Neonatal yoksunluk sendromu (NYS) prenatal bagimlilik yapan maddelere maruziyet sonrasi cekilme semptomlariyla ve fiziksel bagimlilikla karakterize klinik bir durumdur. Opioid disindaki tum ilac siniflari icin bu belirtiler genellikle kendi kendini sinirlar ve farmakolojik tedavi gerektirmez. Gelismis ulkelerde hamile kadinlarin %4.4'u yasadisi ilac kullanmaktadir. Gebelik boyunca eroine maruziyet prematurite, aciklanamayan intrauterin gelisme geriligi, plasenta yetmezligi, dogum sonrasi kanama, preeklampsi ve erken membran rupturu de dahil olmak uzere fetal olum ve neonatal morbidite ile iliskilidir. Ulkemizde neonatal yoksunluk sendromu ile dogan bebeklerle ilgili oldukca sinirli sayida olgu sunumu yapilmistir.

Parotit on tanisi ile takip edilen hastanin izleminde makulopapuler dokuntulerin olusmasi, fasiyal paralizi gelismesi ve oftalmotolojik muayenesinde bilateral anterior uveit saptanmasi uzerine Heerfordt sendromu dusunulerek Galyum-67 sintigrafisi yapildi. Yapilan sintigrafide her iki lakrimal bezde, parotis ve tukuruk bezlerinde artmis aktivite izlendi. Sarkoidoz tanisi guclenen hastaya steroid tedavisi baslandi. Yapilan ince igne aspirasyon biyopsisi sonucu ise negatif olarak geldi. Bu olgu, parotit ve fasiyal paralizinin ender gorulen nedenine dikkat cekmek icin sunulmustur.

Heerfordt syndrome, a rare neurological involvement in sarcoidosis, is a syndrome characterized by bilateral uveitis, facial paralysis, fever, and is indicated by signs of parotitis. A fifteen-year-old male patient with fever, and vomiting was admitted to our clinic with a complaint of swelling of parts in both cheeks. During the follow-up of the patient, parotitis, formation of maculopapular rash, facial paralysis, and bilateral anterior uveitis syndrome was developed. Suspecting a diagnosis of Heerfordt, a Gallium-67 scintigraphy was performed. Increased activity was observed in the parotid and salivary and lacrimal glands. The patient was diagnosed with sarcoidosis and steroid therapy was began. The result of the fine needle aspiration biopsy was negative. Neonatal abstinence syndrome (NYS) is a clinical condition characterized by physical dependence and withdrawal symptoms after prenatal exposure to addictive substances. These symptoms are self-limiting and usually do not require pharmacological treatment for all classes of drugs, except for opioids. In developed countries 4.4 % of pregnant women use illegal drugs. Heroin exposure during pregnancy causes prematurity, unexplained intrauterine growth restriction, placental insufficiency, postpartum hemorrhage, including pre-eclampsia and premature rupture of membranes which is associated with fetal death and neonatal morbidity. Sismik olayların yol açtığı bu tür bir etkinin iyonkürenin dinamik yapısı içinde güvenilir şekilde tespit edilebilmesi depremlerin erken tahminini sağlayabilecek önemdedir. Bu çalışmada bir YKS alıcı ağından düzenli olarak elde edilen Toplam Elektron İçeriği (TEİ) ölçümleri kullanılarak iyonküredeki yerel değişimlerin tespit edilmesine yönelik bir teknik geliştirilmiş ve bu tekniğin güvenilir bir deprem öncül sinyali üretip üretemediği 2010 ve 2011 yıllarını kapsayan bir zaman aralığında test edilmiştir. Geliştirilen deprem öncül tespiti tekniğinin bu tarih aralığında Türkiye’de Richter ölçeğinde 5 ve üzeri büyüklükte meydana gelen 23 depremin 15’ini tespit edebildiği ve 8 yanlış alarm verdiği gözlemlenmiştir. Recent studies enlightened the fact that seismic activities cause variations on ionospheric electron content based on electrical fields around stressed rock formations triggering ionization on air and ionosphere. Robust detection of seismic activity triggered variations in dynamic state of the ionosphere has a significant effect on early detection of earthquakes. In this study, a technique based on using Total Electron Content (TEC) measurements obtained from a GPS network and detecting local ionospheric variations is proposed. Reliability of generating an earthquake detection signal by implementing proposed technique is tested over a period of 2 years in 2010 and 2011. It is observed that proposed earthquake detection technique is able to detect 15 out of 23 earthquakes of magnitude larger than 5 in Richter scale while generating 8 false alarms within this time period around Turkey. © 2014 IEEE.

