The aim of this exploratory study was to evaluate diaphragmatic function across various neuromuscular diseases using spirometry-controlled MRI. We measured motion of the diaphragm relative to that of the thoracic wall (cranial-caudal ratio vs. anterior posterior ratio; CC-AP ratio), and changes in the diaphragmatic curvature (diaphragm height and area ratio) during inspiration in 12 adults with a neuromuscular disease having signs of respiratory muscle weakness, 18 healthy controls, and 35 adult Pompe patients – a group with prominent diaphragmatic weakness. CC-AP ratio was lower in patients with myopathies (n=7, 1.25±0.30) and motor neuron diseases (n=5, 1.30±0.10) than in healthy controls (1.37±0.14; p=0.001 and p=0.008), but not as abnormal as in Pompe patients (1.12±0.18; p=0.011 and p=0.024). The mean diaphragm height ratio was 1.17±0.33 in patients with myopathies, pointing at an insufficient diaphragmatic contraction. This was also seen in patients with Pompe disease (1.28±0.36), but not in healthy controls (0.82±0.33) or patients with motor neuron disease (0.82±0.24). We conclude that spirometry-controlled MRI enables us to investigate respiratory dysfunction across neuromuscular diseases, suggesting that the diaphragm is affected in a different way in myopathies and motor neuron diseases. Whether MRI can also be used to evaluate progression of diaphragmatic dysfunction requires additional studies.

Open pelvic fractures are devastating injuries, rare, and with high mortality. Leading causes of mortality are: haemorrhage, infection and associated injuries. The aim of this paper is to point out methods of treating these injuries and great number of prognostic mortality factors. Material - in period from January 2011 to December 2015, 221 patients with pelvis ringfracture were treated in three large clinical centers of Serbia, of which 13(5%) had an open fracture type. We have classified pelvic ring fractures according to the Young - Burgess classification. We have classified injuries according to Gustilo at I, II, and III degree, and the location of the wound according to Faringer classification was distributed in zone I, II and III. Urogenital and intra-abdominal injuries were monitored, and severity of injuries was determined according to Severity Score Injury (ISS) and Trauma Score (TS). Results - there were 6(46%) women and 7(54%) men at the average age of 41(13 - 76). Injuries from traffic trauma are dominant. The most common cause of pelvic ring fracture is an anterior posterior compression - 6(46%), lateral compression - 4(31%) and vertical force in 3 (23%) patients. Dominant injuries are type I and II according to Gustilo, and zone I according to Faringer classification. There were 6 (46%) patients with urogenital injuries, and the same number with intra-abdominal injuries, of which 3(23%) patients have been treated with colon resection and diversion. Due to abundant hemorrhage and hypovolemic shock 2 patients died, and another one died after three days due to sepsis and multisystem organ failure (MSOF). Conclusion - Open pelvic fractures have high mortality rate, due to: haemorrhage, infection, associated abdominal and genitourinary tract injuries, ISS> 25, TS 65 years.

KEY MESSAGE Knowledge of the changes that occur in the abdominal wall after component separation (CS) is essential for understanding the mechanisms of action of the various CS techniques, the changes observed on computed tomography images, and, perhaps most importantly, the anatomic and physiologic changes observed in patients who have undergone CS. Purpose Component separation (CS) techniques are essential adjuncts during most abdominal wall reconstructions. They allow the fulfillment of most modern abdominal wall reconstruction principles, especially primary closure of defects and linea alba restoration under physiologic tension. Knowledge of the post-CS abdominal wall changes is essential to understanding the mechanism of action of the various types of CS, the changes observed on computed tomographic images, and, perhaps most importantly, the anatomic and physiologic changes following CS techniques. Methods A systematic review of the literature was conducted using the PubMed database and other sources to identify articles describing abdominal wall changes after CS Results After excluding non-pertinent articles, 14 articles constituted the basis for this review. Conclusions After reviewing the literature on post CS abdominal wall changes, we conclude the following: (1)The external oblique muscle is significantly displaced laterally after anterior CS, the transversus abdominis muscle shifts very little after posterior CS, and muscle trophism is generally maintained after both techniques. These findings are consistent for both open and minimally invasive CS. (2) The anatomy and physiology of abdominal wall muscles are preserved mainly by the muscles' overlapping function and their ability to undergo compensatory trophism after midline restoration (reloading). (3) Well-performed CS techniques have a low risk of producing bulging and semilunar line hernias. (4) Anterior and posterior CS techniques probably have different mechanisms of action. (5) Current studies on how the nutritional status and postoperative conditioning can alter abdominal wall changes after CS and the mechanisms of the actions involved in anterior and posterior CS are underway.

Purpose: Unstable pelvic ring injuries produced by external rotation of the hemipelvis and a symphyseal disruption are most often treated with internal fixation of the anterior ring, with percutaneous treatment of the posterior ring as needed. In some clinical situations, patients are treated with external fixation for their anterior injuries and the long-term functional outcomes associated with external fixation are not well understood. We ask if there is a difference in functional outcome, between treatment of these injuries with internal versus external fixation, when measured at a minimum of three years after injury. Method: This was a retrospective cohort study performed at a level one regional trauma center. Trauma database review identified 128 patients, with 70 subsequently excluded, with unstable anterior posterior compression (APC) pelvic ring injuries (OTA 61B2.3 & 61C1.2) treated with surgery with minimum three years of follow-up. An intervention of internal fixation versus external fixation of anterior pelvic ring was performed, and depending on the injury, supplemented with posterior iliosacral screw fixation. Main outcome was measured with the Majeed functional outcome score (0-100). Results: Patients treated with external fixation reported a Majeed score of 70 (95% CI 28-100) compared to 79 (95% CI 36-100) in those with internal fixation (p-value 0.28). Subgroups of the Majeed score were not significantly different (p value > 0.05). Open fractures, severity of injury, and ISS were worse in those treated with external fixation. There was no differential loss to follow-up. Conclusion Patients with unstable pelvic ring injuries with symphyseal disruptions treated with external fixation as definitive treatment versus internal fixation may fare no different in the long term.

Background Assistive devices provide balance and stability to those who require a greater base of support, especially during ambulation or in tasks essential to functional daily living. In ambulatory assistive device use, center of pressure (COP) movement as one of the measurements of postural control is a factor when assessing fall risk, with an overall goal of maintaining postural equilibrium. There is a lack of research on assistive devices, such as walkers, regarding measurable outcome variables related to fall risk. The purpose of this study was to determine how much the postural control in single limb stance during a balance perturbance is affected by utilizing three different walker types designed to promote stability: the standard walker, the front-wheeled walker with straight wheels and the front-wheeled walker with caster wheels. Research question Is postural sway control in single limb stance during a balance perturbance affected by walker type? Methods Twenty-three healthy adults participated and gave consent. The NeuroCom® SMART EquiTest® system was utilized to simulate forward falls. Subjects stood on the system’s force plate, which was tipped backwards quickly, forcing subjects to attempt to maintain balance. Each participant experienced 18 simulated perturbations, during which they were asked to maintain balance while using one of three walkers in single-limb stance. Each trial was completed using random assignment of three different walker types. Leg dominance of the stance leg was also randomized for each trial. Results The type of walker and the leg dominance in the standing limb significantly affected postural control in a balance perturbance in the A-P direction. The walker type significantly affected the COP maximum displacement in anterior-posterior (AP) direction. However, the leg dominance significantly affected COP maximum displacement in AP and medio-lateral (ML) directions and COP velocity in AP direction. Significance The findings suggest that when fully grounded, a standard walker is more stable than the front-wheeled walker. However, this does not indicate that the standard walker is more stable than other types of walkers when it is being picked up and moved forward during normal use.

