Objective: To explain the rationale and protocol of the methods and analyses to be used in the LIVER-PAM randomized clinical trial, which seeks to understand whether a higher mean arterial pressure is capable of reducing the incidence of renal dysfunction postoperatively after liver transplantation. Methods: LIVER-PAM is an open-label, randomized, controlled, singlecenter clinical trial. Patients randomized to the intervention group will have a mean arterial pressure of 85 - 90mmHg in the initial 24 hours of postoperative management, while patients in the control group will have a mean arterial pressure of 65 - 70mmHg in the same period. A sample of 174 patients will be required to demonstrate a 20% reduction in the absolute incidence of renal dysfunction, with a power of 80% and an alpha of 0.05. Conclusion: If a 20% reduction in the absolute incidence of renal dysfunction in the postoperative period of liver transplantation is achieved with higher target mean arterial pressure in the first 24 hours, this would represent an inexpensive and simple therapy for improving current outcomes in the management of liver transplant patients. ClinicalTrials.gov Registry:NCT05068713 RESUMO Objetivo: Explicitar o racional e o protocolo de métodos e análises a serem utilizadas no ensaio clínico randomizado LIVER-PAM, que busca entender se um nível mais alto de pressão arterial média é capaz de reduzir a incidência de disfunção renal no pós-operatório de transplante hepático. Métodos: O LIVER-PAM é um estudo clínico randomizado, controlado, unicêntrico e aberto. Pacientes randomizados para o grupo intervenção terão como alvo de pressão arterial média 85 - 90mmHg nas 24 horas iniciais do manejo pós-operatório, enquanto pacientes do grupo controle terão como alvo de pressão arterial média 65 - 70mmHg no mesmo período. Uma amostra de 174 pacientes será necessária para demonstrar redução de 20% na incidência absoluta de disfunção renal, com poder de 80% e alfa de 0,05. Conclusão: Se a redução de 20% da incidência absoluta de disfunção renal no pós-operatório de transplante hepático for obtida com alvos maiores de pressão arterial média nas primeiras 24 horas, o manejo do paciente nesse cenário encontraria uma terapia barata e simples para a melhoria dos desfechos atuais. This study aimed to report the first case of a patient with hepatosplenic schistosomiasis mansoni, refractory ascites and portal vein thrombosis treated with a transjugular intrahepatic portosystemic shunt (TIPS), at the Instituto de Radiologia, Hospital das Clinicas, Faculdade de Medicina, Universidade de Sao Paulo, Brazil. After the procedure, the patient recovered favorably and progressed with portal pressure reduction and no deterioration of the liver function. Endovascular shunt modification is a conservative medical approach that often helps in reducing symptoms significantly, making it a less invasive and a safer alternative to liver transplantation for the treatment of schistosomiasis with portal hypertension.

We read with interest Massoumi et al.'s article(1) regarding 5 patients who developed coronavirus disease 2019 (COVID-19) in the early postoperative period of liver transplantation (LT). None of them exhibited severe disease and they all fully recovered, suggesting that COVID-19 in the early postoperative period of LT might not be associated with unfavorable outcomes. Here, we share our experience with 7 cases, which was not so promising.

International audience; Background: Enhanced Recovery After Surgery (ERAS) is a multimodal, evidence-based, program of care developed to minimize the response to surgical stress, associated with reduced perioperative morbidity and hospital stay. This study presents the specific ERAS Society recommendations for liver transplantation (LT) based on the best available evidence and on expert consensus.Methods: PubMed and ClinicalTrials.gov were searched in April 2019 for published and ongoing RCTs on LT in the last 15 years. Studies were selected by 5 independent reviewers, and were eligible if focusing on each validated ERAS items in the area of adult LT. An e-Delphi method was used with an extended interdisciplinary panel of experts to validate the final recommendations.Results: Forty-three articles were included in the systematic review. Consensus was reached among experts after the second round. Patients should be screened for malnutrition and treated whenever possible. Prophylactic nasogastric intubation and prophylactic abdominal drainage may be omitted, and early extubation should be considered. Early oral intake, mobilization and multimodal-balanced analgesia are recommended.Conclusions: The current ERAS recommendations were elaborated based on the best available evidence and endorsed by the e-Delphi method. Purpose: To evaluate that Connexin (Cx43) plays a role in lesions after hepatic ischemia/reperfusion (IR) injury. Methods: We use Cx43 deficient model (heterozygotes mice) and compared to a wild group. The groups underwent 1 hour ischemia and 24 hours reperfusion. The heterozygote genotype was confirmed by PCR. There is a worldwide problem of waiting time and mortality rate associated with remaining on the waiting list for a liver transplant. However, some situations have been encouraging in terms of determining appropriate recipients and expanding the donor criteria. We herein report a case of useful liver donor with sickle cell anemia for liver transplantation. Here we described a case of liver transplantation from a donor with sickle cell anemia to a recipient with hepatocellular carcinoma who was deemed to be at risk of tumor growth and at risk of being dropped from the waiting list. The literature reveals the importance of using safe donors, and we describe the benefits of using a safe, deceased liver donor with sickle cell anemia who was an adequate option for liver transplantation.

