimetric and surface characteristics of lithium disilicate ceramic. Thus, polishing (followed or not by glazing) is recommended for optical and topographical enhancements when lithium disilicate monolithic restorations require occlusal adjustments.

Objective: To evaluate the fatigue behavior of lithium disilicate crowns with a simplified anatomy against progressive cement/ceramic debonding scenarios. Materials and methods: Lithium disilicate crowns were fabricated via CAD/CAM and luted onto a dentin analogue material using resin cement following the manufacturer's instructions. Then, the different crown regions were isolated with paraffin oil for the absence of chemical adhesion according to four experimental groups (n = 15): Shoulder; Shoulder + Axial; Fully isolated; and Control (no insulation/fully bonded). Load to failure tests (n = 3) were run to determine cyclic fatigue parameters, and the specimens were subsequently submitted to a cyclic fatigue test (n = 12) (initial load 200 N for 5000 cycles, step 100 N, 15,000 cycles/step, frequency 20 Hz) until cracks were observed, and later fracture. The data were analyzed by Kaplan-Meier + Mantel-Cox post-hoc tests for both outcomes (cracks and fracture). Fractographic, cross-sectional surface, and finite element (FEA) analyzes were performed. Results: When it comes to crack occurrence when the chemical adhesion to the occlusal surface is compromised, there is worsening (p < 0.05) in fatigue behavior compared to groups where the occlusal portion of the crown is still bonded. Considering fracture occurrence, there was no difference (p > 0.05) among the tested groups. All cracks occurred in the occlusal portion, first as a radial crack at the ceramic intaglio surface, and posteriorly unleashing a Hertzian cone crack at the top surface, resulting in fractures on the frontal walls. The interface analysis showed no interference of the insulating agent. FEA showed that as the isolated areas increased, there was also an increase in both tensile and shear stresses concentration in the crown and in the cement layer. Conclusion: The chemical adhesion between cement and ceramic is essential for better fatigue behavior of lithium disilicate crowns with a simplified anatomy, especially in the occlusal portion, but the restoration performance is impaired when such adhesion is compromised. There is an increase in crown and cement stress concentration with the progressive loss of chemical bonding of the crown's walls.

The aim of the study was to evaluate whether the region (internal or external) of the CAD/CAM block influences the mechanical performance of restorative materials. Blocks of different CAD/CAM materials (Composites: KAV – Katana Avencia; LULT – LULT Ultimate; Ceramics: ENAM – Vita Enamic; NICE - N!CE; EMP – IPS Empress CAD; VMII – Vita Mark II; EMAX – IPS e.max CAD) were selected, and direct resin composite blocks (APX - Clearfil AP-X; FSUP - Filtek Supreme) were built using the incremental technique on a mold from one of the CAD/CAM blocks. All blocks were sectioned into bar-shaped specimens (1 × 1 × 14 mm), which were separated into two groups according to the region of the block (inside or outside). 3-point bending tests were performed following ISO 6872:2015 and flexural strength data were analyzed using one-way ANOVA and Bonferroni post-hoc tests (p < 0.05). Weibull module and characteristic strength were also calculated. In general, the mean flexural strength values were not statistically different (p > 0.05) between the inside and outside regions of each material, except for LULT (inside > outside) and EMAX (outside > inside). Comparing the materials, a similar strength (only discrete variations) was observed for inside, outside or merged (inside and outside data assembled together) regions: EMAX > KAV = LULT > NICE = APX > EMP = ENAM = VMII ≥ FSUP (merged condition). Characteristic strength at 63.2% failure probability and at 5% failure probability generally corroborates such observations. There were no statistical differences for Weibull module data (inside, outside, or merged), except for KAV and NICE (outside > inside). Thus, the region from where the restoration is milled within the CAD/CAM block generally does not influence the mechanical performance hereof (flexural strength, failure probability, risk of premature failures, and mechanical reliability) of the material, except for LULT and EMAX.

Zirconia has been reported to be suitable for multiunit monolithic restorations. However, as the connector region is the weakest part of the system, studies are needed to determine the optimal connector geometry.The purpose of this in vitro study was to evaluate the load-bearing capacity under fatigue of implant-supported fixed partial prostheses made of monolithic zirconia with different connector cross-sectional geometries.Three-unit monolithic fixed partial prostheses (from mandibular second premolar to the second molar) were fabricated in zirconia (Zenostar T) by computer-aided design and computer-aided manufacture (CAD-CAM). Different connector cross-sectional geometries, with an area of 9 mmThe round (1065 N) and trapezoid (1010 N) groups presented higher FFL values than the square one (870 N) (P≤.05). For CFF, the round connector (358 777) was better than the square (280 310) (P=.006), and the trapezoid (337 773) was statistically similar to both (P.05). No difference among groups was observed in Weibull modulus either for CFF or FFL data. All failures were catastrophic, originating at the base of the connector and propagating toward the occlusal surface of the pontic (region under loading).The connector cross-sectional geometry significantly influenced the mechanical fatigue performance of implant-supported fixed partial prostheses made of monolithic zirconia.

