Endolymphatic hydrops (EH) is considered the histological hallmark of Meniere's disease. Visualization of EH has been achieved by special sequences of inner ear magnetic resonance imaging (MRI) with a gadolinium-based contrast agent via intravenous or intratympanic administration. Although it has been applied for more than 10 years since 2007, a unified view on this technique has not yet been achieved. This paper presents an expert consensus on MRI of endolymphatic hydrops in the following aspects: indications and contra-indications for patient selection, methods of contrast-agent administration (intravenous or intratympanic), MRI sequence selection, the specific scanning parameter settings, and standard image evaluation methods and their advantages and disadvantages. A 45-year-old man presented to the neurotology clinic, suffering from recurrent attacks of vertigo associated with falls, of unknown origin. Pure tone audiometry revealed a profound right-sided and mild left-sided pantonal sensorineural hearing loss. Caloric videonystagmography showed marked canal paresis on the right side. Video-head impulse test, however, showed bilaterally normal gain. Cranial magnetic resonance imaging (MRI) was normal. No neurovascular compression was noted. We performed locally enhanced inner ear MRI (LEIM). LEIM revealed a marked dilation of the endolymphatic space of the right inner ear, that is, endolymphatic hydrops. In light of the history, clinical, functional and imaging results, therefore, the diagnosis of hydropic ear disease (Meni&egrave, re&rsquo, s disease) with vestibular drop attacks could be established. This clinical image case illustrates the usefulness of inner ear imaging for the differential diagnosis of vertigo disorders, such as hydropic ear disease (Meni&egrave, s disease).

Analysis of neural structures in Menière's Disease (MD) is of importance, since a loss of such structures has previously been proposed for this patient group but has yet to be confirmed. This protocol describes a method of in vivo evaluation of neural changes especially well suitable for cranial nerve analysis using magnetic resonance imaging (MRI). MD-patients and normal hearing persons were examined in a 3-T MR-scanner using a scan protocol including strongly T2-weighted 3D gradient-echo-sequence (3D-CISS). In the patient group, MD was additionally confirmed using MRI-based assessment of endolymphatic hydrops. Morphometric analysis was performed using a freeware DICOM viewer. Evaluation of cranial nerves included measurements of cross-sectional areas (CSAs) of the nerves at different levels as well as orthogonal diametric measurements.

Einleitung: Der M. Meniere ist eine Innenohrerkrankung, die mit einer typischen Symptomtrias aus Schwindelattacken, Hörminderung und Tinnitus einhergeht. Die Anzahl und Schwere der Attacken kann sehr variabel sein. Der M. Menière ist charakterisiert durch das Auftreten eines endolymphatischen Hydrops (ELH) im Bereich der Cochlea und des Vestibulums. Ziel dieser Studie war, den Zusammenhang zwischen der Ausprägung des ELH und der Symptomlast zu untersuchen. Methoden: In die Studie wurden 337 Patienten mit M. Menière eingeschlossen. Bei diesen Patienten wurde die Ausprägung des ELH kernspintomographisch anhand einer 4-Punkt-Skala quantifiziert. Zudem wurden anhand eines Schwindelkalenders vier Monate vor bis vier Monate nach dem MRT schwere und leichte Schwindelattacken mithilfe einer 4-Punkt-Likert-Skala erfasst sowie anhand eines Schwindeltagebuchs jeweils eine Woche vor und nach dem MRT die Schwindel- und Begleitsymptomatik dokumentiert. Ergebnisse: Es zeigte sich keine Korrelation zwischen dem cochleären oder vestibulären ELH und der Anzahl der Schwindelattacken in den vier Monaten vor bzw. nach dem MRT (p = 0,34; 0,53; 0,95 und 0,17 für 4, 3,2 bzw. 1 Monat vor MRT und p = 0,35; 0,9; 0,32 und 0,43 für 1, 2, 3 bzw. 4 Monate nach MRT). Auch im kurzfristigen Zeitraum eine Woche vor bzw. nach dem MRT fand sich keine Korrelation zwischen der Ausprägung des ELH und der anhand Attackenanzahl (p = 0,44 und 0,11 für die Woche vor bzw. nach dem MRT) und Fluktuation der begleitenden Ohrsymptomatik erfassten Symptomstärke. Schlussfolgerung: Obwohl der ELH im langfristigen Verlauf zunimmt, scheinen kurz- und mittelfristige Schwankungen der Symptomstärke nicht mit messbaren Veränderungen des ELH einherzugehen. We report here three unusual cases of otomastoiditis due to Francisella tularensis, complicated by cervical abscesses and persistent hearing loss, plus facial paralysis for one patient. Intriguingly, the three patients had practiced canyoneering independently in the same French river, between 2009 and 2014, several days before clinical symptoms onset. The results point out that fresh water exposure may be a potential contamination route for tularemia. Besides, due to the frequent complications and sequelae, we believe that F. tularensis should be considered as a possible etiology in case of otitis media, failure of the conventional antibiotic treatment, and suspicious exposure of the bacteria.

