1. Abernethy Malformation and Hepatocellular Carcinoma: a Serious Consequence of a Rare Disease

Abstract

Congenital portosystemic shunts (CPSS) are a rare vascular consequence of embryogenetic vascular alterations or the persistence of the fetal circulation elements, first described by John Abernethy in 1793 and classified by Morgan and Superina, into complete and partial portosystemic shunts. Its prevalence to this day has not been defined. We present a patient series of a 44-year-old and 47-year-old man and woman, with this rare congenital malformation and underlining hepatocellular carcinoma (HCC) treatment strategies. Over half of the individuals with CPSS have benign or malignant liver tumours, ranging from nodular regenerative hyperplasia, focal nodular hyperplasia, adenomas, HCC and hepatoblastomas. Additionally, it is known that half of individuals with Abernethy malformation type Ib will develop one or multiple types of tumours. There seems to be a direct association with tumorigenesis and CPSS, which is the primary consequence of absent portal flow. Surgery is the treatment of choice, either as a curative resection or orthotopic liver transplantation if recommended as per the criteria, in which replacing the hepatic parenchyma in the setting of an Abernathy malformation will correct the underlining hyper-arterialisation. info:eu-repo/semantics/publishedVersion

More than 2 million adolescents between the age group of 10 and 19years are living with HIV, and millions are at risk of infection. HIV risks are considerably higher among girls, especially in high-prevalence settings such as eastern and southern Africa. In addition to girls, there are other vulnerable adolescent subgroups, such as teenagers, who use intravenous (IV) drugs, gay and bisexual boys, transgender youth, male sex workers, and people who fall into more than one of these categories. Pre-exposure prophylaxis (PrEP) is a new intervention for people at high risk for acquiring HIV, with an estimated HIV incidence of >3%. Recent data from trials show evidence of the efficacy of PrEP as a powerful HIV prevention tool in high-risk populations, including men who have sex with men, HIV-1-serodiscordant heterosexual couples, and IV drug users. The reported efficacy in those trials of the daily use of oral tenofovir, alone or in combination with emtricitabine, to prevent HIV infection ranged from 44% to 75% and was heavily dependent on adherence. Despite the proven efficacy of PrEP in adult trials, concerns remain about its feasibility in real-life scenarios due to stigma, cost, and limited clinician experience with PrEP delivery. Recent studies are attempting to expand the inquiry into the efficacy of such HIV prophylaxis approaches in adolescent populations, but there are still many gaps in knowledge, and no country has yet approved it for use with adolescents. The aim of this review was to identify and summarize the evidence from studies on PrEP for adolescents. We have compiled and reviewed published studies focusing on safety, feasibility, adherence to therapeutics, self-perception, and legal issues related to PrEP in people aged between 10 and 24years. Abstract

Relata-se um caso de hidatidosepolicistica, em homem de 22 anos, clinicamente manifestada por dor no hipocôndrio direito, icterícia obstrutiva, hepatoesplenomegalia, perda de peso e, em estágio final, ascite. O diagnóstico foi após laparotomia exploradora, com biópsia hepática e peritoneal. O paciente evoluiu para o óbito, a despeito de ter sido tratado com mebendazol na dose de 1200 mg/dia, durante um ano. A necrópsia, constatou-se hidatidose do fígado, omento maior e peritônio diafragmático. Não se conseguiu identificar, à microscopia óptica, a espécie de Echinococcus envolvida; com base nos dados epidemiológicos e morfológicos disponíveis, o parasita em apreço poderia ser o Echinococcus vogeli ou o E. oligarthrus. Este parece ser o quarto caso de hidatidose policistica na literatura nacional e é, com muita probabilidade, autóctone de Minas Gerais.The authors report a case of polycystic hydatid disease clinically manifested by right upper quadrant pain, obstructive jaundice, hepatosplenomegaly, weight loss, and, at end stage, ascitis. The diagnosis was made by exploratory laparotomy, with hepatic and peritonial biopsy. The patient died despite of receiving a high dose schedule of mebendazole for about twelve months. At autopsy, it was found hepatic, great epiplon and diaphragmaticperitonium polycystic hydatidosis. Epidemiologic and morphologic data indicate that the parasite can be classified as Echinococcus vogeli or E. oligarthrus. This seems to be the fourth case of polycystic hydatid disease in Brazilian literature, and is probably autochthonous of Triângulo Mineiro (Southwest of Minas Gerais State).

Abstract

Background: The health risks associated with moderate alcohol consumption continue to be debated. Small amounts of alcohol might lower the risk of some health outcomes but increase the risk of others, suggesting that the overall risk depends, in part, on background disease rates, which vary by region, age, sex, and year. Methods: For this analysis, we constructed burden-weighted dose-response relative risk curves across 22 health outcomes to estimate the theoretical minimum risk exposure level (TMREL) and non-drinker equivalence (NDE), the consumption level at which the health risk is equivalent to that of a non-drinker, using disease rates from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2020 for 21 regions, including 204 countries and territories, by 5-year age group, sex, and year for individuals aged 15-95 years and older from 1990 to 2020. Based on the NDE, we quantified the population consuming harmful amounts of alcohol. Findings: The burden-weighted relative risk curves for alcohol use varied by region and age. Among individuals aged 15-39 years in 2020, the TMREL varied between 0 (95% uncertainty interval 0-0) and 0·603 (0·400-1·00) standard drinks per day, and the NDE varied between 0·002 (0-0) and 1·75 (0·698-4·30) standard drinks per day. Among individuals aged 40 years and older, the burden-weighted relative risk curve was J-shaped for all regions, with a 2020 TMREL that ranged from 0·114 (0-0·403) to 1·87 (0·500-3·30) standard drinks per day and an NDE that ranged between 0·193 (0-0·900) and 6·94 (3·40-8·30) standard drinks per day. Among individuals consuming harmful amounts of alcohol in 2020, 59·1% (54·3-65·4) were aged 15-39 years and 76·9% (73·0-81·3) were male. Interpretation: There is strong evidence to support recommendations on alcohol consumption varying by age and location. Stronger interventions, particularly those tailored towards younger individuals, are needed to reduce the substantial global health loss attributable to alcohol.