This study compared the flexural strength under monotonic (static - sσ) and cyclic load application (fatigue - fσ), hardness (H) and fracture toughness (KIC) of different layers of a multi-layered zirconia (IPS e.max ZirCAD MT Multi, Ivoclar). Each layer was sectioned, classified into three groups according to yttria content (4-YSZ, 4/5-YSZ and 5-YSZ), and shaped on samples for flexural strength and fracture toughness tests (bars: 1.0 × 1.0 × 11 mm); and Vickers hardness test (plates: 1.5 × 4.0 × 5.0 mm). Flexural strength under monotonic load application (sσ; n = 10) was obtained through two different devices (three-point-bending and ball-in-hole device) and fatigue flexural strength (fσ; n = 15; initial load: 10 N; step-size: 5 N; 10,000 cycles/step) was assessed using a ball-in-hole device under cyclic load application. Vickers hardness test (n = 5), fracture toughness test (n = 10), and additional analyzes (Finite Element Analysis - FEA, Energy-dispersive X-ray spectroscopy - EDS and Scanning Electron Microscopy - SEM) were also performed. No differences were found between the different devices in the monotonic flexural strength test, and FEA showed similar tensile stress distribution for the two devices. 4-YSZ showed higher values of flexural strength under monotonic and cyclic load application modes (sσ = 1114.73 MPa; fσ = 798.84 MPa), and fracture toughness (KIC = 3.90 MPa√m). 4/5-YSZ had an intermediate sσ; however, fσ was similar to 5-YSZ (404.00–429.36 MPa) and KIC similar to 4-YSZ (KIC = 3.66 MPa√m). No statistical differences were found for hardness and Weibull modulus for fatigue flexural strength data. The amount of yttria in the layer compositions increased from 4-YSZ to 5-YSZ, and larger zirconia crystals were observed in the topographic images of 5-YSZ. Failures in the flexural strength and toughness tests started from the face subjected to tensile stress. Different layers of the multi-layered zirconia blank presented distinct mechanical properties. 4-YSZ (cervical layer) presented the highest flexural strength under monotonic and cyclic loads (fatigue), and higher fracture toughness even similar to the transition layer (4/5-YSZ). Hardness was similar between the layers. The ball-in-hole device performed similarly to the three-point bending device and can be used as an alternative to the traditional method.

Objective: To characterize the effect of pigmentation techniques on the fatigue behavior of a 4YSZ ceramic adhesively luted onto dentin analog (glass fiber-reinforced epoxy resin). Methods: 4YSZ ceramic discs (Ø= 10 mm, 1 mm thickness) were allocated into 5 groups: Ctrl – unshaded (IPS e.max ZirCAD MT BL); Manuf – shaded by the manufacturer (IPS e.max ZirCAD MT A2); Brush – unshaded pigmented, pre-sintering, using a brush; Stain – unshaded pigmented, post-sintering, during glaze application; Brush+Stain – combination of both techniques. Color assessments ensured the same perceived color (Vita Classical A2). Dentin analog discs (Ø= 10 mm, 2.5 mm thickness) were obtained, paired with the 4YSZ discs, and adhesively luted using a resin cement. Cyclic fatigue testing (n = 15) was run (20 Hz; 10,000 cycles/step, initial load 200 N; step-size 100 N up to 700 N; then 50 N until specimen failure). Fractographic, roughness and topography analyses were performed. Results: No statistical (p > 0.05) detrimental influence were observed for fatigue outcomes (Ctrl = Manuf = Brush = Stain = Brush+Stain). However, the Brush+Stain technique induced inferior statistical (p < 0.05) fatigue performance compared to Stain. All failures were radial cracks with origin at the ceramic intaglio surface. The staining technique triggered statistically higher roughness (p < 0.05). Uniformly sized zirconia crystals were seen in the Ctrl, Manuf and Brush groups, and a smooth vitreous surface with encrusted pigments in the Stain and Brush+Stain groups. Significance: Pigmentation techniques (Vita Classical A2 shade) have no detrimental effect on the mechanical fatigue properties of 4YSZ ceramic adhesively luted onto dentin analog.

Biological complications of implant-supported prostheses remain a concern. Therefore, standardizing hygiene protocols to promote their maintenance is important.The purpose of this scoping review was to identify available hygiene guidance for home care procedures, as well as periodicity and protocols for the professional maintenance of implant-supported prostheses.This study was based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, and the study protocol was made available at: https://osf.io/5jdeh/. The search was last performed in the PubMed database on March 15, 2022 and was undertaken by 2 independent researchers. Clinical studies and reviews that evaluated home care and maintenance protocols for dental implant-supported prostheses (fixed or removable), without language restriction or year of publication, were included. A descriptive analysis was performed considering study characteristics (study design, restorative assembly, maintenance strategies, eligibility criteria, number of included studies, quality/level of evidence, and main findings).The initial search yielded 3138 studies, of which 18 were included for descriptive analysis (6 critical reviews, 4 clinical trials, 3 systematic reviews, 2 guideline reports, 2 retrospective studies, and 1 transversal study). The main instrument recommended for home care was the use of a conventional toothbrush associated with triclosan-containing toothpaste in addition to interproximal aids (brushes or floss) for all types of restorations. The use of irrigation instruments was also frequently considered. For professional maintenance, almost all studies reported a positive effect of regular recalls every 3 months during the first year, followed by less regular recalls according to the motivation of the patient and home care efficacy. At each recall, the professional should evaluate the patient history, oral tissues, implant, abutments, and restorations, as well as perform professional cleaning of the prostheses with appropriate instruments so that the implants and abutments may be preserved.Establishing an adequate hygiene protocol is indispensable for implant-supported restorations. To do so, the professional must guide and support the patient's home care by taking into account the patient's motivation and efficacy during the hygiene procedures. Also, for professional maintenance, the periodicity of recalls should be constant and include an evaluation of the condition of the restoration and adjacent tissues, followed by professional cleaning of prostheses, implants, and abutments, in addition to new instructions to improve patient home care.