Introduction: There has always been debate regarding the relationship between vertical growth pattern and obstruction of the upper and lower pharyngeal airways. Objectives: The present study was conducted to compare the dimensions of airway in cephalometric radiographs of patients with skeletal class I, II and III malocclusions, which all have a vertical growth pattern. Methods: 66 lateral cephalometric radiographs, all of which had a vertical growth pattern, were selected, and divided into three groups (class I, II and III). The points and reference lines required to measure the area of the airway were identified. The percentage of the nasopharyngeal area occupied by the airway was calculated, and data were analyzed by SPSS version 25 software. Background Readmission after spine surgery is costly and a relatively common occurrence. Previous research identified several risk factors for readmission; however, the conclusions remain equivocal. Machine learning algorithms offer a unique perspective in analysis of risk factors for readmission and can help predict the likelihood of this occurrence. This study evaluated a neural network (NN), a supervised machine learning technique, to determine whether it could predict readmission after 3 lumbar fusion procedures. Methods The American College of Surgeons National Surgical Quality Improvement Program database was queried between 2009 and 2018. Patients who had undergone anterior, lateral, and/or posterior lumbar fusion were included in the study. The Python scikit Learn package was used to run the NN algorithm. A multivariate regression was performed to determine risk factors for readmission. Results There were 63,533 patients analyzed (12,915 anterior lumbar interbody fusion, 27,212 posterior lumbar interbody fusion, and 23,406 posterior spinal fusion cases). The NN algorithm was able to successfully predict 30-day readmission for 94.6% of anterior lumbar interbody fusion, 94.0% of posterior lumbar interbody fusion, and 92.6% of posterior spinal fusion cases with area under the curve values of 0.64–0.65. Multivariate regression indicated that age >65 years and American Society of Anesthesiologists class >II were linked to increased risk for readmission for all 3 procedures. Conclusions The accurate metrics presented indicate the capability for NN algorithms to predict readmission after lumbar arthrodesis. Moreover, the results of this study serve as a catalyst for further research into the utility of machine learning in spine surgery.

Background: The soft tissue thickness is affected by anterior posterior skeletal relationship. This study has been designed to evaluate the soft tissue thickness among different anterior posterior skeletal classifications. Material and Methods: In this cross-sectional study, 206 digital lateral cephalometric radiographs from patients undergoing treatment at several orthodontic clinics were evaluated. The skeletal group was determined by the ANB angle. The planned points were determined on digital radiographs using the Digimizer.V4.1.1.0 and then the measurements were done. Data were analyzed by Kruskal-Wallis, Mann-Whitney, Pearson Correlation and Spearman tests. Results: Significant difference showed between soft tissue thickness at Subnasale between class I and II, at Subnasale, Labrale Superius, Stomion Superius, Stomion Inferius between class I and III and at Subnasale, Labrale Superius, Stomion Superius, Stomion Inferius, between class II and III. Among the women, soft tissue thickness at subnasale were higher in class I group compared to class II, Subnasale, Labrale Superius, Stomion Superius, Stomion Inferius were higher in class III compared to class I and at Labrale Inferius was higher in class II compared to class III. Among the men, soft tissue thickness at Stomion Superius, Stomion Inferius and Labrale Superius were higher in class III group compared to class I and II. Conclusion: We established that soft tissue thickness in some landmarks were significantly different between skeletal groups and gender. There is a correlation in soft tissue thickness and skeletal relationship at Stomion Inferius, Subnasale, Labrale Superius, Stomion Superius. The orbitofrontal cortex (OFC) plays a critical role in the flexible control of behaviors and has been the focus of increasing research interest. However, there have been a number of controversies around the exact theoretical role of the OFC. One potential source of these issues is the comparison of evidence from different studies, particularly across species, which focus on different specific sub-regions within the OFC. Furthermore, there is emerging evidence that there may be functional diversity across the OFC which may account for these theoretical differences. Therefore, in this review we consider evidence supporting functional heterogeneity within the OFC and how it relates to underlying anatomical heterogeneity. We highlight the importance of anatomical and functional distinctions within the traditionally defined OFC subregions across the medial-lateral axis, which are often not differentiated for practical and historical reasons. We then consider emerging evidence of even finer-grained distinctions within these defined subregions along the anterior-posterior axis. These fine-grained anatomical considerations reveal a pattern of dissociable, but often complementary functions within the OFC. (PsycInfo Database Record (c) 2021 APA, all rights reserved).

(1) To evaluate the reliability of 9 commonly used quantitative parameters of the trochlear morphology on computed tomography (CT) and (2) to analyze for differences in the reliability regarding patient subgroups (patellofemoral instability [PFI] vs non-PFI).A retrospective analysis of lower-limb CT scans performed between August 1996 and February 2013 was performed. The CT scans of all patients with PFI and 30 randomly selected cases without a history of PFI (non-PFI) were included. The following measurements were performed on 1 proximal axial CT slice at the entrance of the trochlear groove and 1 slice 5 mm further distal: relative medial, central, and lateral trochlear height; trochlear depth; relative transverse trochlear shift; trochlear facet asymmetry; sulcus angle; and medial and lateral trochlear slope. Four investigators performed the measurements independently, and intraclass correlation coefficients (ICCs) were calculated for the entire study group, as well as for the PFI and non-PFI groups separately.In total, 66 cases (36 PFI cases) were included in the study. We found almost perfect inter-rater and intrarater agreement for the trochlear height on both axial CT slices (ICC, 0.831-0.977). For the other measurements, we found only fair reliability (ICC0.4) on the proximal CT slice, whereas on the distal CT slice, at least moderate reliability (ICC0.4) was observed. ICCs were lower for many parameters in the PFI group. Angular values were less reliable than linear values. In particular, measurements involving the medial facet (i.e., sulcus angle, medial trochlear slope, and trochlear facet asymmetry) were less reliable.When interpreting quantitative parameters defining the trochlear morphology, one must taken into account the considerably lower reliability of angular parameters such as the commonly used sulcus angle compared with linear measurements. Radiologic measurements are less reliable in cases of PFI than in subjects without instability.Level III, retrospective case-control study.

