Fall from ladders is increasingly identified as a significant cause of injury and mortality, yet large-scale research into ladder fall outcomes and trends is limited.To explore the nature and severity of injuries resulting from ladder falls and to determine predictors of Injury Severity Score (ISS) and 6-month mortality.Data were obtained from the New South Wales (NSW) Trauma Registry, Admitted Patient Data Collection and Registry of Births, Deaths, and Marriages on patients aged 15 and over who had major trauma from a ladder fall and were admitted to hospital between January 1st, 2012, and July 31st, 2019. Data linkage and descriptive statistics were carried out alongside bivariate and multivariable regression analysis.963 patients injured after ladder falls were identified. The mean age was 61.9 years (SD 14.2), 91.0% were male, and 489 (50.8%) were born in Australia. The height of fall was between one and five meters in 827 (86.2%) patients, and the place of fall was home and residential places in 27.5%. The most common body areas injured were the head (26.5%), spine (21.2%) and thorax (20.6%), and the median injury severity score was 17. The median length of stay of patients' in-hospital and intensive care unit was six days and two days, respectively. Six months post-discharge mortality was 6.4%. The unadjusted association between the presence of comorbidities or socio-economic class and ISS or mortality was not statistically significant. Increasing ISS was found to be associated with increasing age (Estimate (Est), 15.2; 95% Confidence Interval (CI), 12.3-18.1) and a fall height greater than five metres (Est, 5.8; CI, 3.2-8.4). Mortality was found to be associated with increasing age (Odds ratio (OR), 1.06; CI, 1.03-1.08) and increasing ISS (OR, 1.19; CI, 1.15-1.24).People presenting to the hospital after falling from a ladder were predominately male, aged over 60 and had fallen in a residential setting. Increasing age and fall height are associated with more severe injuries.

Chromosomal instability (CIN) drives cell-to-cell heterogeneity, and the development of genetic diseases, including cancer. Impaired homologous recombination (HR) has been implicated as a major driver of CIN, however, the underlying mechanism remains unclear. Using a fission yeast model system, we establish a common role for HR genes in suppressing DNA double-strand break (DSB)-induced CIN. Further, we show that an unrepaired single-ended DSB arising from failed HR repair or telomere loss is a potent driver of widespread CIN. Inherited chromosomes carrying a single-ended DSB are subject to cycles of DNA replication and extensive end-processing across successive cell divisions. These cycles are enabled by Cullin 3-mediated Chk1 loss and checkpoint adaptation. Subsequent propagation of unstable chromosomes carrying a single-ended DSB continues until transgenerational end-resection leads to fold-back inversion of single-stranded centromeric repeats and to stable chromosomal rearrangements, typically isochromosomes, or to chromosomal loss. These findings reveal a mechanism by which HR genes suppress CIN and how DNA breaks that persist through mitotic divisions propagate cell-to-cell heterogeneity in the resultant progeny.

Polystichum suiyangense (Dryopteridaceae), a new species from Suiyang County, Guizhou, China, is described and illustrated. It is a member of P. sect. Sphaenopolystichum and similar to P. auriculum, P. bifidum, and P. martinii, but differs from them in its frond texture, leaf color, scales on petiole and rachis, pinna shape, and pinna apex. The new species is currently known only from the type locality and is classified as CR (critically endangered) following the IUCN criteria.

This study utilized a two-dimensional numerical viscous wave tank to simulate the run-up and run-down processes of non-breaking solitary waves on a steep seawall. The research aimed to investigate the transformation between wave potential energy and kinetic energy, the evolution mechanisms of the wave and flow fields, and the correlation between the dynamic pressure gradient and the reverse flow near the sloping bed. The numerical model results were consistent with laboratory measurements of free surface elevations and flow velocity profiles, demonstrating the accuracy of the numerical model. This study focused on a solitary wave with a wave-height-to-water-depth ratio of 0.15, propagating on a representative seawall with a steep slope of 1:3 along the western coast of Taiwan. The simulation results indicate that the maximum run-up height occurs when the potential energy is at its highest. Undertow is caused by the adverse pressure gradient within the flow field, and the dynamic pressure on the sloping bed is directly proportional to the free surface elevation. Therefore, by observing the spatial changes in the free surface elevation, we can indirectly determine the occurrence time of undertow.

