Il presente articolo esplora il significato del concetto di “istituzione”, prestando particolare attenzione all’esposizione che Merleau-Ponty ne dà nelle pagine del corso tenuto al Collège de France nel 1954-1955, L’institution, la passivité. Il saggio mira a mostrare come, attraverso tale nozione, sia possibile pensare una concezione dell’agire, della prassi e quindi della storia che connette il lato attivo e passivo del fare. Il contributo prende le mosse da un’analisi delle critiche che Merleau-Ponty rivolge a Sartre nel corso de Le avventure della dialettica, per mostrare il bersaglio polemico al quale si rivolge la concezione dell’azione sviluppata nel corso sull’istituzione. Dopodiché, prende in considerazione più da vicino l’articolazione del tema nel corso sull’istituzione. Infine, cerca di descrivere la concezione della storia che emerge da tale plesso concettuale. In conclusione, il contributo propone un’apertura del problema dell’istituzione a problemi della contemporaneità, come quello dell’agency., The aim of this paper is to explore the merleau-pontyan concept of “institution” by focusing on its development along the 1954-55’s course held by Merleau-Ponty at the Collège de France, titled L’institution, la passivité. The paper intends to show how, by means of this notion, it is possible to think both the active and passive sides of action within individual action, praxis and therefore history. The essay starts with an overview of Merleau-Ponty’s critiques to the sartrian philosophy developed in Adventures of Dialectic, in order to show Merleau-Ponty’s polemical aim against which Merleau-Ponty develops his conception of action. After that, it deepens Merleau-Ponty’s notion of institution following the omonimous course. Finally, the paper tries to describe the conception of history that can be derived from this framework. In the final lines, the paper attempts to open up the concept of institution at the light of some contemporary theoretical problems, such as the problem of agency.

Aim. A wide range of clinical phenomena have been reported with dose reduction or drug discontinuation of Selective Serotonin Reuptake Inhibitors (SSRIs) or Serotonin Norepinephrine Reuptake Inhibitors (SNRIs). In 2015, a new classification of SSRIs/ SNRIs withdrawal (i.e., new withdrawal symptoms, rebound symptoms withdrawal, persistent post-withdrawal disorders) was outlined on the basis of the literature and clinical observations. A semistructured clinical interview, the Diagnostic clinical Interview for Drug Withdrawal 1-New Symptoms of SSRI and SNRI (DID-W1), was developed for identifying and differentiating such syndromes. Its inter-rater reliability has been tested. Methods. Seventeen consecutive outpatients with a history of SSRI or SNRI dose reduction or discontinuation were assessed independently by 2 clinicians at different times during the same day. Percent agreement, Cohen's kappa, and the squared correlation coefficient were used to measure inter-rater reliability. Results. The percent agreement for the whole interview was 97.06%, the Cohen's kappa 0.85 (95% CI of 0.61-1.08), the squared correlation coefficient 0.72. Discussion and conclusions. The kappa values indicated excellent inter-rater agreement. Validity evaluation and comparison with other instruments need to be performed. The DID-W1 may help diagnosing the clinical phenomena related to SSRI and SNRI discontinuation, their differentiation from relapse, and the potential iatrogenic origin of psychiatric symptoms in clinical practice.

- Abstract
Psychology as a science offers an enormous diversity of theories principles and methodological approaches to understand mental health abnormal functions and behaviours and mental disorders. A selected overview of the scope current topics as well as strength and gaps in Psychological Science may help to depict the advances needed to inform future research agendas specifically on mental health and mental disorders. From an integrative psychological perspective most maladaptive health behaviours and mental disorders can be conceptualized as the result of developmental dysfunctions of psychological functions and processes as well as neurobiological and genetic processes that interact with the environment. The paper presents and discusses an integrative translational model linking basic and experimental research with clinical research as well as population based prospective longitudinal studies. This model provides a conceptual framework to identify how individual vulnerabilities interact with environment over time and promote critical behaviours that might act as proximal risk factors for ill health and mental disorders. Within the models framework such improved knowledge is also expected to better delineate targeted preventive and therapeutic interventions that prevent further escalation in early stages before the full disorder and further complications thereof develop. In contrast to conventional "personalized medicine" that typically targets individual (genetic) variation of patients who already have developed a disease to improve medical treatment the proposed framework model linked to a concerted funding programme of the "Science of Behaviour Change" carries the promise of improved diagnosis treatment and prevention of health risk behaviour constellations as well as mental disorders.

