Artificial intelligence (AI) is an advanced scientific technology that can provide strong ability to assist in analysis and diagnosis of almost every type of data, therefore; AI widely used in medical fields, which is applied in the diagnosis and early detection of diseases. Kidney disease is one of the common diseases that are diagnosed and the necessary treatments are suggested by artificial intelligence. In this research, a logic system was used. The fuzzy logic system (FLS) is one of the artificial intelligence systems for diagnosing kidney diseases, where the fuzzy logic system divided into five variable inputs, namely urea, creatinine, glucose, bun, and uric acid, and they represented laboratory tests of the patients, this variables and also three outputs were identified, which are chronic inflammation and kidney failure, stones and salts, acute inflammation of the kidneys and bladder, which is the result of the medical diagnosis of the disease. Five memberships for inputs and three memberships for outputs are used in FLS. Diseases are concluded based on the values of the inputs, and thus the system proved its effectiveness and accuracy in diagnosis and this system is considered an aid to the specialized doctors in the field of kidney diseases.

VEXAS (vakuoler, E1-enzym, genvariant på X-kromosomet, utoinflammation, somatisk)-syndrom er et autoinflammatorisk syndrom først beskrevet i 2020. VEXAS-syndrom skyldes en genvariation med dysfunktion i ubiquitin-like modifier activating enzyme 1-genet (UBA1) i hæmatopoietiske stamceller i knoglemarven, hvilket fører til systemisk inflammation. Sygdommen rammer primært midaldrende til ældre mænd, da UBA1 er lokaliseret på X-kromosomet. De hyppigste symptomer er hududslæt, feber, vægttab, øjenaffektion, kondritis, artralgier og venøse tromboembolier. I knoglemarvsaspiratet er der karakteristiske vakuoler i myeloide og erytroide stamceller, og det kan være ledsaget af regelret myelodysplasi (MDS), monokolonal gammopati af ukendt betydning eller myelomatose [1]. VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a newly discovered complicated autoinflammatory syndrome associated with haematological and rheumatological manifestations, recognized for the first time in 2020. In this case report, we describe the first case of VEXAS syndrome in the North Denmark Region. A 76-year-old male was briefly admitted with COVID-19 and a myriad of symptoms including jaw pain, arthralgia, skin rash, malaise, intermittent fever and weight loss. After a prolonged diagnostic evaluation, VEXAS syndrome was suspected and confirmed with the presence of a mutated ubiquitin-like modifier activating enzyme 1 (UBA1) gene.

6. Spatiotemporal profile of altered neural reactivity to food images in obesity: Reward system is altered automatically and predicts efficacy of weight loss intervention

IntroductionObesity presents a significant public health problem. Brain plays a central role in etiology and maintenance of obesity. Prior neuroimaging studies have found that individuals with obesity exhibit altered neural responses to images of food within the brain reward system and related brain networks. However, little is known about the dynamics of these neural responses or their relationship to later weight change. In particular, it is unknown if in obesity, the altered reward response to food images emerges early and automatically, or later, in the controlled stage of processing. It also remains unclear if the pretreatment reward system reactivity to food images is predictive of subsequent weight loss intervention outcome.MethodsIn this study, we presented high-calorie and low-calorie food, and nonfood images to individuals with obesity, who were then prescribed lifestyle changes, and matched normal-weight controls, and examined neural reactivity using magnetoencephalography (MEG). We performed whole-brain analysis to explore and characterize large-scale dynamics of brain systems affected in obesity, and tested two specific hypotheses: (1) in obese individuals, the altered reward system reactivity to food images occurs early and automatically, and (2) pretreatment reward system reactivity predicts the outcome of lifestyle weight loss intervention, with reduced activity associated with successful weight loss.ResultsWe identified a distributed set of brain regions and their precise temporal dynamics that showed altered response patterns in obesity. Specifically, we found reduced neural reactivity to food images in brain networks of reward and cognitive control, and elevated reactivity in regions of attentional control and visual processing. The hypoactivity in reward system emerged early, in the automatic stage of processing (< 150 ms post-stimulus). Reduced reward and attention responsivity, and elevated neural cognitive control were predictive of weight loss after six months in treatment.DiscussionIn summary, we have identified, for the first time with high temporal resolution, the large-scale dynamics of brain reactivity to food images in obese versus normal-weight individuals, and have confirmed both our hypotheses. These findings have important implications for our understanding of neurocognition and eating behavior in obesity, and can facilitate development of novel integrated treatment strategies, including tailored cognitive-behavioral and pharmacological therapies.

8. Quantification of kidney microperfusion using non-invasive optical imaging

Background:Kidney transplantation is the preferred treatment of choice for selected patients with end-stage kidney disease (ESKD). Unfortunately, there is an imbalance between organ supply and organ demand, which could be reduced by accepting more marginal kidneys. There is therefore a need of improved preservation methods to deal with the increasing number of marginal donor kidneys, and in parallel, a need to introduce new diagnostic measures that allow quantitative information about the viability and microcirculation/oxygenation in the kidney.Laser speckle contrast imaging (LSCI), fluorescence based imaging (FBI) and optical coherence tomography (OCT) are optical imaging technologies, providing images that reflect microperfusion, microvascularisation and microanatomy. Due to their high spatial resolution and non-invasive performance (FBI is minimally invasive, due to the demand of contrast agents), we believe that these imaging technologies are promising methods for evaluating kidney viability prior to transplantation. Besides, information about such intrarenal microcirculatory markers could become beneficial for evaluating potential marginal donor kidneys in future.The aim of this study was to evaluate the feasibility of LSCI, FBI and OCT for assessment of kidney microperfusion and microvascularisation.Methods:Porcine kidneys are withdrawn from healthy pigs and connected to an ex vivo machine perfusion system with blood as an oxygen carrier. LSCI, FBI, and OCT systems were then placed close to the kidney. All systems are able to perform imaging in the renal cortex. The renal cortex’ microperfusion is gradually altered during the experimental procedure by introducing an ex vivo embolization model based on microsphere infusion in the arterial side of the ex vivo perfusion system. By positioning a reference tissue perfusion- and oxygenation sensor in the kidney parenchyma during the imaging procedures, we can compare our optical imaging-based (LSCI, FBI, and OCT) measurements with measurements obtained from reference invasive techniques.Altogether, by performing LSCI, FBI, and OCT on ex vivo perfused kidneys, we seek to quantify kidney microperfusion and microvascularisation and validate these measurements against an accepted reference method: an invasive microprobe (Oxford Optronics perfusion and oxygenation sensors).

