Objective. To identify the association of a number of clinical features and structural and functional characteristics of the heart with the progression of chronic heart failure (CHF) in patients one year after ST-segment elevation myocardial infarction (MI) (STEMI) with preserved and reduced left ventricular ejection fraction (EF) (LVEF).Design and methods. 120 patients with STEMI were included in a prospective study. During the study, all patients underwent an echocardiographic study using a Sonos 2500 device (Hewlett Packard, USA) on the 1st day (point I), on the 12th day (point II) of hospitalization, and also after 1 year (point III). Depending on the parameters of EF on the 1st day of the disease, the total sample of patients was divided into two: the 1st group — with preserved LVEF was represented by 86 (71,7 %), the 2nd group — with a reduced LVEF was represented by 34 (28,3 %) patients.Results. A total of 19 (15,8 %) adverse events were registered. In two cases, a fatal outcome (1,7 %) was recorded, the cause of which was repeated MI, in five (4,2 %) patients decompensation of СHF was noted, in eight (6,7 %) patients a clinic of progressive angina was traced, in four (3, 3 %) of patients were diagnosed recurrent MI. Deterioration of systolic and diastolic function was established one year after STEMI with preserved (≥ 50 %) LVEF: 17,6 % of patients began to correspond to the intermediate range of EF (40-49 %), the number of patients with diastolic dysfunction increased by 10 % compared to with acute study.Conclusions. Within a year after a STEMI with initially preserved LVEF, there is a deterioration in myocardial function in the form of a decrease in myocardial contractility and an increase in the number of patients with diastolic dysfunction.

Objective: to evaluate the efficacy and safety of glycosaminoglycan-peptide complex (GPC, Rumalon) used in patients with knee osteoarthritis (OA) and comorbidities (hypertension and/or type 2 diabetes mellitus (T2DM).Patients and methods: A 10-month multicenter (13 centers from 10 subjects of the Russian Federation) prospective study included 179 patients with Stages II–III primary tibiofemoral knee OA and comorbidity, a walking pain intensity of ≥40 mm on a visual analogue scale (VAS), and a need for nonsteroidal anti-inflammatory drugs (for at least 30 days in the previous 3 months). The efficiency of treatment was evaluated from the changes in VAS scores for knee pain intensity during walking, the overall WOMAC score and scores of its individual indicators, laboratory parameters, and ultrasound data. All the patients received 2 cycles of GPC treatment with 25 injections, by following the manufacturer's instructions.Results and discussion. Even at 2 months after starting the treatment, there was a statistically significant reduction in pain during walking from 60 (50–69) to 40 (27–53.5) mm on the VAS (p< 0.05), which persisted during the follow-up period. A similar pattern was found when assessing the WOMAC index and its components. The drug demonstrated an anti-inflammatory effect: a decrease in the number of patients with synovitis from 55.7 to 39.2% (p=0.02), the level of high-sensitivity C-reactive protein (hsCRP) from 3.2 (1.3–6.2) to 2.6 (1.2–5.7) mg/l (p< 0.05). GPC was ascertained to have a safety profile.Conclusion. The data of this study confirm the good therapeutic effect and safety of GPC Rumalon in knee OA patients with hypertension and/or T2DM. The drug is shown to have a rapid analgesic and anti-inflammatory effect, as well as a positive influence on all clinical manifestations of OA.

Osteoarthritis (OA) belongs to diseases with high comorbidity and most frequently concurrent with obesity, diabetes mellitus (DM), hypertension, and other cardiovascular diseases (coronary heart disease, atherosclerosis), gastrointestinal tract diseases, and chronic diseases of the lung and kidney. Irrational treatment of OA in the presence of comorbidity and without considering characteristics of drug interactions leads to a pronounced increase in the number of adverse reactions (ARs) and to aggravation of the course of all concomitant diseases. From this point of view, therapy seems to be relevant when the latter involves drugs that have both symptom- and structure-modifying properties and have a high safety profile.Objective: to compare the safety of alternating and standard treatment regimens with Alflutop® in patients with knee OA.Patients and methods. 130 patients were enrolled in the trial who had Kellgren-Lawrence Grade II–III primary tibiofemoral knee OA with pain intensity on walking ≥40 mm on a visual analogue scale and who needed to take nonsteroidal anti-inflammatory drugs (≥30 days in the previous 3 months). The patients were randomized into two groups: Group 1 was prescribed Alflutop® 1.0 ml intramuscularly (IM) daily for 20 days (a standard regimen); Group 2 was given 2 ml IM every other day (a total of 10 injections) (an alternating regimen). The duration of follow-up was 14 weeks. The safety of Alflutop® was evaluated by the incidence of ARs and serious ARs (SARs) varying in severity according to medical records, laboratory tests, physical examination, assessment of a patient' vital signs, and electrocardiography (ECG). The patients were examined at the beginning, at the end, and 1 month after therapy.Results and discussion. No SARs were recorded during the study period and follow-up. There were 10 ARs in the group of patients receiving Alflutop® in the standard regimen and 19 ARs in the other group (the alternating regimen). All ARs corresponded to mild and moderate severity, were unassociated with the test drug, and resolved by the end of the follow-up. 12-lead ECG identified only clinically insignificant abnormalities in the patients of both groups. Patients without DM displayed no clinically significant increase in glucose levels. Those with DM had no increased glycemia tendency. Biochemical studies in both groups revealed only clinically insignificant abnormalities, the frequency of which was insignificant.Conclusion. This study has confirmed the comparable high safety of Alflutop® in both standard and alternative therapy regimens. It has also shown that the drug has a good safety profile and can be recommended for wide clinical application in any use regimen: 1 ml daily (a total of 20 injections) or 2 ml every other day (a total of 10 injections).

