Abstract: Coronary artery disease (CAD) is a pathological process characterized by atherosclerotic plaque accumulation in the epicardial arteries, whether obstructive or non-obstructive. This process can be modified by lifestyle adjustments, pharmacological therapies, and invasive interventions designed to achieve disease stabilization or regression. The disease can have long, stable periods but can also become unstable at any time, typically due to an acute atherothrombotic event caused by plaque rupture or erosion. However, the disease is chronic, most often progressive, and hence serious, even in clinically apparently silent periods. The dynamic nature of the CAD process results in various clinical presentations, which can be conveniently categorized as either acute coronary syndromes (ACS) or chronic coronary syndromes (CCS). The Guidelines presented here refer to the management of patients with CCS. The natural history of CCS is illustrated in Figure 1.

Abstract: This is the second iteration of the European Society of Cardiology (ESC) and European Association for the Study of Diabetes (EASD) joining forces to write guidelines on the management of diabetes mellitus (DM), pre-diabetes, and cardiovascular disease (CVD), designed to assist clinicians and other healthcare workers to make evidence-based management decisions. The growing awareness of the strong biological relationship between DM and CVD rightly prompted these two large organizations to collaborate to generate guidelines relevant to their joint interests, the first of which were published in 2007. Some assert that too many guidelines are being produced but, in this burgeoning field, five years in the development of both basic and clinical science is a long time and major trials have reported in this period, making it necessary to update the previous Guidelines.

3. Gaussian Estimates: A Brief History

Abstract: Two-sided Gaussian estimates for the fundamental solution of a second order linear parabolic differential equation are upper and lower bounds in terms of the fundamental solution of the classical heat conduction equation. In his seminal 1958 paper Nash stated, without proof, two-sided non-Gaussian bounds for the fundamental solution of a uniformly parabolic divergence structure equation assuming only boundedness of the coefficients. In his 1967-1968 papers Aronson derived truly Gaussian estimates for the fundamental solutions of a large class of linear parabolic equations (including the divergence structure equation) under minimal non-regularity assumptions on the coefficients. Subsequently in 1986 Fabes & Stroock derived Gaussian estimates for the divergence structure equation directly from the ideas of Nash and went on to prove Nash's continuity theorem and the Harnack inequality as a consequence of their estimate. In this note I describe these results together with various extensions.

4. Large Arrays of Linearly Coupled Josephson Junctions: A Survey

Abstract: In this note I survey the extensive literature on the dynamics of large series arrays of identical current biased Josephson junctions coupled through various shared loads. The equations describing the dynamics are invariant under permutation of the junctions so that in addition to the usual dynamical systems and numerical methods, group theoretic methods can be applied. In practice it is desirable to operate these circuits at a stable in-phase oscillation. The works summarized here are devoted to the study of where in parameter space the in-phase oscillations are stable and how the stability is lost. In particular, they focus on the variety of states produced through bifurcation as the in-phase oscillation loses stability. These states include discrete rotating waves and semirotors.

5. The justification of legal punishment in Kant’s philosophy

Abstract: The subject matter of the article is the problem of justification of punishment within Kant’s practical philosophy. Modern interpretations tend to reduce this problem to the issue of “retributivism”: To what extent is Kant’s theory of punishment to be regarded as retributivist? While acknowledging the significance of this question the author stresses a more fundamental one lying behind it: Is a non-contradictory theory of punishment conceivable at all within Kant’s philosophy? It is demonstrated that a solution of this question largely determines a justification of the doctrine of right as such as well as a solution of the problem of relation between right and ethics in Kantian philosophy. Some recent interpretations of Kant’s theory of punishment are examined, particularly those by O. Hoeffe and B. Byrd. It is demonstrated that neither is actually compatible with Kant’s statement that punishment is a categorical imperative. Futhermore, it is shown that this statement is crucial and necessary for Kant’s universalist project of justification of right. At the same time, it is shown that it is the universalism of Kant’s practical philosophy that leads to a kind of paradox of punishment: the categorical imperative of punishment might well demand those very actions which the categorical imperative as we know it from the “Groundwork” seems to forbid. It is proposed to see this paradox as another antinomy of practical reason. The hypothesis is offered that the separation of the principles of virtue directed to an individual will and right covering the public sphere can be considered as an attempt to solve this very antinomy.

6. Harnack, Holder, Gauss and Widder: Serrin's Parabolic Legacy

Abstract: James Serrin's fundamental contributions to the theory of quasilinear elliptic equations are well-known and widely appreciated. He also made less well-known contributions to the theory of quasilinear parabolic equations which we dicuss in this note. Jurgen Moser gave greatly simplified proofs of the De Giorgi-Nash regularity results for linear divergence structure elliptic and parabolic differential equations using an original iterative technique. Serrin extended Moser's techniques and applied them to the study of divergence structure quasilinear elliptic equations, and in collaboration with Aronson, to divergence structure quasilinear parabolic equations. Specifically, among other results, they proved a maximum principle, Holder continuity of generalized solutions and derived a Harnack principle for a very broad class of quasilinear parabolic equations. In subsequent work, Aronson applied these results to study non-negative solutions to divergence structure linear equations without regularity assumptions on the coefficients. The results include a two-sided Gaussian estimate for the fundamental solution and a generalization of the Widder Representation Theorem.