BackgroundType 2 diabetes mellitus (T2D) is associated with an increased fracture risk. There is little evidence for the effects of glucagon-like peptide 1 receptor agonists (GLP-1RA) on fracture risk in T2D. We aimed to investigate the risk of major osteoporotic fractures (MOF) for treatment with GLP-1RA compared to dipeptidyl peptidase 4 inhibitors (DPP-4i) as add-on therapies to metformin.MethodsWe conducted a population-based cohort study using Danish national health registries. Diagnoses were obtained from discharge diagnosis codes (ICD-10 and ICD-8-system) from the Danish National Patient Registry, and all redeemed drug prescriptions were obtained from the Danish National Prescription Registry (ATC classification system). Subjects treated with metformin in combination with either GLP-1RA or DPP-4i were enrolled from 2007 to 2018. Subjects were propensity-score matched 1:1 based on age, sex, and index date. MOF were defined as hip, vertebral, humerus, or forearm fractures. A Cox proportional hazards model was utilized to estimate hazard rate ratios (HR) for MOF, and survival curves were plotted using the Kaplan-Meier estimator. In addition, Aalen’s Additive Hazards model was applied to examine additive rather than relative hazard effects while allowing time-varying effects.ResultsIn total, 42,816 individuals treated with either combination were identified and included. After matching, 32,266 individuals were included in the main analysis (16,133 in each group). Median follow-up times were 642 days and 529 days in the GLP-1RA and DPP-4i group, respectively. We found a crude HR of 0.89 [0.76–1.05] for MOF with GLP-1RA compared to DPP-4i. In the fully adjusted model, we obtained an unaltered HR of 0.86 [0.73–1.03]. For the case of hip fracture, we found a crude HR of 0.68 [0.49–0.96] and a similar adjusted HR. Fracture risk was lower in the GLP-1RA group when examining higher daily doses of the medications, when allowing follow-up to continue after medication change, and when examining hip fractures, specifically. Additional subgroup- and sensitivity analyses yielded results similar to the main analysis.ConclusionIn our primary analysis, we did not observe a significantly different risk of MOF between treatment with GLP-1RA and DPP-4i. We conclude that GLP-1RA are safe in terms of fracture.

The aims of our study can be clarified through the following questions: First: Does the exercise of the supervisory role in the economic unit require an important and essential role for the auditor based on the transformational leadership and self-marketing component. Second: Exercising the supervisory role in the economic unit requires adopting the arts of self-marketing for the accounts auditor to convey the content of his ideas and his method of work. Third: Does the application of international standards require the auditor’s involvement in a transformative leadership role, in order to reduce tensions and differences of opinion regarding the application of the mentioned standards. Fourth: Do workers in the various departments of the economic unit and mainly in the accounting departments need to be directed and led towards the new reality of applying international standards, or not.

12. Impacts of severity of Covid-19 infection on the morbidity and mortality of surgical patients

One of the challenges of surgery on patients with active SARS-CoV-2(severe acute respiratory syndrome coronavirus 2) infection is the increased risk of postoperative morbidity and mortality.This study will describe and compare the postoperative morbidity and mortality in asymptomatic patients or those with mild infection with those with severe COVID-19 infection undergoing elective or and emergency surgery.This is a retrospective study of 37 COVIDThere was no significant difference in gender between the two groups. There were 5 females (2 in group 1, and 3 in group 2) and 32 males (12 in group 1, and 20 in group 2). Mean age for all patients was 49.8years (38 for group 1 and 57 for group2). Median age for all patients was 50 years (37.5 for group 1 and 57 years for group 2). Sepsis developed in 7 patients (1 patient in group 1 and in 6 patients in group 2). Statistically there was no significant difference in occurrence of sepsis between the two groups. There was a significant difference in the intensive care stay between the two groups (higher in group 2). Four deaths were reported in group 1 and fourteen in group 2. Eighteen out of thirty-seven patients died.Severity of COVID-19 infection will prolong the hospitalization and ICU stay in surgical patients with no significant effect on mortality.

