Background Urinary leakage after removal of the nephrostomy tube post-percutaneous nephrolithotomy (PNL) is a common complication which could be secondary to ureteral stone fragments, blood clots, ureteral edema at the ureteropelvic or ureterovesical junction, or rupture calyx. Unfortunately, it could impact the patient’s hospital stay causing it to be lengthy, delaying time to return to work, and the negative psychological effect. The current study aimed at assessing the role and effectiveness of alpha blockers (tamsulosin) in minimizing the duration of urinary leakage post-percutaneous nephrolithotomy (PNL). Methods In this prospective randomized clinical study, 62 cases of PNL were included. Physical examination, laboratory investigations and radiological work up (KUB and plain computed tomography) were done. Cases were randomized into two equal groups: Group A (31 cases) received tamsulosin perioperatively; Group B (31 cases) did not receive tamsulosin. The two groups were followed postoperatively as regards duration of urinary leakage, urinary catheterization and hospital stay. Results Duration of urinary leakage was statistically significantly shorter in group A (10.61 ± 6.66 h) compared to group B (21.48 ± 12.41 h) (p-value ˂ 0.001). Similarly, duration of hospital stay was shorter in group A (2.52 ± 0.72 days) compared to group B (3.10 ± 0.98) (p-value 0.020). Conclusions In patients undergoing PNL for renal stones and receiving tamsulosin, the duration of urinary leakage, and eventually hospital stay was shorter compared to those not receiving tamsulosin.

The main objective of this study is to reach a better relationship between the performance and the cost of any project. The Value Engineering characterized by a distinctive methodology for studying, analyzing and evaluating the projects; As work out with this study aims to raise the value of the project through the formulation of a valuable compromise curriculum works on measuring the special needs of users of the project and linked to cost in order to delete the unnecessary cost to achieve the balance between the performance of the project and its cost, thereby increasing its quality and value. The work plan of the Value Engineering is summarized, first, in gathering information about the project from documented sources, second, the functions of project are being analysed hence the performance is linked to the cost by classifying those functions, third, the team work moves to the phase of generating innovative ideas, proposals and alternatives of the project achieve the same performance and at a lower cost and delete the unnecessary functions, fourth, those alternatives will be evaluated, tested the best of them and delete the inappropriate ideas. The study of the requirements of users’ project is the main step to create the vacuum compatible with the needs of these users through the ruling values of the society. Functionality is determined to design this valuable project through the integration of structural, cultural, natural and interactive functions. Finally it can be concluded that linking the architectural study of the project with the psychology of the user to design spaces is humanly and functionally fit with the current and the future user.

The purpose of this case-control study was to investigate whether cervical mechanical deficits differ between patients with shoulder impingement syndrome (SIS) and control subjects. A total of 40 non-athletic males and females participated in this study. They were divided into two groups: group A, including 20 patients with SIS with a mean age of 36 years, and group B, including 20 control subjects with a mean age of 34 years. Each participant was assessed for pain and disability using shoulder pain and disability index (SPADI), active joint angular reproduction (AJAR) at 30o cervical flexion and extension using inclinometer, and craniovertebral angle (CVA) using photography. The results of the study showed that there was a statistically significant positive correlation between AJAR error at 30o cervical flexion and both shoulder pain and disability (r > 3, p =