Aim. To study the blood coagulation status by various laboratory methods in patients after pulmonary embolism (PE) receiving long-term anticoagulant therapy.Material and methods. The blood of 23 patients with pulmonary embolism, who received long-term anticoagulant therapy, was studied. The study of coagulation profile, D-dimer, thrombodynamics, thromboelastography and thrombin generation test were carried out.Results. The thrombin generation test shows a significant increase in the time of its formation, while the maximum amount of thrombin formed is half that of the reference values. There is a slightly increased median fibrin clot growth rate in the thrombodynamics test — 30,4 gm/min with a normal coagulation rate of 20-29 gm/min. The result of thromboelastography also reflects the blood hypocoagulation, in terms of R, Angle a and CI.Conclusion. Integral methods for assessing the thrombotic readiness in combination with a routine coagulation panel demonstrate a complete picture of blood coagulation potential in patients after pulmonary embolism requiring long-term anticoagulant therapy.

Aim. To assess the prevalence, severity and prognostic value of renal dysfunction (RD) in patients with pulmonary embolism (PE) of the Russian population, as well as to determine the RD significance as a marker that improves the predictive ability of current risk stratification systems.Material and methods. From April 2018 to April 2019, patients hospitalized due to PE were sequentially included in the Russian multicenter observational prospective registry SIRENA. RD was diagnosed at a glomerular filtration rate (GFR) 2. Risk of early (hospital or 30-day) death was stratified in accordance with the current 2019 ESC Clinical Guidelines. During the study, we analyzed inpatient mortality and complication rate.Results. A total of 604 patients (men, 293 (49%); women, 311 (51%)) were in the study. RD was detected in 320 (53%) patients, while severe dysfunction — in 63 (10%) ones. In addition, 71 (12%) patients had high death risk, 364 (61%) — intermediate, 164 (27%) — low. During hospitalization, 107 (18%) patients died, including 32% from the high-risk group, 20% — moderate, and 7% — low. RD in the deceased patients was diagnosed more often, while GFR 2 reliably predicted hospital mortality (sensitivity, 67%; specificity, 72%; AUC=0,72; pConclusion. In patients with PE of the Russian population, there is a high incidence of RD, which is diagnosed in every second patient and is severe in 10% of cases. The presence of RD is associated with a significant increase in in-hospital mortality, while the risk of death increases with a decrease in GFR. The addition of RD, considered as a decrease in the estimated GFR 2, to the sPESI improves risk stratification and allows identification of patients at high risk of in-hospital death.

Aim: Identification of predictors of unfavorable annual prognosis in patients with acute coronary syndrome with ST-segment elevation (STEMI). Material and methods: The study included 69 patients with STEMI who were hospitalized in the period from March 2014 to January 2017 in the hospital named after Zhadkevich. The follow-up period was 12±3 months after the index STEMI Longterm prognosis was established in 62 (89.8%) patients. Results: the fatal outcome was observed in 7 (11.3%) patients. Nonfatal myocardial infarction developed in 10 (16.2%), hospitalization with heart failure decompensation - in 5 (8%), nonfatal acute cerebrovascular accident - in 3 (4.8%) patients. Predictors of adverse prognosis were: age >60 years, level of brain natriuretic peptide (BNP) >29.2 pmol/l, C-reactive protein (CRP) >4.7 mg/l, left ventricular ejection fraction (LVF) 60 years and BNP level >29.2 pmol/l. Conclusion: Adverse factors affecting the long-term outcome of the disease (12±3 months after the STEMI) are: BNP level >29.2 pmol/l, CRP level >4.7 mg/l, LV FV value 60 years.