BACKGROUND: Arterial 18fluorodeoxyglucose (FDG) positron emission tomography (PET) is considered a measure of atherosclerotic plaque macrophages and is used for quantification of disease activity in clinical trials, but the distribution profile of FDG across macrophages and other arterial cells has not been fully clarified. OBJECTIVES: The purpose of this study was to analyze FDG uptake in different arterial tissues and their contribution to PET signal in normal and atherosclerotic arteries. METHODS: Wild-type and D374Y-PCSK9 transgenic Yucatan minipigs were fed a high-fat, high-cholesterol diet to induce atherosclerosis and subjected to a clinical FDG-PET and computed tomography scan protocol. Volumes of arterial media, intima/lesion, macrophage-rich, and hypoxic tissues were measured in serial histological sections. Distributions of FDG in macrophages and other arterial tissues were quantified using modeling of the in vivo PET signal. In separate transgenic minipigs, the intra-arterial localization of FDG was determined directly by autoradiography. RESULTS: Arterial FDG-PET signal appearance and intensity were similar to human imaging. The modeling approach showed high accuracy in describing the FDG-PET signal and revealed comparable FDG accumulation in macrophages and other arterial tissues, including medial smooth muscle cells. These findings were verified directly by autoradiography of normal and atherosclerotic arteries. CONCLUSIONS: FDG is taken up comparably in macrophage-rich and -poor arterial tissues in minipigs. This offers a mechanistic explanation to a growing number of observations in clinical imaging studies that have been difficult to reconcile with macrophage-selective FDG uptake. Reservoir modeling involves the construction of a computer model for prediction purposes and making decisions regarding the best operation of the reservoir. The environmental consequences of dams and reservoirs are numerous and varied, and includes direct impacts to the physical, chemical and biological properties of reservoirs and environments. This paper dealt with the study of the dissolved oxygen and the BOD concentrations in the inflow streams as well as those in Lake Morii, as well as the effects of phytoplankton, chlorophyll and reaeration rate using simulations in Mike Zero software. The main objectives of this research are to investigate the concentration of dissolved oxygen if they are among the required of water quality standards and how they are dispersed in the lake during a specific time period, as well as reducing concentration the BOD when it reached highest level, especially in summer time The simulations obtained gave us very good results about the dissolved oxygen and BOD concentrations in the reservoir.

Purpose of Review: Diabetes mellitus (DM) is associated with increased fracture risk. The aim of this systematic review was to examine the effects of different classes of glucose-lowering drugs on fracture risk in patients with type 2 DM. The heterogeneity of the included studies did not allow formal statistical analyses. Recent Findings: Sixty studies were included in the review. Metformin, dipeptidylpeptidase-IV inhibitors, glucagon-like peptide-1 receptor agonists, and sodium–glucose cotransporter 2-inhibitors do not appear to increase fracture risk. Results for insulin and sulphonylureas were more disparate, although there may be an increased fracture risk related to hypoglycemia and falls with these treatments. Glitazones were consistently associated with increased fracture risk in women, although the evidence was sparser in men. Summary: New glucose-lowering drugs are continuously being developed and better understanding of these is leading to changes in prescription patterns. Our findings warrant continued research on the effects of glucose-lowering drugs on fracture risk, elucidating the class-specific effects of these drugs.

The strength of (normal or high-strength) concrete is affected by the maximum size of a well-graded coarse aggregate. Concrete mixes containing larger coarse aggregate particles need less mixing water than those containing smaller coarse aggregates, In other words, small aggregate particles have more surface area than a large aggregate particle. In this research, about twenty-two mixtures were covered to study the effect of the MSCA, on compressive strength of (normal strength concrete) and Sixteen mixtures to study the effect of the maximum size of coarse aggregate on compressive strength for (high strength concrete). The concrete mixture is completely redesigned according to the maximum size of coarse aggregate needs and maintaining uniform workability for all sizes of coarse aggregate. The American design method was adopted ACI 211.1, for normal concrete. ACI 211-4R, the design method was adopted for high strength concrete. And use the MSCA with dimensions (9.5, 12.5, 19, 25, 37.5, and 50) mm for normal strength concrete and the MSCA (9.5, 12.5, 19, and 25) mm for high strength concrete. The slump was fixed (75-100) mm for normal strength concrete. Slump is fixed to (25-50) mm for high strength concrete before added Superplasticizer high range water reducer (HRWR). With Fineness Modulus (F.M) fixed to 2.8 for both normal concrete and high-strength concrete. According to the results of the tests, the compressive strength increases with the increase in the MSCA, of the normal concrete and also high – strength concrete. And the effect of the MSCA, on the compressive strength of normal concrete, is higher than that of high-strength concrete.

Psychological distress following cancer diagnosis may lead to mental health complications including depression and anxiety. Non-Hodgkin lymphomas (NHLs) include indolent and aggressive subtypes for which treatment and prognosis differ widely. Incident use of psychotropic drugs (PDs - antidepressants, antipsychotics, and anxiolytics) and its correlation to lymphoma types can give insights into the psychological distress these patients endure. In this prospective matched cohort study, we used nationwide population-based registries to investigate the cumulative risk of PD use in NHL patients compared to a sex- and age-matched cohort from the Danish background population. In addition, contact patterns to psychiatric departments and incident intentional self-harm or completed suicide were explored. In total, 8,750 NHL patients and 43,750 matched comparators were included (median age 68; male:female ratio 1.6). Median follow-up was 7.1 years. Background: Type 2 diabetes mellitus (T2D) is associated with an increased risk of fractures. Research on the effects of sodium-glucose cotransporter 2 (SGLT-2) inhibitors is scarce and unsettled. We aimed to investigate the risk of major osteoporotic fractures (MOF) – i.e., hip, vertebral, humerus, and forearm fractures – with SGLT2 inhibitors compared to glucagon-like peptide 1 (GLP-1) receptor agonists when either is used in combination with metformin.Methods: We conducted a population-based cohort study using discharge diagnosis codes from the Danish National Patient Registry and data on all redeemed drug prescriptions from the Danish National Prescription Registry. Subjects treated with metformin in combination with either SGLT2 inhibitors or GLP-1 receptor agonists between 2012 and 2018 were identified. Subjects were then propensity-score matched 1:1 based on age, sex, and index date. Survival curves were plotted using the Kaplan-Meier estimator. A Cox proportional hazards model was utilized to estimate crude and adjusted hazard rate ratios (HR) for MOF. Finally, Aalen’s Additive Regression (AAR) model was used to examine a possible additive rather than multiplicative effect of SGLT2 inhibitors on fracture hazard while allowing time-varying covariate effects.Results: We identified 27,543 individuals treated with either combination. After matching, 18,390 individuals were included in the main analysis (9,190 in each group). Median follow-up times were 355 [interquartile range (IQR) 126-780] and 372 [IQR 136-766] days in the SGLT2 inhibitor and GLP-1 receptor agonist group, respectively. The crude HR for MOPF was 0.77 [95% CI 0.56-1.04] with SGLT2 inhibitors compared to GLP-1 receptor agonists. The fully adjusted model yielded an unaltered HR of 0.77 [95% CI 0.56-1.05]. Results were similar across subgroup- and sensitivity analyses. Similarly, the multivariate AAR model yielded a non-significant difference between the two exposure groups.Conclusion: These results suggest that SGLT2 inhibitors have no effect on fracture risk when compared to GLP-1 receptor agonists. This is in line with results from previous studies.