Su contenido se basa en los resultados de revisiones sistemáticas, meta-análisis y estudios controlados. La evidencia acumulada indica que la perspectiva psicosomática podría contribuir significativamente a mejorar los resultados de la atención sanitaria, mediante una práctica clínica holística, la evaluación de los factores psicosociales implicados en la vulnerabilidad individual y la integración de intervenciones psicológicas en medicina. Dicha evidencia constituye el terreno ideal para afrontar necesidades emergentes en sanidad, como la gestión de enfermedades crónicas y de síntomas inexplicables, el paciente como productor activo de salud, la prevención temprana y una valoración de resultados que supere el reduccionismo biomédico. This paper provides an updated review of the main contributions of Psychosomatic Medicine to clinical and preventive medicine. Metaanalyses, comprehensive reviews and controlled studies were selected. Current advances in the field indicate that health care can be improved by using a psychosomatic approach that takes into account psychosocial factors affecting individual vulnerability, the holistic consideration of patient care, and the integration of psychological interventions in medicine. Consolidated evidence in Psychosomatic Medicine provides the ideal background for dealing with new needs emerging in medical practice, including chronic illness management, somatic unexplained symptoms, the shift in the role of patients from health consumers to actively health producers, primary early-life prevention, and the appraisal of health outcomes beyond the biomedical reductionism.

he aim of this critical review was to outline emerging trends and perspectives of clinical pharmacopsychology, an area of clinical psychology that is concerned with the psychological effects of medications. The historical development of clinical pharmacopsychology (Kraepelin, Pichot, Kellner, Di Mascio, Shader, Bech) is outlined, with critical review of its most representative expressions and reference to current challenges of clinical research. Clinical pharmacopsychology is concerned with the application of clinimetric methods to the assessment of psychotropic effects of medications (including behavioral toxicity and iatrogenic comorbidity) and the interaction of drugs with specific and non-specific treatment ingredients. Clinical pharmacopsychology offers a unifying framework for the understanding of clinical phenomena in medical and psychiatric settings. Research in this area deserves high priority.

Introduction: The psychosomatic characteristics of cyclothymic disorder are scarcely investigated in the literature (Van Meter et al, 2012). The aim of this study is to integrate the diagnostic taxonomy with psychosomatic assessment in cyclothymic patients. Introduction: Demoralization constitutes a cluster of psychological symptoms particularly prevalent in the medical context. We conducted a systematic review with the aim of supporting the evidence for a differentiation of demoralization from major depressive disorder (MDD). Methods: A qualitative systematic review following PRISMA criteria was conducted. Utilizing the keyword “demoralization” in databases PubMed, PsycINFO, Web of Knowledge, an electronic search was performed, supplemented by a manual search. 60 studies were selected. Study selection criteria included the use of medical samples and of instruments validated to assess demoralization. Four instruments were identified. Three are self-report, the Demoralization Scale (DS), the Psychiatric Epidemiological Research Interview-Demoralization Scale (PERI-D), and the Subjective Incompetence Scale (SIS) and one is a structured interview for the Diagnostic Criteria for Psychosomatic Research (DCPR) (Mangelli et al., 2003). Results: Only the DCPR interview and the DS have been used to detect demoralized cases in absence of MDD. The DCPR interview was applied to assess demoralization across various medical conditions, finding a prevalence ranging from 7% in coronary heart disease to 40% in endocrine diseases. The DS has been administered mostly in cancer patients showing a prevalence of 5-27.4%. To date, no studies using the PERI-D and the SIS have focused on such a differentiation. Discussion/Conclusions: Using validated instruments, demoralization appears to entail specific clinical features and emerges as a distinct condition from MDD. However, the reported prevalence rates may vary due to the characteristics of the assessment instruments.

