The antiangiogenic properties of cabozantinib led to its use as a monotherapy for the treatment of metastatic renal cell cancer (RCC), and quickly resulted in this treatment becoming part of the standard of care for these tumors. Since the advent of immune checkpoint inhibitors (ICIs), new standards of care have emerged in first-line settings, involving dual ICI or ICI–VEGF-TKI (including ICI–cabozantinib) combination treatments, and leading to a more complex algorithm of care. Cabozantinib remains an option in second-line settings and is still a first-line standard of care treatment in cases where the use of ICIs is contraindicated. This review focuses on the selection of patients who may benefit most from cabozantinib therapy, including those with bone and brain metastases and those with a non-clear cell RCC histology. The need to consider disease-related symptoms, comorbidities, age, drug interactions and biomarker analyses in the choice of therapeutic strategy is also highlighted. Finally, the perspectives for the use of cabozantinib in RCC treatment are discussed.Keywords: renal cancer, cabozantinib, patient selection, prognosis, efficacy, safety

International audience; The tumor microenvironment is a complex ecosystem almost unique to each patient. Most of available therapies target tumor cells according to their molecular characteristics, angiogenesis or immune cells involved in tumor immune-surveillance. Unfortunately, only a limited number of patients benefit in the long-term of these treatments that are often associated with relapses, in spite of the remarkable progress obtained with the advent of immune checkpoint inhibitors (ICP). The presence of “hot” tumors is a determining parameter for selecting therapies targeting the patient immunity, even though some of them still do not respond to treatment. In human studies, an in-depth analysis of the organization and interactions of tumor-infiltrating immune cells has revealed the presence of an ectopic lymphoid organization termed tertiary lymphoid structures (TLS) in a large number of tumors. Their marked similarity to secondary lymphoid organs has suggested that TLS are an “anti-tumor school” and an “antibody factory” to fight malignant cells. They are effectively associated with long-term survival in most solid tumors, and their presence has been recently shown to predict response to ICP inhibitors. This review discusses the relationship between TLS and the molecular characteristics of tumors and the presence of oncogenic viruses, as well as their role when targeted therapies are used. Also, we present some aspects of TLS biology in non-tumor inflammatory diseases and discuss the putative common characteristics that they share with tumor-associated TLS. A detailed overview of the different pre-clinical models available to investigate TLS function and neogenesis is also presented. Finally, new approaches aimed at a better understanding of the role and function of TLS such as the use of spheroids and organoids and of artificial intelligence algorithms, are also discussed. In conclusion, increasing our knowledge on TLS will undoubtedly improve prognostic prediction and treatment selection in cancer patients with key consequences for the next generation immunotherapy.

CTLA-4 (cytotoxic T lymphocyte antigen-4), PD-1 (programmed cell death protein 1) or PDL-1 (programmed cell death ligand protein 1) represent ICIs that can be used as monotherapy or in combination with other agents. The toxicity profiles of ICIs differ from the side effects of cytotoxic agents and come with new toxicities like immune-related adverse events. Typically, these toxicities occur in all organs. However, the main organs affected are the skin, digestive, hepatic, lungs, rheumatologic, and endocrine. Most of the immune toxicity that occurs is low grade but some more severe toxicities can occur that require a rapid diagnosis and appropriate treatment. The recognition of symptoms by physicians and patient is necessary to resolve them rapidly and adapt treatment to allow the toxicity to resolve.Keywords: immune check point inhibitor, toxicity, corticosteroids, immunosuppressive treatments

National audience; Immune checkpoint inhibitors (ICIs) have improved cancer prognosis but have not been evaluated specifically in sarcomatoid carcinoma (SC), a rare lung cancer subtype with poor prognosis. As such, our study sought to retrospectively assess the efficacy of ICI in SC.