BACKGROUND: Liver transplantation is performed at large transplant centers worldwide as a therapeutic intervention for patients with end-stage liver diseases. AIM: To analyze the outcomes and incidence of liver transplantation performed at the University of São Paulo and to compare those with the State of São Paulo before and after adoption of the Model for End-Stage Liver Disease (MELD) score. METHOD: Evaluation of the number of liver transplantations before and after adoption of the MELD score. Mean values and standard deviations were used to analyze normally distributed variables. The incidence results were compared with those of the State of São Paulo. RESULTS: There was a high prevalence of male patients, with a predominance of middle-aged. The main indication for liver transplantation was hepatitis C cirrhosis. The mean and median survival rates and overall survival over ten and five years were similar between the groups (p>0.05). The MELD score increased over the course of the study period for patients who underwent liver transplantation (p>0.05). There were an increased number of liver transplants after adoption of the MELD score at this institution and in the State of São Paulo (p0,05). INTRODUÇÃO: Variações na anatomia da artéria hepática são comuns, com incidência de 20-50%. No transplante hepático, reconstruções durante a operção de bandeja são frequentemente necessárias para proporcionar anastomoses arteriais adequadas. O uso de "patch" é frequente, visando reduzir a incidência de complicações. Entretanto, quando está presente a variação da artéria hepática direita, ramo da artéria mesentérica superior, a reconstrução ocasionalmente produz torções e problemas de fluxo. MÉTODOS: Descreve-se uma técnica cirúrgica alternativa para reconstrução da variação da artéria hepática direita usando um "patch de Carrel" da artéria mesentérica superior. O "patch" é anastomosado no coto da artéria esplênica permitindo orientação vertical e bom fluxo sanguíneo. RESULTADOS: Entre 120 transplantes hepáticos, quatro casos consecutivos de variação da artéria hepática direita foram reconstruídas utilizando essa técnica. Todos eles apresentaram patência e bom fluxo no pós-operatório. CONCLUSÃO: A técnica proposta mostra-se interessante método alternativo para reconstrução da variação da artéria hepática direita no transplante hepático. INTRODUCTION: Variations on the anatomy of the hepatic artery are common, with incidence of 20-50%. In liver transplantation, back-table reconstruction is often necessary for an easier and prompt arterial anastomosis and so, the use of arterial patches has been related to lower the incidence of complications. However, when a right hepatic artery variation from the superior mesenteric artery is present, the reconstruction occasionally produces twisting and flow problems. METHODS: Is described a surgical alternative for right hepatic artery variation reconstruction using a Carrel-patch from the superior mesenteric artery. The patch is anastomosed with the splenic artery stump to allow vertical orientation and improve blood flow. RESULTS: Among 120 liver transplants, four consecutive cases of right hepatic artery variation were reconstructed using this technique. All of them showed good flow and patency in postoperative period. CONCLUSION: The proposed technique proved to be an interesting alternative for the reconstruction of right hepatic artery variation in liver transplantation.

