Aims: Quality assurance in radiotherapy (QART) is essential to ensure the scientific integrity of a clinical trial. This paper reports the findings of the retrospective QART assessment for all centres that participated in PORTEC-3; a randomised controlled trial that compared pelvic radiotherapy with concurrent chemoradiotherapy to the pelvis followed by adjuvant chemotherapy. The trial showed an overall survival benefit for the addition of the chemotherapy in the management of women with high-risk endometrial cancer. Materials and methods: Clinicians were invited to upload a randomly selected case/s treated at each of the participating sites. Panel reviewers analysed the contours to certify that the target volumes and organ at risk structures were contoured according to guidelines. The results were categorised into acceptable, minor variation, major variation or unevaluable. The radiotherapy plans were dosimetrically evaluated using the well-established Trans-Tasman Radiation Oncology Group (TROG) protocol. Results: Between August 2010 and January 2018, data from 146 patients of 686 consecutively treated patients were retrospectively reviewed. All 16 Australia and New Zealand and 71 of 77 international centres uploaded data for evaluation. In total, 3514 dosimetric and contour variables were reviewed. Of these, 3136 variables were deemed acceptable (89.2%), with 335 minor (9.6%) and 43 major variations (1.2%). Major contour variations included the clinical target volume vaginal vault, clinical target volume parametria and differential planning target volume vault expansion. Conclusion: The results of the QART assessment confirmed high uniformity and low rates of both minor and major deviations in contouring and dosimetry in all sites. Objective Recent clinical trials have demonstrated that colchicine may have metabolic and cardiovascular and benefits in at-risk patients; however, the mechanisms through which colchicine may improve outcomes are still unclear. We sought to examine colchicine’s effects on circulating inflammatory and metabolic molecules in adults with obesity and metabolic syndrome (MetS). Methods Blood samples were collected pre- and post-intervention during a double-blind randomized controlled trial in which 40 adults with obesity and MetS were randomized to colchicine 0.6 mg or placebo twice-daily for 3 months. Serum samples were analyzed for 1305 circulating factors using the SomaScan Platform. The Benjamini–Hochberg procedure was used to adjust the false discovery rate (FDR) for multiple testing. Results At baseline, age (48.0 ± 13.8 vs. 44.7 ± 10.3 years) and BMI (39.8 ± 6.4 vs. 41.8 ± 8.2 kg/m2) were not different between groups. After controlling for the FDR, 34 molecules were significantly changed by colchicine. Colchicine decreased concentrations of multiple inflammatory molecules, including C-reactive protein, interleukin 6, and resistin, in addition to vascular-related proteins (e.g., oxidized low-density lipoprotein receptor, phosphodiesterase 5A). Conversely, relative to placebo, colchicine significantly increased concentrations of eight molecules including secreted factors associated with metabolism and anti-thrombosis. Conclusions In adults with obesity, colchicine significantly affected concentrations of proteins involved in the innate immune system, endothelial function and atherosclerosis, uncovering new mechanisms behind its cardiometabolic effects. Further research is warranted to investigate whether colchicine’s IL-6 suppressive effects may be beneficial in COVID-19.

A series of new calamitic liquid crystals, 2-[3-ethoxy-(4-alkyloxybenzoyloxy)phenyl]benzothiazoles comprising a benzothiazole core, terminal alkoxy chain, ester linker and lateral ethoxy substituent were synthesized and characterized. This series comprises nine members wherein the members differ by the length of alkyloxy chain (CnH2n + 1O-, where n = 2, 4, 6, 8, 10, 12, 14, 16, 18). The thermal properties of the compounds were analyzed by differential scanning calorimetry (DSC), and the textures were identified by polarized optical microscope (POM). The member with the shortest alkoxy chain (n = 2) is a non-mesogen. The remaining members exhibited monotropic nematic phase ranging from the low member (n = 4) until the highest member (n = 18) of the series. The influence of alkoxy chain length on the mesomorphic was studied. In addition, the mesomorphic properties of the benzothiazole liquid crystals were compared with these compounds and the effect of the lateral substituents on their mesomorphism was studied.