Bu çalışmada, belirli bir coğrafik bölge üzerinden bulunan bu profilin, en küçük kareler yöntemi ile doğrusal ve küresel düzlem modellerine oturtulmakta incelenmiştir. IRI-PlasG ile gerçeğe yakın profiller elde edilerek, bahsi geçen modellerin Ankara çevresindeki bir bölge için bu profillere uygunluğu gösterilmektedir The plasma frequency - height profile of the ionosphere has an important impact on sky-wave HF communications. In this study, the profile obtained over a geographical region is fit to linear and spherical plane models by using the least-squares method. Conformity of these models to the plasma frequency-height profile is verified by comparing to the profile provided by IRI-Plas-G for a region around Ankara. © 2014 IEEE.

In this study, research activities of IONOLAB group on remote sensing and 2-D imaging of ionosphere in the last 10 years will be summarized. In TUBITAK EEEAG 105E171 and 109E055 projects, a novel Total Electron Content (TEC) estimation method, IONOLAB-TEC, is developed using the dual frequency Global Positioning System (GPS) pseudo range and phase delay recordings. The IONOLAB-TEC computation is provided from www.ionolab.org as an important and unique Space Weather service that can estimate robust, reliable and accurate single station TEC values. The sparse estimates of GPS-TEC in space and time are interpolated regionally and globally and TNPGN-Active IONOLAB-TEC values are mapped using novel space-time interpolation methods. Ionospheric climatic model IRI-Plas is used as a background for fast and robust electron density distributions first time in literature. Critical Ionosperic parameters are interpolated to obtain global and regional distributions. Critical frequency and height maps are provided in www.ionolab.org as another Space Weather Service. There have been physical and empirical studies for modeling the ionosphere. International Reference Ionosphere extended to Plasmasphere (IRI-Plas) is the most recent model developed for this purpose. However, IRI-Plas presents a model about the ionosphere and its compliance with the instantaneous state of the ionosphere does not provide the accuracy needed for engineering purposes. One of the important information sources about the instantaneous state of the ionosphere is GPS signals. In this study, constructing the ionosphere which is compatible with both the instantaneous ionosphere measurements and the physical structure of the ionosphere is presented as an optimization problem, and solved by using the particle swarm optimization technique. The ionosphere over Turkey is investigated by using the proposed optimization method and the importance of the instantaneous ionosphere measurements obtained from GPS signals is demonstrated.

