Abstract

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is associated with cardiovascular comorbidities such as left ventricular (LV) hypertrophy. Whether OSA is an independent etiological factor for this hypertrophic remodeling is yet unknown. Continuous positive airway pressure partially reverses this hypertrophy, but data regarding the effect of mandibular advancement devices on LV remodeling are scarce. The aim of this prospective trial is to evaluate the effect of mandibular advancement device therapy on LV geometry and function in patients with OSA. METHODS: At baseline and 6-month follow-up, participants underwent a home sleep apnea test, 24-hour ambulatory blood pressure monitoring and a 2-dimensional Doppler and tissue Doppler echocardiography. RESULTS: Sixty-three patients (age: 49 ± 11 years; body mass index: 27.0 ± 3.4 kg/m(2); baseline apnea-hypopnea index home sleep apnea test: 11.7 [8.2; 24.9] events/h) completed the 6-month follow-up visit. Overall, blood pressure values and parameters of LV function were within normal ranges at baseline and did not change under mandibular advancement device therapy. In contrast, the interventricular septum thickness was at the upper limits of normal at baseline and showed a significant decrease at 6-month follow-up (11.1 ± 2.1 mm vs 10.6 ± 2.0 mm, P = .03). This significant improvement is only found in responders but not in nonresponders. There was no correlation between the decrease of interventricular septum thickness and the change in blood pressure. CONCLUSIONS: In mildly obese, normotensive patients with OSA we observed significant reverse hypertrophic remodeling after 6 months of successful mandibular advancement device therapy, with maintained normotensive systemic blood pressure. This suggests that OSA is an independent factor in the pathophysiology of LV hypertrophy in these patients. Abstract

Background Arterial stiffness is associated with increased risk for cardiovascular disease. The purpose of this study is to investigate the arterial stiffness and myocardial deformation in patients with poorly controlled diabetes mellitus type 2 before and after glycemic control by optimal medication. Methods In 50 patients with uncontrolled type 2 diabetes(age:52±10years)and 25 controls of similar age and sex and no atherosclerotic risk factors we measured at baseline and 6 months after glycemic control a) carotid-femoral pulse wave velocity(PWVc m/sec-Complior SP ALAM),central systolic blood pressure(cSBP -mmHg),augmentation index(AI%), of the aortic pulse wave(ArteriographTensioMed) b)S',E'(m/sec)andE'/A'of mitral annulus by Tissue Doppler c)LV longitudinal strain(GLS-%),systolic(LongSr-l/sec)and diastolic(LongSrE-l/sec)strain rate, twisting(Tw-deg),peak twisting(Tw)and untwisting(unTw-deg/sec)velocity using speckle tracking echocardiography.The degree of LV untwisting was calculated as the percentage difference between peak twisting and untwisting at MVO(%dp PeakTw-UntwMVO)and between peak twisting and untwisting at peak and end of the mitral inflow E wave d)perfusion boundary region(PBR- micrometers)of the sublingual arterial microvessels(ranged from 5-25 micrometers)using Sideview,Darkfield imaging(Microscan,Glycocheck).Increased PBR is considered an accurate index of reduced endothelial glucocalyx thickness because of a deeper RBC penetration in the glucocalyx e) Flow mediated dilatation(FMD) of the brachial artery and percentage difference of FMD (FMD%). Abstract

Aims Compare the diagnostic accuracy and prognostic value of echo contrast enhanced 2D and 3D Dobutamine stress echocardiography (DSE). Methods and results We included 718 patients indicated for DSE. All had standard 2D, and contrast enhanced left ventricular opacification (LVO) for 2D and 3D acquisitions at rest and peak stress. Chi-square test was done to assess relationship between DSE result and early revascularization. Kaplan–Meier plots with Logistic regression analysis predicted late major adverse cardiovascular events (MACE) at a maximum follow-up of 84 months. The mean age was 63 ± 13 years (61% males) and follow-up was obtained in 692/718 (96.4%) patients. Only 32% had excellent baseline image quality. The DSE was abnormal in 19.4% patients on 2D, in 17.1% on 2D-LVO and in 19.1% on 3D-LVO. Early revascularization was performed in, respectively, 32.8%, 45.8%, and in 48.5% of stress-positive 2D, 2D-LVO, and 3D-LVO studies. After excluding the 66 patients receiving early revascularization 68/626 (10.9%) had MACE at a maximum follow-up of 84 months. Kaplan–Meier plots showed that stress-positive 2D-LVO and 3D-LVO studies not receiving early revascularization when assessed separately and combined had significantly worse outcomes for MACE compared with stress-negative patients (OR 3.69; 95% CI: 1.54–8.87; P = 0.011, OR 4.54; 95% CI: 1.72–12.93; P = 0.008, and OR 7.07, 95% CI: 1.62–25.16; P = 0.001, respectively). Conclusion Combined use of 2D- and 3D-LVO DSE is ready for prime time considering the feasibility, improved diagnostic accuracy and prognostic value.