A specific treatment may cause or precipitate lasting adverse effects on the course of illness. Such vulnerabilities, that are subsumed under the definition of “iatrogenic comorbidity”, may manifest themselves during treatment administration and/or after its discontinuation. The changes are persistent and not limited to a short phase, such as in the case of withdrawal reactions. Iatrogenic comorbidity plays an important and yet neglected role in mental health. There are several examples in psychopharmacology: bipolar course induced by antidepressant drugs in depressed patients who presented with allegedly unipolar features; persistent postwithdrawal disorders after long term use of SSRI and/or SNRI; tardive dyskinesia after long term use of antipsychotics. Iatrogenic comorbidity may occur also as a result of failed psychotherapies. It is thus of crucial importance to incorporate treatment history in mental health assessment. The concept of “iatrogenic comorbidity” may demarcate major prognostic and therapeutic differences among patients who otherwise seem to be deceptively similar since they share the same psychiatric diagnosis.

Introduction: Demoralization is a frequent psychological response to cancer. However, the impact of demoralization on quality of life in cancer patients has been virtually neglected by the literature. The aim of this study was to examine the relationship between demoralization and quality of life in cancer patients. Methods: A sample of 142 women with a diagnosis of breast cancer (mean age 53.4 ± 10.8 years; 75.4% with primitive cancer and 24.6% with metastatic cancer) underwent the demoralization section of the Structured Interview according to the Diagnostic Criteria for Psychosomatic Research (DCPR) and completed the following self-report questionnaires: Demoralization Scale (DS), Psychiatric Epidemiology Research Interview-Demoralization (PERI-D)., Subjective Incompetence Scale I and II (SIS-I and SIS-II), and the Short Form-36 Health Survey (SF-36). Results: Demoralization according to the DCPR was found in 24.6% of participants and was significantly associated with lower scores on all the SF-36 scales, except for “physical functioning” and “role limitations due to physical health”. The DS, PERI-D, SIS-I, and SIS-II were negatively correlated with all the SF-36 scales: higher demoralization scores corresponded to a worse quality of life. Conclusions: Demoralization seems to play a detrimental role on satisfaction for one’s own quality of life in breast cancer patients. It is also possible that a diminished quality of life may increase the vulnerability to demoralization. The use of psychotherapeutic treatments for demoralization could enhance quality of life in patients with a diagnosis of cancer.

Introduction: A diagnosis of cancer is a stressful event which has the potential to elicit psychological disturbances, especially depressive symptoms, and to decrease quality of life. Resilience may be crucial to preserve mental health in patients coping with cancer. The aim of this study was to examine the relationship between resilience and quality of life, depression and demoralization in oncology. Methods: One hundred and forty-two women with breast cancer (mean age 53.4 ± 10.8 years; 75.4% with primitive cancer and 24.6% with metastatic cancer) underwent the following battery: Structured Clinical Interview for DSM-IV, demoralization section of the Structured Interview according to the Diagnostic Criteria for Psychosomatic Research (DCPR), ConnorDavidson Resilience Scale (CD-RISC), Short Form-36 Health Survey (SF-36), Beck Depression Inventory (BDI), and Demoralization Scale (DS). Results: Women with DSM-IV major depressive disorder (21.8% of participants) and those with demoralization according to the DCPR (24.6% of participants) had significantly lower levels of resilience (CD-RISC) than those without these diagnoses. Lower scores on the CD-RISC were significantly associated with higher levels of demoralization (DS) and depressive symptoms (BDI) and with a worse quality of life on all the SF-36 scales, except for “pain”. Conclusions: Our findings suggest a protective role of resilience against the development of depressive symptomatology, which was found to be significantly associated with a poor outcome in cancer patients. Enhancement of resilience, through specific psychological interventions, may be crucial to prevent depressive symptoms and to maintain quality of life in cancer patients.