Aim:Total body irradiation (TBI) is an external radiotherapy technique in which the whole body including the superficial regions is required to receive the therapeutic dose. The purpose of this study is to evaluate the received surface dose during TBI technique.Methods and materials:The anterior/posterior (AP/PA) TBI was implemented with 18-MV photon beam at 312-cm treatment distance for human-like phantom. The GAFCHROMIC-EBT3 films were used for superficial dose measurements.Results and discussion:The percentage of surface-absorbed dose relative to the prescription point for 8 points of measurements was between 102·78–121·48% and 104·51–127·43% at 5 and 10 mm depth, respectively. In the chest wall region due to the presence of lung blocks, the absorbed dose was below the acceptable level, so an electron boost was required to increase the chest wall absorbed dose.Conclusions:According to the results, the implemented technique was able to deliver sufficient dose to the shallow surface of phantom’s body.

Study Design: Retrospective comparative study. Objectives: Although some studies have discussed the use of lateral mass screws (LMSs) in patients with cerebral palsy (CP), it is unclear whether posterior LMS fixation alone is a suitable method. We aimed to compare the clinical, radiological, and surgical outcomes of 2 surgical modalities, namely, combined anterior-posterior (A-P) instrumented fusion and posterior fusion alone, in athetoid-type CP patients with cervical myelopathy (CM). Methods: We analyzed 63 patients with athetoid-CP and CM who underwent posterior fusion only with LMS (group A, 35 patients) and A-P fusion (group B, 28 patients). The primary outcome was the 1- and 3-year fusion rates for the surgical segments. The secondary outcomes included the clinical outcomes based on pain intensity determined using the visual analog scale score, neck disability index, and 17-point Japanese Orthopedic Association score, radiological, and surgical outcomes. Results: Fusion was achieved at 3 years postoperatively in 22 of 35 patients (63%) in group A and in 26 of 28 patients (93%) in group B ( P = 0.02). The posterior neck pain intensity was also significantly lower in group B than in group A 2 and 3 years postoperatively ( P = 0.02 and 0.01, respectively). The incidence of screw loosening and implant-related problems was higher in group A (60%) than in group B (21%) ( P = 0.01). The other clinical and radiological parameters were similar between the groups. Conclusions: For athetoid CP-induced CM, combined A-P fusion would result in superior clinical and radiological outcomes compared to posterior fusion alone.

Given the importance of the lower-limb strength and strength balance in soccer players and its relationship with injury prevention and performance, the present study compared quadriceps and hamstrings strength, the conventional (Hconc:Qconc), functional (Hecc:Qconc) hamstrings-to-quadriceps ratio and inter-limb strength asymmetry in professional, elite academy and amateur male soccer players. In this cross-sectional study, two hundred-six soccer players (professional = 75, elite academy = 68, amateurs = 63) volunteered to participate. Quadriceps and hamstrings isokinetic peak torque was investigated at 60°.s-1 in both the concentric and eccentric modality and at 300°.s-1 in the concentric modality. The conventional Hconc:Qconc, functional Hecc:Qconc ratio and quadriceps and hamstrings inter-limb strength asymmetry were then calculated. Professional players presented greater quadriceps and hamstrings strength than elite academy (effect size from small to moderate) and amateur players (moderate to very large). Both the conventional Hconc:Qconc and functional Hecc:Qconc ratio were greater in professional than elite academy and amateur players (small to moderate). Overall, quadriceps and hamstrings inter-limb strength asymmetry was greater in amateurs than professional (small to very large) and elite academy (trivial to large) players. The present findings provide coaches and medical staffs with normative lower-limb muscle strength data on professional, academy and amateur soccer players. Overall lower-limb muscle strength and inter-limb strength asymmetry could be used to evaluate possible inference on injury prevention and performance. The hamstrings-to-quadriceps ratio poorly differentiates between the soccer players background and offers limited prediction for injury prevention and performance.

Recently, evidence has emerged suggesting functional diversity is also present along the anterior–posterior axis of the rodent OFC. However, the patterns of anatomical connections that underlie these differences have not been well characterized. Here, we use the retrograde tracer cholera toxin subunit B (CTB) to simultaneously label the projections into the anterior lateral (ALO), posterior lateral (PLO), and posterior ventral (PVO) portions of the rat OFC. Our methodological approach allowed us to simultaneously compare the density and input patterns into these OFC subdivisions. We observed distinct and topographically organized projection patterns into ALO, PLO, and PVO from the mediodorsal and the submedius nuclei of the thalamus. We also observed different levels of connectivity strength into these OFC subdivisions from the amygdala, motor cortex, sensory cortices and medial prefrontal cortical structures, including medial OFC, infralimbic and prelimbic cortices. Interestingly, while labelling in some of these input regions revealed only a gradient in connectivity strength, other regions seem to project almost exclusively to specific OFC subdivisions. Moreover, differences in input patterns between ALO and PLO were as pronounced as those between PLO and PVO. Together, our results support the existence of distinct anatomical circuits within lateral OFC along its anterior–posterior axis.

The purpose of this study was to evaluate the rate of return to play (RTP) in patients who underwent Type V superior labrum anterior–posterior (SLAP) repair compared to patients who underwent isolated Bankart repair in the setting of traumatic anterior shoulder instability. A retrospective review of patients who underwent arthroscopic Bankart repair and SLAP repair by a single surgeon between 2012 and 2017 was performed. Additionally, these were pair-matched in a 1:2 ratio for age, sex, sport and level of pre-operative play, with those undergoing isolated arthroscopic Bankart repair alone as a control group. RTP, level of RTP and the timing of RTP were assessed. The study included a total of 96 patients, with 32 in the study group and 64 in the control group, and a mean follow-up of 59 months. Overall, there was no significant difference in the overall rate of return to play (26/32 (81.3%) vs 56/64 (87.5%), n.s), but there was a significantly higher rate of RTP at the same/higher level in the control group (14/32 (43.6%) vs 43/64 (67.2%), p = 0.0463). There was no significant difference in timing of RTP between the groups (n.s). There was no significant difference in recurrent instability (6/32 (18.8%) vs 5/64 (7.8%), n.s) but there was a significant difference in revision rates (5/32 (15.6%) vs. 2/64 (3.1%), p = 0.0392) between the Type V SLAP repair group and the control group. Following arthroscopic repair, patients with Type V SLAP tears had a similar overall rate of RTP when compared directly to a control group of patients who underwent arthroscopic Bankart repair alone. However, those who underwent Type V SLAP repair reported significantly lower rates of RTP at the same or higher level compared to the control group. III.