Gamma delta (γδ) T cells are sentinels of the intestinal barrier, protecting the host from aggressions. In cancer, they have a crucial role in immunosurveillance and the stromal infiltration of γδ T cells is the more important prognostic factor among all immune cells. Indeed, this population could be an interesting therapeutic target in oncoimmunology. The activation of γδ T cells is tightly regulated by several cytokines and especially interleukin-18 (IL-18). IL-18 is a proinflammatory cytokine corresponding to the most powerful soluble activator of γδ T cells, involved in tissue homeostasis in the epithelial barriers. It is secreted through the activation of the inflammasome, a cytosolic complex consisting in Nod-Like Receptors (NLRs). However, in vitro, some cancer cell lines do not secrete IL-18, probably due to a loss of expression and/or activation of inflammasome components. We have assessed the protein expression in vivo of inflammasome components, and their correlation with clinical outcome of colorectal cancer patients. The expression was evaluated in 104 colorectal tumours, matched with normal tissue for each patient, through an immunofluorescence assay on a tissue micro-array (TMA). We also performed a cytokeratin staining to assess the expression of each components specifically in epithelial cells from tumour or normal tissue. We showed a decreased expression of all inflammasome components, including caspase-1 and cytokines, in the tumour epithelium compared to normal tissue. However, only the expressions of NLRC4, NLRP6 or IL-18 were associated to a more aggressive disease and to a worse outcome. Furthermore, combined downregulation of these 3 components was correlated to a poor prognosis. Indeed, only 22.2% of patients NLRC4low/NLRP6low/IL-18low were still alive after 5 years, versus 85.2% for NLRC4high/NLRP6high/IL18high patients. Furthermore, the downregulation of NLRC4 and IL-18 was associated with a decreased immune infiltration especially for CD3 positive T cells. We also showed in vitro that NLRC4 reexpression was associated to a decreased proliferation of metastatic colon cancer cell line. Finally, at a transcriptomic level, the expression of NLRC4 in these cell lines was correlated to an interferon type I signature. All these results show that the downregulation of NLRC4, NLRP6 and IL-18 by tumour cells could constitute a resistance mechanism to the antitumor immune response and seems to be an important prognostic factor. The activation of γδ T cells could also been regulated by molecules that are not cytokines, depending on tumour metabolism. Indeed, we showed that the activation of AMPK in colon cancer cell lines induced the expression of the stress antigen receptor EphA2, recognised by human γδ T lymphocytes. Furthermore, the in vitro induction of AMPK activation favoured the released of small soluble molecules that modulate the immune response. Indeed, after several chromatography of the supernatant from reprogrammed cancer cell lines, we have identified a peptide that is able to inhibit the activation of B cells in an in vitro model. We are trying to identify the mechanisms underlying this inhibition. In conclusion, we have identify potential therapeutic targets in cancer treatment, alone or in association with chemotherapies and/or immunotherapies. This could help to improve antitumor immune response and patient outcomes., Les lymphocytes T gamma delta sont des sentinelles des barrières épithéliales qui jouent un rôle majeur dans la défense contre les agressions. Dans le cancer, elles participent à l'immunosurveillance et leur présence au sein du stroma tumoral constitue le principal facteur pronostic positif. Il s’agit donc d’une population qui pourrait constituer une cible thérapeutique privilégiée en oncoimmunologie. La fonction des LTgd est finement régulée par un ensemble de cytokines et en particulier par l’interleukine 18 (IL-18). L’IL-18 est une cytokine proinflammatoire correspondant à l’un des plus puissants stimulateurs solubles des LTgd, impliquée dans l’homéostasie tissulaire au niveau des barrières épithéliales. Elle est sécrétée grâce à l’activation de l’inflammasome, complexe cytosolique composé de Nod-Like Receptors (NLRs). Néanmoins, in vitro, certaines lignées cancéreuses n’en sécrètent plus, probablement du fait de la perte d'expression des constituants de l'inflammasome.Nous avons donc voulu évaluer l’expression protéique in vivo des constituants des inflammasomes, et leur impact pronostic en clinique. L’expression protéique a été analysée dans 104 tumeurs