Aim . To create a new prognostic scale for in-hospital mortality risk assessment in patients with pulmonary embolism (PE). Material and methods . The study was carried out on the basis of Russian register of acute pulmonary embolism SIRENA. Results . Based on the Russian register of acute pulmonary embolism SIRENA (n=609; women — 50,7%; mean age — 63,0±1,5 years), independent predictors of in-hospital death were determined: left ventricular ejection fraction

The report presents the results of a study of the total and intrinsic nonlinearities as applied to the outer satellites of Jupiter observed on a short arc. The relationship between the nonlinearities and the conditions of satellite observations is revealed. In particular, it is shown that the total nonlinearity is very strong when the observation period is less than 0.1 of the orbital period, while the intrinsic nonlinearity is weak enough for all satellites, which indicates the possibility of using nonlinear methods for adequate modeling of their orbital uncertainty.

This paper presents the results of a study of total and intrinsic nonlinearities in inverse problems of the dynamics of Jupiter’s Outer Satellites, observed on very short orbital arcs. The relationship between nonlinearity and the conditions of satellite observations is revealed. In particular, it is shown that a very strong total nonlinearity occurs when the observation period is less than 0.1 of the orbital period. In addition, it is shown that the intrinsic nonlinearity is rather weak for all the satellites. This indicates the possibility of using nonlinear methods for adequate simulating their orbital uncertainty.

Aim. To evaluate the incidence and severity of acute kidney injury (AKI), as well as its ability to reclassify the risk of premature mortality and association with inhospital mortality in patients with pulmonary embolism (PE) in the Russian population.Material and methods. From April 2018 to April 2019, the SIRENA Russian Multicenter Registry included patients with PE, as well as the deceased, in whom the PE was detected by autopsy. AKI was diagnosed according to current KDIGO guidelines (2012). Creatinine calculated according to the MDRD equation and corresponding to a glomerular filtration rate of 75 ml/min/1,73 m2 (baseline) was taken as the initial one, with subsequent assessment relative to the parameter value upon admission. The risk stratification of early death was carried out in accordance with the current ESC clinical guidelines (2019).Results. A total of 604 patients with PE were examined (men — 293 (49%), women — 311 (51%), mean age — 64±15 years). AKI was diagnosed in 223 (37%) of them. Stage 1 AKI was detected in 146 (65%), 2 — in 55 (25%), 3 — in 22 (10%) patients. Prior chronic kidney disease was recorded in 61 (10%) patients. Seventy-one (12%) patients had a high risk of death, 364 (61%) — intermediate risk, and 164 (27%) — low risk. The AKI incidence increased as the severity of PE increased: at low risk of death — 26%, intermediate — 38%, high — 59% (pConclusion. Thirty-seven percent of Russian patients with PE have AKI diagnosed by baseline creatinine. In 2/3 of the examined patients, stage 1 AKI is observed. The AKI incidence increases as the severity of PE increases. The presence of AKI reclassifies patients into a higher risk category for death and is associated with a significant increase in inhospital mortality.

The synthesized sample of covellite was investigated by NQR 63,65Cu methods. The experimentally determined values of the quadrupole frequency νQ and the asymmetry parameter η have been used to study the distribution of electron density in the region of the quadrupole nuclei in position Cu1. It is found that Cu-S bond in position Cu1 is ionic, and the gap LUMO-HOMO decreases with increasing temperature.

The results of studying natural samples of CuFeS2 chalcopyrite mineral from hydrothermal ore manifestations of island arcs of the Pacific Ocean by 63Cu nuclear magnetic resonance (63Cu NMR) in a local field at room temperature are presented. The asymmetric shape of the detected resonance lines in the 63Cu NMR spectrum indicates the presence of at least two overlapping lines. The presence of two overlapping central components can be a consequence of the occurrence of regions with different types of structural distortion near the resonant nuclei. These results show that the pulsed 63Cu NMR method can be an effective method for studying the physical properties of deep-sea polymetallic sulfides of the global ocean.