In this study, our aim was to evaluate the significance of the change in renal pelvis anterior-posterior diameter (RPAPD) before and after micturition between vesicoureteral reflux (VUR)-positive and -negative patients to whom had voiding cystourethrography (VCUG) was performed.In this study, 69 children, age ranging from 0 to 12 years, were included. Before the VCUG imaging, the RPAPD was measured first with a full bladder and then after urination via ultrasound (US). The differences between in RPAPD measurements were noted and values compared made among VUR-positive and -negative children. Data distribution was inhomogeneous, and the Wilcoxon Sign Rank test was utilized instead of Student t test. There was no statistically significant difference in prevoiding and postvoiding RPAPD in VUR (+) and VUR (-) patients (P = 0.672). There was no statistically significant relation between VUR and the presence of hydronephrosis (P = 0.126). Vesicoureteral reflux is more common in patients with urinary tract infections (UTI) (P = 0.001). There was no statistically significant relationship between prevoiding and postvoiding RPAPD change and VUR diagnosis (P = 0,164).Ultrasound is the modality of choice for urinary system evaluation. Diagnosis of hydronephrosis via US is not sufficient in predicting VUR; however, indirect findings may reveal the diagnosis. A decrease in RPAPD in postvoiding US evaluation may not rule out the VUR diagnosis for this reason further imaging modalities, such as VCUG, should be taken into consideration for the patients with clinical suspicion.

Introduction: Sacral hiatus is the chief location for caudal epidural anesthesia during surgery of perineal region and also for a pain free parturition. Thus, this study was conducted to examine, measure and record the morphometry of sacral hiatus and to analyze it for any anatomical variations with clinical implications. Methods: The cross-sectional study was carried in the Department of Anatomy, Sikkim Manipal Institute of Medical Science, Gangtok, Sikkim. For the study seventy dry human sacral bone with entire sacral hiatus were measured. The shape of the sacral hiatus was observed. Level of the apex, base, length, width and anterior posterior diameter of sacral hiatus was measured with Digital Vernier Caliper. Majority of patients with unilateral hydronephrosis (HN) detected on ultrasound do not require pyeloplasty. The measurement of the cortical transit time (CTT) has been demonstrated by several authors to predict the need for patients who may require pyeloplasty. The study aimed to assess if CTT would have predicted a drop in differential renal function (DRF) in patients with unilateral HN on the affected side and to assess whether CTT would differ on the first renogram between those patients who had a pyeloplasty and those who did not have a pyeloplasty. Sixty-eight patients with at least two renograms with unilateral HN with a normal contralateral kidney were observed retrospectively. The CTT was recorded for each kidney. Renograms were processed three times to measure the DRF. The mean of the three DRF measurements was used for analysis. The mean CTT of the left and right hydronephrotic kidneys was 6.0 min and 6.7 min, respectively. The relationship between CTT and DRF as well as CTT and anterior posterior diameter in the first renogram of those patients who did not have a pyeloplasty was statistically significant (P < 0.05). In the 20 patients who had a pyeloplasty, there was a drop of more than 10% in the DRF of three patients. No significant difference was found in CTT or DRF when comparing the group who had surgery against the group who did not have surgery. The current study was unable to demonstrate in our series of patients that CTT can predict a drop in DRF in those patients who would require pyeloplasty.

Background Historically, anterior-posterior (AP) spread assessments were often used to determine the length that a distal cantilever could be extended off an implant-supported fixed full-arch prosthesis. Types of Studies Reviewed The authors searched the literature for articles that used AP spread to calculate cantilever size to be constructed off implants bearing a fixed implant-supported full-arch rehabilitation. Results The data indicate that the relationship between AP spread and cantilever length is not linear and many influences (such as beam theory, cantilever size differences in the mandible versus maxilla, number and distribution of placed implants, prosthetic materials, and framework design) need to be considered when computing cantilever length with respect to fixed implant-supported prostheses. Practical Implications Recommendations using AP spread assessments to compute cantilever lengths have not been validated by means of prospective scientific evaluations. Therefore, AP spread evaluation is just one of many issues that need to be considered when determining distal cantilever length associated with a fixed full-arch implant-bearing prosthesis.

Voiced speech is generated by the glottal flow interacting with vocal fold vibrations. However, the details of vibrations in the anterior-posterior direction (the so-called zipper-effect) and their correspondence with speech and other glottal signals are not fully understood due to challenges in direct measurements of vocal fold vibrations. In this proof-of-concept study, the potential of four parameters extracted from high-speed videoendoscopy (HSV), electroglottography, and speech signals to indicate the presence of a zipper-type glottal opening is investigated. Comparison with manual labeling of the HSV videos highlighted the importance of multiple parameter-signal pairs in indicating the presence of a zipper-type glottal opening.

Background: Assessing and correcting malalignment is important when treating calcaneus fractures. The Harris axial view is commonly utilized to assess varus deformity but may be inherently inaccurate due to its tangential nature. The anterior-posterior (AP) calcaneal profile view is a novel radiographic view that is easily obtained with demonstrated increased accuracy for assessing calcaneal axial alignment. Methods: Five nonpaired ankle cadaveric specimens were used in this investigation. Oblique osteotomies were created in relation to the long axis, and varus deformities were produced by inserting solid radiolucent wedges into the osteotomies to create models of 10, 20, and 30 degrees of angulation of the calcaneal tuberosity. Specimens were imaged using both the Harris axial view and the AP calcaneal profile view. Results: For cadavers with 10 degrees of actual varus angulation, the mean Harris axial view angle and the AP calcaneal profile view angle were 10.9 ± 4.8 (range, 5.5-16.0) degrees and 13.0 ± 5.5 (range, 7.3-20.9) degrees, respectively. For cadavers with 20 degrees of actual varus angulation, the mean Harris view angle and the AP calcaneal profile view angle were 11.5 ± 2 (range, 8.2-13.6) degrees and 18.1 ± 4.8 (range, 11.7-23.5) degrees, respectively ( P = .005). On pairwise comparison with Bonferroni correction, there was a significant difference between the Harris axial view angle and both the AP calcaneal profile view angle ( P = .012) and actual angulation ( P = .011). For cadavers with 30 degrees of actual varus angulation, the mean Harris axial view angle and the AP calcaneal profile view angle were 18.3 ± 4.3 (range, 13.3-23.6) degrees and 28.3 ± 2.9 (range, 24.4-31.1) degrees, respectively ( P < .001). On pairwise comparison with Bonferroni correction, there was a significant difference between the Harris axial view angle and both the AP calcaneal profile view angle ( P = .001) and actual angulation ( P < .001). There was no significant difference between the AP calcaneal profile view angle and actual angulation ( P > .999). Conclusion: The AP calcaneal profile view is a novel radiographic view that is easily obtained with demonstrated increased accuracy for assessing calcaneal axial alignment. While both views demonstrated similar measurement error for lesser degrees of varus malalignment, the AP calcaneal profile view demonstrated more accurate measurement of increasing heel varus compared with the Harris view. Clinical Relevance: The AP calcaneal profile view could be used in addition to other radiographic views when treating displaced, intra-articular calcaneus fractures to help optimize correction of hindfoot alignment.