Recently, the combustion chemistry of tetrahydrofurfuryl alcohol (THFA), a potential biofuel, was investigated in a stoichiometric 20 mol % THFA/methane co-fueled premixed flame at 5.3 kPa by our group (Tran, L.-S.; Carstensen, H.-H.; Foo, K. K.; Lamoureux, N.; Gosselin, S.; Gasnot, L.; El-Bakali, A.; Desgroux, P. Experimental and modeling study of the high-temperature combustion chemistry of tetrahydrofurfuryl alcohol. Proc. Combust. Inst. 2021, 38, 631-640, 10.1016/j.proci.2020.07.057). With regard to this, we continue to further explore the combustion chemistry of this biofuel to understand the influence of THFA-doping amounts on the flame chemistry of its mixture with methane and the impact of the alcohol function of THFA on the product spectrum compared to its non-alcoholic fuel counterpart, i.e., 2-methyltetrahydrofuran (MTHF). To accomplish the above said objective, a methane flame, a 10% THFA/methane flame, and a 20% MTHF/methane flame were additionally analyzed at similar conditions using gas chromatography for quantitative species detection and NO laser-induced fluorescence thermometry. More than 40 species (reactants, CO, CO2, H2O, H2, and about 14 hydrocarbons as well as 26 oxygenated intermediates up to 5 carbon atoms) were quantified for each doped biofuel flame. The product distributions and consumption pathways of THFA are similar for the 10 and 20% THFA-doped flames. The maximum yields of most products increase linearly with the amount of doped THFA. However, some species do not follow this trend, indicating interaction chemistry between methane and THFA, which is found to be mainly caused by the reaction of the methyl radical. The difference in the chemical structure in THFA and MTHF has no notable impact on the mole fractions of CO, CO2, H2O, and H2, but significant differences exist for the yields of intermediate species. The doped THFA flame produces more aldehydes, alcohols, and ethers but forms clearly less ketones and hydrocarbons. A slightly upgraded version of our previous kinetic model reproduces most experimental data well and is able to explain the observed differences in intermediate production. © 2021 American Chemical Society.

Introduction: Treatment plan evaluation requires knowledge of the effect of the plan, not only on the intended target, but also the surrounding normal tissues that are unavoidably irradiated. Recent literature has provided estimations of tolerance doses and proposed dose-volume constraints for many of the organs at risk. However, very few of these recommendations have been independently validated. This study details how constraints proposed for the rectum were tested using data from the RT01 randomised prostate radiotherapy trial. Method: An independent validation of the rectal dose-volume constraints used in the CHHiP trial and proposed recently by Fiorino et al. was performed. The constraints were applied retrospectively to the treatment plans collected from the RT01 trial. Odds ratios (OR) were calculated to compare the reported incidence of specific late rectal toxicity end points in the group of patients whose treatment plan met a specified dose-volume constraint compared to the group of patients who failed that constraint. Results: Statistically significant ORs were observed for every constraint tested (except 75 Gy) for at least one clinical end point. For the CHHiP constraints between 60 and 70 Gy, the ORs calculated for rectal bleeding (RMH score defined in protocol) exceeded 2.5 (P!0.02). Similarly the ORs for CHHiP constraints between 30 and 65 Gy exceeded 2.4 (P!0.021) for urgency (UCLA PCI). The Fiorino constraints between 40 and 60 Gy resulted in ORs O2 (P!0.02) for loose stools (UCLA PCI) Conclusion: Implementing rectal dose-volume constraints from 30 Gy up to the prescription dose will result in a decrease in the incidence of late rectal toxicity. Constraints for doses as low as 30 Gy were statistically significant, further challenging the concept that the rectum is a serial structure where the maximum dose to the organ is the only consideration.