تعتبر اسرة البلانتجنت من أطول الأسرات الحاکمة في تاريخ إنجلترا في العصور الوسطى, فقد جاوز فترة حکمها ما يزيد عن قرنين من الزمان. وکان من الطبيعي مع طول هذه الفترة أن يحدث في المجتمع الکثير من التغيرات الفکرية والاجتماعية, ولعل هذا ما يدفع المرء للبحث عن أحد المفاهيم الاجتماعية في إنجلترا في تلک الفترة ومحاولة تحليل مفهوم الزواج من الناحية الدينية والسياسية والادبية وصولا إلى المفهوم الشعبي الذي يمکن تلمسه بوضوح من خلال القصص الشعبي وسجلات الموتى وسجلات العامة. لقد تأثرت عقلية رجال الدين الإنجليز بالنصوص الواردة في الکتاب المقدس وکتابات الآباء الأوائل والحراک المجتمعى في إنجلترا، وقد انعکس ذلک من خلال تعدد وتضارب الآراء حول مفهوم وطبيعة الزواج. هذا التنوع والتضارب أعاد إنتاج عددا من الآراء الدينية التى ترسخت وتطورت في عقلية المجتمع الإنجليزي لتؤثر في النهاية على طبيعة وشکل العلاقة الأسرية في البيت الإنجليزي؛ ويظهر ذلک في رؤية اللاهوتى يوحنا من سالزبورى John of Salisbury - في القرن الثانى عشر- للزوجه بأنها " مثل الشخص القذر... وکالضفدع الأصفر تتلون بالملابس بلون الزعفران لإغراء الرجال". ويرى معاصره رئيس أساقفة روين بطرس من بلويس Peter of Blois " أن الزوجات يکسرن الرجال بعقودهن المغذية بالکره ويدمرن زواجهن ويفسدن حياة الکهنة ". ويرى إيلرد من ريفولAelred of Rievaulx (المتوفى 1167م) " أن أجمل ما في الزوجة هو الخبز والطعام الشهى... وعلى الرجال أن يحاولوا الابتعاد عنها ... لأنها لا ترضي الشهوة أبدا ". وتعزو ايف سالزبوري Eve Salisbury هذا الموقف العدائي تجاه الزوجة إلي أن کتاب الکنيسة الإنجليزية قد تأثروا بآباء الکنيسة الأوائل أمثال ترتوليان Tertullian وأويجن Origen وجيروم Jerome الذين اتخذوا من أسلوب حياة الزهد وانصهار الرغبة الجنسية داخل التقوى الدينية منهجا في الحياة. ويتفق کاراس Karras مع تحليل ايف ويضيف أن رجال الدين الإنجليزي کان ينقصهم التجديد وعدم تکرار العظات المذکوره عند کلا من جيروم وغيره من آباء الکنيسة الآوائل. وإن کان ذلک التحليل يبدو منطقيا من حيث اتفاق المضمون الذي أشارت إليه إيف سالزبوري في دعوتها للزهد وإنکار الرغبة الجنسية وملذات الحياة عموما، لکن يبدو أن المقصد الحقيقي لغالبية الکتاب الأوائل أو کتاب رجال الدين الإنجليز وقتذاک أستقي من رسالة القديس بولس للأرامل وغير المتزوجين من الرجال الذين دعاهم في حالة عدم الإستطاعة على الصبر بطاعة أوامر الرب بالتنسک أو الرهبنة " فليتزوجوا، لأن الزواج أصلح من التحرق". وقد استغل بعض رجال الدين الأحداث الواردة في القصة الرومانسية "حلم کونت أنجوLa patience de le Comtess d'Anjou " - المکتوبة عام 1158- حول عشق الملک هنري الثاني لطعام وجمال الملکة إلينور من أکوتين قبل تتويجه، وما صاحب ذلک من أخبار عن العلاقة الشائنة بينهما بالاراضى المقدسة ثم أحداث العصيان الذي ساهمت فيها الملکة عام 1173؛ لاستدعاء تلک الفکرة وتسخيرها لتسفيه الزواج, وظهر ذلک جليا عند عدد من کتاب التاريخ السياسي من رجال الدين في القرن الثانى عشر أمثال وليم من نيوبورج الذي عرض ضمن أحداث عام 1196م صورة خيانة الزوجة مع شاب من الجيران في مقابل سذاجة الزوج أنکسيوس Anxious الذي تقبل وضع الزنا الذي رآه بعينه لأن دموع زوجته قهرته.