Background: Superior labrum anterior-posterior (SLAP) lesion is a common shoulder injury, particularly in overhead athletes. While surgical management has traditionally consisted of SLAP repair, high rates of revision and complications have led to alternative techniques, such as biceps tenodesis (BT). While BT is commonly reserved for older nonoverhead athletes, indications for its use have expanded in recent years. Purpose: To determine functional outcomes and return-to-sport rates among overhead athletes after BT for SLAP tear. Study Design: Systematic review. Methods: A systematic review was performed for any articles published before July 2019. The search phrase “labral tear” was used to capture maximum results, followed by keyword inclusion of “SLAP tear” and “biceps tenodesis.” Inclusion criteria included outcome studies of BT for isolated SLAP tear in athletes participating in any overhead sports, not limited to throwing alone. Abstracts and manuscripts were independently reviewed to determine eligibility. When clearly delineated, outcome variables from multiple studies were combined. Results: After full review, 8 articles met inclusion criteria (99 athletes; mean age, 19.8-47 years), with baseball and softball players the most common among them (n = 62). Type II SLAP tear was the most common diagnosis, and 0% to 44% of athletes had a failed previous SLAP repair before undergoing BT. Only 1 study included patients with concomitant rotator cuff repair. Open subpectoral BT was most commonly used, and complication rates ranged from 0% to 14%, with wound erythema, traumatic biceps tendon rupture, brachial plexus neurapraxia, and adhesive capsulitis being reported. Combined reported postoperative functional scores were as follows: American Shoulder and Elbow Surgeons, 81.7 to 97; 12-Item Short Form Health Survey physical, 50 to 54; visual analog scale for pain, 0.8-1.5; Kerlan Jobe Orthopaedic Clinic, 66 to 79; and satisfaction, 80% to 87%. The overall return-to-sports rate for overhead athletes was 70% (60 of 86). For studies that clearly delineated outcomes based on level of play/athlete, the combined return-to-sports rate was 69% (11 of 16) for recreational overhead athletes, 80% (4 of 5) for competitive/collegiate athletes, and 60% (18 of 30) for professionals. Conclusion: BT in the overhead athlete offers encouraging functional outcomes and return-to-sports rates, particularly in the recreational athlete. It can be successfully performed as an index operation rather than SLAP repair, as well as in a younger patient population. Careful consideration should be given to elite overhead athletes, particularly pitchers, who tend to experience poorer outcomes.

Background Gait perturbations, occurring in any direction in daily life, may result in a fall. In fall prevention, gait perturbation training is a promising approach. Treadmill perturbations in anterior-posterior direction can easily be applied by accelerations or decelerations of the belt, but it is unknown whether training effects transfer to reactive recovery in medio-lateral direction. We aimed to evaluate the transfer and retention effects of gait training with treadmill perturbations in anterior-posterior direction to medio-lateral reactive recovery. Methods 30 community dwelling older adults (>65 years) participated in this study. They were randomly assigned to a treadmill training session either with 16 anterior-posterior perturbations or with treadmill walking. The assessments contained a walking trial with 4 anterior-posterior and 4 medio-lateral perturbations. Deviations in trunk velocity from unperturbed walking were summed over the first three strides after perturbation as a measure of recovery. Findings An exposure to gait perturbations during the baseline assessment led to significant improvement of recovery responses. For anterior-posterior perturbations, both groups showed better recovery immediately and 1-week post-intervention, and no group x time interaction was found.. For medio-lateral perturbations, both groups showed better recovery immediately and 1-week post-intervention, and again no group × time interaction. Interpretation Baseline assessment with perturbations in anterior-posterior and medio-lateral directions caused significant improvements that were retained. Short-term training can be effective in dynamic stabilization of one's trunk, but our findings do not exclude that multi-directional perturbations may be needed.

The aim of this study was to determine the asymmetry of cervical multifidus (MF) and longus colli (LC) muscles in patients with unilateral chronic radicular neck pain (CRNP) and healthy subjects using ultrasonography.Ninety five individuals (50 patients with unilateral CRNP and 45 healthy subjects) participated in this study. The size of the cervical MF and LC muscles; anterior posterior dimension (APD) and lateral dimension (LD); were bilaterally measured in a relaxed state using ultrasonography.Patients with CRNP showed greater asymmetry in the cervical MF and LC muscles size than the healthy subjects. The mean percentages differences between sides for MF and LC muscles size was higher in patients than that of healthy subjects (for both APD and LD; P0.05). The ratio of smaller to larger dimensions of the cervical MF and LC muscles for patients were significantly less than that of healthy controls (for both APD and LD; P0.05).The results indicated that cervical MF and LC muscles showed higher asymmetry between sides and lesser ratio of smaller size to larger size in patients with CRNP as compared with healthy subjects. Current results confirmed the presence of MF and LC muscles atrophy in subjects with CRNP.

Hypothesis: The purpose was to investigate joint stability and range of motion after a Bankart repair without superior labral anterior-posterior (SLAP) repair (termed “Bankart repair”) and after combined Bankart and SLAP repairs (termed “combined repair”). Methods: Eight fresh-frozen shoulders were used. Combined Bankart and SLAP lesions were created (10- to 6-o'clock positions). The labrum and capsule were repaired at the 2-o'clock, 3:30 clock-face, and 5-o'clock positions in the Bankart repair group and at the 11-o'clock, 1-o'clock, 2-o'clock, 3:30 clock-face, and 5-o'clock positions in the combined repair group. The internal- and external-rotation ranges of motion were determined with the arm positioned at 0° and 60° of glenohumeral abduction. The rotation angle was defined when a constant torque of 200 N-mm was applied. Joint stability was measured with a custom stability-testing device. The peak translational force in the anterior-posterior direction was measured with the arm at the end range of external rotation. Results: External rotation angles were greater at 0° and 60° of abduction in the Bankart repair group than in the combined repair group (0° of abduction, P < .01; 60° of abduction, P < .05). The internal rotation angle was greater at 60° of abduction in the Bankart repair group than in the combined repair group (P < .01). The stability between the 2 groups was not significantly different (P = .60). Conclusion: In patients with combined Bankart and SLAP lesions and the need for a wide range of motion, a Bankart repair alone may provide a greater range of motion without compromising the joint stability at the end range compared with a combined repair.

Background: The relationship between the dimensions of the cranial base and skeletal anterioposterior problem has been controversial for years. The aim of this study was to determine the relationship between the anterioposterior cephalometric indicators and the cranial base cephalometric indicators in an Iranian population. Materials and Methods: In this historical cohort cephalograms of 100 skeletal Class I patients, 101 skeletal Class II patients, and 98 skeletal Class III patients were selected. The cephalograms were traced manually and the indicators were measured. Finally, data were analyzed by SPSS software using the Mann–Whitney test and Pearson’s correlation test. The significance level was set at 0.05. In cases that the correlation coefficient (r) was 0.6 or higher, linear regression was used. Results: The dimensions of the cranial base are significantly larger in men than that in women. Anterior cranial base length (SN) showed statistically significant difference between Class I and Class II groups (P < 0.05). BaSN, ArSN, and SN‑FH showed statistically significant differences between Class II and Class III groups (P < 0.05). Conclusion: Smaller cranial base angle in the skeletal Class III malocclusion compared to skeletal Class II malocclusion has been demonstrated in this study. A significant correlation between the cranial base angle, the cranial base dimension, and the effective length of the maxilla was observed, and the smaller cranial base angle in Class III malocclusion was also confirmed. These findings indicate that the cranial base can affect the development of maxilla and mid‑face.