colorectales appariées aux tissus sains, par immunofluorescence sur un tissue micro-array (TMA). Un comarquage de la cytokératine a été effectué pour évaluer l’expression de ces composants spécifiquement dans les cellules épithéliales, qu’elles soient tumorales ou saines. Nos résultats ont montré une diminution de l’expression de l’ensemble des constituants de l’inflammasome dans l’épithélium des tissus tumoraux, ainsi que de la caspase-1 et des interleukines. Néanmoins, seules les pertes d’expression de NLRC4, de NLRP6 ou de l’IL-18 étaient associées à une maladie plus avancée ainsi qu’à la mortalité des patients. De plus, la perte combinée de ces 3 marqueurs était associée à un pronostic encore plus mauvais. Ainsi, seulement 22.2% des patients NLRC4bas/NLRP6bas/IL-18bas étaient encore vivants à 5 ans contre 85.2% pour les patients NLRC4haut/NLRP6haut/IL-18haut. De plus, la perte d’expression de NLRC4 et de l’IL-18 semblait corrélée à une diminution plus importante de l’infiltrat en cellules immunitaires et en particulier en lymphocytes T CD3+. Nous avons également montré grâce à un modèle in vitro que la réexpression de NLRC4 était associée à une diminution de la prolifération des cellules tumorales coliques métastatiques. Enfin, en trancriptomique, cette réexpression de NLRC4 induisait l’expression de médiateurs de la voie de l’interféron de type I. Dans l’ensemble, ces résultats montrent que la perte d’expression protéique de NLRC4, NLRP6 et IL-18 par la tumeur pourrait constituer un mécanisme de résistance à l’immunité antitumorale et semblerait être un facteur pronostic majeur.Dans un second temps, nous avons étudié la régulation des LTgd par des molécules solubles non cytokiniques, dépendantes du métabolisme tumorale. Ainsi, nous avons montré que l’activation de l’AMPK dans les cellules cancéreuses de côlon permettait d’augmenter l’expression d’un antigène de stress appelé EphA2 reconnu par les LTgd humains. De plus, l’induction in vitro de l'activation de l’AMPK permettait la sécrétion de petits peptides qui modulent la réponse immunitaire. Ainsi, par une série de chromatographies sur les surnageants de cellules tumorales reprogrammées, nous avons pu identifier un peptide capable d’inhiber l’activation des LB in vitro. Nous tentons actuellement d’identifier d’autres cibles immunitaires, mais également d’identifier les mécanismes qui induisent cet effet.Ces 2 axes constituent autant de cibles potentielles en thérapeutiques anticancéreuse, seules ou en association avec les chimiothérapies ou avec les immunothérapies actuelles. Ceci pourrait ainsi permettre d’améliorer les réponses observées dans les cancers.

International audience; ROS1 fusions are rare mutations that preferentially concern young and non-smoker women. The ROS1-rearranged protein conserves an intact tyrosine kinase domain, leading to the constitutive activation of the ROS1 tyrosine kinase function and of its downstream pathways, that are known to be involved in tumorigenesis. These molecular abnormalities have shown their oncogenic potential in animals’ models and in human, with an early effect on carcinogenesis. Several partners have been identified. Patients with non-small cell lung cancers (NSCLC) harbouring ROS1 alterations can receive specific targeted therapies. Indeed, crizotinib has recently been approved in France in advanced ROS1-rearranged NSCLC. We propose a review of the oncogenic role of ROS1 rearrangements, the different methods for its diagnosis, and the available treatments.; Les réarrangements de ROS1 sont des anomalies rares qui concernent plutôt des femmes jeunes, non ou peu fumeuses. La protéine ROS1 issue de ces réarrangements conserve un domaine tyrosine kinase intact, ce qui entraîne l’activation constitutive de la kinase et des voies de signalisation d’aval, connues comme impliquées dans la tumorigenèse. Ces anomalies ont montré leur potentiel oncogénique dans les modèles animaux mais également chez l’homme, intervenant de manière précoce dans la carcinogenèse. Plusieurs partenaires différents ont été identifiés, et les tumeurs portant cette altération moléculaire peuvent faire l’objet de thérapeutiques spécifiques dans le cancer bronchique non à petites cellules (CBNPC). Ainsi, le crizotinib a récemment obtenu une autorisation de mise sur le marché (AMM) dans le traitement du CBPNC avancé ROS1 positif. Cet article effectue une revue des données concernant le rôle oncogénique de ROS1, les différentes modalités diagnostiques de l’altération et les traitements actuellement disponibles pour cibler ces tumeurs.