Objectives: To evaluate longevity of metal-ceramic single crowns cemented using a self-adhesive resin cement in a prospective clinical analytical study. Methods: A total of 152 teeth received resin composite prosthetic cores and metal-ceramic crowns cemented with a self-adhesive resin cement. The patients included in the sample were recalled for clinical and radiography evaluation in an up-to-106-month period after the final cementation procedures, with an average of 62 months of follow-up. Next, 93% of the sample (142 teeth) were evaluated regarding the treatment survival rate, analysed considering the loss of crown retention (crown debonding) as the primary outcome. In addition, post debonding, root fracture and tooth loss occurrences were also recorded as secondary outcomes to evaluate the success rate of the prosthetic treatment. The aesthetic parameters were also evaluated according to the FDI criteria. The Kaplan-Meier method and Cox regression, with 95% confidence interval was applied for the statistical analysis. Results: Regarding the primary outcome, the metal-ceramic crowns cemented with self-adhesive resin cement presented a high survival rate (93%), with 8 crown debondings occurring after the evaluation period. For secondary outcomes, 9 root fractures, 4 post debondings and 3 tooth losses (1 due to caries and 2 due to periodontal disease) occurred, generating a success rate of 72%. All crowns had a score 1 on the FDI criteria, indicating that they were clinically excellent or very good regarding the aesthetic parameters.Conclusion: The metal-ceramic crowns luted with a self-adhesive resin cement presented a survival rate of 93% after an average follow-up of 62 months. Furthermore, the restorations remained aesthetically satisfactory over time, without changes that would indicate prosthetic retreatment. A success rate of 72% was obtained considering the secondary outcome, mainly related to intraradicular retainer failures (root fractures or post debonding). Clinical Significance: The self-adhesive resin cement is clinically indicated for cementation of metal-ceramic crowns.

To identify and discuss the available surface treatments and adhesives for polyetheretherketone (PEEK) to increase its bond strength to resin-based materials used in dentistry.The reporting of this scoping review was based on PRISMA. The study protocol was made available at: https://osf.io/4nur9/. Studies which evaluated PEEK surface treatments and its bond strength to resin-based materials were selected. The search was performed in PubMed, Scopus, Web of Sciences and Cochrane databases. The screening was undertaken by 3 independent researchers using the Rayyan program. A descriptive analysis was performed considering study characteristics and main findings (title, data of publication, authors, PEEK characteristics, surface treatments, control group, bonded set, luting agent, specimen geometry, storage, thermocycling, pre-test failures, test geometry, failure analysis, main findings, and compliance with normative guidelines).The initial search yielded 1965 articles, of which 32 were included for descriptive analysis. The review showed that the use of surface treatments and adhesives are important to promote bond strength to PEEK. Up until now, various surface treatments have been explored for bond improvement to PEEK. Sulfuric acid etching is commonly reported as promoting the highest bond strength, followed by alumina-particle air abrasion. Regarding adhesives, the use of a specific adhesive containing MMA, PETIA (pentaerythritol triacrylate), and dimethacrylates yields the best adhesive performance.Sulfuric acid etching and alumina particle air abrasion followed by application of bonding agents containing MMA, PETIA and dimethacrylates are the most effective choices to increase resin-based materials' adhesion to PEEK.

This study aims to characterize the effect of pre- and/or post-sintering pigmentation techniques on the fatigue behavior of a 4YSZ ceramic (4 mol% yttrium stabilized zirconia). First, 4YSZ ceramic discs (IPS e.max ZirCAD, 15 mm diameter, and 1.2 mm thickness) were obtained and allocated according to the 'pigmentation technique' factor into 5 groups: Ctrl - unshaded ceramic disc (IPS e.max ZirCAD BL); Manuf - ceramic discs shaded by the manufacturer at block formation (IPS e.max ZirCAD MT A2); Brush - unshaded ceramic disc pigmented in the pre-sintered stage using a brush; Stain - unshaded ceramic disc pigmented in the post-sintering stage using a stain glaze (shade A2); Brush + Stain - combination of Brush and Stain techniques. Objectives To evaluate different direct restoration techniques on various cavity designs in anterior endodontically treated teeth (ETT). Materials and methods Ninety upper central incisors (n = 90) were selected, endodontically treated, and divided into three groups (n = 30) accordingly to the cavity design: minimal endodontic cavity access (group A), endodontic access + mesial class III cavity (group B), and endodontic access + two class III cavities (group C). Three subgroups (n = 10) were then created accordingly to the restoration technique: nano hybrid composite restoration (subgroup a), glass fiber post + dual-cure luting cement (subgroup b), and bundled glass fiber + dual-cure luting cement (subgroup c). Samples underwent micro-CT scan, chewing simulation, and a second micro-CT scan. 3D quantification (mm3) of interfacial gap progression was performed; then, samples underwent fracture resistance test. Data were statistically analyzed setting significance at p < 0.05. Results Groups A and B showed significantly lower interfacial gap progression compared with group C. Subgroup b performed significantly better compared with subgroups a and c. Improved fracture strength was reported for group C compared with group A, while both subgroups b and c performed better than subgroup a. Conclusions Cavity design significantly influenced interfacial gap progression and fracture resistance. Fiber posts significantly lowered gap progression and improved fracture resistance while bundled fibers only increased fracture resistance. A significant reduction of non-repairable fractures was recorded when fibers were applied. Clinical relevance A minimally invasive approach, conserving marginal crests, should be applied whenever possible. Inserting a fiber post is indicated when restoring anterior ETT, in order to reduce gap progression, improve fracture resistance, and avoid catastrophic failures.

Objective This study aimed to assess the characteristics of different designs of systematic reviews (SRs) registered in the International Prospective Register of Systematic Reviews (PROSPERO) about COVID‐19. Methods The search was performed in the PROSPERO database using the strategy proposed by the database and considered only human studies. The last date of the search was April 27, 2020. Full text of all records was accessed, and data were extracted by a single researcher, which was further double‐checked by another researcher. A descriptive analysis was performed considering record characteristics using tables. Results We included 564 records from which the vast majority were registered as SRs (n = 513, 91%). In general, we found poor reporting and missing or confusing information, since 84% of the records (n = 474) did not report the full search that would be adopted, 16% (n = 90) did not report clearly the databases that would be used, and 49.1% (n = 277) did not report the number of primary outcomes. The main focus of most of the records involved clinical, epidemiological, complication, and laboratory characteristics (n = 173, 30.7%) or the treatment of COVID‐19 (n = 138, 24.5%). Conclusion A large number of SRs about COVID‐19 have been conducted, and many of the assessed records were poorly reported and would be difficult to replicate. Besides, collected data points to an epidemic of redundant reviews on COVID‐19.