Abstract: Aims We aimed to determine the relation between baseline systolic blood pressure (SBP), change in SBP, and worsening renal function (WRF) in acute heart failure (AHF) patients enrolled in the Pre-RELAX-AHF trial. Methods and resultsThe Pre-RELAX-AHF study enrolled 234 patients within 16 h of admission (median 7 h) for AHF and randomized them to relaxin given intravenous (i.v.) for 48 h or placebo. Abstract: A 17-year-old boy presented with an atypical manifestation of acute appendicitis. The clinical aspect, radiological investigations and peroperative aspect of the appendix were not conclusive but nevertheless a neuroendocrine tumour (carcinoid tumour) of the appendix was suspected. After ileocaecal resection and resection of pathological lymph nodes, histopathological evaluation revealed the diagnosis: a periappendicular mass without any sign of malignancy. In retrospect, ileocaecal resection was performed for a benign disease. This case illustrates that an unusual presentation of a common disease occurs more frequently than a typical presentation of a rare disease

Abstract: A 14-year-old boy presented with acute abdominal pain, vomiting and diarrhoea. Colo-colic intussusception was diagnosed by means of a colonic contrast X-ray. The intussusception was successfully reduced during this procedure. Hundreds of polyps were seen throughout the entire colon. Genetic research showed a mutation of the MutYH gene. Proctocolectomy with ileoanal pouch anastomosis was carried out. The pathology specimen showed an intramucosal carcinoma and multiple adenomas. MutYH-associated polyposis coli is an autosomal recessive disease that occurs as a result of a mutation in the MutYH gene. This will lead to polyposis coli. An intussusception is a rarely seen symptom. Patients need preventive surgical treatment because of the high risk developing a colorectal carcinoma

Abstract: OBJECTIVE: To describe surgical complications, and patient and kidney transplant survival rates in children who have had a kidney transplant. DESIGN: Retrospective. METHOD: Data were analysed concerning the children who had been treated with a renal transplantation in the period 1985-2001 because of terminal renal insufficiency in the Emma Children's Hospital of the Academic Medical Centre in Amsterdam, The Netherlands, with arbitrary end date October 7, 2002. RESULTS: In the study period, 55 primary kidney transplantations were performed on 24 girls and 31 boys. 13 living related and 42 post mortem transplantations were performed. The extra-peritoneal approach was performed in all recipients. Up to October 7, 2002, 14 surgical complications (25%) developed: 4 cases of renovascular thrombosis, 4 urinary leakages, 6 urethral strictures, 1 urethral necrosis, 5 haematomas and 3 lymphoceles. 6 patients lost their grafts due to surgical complications (11%); 4 due to thrombosis, 1 due to urethral necrosis and 1 due to haemorrhage or haematoma. Due to hypertension secondary to the underlying kidney disease 2 patients died within one year after transplantation from hypertensive encephalopathy and cerebral bleeding respectively. The one- and five-year graft survival was 83% and 74% for living related transplantations respectively, and for post mortem transplantations 78% and 68% respectively. The main causes of graft loss were chronic rejection (9/21; 43%), acute rejection (4/21; 19%), thrombosis (4/21; 19%) and surgical complications (2/21; 10%). Primary nonfunction was the only factor with a negative prognostic value for graft survival

Abstract: The first description of oesophageal atresia dates back to 1670. Oesophageal atresia used to be regarded as a lethal disease, but since the successes of Leven and Ladd in 1939 and Haight in 1941 surgical correction is possible. The Dutch history ofoesophageal atresia begins in 1946. After the successes in America became known, several Dutch surgeons started to treat patients with oesophageal atresia, notably J.ten Kate, L.D.Eerland, M.Schoorl and P.J.Kooreman. In those days, paediatric surgery did not yet exist as a separate specialty. Today, paediatric surgery is concentrated in 6 paediatric surgical centres in the Netherlands. Thanks to the pioneers mentioned and the concentration of knowledge and expertise in the centres, the mortality ofoesophageal atresia patients in the Netherlands has decreased to approximately 9%, despite the fact that currently the gestational age and birth weight of patients is lower and the number of patients with comorbidity is higher

Abstract: Three children presented with bilious vomiting due to malrotation at the age of 3, 5 and 12 years, respectively. They were treated surgically and recovered fully. In the period 1989-2002, 12 patients were operated for malrotation after the first year of life in the Emma Children's Hospital AMC, Amsterdam, the Netherlands. The mean age at operation was 5 years (range: 1-15). The most important symptoms were (bilious) vomiting and abdominal pain. Four patients had a history of bilious vomiting in the neonatal period. Nine patients had been previously admitted to a hospital with abdominal complaints. The most sensitive imaging technique was an upper gastrointestinal contrast study. Malrotation with intermittent volvulus can cause recurrent abdominal complaints and vomiting in children. Bilious vomiting is pathologic at any age and should lead to further investigations, preferably an upper gastrointestinal contrast study to exclude malrotation or other obstructions

In particular, we analyze the existence and uniqueness of source-type self-similar solutions in planar and circular symmetries. We find that for shear-thinning fluids there exist a family of such solutions representing both finite and zero contact angle drops and that the solutions with zero contact angle are unique.