Graph Neural Networks (GNNs) have emerged as the de facto standard for representation learning on graphs, owing to their ability to effectively integrate graph topology and node attributes. However, the inherent suboptimal nature of node connections, resulting from the complex and contingent formation process of graphs, presents significant challenges in modeling them effectively. To tackle this issue, Graph Structure Learning (GSL), a family of data-centric learning approaches, has garnered substantial attention in recent years. The core concept behind GSL is to jointly optimize the graph structure and the corresponding GNN models. Despite the proposal of numerous GSL methods, the progress in this field remains unclear due to inconsistent experimental protocols, including variations in datasets, data processing techniques, and splitting strategies. In this paper, we introduce OpenGSL, the first comprehensive benchmark for GSL, aimed at addressing this gap. OpenGSL enables a fair comparison among state-of-the-art GSL methods by evaluating them across various popular datasets using uniform data processing and splitting strategies. Through extensive experiments, we observe that existing GSL methods do not consistently outperform vanilla GNN counterparts. However, we do observe that the learned graph structure demonstrates a strong generalization ability across different GNN backbones, despite its high computational and space requirements. We hope that our open-sourced library will facilitate rapid and equitable evaluation and inspire further innovative research in the field of GSL. In this paper, the tool-tip responses for ball end milling, due to the coupling effects of a flexible rotor-bearing system, are investigated numerically. The milling machine tool spindle is modelled as the flexible rotor-bearing system. The critical speeds, natural modes, and unbalance responses of the system are calculated by applying the generalized polynomial expansion method. This generalized polynomial expansion method expresses the displacement as a series formed by the product of generalized coordinates and axial coordinate polynomials. According to the dynamic cutting force obtained by some scholars in the past, combined with the characteristics of the flexible rotor, the dynamic response of the tool-tip for ball end milling is numerically analyzed. The responses, including time histories, orbits, and FFT diagrams, are plotted to analyze the dynamic behaviors of the tool-tip. The coupling effects of the flexible rotor-bearing system on the system for ball end milling are first studied using the generalized polynomial expansion method. Unlike previous studies, the natural frequency varies with spindle speed and which of the different modes are included in the tool-tip response depends mainly on the spindle speed. Thanks to the gyroscopic effect, the critical speeds and responses of tool-tips can be discussed with respect to various spindle speed and tool flutes. The natural modes are accurately determined, and will excite critical speeds for certain modes, including forward and backward modes, thereby significantly affecting tool-tip response. In addition, the cutting force component associated with the tool-tip response affects the rotor-bearing system parameters, complicating the issue. Milling at higher spindle speed (2160–19,950 rpm), an important new result is found that the tool-tip oscillates with the cutting-force frequency, accompanied by a longer period vibration of the first backward mode of the rotor-bearing system. It can also be seen from the frequency spectrum analysis that, as the spindle speed increases, the peak amplitude of the first backward mode becomes larger. Milling at lower spindle speed (960, 1320 rpm), the in-plane vibration trajectory of the tool-tip gradually expands outwards clockwise around the origin until a stable loop is reached. This is because only the first backward mode of the rotor-bearing system is excited. Considering the coupling effect of the rotor-bearing system to perform the vibration analysis of the milling machine system, the parameters of the system can be designed or the spindle speed can be selected to avoid severe vibration during machining.

Comparative analysis of primate genomes within a phylogenetic context is essential for understanding the evolution of human genetic architecture and primate diversity. We present such a study of 50 primate species spanning 38 genera and 14 families, including 27 genomes first reported here, with many from previously less well represented groups, the New World monkeys and the Strepsirrhini. Our analyses reveal heterogeneous rates of genomic rearrangement and gene evolution across primate lineages. Thousands of genes under positive selection in different lineages play roles in the nervous, skeletal, and digestive systems and may have contributed to primate innovations and adaptations. Our study reveals that many key genomic innovations occurred in the Simiiformes ancestral node and may have had an impact on the adaptive radiation of the Simiiformes and human evolution.