Objective: to assess the long-term safety and efficiency of tocilizumab (TCZ) therapy in patients with early rheumatoid arthritis (RA) of moderate and high activity, who have completed the basic WA19926 study, as well as the rate of sustained drug-free remission.Subjects and methods. A long-term open-label multicenter Phase III extension study (ML28124) was conducted using a group of 49 patients (36 (73.5%) women and 13 (26.5%) men; mean age 53.3±10.8 years) with early RA of moderate and high activity. All the patients received an intravenous infusion of TCZ 8 mg/kg every 4 weeks for 104 weeks (a total of 27 infusions). The safety assessment criteria were the rate and severity of all adverse events (AE), serious AE (SAE), and AE of particular interest; the rate of AE causing drug dosage changes or withdrawal from the study; the frequency of clinically significant laboratory abnormalities. The analysis of efficiency (secondary end points) included changes of DAS8, which was calculated using erythrocyte sedimentation rate (ESR) (DAS28-ESR) and SDAI, the of tender joint count (TJC) and swollen joint count (SJC); the number of patients who had achieved drug-free remission; and the time to a RA exacerbation in patients who had achieved non-drug remission.Results and discussion. The total rate of AE was found to be 69.4%; that of SAE was 10.2%; SAE were 6.9% of the the total number of AE; AE of particular interest were 17.2% of the total number of AE. More than one-third (35.6%) of the AE caused drug dose changes or therapy discontinuation or complete cessation. The laboratory clinically significant abnormalities included those in complete blood cell counts (blood alanine aminotransferase, aspartate aminotransferase, total and direct bilirubin, creatinine, and glucose). The evaluation efficiency analysis showed that the main disease activity measures (DAS28-ESR, SDAI, SJC, and TJC) decreased at 104-week follow-up versus at baseline. The rate of drug-free remission was 71.4%; that of recurrence was 40.0% (the median time to recurrence was 23 weeks). The findings suggest that the safety profile of TCZ is acceptable in the long-term treatment of early RA and correspond to earlier results.

The DFT-PBE method was used to calculate the structural and energy characteristics of all possible adsorption complexes of phenylacetylene (PA) and styrene (St) molecules with the Pd{111} surface. According to the calculations, the acetylene group of the PA molecule in the energetically preferred complexes is bound to three or four Pd atoms and the plane of the phenyl ring is parallel to the Pd surface. In the case of deficiency of fcc sites, the phenyl ring of the PA molecule is displaced from the Pd{111} surface. The number of Pd atoms required for the compact adsorption of two PA molecules is the same as that for the adsorption of one St molecule.

The results of investigating natural samples of chalcopyrite mineral CuFeS2 from massive oceanic sulfide ores of the Mid-Atlantic ridge by the 63Cu nuclear magnetic resonance (NMR 63Cu) in a local field at room temperature are presented. The significant width of the resonance lines found in the 63Cu NMR spectrum directly testifies to a wide distribution of local magnetic and electric fields in the investigated chalcopyrite samples. This distribution can be the consequence of an appreciable deviation of the structure of the investigated chalcopyrite samples from the stoichiometric one. The obtained results show that the pulsed 63Cu NMR can be an efficient method for studying the physical properties of deep-water polymetallic sulfides of the World Ocean.

The mechanism of formation of 1,5-diketone, a key intermediate in the stereoselective assembly of 2,3,4-trimethyl-7-methylidene-1,5-di(thiophen-2-yl)-6,8-dioxabicyclo[3.2.1]octane, and the conformational structures of the reagents and products were studied by quantum chemical calculations at the MP2/6-311++G**//B3LYP/6-31G* level of theory with solvent effects treated within the polarizable continuum model. The activation barrier for the addition of the 1-oxo-1-(thiophen-2-yl)propan-2-ide ion (1), which is generated from 1-(thiophen-2-yl)propan-1-one, to acetylene is 15.2 kcal mol–1. The reaction affords β,γ-unsaturated ketone. The subsequent addition of carbanion 1 to the C=C double bond of the resulting β,γ-unsaturated ketons yields 1,5-diketone and occurs with insignificant (less than 1 kcal mol–1) activation barrier.

Aim.To study the clinical course and management of patients with pulmonary embolism (PE) of various age groups hospitalized in a cardiology hospital.Material and methods.This prospective single-center study in the period from 2016 to 2018 included 154 patients with PE verified by computed tomography. Statistical processing was conducted using the MedCalcVersion 16.2.1 software package (Softwa, Belgium).Results.In all groups, female patients dominated, but the highest number of women (70,7%) belonged to the group of senile patients, while in the group Conclusion.In the Kemerovo Oblast, PE most often develops in patients aged 60-75 years and is characterized by a more severe clinical course compared with patients younger than 60 years. Patients over the 60 years of age have severe cardiovascular comorbidity status, atrial fibrillation/flutter and recurrent PE. Surgical treatment for senile patients is limited due to the high risk of postoperative complications, which specifies high mortality. Patients