This study aimed to evaluate the influence of ceramic surface treatments, resin cement viscosities, and storage regimens on the fatigue performance of bonded glass-ceramics (lithium disilicate, LD; feldspathic, FEL). Ceramic discs (Ø = 10 mm; thickness = 1.5 mm) were allocated into eight groups per ceramic (n = 15), considering three factors: “ceramic surface treatment” in two levels – 5% hydrofluoric acid etching and silane-based coupling agent application (HF), or self-etching ceramic primer (E&P); “resin cement viscosity” in two levels – in high or low viscosity; and “storage regimen” in two levels – baseline, 24 h to 5 days; or aging, 180 days + 25,000 thermal cycles. Adhesive luting was performed onto glass fiber-reinforced epoxy resin discs (Ø = 10 mm; thickness = 2 mm) and the bonded assemblies were subjected to cyclic fatigue tests: initial load = 200 N; step-size = 25 N (FEL) and 50 N (LD); 10,000 cycles/step; 20 Hz. Scanning electron microscopy (SEM) inspections were performed. Regarding the LD ceramic, the fatigue behavior was reduced after aging for HF_HIGH and E&P_LOW conditions, while stable performance was observed for HF_LOW and E&P_HIGH. Regarding the FEL results, aging negatively affected HF_HIGH, E&P_HIGH, and E&P_LOW, being that only the HF_LOW condition presented a stable behavior. The failure initiated from defects on the etched surface of the ceramics, where the cross-sectional analysis commonly revealed unfilled areas. Long-term aging might induce a decrease in mechanical behavior. The ‘ceramic microstructure/surface conditioning/resin cement viscosity relationships’ modulate the fatigue performance of lithium disilicate and feldspathic glass-ceramics.

The characteristics of dental ceramics have been extensively studied over the years to provide highly qualified materials for use in prosthetic restorations. The ability to adhere to dental substrates, outstanding aesthetics (translucency, color, and substrate masking ability) and improved mechanical properties provide these materials with optical features and high strength to withstand masticatory stimuli. Different classifications are adopted, and it is generally considered that glass-ceramics have better optical characteristics due to the high glass content, and polycrystalline ceramics have superior strength favored by their densified and organized crystals, hampering crack growth. This knowledge was largely built-up during years of scientific research through different testing methodologies, but mainly employing static loads. It is important to not only take into account the intensity of loads that these materials will be exposed to, but also the effect of the intermittence of cyclic load application leading to mechanical fatigue and the influence of factors related to the crack origin and their propagation under this condition. Furthermore, the bonding surface of ceramic restorations requires surface treatments that improve the bond strength to luting agents; however, these treatments require caution because of their potential to produce defects and affect the structural behavior. Moreover, ceramic restorations often require internal adjustments for proper seating or external adjustments for fitting the occlusal contact with the antagonist. In this sense, finishing/polishing protocols may alter the defect population, as luting agents may also interact by filling in the superficial defects on the restoration intaglio surface. Thus, the balance among all these factors will define the performance of a restorative setup, as well as the posterior exposure to the humid environment and the masticatory stimuli (cyclical loading), which may favor developing slow and subcritical growth of cracks in ceramic materials and the degradation of the bond interface. Therefore, it is essential that the concepts which explain the fatigue mechanism are understood, as well as the crack propagation and failure patterns of restorative ceramic materials.

Objectives To evaluate the effect of different artificial aging methods on the bond strength of a resin composite associated with a universal adhesive (Scotchbond Universal) used under two etching approaches (self-etch [SE] or etch-and-rinse [ER]) to enamel and dentin substrates. Materials and Methods A total of 96 noncarious human third molars were prepared and randomly divided according to three factors (n = 6): substrate (enamel and dentin), adhesive approach (SE and ER), and aging method (water storage for 24 hours, 6 months, or 1 year; subjected to 10,000, 20,000, or 30,000 thermal cycles; and sodium hypochlorite [NaOCl] storage for 1 or 5 hours). Statistical Analysis Microshear bond strength tests were conducted, and the collected data (MPa) were subjected to three-way analysis of variance (ANOVA) with post hoc Bonferroni tests (p < 0.05) and Weibull analysis. The failure pattern was also evaluated. Results Three-way ANOVA revealed that the factors “substrate” (p = 0.00) and “aging method” (p = 0.00) had a significant effect on the bond strength, but the factor “adhesive approach” did not (p = 0.84). The bond strength in the enamel group for the SE approach was negatively affected under 20,000 and 30,000 thermal cycles. Weibull presented the highest m in the NaOCl storage for the 5 hours group to enamel using the SE and to dentin using ER approaches. Adhesive/mixed failures were predominant for all groups. Conclusion Thermocycling aging (20,000 and 30,000 cycles) significantly reduced the bond strength to enamel using the SE approach. On the contrary, storage with the NaOCl method proved to increase bond strength under the evaluated conditions.