ObjectivePatients with diabetes mellitus have an increased risk of fractures; however, the underlying mechanism is largely unknown. We aimed to investigate whether the risk of major osteoporotic fractures in diabetes patients differs between subjects initiated with alendronate and denosumab, respectively.Methods and Research DesignWe conducted a retrospective nationwide cohort study through access to all discharge diagnoses (ICD-10 system) from the National Danish Patient Registry along with all redeemed drug prescriptions (ATC classification system) from the Health Service Prescription Registry. We identified all subjects with a diabetes diagnosis between 2000 and 2018 and collected data on the first new prescription of anti-osteoporotic treatment between 2011 and 2018. Exposure was defined as either alendronate or denosumab treatment initiated after diabetes diagnosis. Outcome information was collected by identification of all major osteoporotic fracture (MOF) diagnoses, i.e., hip, spine, forearm, and humerus, from exposure until 2018 or censoring by emigration or death. The risk of fracture was calculated as hazard ratios (HR) using multiply adjusted Cox proportional models with death as a competing risk.ResultsWe included 8,745 subjects initiated with either alendronate (n = 8,255) or denosumab (n = 490). The cohort consisted of subjects with a mean age of 73.62 (SD ± 9.27) years, primarily females (69%) and suffering mainly from type 2 diabetes (98.22%) with a median diabetes duration at baseline of 5.45 years (IQR 2.41–9.19). Those in the denosumab group were older (mean 75.60 [SD ± 9.72] versus 73.51 [SD ± 9.23] years), had a higher proportion of women (81% versus 68%, RR 1.18 [95% CI 1.13–1.24], and were more comorbid (mean CCI 2.68 [95% CI 2.47–2.88] versus 1.98 [95% CI 1.93–2.02]) compared to alendronate initiators. In addition, denosumab users had a higher prevalence of previous fractures (64% versus 46%, RR 1.38 [95% CI 1.28–1.48]). The adjusted HR for any MOF after treatment initiation with denosumab was 0.89 (95% CI 0.78–1.02) compared to initiation with alendronate.ConclusionThe risk of incident MOF among subjects with diabetes was similar between those initially treated with alendronate and denosumab. These findings indicate that the two treatment strategies are equally effective in preventing osteoporotic fractures in subjects with diabetes.

Background The mechanisms by which hypertension accelerates coronary artery disease are poorly understood. Patients with hypertension often have confounding humoral changes, and to date, no experimental models have allowed analysis of the isolated effect of pressure on atherosclerosis in a setting that recapitulates the dimensions and biomechanics of human coronary arteries. Objectives This study sought to analyze the effect of pressure on coronary atherosclerosis and explore the underlying mechanisms. Methods Using inflatable suprarenal aortic cuffs, we increased mean arterial pressure by >30 mm Hg in the cephalad body part of wild-type and hypercholesterolemic proprotein convertase subtilisin kexin type 9 (PCSK9)D374Y Yucatan minipigs for >1 year. Caudal pressures remained normal. Results Under hypercholesterolemic conditions in PCSK9D374Y transgenic minipigs, cephalad hypertension accelerated coronary atherosclerosis to almost 5-fold with consistent development of fibroatheromas that were sufficiently large to cause stenosis on computed tomography angiography. This was caused by local pressure forces, because vascular beds shielded from hypertension, but exposed to the same humoral factors, showed no changes in lesion formation. The same experiment was conducted under normocholesterolemic conditions in wild-type minipigs to examine the underlying mechanisms. Hypertension produced clear changes in the arterial proteome with increased abundance of mechanical strength proteins and reduced levels of infiltrating plasma macromolecules. This was paralleled by increased smooth muscle cells and increased intimal accumulation of low-density lipoproteins in the coronary arteries. Conclusions Increased pressure per se facilitates coronary atherosclerosis. Our data indicate that restructuring of the artery to match increased tensile forces in hypertension alters the passage of macromolecules and leads to increased intimal accumulation of low-density lipoproteins. ObjectiveA link has been proposed between glucose homeostasis and bone metabolism. Bisphosphonates are first-line treatment of osteoporosis, and we aimed to investigate whether the risk of developing type 2 diabetes was associated with prior use of alendronate.Research Design and MethodsWe conducted a population-based nested case-control study through access to all discharge diagnoses (ICD-10 system) from the National Danish Patient Registry along with all redeemed drug prescriptions (ATC classification system) from the Health Service Prescription Registry. All cases with a diagnosis of type 2 diabetes between 2008 and 2018 were matched on sex and age with 3 randomly selected controls by incidence-density sampling. Exposure was defined as ever use of alendronate and further grouped as effective and compliant use. ORs were calculated by conditional logistic regression analysis with adjustment for several confounders and test for trend for dose-response relationship.ResultsWe included 163,588 patients with type 2 diabetes and 490,764 matched control subjects with a mean age of 67 years and 55% male subjects. The odds of developing type 2 diabetes were lower among ever users of alendronate (multiple adjusted OR: 0.64 [95% CI 0.62-0.66]). A test for trend suggested a dose-response relationship between longer effective use of alendronate and lower risk of type 2 diabetes.ConclusionThese results suggest a possible protective effect of alendronate in a dose-dependent manner against development of type 2 diabetes.