Neurodevelopmental disorders (NDDs) result from impaired development and functioning of the brain. Here, we identify loss-of-function variation in ZFHX3 as a novel cause for syndromic intellectual disability (ID). ZFHX3, previously known as ATBF1, is a zinc-finger homeodomain transcription factor involved in multiple biological processes including cell differentiation and tumorigenesis. Through international collaboration, we collected clinical and morphometric data (Face2Gene) of 41 individuals with protein truncating variants (PTVs) or (partial) deletions of ZFHX3 . We used data mining, RNA and protein analysis to identify the subcellular localization and spatiotemporal expression of ZFHX3 in multiple in vitro models. We identified the DNA targets of ZFHX3 using ChIP seq. Immunoprecipitation followed by mass spectrometry indicated potential binding partners of endogenous ZFHX3 in neural stem cells that were subsequently confirmed by reversed co-immunoprecipitation and western blot. We evaluated a DNA methylation profile associated with ZFHX3 haploinsufficiency using DNA methylation analysis on whole blood extracted DNA of six individuals with ZFHX3 PTVs and four with a (partial) deletion of ZFHX3 . A reversed genetic approach characterized the ZFHX3 orthologue in Drosophila melanogaster . Loss-of-function variation of ZFHX3 consistently associates with (mild) ID and/or behavioural problems, postnatal growth retardation, feeding difficulties, and recognizable facial characteristics, including the rare occurrence of cleft palate. Nuclear abundance of ZFHX3 increases during human brain development and neuronal differentiation in neural stem cells and SH-SY5Y cells, ZFHX3 interacts with the chromatin remodelling BRG1/Brm-associated factor complex and the cleavage and polyadenylation complex. In line with a role for chromatin remodelling, ZFHX3 haploinsufficiency associates with a specific DNA methylation profile in leukocyte-derived DNA. The target genes of ZFHX3 are implicated in neuron and axon development. In Drosophila melanogaster , z fh2, considered to be the ZFHX3 orthologue, is expressed in the third instar larval brain. Ubiquitous and neuron-specific knockdown of zfh2 results in adult lethality underscoring a key role for zfh2 in development and neurodevelopment. Interestingly, ectopic expression of zfh2 as well as ZFHX3 in the developing wing disc results in a thoracic cleft phenotype. Collectively, our data shows that loss-of-function variants in ZFHX3 are a cause of syndromic ID, that associates with a specific DNA methylation profile. Furthermore, we show that ZFHX3 participates in chromatin remodelling and mRNA processing.

The key challenge for high-power delivery through optical fibers is overcoming nonlinear optical effects. To keep a smooth output beam, most techniques for mitigating optical nonlinearities are restricted to single-mode fibers. Moving out of the single-mode paradigm, we show experimentally that wavefront-shaping of coherent input light that is incident on a highly multimode fiber can increase the power threshold for stimulated Brillouin scattering (SBS) by an order of magnitude, whilst simultaneously controlling the output beam profile. The theory reveals that the suppression of SBS is due to the relative weakness of intermodal scattering compared to intramodal scattering, and to an effective broadening of the Brillouin spectrum under multimode excitation. Our method is efficient, robust, and applicable to continuous waves and pulses. This work points toward a promising route for suppressing detrimental nonlinear effects in optical fibers, which will enable further power scaling of high-power fiber systems for applications to directed energy, remote sensing, and gravitational-wave detection.

Stimulated Brillouin scattering (SBS) is an important nonlinear optical effect which can both enable and impede optical processes in guided wave systems. Highly multi-mode excitation of fibers has been proposed as a novel route towards efficient suppression of SBS in both active and passive fibers. To study the effects of multimode excitation generally, we develop a theory of SBS for arbitrary input excitations, fiber cross section geometries and refractive index profiles. We derive appropriate nonlinear coupled mode equations for the signal and Stokes modal amplitudes starting from vector optical and tensor acoustic equations. Using applicable approximations, we find an analytical formula for the SBS (Stokes) gain susceptibility, which takes into account the vector nature of both optical and acoustic modes exactly. We show that upon multimode excitation, the SBS power in each Stokes mode grows exponentially with a growth rate that depends parametrically on the distribution of power in the signal modes. Specializing to isotropic fibers we are able to define and calculate an effective SBS gain spectrum for any choice of multimode excitation. The peak value of this gain spectrum determines the SBS threshold, the maximum SBS-limited power that can be sent through the fiber. We show theoretically that peak SBS gain is greatly reduced by highly multimode excitation due to gain broadening and relatively weaker intermodal SBS gain. We demonstrate that equal excitation of the 160 modes of a commercially available, highly multimode circular step index fiber raises the SBS threshold by a factor of 6.5, and find comparable suppression of SBS in similar fibers with a D-shaped cross-section.

Alzheimer’s disease (AD) is one of the most common neurodegenerative diseases characterized by cognitive deficits and dementia. AD entails predominant pathological characteristics including amyloid beta (Aβ) plaque formation, neurofibrillary entanglements, and brain atrophy, which gradually result in cognitive dysfunctions. Studies showed that these pathological changes are found in a myriad of brain structures, including the claustrum (CLA), a nucleus that penetrates deeply into the brain and is extensively interconnected to various brain structures. The CLA modulates many aspects of cognitive functions, with attention, executive function, visuospatial ability, language, and memory in particular. It is also implicated in multiple neuropsychiatric disorders, of which one worthy of particular attention is AD-related cognitive impairments. To inspire novel AD treatment strategies, this review has summarized the CLA functionality in discriminative cognitive dysfunctions in AD. And then propose an array of potential mechanisms that might contribute to the cognitive impairments caused by an abnormal CLA physiology. We advocate that the CLA might be a new promising therapeutic target in combination with existing anti-AD drugs and brain stimulation approaches for future AD treatment.