Introduction: The psychosocial correlates of hyperandrogenism in adolescent and young women are getting increasing attention. The aim of this study was to assess allostatic overload (AO), based on specific clinimetric criteria, and related psychological distress in a population of adolescent and young females with hyperandrogenic states. Methods: This was a cross-sectional study involving high school female students, aged 16-19 years. For the specific purposes of this investigation, we focused on the subsample of students whose clinical state was assessed by medical examination (n= 835). Psychological evaluation included three self-report questionnaires (Symptom Questionnaire-SQ, the Psychosocial Index-PSI, and the Psychological Well-Being scales-PWB). The presence of AO was determined according to clinimetric criteria. Results: One hundred eighty-four participants (22.5%) were classified as having AO. They showed significantly higher levels of psychological distress (on the SQ) and impaired well-being (on the PWB) compared to those who did not present with AO. A significant association was found between the presence of AO and hyperandrogenic states. Conclusions: These findings lend support to the clinical utility of the clinimetric criteria for the assessment of AO in medical settings and highlight the importance of psychosocial aspects of hyperandrogenism among adolescent and young women.

Comorbidity is a well‐established and documented phenomenon in mental disorders and medicine with heuristic value. The concept of comorbidity remains however poorly defined and lacks a comprehensive and coherent theoretical framework. There is a need to develop coherent methodological strategies in order to promote a fuller understanding of the implications of comorbidity and to exploit its potential value with regard to etiopathogenic and therapeutic issues. This position paper makes recommendations of improved methodological standards and procedures and discusses a range of options that can provide incremental information that is likely to improve therapeutic outcomes. An increasing number of investigations in the positive psychology area has suggested the benefits of improving well-being and positivity. However, observations from clinical domains raised some concerns on the effectiveness and utility of such interventions, because excessively elevated levels in positive domains could become detrimental and impede flourishing. The aim of this article is to describe these polarities in well-being and to suggest specific therapeutic strategies to promote flourishing and resilience. Well-being therapy (WBT) is a psychotherapeutic technique based on Ryff's conceptual model, that has been developed and tested in a number of randomized controlled trials. The differential technical characteristics and indications of WBT are described, with special reference to the promotion of an individualized and balanced path to achieve optimal human functioning, avoiding the polarities in positive psychological dimensions. Un número creciente de investigaciones en el área de la Psicología positiva sugiere que incrementar el bienestar y la positividad conlleva beneficios. Sin embargo, observaciones del ámbito clínico han suscitado algunas cautelas sobre la efectividad y utilidad de dichas intervenciones ya que niveles excesivamente elevados en los dominios positivos pudieran ser perjudiciales e impide el florecimiento. El objetivo de este artículo es describir estas polaridades en el bienestar y sugerir estrategias terapéuticas para promover el florecimiento y la resiliencia. La terapia del bienestar (TB) es una técnica de psicoterapia, basada en el modelo conceptual de Ryff, que ha sido desarrollada y sometida a prueba en una serie de ensayos clínicos aleatorizados. Se describen las características técnicas diferenciales y las indicaciones de la TB, con especial referencia a la promoción de una vía individualizada y equilibrada para conseguir un funcionamiento humano óptimo, evitando las polaridades en las dimensiones psicológicas positivas.

Introduction: Type 1 diabetes has widely been associated with an increased prevalence of mental health difficulties, including anxiety and depression. In the setting of chronic diseases, a more comprehensive assessment which integrates psychiatric diagnoses with an evaluation of subthreshold symptomatology of distress, has recently emerged. The aim of this controlled study was to evaluate the incremental utility of clinimetric criteria of allostatic overload in the assessment of distress in patients with type 1 diabetes. Methods: 31 young adult patients with type 1 diabetes (30.63±7.60) and 30 matched healthy controls were interviewed for the presence of psychiatric disorders, psychosomatic syndromes with the following instruments: the Structured Clinical Interview for DSM Disorders (SCID-I), the Diagnostic Criteria for Psychosomatic Research (DCPR). The presence of distress in the form of allostatic overload (AO) was also assessed according to specific criteria (Fava et al., 2010). χ2 was used to analyze socio-demographic and clinical differences between patients and controls. Results: No differences emerged between the two groups in socio-demographic variables. As for psychological variables, patients and controls did not significantly differ in terms of frequency of psychiatric disorders or psychosomatic syndromes, except for the diabetic patients who reported past depressive episodes more frequently than controls (p=0.01). Interestingly, individuals with diabetes presented greater psychological distress in terms of AO than healthy subjects (p=0.037). Discussion/Conclusions: AO entails specific clinical features and emerges as a distinct condition from other form of distress. Thus, clinimetric measures such as AO more than diagnostic criteria, may help to identify important information and clinically relevant distress differences in patients with chronic illness such as type 1 diabetes

