Abstract

Neuroblastoma (NB), the most common and deadly extracranial solid tumour of childhood, represents a challenging in paediatric oncology. Soluble tumour necrosis factor (TNF)-related apoptosis-inducing ligand (sTRAIL) is a cancer cell-specific molecule exerting remarkable anti-tumour activities against paediatric malignancies both in vitro and in preclinical settings. However, due to its too fast elimination and to the undesired related side effects, the improvement of sTRAIL in vivo bioavailability and the specific delivery to the tumour is mandatory for increasing its therapeutic efficacy. In this manuscript, we developed an innovative pegylated liposomal formulation carrying the sTRAIL at the outer surface (sTRAIL-SL) with the intent to improve its serum half-life and increase its efficacy in vivo, while reducing side effects. Furthermore, the possibility to combine sTRAIL-SL with the proteasome inhibitor Bortezomib (BTZ) was investigated, being BTZ able to sensitize tumour cells toward TRAIL-induced apoptosis. We demonstrated that sTRAIL preserved and improved its anti-tumour activity when coupled to nanocarriers. Moreover, sTRAIL-SL ameliorated its PK profile in blood allowing sTRAIL to exert a more potent anti-tumour activity, which led, upon BTZ priming, to a statistically significant enhanced life spans in two models of sTRAIL-resistant NB-bearing mice. Finally, mechanistic studies indicated that the combination of sTRAIL with BTZ sensitized sTRAIL-resistant NB tumour cells to sTRAIL-induced cell death, both in vitro and in vivo, through the Akt/GSK3/β-catenin axis-dependent mechanism. In conclusion, our results suggest that sTRAIL-SL might be an efficient vehicle for sTRAIL delivery and that its use in clinic, in combination with BTZ, might represent an adjuvant strategy for the treatment of stage IV, sTRAIL-resistant, NB patients.

Abstract

Lipoprotein(a) [Lp(a)] is a plasma lipoprotein that consists of a low-density lipoprotein (LDL)-like particle containing APO B-100 and apolipoprotein(a), linked by a disulphide bridge. There is evidence that higher serum level of Lp(a) is a predictor of various vascular diseases, such as myocardial infarction, coronary stenosis, re-occlusion of aortocoronary bypass vein grafts, peripheral atherosclerosis and cerebral infarction [1-4]. We describe a young man with a cryptogenic stroke with very high serum level of Lp(a) as the only vascular risk factor.

Abstract

Cervical artery dissection is an important cause of stroke in young patients and accounts of 10%–20% of stroke or TIA in patients aged less than 50 years. Basilar artery occlusion (BAO) is an infrequent cause of acute stroke, which invariably leads to death or long-term disability if not recanalized. We describe three patients with BAO caused by vertebral dissection, successfully treated with intra-arterial thrombolysis. The lysis of the occluding embolus was obtained by injection of the thrombolytic drug directly or near the thrombus without haemor-rhagic complications. Our cases confirm the safety and efficacy of intra-arterial thrombolysis in patients with BAO due to a vertebral artery dissection.

Abstract

Moya-Moya is a rare cerebrovascular occlusive disease characterized by bilateral stenosis or occlusion at the terminal portion of the internal carotid artery and abnormal vascular network at the base of the brain, named "moya-moya". In children, Moya-Moya disease usually presents with ischemic cerebrovascular events, mainly TIA or lacunar stroke, leading to mental deterioration. In adults, especially in females, it presents with intracranial haemorrhages. We describe the case of an adult patient with an atherosclerotic Moya-Moya disease which presented with a cerebral borderzone infarction.