Nickel–cobalt alloys were prepared by alloy electrodeposition with a sulfamate bath, and the mechanical properties on the micro-scale were evaluated for the application as micro-components in miniaturized electronic devices. Nickel bromide and a commercially available surface brightener were used as the additives. The cobalt content increased from 21.5 to 60.1 at.% after addition of nickel bromide into the bath, and the grain size refined from 21.1 to 13.2 nm when the surface brightener was used. The mechanical properties on the micro-scale were evaluated by micro-compression test using micro-pillar type specimens fabricated by a focused ion beam system to take the sample size effect into consideration. The yield strength of the nickel–cobalt alloy having an average grain size at 13.9 nm and cobalt content of 66.6 at.% reached 2.37 GPa, revealing influences from the sample size, grain boundary strengthening, and solid solution strengthening effects.

Summary of the xenograft and tumor-sphere formation assays

Purpose:Hepatitis promotes the development and recurrence of hepatocellular carcinoma (HCC). Receptor tyrosine kinases (RTK) play critical roles in the development of many cancers. We explored the potential roles of RTKs in hepatitis-related liver cancers.Experimental Design:We conducted loss-of-function screening to elucidate the roles of RTKs in the development of HCC in vitro and in vivo.Results:Many RTKs were coexpressed in HCC and were involved in tumor development and growth. Of these, TYRO3 promoted tumor growth and was clinically associated with hepatitis activity and poor prognosis. In mice, chemical-induced hepatitis transcriptionally activated Tyro3 expression via IL-6/IL6R–STAT3 signaling. Moreover, hepatitis-associated apoptotic cells facilitated the presentation of GAS6, a TYRO3 ligand, to further activate TYRO3-mediated signaling. Furthermore, TYRO3 activation elicited intracellular SRC- and STAT3 signaling. In mice, hepatitis and Tyro3 synergistically promoted HCC development. Silencing TYRO3 expression or inhibiting its kinase activity suppressed xenograft HCC growth in nude mice.Conclusions:Many RTKs are simultaneously involved in HCC development. Hepatitis exerts dual effects on the activation of TYRO3-mediated signaling in HCC cells, which further elicits the “TYRO3–STAT3–TYRO3” signaling loop to facilitate tumor growth. Our findings unveil a previously unrecognized link between RTKs and hepatitis-associated HCC and suggest TYRO3 as a marker and therapeutic target for the HCCs with higher hepatitis activity.

Correlation between TYRO3 level and clinicopathologic features in 120 HCC patients

Many intensive care unit patients who undergo endotracheal extubation experience extubation failure and require reintubation. Because of the high mortality rate associated with reintubation, postextubation respiratory management is crucial, especially for high-risk populations. We conducted the present study to compare the effectiveness of oxygen therapy administered using high-flow nasal cannulae (HFNC) and noninvasive positive pressure ventilation (NIPPV) in preventing reintubation among patients receiving prolonged mechanical ventilation (PMV). This single-center, prospective, unblinded randomized controlled trial was at the respiratory care center (RCC). Participants were randomized to an HFNC group or an NIPPV group (20 patients in each) and received noninvasive respiratory support (NRS) administered using their assigned method. The primary outcome was reintubation within7 days after extubation. None of the patients in the NIPPV group required reintubation, whereas 5 (25%) of the patients in the HFNC group required reintubation (P = 0.047). The 90-day mortality rates of the NIPPV and HFNC groups (four patients [20%] vs. two patients [10%], respectively) did not differ significantly. No significant differences in length of RCC stay, length of hospital stay, time to liberation from NRS, and ventilator-free days at 28-day were identified. The time to event outcome analysis also revealed that the risk of reintubation in the HFNC group was higher than that in the NIPPV group (P = 0.018). Although HFNC is becoming increasingly common as a form of postextubation NRS, HFNC may not be as effective as NIPPV in preventing reintubation among patients who have been receiving PMV for at least 2 weeks. BackgroundThe aim of our study was to externally validate the predictive capability of five developed coronavirus disease 2019 (COVID-19)-specific prognostic tools, including the COVID-19 Spanish Society of Infectious Diseases and Clinical Microbiology (SEIMC), Shang COVID severity score, COVID-intubation risk score-neutrophil/lymphocyte ratio (IRS-NLR), inflammation-based score, and ventilation in COVID estimator (VICE) score.MethodsThe medical records of all patients hospitalized for a laboratory-confirmed COVID-19 diagnosis between May 2021 and June 2021 were retrospectively analyzed. Data were extracted within the first 24 h of admission, and five different scores were calculated. The primary and secondary outcomes were 30-day mortality and mechanical ventilation, respectively.ResultsA total of 285 patients were enrolled in our cohort. Sixty-five patients (22.8%) were intubated with ventilator support, and the 30-day mortality rate was 8.8%. The Shang COVID severity score had the highest numerical area under the receiver operator characteristic (AUC-ROC) (AUC 0.836) curve to predict 30-day mortality, followed by the SEIMC score (AUC 0.807) and VICE score (AUC 0.804). For intubation, both the VICE and COVID-IRS-NLR scores had the highest AUC (AUC 0.82) compared to the inflammation-based score (AUC 0.69). The 30-day mortality increased steadily according to higher Shang COVID severity scores and SEIMC scores. The intubation rate exceeded 50% in the patients stratified by higher VICE scores and COVID-IRS-NLR score quintiles.ConclusionThe discriminative performances of the SEIMC score and Shang COVID severity score are good for predicting the 30-day mortality of hospitalized COVID-19 patients. The COVID-IRS-NLR and VICE showed good performance for predicting invasive mechanical ventilation (IMV).