BACKGROUND Spinal and pelvic injuries during an unexpected perturbation are closely related to spinal stability, which is known to be controlled by abdominal stabilization maneuvers. This study aimed to evaluate the effects of unexpected perturbations on trunk stability and abdominal stabilization strategies in 42 sedentary adults while sitting. MATERIAL AND METHODS Abdominal stabilization strategies consisted of bracing and hollowing maneuvers. Abdominal bracing maneuvers (ABM) were focused on the abdominal wall muscles [inferior oblique (IO), exterior oblique (EO)], and abdominal hollowing maneuvers (AHM) were focused on deep muscle (TrA) activation. The subjects were instructed in abdominal stabilization maneuvers. Afterward, subjects were seated in a chair that could be moved forward or backward suddenly with the support surface. RESULTS Angular displacements of the upper thorax, lower thorax, and lumbopelvic during unexpected perturbation, with different abdominal stabilization maneuvers, were measured. During forward perturbation (d=0.71, F=10.324, P=0.001) and backward perturbation in high speed (d=0.62, F=9.265, P=0.011), there were significant differences in angular displacements of the upper thorax between hollowing and bracing maneuvers. Additionally, significant differences were found in the lumbopelvic angular displacement between the hollowing and bracing maneuvers (d=0.62, F=4.071, P=0.044). CONCLUSIONS Our findings indicate that the ABM is a better stabilizing technique for the upper thorax, and the AHM is a better stabilizing technique for the lumbopelvic region during unexpected perturbations at high speed in the seated position.

A major issue of modern Neuroscience is to understand how the activity of cells and populations represents multiple spatial and motor features during goal-directed movements. The direction and distance (depth) of arm movements often appear to be controlled independently at the behavioral level, but it is unknown whether they share neural resources or not. We addressed this issue in parietal cortex that is known to integrate multiple signals. Using information theory, singular value decomposition and dimensionality reduction methods we compared direction and depth effect magnitude and measured their convergence across three parietal areas during an instructed delay 3D arm movement task. All methods showed a stronger direction effect during early movement preparation, whereas depth signals prevailed during movement execution. Going from anterior to posterior sectors, we found an increase in the number of cells that processed both signals and a stronger influence of depth information. These findings suggest a serial direction and depth processing that supports behavioral evidence. In addition, they reveal a gradient of joint versus independent control of these features in parietal cortex that could be framed in the context of its functional heterogeneity and its role in sensorimotor transformations.

Degenerative mitral regurgitation repair using a measured algorithm could increase the precision and reproducibility of repair outcomes.Direct and echocardiographic measurements guide the repair to achieve a coaptation length of 5 to 10 mm and minimize the risk of systolic anterior motion. Leaflet reconstruction restored the normal 2 to 1 ratio of anterior to posterior leaflet length without residual prolapse or restriction. The choice of ring size was based on anterior leaflet length, the distance from the leaflet coaptation point to the septum, and the anterior-posterior ring dimension. Freedom from reoperation and mitral regurgitation recurrence were based on multistate models.One thousand fifty-one patients had mitral surgery and 1026 (97.6%) were repaired. A2 length was 27.2 ± 4.5 mm; and the reconstructed posterior leaflet was 13.9 ± 2.3 mm. Median ring size was 34 mm and strongly correlated to A2 length (R = 0.76; P .001). The coaptation length at P2 after repair was 6.4 ± 1.7 mm and 87% of measurements were between 5 and 10 mm. Results at predischarge and 10 years, respectively, included mild regurgitation (7.5% and 26.1%), moderate (0.7% and 15.6%), moderate to severe (0% and 1.4%), and severe (0% and 0%), with mean mitral gradient values 3.5 ± 1.5 and 2.9 ± 1.2 mm Hg, respectively. Systolic anterior motion at discharge and last follow-up were 0.2% and 1.1%, respectively. Ten-year freedom from mitral valve reoperation was 99.7%.A simple, reproducible, measured algorithm for degenerative mitral valve repair provides excellent early and late results and is a useful adjunct to established surgical techniques.

Scoliosis is a common medical condition, which occurs most often during the growth spurt just before puberty. Untreated Scoliosis may cause long-term sequelae. Therefore, accurate automated quantitative estimation of spinal curvature is an important task for the clinical evaluation and treatment planning of Scoliosis. A couple of attempts have been made for automated Cobb angle estimation on single-view x-rays. It is very challenging to achieve a highly accurate automated estimation of Cobb angles because it is difficult to utilize x-rays efficiently. With the idea of developing methods for accurate automated spinal curvature estimation, AASCE2019 challenge provides spinal anterior-posterior x-ray images with manual labels for training and testing the participating methods. We review eight top-ranked methods from 12 teams. Experimental results show that overall the best performing method achieved a symmetric mean absolute percentage (SMAPE) of 21.71%. Limitations and possible future directions are also described in the paper. We hope the dataset in AASCE2019 and this paper could provide insights into quantitative measurement of the spine.

To evaluate age and fall-risk related changes in balance ability from measures of bipedal quiet stance, this study aims to investigate the characteristics of 'return to central' - Centre of Pressure (COP) trajectories. COP trajectories were extracted from 60-second COP recordings in bipedal stance. In anterior posterior direction, age was associated with a greater number of detected trajectories, increased velocity and more stringent control. No differences related to fall risk were established or to age or fall risk in mediolateral direction. The characteristics 'return to central' COP trajectories provided insight into the working of the postural control system and can be further developed for application if testing balance under challenging conditions is too risky.

Background We retrospectively analyzed the initial results of laparoscopic pyeloplasty, among pediatric patients undergoing the procedure by transperitoneal access. Methods We retrospectively reviewed the medical records of patients who were operated with transperitoneal laparoscopy-assisted pyeloplasty at our institution between 2015 and 2020. Results The mean age of the 51 patients (M/F = 36/15) was 59 ± 49.8 months. The mean preoperative renal pelvis anterior–posterior diameter was 32.9 ± 13.4 mm, and the relative renal function was 42 ± 12.1% on the operation side. Forty-three (84.3%) patients had no excretion on MAG3 examination preop. The number of patients with no excretion decreased to 10 (19.7%) after surgery. One of them underwent a successful endopyelotomy; eight of them underwent a successful laparoscopic re-pyeloplasty. There was nothing to do in only one patient whose DRF decreased to 11%. The mean anterior posterior diameter decreased significantly to 16.7 ± 11.2 mm after surgery (p Conclusion Preoperative increased renal pelvis anterior–posterior diameter detected in our study was found to be a risk factor in the failure of transperitoneal laparoscopy-assisted pyeloplasty procedure.