There have been major transformations in health-care needs in the past decades. Chronic disease is now the principal cause of disability, and the use of health services for patients with chronic diseases consumes almost 80 % of health expenditures. The exponential spending on preventive medication justified by the potential long-term benefits to a small segment of the population is now being challenged, whereas the benefits of modifying lifestyle by population-based measures are increasingly demonstrated and are in keeping with the biopsychosocial model. Medically unexplained symptoms occur in up to 30–40 % of medical patients and increase medical utilization and costs. The traditional medical specialties, based mostly on organ systems (e.g., cardiology, gastroenterology), appear to be more and more inadequate in dealing with symptoms and problems which cut across organ system subdivisions. The need for a holistic approach is underscored by the implementation of interdisciplinary services. The need to include consideration of functioning in daily life, productivity, performance of social roles, intellectual capacity, emotional stability, and well-being, has emerged as a crucial part of clinical investigation and patient care. These aspects have become particularly important in chronic diseases, where cure cannot take place and also extend over family caregivers of chronically ill patients and health providers.

A specific psychotherapeutic strategy for increasing psychological well-being and resilience, well-being therapy (WBT), based on Ryff's conceptual model, has been developed and tested in a number of randomized controlled trials. The findings indicate that flourishing and resilience can be promoted by specific interventions leading to a positive evaluation of one's self, a sense of continued growth and development, the belief that life is purposeful and meaningful, the possession of quality relations with others, the capacity to manage effectively one's life and a sense of self-determination. A decreased vulnerability to depression, mood swings and anxiety has been demonstrated after WBT in high-risk populations. School interventions based on the principles of WBT have been found to yield both promotion of well-being and decrease of distress compared with control groups. The differential technical characteristics and indications of WBT are described, with a special reference to the promotion of an individualized and balanced path to achieve optimal human functioning, avoiding the polarities in positive psychological dimensions.

'Clinimetrics' is the term introduced by Alvan R. Feinstein in the early 1980s to indicate a domain concerned with indexes, rating scales and other expressions that are used to describe or measure symptoms, physical signs and other clinical phenomena. Clinimetrics has a set of rules that govern the structure of indexes, the choice of component variables, the evaluation of consistency, validity and responsiveness. This review illustrates how clinimetrics may help expanding the narrow range of information that is currently used in clinical science. It will focus on characteristics and types of clinimetric indexes and their current use. The clinimetric perspective provides an intellectual home for clinical judgment, whose implementation is likely to improve outcomes both in clinical research and practice.

AIM: The aim of the present study was to validate the Italian version and to assess the clinimetric properties of Ahearn and Carroll's Visual Analogue Scales for Bipolarity (MVAS-BP), a self-rating questionnaire measuring affective state. METHOD: MVAS-BP consist of 26 items: 1 item assesses overall mood, 2 items anger and 23 items are based on the Carroll model of bipolar disorder (Consummatory Reward, Incentive Reward, Central Pain, Psychomotor Regulation). MVAS-BP have been translated into Italian and administered with Kellner's Symptom Questionnaire (SQ), Ryff's Psychological Well-Being Scales (PWB), and Cloninger's Tridimensional Personality Questionnaire (TPQ) to a normative sample of 450 people. The assessment was repeated after one month. Test-retest reliability, means scores, differences according to sociodemographic variables and correlations among the scales of MVAS-BP, SQ, PWB and TPQ were performed in order to assess clinimetric properties. RESULTS: Test-retest coefficients were highly significant. Significant sex differences were found in the scales of consummatory reward, central pain, psychomotor regulation and anger. MVAS-BP scales were significantly related with symptom and well-being scales, but with very low correlation coefficients, and only with one personality dimension, TPQ Harm Avoidance. DISCUSSION: MVAS-BP has been found to possess clinimetric properties and to record specific and different information from those investigated by other questionnaires.