Various bibliographic elements were collected, including the annual number of publications, countries/regions, and keywords. CiteSpace, a scientometric software, was used to conduct bibliometric analyses. Also, the patsnap global patent database was used to analyze the patent situation of plant adaptogens in the field of cosmetics up to 2021.Results: We found that the effects of plant adaptogens on skin diseases mainly involve atopic dermatitis, acne, allergic contact dermatitis, psoriasis, eczema, and androgenetic alopecia, etc. And the effects on skin health mainly involve anti-aging and anti-photoaging, anti-bacterial and anti-fungal, anti-inflammatory, whitening, and anti-hair loss, etc. Also, based on the results of patent analysis, it is found that the effects of plant adaptogens on skin mainly focus on aging retardation. The dermatological effects of plant adaptogens are mainly from Fabaceae Lindl., Araliaceae Juss. and Lamiaceae Martinov., and their mainly efficacy phytochemical components are terpenoids, phenolic compounds and flavonoids.Conclusion: The plant adaptogens can repair the skin barrier and maintain skin homeostasis by regulating the skin HPA-like axis, influencing the oxidative stress pathway to inhibit inflammation, and regulating the extracellular matrix (ECM) components to maintain a dynamic equilibrium, ultimately achieving the treatment of skin diseases and the maintenance of a healthy state.Graphical Abstract: Keywords: plant adaptogens, dermatology, skin HPA-like axis, ECMs, skin homeostasis, pharmacophylogeny

Guangxia An, Chen & Shi, gen. nov. Type species: Guangxia longlina An, Chen & Shi , sp. nov. Generic characters. Body small. Head hypognathous. Fastigium verticis conical, apex rounded, with longitudinal sulcus on dorsal surface. Compound eyes globular, protruding forward. Frons slightly sloping backward. Apical segment of maxillary palpus approximately equated in length with subapical segment, apex swollen and rounded. Pronotum slightly elongate backward, with a median carina; lateral lobe longer than deep, humeral sinus absent. Thoracic auditory spiracle exposed, peanut shape. Tegmina shorter, only reaching apex of abdomen, tapering, apices slightly and obliquely truncated; hind wings slightly shorter than tegmina. All femora without spines on ventral surfaces; tibial tympana open on both sides; genicular lobe of postfemora with apex rounded; posttibia with a pair of dorsal apical spurs and 2 pairs of ventral apical spurs. Posterior margin of male tenth abdominal tergite concave, with a pair of posterior processes; cercus with 2 branches; genitalia entirely sclerotized, exposed; subgenital plate with styli. Ovipositor slightly upcurved, ventral valvulae with apices hooked. Discussion. The new genus is similar to Aphlugiolopsis Wang, Liu & Li, 2015, but differs in: posterior margin of male tenth abdominal tergite concave, with a pair of posterior processes; styli positioned on apex of subgenital plate. It resembles Neocononicephora Gorochov, 1998, but differs in: posttibia with 3 pairs of apical spurs; male genitalia entirely sclerotized. Etymology.