Abstract

The aim of this study was to evaluate the risk of recurrent ischaemic cerebrovascular events (stroke or transient ischaemic attack (TIA)) in patients with patent foramen ovale (PFO) or atrial septal aneurysm (ASA) treated with different therapeutic regimens. We enrolled 86 patients aged 18-60 years with an unexplained ischaemic stroke or TIA referred to our inpatient department in the period May 1994-December 1999. Follow-up lasted until April 2003. Patients were excluded if the stroke or TIA was related to large-artery atherosclerosis, small artery occlusion, major cardiac sources of embolism or other uncommon causes. During a follow-up (mean+/-SD) of 64.1+/-28.8 months (range 8.1-105.6) a recurrent ischaemic cerebrovascular event occurred in 11/86 patients (12.8%) (5 TIA and 6 strokes). Eight events (4 TIA, 4 strokes) occurred in the 59 patients with PFO alone, three (1 TIA, 2 strokes) in the 21 with PFO plus ASA and none in the 6 patients with ASA alone. In the overall population the cumulative risk of recurrent stroke/TIA was 1.2% at 2 years, 5.5% at 4 years, 7.6% at 6 years and 23.6% at 8 years, and was similar in patients with PFO alone vs. patients with PFO plus ASA (9.0% vs. 6.1% at 6 years, 26.0% vs. 23.1% at 8 years; p0.05). Nine cerebral ischaemic events (4 TIA, 5 strokes) occurred in the 48 patients treated with antiplatelet drugs (7 in patients with PFO, 2 in patients with PFO plus ASA), and two (1 TIA, 1 stroke) in the 17 patients treated with oral anticoagulants (1 with PFO, 1 with PFO plus ASA). No events occurred in patients submitted to transcatheteral closure.

6. Gait and bilateral limb ataxia as isolated feature of a lower midbrain tegmental infarction

Abstract

Sirs: The clinical picture of midbrain infarction is dominated by third-nerve palsy, conjugate or disconjugate gaze impairment, contralateral motor and sensory deficit and homolateral or contralateral limb ataxia [1–3]. Among various mesencephalic syndromes of vascular origin, those related to the lower midbrain lesion are rare [2, 3]. We describe a patient with an isolated lower tegmental midbrain infarction, with gait and bilateral limb ataxia as isolated features. A 71-year-old man complained of dizziness, imbalance, slight hypersomnolence, and bilateral clumsiness of arms and legs. There was a worsening of symptoms and 12 hours later at admission neurological examination revealed slight hypersomnolence, dysartric speech and pronounced gait and bilateral limb ataxia. The NIHSS score was four. The patient walked with a wide-based, staggering gait without tendency to deviate to one side. The stance was on a broad base with impossibility standing with their feet together or in tandem position. Romberg sign was absent. At the finger-to-nose and at the heel-to-shine tests important dysmetria was present. The movement path of the limbs was erratic with relevant hypermetria. Rapid alternating movements were irregular bilaterally. The rest of the neurological examination was normal. One year before he suffered from an anterior circulation infarction involving the frontal branches of the right middle cerebral artery, without any residual deficit. Regarding conventional vascular risk factors, the patient suffered from hypertension and non-valvular atrial fibrillation. He was taking oral anticoagulants, with INR value ranging between 2 and 3. Non contrast cranial CT showed only the previous right frontal infarction. Five days later brain MRI revealed a small ischemic lesion in the middle tegmental portion of the lower midbrain (Fig 1 A–C); multiple silent small lacunar infarctions were present in the white matter bilaterally. Extracranial duplex ultrasonography and MR-angiography of carotid, vertebral and basilar arteries were normal. Transthoracic and transoesophageal echocardiography failed in revealing intra-cardiac thrombi. Eight-months later gait and bilateral limb ataxia and dysartria were the isolated and persistent neurological deficits. Palatal myoclonus was absent and the modified Rankin Scale was 3. Our patient presented an infarction in the middle portion of the midbrain tegmentum, with gait and bilateral cerebellar limb ataxia as dominant and persistent symptoms. The vascularization of the midbrain regions is complex, and the region close to the midline is supplied by the paramedian branches from the basilar artery (Fig 1D). Generally, in midbrain infarction the cerebellar ataxia is overshadowed by upper-midbrain and thalamic-subthalamic signs [1–2]. A tetra-ataxia has been reported in patients with midbrain infarction [1–6] but in none of them it was the isolated feature. In these cases the bilateral ataxia was associated to other neurological deficits, such as hemiparesis, dysphagia, dysphonia, eye-movement disorders or palatal myoclonus. Moreover, a bilateral ataxia was the sole manifestation of a rostral pontine tegmental infarction [7] and in patients with heat stroke [8]. In our patient the clinical picture may be considered a partial form of the Wernekink commissure syndrome where bilateral ataxia is associated to ocular abnormalities and palatal myoclonus. Indeed oculomotor deficit and delayed palatal myoclonus were absent and cerebellar signs were isolated except for the transient hypersomnolence related to the involvement of the reticular formation located dorsally to the superior cerebellar peduncles Supported by Piedmont Region, Ricerea Scientifica Applicata 2004.