3-OH-3-methyl glutaric aciduria is a metabolic disease that shows autozomal recessive heredity and presents with life threatening hypoglycemia attacks in the first few years of life. Here, a case of metabolic attack developed after an infection is presented. A case of a five months old male infant was brought to our hospital with complaints of vomiting and diarrhea. His mother and father were relatives. The general condition of the patient was poor with tachypnea, hypotonia and he also had hepatomegaly. Laboratory findings were: leucocyte 35.400/mm3, hemoglobin: 9.6 g/dL, hematocrit: %29.3, thrombocyte: 559.000/ mm3; blood glucose: 32 mg/dL, aspartate aminotransferase: 587 U/L, alanine aminotransferase: 582 U/L, total bilirubin: 1.6 mg/dl, direct bilirubin: 0.6 mg/dl. Artery blood gas findings were pH: 7.36, pCO2: 17.1, pO2: 89.3, HCO3: 9.6, serum ammonia level: 205 mcg/dL. Urine blood amino acid analysis findings showed an increase in the excretion of 3-OH valeric acid, 3-methyl glutaconic acid, and a 3-OH-3-methly glutaric acid. It should be remembered that hereditary metabolic diseases are not rare and metabolic diseases should be considered in patients with clinical findings of sepsis or Reye's syndrome., 3-OH-3-metil glutarik asidüri, otozomal resesif kalıtım gösteren ve yaşamın ilk yılında hayatı tehdit edici hpoglisemi atakları ile seyreden metabolik hastalıktır. Bu yazıda, enfeksiyon sonrası metabolik atak gelişen bir olgu sunulmuştur. Beş aylık erkek hasta kusma, ishal şikayeti ile hastanemize başvurdu. Anne baba arasında akrabalık vardı. Genel durumu kötü, takipneik, hipotonik olan hastanın hepatomegalisi mevcuttu. Laboratuvar incelemelerinde; kanda lökosit 35.400/mm3, hemoglobin: 9.6 g/dL, hematokrit: %29.3, trombosit: 559.000/mm3, kan şekeri: 32 mg/dL, aspartat aminotransferaz: 587 U/L, alanin aminotransferaz: 582 U/L, total bilirübin: 1,6 mg/dL, direk bilirübin: 0.6 mg/dL bulundu. Arter kan gazında; pH: 7.36, pCO2: 17.1, pO2: 89.3, HCO3: 9.6, serum amonyak: 205mcg/dL idi. İdrar kan aminoasitlerinin incelemesi sonucunda; 3-OH valerik asit, 3-metil glutakonik asit ve 3-OH- 3-metil glutarik asit atılımında artış saptandı. Kalıtsal metabolik hastalıkların nadir olmadığı, sepsis veya Reye sendromu benzeri klinik bulgularla başvuran hastalarda metabolik hastalığın mutlaka düşünülmesi gerektiği hatırlanmalıdır.

Conference Name: AOGS-AGU (WPGM) Joint Assembly, 2012 Date of Conference: 13-17 August 2012 Constant monitoring and prediction of Space Weather events require investigation of the variability of total electron content (TEC), which is an observable feature of ionosphere using dual-frequency GPS receivers. Due to various physical and/or technical obstructions, the recordings of GPS receivers may be disrupted resulting in data loss in TEC estimates. Data recovery is very important for both filling in the data gaps for constant monitoring of ionosphere and also for spatial and/or temporal prediction of TEC. Spatial prediction can be obtained using the neighboring stations in a network of a dense grid. Temporal prediction recovers data using previous days of the GPS station in a less dense grid. In this study, two novel and robust spatio-temporal interpolation algorithms are introduced to recover TEC through optimization by using least squares fit to available data. The two algorithms are applied to a regional GPS network, and for a typical station, the number of days with full data increased from 68% to 85%.