Purpose The aim of this study was to inspect the mechanical fatigue behavior of an implant-supported restorative system using polyether ether ketone (PEEK) and yttria partially stabilized zirconia polycrystals (YZ) as materials for customized definitive implant-supported hybrid abutments, supporting two types of all-ceramic restorations: translucent zirconia (TZ) and lithium disilicate (LD) monolithic crowns. Materials and methods Forty Morse taper implants were included in epoxy resin. Titanium intermediary abutments were placed, and the specimens were randomly allocated into four groups (n = 10) according to the customized hybrid abutment material (PEEK or YZ) and the monolithic crowns (TZ or LD) representing a maxillary central incisor crown. The specimens were subjected to a mechanical fatigue test (step-stress analysis) by means of an initial 200-N load for 5,000 cycles and subsequent increase of 50 N (step-size) at each 10,000 cycles, until failure occurred. The load at failure and number of cycles until failure were recorded; survival probabilities and specimen displacement were calculated for each step. The failure pattern was evaluated, and the Weibull modulus was obtained for each condition. Results Fatigue of both types of crowns was not influenced by the abutment material (LD-PEEK = LD-YZ; TZ-PEEK = TZ-YZ). In the PEEK abutment, the values obtained in the LD and TZ crowns showed no statistical difference; however, in the YZ abutment, the TZ crown presented a load at failure value that was statistically higher than that for LD. Failure pattern analysis revealed a higher prevalence of crown fracture for LD groups, while screw/implant platform fractures were shown for TZ groups. Conclusion YZ and PEEK hybrid abutments promoted similar fatigue levels regardless of the crown materials, TZ crowns promoted a higher fatigue level than LD ones when associated with YZ abutments, and LD crowns promoted a similar fatigue level to TZ ones when associated with PEEK custom abutments. Higher prevalence of crown fractures was shown for LD and screw/implant platform fractures for TZ groups.

This systematic review aims to explore and compile the effect of adhesive luting on the mechanical properties of dental ceramics used as restorative materials.The PubMed/MEDLINE, Web of Science and Scopus databases were searched on January 31st, 2021 to select laboratory studies written in English, without publishing-date restrictions, which compared the mechanical properties of commercially available dental ceramics as restorative materials luted using adhesive vs non-adhesive strategies. A total of 20 (out of 2039) studies were eligible and included in the analysis. Two authors independently selected the studies, extracted the data and assessed the risk of bias. Mean differences (RevMan5.1, random effects model, α = 0.05) were obtained by comparing resistance values of adhesive and non-adhesive conditions (global analysis). Subgroup analyses were performed considering ceramic composition and aging.In the global analysis, adhesive luting induced higher mechanical resistance values compared to non-adhesive luting (p ≤ 0.01). The same effect was observed for glass and alumina ceramics (p ≤ 0.01), but not for zirconia polycrystals (p = 0.83). Adhesive luting was favorable in both the aged and non-aged subgroup analysis (p ≤ 0.01). High heterogeneity was found in all meta-analyses. All analyzed studies in the systematic review scored negatively for risk of bias in most of the factors considered.Adhesive luting reinforces the mechanical properties of dental ceramics used as restorative materials, with the exception of zirconia polycrystals.

Objective: This investigation evaluated the effect of two sintering modes of a translucent zirconia (Y-TZP) on its surface roughness, topography, phase-transformation (t → m), translucency and biaxial flexure fatigue strength. Materials and Methods: To do so, 50 Y-TZP discs (Ø = 15 mm; thickness = 1.2 mm; IPS e.max ZirCAD LT) were prepared and divided into two groups: Standard mode (SM) and Fast mode (FM). Staircase fatigue testing was performed (piston-on-three balls set-up, ISO 6872:2015), as well as surface roughness, profilometry, scanning electron microscopy (SEM-FEG), energy dispersive X-ray spectroscopy (EDX), phase transformation (t → m) using X-ray diffraction analysis (XRD), translucency parameter analysis (TP and TP00) and fractography. Results: The results showed no statistical significant differences for roughness parameters (p > 0.05, SM: Ra = 0.13 ± 0.02, Rz = 1.21 ± 0.26 and RSm = 24.91 ± 2.19; FM: Ra = 0.14 ± 0.03, Rz = 1.32 ± 0.25 and RSm = 24.68 ± 2.16) or flexural fatigue strength (SM: 512 (464–560) MPa; FM: 542 (472–611) MPa) between the groups. In addition, similarity in surface morphological features (SEM and profilometry), composition and phases (EDX and XRD) was observed between the firing protocols. Fractography showed that the failure origin occurred on the tensile side. Sintering mode did not affect the TP (F = 0.001, p = 0.97) and TP00 (F = 0.12, p = 0.72). Conclusions: Therefore, the fast-sintering mode is suggested as a viable alternative to the standard mode since it does not influence the evaluated surface morphology, microstructure, fatigue strength and translucency of a translucent monolithic zirconia. Clinical Significance: The fast sintering mode is a viable alternative for zirconia without compromising its topography, microstructure, mechanical performance or translucency.

This study aims to characterize the effect of shading techniques on the fatigue behavior of a 4YSZ ceramic (4 mol% yttrium stabilized zirconia) adhesively bonded to a dentin analogue (fiber-reinforced epoxy resin). 4YSZ ceramic discs (IPS e.max ZirCAD, Ø = 10 mm and 1 mm of thickness) were allocated according to the factor 'shading technique' into 4 groups: Brush- unshaded ceramic disc (IPS e.max ZirCAD BL) pigmented at the pre-sintered stage with pigment solution applied manually using a round liner brush; Immersion- unshaded ceramic disc pigmented through immersion in the solution for 1 s on only one side of the ceramic disc; Manufacturer- specimens already shaded by the manufacturer (IPS e.max ZirCAD MT A2 - Manufacturer group); Control- a control condition with absence of pigment (i.e. non-pigmented specimens). How best to restore endodontically treated teeth is still unclear because many types of material and techniques are involved.The purpose of this overview of systematic reviews of clinical studies was to assess the available evidence-based literature on the influence of associated clinical factors on the clinical performance (survival, failure rate, or success) of restored endodontically treated teeth.PubMed, Scopus, and Web of Science were searched from inception until September 27, 2021, to identify systematic reviews. Two reviewers independently screened titles, abstracts, and full-text articles, and each reviewer extracted the data of half of the included studies. A single reviewer with an independent verifier completed the quality appraisal. A descriptive analysis of the collected data was made.A total of 36 systematic reviews fulfilled the inclusion criteria. The factors most assessed were the type of coronal restorative technique (n=22) and type of post (n=22). The results indicated that metal and fiber posts seem to present similar clinical performance, endocrowns could be a suitable restorative option, there is no evidence to confirm whether single crowns are better or worse than direct restoration, the maintenance of the coronal structure is a fundamental factor, and the ferrule effect is still a controversial topic. Most of the studies (n=30, 81%) presented a final Grading of Recommendations, Assessment, Development and Evaluations (GRADE) rating of "critically low."The post type does not appear to influence the clinical performance of restored endodontically treated teeth. However, conclusive evidence to suggest how different materials or techniques for restoring coronal tooth structure affect the clinical performance of such restorations, as well as the impact of the ferrule effect, is still lacking.