Abstract

Purpose: No consensus exists for the diagnosis of heart failure with preserved ejection fraction (HFPEF). Current studies recommend stress echocardiography protocols, but single standard peak or timing measurements are limited towards the complexity of the disease. We show the contribution of a combined analysis of multiple myocardial velocity patterns and investigate their relative relevance for the characterization of HFPEF. Methods: Velocity traces from 55 subjects were examined (69±6 years, 22 healthy, 19 HFPEF and 14 breathless subjects). Data came from tissue Doppler acquisitions at rest and exercise at the basal septum and lateral wall, and were temporally aligned to a common reference for comparison. Each phase of the cycle at each stage of the protocol was identified for analysis (figure). Unsupervised machine learning (multiple kernel learning) was used to characterize this population and automatically determine the relevance of each velocity pattern. Results: The learning found a discrimination algorithm that performed well in agreement with diagnosis based on current guidelines (sens=78.9%, spec=86.3%, K=0.65). The importance given to early diastole at exercise was substantially higher, while the isovolumic contraction was the lowest contributor. The breathless subjects were associated to their closest subgroup. Conclusion: The characterization of HFPEF is improved by a combined analysis of multiple velocity traces from stress echo studies using machine learning. The method additionally suggests features of interest to be used in clinical diagnosis.

Abstract

We studied protein kinase C (PKC) isozyme expression and activity distribution in two models of chronically ischemic canine myocardium: (1) single vessel obstruction (SVO), produced by tight stenosis of LAD followed by preconditioning and acute ischemia (40 min); (2) three vessel obstruction (3VO), produced by LAD-stenosis and gradual occlusion of right coronary artery and left circumflex. In both models after 8 weeks of chronic ischemia the dogs were either sacrificed or had PTCA of the LAD with a follow up of another 4 weeks. Control dogs were sham operated. PKC activity was measured in subcellular fractions of tissue samples from anterior and posterior regions in the presence of histone and gamma-[32P]-ATP. PKC isozymes were detected by Western blotting. All regions perfused by the obstructed coronaries were dysfunctional at 8 weeks when compared to baseline, with improvement of anterior wall function after PTCA of LAD. PKC activity was elevated in the membrane fraction of SVO, but unchanged in the 3VO model. PKCs alpha, epsilon, and zeta prevailed in cytosol fraction of the controls (cytosol/membrane ratios were +/- 3.34, 1.38 and 4.56 for alpha, epsilon and zeta, respectively), consistent with PKC activity distribution, while delta was not detected. There was no significant difference between the groups concerning the relative membrane amount of the isozymes. PKCs alpha and epsilon were decreased in the cytosol fraction of both models at 8 weeks (for anterior region, by 56 and 57% in SVO and by 49 and 46% in 3VO, respectively) without there being any differences between anterior and posterior regions, and were low also in the PTCA group. PKC zeta distribution however varied between the two models. The amount of PKC zeta isozyme was downregulated by 45% after 8 weeks of chronic ischemia and returned towards the control values after PTCA in the anterior region of SVO, while it did not change in anterior wall after 8 weeks in 3VO but was significantly decreased (by 47%) in posterior region after PTCA. In conclusion, our results suggest modified PKC signalling in chronically ischemic canine myocardium.