Background. Methods. Two methods were used the descriptive method in measurement of the brain dominance and hypocritical personality, and the experimental method to determine the effectiveness of using the SWOT and Kaizen strategies in developing the brain dominance and reduce the hypocritical personality with fourth stage students in Faculty of Physical Education and Sports Sciences, sample of the research 60 students and duration of the procedures 14 weeks. Results. The SWOT and Kaizen are of the important strategies. Teachers should use it with their students to learn and develop brain dominance and reduction hypocritical personality for students. Conclusion. Brain dominance and hypocritical personality is centered within the moderate normative level of the students, SWOT and Kaizen strategies play a small role in development of the brain dominance and effective in reducing the hypocritical personality for students, there is no difference between the Kaizen and SWOT strategies in developing the brain dominance and a SWOT strategy is better than Kaizen's strategy for reducing students' hypocritical personality.

Background Routine CT scans may increasingly be used to document normal aortic size and to detect incidental abdominal aortic aneurysms. Purpose To determine whether ultra-low-dose non-contrast CT (ULDNC-CT) can be used instead of the gold standard CT angiography (CTA) for assessment of maximal abdominal aortic diameter. Materials and Methods This retrospective study included 50 patients who underwent CTA and a normal-dose non–contrast CT for suspected renal artery stenosis. ULDNC-CT datasets were generated from the normal-dose non–contrast CT datasets using a simulation technique. Using the centerline technique, radiology consultants ( n = 4) and residents ( n = 3) determined maximal abdominal aortic diameter. The limits of agreement with the mean (LOAM) was used to access observer agreement. LOAM represents how much a measurement by a single observer may plausibly deviate from the mean of all observers on the specific subject. Results Observers completed 1400 measurements encompassing repeated CTA and ULDNC-CT measurements. The mean diameter was 24.0 and 25.0 mm for CTA and ULDNC-CT, respectively, yielding a significant but minor mean difference of 1.0 mm. The 95% LOAM reproducibility was similar for CTA and ULDNC-CT (2.3 vs 2.3 mm). In addition, the 95% LOAM and mean diameters were similar for CTA and ULDNC-CT when observers were grouped as consultants and residents. Conclusions Ultra-low-dose non–contrast CT exhibited similar accuracy and reproducibility of measurements compared with CTA for assessing maximal abdominal aortic diameter supporting that ULDNC-CT can be used interchangeably with CTA in the lower range of aortic sizes.

Botnets can simultaneously control millions of Internet-connected devices to launch damaging cyber-attacks that pose significant threats to the Internet. In a botnet, bot-masters communicate with the command and control server using various communication protocols. One of the widely used communication protocols is the ‘Domain Name System’ (DNS) service, an essential Internet service. Bot-masters utilise Domain Generation Algorithms (DGA) and fast-flux techniques to avoid static blacklists and reverse engineering while remaining flexible. However, botnet’s DNS communication generates anomalous DNS traffic throughout the botnet life cycle, and such anomaly is considered an indicator of DNS-based botnets presence in the network. Despite several approaches proposed to detect botnets based on DNS traffic analysis; however, the problem still exists and is challenging due to several reasons, such as not considering significant features and rules that contribute to the detection of DNS-based botnet. Therefore, this paper examines the abnormality of DNS traffic during the botnet lifecycle to extract significant enriched features. These features are further analysed using two machine learning algorithms. The union of the output of two algorithms proposes a novel hybrid rule detection model approach. Two benchmark datasets are used to evaluate the performance of the proposed approach in terms of detection accuracy and false-positive rate. The experimental results show that the proposed approach has a 99.96% accuracy and a 1.6% false-positive rate, outperforming other state-of-the-art DNS-based botnet detection approaches.

The paper addresses the debate on AI robots’ legal status, which often confuses the jurisprudence and decision-makers between these synthetic drugs’ legal effect and the legal personality status. If only the theories of jurisprudence or philosophers’ opinions contribute to the law’s development, engineers with their inventions also contribute to its effect. It considers the trends in this field and the actions made by specialists in artificial intelligence, and the independence of robots based on the latter. The lack of systems of responsibility traditional to absorb the damage caused by AI technologies. The European civil law commission for robotics is awarded the most sophisticated and independent robot’s legal personality for holding them responsible for themselves and not human beings. Still, this proposal faces many legal, moral, and philosophical challenges. According to this framework, the paper aims to shed more light on, discuss and analyze such an intelligent position robot might have from legal, ethical, and philosophical aspects. The article also explores the possibility of granting autonomous robots a new legal status that removes them from the circle of things and places them within the scope of personal. What is the point of giving this legal personality to robots based on artificial intelligence advocated by international institutions and researchers in law? The results show that granting legal personality within intelligent robots’ philosophical concept is possible as it exists. However, robots’ moral nature differs from a legal person’s existence because we see and feel the latter