Date of Conference: 22-27 April 2012 In recent years, a strong coupling between ionospheric disturbances and seismic activity has been observed through the increase of ion temperatures, critical frequencies of ionospheric layers and Total Electron Content (TEC) before high magnitude earthquakes. TEC is defined as total number of electrons over a ray path through the ionosphere. TEC can be estimated in a cost-effective way with dual-frequency Global Positioning Satellite (GPS) System receivers. The unit of TEC is given by TECU where 1 TECU = 1016 el/m2. In this study, the disturbances in daily TEC values before 23 October 2011, Mw=7.2 Earthquake in Van, Turkey are investigated using Turkish National Permanent GPS Network (TNPGN-Active). Earthquake Day Period (EDP) is chosen between October 1 and 31, 2011. Daily TEC values, for each station and each day, are estimated as IONOLAB-TEC (www.ionolab.org) with 30 s time resolution. EDP-TEC values are compared with an Average Quiet Day TEC (AQDT) which is obtained by averaging the TEC values between 25 and 28 March, 2011. Statistical comparison is accomplished using Symmetric Kullback-Leibler Divergence (SKLD), which is also a method for measuring entropy of a system. It has been previously observed that SKLD is a better method for measuring the amount of disturbances compared to L2 norm and cross-correlation coefficient. AQDT is also compared with magnetically Quiet Day Period (QDP) from 25 to 28 April, 2011, during which Kp and Dst indices indicate a very quiet ionospheric and magnetospheric period. Also, in order to measure the variability between the consecutive days, TEC values for each day during EDP and QDP are compared with the TEC values of the following day. A third measure of W-index is also applied to identify the local disturbances in the ionosphere, where TEC of a given day is compared to the median of seven days prior to the day of investigation logarithmically. Since W-index is obtained for each epoch, the within-the-day variability can also be monitored. It has been observed that peak TEC values for all stations in TNPGN increase 10 to 15 TECU two days prior to the earthquake. The SKLD values for comparison of EDP and AQDT also peak on 21st of October, 2011, two days prior to the earthquake. Since ionospheric disturbance can be observed on all days prior to the earthquake, comparison of TEC for consecutive days for each station using SKLD does not provide extra information. The W-index values indicate that there may be small scale variability for stations closer to the earthquake epicenter. When compared with previous earthquakes that occurred in Turkey with magnitudes 4.5 and 5.2 on Richter scale, this 7.2 magnitude earthquake has been felt as an ionospheric disturbance for stations especially on Northern Anatolian Fault. The results also indicate the need for constant monitoring and statistical decision theory for detection of earthquake precursors.

Modeling and monitoring of ionospheric variability is a major part of satellite communication, navigation and positioning systems. Total Electron Content (TEC), which is defined as the line integral of the electron density along a ray path, is one of the parameters to investigate the ionospheric variability. Dual-frequency GPS receivers, with their world wide availability and efficiency in TEC estimation, have become a major source of global and regional TEC modeling. When Global Ionospheric Maps (GIM) of International GPS Service (IGS) centers (http://iono.jpl.nasa.gov/gim.html) are investigated, it can be observed that regional ionosphere along the midlatitude regions can be modeled as a constant, linear or a quadratic surface. Globally, especially around the magnetic equator, the TEC surfaces resemble twisted and dispersed single centered or double centered Gaussian functions. Particle Swarm Optimization (PSO) proved itself as a fast converging and an effective optimization tool in various diverse fields. Yet, in order to apply this optimization technique into TEC modeling, the method has to be modified for higher efficiency and accuracy in extraction of geophysical parameters such as model parameters of TEC surfaces. In this study, a modified PSO (mPSO) method is applied to regional and global synthetic TEC surfaces. The synthetic surfaces that represent the trend and small scale variability of various ionospheric states are necessary to compare the performance of mPSO over number of iterations, accuracy in parameter estimation and overall surface reconstruction. The Cramer-Rao bounds for each surface type and model are also investigated and performance of mPSO are tested with respect to these bounds. For global models, the sample points that are used in optimization are obtained using IGS receiver network. For regional TEC models, regional networks such as Turkish National Permanent GPS Network (TNPGN-Active) receiver sites are used. The regional TEC models are grouped into constant (one parameter), linear (two parameters), and quadratic (six parameters) surfaces which are functions of latitude and longitude. Global models require seven parameters for single centered Gaussian and 13 parameters for double centered Gaussian function. The error criterion is the normalized percentage error for both the surface and the parameters. It is observed that mPSO is very successful in parameter extraction of various regional and global models. The normalized reconstruction error varies from 10-4 for constant surfaces to 10-3 for quadratic surfaces in regional models, sampled with regional networks. Even for the cases of a severe geomagnetic storm that affects measurements globally, with IGS network, the reconstruction error is on the order of 10-1 even though individual parameters have higher normalized errors. The modified PSO technique proved itself to be a useful tool for parameter extraction of more complicated TEC models.

Gerçek veriye dayalı sonuçlar, geliştirilen tekniğin yüksek başarımlı kestirimler ürettiğini göstermiştir. GPS signals are crucial, because they are used to estimate the electron density in the ionosphere. However, sometimes GPS receivers can not receive signals. In order to estimate ionospheric electron density during this cutoff, the interpolation of the data is necessary. In this paper, a new interpolation scheme that uses spatio-temporal correlation in the GPS network is proposed. The simulation results on real data show that the proposed technique produces promising results. © 2011 IEEE.