Abstract

Thiamine deficiency can have cardiovascular and neurological manifestations. Cardiac beriberi is classically thought to represent a high-output state with oliguria and lactic acidosis. The condition can, however, also present itself with a low cardiac output and fulminant vascular collapse, or as an acute fatal form, causing sudden death, without clear-cut signs of cardiomegaly. In the western society beriberi is mainly encountered in alcoholics. We report on two cases, one with high-output failure and the other with low-output failure and cardiovascular collapse. In both patients the diagnosis of shoshin syndrome was made, and and both showed a spectacular improvement of congestive heart failure symptoms after treatment with thiamine. A therapeutic trial with thiamine is the only way to rapid diagnosis.

Abstract

Background: Myocardial perfusion imaging and infarct size determinalion using myocardial conlrast echocardiography (MCE), and cardiac magnelic resonance imaging (MRI) are well eslablished lechniques. Presenlly mullislice cardiac compuler lomography (MSCT) is used for morfologic evalualion of the myocardium and coronary arleries, as well as coronary calcium measuremenl. We hypolhesize lhal MSCT could also delermine myocardial perfusion delecls and inlarcl size due I0 reduced contrast enhancemenl ol lhe inlarcled myocardium. Methods: Seven patienls with a firsl ST-elevalion myocardial inlarclion (MI), and positive myocardial enzymes who were treated with primary perculaneous coronary intervention were included. Within 7 days el lhe MI all palienls had a MCE (real time and Iriggered imaging wilh sonovue), MRI for perlusion using delayed contrasl enhancement, and a cardiac MSCT study. To assess myocardial pedusion delecls on MSCT, the window levels el lhe examinalion were visually adjusled to maximize the difference between conlrast enhanced and less enhanced lissue. Colour coding was performed according to the Houesfield unils el myocardial tissue Io furlher discriminale between transmural diflrences in perfusion. The lell ventricle was divided into 17 segments, and lor each lechnique performed lhe segmental myocardial perfusion was classified as 0: normal, 1 : less than 50% Iransmuralily el perfusion delecl, 2: more than 50% Iransmurality or, 3: transmural delecl. The scoring was done by 3 blinded independent observers. Results: Wall motion abnormalilies were associaled with perfusion delesls observed wilh MCE and MRI. There was moderale agreement belween MRI and MSCT for perlusion delecls al 74% (kappa = 0.48), a good agreemenl between MCE and MSCT al 86% (kappa = 0.60), and a very good concordance between combined MCE and MRI findings Versus MSCT al a kappa value el 0.66 (agreement al 89%). The sensilivity and specificily el MSCT Io assess myocardial pedusion was respeclively, 86% and 85% Conclusion: MSCT appears to be a promising technique to simullaneously assess myocardial and coronary artery morfology as well as segmenlal myocardial perfusion to delermine inlarcl size.

Abstract

Introduction: The corrected TIMI frame count (cTFC) has been shown to be an index of coronary flow. Tissue Doppler imaging (TDI) can detect early ventricular dysfunction not detected by conventional methods. We hypothesize that: 1. TDI can detect abnormal regional function in non infarcted but chronically hypoperfused myocardium, 2. Improved cTFC post percutaneous coronary intervention (PCI) is associated with early functional improvement. Methods: Twenty nine patients (M/F: 19/10, age 67±9 years) with stable angina, and without prior myocardial infarction underwent cardiac catheterization, during which wall motion analysis (centerline method), quantitative coronary angiography (QCA) and CTFC (using a frame counter on a cine viewer) were measured. All patients had a standard 2-D and Doppler echocardiography and Pulsed wave TDI of systolic (Sm) and diastolic velocities (early: Em, late: Am) from the perfusion territories of the left anterior descendens (LAD), the circumflex (CX) and the right coronary artery (RCA), using the apical views, before and 24 hours after PCI. Results: All patients underwent elective PCI (19 LAD, 4 CX and 6 RCA stenoses, range 61 94%). There was TIMI grade 3 flow before and after PCI. Regional wall motion (centerline method and wall motion score with echocardiography) was normal in all patients before and after PCI. Significant improvement in the following parameters was observed post PCI (mean ± SD): QCA, 72±12% to 8±6%; cTFC, 35±19 to 22±8; Sm, 5.4±1.1 to 8±1.4; p < 0.005 for all. An improvement was also seen in diastolic function E/A ratio from 0.87±0.23 to 0.97±0.2. A significant correlation was found between cTFC and stenosis severity pre PCI (r=0.58, p=0.0028), and the improvement in cTFC and the regional Sm values post PCI (r = 0.79, p