L’adolescenza, e in particolare i suoi aspetti positivi quali la qualità della vita, la felicità e il funzionamento sociale, ha attirato negli ultimi anni una sempre maggior attenzione. Un setting ideale per la promozione delle relazioni interpersonali e del funzionamento ottimale nei giovani è la scuola, che può essere utile non solo per favorire l’apprendimento e i processi educativi, ma anche come “istituzione positiva” per facilitare lo sviluppo sociale e umano. Lo scopo di questo studio è stato di verificare l’efficacia di un innovativo intervento scolastico per promuovere il benessere psicologico nell’adolescenza, messo a confronto con un protocollo di attenzione-placebo. I risultati hanno evidenziato l’efficacia di questo intervento nel promuovere il benessere psicologico, in particolare la crescita personale, rispetto al gruppo attenzione-placebo. Inoltre, c’è stata una diminuzione del distress, in particolare dell’ansia e della somatizzazione. Sviluppare il benessere psicologico per mezzo di un intervento scolastico strutturato e focalizzato sulla promozione del funzionamento ottimale, come la WBT, potrebbe migliorare il funzionamento individuale, lo stile di coping e il processo di sviluppo nei giovani

L’insegnamento della psicosomatica risulta di importanza fondamentale in ambito psicologico in quanto contribuisce alla formazione di un approccio in grado di collocare le componenti biologiche, psicologiche e sociali in un quadro di riferimento multidisciplinare, armonizzando la clinica psicologica con quella medica. Nell’ambito della medicina sia generale che specialistica, la psicosomatica offre un ampliamento significativo delle capacità di comprensione e di valutazione dei fenomeni clinici, con ricadute sulla possibilità di coinvolgere il paziente come partner attivo della terapia. Lo stile di vita appare come un elemento di sempre maggiore importanza nel modulare la vulnerabilità individuale alla malattia e solo un approccio psicosomatico è in grado di rendere operativa la sua modificazione da parte del paziente. Inoltre, le branche specialistiche della medicina (basate sulla suddivisione clinica in apparati come quello cardiovascolare e gastroenterico) appaiono sempre più inadeguate nel fornire risposte sia a problematiche complesse che non si collocano all’interno di un singolo apparato sia a quadri clinici sempre più diffusi di comorbilità. La medicina psicosomatica fornisce le basi cliniche e concettuali per approcci multidisciplinari (ad esempio, psiconeuroendocrinologia, psico-oncologia). Questo manuale non si propone di esaminare dettagliatamente il ruolo dei fattori psicosociali all’interno di specifiche patologie mediche, ma di illustrare in che cosa consista l’approccio psicosomatico e quali implicazioni possa avere a livello della clinica psicologica e della clinica medica. È una testimonianza della ricchezza e della vitalità della ricerca psicosomatica attuale.

La promozione del benessere psicologico costituisce un aspetto importante in ambito evolutivo. Sono stati effettuati 3 studi controllati in ambito scolastico (2 nelle medie inferiori e 1 in quelle superiori) relativi ad interventi brevi nell’ambito della classe basati sui principi della Well-Being Therapy (WBT). Questi studi hanno permesso di evidenziare come: a) anche poche sedute di WBT nell’ambito della classe che comprendano anche esercizi da effettuarsi a casa sono in grado di produrre delle modificazioni in alcune componenti del benessere e del disagio psicologici; b) queste modificazioni persistono nel tempo; c) gli interventi sono significativamente più efficaci di quelli effettuati con gruppi attenzione-placebo; d) gli interventi basati sulla WBT sono sostanzialmente equivalenti a quelli fondati sulla psicoterapia cognitivo-comportamentale, ma esistono degli aspetti differenziali che introducono la possibilità di una loro combinazione sequenziale. Anche se la WBT è stata sviluppata e validata in ambito clinico, i 3 studi controllati suggeriscono una grande potenzialità in ambito preventivo.

