Müllerianosis is a term used to indicate lesions composed of an admixture of two or three types of müllerian-derivation glands in heterotopic sites. In this report we describe a case of spinal cord müllerianosis which occurred in a 42-year-old woman. The patient had suffered from catamenial lumbago and sciatica of three years duration before undergoing laminectomy of L2-L3 with excision of a polypoid mass that compressed nerve trunks. At histological examination, the lesion was composed of endocervical, endometrial and tubal glands within a smooth muscle nodule. These features were consistent with a diagnosis of müllerianosis. This is a very uncommon form of presentation of müllerianosis that must be correctly identified since patients can benefit from hormonal therapy.

Unter dem Druck einer landesweiten krassen Unterversorgung der wachsenden Gruppe von Patienten mit Doppeldiagnosen wurde 1993 in den Universitären Psychiatrischen Diensten Bern ein stationäres Behandlungsangebot für Patienten mit substanzgebundenem Suchtverhalten und einer anderen schweren psychischen Störung eingerichtet. Der Schwerpunkt der Forschung lag in der Entwicklung, Beschreibung und Evaluation einer Übergangstherapie zwischen Krisenintervention und Rehabilitationseinrichtung, die sich durch ein integratives Behandlungsangebot unter Berücksichtigung der besonderen Merkmale von Doppeldiagnosepatienten auszeichnet. Zur Evaluation der Behandlungseffekte und des Verlaufs wurden bei Eintritt, während der Behandlung und nach Ablauf eines Jahres nach Austritt (1-Jahres-Katamnese) verschiedene störungsspezifische und psychosoziale Merkmale erfaβt. Während die Ergebnisse des PräKatamnese-Vergleichs in Bezug auf bestimmte Indikatoren eines erfolgreichen Beginns der Rehabilitation sowie in Bezug auf psychopathologische Symptome ermutigend ausfallen, verringert sich der Suchtmittelkonsum nicht. Jedoch wiesen Patienten, die vor der Behandlung extrem häufig Suchtmittel einnahmen, eine deutliche Verringerung ihres Suchtmittelkonsums auf. Verlaufsbeobachtungen während der stationären Behandlung zeigen, daβ polytoxikoman Schizophrene zwar besser lernen, keine oder weniger häufig Suchtmittel zu konsumieren als Patienten mit anderen Doppeldiagnosen (z.B. Persönlichkeitsstörungen und Alkoholabhängigkeit). Dieser Lernfortschritt kann aber von Schizophrenen nicht stabilisiert und in die Realsituation transferiert werden. Signifikante Korrelationen zwischen der Häufigkeit suchtmittelbezogener Rückfälle während der stationären Behandlung und akuten psychotischen Symptomen sowie hoher Reizbarkeit verweisen auf mögliche Zusammenhänge zwischen Sucht und psychischer Störung., Because of a nationwide lack of medical care of thc increasing group of patients with dual diagnoses in the Psychiatrie Services of the University of Berne in 1993. a ward was established for patients with mental and sub­stance disorders. The research concentrates on development, description and evaluation of an inpatient transitional therapy between crisis interven­tion and rehabilitation. The transitional therapy was integrative and consi­ders both disorders. At the beginning, during the treatment and I year af­ter the treatment assessments (c.g frequency of substance consumption, seriousness of psychopathology, subsistence) were completed to evaluate the transitional therapy. The comparison between assessments at the beginning of treatment and 1 year after the treatment reveals that psychopathology decreased, hous­ing and subsistence improved, but substance abuse did not change. Only patients who frequently took substances before treatment consumed less at follow-up. During the treatment schizophrenics with polysubstance abuse learned better to consume no or less substances than patients with other dual diagnosis (e.g. personality disorders and alcohol dependence) but they cannot stabilize and generalize this progress for everyday life. Acute psychotic symptoms and high irritability of patients with other dual diagnoses correlate with substance relapses during the treatment. These results refer to a possible interdependency of substance disorders and other mental disorders.

18 subjects with symptomless HIV infection were investigated with multimodal evoked potentials for possible CNS involvement and again after an 8-12 month interval. 13 subjects showed neuropsychological changes, which were confirmed at the second examination. The 5 subjects found normal remained so at the second examination. On WAIS assessment the only patient to earn pathological scores was the one with the greatest evoked potentials changes. Thus the evoked potentials procedure proved capable of identifying early CNS involvement by HIV infection.