Introduction. Vestibular schwannomas (VS) are benign tumours of the vestibular nerve and can lead to hearing loss, tinnitus, vertigo, facial palsy, and brainstem compression. Audiovestibular diagnostic tests are essential for detection and treatment planning. Methods. Medline was used to perform a systematic literature review with regard to how audiovestibular test parameters correlate with symptoms, tumour size, and tumour location. Results. The auditory brainstem response can be used to diagnose retrocochlear lesions caused by VS. Since hearing loss correlates poorly with tumour size, a retrocochlear lesion is probably not the only cause for hearing loss. Also cochlear mechanisms seem to play a role. This can be revealed by abnormal otoacoustic emissions, despite normal ABR and new MRI techniques which have demonstrated endolymphatic hydrops of the inner ear. Caloric and head impulse tests show frequency specific dynamics and vestibular evoked myogenic potentials may help to identify the location of the tumour regarding the involved nerve parts. Conclusion. In order to preserve audiovestibular function in VS, it is important to stop the growth of the tumour and to avoid degenerative changes in the inner ear. A detailed neurotological workup helps to diagnose VS of all sizes and can also provide useful prognostic information.

Einleitung: Die Cochlea Implantation (CI) wird seit vielen Jahren durchgeführt. Eine mögliche Komplikation ist das Auftreten von Gleichgewichtsstörungen. Diese Studie soll den Zusammenhang zwischen der Elektrodeninsertionstiefe in der Cochlea und dem Auftreten von Schwindel sowie den Einfluss auf die Gleichgewichtsfunktion untersuchen. Methoden: 41 erwachsene Patienten nach CI zwischen 2006 und 2012 wurden eingeschlossen. Die Elektrodeninsertionstiefe wurde mit Hilfe der postoperativen Röntgenaufnahme nach Stenvers ausgewertet. Alle Patienten wurden nach subjektiven Gleichgewichtsstörungen mit Hilfe eines Fragebogens abgefragt. Die Bogengangsfunktion wurde mittels kalorischer Prüfung in der Videookulographie gemessen und die Sacculusfunktion mittels vestibulär evozierter myogener Potentiale bestimmt. Ergebnisse: Die durchschnittliche Insertionstiefe der Elektrode betrug 464°. Es konnte kein statistischer Zusammenhang zwischen der Insertionstiefe und dem Auftreten von Schwindel festgestellt werden. Die Bogengangs-, sowie die Sacculusfunktion zeigten keine signifikanten Unterschiede in Bezug auf die Insertionstiefe. Weiterhin konnte keine statistische Differenz zwischen den verschiedenen CI Herstellern und den subjektiven und objektiven Messungen festgestellt werden. Schlussfolgerungen: In unserer Studie zeigte die Variabilität der Elektrodeninsertionstiefe keinen signifikanten Einfluss auf den untersuchten Faktoren. Die Röntgenübersichtsaufnahme nach Stenvers ist eine schnelle, einfache und kostengünstige Methode, um die Elektrodeninsertionstiefe nach CI zu bestimmen. Allerdings kann hiermit nicht eine mögliche Dislokation der Elektrode zwischen den Skalen festgestellt werden. Einleitung: Botulinumtoxin A (BTA) wird in der HNO-Heilkunde bei zahlreichen Indikationen eingesetzt. Für die meisten funktionellen Indikationen fehlen Studien mit validierten Instrumenten, die die Auswirkung der Therapie auf die Lebensqualität untersuchen, einem Parameter, der heute als wesentliches Evaluationskriterium einer Therapie angesehen wird. Da oft eine Off-label-Anwendung vorliegt, hat diese Fragestellung eine medikolegale und ökonomische Bedeutung. Methoden: 40 Patienten, die mit BTA auf Grund funktioneller Störungen in unserer Klinik behandelt wurden, erhielten das Glasgow Benefit Inventory. Dieser validierte Fragebogen misst die Auswirkung