INTRODUÇÃO: A trombose de veia porta foi considerada contraindicação ao transplante de fígado no passado em razão da elevada morbi-mortalidade. Diversos avanços permitiram melhora dos resultados. OBJETIVO: Revisão dos avanços e das estratégias cirúrgicas utilizadas para realização do transplante de fígado na vigência de trombose de veia porta. MÉTODO: Revisão da literatura nas bases de dados Medline, Scielo, Lilacs cruzando os descritores: portal vein thrombosis, liver transplantation, vascular complications, jump graft, graft failure, multivisceral transplant. Foram estudados a epidemiologia, fatores de risco, classificação, diagnóstico, estratégias cirúrgicas e resultados. CONCLUSÃO: A trombose de veia porta deixou de ser contraindicação para o transplante hepático. O cirurgião dispõe atualmente de uma série de estratégias para realização do transplante, variando conforme o grau da trombose. Apesar de implicar em maior morbidade e taxas de re-trombose, os resultados do transplante na presença de trombose portal são semelhantes aos observados nas séries habituais. BACKGROUND: Portal vein thrombosis was considered a contraindication for liver transplantation in the past because of the high morbidity and mortality rates. Many advances made the results better. AIM: Review the advances and surgical strategies for liver transplantation in presence of portal vein thrombosis. METHOD: Survey of publications in Medline, Scielo and Lilacs databases. Headings crossed: portal vein thrombosis, liver transplantation, vascular complications, jump graft, graft failure, multivisceral transplant. Data analyzed were epidemiology, risk factors, classification, diagnosis, surgical strategies and outcomes. CONCLUSION: Portal vein thrombosis is not a contraindication for liver transplantation anymore. There are many strategies to perform the liver transplantation in this condition, depending on portal vein thrombosis grade. Regardless higher morbidity and re-trhombosis rates, the outcomes of liver transplantation in portal vein thrombosis are similar to series without portal vein thrombosis.

Introduction. Agenesis of the right lobe of the liver is a rare finding and was defined as the absence of liver tissue on the right side, with preservation of the middle hepatic vein, without previous disease or surgery. It is usually an incident finding reveled by imaging exams or during abdominal surgery. Case Report. A 32-year-old male patient was admitted to the hospital for abdominal discomfort and loss of appetite. Imaging studies revealed the absence of the right hepatic lobe and hypertrophied left hepatic segments. Discussion. Anomalies of hepatic morphology are rare and correspond to developmental defects during embryogenesis, are a rare diagnosis, and are generally diagnosed incidentally based on imaging. Agenesis or hypoplasia of the right lobe may predispose the patient to the development of portal hypertension and esophageal varices. Surgical knowledge of such anatomical agenesis is necessary for surgical planning, for the appropriate identification of intraoperative surgical findings, and for the design of the postoperative approach to therapy. Conclusion. Agenesis of the right hepatic lobe is a rare condition. We want to highlight the importance of understanding the condition. Surgeons must recognize the entity in order to deal appropriately with the findings.

Abdominal hernias are very frequent in cirrhotic patients with ascites. The hernias usually present as umbilical, inguinal, incisional, or femoral. However, these patients can also develop uncommon hernias such as pelvic hernias because of pelvic floor weakness and high abdominal pressure due to ascites. We present the first case of a cirrhotic patient with ascites that developed a giant sciatic hernia mimicking a perianal abscess.

RACIONAL: As hérnias de parede abdominal são um problema frequente no paciente cirrótico. Elas costumam ser volumosas e muito sintomáticas. OBJETIVO: Avaliar a qualidade de vida, através da aplicação do questionário Short Form-36 (SF-36), nos pacientes com hérnia e cirrose. MÉTODOS: Foram avaliados 39 pacientes cirróticos com hérnia de parede abdominal estratificados pela classificação de Child e submetidos ao questionário SF-36 por um mesmo profissional. RESULTADOS: A média dos escores obtidos no SF-36 mostra diminuição em todos os domínios nos pacientes cirróticos, principalmente no aspecto físico. O grupo Child B mostrou pior avaliação ao estado geral de saúde e capacidade funcional em relação ao grupo Child A, contudo não houve diferença em relação ao aspecto físico. CONCLUSÃO: A presença de hérnia de parede abdominal ocasiona piora na qualidade de vida do paciente cirrótico. A deterioração da função hepática está relacionada a pior qualidade de vida. A hérnia abdominal leva a restrição importante no aspecto físico também no Child A. BACKGROUND: Abdominal hernias are frequent issue in cirrhotic patients. These hernias tend to be large and symptomatic. AIM: To analyze the quality of life using a Short Form-36 (SF-36) questionnaire in cirrhotic patients with abdominal hernia. METHODS: Thirty nine cirrhotic patients with abdominal wall hernia were analyzed. They were stratified according to Child´s classification and answered the SF-36 questionnaire administered by a same professional. RESULTS: The scores from SF-36 show impairment in all questionnaire domains in cirrhotic patients, especially in the physical aspect. The Child B group showed worse health general condition and functional capacity comparing with Child A group; however it did not show any significant difference related to physical aspect. CONCLUSION: The presence of abdominal hernia leads to worse quality of life in cirrhotic patients. Hepatic function deterioration is related to a worse quality of life. Abdominal hernia causes an important restriction in physical aspect even in Child A.