Dois casos clínicos são apresentados para explorar as diferenças técnicas e discutir as vantagens e desvantagens do uso de zircônia folheada ou monolítica para confeccionar coroas únicas posteriores. O primeiro caso descreve as etapas clínicas na fabricação de uma coroa monolítica em um segundo pré-molar inferior esquerdo usando zircônia de alta translucidez. Mostra a utilização de um preparo dentário conservador baseado nas propriedades mecânicas superiores que este material apresenta, bem como nas características ópticas finais alcançadas pelo sombreamento e coloração. No segundo caso, foi realizado um tratamento restaurador convencional em bicamada com estrutura de zircônia seguida de estratificação com cerâmica de feldspato no primeiro molar inferior esquerdo. A literatura recente indica que cada uma dessas alternativas restauradoras apresenta vantagens e desvantagens específicas. Fatores como desempenho mecânico, fratura, características estéticas, sucesso clínico, taxas de complicações, desempenho de adesão e desgaste do antagonista são discutidos comparando os dois conjuntos restauradores. Os dados destacam que as coroas monolíticas evitam um grande problema relatado nas restaurações de duas camadas: o lascamento da cerâmica de recobrimento. As coroas monolíticas também permitem preparações dentárias minimamente invasivas, aumentando assim a preservação do remanescente dentário. No entanto, faltam dados que sustentem a similaridade do desempenho estético entre as montagens monolítica e bicamada, sendo a previsibilidade de uso restrita para a região posterior, pois casos que demandam alto apelo estético continuam a utilizar fundamentalmente as restaurações bicamada. Não foram encontradas falhas, e a satisfação do paciente foi relatada em ambas as técnicas após o acompanhamento de 12 meses.

OBJECTIVE This study aimed to evaluate whether a master and PhD thesis in Brazilian graduate programs in dentistry can be based on a systematic review (SR). METHODS In this cross-sectional study, we surveyed the coordinators of Brazilian graduate programs in dentistry using a self-administered questionnaire including questions about whether an SR meets requirements for approval of a master or PhD thesis, instructions regarding how to use an SR as the method for a master or PhD thesis, and reasons and barriers that SRs were not recognized as acceptable for a master or PhD thesis. The responses were analyzed descriptively, and tables and graphs were created to summarize the data. RESULTS Among 101 graduate programs invited to participate in the study, 38 (37.6%) returned the questionnaire. Over three-quarters of surveyed programs (n = 30, 78.1%) answered that SRs, in whole or in part, meet the criteria for a master or PhD thesis. Likewise, most programs answered that a master or PhD thesis could be exclusively based on an SR (n = 28, 73.7%). The same number of programs pointed out that one possible reason for not recognizing SRs as a master or PhD thesis is either the lack of SR expertise among committee members (n = 28, 73.7%) or the lack of adequate training of candidates in SR methodology (n = 26, 68.4%). CONCLUSIONS SRs are well accepted as a master or PhD thesis by Brazilian graduate programs in dentistry. Educational initiatives should be encouraged to promote understanding of SR methodology and increase acceptance among graduate programs worldwide.

This study compared the physical properties of conventional and monolithic yttria-zirconia (Yz) materials after low-temperature degradation (LTD). Bar-shaped specimens (2.0 mm thick × 4.0 mm wide × 16.0 mm in length) were produced. Autoclave aging was executed at 134°C under 2 bar pressure for 5 h. The surface characteristics were assessed using a confocal laser microscope, microstructural phase alterations via an X-ray diffractometer (XRD), 3-point flexural strength using a universal testing machine, Vickers microhardness via a digital microhardness tester, and toughness using a durometer. Fractured specimens and indented surfaces was observed via a scanning electron microscope for fractography characterization. All of the data underwent specific statistical tests considering their distribution and homogeneity. Regarding topography, different Yz materials were very similar after LTD. Conventional Yz materials were smoother. The XRD data support that both materials were mainly composed of tetragonal crystals, whereas slight monoclinic peaks were detected. Monolithic Yz showed higher flexural strength than conventional Yz. All of the fractures started on surface defects at the side where tensile stress was concentrated (opposite to load application) and propagated to the opposite side. Vickers microhardness and toughness analysis found that conventional Yz had higher performance than monolithic Yz. This study has found that there are slight differences in strength, hardness, and toughness between conventional and monolithic yttria-stabilized zirconia ceramics, although both materials demonstrate adequate performance for clinical use.