PADUA, RENAL, ABC and MAP nephrometry scores were determined from preoperative axial images. Unadjusted and adjusted associations between overall scores, individual components, surgical approach, complications and oncological outcomes were determined using univariate and multivariate logistic regressions.Results: A total of 189 partial nephrectomies were performed in 181 patients, via an open or a laparoscopic approach. Among scoring systems, only the MAP classification, which assesses adherent perinephric fat, was associated with severe surgical complications as well as with operative time (p< 0.05). Among all components of the PADUA and RENAL scores, only proximity of the tumor to the collecting system was associated with overall surgical complication rates, while the diameter of the tumor influenced the operative time (p< 0.05). The ABC score was not relevant. Male gender, antiplatelet therapy, and a laparoscopic approach were associated with higher overall surgical complication rates (p< 0.05). The number of oncologic recurrences during follow-up was too low to run statistical analyses.Conclusion: Nephrometry scores could be simplified to predict surgical complications after partial nephrectomy. Aim To compare the prediction error between Barrett Toric calculator and the new online AcrySof Toric calculator which incorporated Barrett astigmatism algorithm in Chinese cataract eyes with normal axial length and anterior chamber depth (ACD). Methods Prospective case-control study. All the cases had axial length (21-26 mm) with ACD no less than 2.4 mm. Keratometric values were measured by LenSTAR 900. The Barrett Toric calculator was used in group 1. In group 2, SRK-T formula was used to determine the spherical power of the Toric lens, and subsequent calculation of the cylinder type was performed using the new online Alcon Toric calculator. At 1 and 3mo after surgery, a comprehensive subjective optometry was performed. The predicted residual astigmatism calculated by the two calculators was compared with that obtained by postoperative refraction, and the difference was defined as the astigmatism correction error [error of refractive astigmatism (ERA)]. The error magnitude (EM) refers to the algebraic deviation of ERA, and the error vector (EV) indicates the vector deviation of ERA. The influence of the two calculation methods on the correction accuracy of toric IOL was quantitatively analyzed. Results The |EM| obtained at 1mo after surgery were 0.21±0.12 D, 0.22±0.18 D in group 1 and group 2 respectively, and correspondingly turned to be 0.19±0.13 D, 0.20±0.19 D at 3mo after surgery, with no statistical difference (P=0.633, P=0.877). The vector analysis showed that |EV| values in two groups at 1mo after surgery were 0.29±0.14@105 (D@angle) and 0.35±0.20@113 (D@angle), respectively, whereas |EV| values 3mo after surgery were 0.27±0.16@86 (D@angle) and 0.32±0.23@102 (D@angle), respectively. The differences between the groups were not statistically significant (P=0.119, P=0.261). Conclusion The clinical effect of Barrett Toric calculator has a much more accurate tendency than that of new online AcrySof Toric calculator, but is not evident in cases with normal axial length and normal anterior posterior ratio.

Given the current increase in the incidence of coronary artery disease in younger women as well as the high lifetime risk of developing an x-ray-induced malignancy in this population, we aimed at assessing chest radiation in 206 women ≤55 years old undergoing coronary calcium scoring (CACS) by using a Monte Carlo simulation tool. Our data indicate that the simulated radiation dose of the female breast during CACS depends substantially on the starting position of the x-ray tube, with an almost 2 times excess of breast radiation exposure being measured during anterior-posterior tube positioning. Thus, an additional technical feature taking into account the position of the x-ray tube when acquisition is triggered might be an important tool to reduce radiation exposure of the female breast during CACS.

Background: The key determinants when planning surgery in patients with CSM are the direction of compression, number of levels, sagittal alignment and instability. However there is no literature that compares the clinical and functional outcomes following different approaches in patients selected for surgery. Aims: Prospective non-randomized study that aims to compare the clinical and functional outcomes following surgical approaches with the goal of planning the optimal surgical strategy. Material and Methods: 75 patients- 61 males and 14 females (mean age: 64.2 years) with CSM underwent spinal decompression using an anterior (30), posterior (35) or combined approach (10).The surgical approach was selected based on the above mentioned key determinants. Functional disability was measured using the modified Japanese Orthopaedic Association score for myelopathy. Based on this the recovery rate was calculated. The mean followup duration was 21 months (range 6-72 months). Results: The preoperative mJOA score was 11.01 and the functional disability was graded as mild in 15, moderate in 50 and severe in 10. There are contrasting views on the role of vision in modifying postural organization (information-driven and postural facilitation) and limited direct tests of the underlying postural mechanisms. Here, we examined whether the distinction between the two views is appropriate given that both are interrelated parts of task constraints modulating postural coordination and control. The study investigated whether changes in the organization of the postural system are a function of the visual precision demands of a task and, in addition, whether such organization could be described as reflecting an intermittent controller. Sixteen participants were instructed to maintain quiet postural stance while fixating a point at different viewing distances (25, 50, 135, 220, 305 cm) or standing with eyes closed. The 25-cm condition showed the lowest standard deviation of the center of pressure (COP) and the highest correlation dimension (CD) in the anterior posterior direction. Analyses revealed that, contrary to the intermittent controller hypothesis, adaptations in the continuous COP and center of mass (COM) coupling characterized the observed changes in CD. The findings show that the natural act of looking to the same feature in the environment as a function of visual viewing distance can lead to quantitative and qualitative changes in the dynamics of posture. This is consistent with the view that postural facilitation and information availability are integrated in the perceptual-motor dynamics.

Context: Surgical intervention is imperative when spinal tuberculosis (TB) is accompanied by severe spinal damage or kyphotic deformity. As one-stage anterior-only or posterior-only surgery for thoracic and lumbar spinal TB has many disadvantages, combined anterior-posterior surgery was proposed to be a more effective strategy. Objective: To examine the clinical outcomes of one-stage combined anterior-posterior surgery for patients with spinal TB. Design: Retrospective investigation design. Setting: All patients were enrolled at the Hangzhou Red Cross Hospital between August 2002 and October 2014. Participants: Sixty-seven patients with thoracic and lumbar spinal TB were studied. Interventions: All patients were treated with one-stage surgery using a combined anterior-posterior approach. Outcome measures: The patients were evaluated preoperatively and postoperatively by measuring their neurological function using the visual analogue scale (VAS) and the Frankel grades, and spinal deformity using the Cobb angle and radiological examinations. All patients were followed up for at least 11 months and up to 96 months. Results: There was a significant postoperative improvement in neurological outcomes, according to VAS scores and Frankel grades. Kyphotic angles were corrected significantly and were maintained during the final follow-up. Bone fusion was achieved within 4–7 months. Conclusion: One-stage surgical treatment via a combined anterior-posterior approach is an effective and feasible method for treating spinal TB.