La riabilitazione può essere intesa come un processo di apprendimento e ri-apprendimento capace di stimolare e sostenere le risorse adattive in rapporto al processo di sviluppo. Al di là degli ambiti d’intervento legati alla disabilità, oggi si va sempre più delineando un’area di intervento riabilitativo riguardante il cosiddetto rischio psicosociale. La riabilitazione psicosociale si rivolge quindi a quelle condizioni familiari e sociali che, al di là dell’esistenza di una patologia psichica più o meno invalidante, configurano un rischio in grado di porre a repentaglio i potenziali evolutivi del bambino o dell’adolescente, compromettendo la sua salute mentale. In essa confluiscono i contributi della psicologia clinica e dello sviluppo, della neuropsichiatria infantile, della psicologia della famiglia, della psicologia sociale e giuridica, della psichiatria forense, designando un’area che risulta particolarmente feconda per le potenziali applicazioni operative. Le situazioni di rischio psicosociale sono innumerevoli: - bambini e adolescenti vittime di maltrattamento, abuso, violenza intrafamiliare, o in condizione di trascuratezza; - bambini e adolescenti cresciuti in famiglie con genitori affetti da disturbi mentali o da gravi sociopatie; - problemi connessi all’adozione o all’affidamento extrafamiliare; - bambini e adolescenti vittime di eventi traumatici di origine ambientale; - bambini e adolescenti portatori di disturbi mentali cronici e di condizioni di disabilità; - giovani e adolescenti devianti, autori di comportamenti aggressivi e violenti, individuali o di gruppo. Il volume si rivolge agli operatori che agiscono in questo settore (neuropsichiatri infantili, psicologi, assistenti sociali, educatori), offrendo, all’insegna di un approccio integrato e multidisciplinare, principi ed esperienze cliniche utili per valutare le situazioni a rischio e per programmare e monitorare gli interventi più efficaci.

- Abstract
The issue of conflict of interest has brought clinical medicine to an unprecedented crisis of credibility. Corporate actions that have placed profit over public health have become regular news in the media. The public seems to be increasingly sceptical of the integrity of medical practice, including psychiatry. Clinicians are more and more disoriented because of the discrepancy between the campaigns to shape a favourable climate of opinion for new drugs and the disappointing results in practice. Attempts to control conflict of interests by simple disclosure have yielded very limited results. A radical proposal for addressing the issue of conflict of interest in psychiatry and regaining credibility is advanced. It is based on the definition of “substantial” conflict of interest: being an employee of a private company; being a regular consultant or in the board of directors of a company; being a stockholder of a company related to the field of research; owning a patent directly related to the published work. Occasional consultancies, grants for performing investigations, or receiving honoraria or refunds in specific occasions would not be a source of substantial conflict of interest. Psychiatric investigators who hold positions in scientific societies, medical journals (editorship), groups for guidelines and clinical matters, should be devoid of substantial conflict of interest. Disclosure is no longer sufficient for the independence of the psychiatric field.

- Abstract
L'impiego di sistemi diagnostici categoriali è indispensabile nel processo diagnostico, per procedere all'identificazione del caso clinico o per l'inclusione dei soggetti in uno studio. Questi criteri non forniscono tuttavia un valore di soglia adeguato rispetto all'intensità dei sintomi e dovrebbero essere integrati, sotto il profilo clinico, secondo i princìpi della clinimetria, da informazioni derivanti dalla somministrazione di altri strumenti diagnostici di tipo dimensionale, basati sulle scale psicologiche sia etero che autovalutative. Tra queste, il Symptom Questionnaire costituito da brevi e semplici item a risposte sì/no si è rivelato valido e attendibile. Le scale di autovalutazione che ne fanno parte (depressione, ansia, somatizzazione, rabbia-ostilità) sono sensibili nel discriminare popolazioni differenti. In studi cross-over condotti in doppio-cieco si è constatata la capacità delle scale del Symptom Questionnaire nel discriminare tra gli effetti di uno psicofarmaco e il placebo. L'uso del Symptom Questionnaire è inoltre vantaggioso in studi con campioni esigui di soggetti in cui la sensibilità delle scale è importante e in popolazioni di soggetti con scarse capacità verbali.