Abstract

Various vascular syndromes have been described in patients with midbrain infarction.1,2 The clinical picture is dominated by third-nerve palsy, conjugate or disconjugate gaze impairment, contralateral motor and sensory deficit and homolateral or contralateral limb ataxia. We describe a patient with a pontomesencephalic tegmental infarction with hearing loss and hypogeusia as dominant features. A 52-year-old man had dizziness and loss of taste while eating dinner. The next morning upon awakening, he developed sensory changes of the left half of his face, clumsiness of the right arm and leg, and sensation of bilateral ear wadding. At admission, neurologic examination revealed right limb ataxia, decreased pinprick and touch sensation over the left half of his face, and bilateral hypoacusia, more marked on the left side. There was a marked diminution of taste on the left side of the tongue. His taste was examined by application of 0.5 M NaCl, 10% glucose, 0.2 M acetic acid, and 0.02 M quinine. The rest of the neurologic examination was normal. Limb ataxia and left face hypoesthesia gradually disappeared, and 10 days later only left tongue hypogeusia …

Abstract

WOS: 000368783600015, By analyzing a data set of 2.92 fb(-1) of e(+) e(-) collision data taken at root s = 3.773 GeVand 106.41 x 10(6) psi(3686) decays taken at root s = 3.686 GeVwith the BESIII detector at the BEPCII collider, we measure the branching fraction and the partial decay width for psi(3770)->gamma chi c0 to be B(psi(3770)->gamma chi c0) = (6.88 +/- 0.28 +/- 0.67) x 10(-3) and Gamma[psi(3770)->gamma chi c0] = (187 +/- 8 +/- 19) keV, respectively. Abstract

Background and Objectives: Renal transplant recipients (RTRs) have a 2- to 7-fold risk of developing a neoplasm compared to general population. Bladder urothelial neoplasms in this cohort has an incidence of 0.4–2%. Many reports describe a more aggressive behavior. The objective of this study is to describe oncologic characteristics of bladder urothelial neoplasms in RTRs and to evaluate its recurrence, progression, and survival rates. Methods: A retrospective multicentered study was performed evaluating all de novo bladder urothelial neoplasms cases in RTRs from 1988 to 2014. Descriptive statistical analysis and evaluation of recurrence, progression, and survival rates were performed. Results: A total of 28 de novo bladder transitional cell carcinomas (TCCs) were identified (incidence rate 0.64%). Cancer-specific survival rates were 100, 75, and 70% after 1, 5, and 10 years, respectively. Age at diagnosis superior to 60 years was found to be a statistically significant variable for recurrence risk. Progression rate was 14%. Presence of CIS was significantly associated with progression. All cancer-specific deaths were in the high-risk group and all were progressions from non-muscle invasive to muscle invasive bladder cancer. Conclusions: Bladder urothelial neoplasms following renal transplant is associated with a trend toward worst prognosis. Early aggressive treatments, such as early radical cystectomy, might be advisable to reduce cancer-specific deaths.
