Abstract

Background & Aims: Over the last decade, clinical experiences and research studies raised concerns regarding use of proton pump inhibitors (PPIs) as part of the diagnostic strategy for eosinophilic esophagitis (EoE). We aimed to clarify the use of PPIs in the evaluation and treatment of children and adults with suspected EoE to develop updated international consensus criteria for EoE diagnosis. Methods: A consensus conference was convened to address the issue of PPI use for esophageal eosinophilia using a process consistent with standards described in the Appraisal of Guidelines for Research and Evaluation II. Pediatric and adult physicians and researchers from gastroenterology, allergy, and pathology subspecialties representing 14 countries used online communications, teleconferences, and a face-to-face meeting to review the literature and clinical experiences. Results: Substantial evidence documented that PPIs reduce esophageal eosinophilia in children, adolescents, and adults, with several mechanisms potentially explaining the treatment effect. Based on these findings, an updated diagnostic algorithm for EoE was developed, with removal of the PPI trial requirement. Conclusions: EoE should be diagnosed when there are symptoms of esophageal dysfunction and at least 15 eosinophils per high-power field (or approximately 60 eosinophils per mm2) on esophageal biopsy and after a comprehensive assessment of non-EoE disorders that could cause or potentially contribute to esophageal eosinophilia. The evidence suggests that PPIs are better classified as a treatment for esophageal eosinophilia that may be due to EoE than as a diagnostic criterion, and we have developed updated consensus criteria for EoE that reflect this change. © 2018 AGA Institute

Abstract

Systematic reviews and meta-analyses have become increasingly important in health care. Clinicians read them to keep up to date with their field,1,2 and they are often used as a starting point for developing clinical practice guidelines. Granting agencies may require a systematic review to ensure there is justification for further research,3 and some health care journals are moving in this direction.4 As with all research, the value of a systematic review depends on what was done, what was found, and the clarity of reporting. As with other publications, the reporting quality of systematic reviews varies, limiting readers' ability to assess the strengths and weaknesses of those reviews. Several early studies evaluated the quality of review reports. In 1987, Mulrow examined 50 review articles published in 4 leading medical journals in 1985 and 1986 and found that none met all 8 explicit scientific criteria, such as a quality assessment of included studies.5 In 1987, Sacks and colleagues6 evaluated the adequacy of reporting of 83 meta-analyses on 23 characteristics in 6 domains. Reporting was generally poor; between 1 and 14 characteristics were adequately reported (mean = 7.7; standard deviation = 2.7). A 1996 update of this study found little improvement.7 In 1996, to address the suboptimal reporting of meta-analyses, an international group developed a guidance called the QUOROM Statement (QUality Of Reporting Of Meta-analyses), which focused on the reporting of meta-analyses of randomized controlled trials.8 In this article, we summarize a revision of these guidelines, renamed PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses), which have been updated to address several conceptual and practical advances in the science of systematic reviews (Box 1). Abstract

GRADE requires guideline developers to make an overall rating of confidence in estimates of effect (quality of evidence high, moderate, low, or very low) for each important or critical outcome. GRADE suggests, for each outcome, the initial separate consideration of five domains of reasons for rating down the confidence in effect estimates, thereby allowing systematic review authors and guideline developers to arrive at an outcome-specific rating of confidence. Although this rating system represents discrete steps on an ordinal scale, it is helpful to view confidence in estimates as a continuum, and the final rating of confidence may differ from that suggested by separate consideration of each domain. An overall rating of confidence in estimates of effect is only relevant in settings when recommendations are being made. Abstract

We study both experimentally and theoretically the self-assembly of binary block copolymers in dilute solution, where self-assembly is triggered by changing the solvent from the common good solvent THF to the selective solvent water, and where the two species on their own in water form vesicles and spherical micelles respectively. We find that in water the inter-micellar exchange of these block copolymers is very slow so that the self-assembled structures are in local but not global equilibrium (i.e., they are non-ergodic). This opens up the possibility of controlling micelle morphology both thermodynamically and kinetically. Specifically, when the two species are first dissolved in THF before mixing and self-assembly (`premixing') by dilution with water, the morphology is found to depend on the mixing ratio of the two species, going gradually from vesicles via `bulbed' rods, rings, Y-junctions and finally to spherical micelles as we increase the proportion of the sphere-formers. On the other hand, if the two species are first partially self-assembled (by partial exchange of the solvent with water) before mixing and further self-assembly (`intermediate mixing'), novel metastable structures, including nanoscopic pouches, emerge. These experimental results are corroborated by self-consistent field theory calculations (SCFT) which reproduce the sequence of morphologies seen in the pre-mixing experiments. SCFT also reveals a clear coupling between polymer composition and aggregate curvature, with regions of positive and negative curvature being stabilized by an enrichment and depletion of sphere formers respectively. Our study demonstrates that both thermodynamic and kinetic blending of block copolymers are effective design parameters to control the resulting structures and allow us to access a much richer range of nano-morphologies than is possible with monomodal block copolymer solutions., 31 pages including 14 figures (double-spaced)