Blockchain is a decentralized database that stores a growing chain of blocks formed and secured by the consensus of a network node. Blockchain technology is a solution to applications and smart programs so that no third party (peer-to-peer network) is required to store and verify transactions in a safe and reliable manner, such as cryptocurrencies, smart contracts, health care, and the Internet of things (IoT). However, this technology suffers from some limitations, such as network latency and slow transaction validation. In this paper, the proposed method is implemented and discussed by improving the topology of the peer-to-peer blockchain structure to a decentralized topology (structured topology in clusters). This aims to speed up transactions broadcasting time, as well as to reduce the enormous time for nodes consensus mechanism to validate transactions through the intra- and inter-cluster parallel mining in the Proof of Work (PoW) mechanism. The test results demonstrate the feasibility of the proposed method in reducing the consensus time, as well as improve the scalability of the network.

Background: Patient satisfaction is one of the major outcomes used in many countries to assess the quality of healthcare. Aim of the study: To assess the patient satisfaction towards health services in Al-Qaim city and its suburbs. Methodology: The researchers formulated a questionnaire, write them down in Google form Application and then questionnaires were reviewed by two experts. After that the link was sent disseminated to people residing at Al-qaim city. Two weeks later the researchers stopped looking for responses and spreadsheet was sent for statistical analysis. Results: The overall findings of this study revealed that 75% of respondents were unsatisfied with healthcare services.

Purpose of Review: Type 2 diabetes mellitus (T2DM) is associated with an increased fracture risk. Weight loss in T2DM management may result in lowering of bone mass. In this systematic literature review, we aimed to investigate how exercise affects bone health in people with T2DM. Furthermore, we examined the types of exercise with the potential to prevent and treat bone fragility in people with T2DM. Recent Findings: Exercise differs in type, mechanical load, and intensity, as does the osteogenic response to exercise. Aerobic exercise improves metabolic health in people with T2DM. However, the weight-bearing component of exercise is essential to bone health. Weight loss interventions in T2DM induce a loss of bone mass that may be attenuated if accompanied by resistance or weight-bearing exercise. Summary: Combination of weight-bearing aerobic and resistance exercise seems to be preventive against excessive bone loss in people with T2DM. However, evidence is sparse and clinical trials investigating the effects of exercise on bone health in people with T2DM are warranted.

The study was conducted on seventy individuals of both genders who have been exposed to human cytomegalovirus, a common illness in Iraq. Total cases of human cytomegvirus associated with malignant brain tumors were detected by a real time PCR technique. This resulted in only thirty-six cases of true infection. Of these 24 cases were female, while 12 cases of male infected. The titer to assay the presence of anti- and pro-inflammatories was assessed in sera of all patients by using ELISA kits to evaluate cytokines. BACKGROUND: Immunization with oxidized LDL (oxLDL) reduces atherosclerosis in rodents. We tested the hypothesis that treatment with a human recombinant monoclonal antibody against oxLDL will reduce the burden or composition of atherosclerotic lesions in hypercholesterolemic minipigs.METHODS AND RESULTS: Thirty-eight hypercholesterolemic minipigs with defective LDL receptors were injected with an oxLDL antibody or placebo weekly for 12weeks. An 18F-fluorodeoxyglucose positron emission tomography (FDG PET) scan (n=9) was performed before inclusion and after 3months of treatment. Blood samples were obtained prior to each injection. Following the last injection all animals were sacrificed, and the heart, aorta, and iliac arteries were removed. The left anterior descending coronary artery was sectioned at 5mm intervals for quantitative and qualitative assessments of atherosclerosis, including immunohistochemical phenotyping of macrophages using a pan-macrophage marker (CD68) and markers for putative pro-atherogenic (cathepsin S) and atheroprotective (CD163) macrophages. Aorta, right coronary artery, and left iliac artery were stained en face with Sudan IV and the amount of atherosclerosis quantified. There was no effect of treatment on plasma lipid profile, vascular FDG-PET signal or the amount of atherosclerosis in any of the examined arteries. However, immunostaining of coronary lesions revealed reduced cathepsin S positivity in the treated group compared with placebo (4.8% versus 8.2% of intima area, p=0.03) with no difference in CD68 or CD163 positivity.CONCLUSIONS: In hypercholesterolemic minipigs, treatment with a human recombinant monoclonal antibody against oxLDL reduced cathepsin S in coronary lesions without any effect on the burden of atherosclerosis or aortic FDG-PET signal.