Lung cancer is the leading cause of cancer death in France. In above 75% of cases, disease is too advanced to permit curative treatments. Adenocarcinomas are the principal histologic type, with above 10% of epidermal growth factor (EGFR) mutations. These mutations lead to constitutive activation of this receptor. Tumors presenting EGFR mutations have a dramatically response rate to tyrosine kinase inhibitors (TKI). Responses occur during 10 to 12 months in median. After this period, all patients improve relapses because of emergence of tumor resistance. Several therapeutics strategies are proposed but there are not therapeutics standards for treatment of these resistances. The aim of this study was to analyze clinical and molecular characteristics of relapses of 69 patients treated at Bergonie Institute (Bordeaux) and to evaluate therapeutics strategies used. We have shown that oligometastatic patients might benefit of local treatments, with TKI continuation. For others relapses, a biopsy must be realized. Molecule assessment of these biopsies can guide treatments. Slow and asymptomatic progressions are often due to EGFR-dependant progressions, and can be treated with TKI continuation or chemotherapy and rechallenge. EGFR-independent relapses lead to rapid or symptomatic progressions and must be treated with chemotherapies. New treatments like new generation TKI or immunotherapies will change these strategies and might be considered in future trials., Environ 10% des adénocarcinomes bronchiques sont porteurs d’une mutation du gène du récepteur de l’epidermal growth factor (EGFR). Cette mutation rend le récepteur constitutivement actif et sensibilise la tumeur aux inhibiteurs de tyrosine kinase (ITK) visant à bloquer l’activation du récepteur. Les patients répondent en médiane pendant 10 à 12 mois à ces traitements, avant de présenter une progression tumorale liée à l’apparition de phénomènes de résistances moléculaires. Il n’existe actuellement aucun standard pour la prise en charge de ces progressions sous ITK. Ce travail de thèse a donc porté sur l’analyse des modalités de rechute de 69 patients suivis à l’Institut Bergonié (Bordeaux) et sur les stratégies qui ont été mises en œuvre au décours de la progression. Nous avons ainsi montré avec les données de la cohorte et de la littérature que les patients présentant une rechute oligo-métastatique doivent bénéficier d’un traitement local permettant la poursuite de l’ITK. Les progressions disséminées doivent faire l’objet d’une rebiopsie chaque fois que possible. Les résultats de l’analyse moléculaire de ces biopsies guideront ainsi le traitement. Les progressions disséminées, lentes et asymptomatiques, sont souvent liées à une résistance dépendante de l’EGFR, et peuvent être traitées par la poursuite de l’ITK ou l’initiation d’une chimiothérapie avec rechallenge ensuite par ITK. Enfin, les progressions rapides sont plutôt le fait de résistances indépendantes de la voie EGFR, et ont été majoritairement traitées par chimiothérapie dans notre étude. Actuellement, ces données sont remises en cause. En effet, les patients présentant une mutation T790M pourront bénéficier d’un traitement par ITK de 3e génération. Il faudra maintenant définir la stratégie thérapeutique des patients mutés EGFR avec l’arrivée de ces molécules. De plus, les immunothérapies vont également modifier les conduites thérapeutiques dans les prochaines années, et il faudra définir leur place par rapport aux ITK.