Orientadores: Elaine Cristina de Ataíde, Nelson Marcio Gomes Caserta Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Introdução: A invasão microvascular do carcinoma hepatocelular (CHC) é sabidamente um fator que influencia negativamente o prognóstico do paciente submetido a ressecção cirúrgica. Objetivo: Avaliar se dados quantitativos do estudo dinâmico pelo meio de contraste da tomografia computadorizada (TC) pode ser utilizado para predizer invasão microvascular (IMV) no CHC. Material e métodos: Foi realizada análise retrospectiva dos dados das TC e exames anatomopatológicos (AP) das lesões com CHCs em pacientes submetidos a transplante ou ressecção hepática de março de 2010 a novembro de 2017. Duzentas lesões diagnosticadas como CHCs de 125 pacientes foram analisadas pelo mesmo radiologista experiente, de modo cego em relação ao resultado do AP. Regiões de interesse (ROI) foram obtidas das lesões tumorais e de áreas do fígado adjacente nas imagens pré-contraste, fases arterial, portal e de equilíbrio da TC pré-operatória. Os padrões quantitativos de realce e de desimpregnação ("washout") foram comparados com a presença histológica de IMV. Foram estudadas a diferença de atenuação - DA nas fases arterial, portal e de equilíbrio (definida como a diferença entre a atenuação da lesão e a atenuação do parênquima hepático adjacente nas fase arterial, portal e de equilíbrio, respectivamente), a razão de atenuação - RA nas fases portal e de equilíbrio (definida como 100 x razão de atenuação do parênquima hepático adjacente e da lesão nas fases portal e equilíbrio, respectivamente) e a razão de washout relativo ¿ RWR nas fases portal e de equilíbrio (definidas como 100 x razão da diferença de atenuação da fase arterial para a portal ou da fase arterial para a de equilíbrio, respectivamente) entre CHCs com IMV e aqueles sem IMV. Para estudo dos parâmetros foi utilizado o método das Equações de Estimativas Generalizadas - EEG (Generalized Estimating Equations-GEE). Para avaliação da acurácia dos parâmetros e determinação de ponto de corte para discriminar a IMV foi utilizada a curva ROC para medidas repetidas. O nível de significância adotado foi 5%. Resultados: Dos 200 CHCs, 77 (38,5%) tinham IMV. Nenhum dos parâmetros quantitativos pela TC foram preditivos de IMV. Não houve diferença estatisticamente significativa da razão de atenuação (RA) entre CHCs com IMV e aqueles sem IMV tanto nas fases portal (RA de 114,70 para aqueles com IMV e de 115,89 para aqueles sem IMV) como na fase de equilíbrio (RA de 126,74 para aqueles com IMV e de 128,20 para aqueles sem IMV). Também não houve diferença significativa na razão de washout relativo (RWR) nas fases portal e de equilíbrio entre CHCs com IMV e aqueles sem IMV (RWR de 15,08 para aqueles com IMV e de 8,20 para aqueles sem IMV na análise da fase portal; RWR de 31,45 para aqueles com IMV e de 26,31 para aqueles sem IMV na análise da fase de equilíbrio). A Diferença de Atenuação (DA) em nódulos com IMV e sem IMV também não teve diferença estatística significativa (DA de 35,28 para aqueles com IMV e de 29,62 para aqueles sem IMV na fase arterial; DA de -8,56 para aqueles com IMV e -8,39 nos sem IMV na fase portal; DA de -14,56 nos nódulos com IMV e -14,81 naqueles sem IMV para a fase de equilíbrio). Conclusão: Não houve relação dos parâmetros quantitativos da TC dinâmica pelo meio de contraste e a invasão microvascular dos CHCs. Todas as características estudadas falharam como fatores preditivos de invasão microvascular Abstract: Background: Microvascular invasion (MVI) is well known to negatively influence outcomes following surgical treatment of hepatocellular carcinoma (HCC). The aim of this study was to investigate whether quantitative measurements of dynamic computed tomography (CT) could be useful in predicting MVI in HCC. Methods: We retrospectively reviewed CT data and anatomopathological reports from focal liver lesions with HCC in patients who underwent to liver transplantation (LT) or surgical resection (SR) from march 2010 to november 2017. Two hundred surgically HCCs from 125 consecutive patients were blindly analyzed by a radiologist, without knowledge of the pathologic results. Regions of interest (ROIs) of lesions and