Study Design.Retrospective multicenter study.Objective.Analysis of impact of conservative and surgical treatments on functional outcome of geriatric odontoid fractures.Summary of Background Data.Treatment of odontoid fractures in aged population is still debatable.Methods.One hundred fourty-seven consecutive odontoid fractures in elderly patients were classified according to Anderson-D'Alonzo and Roy-Camille classifications. Philadelphia type collar was always positioned and kept as a treatment whenever acceptable. Halo-vest, anterior screw fixation, C1-C2 posterior arthrodesis, and occipito-cervical fixation were the other treatments adopted. Conservative or surgical treatment strategy was more significantly influenced by antero-posterior displacement (< or >5 mm) and by surgeon decision. On admission ASA, modified Rankin scale (mRS-pre) and Charlson Comorbidity Index (CCI) were assessed. Modified Rankin scale (mRS-post), Neck Disability Index (NDI), and Smiley Webster Pain Scale (SWPS) were administered 12 to 15 months after treatment to estimate functional outcome in terms of general disability, neck-related disability, and ability to return to work/former activity. Risk of treatment crossover was calculated considering factors affecting outcome. Fracture healing process in terms of fusion-stability, no fusion-stability, no fusion-no stability was evaluated at 12 months through a cervical computed tomography (CT) scan. Dynamic cervical spine x-rays were obtained whether necessary. No fusion-stability was considered an adequate treatment goal in our geriatric population. Chi square/Fisher exact test and logistic regression were performed for statistical anal.Results.Overall 67 patients were treated conservatively whereas 80 underwent surgery. Collar was adopted in 45 patients, while anterior odontoid fixation and C1-C2 posterior arthrodesis were preferred for 30 patients each. 79.8% of patients showed good outcomes according to NDI. No significant differences were observed between patients of 65 to 79 years and more than or equal to 80 years (P=0.81). Background: Spinal cord stimulation (SCS) is a technique used worldwide to treat several types of chronic neuropathic pain refractory to any conservative treatment. The aim of this data collection is to enforce evidence of SCS effectiveness on neuropathic chronic pain reported in the literature and to speculate on the usefulness of the trial period in determining the long–term efficacy. Moreover, the very low percentage of undesired side effects and complications reported in our case series suggests that all implants should be performed by similarly well-trained and experienced professionals. Method: A multicentric data collection on a common database from 11 Italian neurosurgical departments started 3 years ago. Two different types of electrodes (paddle or percutaneous leads) were used. Of 122 patients, 73 % (N = 89) were submitted to a trial period, while the remaining patients underwent the immediate permanent implant (N = 33). Statistical comparisons of continuous variables between groups were performed. Results: Most of the patients (80 %) had predominant pain to their lower limbs, while only 17 % of patients had prevalent axial pain. Significant reduction in pain, as measured by variation in visual analogue scale (VAS) score, was observed at least 1 year after implantation in 63.8 % of the cases, 59.5 % of patients who underwent a test trial and 71.4 % of patients who underwent permanent implant at once. No statistical differences were found between the lower-limb pain group and the axial pain group. Conclusions: No relevant differences in long-term outcomes were observed in previously tested patients compared with patients implanted at once. Through this analysis we hope to recruit new centres, to give more scientific value to our results. © 2015, Springer-Verlag Wien

Una delle sfide maggiori che le società contemporanee si trovano a dover affrontare è quella della conciliazione tra famiglia e lavoro. Negli ultimi anni si stanno moltiplicando gli studi e le ricerche su questo argomento e le soluzioni proposte sono molteplici, ma la strada da fare è ancora molta e in salita, soprattutto in Italia, dove prevale un contesto culturale caratterizzato da una divisione tradizionale dei ruoli. poichè manca una visione relazionale, le politiche di conciliazione oscillano tra l'essere politiche delle pari opportunità, politiche per l'infanzia ecc senza che la famiglia abbia in realtà alcuna presenza come soggetto sociale. la risposta può venire dal passaggio da una visione di tipo lavoristico a una visione relazionale della conciliazione che abbia come protagonista la famiglia come soggetto sociale
