The development of a tool to track participation in activity-based therapy (ABT) for people with spinal cord injury or disease (SCI/D) was identified as a priority of the Canadian ABT Community of Practice. The objective of this study was to understand multi-stakeholder perspectives on tracking ABT participation across the continuum of care. Forty-eight individuals from six stakeholder groups (persons living with SCI/D; hospital therapists; community trainers; administrators; researchers; and funders, advocates and policy experts) were recruited to participate in focus group interviews. Participants were asked open-ended questions concerning the importance of and parameters around tracking ABT. Transcripts were analyzed using conventional content analysis. Themes reflected the Who, What, Where, When, Why and How of tracking ABT. Participants described the importance of involving hospital therapists, community trainers and individuals with SCI/D in tracking ABT to capture both subjective and objective parameters across the continuum of care and injury trajectory. Digital tracking tools were favoured, although paper-based versions were regarded as a necessity in some circumstances. Findings highlighted the importance of tracking ABT participation for individuals with SCI/D. To identify the characteristics of activity-based therapy (ABT) that individuals with spinal cord injury and disease (SCI/D) participate in across the continuum of care.A search of 8 databases was conducted from inception to 4 March 2020: Medline, CINAHL, Embase, Emcare, PEDro, APA PsycINFO, Cochrane Database of Systematic Reviews, and the CENTRAL. The search strategy used terms identifying the population (SCI/D) and concept (ABT).Original studies involving individuals with SCI/D ≥16 years of age participating in ABT interventions for1 session were included in the review. The Joanna Briggs Institute guidelines for scoping reviews were followed. The initial search produced 2306 records. Title, abstract, and full-text screening by 2 independent reviewers yielded 140 articles.Data extraction was conducted by 3 independent reviewers and charted according to key themes. Data fields included participant demographics, ABT interventions, exercises, parameters, technology, and setting. Data synthesis included frequency counts and descriptive analysis of key themes.Eighty percent of participants were male. Eighty-seven percent of studies included individuals with tetraplegia (26% exclusive). Fifty-six percent of studies occurred in a research lab. Fifty-four percent of studies were single modality interventions encompassing the whole body (71%). Sixteen main types of ABT exercises were identified. The most common were treadmill training (59%), muscle strengthening (36%), and overground walking (33%). Electrical stimulation (50%) and virtual reality (6%) were used in combination with an ABT exercise. Eighty-four types of parameters were identified. Six were general intervention parameters and 78 were specific to the type of ABT exercise. Sixteen main categories of technology were reported. The most common were motorized treadmills (47%) and transcutaneous electrical stimulation (44%).The characteristics of ABT are diverse in scope. The results will inform the content to include in tools that track ABT participation and performance.

South Koreans, including church members, remain deeply divided about the pathway to any future reunification with North Korea. One form of division lies in the contested South Korean identities regarding North Korea and their implications for national identity, policy, and how South Koreans must change for a peaceful future with the North to become possible. This article identifies four prevailing South Korean identities regarding North Korea and reunification: Enemy Nation, Another Country, Disadvantaged Sibling, and Ruptured Family. We then consider five paradigms from the Christian tradition as responses to the contested identities via the biblical priority for peace and reconciliation.

The challenges of Flight Test require a rigorous and methodical approach to safety. Like any aviation endeavor the human and financial cost of a mishap drives a need for exceptional rigor in the safety program. The uniqueness of flight test adds the additional element of unfamiliarity with the vehicle's characteristics. Developing an understanding of these characteristics is the purpose of flight test. Likewise, the purpose of flight test safety is to reveal vehicle characteristics in a controlled manner so that discovery of aircraft shortcomings does not lead to catastrophe. The principal element in maintaining flight test safety is a good test plan. Guidance for test planning for the U.S. Navy is specified in the Naval Air Systems Command test plan policy and the corresponding test planning handbook. The test plan policy provides a process that includes risk identification and management and ultimately the determination of the overall risk level associated with the test. Oversight of flight test planning, including safety and risk management is provided by the Test and Experimentation Coordination Team (TECT). The TECT is a two person team that is responsible for the approval of all test plans and formally accepts the risk associated with the test. The two person team consists of the Chief Test Pilot (CTP) and the Chief Test Engineer (CTE). The CTP provides military aviation, mission experience, and the test pilot perspective while the CTE provides an engineering perspective combined with significant experience in flight test. With approval of the test plan, the TECT has deemed that the risks, as mitigated in the test plan, are outweighed by the gains in knowledge of the system under test. This paper details how the test plan addresses safety and is used to manage the associated risks involved with flight test.