von Interventionen im HNO-Bereich auf die gesundheitsbezogene Lebensqualität. Es können Scores von –100 (maximaler negativer Effekt) bis 100 (maximaler positiver Effekt) erzielt werden. Ergebnisse: 29 Patienten (Rücklaufquote 72,5%) erreichten folgende durchschnittliche Gesamtscores: 1,2 (Sialorrhoe, n=7), 22,6 (Frey-Syndrom, n=8), 20,6 (Ohrgeräusch bei palatalem Tremor, n=5), 15,0 (Stimmprothesenstörung bei Laryngektomierten durch pharyngoösophagealen Spasmus, n=5), 38,9 (spasmodische Dysphonie, n=2) sowie 27,8 (oromandibuläre Dystonie, n=2). Der Anteil an behandelten Patienten mit einem Anstieg der Lebensqualität, d.h. positivem Gesamtscore, unterschied sich deutlich bei den einzelnen Indikationen: Sialorrhoe 29%, Frey-Syndrom 88%, palataler Tremor 60%, Stimmprothesenstörung 60%, spasmodische Dysphonie 100%, oromandibuläre Dystonie 100%. Schlussfolgerungen: Eine Therapie mit BTA kann bei allen untersuchten Indikationen – jedoch nicht bei allen Patienten – die Lebensqualität steigern. Multiple Sclerosis (MS) is one of the most frequent organic diseases of the nervous system, with a prevalence of 30-60 per 100,000 inhabitans. It is charcterized by an inflammatory destruction of the myelin sheaths in the white matter of the central nervous system, which may lead to severe disability and death. The underlying mechanism has not been clearly elucidated yet, but involves an attack of the body’s immune system against some of its own neural tissue antigens. One of the immunopathologic hallmarks of MS is the chronic intrathecal production of immunoglobulin (Ig). This contains IgG of very restricted variability, i.e. oligoclonal IgG, and in addition, recognizes a panel of different pathogens such as measles, rubella, and herpes zoster virus. While the antigen-specificity of the largest part of oligoclonal IgG in multiple sclerosis is unknown, the oligoclonal IgG arising during CNS infections are reactive against the specific pathogen. Recently, a link between Chlamydia (C.) pneumoniae and multiple sclerosis has been claimed. To test the possible role of C. pneumoniae in multiple sclerosis, we analyzed a) whether there is intrathecal IgG production against C. pneumoniae in multiple sclerosis and b) whether the oligoclonal IgG in the CSF of multiple sclerosis patients recognize C. pneumoniae. By studying paired serum/CSF samples from 120 subjects (definite multiple sclerosis: 46; probable multiple sclerosis: 12; OIND: 35; OND: 27) by ELISA, we found that 24% of all patients with definite multiple sclerosis, but only 5% of patients with other inflammatory or non-inflammatory diseases produced IgG specific for C. pneumoniae intrathecally (definite multiple sclerosis versus OIND: p = 0.027). The presence of intrathecal IgG to C. pneumoniae was independent of the duration of disease and relatively stable over time. The major CSF oligoclonal IgG bands from multiple sclerosis-patients with an intrathecal Ig-production to C. pneumoniae did not react to C. pneumoniae by IEF-Western as seen by isolectric focusing and subsequent affinity-mediated immunoblot (IEF-Western) towards purified elementary bodies and reticulate bodies of C. pneumoniae. By contrast, the IgG in the CSF of control patients with neuroborreliosis strongly reacted with their specific pathogen, Borrelia burgdorferi by IEF-Western. Concomitant analysis of the CSF of 23 patients with a nested PCR for C. pneumoniae was negative in all cases. Together, these findings strongly suggest that the immune response to C. pneumoniae is part of a polyspecific intrathecal Ig production, as is commonly observed with other pathogens. This argues against a specific role of C. pneumoniae in multiple sclerosis.