to study the role of clinical factors and treatment tactics in the development of adverse outcomes in patients with non-STelevation acute coronary syndrome (NSTEACS) for five years follow up.The study included 415 patients with NSTEACS observed for 60±4 months from the date of discharge. Adverse events (AE) were unstable angina (UA), myocardial infarction (MI), ischemic stroke or patient's death. The favorable 5-year outcome was identified in 200, adverse outcome - in 178 patients. Statistical processing was performed using STATISTICA 8.0 software package and MedCalc 11.0.Total mortality for 60 months was 16.1% (n=61). Highest mortality, maximal number of hospitalizations due to MI (41.1%) and UA (49.6%) were observed during first 12 months. Greatest impact on the development of AE produced: history of MI, higher GRACE score, age54 years (p=0.0021), stenosis in brachiocephalic arteries, ischemic changes on admission ECG and reduced left ventricular ejection fraction (LVEF) ≤ 52% (p=0.0001). Five-year survival rate was influenced by coronary angiography (p=0.0001) and percutaneous coronary intervention (PCI) during index hospitalization (p=0.0039).Mortality and frequency of AE in this register were close to generally reported. The observed adverse impact on the long-term outcome of such risk factors such as age over 60 years, history of MI, ischemic ECG changes at admission, decreased LVEF corresponded to literature data. Our study proved the need for coronary imaging in patients with NSTEACS to determine treatment strategy in in hospital period as these factors affected the 5-year survival.During five-year follow-up AE occurred in 47.1% of patients with NSTEACS. Maximal number of patients with AE was registered during the first (24.7%) and fifth (13.8%) year of observation. The development of AE was associated with older age, history of MI and coronary artery bypass grafting, ischemic ECG changes, decreased LVEF, higher GRACE score, and brachiocephalic artery stenosis. Carrying out diagnostic coronary angiography and PCI with stenting during hospitalization in patients with NSTEACS was associated with favorable 5-year outcome.

The geometric structures of the conformers of 2,3,4-trimethyl-7-methylidene-1,5-di-(thiophen-2-yl)-6,8-dioxabicyclo[3.2.1]octane were studied by the MP2/6-311++G**// B3LYP/6-31G* method. The average deviations of the calculated bond lengths from those determined by X-ray diffraction are no larger than 0.02 A, and the deviations of the bond angles and dihedral angles are 0.7 and 3.5°, respectively. In the gas phase, the chair conformation is thermodynamically ∼7.3 kcal mol−1 more favorable than the boat conformation, and, taking into account the solvent effect of DMSO, the chair conformation is ∼9.4 kcal mol−1 more favorable. For formaldehyde and a series of ketones, including alkyl hetaryl ketones, the enthalpies of competitive formation of acetylenic alcohols and 7-methylidene-6,8-dioxabicyclo[3.2.1]octanes were calculated. The formation of the latter compounds is characterized by a considerable decrease in the enthalpy (to −91 kcal mol−1).

Alkyl aryl (hetaryl) ketones react with acetylene under atmospheric pressure in the superbasic system KOH-EtOH-H2O-DMSO at 10–15°C (2 h) to give the corresponding tertiary propargyl alcohols in up to 91% yield. The procedure requires no large excess of KOH and low-boiling inflammable solvents, produces few wastes, and is safe and convenient on the laboratory scale; there are no limitations for its large-scale application.

To study the relationship between polymorphous G-1082A (rs3024491) and C-592A (rs1800872) variants of the IL10 gene and multifocal atherosclerosis (MFA) in patients with acute coronary syndrome (ACS) without segment ST elevation.Genotypes of polymorphous G-1082A (rs3024491) and C-592A (rs1800872) variants of the IL10 gene were determined in 178 patients. Interleukin-10 (IL-10) level was measured in 93 of them using solid-phase immunoenzymatic assay. All patients underwent visualization of coronary and peripheral arteries.C-592A (rs1800872) polymorphism ofthe IL10 gene tended to be associated with type 2 diabetes mellitus in the carriers of the CC (gg) genotype and with elevated concentration of high density lipoproteins and reduced intima-media thickness in the carriers of the AA (tt) genotype. The frequency of MFA in the carriers of different genotypes of rs1800872 polymorphism was practically identical while the CA(gt) genotype was associated with more severe manifestations of atherosclerosis and AA(tt) genotype with lower frequency of peripheral artery stenosis. Patients with AA(tt) genotype of rs1800872 polymorphism had higher IL-10 levels. No relationship between rs3024491 polymorphism of IL10 gene, blood IL-10 level, clinical risk factors, and MFA was documented.CC (gg) genotype of C-592A (rs1800872) polymorphism of the IL10 gene in patients with acute coronary syndrome (ACS) without segment ST elevation was associated with type 2 diabetes mellitus while the AA(tt) genotype of the same polymorphism with elevated concentration of high density lipoproteins, reduced intima-media thickness, low frequency of peripheral artery stenosis, and increased IL-10 production. CA (gt) genotype of rs1800872 polymorphism of the IL10 gene was associated with MFA.