Information regarding the masking ability of ceramic crowns over different implant abutment materials is scarce.The purpose of this in vitro study was to evaluate the masking ability of different monolithic or bilayer ceramic materials with different thicknesses over substrates indicated for implant restorations by using opaque and translucent evaluation pastes.Disk-shaped specimens, shade A1 (VITA Classic; Ø10×1.5 to 2.5 mm), of different ceramics (a bilayer system [yttria-stabilized zirconia infrastructure+porcelain veneer: Zir+Pc] and monolithic systems [lithium disilicate under low, medium, or high translucency: LtLD, MtLD, or HtLD, respectively, and a high-translucent yttria-stabilized zirconia: HtZir]) were made (n=4). The color difference (ΔEThe most predictable masking ability was seen with Zir+Pc regardless of the evaluation paste used. Nevertheless, under 1.5-mm thickness, Zir+Pc did not adequately mask Ti (ΔEMonolithic ceramics should be used with caution over discolored implant abutments. Bilayer systems (Zir+Pc) were the most predictable approach to adequately masking discolored substrates such as PEEK or Ti. An increased restoration thickness provided higher masking ability for all restorative materials tested.

It aims on evaluate the effect of the test environment on static fatigue behavior of lithium disilicate-based (LD), and yttrium oxide-stabilized zirconia (YSZ) ceramics. Specimens of LD (IPS e.max CAD, Ivoclar Vivadent) and YSZ (IPS e.max ZirCAD MO, 3 mol% Y2O3, Ivoclar Vivadent) were randomly allocated into three groups: tested in air, inert (paraffin oil, Sigma Aldrich) or distilled water. The static fatigue test (n=15) was performed using a piston-on-three ball assembly, adapted from ISO 6872, as follows: starting load 100 N for LD and 300 N for YSZ; loading application time set to 1 hour for each loading step; step size of 50 N for LD and 100 N for YSZ, applied successively until fracture. Data from static fatigue strength (MPa) and time to fracture (hours) were recorded. Fractographic analysis was executed. Survival analysis corroborates absence of influence of environment on static fatigue outcomes (fatigue strength, time to fracture and survival rates) for YSZ. For LD, specimens tested in air presented statistically superior survival rate and static fatigue strength (p= 0.025). In regards of time to fracture, LD tested in air were superior than when tested in distilled water (p=0.019) or inert (p=0.017) environments. No statistical differences for Weibull modulus were observed. Failures started on the tensile stress surface. Thus, the test environment did not affect slow crack growth (SCG) mechanisms during static fatigue test of YSZ ceramics, but it plays a significant role for the static fatigue behavior of lithium disilicate-based glass ceramics, indicating a high susceptibility to SCG. Resumo O presente estudo objetivou investigar a influência do ambiente de teste no comportamento a fadiga estática (resistência à fratura e tempo para falha) de cerâmicas à base de dissilicato de lítio (LD) e zircônia estabilizada com ítria (YSZ). Espécimes de LD (IPS e.max CAD, Ivoclar Vivadent) e YSZ (IPS e.max ZirCAD MO, 3mol% Y2O3, Ivoclar Vivadent) foram obtidos e alocados aleatoriamente em três grupos: testado em ar, em líquido inerte (óleo de parafina) ou em água destilada. O teste de fadiga estática (n= 15) foi realizado usando o teste pistão sobre três esferas, ISO 6872, da seguinte forma: carga inicial 100 N para LD e 300 N para YSZ; tempo de aplicação de carregamento definido para 1 hora para cada etapa de carregamento; tamanho do passo de 50 N para LD e 100 N para YSZ, aplicados sucessivamente até a fratura. Dados de resistência à fadiga estática (MPa) e tempo até a fratura (horas) foram registrados. A análise fractográfica foi executada. A análise de sobrevivência corrobora a ausência de influência do ambiente de teste nos resultados de fadiga estática (resistência à fadiga, tempo de fratura e taxas de sobrevivência) para a YSZ. Para LD, os corpos de prova testados em ar apresentaram taxa de sobrevivência e resistência à fadiga estática estatisticamente superiores (p= 0,025). Em relação ao tempo de fratura, os espécimes LD testados em ar foram superiores aos testados em água destilada (p= 0,019) ou em ambiente inerte (p= 0,017). Não foram observadas diferenças estatísticas para o módulo de Weibull. Todas as falhas começaram no lado onde se concentra a tensão de tração (lado de baixo) durante o teste. Com base nisso, observa-se que o ambiente de teste não afetou os mecanismos de crescimento lento de trinca durante o teste de fadiga estática da cerâmica YSZ, mas desempenha um papel significativo na resistência à fadiga estática da cerâmica de vidro à base de dissilicato de lítio, indicando alta susceptibilidade ao crescimento lento e subcrítico de trincas.

To evaluate whether an adhesive application after surface treatment on a lithium disilicate ceramic (LD) has an influence on its load-bearing capacity under fatigue.LD discs (Ø= 10 mm; thickness= 1 mm) were allocated into 8 groups (n= 15), considering 3 factors: "ceramic surface treatment" - HF: hydrofluoric acid + universal primer application; or MEP: single-component ceramic primer; "adhesive application" - with or without; and "aging protocol" - baseline: 24 h to 7 days; or aging: 180 days of storage + 25,000 thermal cycles. The LD discs were adhesively bonded to glass fiber-reinforced epoxy resin discs (Ø= 10 mm; thickness= 2 mm) and stored according to the condition and each group. Cyclic fatigue testing (initial load= 100 N; step size= 100 N until600 N and after step size= 25 N to failure; 10,000 cycles/step; 20 Hz frequency) was performed. Fractographic and adhesive interface analyzes were also performed. The collected data were then analyzed by Kaplan Meier and Mantel-Cox tests and One-way ANOVA.The adhesive application in the baseline condition had no influence on the load-bearing capacity under fatigue when the HF surface treatment was performed, however, adhesive application for the MEP treatment led to worse results than without it. The adhesive application in the aged condition showed worse fatigue outcomes for both treatments. All specimens presented radial cracks. MEP treatment followed by adhesive application presented the thickest luting layer.The adhesive application after surface treatments of a lithium disilicate glass-ceramic is detrimental to its load-bearing capacity under fatigue when adhesively luted onto a supporting substrate.