Introduction: A cancer diagnosis is associated with profound psychological distress that potentially can lead to mental health problems such as depression and anxiety. Non-Hodgkin lymphomas (NHLs) are a heterogenous group of indolent and aggressive cancer diseases with high variability in treatment selections and patient outcomes. Some patients are chronically ill with recurrent need for mild chemotherapy whereas others face immediately life-threatening, yet curable disease. To gain valuable insights into the psychological distress associated with NHLs, the present study investigated the incident psychotropic drug (PD - antidepressants, anxiolytics, and antipsychotics) use, contact patterns to psychiatric departments, and intentional self-harm (including suicide) in Danish NHL patients relative to sex- and age matched individuals from the background population. Methods: The study was carried out as a nationwide population-based matched cohort study based on prospectively collected data from several Danish registries. All adult NHL patients (≥18 years) diagnosed between 2005 and 2015 were identified in the Danish Lymphoma Registry and included if they had not been treated with any kind of PD within the last 10 years prior to date of NHL diagnosis (index date). NHL patients were matched on age and sex with five random comparators from the Danish background population on the index date. Comparators had to be alive and without PD use 10 years prior to the index date. Incident PD use was defined as first redeemed prescription of PD after index date. Prescriptions were captured in the National Prescription Registry and described by cumulative incidences using the Aalen-Johansen estimator with death and NHL relapse as competing risk. Contacts with psychiatric departments and registration of intentional self-harm or completed suicide were captured in the Danish National Patient Registry. Patients were subcategorized according to type of lymphoma (Table 1). Results: In total, 7,201 NHL patients and 36,005 matched comparators were included (median follow-up 7.1 years). Follicular lymphoma (FL, 44.4%) and diffuse large B-cell lymphoma (DLBCL, 41.0%) were the most common subtypes (Table 2). Two-year cumulative incidence of PD use was higher in NHL patients overall (16.2%, 95%CI 15.4-17.0%) compared to matched comparators (5.7%, 95%CI 5.5-5.9%). Patients with aggressive NHL subtypes tended to have the highest incidence of PD use (Figure 1). Antidepressants (two-year cumulative incidence, 9.0%, 95%CI 8.4-9.6) and anxiolytics (8.2%, 95%CI 7.6-8.8) were the most used PDs in all NHL subtypes. The risk of PD use was higher in the first years following diagnosis, but except for patients with indolent NHL subtypes, the risk of PD use normalized over time to that of the background population. As for the risk of any psychiatric department contacts, there was no difference in two-year cumulative incidences between NHL patients (range 0.6-0.9%) and the matched comparators (range 0.6%-0.9%), whereas the two-year cumulative incidence of intentional self-harm and suicide was slightly higher for NHL patients (0.3%) compared to the matched comparators (0.2%, p=0.01). Conclusion: This study suggests that NHL patients have a significantly higher risk of mental health problems compared to the Danish background population, (when) using PD prescriptions as a proxy measure. The risk of intentional self-harm and completed suicide was also higher, but numerical differences were very small. Overall, the results emphasize the need for directing clinical attention on mental health in newly diagnosed NHL patients and screening for relevant symptoms during follow-up to provide best possible support to patients suffering from anxiety and depression. Objective Recent clinical studies have suggested an increased risk of elevated arterial pressure in patients with hydronephrosis. Animals with experimentally induced hydronephrosis develop hypertension, which is correlated to the degree of obstruction and increased oxidative stress. In this prospective study we investigated changes in arterial pressure, oxidative stress, and nitric oxide (NO) homeostasis following correction of hydronephrosis. Methods Ambulatory arterial pressure (24 h) was monitored in pediatric patients with hydronephrosis (n = 15) before and after surgical correction, and the measurements were compared with arterial pressure measurements in two control groups, i.e. healthy controls (n = 8) and operated controls (n = 8). Markers of oxidative stress and NO homeostasis were analyzed in matched urine and plasma samples. Results The preoperative mean arterial pressure was significantly higher in hydronephrotic patients [83 mmHg; 95% confidence interval (CI) 80–88 mmHg] than in healthy controls (74 mmHg; 95% CI 68–80 mmHg; p < 0.05), and surgical correction of ureteral obstruction reduced arterial pressure (76 mmHg; 95% CI 74–79 mmHg; p < 0.05). Markers of oxidative stress (i.e., 11- dehydroTXB2, PGF2α, 8-iso-PGF2α, 8,12-iso-iPF2α-VI) were significantly increased (p < 0.05) in patients with hydronephrosis compared with both control groups, and these were reduced following surgery (p < 0.05). Interestingly, there was a trend for increased NO synthase activity and signaling in hydronephrosis, which may indicate compensatory mechanism(s). Conclusion This study demonstrates increased arterial pressure and oxidative stress in children with hydronephrosis compared with healthy controls, which can be restored to normal levels by surgical correction of the obstruction. Once reference data on ambulatory blood pressure in this young age group become available, we hope cut-off values can be defined for deciding whether or not to correct hydronephrosis surgically. In this study, the reduced scale wall models were used (they are believed to resemble as much as possible the field conditions) to study the shrinkage behavior of reactive powder concrete (RPC) base restrained walls. Six base restrained RPC walls were casted in different length/height ratios of two ratios of steel fiber by volume in Summer. These walls were restrained by reinforced concrete bases to provide the continuous base restraint to the walls. The mechanical properties of reactive powder concrete investigated were; compressive strength between (75.3 – 140.1) MPa, splitting tensile strength between (5.7 – 13.9) MPa, flexural tensile strength (7.7 – 24.5) MPa, and static modulus of elasticity (32.7 – 47.1)GPa. Based on the observations of this work, it was found that the cracks did not develop in the reduced scale of the reactive powder concrete (RPC) walls restrained from movement at their bases for different L/H ratios (2, 5, and 10) and for two ratio of steel fiber (1% & 2%) during 90 days period of drying conditions. Moreover, the shrinkage values increase toward the edges. Based on the results of this work, the increase in the maximum shrinkage values of walls with 1% steel fiber were (29%, 28%, 28%) of the maximum shrinkage values of walls with 2% steel fiber of length/height ratios of (2, 5, and 10) respectively. The experimental observation in beam specimens showed that the free shrinkage, tensile strain capacity and elastic tensile strain capacity (at date of cracking) of beams with 1% steel fiber were higher than the beams with 2% steel fiber by about (24%), (45%) and (42%) respectively

Cloud computing is an emerging technology that came with a paradigm shift in computing and service hosting. The massive demand for cloud computing services required various techniques and new approaches to deliver reliable service efficiently and on time for users. These approaches include using new network topology, server and network virtualization, use of software-defined networks technology and network load balancing. Load balancing is an essential requirement for a fair and optimal distribution of services and bandwidth among farm servers. Various techniques for software-defined networking (SDN) architecture in load balancing are proposed. Research conducted is divided into static and dynamic load balance schemes. However, these approaches are expansive and use the same scheme for different servers. SDN is a powerful and programmable network technology. OpenFlow and OpenStack is part of SDN architecture that could provide services type, server load, and link health to a central location; thus, it provides better decision and configuration options. This search proposal explores the benefits and outcomes using SDN in cloud computing as a load balancing system.