Compounds of indole series occur widely in wild� life. Among them are important regulators of physio� logical processes (serotonin, melatonin, tryptophan, etc.); indole alkaloids, including those widely used in medicine (reserpine, strychnine, brevicolline, aima� line, vincamine, physostigmine, etc.); synthetic phar� maceuticals (indopan, indomethacin, etc.); hor� mones; enzymes (tryptophanase); hallucinogens [1]. Indole is used as a starting material for the indus� trial synthesis of heteroauxins (3�indolylacetic and 3� indolylbutyric acids) and tryptophan and in perfumery industry for designing fragrance compositions as per� fume fixative. 4,5,6,7�Tetrahydroindole ( I) can be used for the synthesis of indole and its not readily available 2�func� tionalized derivatives. As distinct from indole itself, which reacts with electrophilic agents only at the 3�position, indole I, being pyrrole in chemical nature, undergoes selective functionalization at the 2�position in similar reactions. The further dehydrogenation of 2�functionalized tetrahydroindoles leads to 2�func� tionalized indoles, for example, methylisotryp� tophan [2].

The electronic absorption spectra of radical cations of dipyrroles with a phenylene bridge were studied by laser flash photolysis and quantum chemical methods. Intense absorption bands of the radical cations in the visible region (λmax ≈ 500 nm, emax > 2 · 104 L mol−1 cm−1) are caused by excitation of electrons from single occupied MOs to the LUMO. In the near IR region, calculations predict additional, relatively intense (f≈ 0.27–0.29) electronic transitions associated with excitation of electrons from low-lying MOs to the single occupied MO.

Polymers of 1,4-bis[2-(N-vinyl)pyrrolyl]benzene with free N-vinyl groups in side chains are synthesized in the presence of AIBN (2–5 wt %, 70°C) with a yield of up to 34% and a molecular mass of up to 11.5 × 103. In the presence of cationic catalysts (Me3SiCl, the LiBF4-dimethoxyethane system, and BF3 · OEt2), 1,4-bis[2-(N-vinyl)pyrrolyl]benzene gives macromolecules with alternating 1,2-pyrrolene and ethylidene units in the backbone with yields of 80, 44, and 33%, respectively. The polymers demonstrate paramagnetic and luminescent properties.

The synthesized base compound CuGaTe2 and the Mn-doped compounds Cu0.97Ga0.97Mn0.06Te2 and Cu0.93Ga0.93Mn0.14Te2 have been investigated by using 63Cu and 69Ga NMR spectroscopy. The NMR spectra obtained testify to substantial structural distortions near the cationic positions in the crystal lattice of Mndoped samples. The constants of quadrupole coupling between 63Cu and 69Ga nuclei in the compounds investigated have been estimated.

Objective. To assess efficacy and safety of two-month therapy with Potentilla tincturecombined with diclofenac in pts with osteoarthritis (OA) in comparison with diclofenac monotherapy (control group). Material and methods. 60 pts with OA aged 50-75 years were randomly assigned into two groups 30 pts in each. Knee OA was diagnosed according to ACR criteria (1991). Pain at movement, WOMAC index (on VAS), efficacy assessment by physician and pt and diclofenac requirement were used as outcome measures. 1 tea spoon of Potentilla tincture diluted in 1/3 of water glass was given twice a day before meal as well as diclofenac 100 mg/ day during 2 months. Results. Combined therapy provided prominent decrease of pain which exceeded improvement in control group. Significant decrease of summated WOMAC index was achieved only in the main group. Conclusion. Combination of Potentilla tincture and diclofenac was significantly more effective than treatment with diclofenac alone in OA. Combined therapy provided more pronounced analgesic and anti-inflammatory effect than diclofenac monotherapy.

The deprotonation reaction of radical cations of 1-(2-pyrrolyl)-4-(1-vinyl-2-pyrrolyl)benzene has been studied by nanosecond laser photolysis. Bimolecular rate constants have been determined for the transfer of proton to the heterocyclic base. Analysis of the yields has been carried out of the final products of the radical-cation reaction of 1-(2-pyrrolyl)-4-(1-vinyl-2-pyrrolyl)benzene in the presence and absence of bases. Comparison of the results of impulse and stationary photolysis showed that inhibition of the radical cation reaction occurs at the stage of forming the radical cations.