This essay draws on the author’s work with the Duke Divinity School Center for Reconciliation, which seeks to cultivate and nourish leaders in the ministry of reconciliation. The essay focuses on a six-year initiative to build a peacemaking community of East African leaders. In a context where Christianity is too often captive to destructive conflicts and powers, the facilitators of the Great Lakes Initiative ask: How do we form Christians with the capacity to say “no” to the power of division and violence and to say “yes” to a way of hope? The pedagogical philosophy and curriculum, “Word Made Flesh,” is one answer to this difficult question. The author focuses on five critical questions and corresponding gifts that shape this pedagogy for reconciliation.

Background: SOR-C13 is a first-in-human selective peptide inhibitor of Transient Receptor Potential Vanilloid 6 (TRPV6) oncochannel. TRPV6 is highly elevated in carcinomas including prostate, breast, lung and ovary and is correlated with poor outcomes. TRPV6-mediated Ca2+ entry is responsible for maintaining a high proliferation rate, increasing cell survival and apoptosis resistance. Since SOR-C13 blocks TRPV6-mediated Ca2+ influx it was evaluated as a single agent in patients with metastatic carcinomas. Methods: This was a Phase I, multi-center, open-label, dose escalation study to assess safety and tolerability of SOR-C13 in subjects with advanced carcinomas commonly known to express the TRPV6 channel (NCT01578564). Subjects received intravenous infusion of SOR-C13 over 90 minutes for 3 d on/4 d off, 3 d on/11 d off (21 d/cycle) until tumor progression, prohibitive toxicity or patient withdrawal. Tumors were assessed according to RECIST 1.1. Results: A total of 23 patients were enrolled to 4 dose levels and 119 cycles of therapy were administered. No study drug-related SAEs occurred. Twelve of 22 evaluable subjects (54.5%) had stable disease after 2 cycles. Promising clinical benefit was observed in two patients with advanced metastatic ductal pancreatic cancer. Subject 304 showed stable disease (SD) at dose level of 2.75 mg/kg for 12 weeks, and was removed from the study for persistent grade 2 hepatic transaminase elevation for more than 21 days. Subject 312, treated at dose level of 6.2 mg/kg, showed 7% decrease in tumor size after 2 cycles and a 27% decrease after 4 cycles. Tumor marker CA 19-9 responses were consistent with most changes in tumor size over the treatment cycles with suggestions of a dose related response and correlation with tumor size. The response of this biomarker was particularly evident in the patient initially treated at 6.2 mg/kg where it decreased by 29% after cycle two and by 55% after cycle 4. This patient was removed from the study after a total of 27 weeks 2 days. Both subjects failed at least three prior regimens of anticancer therapy: Subject 304 (i) IMRT/Tomotherapy plus chemoradiation with Folfirinox, (ii) Carboplatin/gemcitabine/Tarceva (iii) Abraxane/Gemcitabine, (iv) IMRT/Tomotherapy; Subject 312 (i) Folfirinox; (ii) gemcitabine and Abraxane; and, (iii) Xeloda. Conclusions: SOR-C13 was safe and well tolerated in subjects with advanced epithelial malignancies. The clinical benefit observed in patients with metastatic pancreatic cancer, particularly 27% reduction of the sum of tumor diameters after 4 cycles warrants further investigation of this novel treatment in Phase II studies. Enfuvirtide is a virologically and immunologically active, injectable antiretroviral fusion inhibitor developed for the treatment of HIV infection. Patients have shown high adherence rates related to self-preparation (reconstitution) and self-administration of enfuvirtide. This review discusses the efficacy and acceptability of enfuvirtide and offers advice to help health care professionals and patients address concerns about self-injection. Practical interventions that can help minimize and manage the impact of injection-site reactions are described, including the use of a needle-free device for administering the drug.