Abstract

An archaeological evaluation excavation was carried out in advance of making a planning application for a dwelling on land adjacent to The Rose Cottage Inn, Alciston, East Sussex. No archaeological features were encountered in Trench B, with only modern deposits being present above the natural. However, Trench A, situated towards the street frontage, revealed the footings of a post-medieval building, possibly dating to the 17th century, cut into the top of the natural and post-medieval material dating to the early 19th Century was recovered from the fill of the footing cut, suggesting it had been demolished by this date

Abstract

Evidence of the benefits of lowering cholesterol in various populations continues to grow, but questions persist about screening and treatment of lipid disorders in women. In this paper, we review the distinct features of the epidemiology of lipids and coronary heart disease in women and data from recent long-term treatment trials specific to women. Although data from primary prevention trials in women remain sparse, recent trials demonstrating benefits of cholesterol reduction across a broad range of cholesterol levels and cardiac risk in men and women bolster the conclusion that benefits may extend to asymptomatic women who are otherwise at high risk for coronary disease. Periodic lipid screening beginning in middle age will identify most women who are at high enough risk to merit drug therapy or more intensive individual lifestyle interventions. More detailed consideration of age, diabetes, blood pressure, specific lipid levels, or the ratio of total cholesterol to high-density lipoprotein cholesterol, and other risk factors can more accurately estimate individual risk of coronary heart disease and identify high-risk women most likely to benefit from lipid reduction. Advice about healthy diet, weight control, and physical activity can benefit all women, but authorities differ on the benefits of routine lipid screening in low-risk younger women.

Combining data from the 1971, 1981 and 1991 Censuses, the purpose of this study is to describe how the local areas in which people live in Britain have changed over the last two decades in the most simple way - through their population density. The population of all of Great Britain is considered, and enumeration district data form the 1971 and 1981 Censuses have been combined with ward level data from the 1981 Census, so that the changing population densities of people living in over 10,000 frozen areas can be considered.

Abstract

The future of the biomedical enterprise and the biomedical libraries that serve it is tied closely to digital information. The changing nature of this type of information will create new pressures on libraries, particularly in health care organizations. Libraries must learn to deal with these pressures. Currently, libraries depend on the Internet primarily for connections to resources and other libraries; thus enhancements to the Internet will impact the libraries of the present and future significantly. This paper provides an overview of the technical capabilities that will be available in the near to midterm, what libraries will be able to do with those capabilities, and how libraries can position themselves to take advantage of the impending changes.

Abstract

Two major hurdles to the widespread use of synthetic nucleic acids as drugs are the biological stability of the compounds and efficiency of cellular penetration. Recent advances in the chemistry of nucleic acids has given rise to highly stable derivatives with an anhydrohexitol backbone. This report addresses the cellular uptake of these molecules. We show that the uptake of HNA in the absence of a carrier is very low but HNA is efficiently internalized with a range of transfection reagents.

Abstract

We describe the computation which resulted in the title of this paper. Furthermore, we give an analysis of the data collected during this computation. From this data, we derive the important observation that in the final stages, the progress of the double large prime variation of the quadratic sieve integer factoring algorithm can more effectively be approximated by a quartic function of the time spent, than by the more familiar quadratic function. We also present, as an update to “Factoring by electronic mail” by Lenstra and Manasse (1990), some of our experiences with the management of a large computation distributed over the Internet. Based on this experience, we give some realistic estimates of the current readily available computational power of the Internet. We conclude that commonly-used 512-bit RSA moduli are vulnerable to any organization prepared to spend a few million dollars and to wait a few months

Abstract

The lavas and pyroclastics of Ascension Island contain a suite of coarse grained igneous blocks which range in composition from olivine-gabbro to peralkaline-granite paralleling, but extending beyond the compositional range of the volcanics. The lavas range from alkali-basalt through hawaiite, trachybasalt, trachyandesite and trachyte to comendite. True basalt is relatively rare and there is a scarcity of analyses with 57 < Si02 < 63 wt %.No high pressure mineral assemblages and hence no possible mantle fragments have been found. Petrographic and isotopic data suggest that a suite of gabbros from Dark Slope Crater crystallised from a magma derived from a MORB-like source. The remaining blocks and all the lavas evolved from magmas derived from a less depleted source.The chemical variation seen in the lavas and blocks is best explained by crystal fractionation mechanisms in a relatively shallow magma chamber. The gabbroic blocks exhibit cumulus textures suggesting that they formed by accumulation of settling crystals. The intermediate to acid blocks compare much more closely in composition with the evolved lavas and are probably their slowly cooled equivalents. There is petrographic evidence that partial melting of intermediate coarse grained material gave rise to melts of granitic composition but these are not chemically equivalent to the acid lavas and blocks. A pegmatoid body crystallised in situ and closed system crystal fractionation alone resulted in a very similar sequence of mineral assemblages to the blocks and lavas and a peralkaline final liquid.High 87 S4/ 86 Sr ratios in the evolved lavas and blocks are attributable to contamination by a small quantity of highly radiogenic oceanic sediment.Comparison with other oceanic volcanoes suggests that these differentiation processes are much less important in determining the evolutionary path of the magma than its apparent starting composition.