areas of adjacent liver were quantitatively measured on pre-contrast, arterial, portal, and equilibrium phase images. The following enhancement profiles were analyzed: Attenuation change (AC), considered as the difference between the attenuation of lesion and the adjacent liver, calculated in all phases (arterial, portal and equilibrium); Percentage attenuation ratio (PAR), defined as 100 x attenuation ratio from adjacent liver parenchyma and the lesion, calculated in portal and equilibrium phases; Relative washout ratio (RWR) of portal and equilibrium phases, defined as 100 x attenuation difference ratio from arterial to portal phase or attenuation difference ratio from arterial to equilibrium phase, respectively. Enhancement profiles were compared with histopathological references of MVI. Generalized Estimating Equations (GEE) were used to parameters analysis. To evaluate accuracy and to determine cutoff values to discriminate MVI among the CT parameters, receiver operating characteristic (ROC) curve for repeated measures was performed. A p value < 0.05 was considered to indicate a significant difference. Results: Of the 200 HCCs, 77 (38,5%) had evidence of MVI on histopathological analysis. None of CT quantitative parameters were predictive of MVI. There was no statistically significant difference in percentage attenuation ratio (PAR) between HCCs with MVI and those without MVI in portal (median attenuation ratio, 114.70 for MVI and 115.89 for no-MVI) or equilibrium (median attenuation ratio, 126.74 for MVI and 128.20 for no-MVI) phases. There was also no statistically significant difference in relative washout ratio (RWR) on portal and equilibrium phases between HCCs with MVI and those without MVI (relative washout, 15.08 for MVI and 8.20 for no-MVI on portal phase, and 31.45 for MVI and 26.31 for no-MVI on equilibrium phase). Attenuation change (AC) in nodules with and those without MVI also had no statistically significant difference (attenuation change, 35.28 for MVI and 29.62 for no-MVI on arterial phase, -8.56 for MVI and -8.39 for no-MVI on portal phase, and -14.56 for MVI and -14.81 for no-MVI on equilibrium phase). Conclusion: There was no relation of preoperatively dynamic CT quantitative parameters with prediction of MVI for HCCs. Objetivo: Estudar os fatores de risco que interferem na incidência do carcinoma cutâneo em pacientes transplantados de fígado. Método: Efetuou-se um estudo observacional retrospectivo em 170 pacientes, dentre 349 pacientes transplantados entre 1997 a 2010 no centro estudado. Sendo incluídos, independente da raça ou sexo, adultos que foram submetidos a transplante de fígado, com enxerto hepático de doadores cadáveres, pela técnica de preservação da veia cava do receptor, independente do tipo de reconstrução venosa supra-hepática realizada, com sobrevida maior ou igual a três anos após o transplante e em uso de tacrolimus ou ciclosporina. Ao conjunto de dados coletados foi ajustado modelo de regressão logística tendo como variável resposta carcinoma cutâneo após o transplante indicado em análise anatomopatológica entre 1997 a 2014. O programa estatístico usado foi SPSS® Statistics, versão 21.0. Resultados: A incidência de carcinoma cutâneo foi de 9,4%. Os fatores preditores foram idade na época do transplante ? 60 anos (p = 0,036), uso de tacrolimus no primeiro ano pós-transplante (OR = 4,526; IC95% = 0,035 - 0,872; p = 0,033), presença de diabetes no 3º ano pós-transplante (OR = 4,317; IC95% = 1,139 - 87,081; p = 0,038) e ceratose actínica (OR = 6,154; IC95% = 1,737 - 110,948; p = 0,013). Conclusão: Os fatores encontrados neste estudo podem contribuir para uma melhor monitorização em programas de vigilância e estratégias associadas à diminuição de incidência de câncer de pele em pacientes tratados com transplante de fígado Abstract: Introduction: Many factors are currently being identified as potential inductors of skin cancer in patients after a liver transplantation ¿ among them, immunosuppressive regime. Objective: Investigate factors interfering in the incidence of skin cancer in patients after a liver transplantation. Methods: We have carried out a retrospective and observational study with 170 transplanted patients out of the 349 who