Trimesityl-BODIPY (TMB), a new derivative of 4,4-difluoro-4-bora-3a,4a-diaza-s-indacene belonging to fluores-cent nanostructures series, was synthesized from the corresponding pyrrole by the Trofimov reaction. This reaction was also employed to obtain 2-[2.2]paracyclophanylpyrrole from 5-acetyl[2.2]-paracyclophane. The spectral properties of TMB have been investigated in dichloromethane, nanolatex (polystyrene) films prepared by rapid solvent evaporation, and microcrystals. Comparative analysis of TMB properties with those of mesityl-BODIPY (MB) was performed. TMB was prepared to minimize π-π interactions in order to preserve luminescence in the aggregate state. Both fluorophores were shown to form fluorescing aggregates in the amorphous state (film). Fluorescence spectra (extinction and lifetime) were also studied. In crystal, MB shows a weaker fluorescence, while TMB behaves as a single fluorescing aggregate with a lifetime of 9.5 ns.

Quaternary ammonium compounds based on resin (diterpene) acids and rosin were prepared. Their colloidal-chemical and biocidal properties were studied, and possible applications were outlined.

2-Arylazo-1-vinylpyrroles (a new group of azo dyes of the pyrrole series) are polymerized under heating (80°C) without initiators and in the presence of AIBN to form intensely colored paramagnetic and conducting polymers with a yield of 92%. By the example of the thermal copolymerization with 1-vinylpyrrolidone, it has been shown that 2-arylazo-1-vinylpyrroles may simultaneously play the roles of initiators and comonomers.

1-Vinyl-2-phenylazopyrroles react with alcohols in the presence of acids or PdCl2 to give 1-(1-alkoxyethyl)-2-phenylazopyrroles in up to 49% yield. In the presence of trifluoroacetic acid 2-methylquinoline unexpectedly formed (yield up to 26%) involving into the reaction the phenylazo and 1-vinyl groups.

Fluorescence quenching of 1,4-bis(1H-pyrrol-1-yl)benzene, 1-(1H-pyrrol-2-yl)-1-(1-vinyl-1H-pyrrol-1-yl)benzene, and 1,4-bis(1-vinyl-1H-pyrrol-2-yl)benzene with chloromethanes (methylene chloride, chloroform, and carbon tetrachloride) in solvents with different polarities follows electron-transfer mechanism. The occurrence of an electron-transfer step is confirmed by formation of short-lived pyrrolylbenzene radical cations. An exception is quenching of fluorescence of 1,4-bis(1-vinyl-1H-pyrrol-2-yl)benzene in n-hexane in the presence of CCl4 and CHCl3 and in pure CCl4. In this case, neutral 1,4-bis(1-vinyl-1H-pyrrol-2-yl)benzene·-Cl radical is formed via recombination of 1,4-bis(1-vinyl-1H-pyrrol-2-yl)benzene radical cation and chloride anion. A relation was found between the nature of the short-lived species detected by laser photolysis and stable product obtained by stationary photolysis.

Conformational analysis of ten N-vinylpyrroles was performed on the basis of experimental 13C-1H and 13C-13C coupling constants and those calculated by high-level quantum-chemical methods, and principal relations between J CC and J CH values and stereochemical structure of these compounds were revealed.

The results of high-level nonempirical quantum-chemical calculations of 13C-13C coupling constants in twelve heteroaromatic ketone oximes are well consistent with the available experimental data. In all the examined compounds, the effect of the unshared electron pair on the oxime nitrogen atom on JCC strongly predominates over conformational and electronic effects, which makes it possible to unambiguously assign JCC values to different configurations.

According to the 1H and 13C NMR data, bifurcated intramolecular hydrogen bond NH⋯N⋯HN in 2,6-bis(2-pyrrolyl)pyridine fixes its molecule in a conformation with syn orientation of the pyrrole rings. An analogous bifurcated hydrogen bond CH⋯N⋯HC is formed in 2,6-bis(1-vinyl-2-pyrrolyl)pyridine. 2-(1-Vinyl-2-pyrrolyl)-6-(2-pyrrolyl)pyridine is characterized by unsymmetrical bifurcated hydrogen bond NH⋯ N⋯HC.

1-Allenylpyrazole and 1-allenyl-1,2,4-triazole react with hydrogen chloride via proton addition at the pyridine-like nitrogen atom (N2 and N4, respectively). In the reaction with 1-allenylpyrazole, 1-[(E)-3-chloro-1-propenyl]pyrazole is also formed via regio- and stereoselective addition of hydrogen chloride to the propadienyl group. 1-Allenylpyrazole and 1-allenyl-1,2,4-triazole act as unidentate ligands with respect to Co, Ni, Cu, Zn, Cd, Pd, and Sn, the donor centers being N2 and N4, respectively. Apart from mononuclear coordination compounds, 1-allenylpyrazole gives rise to polymeric complexes which contain units and blocks formed by the free ligand.