With the improvement in the computer speed and memory, Numerical Methods are frequently used in the solution of electromagnetic problems. Numerical Methods can be classified as the frequency domain and the time domain based methods. While the time domain methods are suitable for modeling of the transient response and wideband problems, the frequency domain methods are suitable for modeling of the steady state response and narrow band problems. A numerical method that has the advantages of both time and frequency domain approaches can be developed. Applying Fractional Fourier Transform in space and/or time can reduce the computational complexity for some cases. The Fractional Fourier Transform is a generalization of the continuous Fourier Transform. In last decades, there are several studies and applications concerning this transform. Generally, it is used in signal processing and noise filtering. In this study, Fractional Fourier Transform is applied to the Maxwell's Equations for the first time in literature. Aim: Henoch Schönlein purpura (HSP) is the most common systemic vasculitis in children. Characterized by small vessel vasculitis. The aim of this study was to determine demoraphic characteristics of HSP patients. Material and Methods: The medical records of 168 children with HSP revieved retrospectiveley. They were administered to Turkish Ministry of Health Ankara Training and Research Hospital Pediatric Clinic between March 2004- December 2007 and followed up at least 6 months. Results: Among the 168 patients enrolled to the study 102 were male (%61) and 66 were female (%39). Mean age was 8.8±3.1 years. Follow up period was 6-66 months (29±17,6). All patients had purpuric skin lesions (%100) , 59 had arthritis/ artralgia, 34 had GIS, 33 had renal and 2 had testicular involvement. Patients were % 80 male who had GIS involvement. Except GIS involvement there is no relationship between sex and other system involvement (p>0,05. Patients who had GIS involvement assessed with USG and found that 5 had intestinal edema (%35), 3 had asit (%22) and 1 had right upper quadrant thickness due to hemorrhage. Among the patients who had renal involvement 12 had (%35) proteinuria less than 40 mg/m2/hour, 2 had nephrotic proteinuria and 20 patients had isolated hematuria. The most common affected joint was found to be as ankle (%73). The illness didn't relapse in 148 of all patients, 12 had one, 5 had two, 3 patients had three reccurrences. Conclusion: In our study purpuric skin lesions are the most common symptom ,secondly arthrit and arthralgia found at patients who were following for HSP. Renal and GIS involvement found %20 of patients. Long term follow up had to be made for the patients especially who had renal involvement with routine ürinalysis for proteinuria, Amaç: Henoch Schönlein Purpurası (HSP) çocukluk çağında sık karşılaşılan sistemik vaskülittir. Çalışmamızda HSP tanısıyla takip edilen hastaların demografik özelliklerinin değerlendirilmesi amaçlandı. Yöntem ve Gereç: 1 Mart 2004/31 Aralık 2007 tarihleri arasında S.B. Ankara Eğitim ve Araştırma Hastanesi Çocuk Sağlığı ve Hastalıkları Kliniğine başvuran HSP tanısı ile izlenen en az 6 ay düzenli izlemlerine devam edilmiş 168 hasta retrospektif olarak değerlendirildi. Bulgular: Çalışmaya dahil edilen hastaların 102'si erkek (%61), 66‘sı kızdı (%39). Yaş ortalaması 8.8± 3.1 yıl olarak bulundu. İzlem süresi 6-66 aydı (29±17,6 ay). Hastaların 168'inde (%100) purpurik döküntü, 59' unda (%35) artrit/artralji, 34‘ünde (%20) gastrointestinal sistem (GİS), 33‘ünde (%20) renal, 2‘sinde (%1) testiküler tutulum saptandı. GİS tutulumu olan çocukların %80'i erkekti. GİS dışında diğer sistem tutulumları ile cinsiyet arasında anlamlı ilişki saptanmadı (p>0,05). GİS tutulumu olan hastaların ultrasonografi incelemesinde 5 hastada (%35) barsak duvarında ödem, 3 hastada (%22) barsak ansları arasında serbest sıvı, 1 hastada ise sağ alt kadranda kanamaya bağlı kalınlaşma saptandı. Böbrek tutulumu olan hastaların 12'sinde (% 35) 40 mg/m2/saate ulaşmayan düzeyde proteinüri, 2'sinde (%6) nefrotik düzeyde proteinüri saptandı. 20 hastada izole hematüri bulundu. En sık ayak bileği (%73) eklemi tutulumu tespit edildi. Yüzkırksekiz hastada hastalık tekrarlamazken, 12 hastada bir, 5 hastada iki, 3 hastada ise üç kez hastalık tekrarladı. Sonuç: Çalışmamızda HSP tanısıyla izlenen hastalarda en sık rastlanan semptom purpurik döküntü iken ikinci sıklıkla artrit/artralji bulunmuştur. Hastaların % 20'sinde GİS ve renal tutulum saptanmıştır. Özellikle renal tutulum olan hastaların uzun dönemde idrar tetkiki ile proteinüri açısından düzenli takiplerinin yapılması gerekmektedir.

