RESUMEN El pioderma gangrenoso (PG) es una enfermedad cutánea rara y dolorosa con una respuesta de tratamiento impredecible. Las lesiones son recurrentes y ocurren en áreas que han sufrido traumas previos. Algunos casos se han relacionado con diferentes tipos de cirugías y, por lo general, no se diagnostican correctamente y se clasifican erróneamente como un proceso infeccioso. La implantación de un sistema de neuroestimulación es un procedimiento quirúrgico con varias complicaciones posibles, generalmente infecciosas, pero también no infecciosas, como reacciones de hipersensibilidad, pero no hay una descripción de la PG. Presentamos un paciente con un síndrome de dolor regional complejo posquirúrgico en ambos pies que desarrolla una lesión necrótica, pustulosa, eritematosa y dolorosa después del implante de un sistema de neuroestimulación medular que se diagnosticó como PG y se trató correctamente gracias a un enfoque multidisciplinario. ABSTRACT Pyoderma gangrenosum (PG) is a rare and painful skin disease with unpredictable treatment response. The lesions are recurrent and occur in areas that have suffered previous trauma. Some cases have been related to different types of surgeries and it is usually underdiagnosed and erroneously classified as an infectious process. The implantation of a neurostimulation system is a surgical procedure with several possible complications, usually infectious but also non-infectious such as hypersensitivity reactions but there is no description of PG. We report a patient with a post-surgical complex regional pain syndrome in both feet that develops a necrotic, pustular, erythematous and painful lesion after the implantation of a spinal cord stimulation which was diagnosed as PG and correctly treated thanks to a multidisciplinary approach.

Mount Mayon in Albay province (Philippines) is an openly-degassing basaltic-andesitic stratovolcano, located on the northern edge of the northwest-trending OAS graben. Its latest eruptions were in Aug-Sept 2006 and Dec 2009. Mayon’s current status is PHIVOLCS’ level 1 with low seismicity dominated mostly local and regional tectonic earthquakes and continuous emission of SO2 from its summit crater. A research collaboration between the Earth Observatory of Singapore-NTU and the Philippine Institute of Volcanology and Seismology (PHIVOLCS) was initiated in 2009, aimed at developing a multi-disciplinary monitoring network around Mayon. BACKGROUND/OBJECTIVE: Artificial sweeteners were thought to be metabolically inactive, but after demonstrating that the gustatory mechanism was also localized in the small intestine, suspicions about the metabolic effects of artificial sweeteners have emerged. The objective of this study was to determine the effect of artificial sweeteners (aspartame and sucralose) on blood glucose, insulin, c-peptide and glucagon-like peptide-1 (GLP-1) levels. SUBJECTS/METHODS: Eight newly diagnosed drug-naive type 2 diabetic patients (mean age 51.5 +/- 9.2 years; F/M: 4/4) and eight healthy subjects (mean age 45.0 +/- 4.1 years; F/M: 4/4) underwent 75 g oral glucose tolerance test (OGTT). During OGTT, glucose, insulin, c-peptide and GLP-1 were measured at 15-min intervals for 120 min. The OGTTs were performed at three settings on different days, where subjects were given 72 mg of aspartame and 24 mg of sucralose in 200 ml of water or 200 ml of water alone 15 min before OGTT in a single-blinded randomized order. RESULTS: In healthy subjects, the total area under the curve (AUC) of glucose was statistically significantly lower in the sucralose setting than in the water setting (P = 0.002). There was no difference between the aspartame setting and the water setting (P = 0.53). Total AUC of insulin and c-peptide was similar in aspartame, sucralose and water settings. Total AUC of GLP-1 was significantly higher in the sucralose setting than in the water setting (P = 0.04). Total AUC values of glucose, insulin, c-peptide and GLP-1 were not statistically different in three settings in type 2 diabetic patients. CONCLUSIONS: Sucralose enhances GLP-1 release and lowers blood glucose in the presence of carbohydrate in healthy subjects but not in newly diagnosed type 2 diabetic patients.