Steroidal ketone oximes, namely pregnenolone oxime, Δ5-cholesten-3-one oxime, and progesterone dioxime react with acetylene in superbasic systems (Trofimov reaction) to afford steroid-pyrrole assemblies. The process is accompanied by prototropic migration of double bonds in the steroid fragment and vinylation of hydroxy groups in sterols with acetylene. The O-vinyl group can readily be removed by methanolysis.

The product of annelation of the N-vinyl pyrrole ring with steroid skeleton of 5-cholestene was obtained in a single regioselective step by the reaction of 5-cholesten-3-one oxime with acetylene in KOH-DMSO by the Trofimov reaction.

In 2-(2-furyl)- and 2-(2-thienyl)pyrroles the heterocycles are in efficient π,π-conjugation. The presumably possible for this compounds intramolecular hydrogen bond N-H···O or N-H···S is lacking. The COCF3 group in position 2 of the pyrrole ring is syn-oriented with respect to pyrrole fragment, and the orientation is fixed by an intramolecular hydrogen bond N-H···O. However no bifurcating hydrogen bonds arise in the molecules containing COCF3 group.

To make a prediction scale using a set of clinical and laboratory prognostic variables for patients with ST-segment elevation myocardial infarction (STEMI) on their electrocardiograms (ECG) who have undergone percutaneous coronary intervention.The study enrolled 154 STEMI patients who had undergone percutaneous coronary angioplasty with stenting of the artery supplying blood to the area of an ischemia. Enzyme immunoassay was used to determine the levels of cytokines and selectins on days 1 and 10.During a year, the authors identified the following independent risk factors of a poor outcome (PO): symptoms of Killip Class III-IV heart failure; a history of smoking; rhythm and conduction disturbances on day 1 of the disease; tumor necrosis factor-alpha levels determined on day 1 of the disease; IL-1alpha and sP-selectin levels measured on day 10. In the made prediction scale, a score was assigned to each independent prognostic variable to estimate the risk of PO. The borderline values of summing the scores, which divided the patients into groups at low, moderate, and high risk for PO, were determined for the practical application of the scale, by using the receiver operating characteristic curve (ROC) analysis. The performance of the model was tested using an independent sample of STEMI patients (n = 50). The probability that the classification of the model was correct amounted to 89.6% (p0.0001).The made prediction scale allows the patients with STEMI to be allocated to groups at low, moderate, high risks for PO during a year. Evaluation of the prognostic efficiency of the new scale versus the known scales PAMI, CADILLAC, and TIMI ST elevation, by plotting the ROC curve and estimating the area under the latter, demonstrated the high predictive ability of the new scale.

The oxime of Δ5-pregnen-3β-ol-20-one reacts with acetylene in the KOH-DMSO system to form a mixture of products containing 3β-vinyloxy-17β-(1-vinyl-2-pyrrolyl)-Δ5-androstene as the major product (63% yield) and also the O-vinyloxime of 3β-vinyloxy-Δ5-pregnen-20-one (10%), 3β-vinyloxy-17α- and 3β-vinyloxy-17β-Δ5-pregnen-20-one (25%, 1:4 ratio).

Ketone oxime O-vinyl ethers having alkyl or phenyl radicals react with trifluoroacetic anhydride in ether in the presence of pyridine, yielding 43-54% of the corresponding ketone oxime O-(trans-4,4,4-trifluoro-3-oxo-1-butenyl) ethers with high stereoselectivity.

Literature data on the alkylation of phenolic compounds by mono-, sesqui-, and diterpenes in the presence of various catalysts are reviewed.

273 In recent years, the synthesis of 4,5,6,7-tetrahydroindole (THI) and N -vinyl-4,5,6,7-tetrahydroindole (VTHI) from cyclohexanone oxime (CHO) and acetylene in MOH–DMSO superbasic media (M is Li, Na, K, Cs; DMSO is dimethyl sulfoxide), an example of pyrrole synthesis by the Trofimov reaction [1–4], has attracted the attention of synthetic chemists [5–8] and chemical engineers [9, 10] as a simple and efficient route to indole and its derivatives (Scheme 1).

Consecutive treatment of ketones with a system NH2OH·HCl-NaHCO3-DMSO and acetylene excess in the presence of KOH (100–120°C, initial acetylene pressure 12–15 at) led to a selective formation of 2-and 2,3-substituted 1-vinylpyrroles in 51–79% yields. Ketones of aliphatic, cycloaliphatic, aromatic, heteroaromatic, and tetrahydronaphthalene series were involved into the reaction.