had undergone a transplant surgery from 1997 to 2010 in the studied center. Being included, patients (18 years old or older), regardless of sex or race, who had undergone a liver transplantation with hepatic graft of deceased donors. This graft was performed with the technique of preservation of the receptor¿s vena cava, with a good survival rate of three or more years after the transplant using tacrolimus or cyclosporine. We have adjusted the logistic regression model to the ensemble of collected data using skin cancer as dependent variable, indicated in anatomopathological analysis between 1997 and 2014. For statistical tests, we have used SPSS® Statistics, version 21.0. Results: The incidence of skin cancer was 9.4%. Predictors were: age at the time of transplantation ? 60 years old (p = 0.036), tacrolimus use in the first year after the transplant (OR = 4.526; IC95% = 0.035 - 0.872; p = 0.033), incidence of diabetes in the third year after the transplant (OR = 4.317; IC95% = 1.139 ¿ 87.081; p = 0.038) and actinic keratosis (OR = 6.154; IC95% = 1.737 ¿ 110.948; p = 0.013). Objetivo: Comparar as alterações mitocondriais hepáticas, após clampeamento total intermitente versus o clampeamento contínuo do pedículo hepático por período de 40 minutos em ratos Wistar. Método: Quarenta e dois ratos Wistar machos, peso de 327,7 ± 51,75g, foram anestesiados com tiopental sódico (iv), com incisão em U no abdome. O pedículo hepático foi isolado e clampeado. Os animais foram distribuídos em três grupos: Grupo Contínuo (GC, n=14, submetidos a 40 minutos isquemia/40 minutos reperfusão). Grupo Intermitente (GI, n=14, submetidos a quatro ciclos de 10 minutos isquemia/10 minutos reperfusão). Grupo Controle (GCL, n=14, submetido a laparotomia por 80 minutos). Método: Efetuou-se um estudo observacional retrospectivo em 402 pacientes transplantados entre 1997 a 2012. Excluíram-se os pacientes que realizaram técnica tradicional (standard), uso de enxertos duplos ou reduzidos e os com dados incompletos. Avaliaram-se os dados do doador e do receptor. Para cada paciente foi calculado o escore BAR por meio do site http://www.assessurgery.com/bar-score/bar-score-calculator, cuja escala varia de 0 a 27 pontos.Os pontos de corte para BAR foram definidos por meio da área sob a curva ROC. A análise estatística foi efetuada usando-se método de Kaplan-Meier (teste log-rank) para SV, teste de Mann-Whitney para comparação entre grupos e análise regressão logística múltipla. Resultado: A SV aos três meses para BAR ? 11 foi de 46% e para BAR < 11 de 77% (p = 0,001); a SV aos 12 meses para BAR ? 11 foi de 44% e para BAR < 11 de 69% (p = 0,001). A análise de regressão múltipla mostrou como fatores determinantes para SV < três meses: escore BAR ? 11 pontos (OR = 3,08; IC95% = 1,75 - 5,42; p = 0,001) e uso de concentrado de hemácias(CH) acima de 6 unidades(u) no intraoperatório (OR = 4,49; IC95% = 2,73 - 7,39; p = 0,001) e para SV < 12 meses: escore BAR ? 11 pontos (OR = 2,94; IC95% = 1,67 - 5,16; p = 0,001) e CH > 6u intraoperatório (OR = 2,99; IC95% = 1,92 - 4,64; p = 0,001). Conclusão: Este estudo demonstra uma habilidade moderada do escore BAR, e realça a importância da hemotransfusão como fator preditor de sobrevida nesta população Abstract: Background: The balance of risk (BAR) is a prediction system after liver transplantation. Receiver operating characteristic (ROC) curve analysis with the Hosmer-Lemeshow test were used to calculate sensitivity, specificity and model calibration. Cutoff value were selected with the best Younden index. Statistical analysis employed the Kaplan-Meier method (log-rank test) for survival, the Mann-Whitney test for group comparison, and multiple logistic regression analysis. Results: 3-month survival was 46% for BAR ? 11 and 77% for BAR < 11 (p = 0.001); 12-month survival was 44% for BAR ? 11 and 69% for BAR < 11 (p = 0.001). Factors of survival < 3 months were BAR ? 11 (OR = 3.08; IC95% = 1.75 - 5.42; p = 0.001), and intrasurgical use of packed red blood cells (RBC) above 6 units (OR = 4.49; IC95% = 2.73 - 7.39; p = 0.001). For survival < 12 months, factors were BAR ? 11 (OR = 2.94; IC95% = 1.67 - 5.16; p = 0.001), and RBC > 6 units (OR = 2.99; IC95% = 1.92 - 4.64; p = 0.001).