This study evaluated the influence of ceramic surface conditioning and storage regimen (baseline vs. aging) on the fatigue performance of simplified lithium disilicate glass-ceramic restorations. A total of 90 ceramic discs (O= 10 mm; thickness= 1.0 mm) were allocated into 6 groups (n= 15), considering 2 factors: “ceramic surface treatment” – CA (only silane-based coupling agent, Monobond N), HF (5% hydrofluoric acid etching), or HF+CA (5% HF acid etching plus silane-based coupling agent); and “storage regimen” – baseline (24 hours – 5 days of distilled water at 37 °C), or long-term aging (180 days of distilled water at 37 °C + 25,000 thermal cycles). After intaglio ceramic conditioning, adhesive bonding (Multilink N) was performed onto epoxy resin discs (O= 10 mm; thickness= 2.5 mm) and the bonded sets were subjected to step-stress fatigue tests (initial load: 200 N; step-size: 50 N; 10,000 cycles per step; 20 Hz). Fatigue data were analyzed using Kaplan-Meier and Weibull statistical analyses. Fractography and topography analyses were also conducted. The fatigue findings demonstrated that the performance among groups for both baseline and aging conditions maintained a tendency: the CA groups had the worst behavior (baseline: 893 N/143,667 cycles; aging: 639 N/84,179 cycles), while the surface etching with HF (baseline: 1247 N/214,333 cycles; aging: 816.67 N/128,333 cycles) and HF+CA groups (baseline: 1290 N/222,333 cycles; aging: 900 N/145,000 cycles) had no statistically significant difference between them. The aging protocol reduced the performance of all groups. The groups with better fatigue performance (HF and HF+CA) did not have statistical differences regarding structural reliability (Weibull modulus). Most failures were radial cracks from the cementation interface, except for CA aging specimens, with 27% failing from debonding. The HF etching led to noteworthy surface topographical alterations. Micromechanical interlocking resulting from HF acid etching remained prevalent in the fatigue behavior. Thus, the silane-based coupling agent (Monobond N) does not need to be applied after HF etching in terms of fatigue behavior outcomes.

This study evaluated the effect of grinding and polishing the inner surface of monolithic discs made of zirconia polycrystals (ZR) and lithium disilicate glass-ceramic (LD) on the load-bearing capacity under fatigue of the restorations bonded onto a dentin analogue material (epoxy resin). ZR and LD ceramic discs (10 mm in diameter, 1 mm in thickness) were produced and randomly allocated into 10 groups considering the internal adjustment approach: Ctrl – No adjustment; F – Grinding with fine diamond bur (46 μm); F + Pol – Grinding with fine diamond bur followed by polishing with 2 tips (finisher and polisher); FF – Grinding with extrafine diamond bur (30 μm); FF + Pol – Grinding with extrafine diamond bur followed by polishing. In addition, discs (10 mm in diameter, 2.5 mm in thickness) of fiber reinforced epoxy resin were produced. Afterwards, the intaglio surface of the ZR discs were air-abraded with 45 μm alumina particles for 10 s, the LD and resin epoxy discs were etched with hydrofluoric acid (5%/20 s and 10%/60 s, respectively), and the treated discs were primed as recommended. Each ceramic disc was luted onto the epoxy resin disc with resin cement. Then, the samples were tested under a step-stress fatigue test (20 Hz, 10,000 cycles/step, step-size of 100 N starting at 200 N, and proceeding until failure detection). Fractographic, topographic and surface roughness analysis were also performed. The adjustments (grinding with or without polishing) (ZR: 733–880 N; LD: 1040–1106 N) triggered a detrimental effect on the fatigue behavior in both ceramics compared with the absence of treatment (control group; ZR: 973 N; LD: 1406 N). The polishing step had no effect on fatigue findings. Thus, grinding the inner surface of the tested ceramics should be avoided wherever possible to prevent introducing damage and its detrimental effects on the fatigue behavior.

To evaluate the fatigue failure load (FFL), cycles for fatigue failure (CFF) and survival rates of different occlusal thicknesses of resin composite simplified crowns with internal adjustments (IA) or without as control (C).30 monolithic simplified crowns of CAD/CAM resin composite (Tetric CAD, Ivoclar) were milled in three different occlusal thicknesses (0.5 mm, 1.0 mm, and 1.5 mm). Half of the crowns were submitted to restricted adjustment with diamond burs on the crown's inner surface and half remained milled without internal adjustment. The samples were treated and adhesively luted onto a prosthetic preparation made of epoxy resin reinforced by glass-fiber substrate. The sets were subjected to a fatigue test (cyclic fatigue: initial load of 200 N; step-size of 50 N; 10,000 cycles/step; 20 Hz; maximum load: 2800 N).Microscopic analysis of tested representative samples was performed. The fatigue data were statistically analyzed (α= 0.05) and the micrograpic images were qualitatively evaluated.All specimens from groups 1.0C and 1.5C survived the cyclic loads, while all 0.5C samples failed during the test. The 1.0IA and 1.5IA crowns showed no statistical difference for FFL (2530 N= 2670 N) and CFF (471,000 cycles= 499,000 cycles) between them, and they were both statistically superior to the 0.5IA (FFL= 1812.50 N; CFF= 327,500 cycles). The 1.0 mm and 1.5 mm crowns presented superior fatigue behavior (2530 N-2800 N) compared to the 0.5 mm crowns (1812 N-2140 N), whether internal adjustment was performed or not. A statistically significant difference was found for FFL and CFF of 0.5 mm crowns with and without internal adjustment, with a deleterious impact of the adjustment for both parameters (FFL and CFF: 0.5C 0.5IA).Resin composite milled crowns can bear high cyclic fatigue loads despite thickness, although thicker crowns had better mechanical behavior; however, the internal adjustment in the composite resin crown leads to a detrimental effect on its fatigue behavior, which presents a risk of mechanical failure due to premature fatigue.