Rhabdomyomatous mesenchymal hamartoma (RMH) is a rare hamartomatous lesion in the dermis and subcutaneous tissue. It is mostly found in the face and neck region of children. We report a case of solitary RMH located in the perineum of an 8-month-old boy. Microscopic examination of specimen showed a disordered collection of mature adipose tissue, skeletal muscle, adnexal elements and nerve bundles, and immunohistochemistry confirmed a RMH. This case emphasizes the possibility of RMH in the perineum of the children. knowing the desire of female students to belong to the community through the social desirability, and to identify the physical psychological manifestations and this is reflected in the level of performance of artistic gymnastics skills on the four devices, the relationship between the social desire and the physical psychological manifestations and performance of the artistic gymnastics skills. The sample of the research female students of the second stage in the faculty of physical education and sports sciences at the University of Babylon-Iraq. with number (30) for the year 2017-2018, use the approach descriptive in the method interconnections relationship and standard level, and use statistical instruments, Arithmetic mean, Standard deviation, Standard level, chi-square x2, cronbach alpha(α) and Kuder-Richardson for stability the scales, and after the distribution of the two scals on the sample and applying the tests of the skills of the gymnastic, the results showed moderate, high and good values, direct and inverse relationships, the results showed that the social desire and the performance of artistic gymnastics skills in the moderate level, physical psychological manifestations in the higher level, the conclusions there is an direct relationship between social desirability and performance of artistic gymnastics skills, and inverse relationship between the physical psychological manifestations with the skills of ground movements and direct with skills of the other device RESUMO Conhecer o desejo das alunas de pertencer à comunidade por meio da desejabilidade social, e identificar as manifestações psicológicas físicas se reflete no nível de atuação das habilidades de ginástica artística nos quatro aparelhos. O principal objetivo do estudo foi verificar a relação entre o desejo social e as manifestações psicológicas e físicas no desempenho das habilidades de ginástica artística. A amostra foi constituída por das alunas s da segunda etapa na faculdade de educação física e ciências do esporte da Universidade da Babilônia-Iraque. número (30) para o ano de 2017-2018, utilizam a abordagem descritiva na relação de interconexões do método e nível padrão, e uso de instrumentos estatísticos, média aritmética, desvio padrão, nível padrão, qui-quadrado x2, cronbach alfa (α) e Kuder-Richardson para estabilidade das escalas, e após a distribuição das duas scals na amostra e aplicando os testes das habilidades da ginástica, os resultados mostraram entre moderada, alta e bons valores e relações diretas e inversas. Os resultados mostraram que o desejo social e o desempenho de habilidades de ginástica artística se situaram no nível moderado, o resultado que o manifestações psicológicas físicas no nível superior e uma relação direta entre desejabilidade social e desempenho de habilidades de ginástica artística, ocorreu relação inversa entre as manifestações psicológicas físicas com as habilidades dos movimentos terrestres e direta com habilidades dos outros dispositivo.

The world lives in a technology revolution that started from invention the computer system, the Internet and now in the Internet of Things (IoT) or Internet of every things (IoE). This revolution impacted the human life strongly, by depending on this technology to enhance our life and inventing new products depending on information technology like social media, mobile applications, video games and so on. This research will present the simplest, yet efficient way to build a classic Arcade Machine by using a Raspberry Pi development board, also this research will highlight how to build and install all related components, such as body structure, buttons and joysticks, programming the Raspberry Pi and adding the video game files. Later, the research will explain how to connect the Arcade Machine to the Internet for remote operation by using MCU ESP8266, relays, and the Wi-Fi function on the Raspberry Pi itself. Beside the video games, the machine can behave as an ordinary computer for the Internet browsing, music listening, image processing, documenting or doing some programing tasks, as the Raspbian operating system has python programming apps and it is possible to install any other programing language needed to execute tasks on Raspberry Pi microcontroller.

Background. Obesity and metabolic syndrome (MetS) are major risk factors for atherosclerotic diseases; however, a causal link remains elusive. Animal models resembling human MetS and its complications, while important, are scarce. We aimed at developing a porcine model of human MetS. Methods. Forty pigs with familial hypercholesterolemia were fed a high fat + fructose diet for 30 weeks. Metabolic assessments and subcutaneous fat biopsies were obtained at 18 and 30 weeks, and fat distribution was assessed by CT-scans. Postmortem, macrophage density, and phenotype in fat tissues were quantified along with atherosclerotic burden. Results. During the experiment, we observed a >4-fold in body weight, a significant but small increase in fasting glucose (4.1 mmol/L), insulin (3.1 mU/L), triglycerides (0.5 mmol/L), and HDL cholesterol (2.6 mmol/L). Subcutaneous fat correlated with insulin resistance, but intra-abdominal fat correlated inversely with insulin resistance and LDL cholesterol. More inflammatory macrophages were found in visceral versus subcutaneous fat, and inflammation decreased in subcutaneous fat over time. Conclusions. MetS based on human criteria was not achieved. Surprisingly, visceral fat seemed part of a healthier metabolic and inflammatory profile. These results differ from human findings, and further research is needed to understand the relationship between obesity and MetS in porcine models.
