Chirurgische Weiterbildungsfacher verzeichnen trotz nach wie vor bestehender hoher Prestigeattribution einen Attraktivitatsverlust im „Berufsmonitoring Medizinstudierende“. Als relevante Entscheidungsfaktoren werden vielfaltige Grunde genannt. Jedoch laufen Entscheidungsprozesse wie die Berufswahl uberwiegend unbewusst ab und werden durch vielfaltige Mechanismen wie Heuristiken, Stereotype und Archetypen beeinflusst. Dies gilt auch fur Entscheidungen Medizinstudierender fur oder gegen ein chirurgisches Fach. Anhand zweier Beispiele im Interviewstil werden haufige psychische Mechanismen und die dadurch entstehenden Verzerrungen aufgezeigt und kommentiert. Bei der Entscheidung fur oder gegen die Gefaschirurgie spielen neben der Vereinbarkeit von Familie und Beruf das Erleben von personlicher Wertschatzung und Respekt eine grose Rolle. Diese Faktoren werden nach individuellen Erlebnissen, insbesondere im Studium (Praktika, Famulaturen, Praktisches Jahr), beurteilt und konnen implizite Verzerrungen, sog. Vorurteile, bestatigen oder korrigieren. Fehlen eigene Erfahrungen, erfolgt eine unbewusste Beurteilung aufgrund verfugbarer Informationen, die oft aus Archetypen, Faustregeln und Berichten bestehen. Daher ist eine gute Sichtbarkeit und Prasenz der Gefaschirurgie im Studium entscheidend fur die Nachwuchsakquise. Hierarchische Strukturen werden als nicht erstrebenswert und zeitgemas erfahren, zudem vermitteln als unerreichbar und unnahbar erlebte Vorgesetzte unerreichbare Perspektiven. Trotz eines hohen Anteils weiblicher Studierender fehlen noch weibliche Rollenvorbilder in Fuhrungspositionen, um Berufsperspektiven zu schaffen. Personliche Erfahrungen haben einen erheblichen Einfluss auf die Berufswahl. Die genannten psychologischen Mechanismen sollten bei allen Masnahmen der Nachwuchsgewinnung von allen Akteuren berucksichtigt werden.

The outstanding early symptom of radiation injury of the intestine is diarrhea, mild to severe in degree. Later manifestations are pain, demonstrable ulceration and stricture formation with partial or complete obstruction. The early lesions, located usually on the anterior wall of the rectum and rectosigmoid, are characterized by an edematous friable mucous membrane. Later ulceration, with a grayish white slough, occurs at the level of the cervix. In time perirectal fibrosis resembling a "frozen" pelvis may develop and result in obstruction. In the severe injuries with stenosis, hemorrhage or persistent severe pain, temporary or permanent colostomy may be required with or without resection of the bowel. Intractable pain may be treated by colostomy and resection of the afferent nerve supply to the rectum. The ideal therapy is, of course, prophylactic, the avoidance of radiation injury. Whether or not it is possible by attention to the details of technic to administer effective carcinocidal doses of irradiation without occasional severe injury to adjacent normal tissue such as the rectum seems questionable at present.

Roentgen therapy has its established place as a valuable palliative procedure in the treatment of human leukemias, lymphomas, and allied diseases. Its general and local effects in these disorders have been the subject of numerous reports by expert radiologists and hematologists. In the last decade radioactive isotopes and recently the nitrogen mustards have been added to the therapeutic arsenal against these diseases. The purpose of this presentation is to furnish an introduction to the papers2 on treatment with these new methods rather than to contribute anything new. As a basis for comparison with the results, complications, ad vantages, and limitations of the therapeutic use of these new compounds, we have reviewed 337 cases of blood dyscrasias treated by external irradiation in Billings Hospital (University of Chicago). Hodgkin's disease is not exclusively a disease of the lymphatic system. Involvement of every organ system including the skeleton has been reported. The present discussion of Hodgkin's disease in bone is based on a review of the literature and a study of 94 previously unreported cases from the Veterans Administration Research Hospital, Chicago, and the Evanston Hospital (Evanston, Ill.) of which 11 showed osseous involvement. Askanazy (2) described the pathologic aspects of osseous involvement in 1920, some twenty years after the clear definition of Hodgkin's disease as an entity by Reed and Sternberg. Grossman and Weis (6) first recorded the radiographic features of the bony lesions in 1922. Incidence The reported incidence of Hodgkin's disease in bone varies widely with the diagnostic criteria.