Global Positioning System provides cost-effective means for TEC estimation. Regularized TEC estimation method (D-TEI) is developed to estimate high resolution, robust TEC values. The method combines measurements of GPS satellites above 10° elevation limit and estimates can be obtained with 30 s time resolution. In this paper, parameters that are used in D-TEI method such as ionospheric height, weighting function, and satellite receiver biases are studied. It is found that TEC estimation results of D-TEI method is almost independent of ionospheric height. Different weighting functions are tried and the weighting function that minimizes non-ionospheric effects is selected. By using satellite and receiver biases in the correct form consistent TEC estimation results are obtained with IGS analysis centers. In this paper, the method is improved to include phase measurements. Taking either pseudorange or phase measurements as input, high resolution, robust TEC estimates are obtained using D-TEI method.

Bu yöntem özellikle iyonosfer fiziği veya uydu haberleşmesi gibi alanlar için iyonosferin incelenmesine olanak sağlamaktadır. Bu yöntemde Küresel Yer Belirleme (KYB) uyduları ve bu uydulardan gelen ışınları toplamak üzere yeryüzüne belirli bir geometride yerleştirilen alıcılar kullanılır. Alıcılar KYB uydularından aldıkları sinyaller ile iyonosfere ait olan Toplam Elektron İçeriği (TEİ) verisinin hesaplanmasına olanak sağlar. TEİ verileri ve tomografik yeniden yapılandırma algoritmaları kullanılarak, iyonosfer elektron yoğunluğu görüntülerinin elde edilmesi sağlanır. Gerçekleştirilen bu çalışmada tek bir GPS uydusu ve bu uyduyu izlemek üzere yeryüzünde belirli bir enlem noktasına yerleştirilmiş bir alıcı senaryosu canlandırıldı. Alıcıda oluşan hatalar ve model hataları bu çalışmada ihmal edildi. Bu senaryo içerisinde değişik açılım işlevleri ve yeniden yapılandırma algoritmaları kullanılarak, iyonosfer elektron yoğunluğunun iki boyutlu olarak elde edilmesi gerçekleştirildi. Sonuçların başarım analizleri ileri model olarak kullanılan IRI-95 ile karşılaştırılarak verildi. Computerized Ionospheric Tomography (CIT) is a method to investigate ionospheric electron density in two or three dimensions. CIT provides a flexible method for studying ionosphere. In CIT, GPS satellites and Earth based receivers are considered to perform tomographic reconstruction. The received signals are processed to calculate Total Electron Content (TEC). TEC values and the tomographic reconstruction algorithms are used together to obtain tomographic images of electron density of ionosphere. In this study, scenario with one GPS satellite and one receiver is simulated. Receiver error and model error are neglected. In simulation, a set of basis functions and algorithms are used and two dimensional tomographic images of ionospheric electron density in height and latitude is obtained. Comparative reconstruction results are given based on IRI-95 forward ionosphere model. © 2005 IEEE.