A Type II SLAP (superior labrum anterior posterior) lesion is a tear of the superior glenoid labrum with involvement of the long head of the biceps tendon insertion. In patients that do not improve with conservative treatment, there is a great deal of variability in the surgical management of these injuries that includes arthroscopic SLAP repair, arthroscopic SLAP repair with biceps tenodesis, biceps tenodesis alone and biceps tenotomy. Each surgical technique has specific effects on a patient's postoperative course and functional recovery. Rehabilitation strategies may be best formulated on an individual basis with an open line of communication between the operating surgeon and the physical therapist. Despite an increased incidence in treatment, there is currently no consensus on the optimal surgical procedure or treatment algorithm for Type II SLAP injuries. However, in middle-aged or older patients (>35) with Type II SLAP tears, either arthroscopic suprapectoral or mini-open subpectoral biceps tenodesis is recommended due to the higher failure rates observed with arthroscopic SLAP repair in this patient group. Although more patients present with a 'Popeye' sign after biceps tenotomy, long-term functional outcome is similar between biceps tenodesis compared to tenotomy. However, more patients will experience biceps fatigue or cramping after the tenotomy procedure. Biceps tenodesis is preferred in younger, more active patients, while tenotomy is preferred in the middle-aged or older and lower demand patients. The aim of this paper is to provide a brief description of the different surgical techniques employed to address Type II SLAP lesions (arthroscopic repair, biceps tenodesis, and biceps tenotomy) and provide a review of available literature regarding outcomes and prognostic factors associated with each technique.

Purpose: Direct magnetic resonance arthrography (MRA) offers increased diagnostic accuracy compared to conventional magnetic resonance imaging (MRI) in the detection of superior labrum anterior-posterior (SLAP) lesions. The aim of the present study was to present the technique of magnetic resonance-guided direct shoulder arthrography (MDSA), to evaluate the diagnostic value of this novel MRA procedure to detect SLAP lesions in comparison to the currently practiced MRI, and to correlate the radiological findings to the respective arthroscopic findings. Material and methods: Fifty-six patients with clinical signs of a SLAP lesion underwent both MRI examination and MDSA prior to arthroscopic surgery. The MRI of both interventions were compared with the arthroscopic findings. Statistical analysis was performed using the McNemar test. Results: Sensitivity, specificity, and accuracy for detecting SLAP lesions were 23%, 88%, and 54% on MRI and 80%, 81%, and 80% on MDSA, respectively. Sensitivity (p < 0.001) and accuracy (p = 0.001) in detection of SLAP lesions were significantly higher by MDSA whereas accuracy showed no significant differences (p = 0.625). Conclusions: The MDSA can be performed in an open 1.0-T MRI scanner with a high level of technical success and a reasonable methodical effort. The modification of MRA provides the requirements as a practicable routine shoulder magnetic resonance examination including arthrography to detect SLAP lesions. The diagnostic value is significantly better than MRI examinations without included arthrography, which currently predominates the clinical practice to investigate shoulder pathology.

Purpose: Exposure of the articular surface is the key to the successful treatment of intra-articular fractures of distal humerus. Anterior, posterior olecranon osteotomy as well as medial and lateral approaches are the four main approaches to the elbow. The aim of this study was to compare the exposure of distal articular surfaces of these surgical approaches. Methods: Twelve cadavers were used in this study. Each approach was performed on six elbows according to previously published procedures. After completion of each approach, the exposed articular surfaces were marked by inserting 0.5 mm K-wires along the margins. The elbow was then disarticulated and the exposed articular surfaces were painted. The distal humeral articular surfaces were then closely wrapped using a piece of fibre-glass screen net with meshes. The exposed articular surfaces and the total articular surfaces were calculated by counting the number of meshes, respectively. Results: The average percentages of the exposed articular surfaces for the anterior, posterior olecranon osteotomy, medial and lateral approaches were 45.7% ± 2.0%, 53.9% ± 7.1%, 20.6% ± 4.9% and 28.5% ± 6.3%, respectively. Conclusion: The anterior and posterior approaches provide greater exposures of distal humeral articular surface than the medial and lateral ones in the treatment of distal humeral fractures. Purpose: To evaluate the in vivo knee laxity in the presence of a partial medial meniscectomy before and after a single-bundle ACL reconstruction with a lateral plasty (SBLP) and to compare it with the knee laxity after a single-bundle ACL reconstruction (SB). Methods: One-hundred and one patients with ACL tear were enrolled in the study and grouped according to the surgical technique and the meniscus treatment: regarding the SBLP technique (n = 55), 31 patients underwent isolated ACL reconstruction (“SBLP Isolated ACL Group”), while 24 patients underwent combined ACL reconstruction and partial medial meniscectomy (“SBLP ACL + MM Group”); regarding the SB technique (n = 46), 33 patients underwent isolated ACL reconstruction (“SB Isolated ACL Group”), while 13 patients underwent combined ACL reconstruction and partial medial meniscectomy (“SB ACL + MM Group”). Anterior–posterior clinical laxity at 30° (AP30) and 90° (AP90) of knee flexion was quantified before and after surgery through a surgical navigation system dedicated to kinematic assessment. Results: In the ACL-deficient status, the antero-posterior laxity was significantly higher in the presence of a combined MM in both the AP30 and the AP90, with no differences between the two surgical techniques. After the ACL reconstruction, both AP30 and AP90 translations decreased significantly (p < 0.0001) compared to the ACL-deficient status. No differences were found for AP30 and AP90 between SBLP Isolated ACL and SBLP + MM groups, while a significantly higher AP90 translation was found for the SB + MM group compared to the SB Isolated ACL group. Moreover, the AP90 translation in the SB ACL + MM group was significantly higher than the one of the other three groups, i.e., SBLP ACL + MM, SB, and SBLP Isolated ACL group. Conclusion: The ACL reconstruction with lateral plasty reduced the AP knee laxity caused by the medial meniscectomy in the context of an ACL surgery. Level of evidence: Level II.

Background Pelvic anterior–posterior compression (APC) fracture is typically associated with pelvic ligament damage. We used magnetic resonance imaging (MRI) to evaluate ligamentous injury associated with pelvic APC fracture. Methods Thirty healthy adults and 26 patients with pelvic APC fractures were enrolled in this study. All healthy adults underwent a series of MRI scans. Pelvic ligament visualization was scored [0 (poor) to 3 (excellent)] to identify the best scanning method. Then, MRI examination of patients with pelvic APC fracture was performed using this method. Results For healthy adults, oblique axial and axial scans provided the best visualization of the anterior sacroiliac ligament [ASL; good and excellent scores in 100% (30/30) and 96.7% (29/30) of cases, respectively; both P 30 mm, with no SBL or SPL injury; two patients had about 25 mm separation with SPL injury. Conclusion Healthy and injured pelvic ligaments can be evaluated using MRI; oblique axial and axial scanning are best for ASL visualization, and oblique sagittal scanning are best for SPL and SBL visualization. Pubic symphysis separation > 25 mm is not necessarily associated with SPL or SBL injury.
