The aim of this research paper is to introduce generative design in social manufacturing, describe the core features of our Generative Design Platform, and demonstrate different types of human-machine interaction with the robotic arm use case supported by this tool. This report describes work in progress.

Background In inferomedially rotated zygomatic fractures sticking in the maxillary sinus, it is often difficult to achieve complete reduction only by conventional intraoral reduction. We present a new intraoral reduction technique using a Kirschner wire and its clinical outcome. Methods Among 39 inferomedially impacted zygomatic fractures incompletely reduced by a simple intraoral reduction trial with a bone elevator, a Kirschner wire (1.5 mm) was vertically inserted from the zygomatic body to the lateral orbital rim in 17 inferior-dominant rotation fractures and horizontally inserted to the zygomatic arch in nine medial-dominant and 13 bidirectional rotation fractures. A Kirschner wire was held with a wire holder and lifted in the superolateral or anterolateral direction for reduction. Following reduction of the zygomaticomaxillary fracture, internal fixation was performed. Results Fractures were completely reduced using only an intraoral approach with Kirschner wire reduction in 33 cases and through an additional lower lid or transconjunctival incision in six cases. There were no surgical complications except in one patient with undercorrection. Postoperative 6-month computed tomography scans showed complete bone union and excellent bone alignment. Four patients experienced difficulty with upper lip elevation; however, these problems spontaneously resolved after manual tissue lump massage and intralesional steroid (Triamcinolone) injection. Conclusions We completely reduced infraorbital rim fractures, zygomaticomaxillary buttresses, and zygomaticofrontal suture fractures in 84% of patients through an intraoral approach alone. Intraoral Kirschner wire reduction may be a useful option by which to obtain effective and powerful reduction motion of an inferomedially rotated zygomatic body.

Background Before the main trial in which respiratory polygraphy will be used to evaluate postextubation sleep apnea in critically ill patients, we performed a prospective pilot study to ensure that any issues with the conduct of the trial would be identified. Methods In the present study, 13 adult patients who had received mechanical ventilation for ≥24 hours were prospectively recruited. Among the patients, 10 successfully completed respiratory polygraphy on the first or second night after extubation. Data regarding the types and doses of corticosteroids, analgesics, sedatives, and muscle relaxants as well as the methods of oxygen delivery were recorded. Results During the night of respiratory polygraphy, all 10 patients received supplemental oxygen (low-flow oxygen, n=5; high-flow oxygen, n=5), and seven patients received intravenous corticosteroids. Three of the 10 patients had a respiratory event index (REI) ≥5/hr. All respiratory events were obstructive episodes. None of the patients receiving high-flow oxygen therapy had an REI ≥5/hr. Two of the seven patients who received corticosteroids and one of the other three patients who did not receive this medication had an REI ≥5/hr. Although low- or high-flow oxygen therapy was provided, all patients had episodes of oxygen saturation (SpO2)

The polymer binder, poly(imide-co-siloxane) (PIS), was synthesized and applied to form a thin cathode layer composites for a thermal battery that has an unusually high operating temperature of 450 °C. The PIS was prepared through cross-linking of the polyimide with polysiloxane. The morphology of FeS2/PIS composites showed that FeS2 particles was coated with the PIS cross-linked gel. The FeS2/PIS composites enabled to fabricate mechanically stable thin cathode layer that was 20–10% of the thickness of a conventional pellet-type cathode. The FeS2/PIS composites were stable up to 400 °C and maintained their morphology at this temperature. PIS coating layers decomposed at 450 °C and a new residue was generated, which was observed by transmission electron microscopy and the compositional change was analyzed. The FeS2/PIS composites showed enhanced thermal stability over that of FeS2 in thermogravimetric analysis. The thermal battery with the PIS polymer binder showed a 20% discharge capacity increase when compared to a conventional pellet-type cathode.
