INTRODUÇÃO: O mecanismo de ação dos inibidores do co-transportador de sódio-glicose 2 (iSGLT2) consiste no aumento da glicosúria. Existe evidência da associação entre a sua toma e a ocorrência de infeções genitais. Porém, a associação com infeções do trato urinário (ITU) não está bem estabelecida. Pretende-se analisar a evidência atual sobre a associação entre iSGLT2 em doentes com diabetes mellitus tipo 2 e a ocorrência de ITU.MÉTODOS: Pesquisadas normas de orientação clínica, meta-análises (MA), revisões sistemáticas e ensaios clínicos controlados e aleatorizados (ECCA) entre outubro de 2011 e outubro de 2021 na The National Institute for Health and Care Excellence, The Cochrane Library e PubMed. Utilizados os termos MeSH “urinary tract infection”, “sodium-glucose transporter 2 inhibitors” e “diabetes mellitus, type 2”. Para atribuição dos níveis de evidência (NE) foi utilizada a escala Strength of Recommendation Taxonomy, da American Academy of Family Physicians.RESULTADOS: Dos 331 resultados identificados, 6 cumpriram os critérios de inclusão: 5 ECCA e 1 MA. Todos classificados com NE 2. A evidência da associação entre a toma de iSGLT2 e ITU foi apenas demonstrada, de forma estatisticamente significativa, na MA e relativamente à dapagliflozina na dosagem de 10 mg.CONCLUSÃO: Os estudos apresentam resultados heterogéneos, inconsistentes e de qualidade moderada. Os iSGLT2 têm afirmado o seu lugar no tratamento de múltiplas patologias pelo que impera a realização de estudos dirigidos à ocorrência de efeitos adversos como a frequência de ITU. Here, we present a longitudinal analysis of two patients who recovered from COVID-19 more than 8 months prior but showed persistent serological detection of anti-SARS-CoV-2 IgM. We still do not know the exact reason for this prolonged persistence of IgM in these patients. To our knowledge, these are the first reports of IgM persistence in the context of SARS-CoV-2 infection and point to the need for no longer using IgM as a diagnostic criterion for acute or recent COVID-19. Introduction: Arterial hypertension is an important risk factor for cardiovascular events, such as acute myocardial infarction or stroke. Chronotherapy consists in using daily (circadian) or monthly hormonal rhythms to treat diseases, enhance the effects of medications and minimize iatrogenesis. It is, however, a poorly explored area, although there is evidence that chronotherapy might be useful in the treatment of hypertension. Primary objective: To evaluate the clinical interest of chronotherapy in the treatment of hypertension. Type of study: Multicentric, observational, cross-sectional and analytical. Research questions: Is the blood pressure pattern (dipper, non-dipper), assessed over 24 hours, conditioned by the time of administration of antihypertensive drugs (evening vs. morning)? Is blood pressure control conditioned by the time of administration of antihypertensive drugs (evening vs. morning)? Methodology: The information will be collected from patient’s clinical computerized process, namely the hypertension pattern (dipper or non-dipper), obtained by ABPM.Patients who do not have a characterization of their hypertension pattern by ABPM, will be asked to do so, in order to fulfill the conditions to integrate the study. Patients will be divided into two study groups, each with 200 patients, that is, 400 patients in total. In group A there will be patients who follow a therapeutic plan with anti-hypertensive medication administered in the afternoon and in group B, there will be patients who follow a therapeutic plan with anti-hypertensive medication administered in the morning. Introdução: A hipertensão arterial (HTA) constitui um importante fator de risco para eventos cardiovasculares (CV), tais como enfarte agudo do miocárdio ou acidente vascular cerebral. A cronoterapia consiste na utilização dos ritmos hormonais, diários (ritmo circadiano) ou mensais, do organismo para tratar doenças, potencializar efeitos de medicações e minimizar iatrogenia. É, no entanto, uma área pouco explorada, embora exista evidência de que a cronoterapia apresenta utilidade no tratamento da HTA. Objetivo primário: Avaliar o interesse clínico que a cronoterapia apresenta no tratamento da HTA. Tipo de estudo: Multicêntrico, observacional, transversal e analítico Questões de investigação: O padrão de pressão arterial (dipper, não-dipper), avaliado ao longo de 24 horas, é condicionado pelo horário de administração dos anti hipertensores (vespertino vs. matutino)? O controlo tensional é condicionado pelo horário de administração dos fármacos anti hipertensores (vespertino vs. matutino)? Metodologia: Recorrer-se-á à consulta do processo clínico informático dos doentes, nomeadamente à documentação através de MAPA do padrão de HTA em dipper ou não-dipper. Aos doentes que não possuem caracterização do seu padrão de HTA por MAPA, o mesmo será solicitado, a fim de cumprirem condições para integrar o estudo. Os doentes serão divididos em dois grupos de estudo, cada um deles com 200 doentes, ou seja, 400 doentes no total. No grupo A estarão os doentes que cumprem plano terapêutico com medicação anti hipertensora em horário vespertino e no grupo B, estarão os doentes que cumprem plano terapêutico com medicação anti hipertensora em horário matutino. Resultados: Utilizar-se-á o método quantitativo. O valor de p definido para se considerar um resultado estatisticamente significativo será de p < 0.05. Conclusão: O ritmo circadiano da pressão arterial é particularmente relevante para a medicina, contudo, continua a ser um conceito frequentemente ignorado. Background. The race to develop a protective vaccine for SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus 2) has raised questions regarding the safety of immunizers, the efficacy in the production of neutralizing antibodies, the longevity of the immune response, especially when related to variables such as sex and age.Objective. The present study evaluated the frequency of seroreactivity for anti-SARS-CoV-2 (S1 and S2) total antibodies and anti-SARS-CoV-2 (RBD - S1) neutralizing antibodies in individuals vaccinated with the immunizing agent Coronavac (Sinovac).Method. The study was cross-sectional and involved a total of 358 individuals divided into two groups. Group 1 consisted of 205 volunteers who were tested for anti-SARS-CoV-2 total antibodies (S1 and S2) and group 2 of 153 individuals tested for the presence of anti-SARS-CoV-2 neutralizing antibodies (RBD - S1).Results. Seroreactivity was above 70% in both groups, with approximately 20% of individuals showing no reactivity. The frequency of anti-SARS-CoV-2 total antibodies (S1 and S2) showed a significantly different distribution between sexes, but not according to age. The frequency of anti-SARS-CoV-2 (RBD - S1) neutralizing antibodies was 100% in the age group from 20 to 40 years, reducing significantly with advancing age to 88.9% (41 to 60 years), 78.7% (61 to 80 years) and 58.3% (>80 years).Conclusion. Our results demonstrate a high prevalence of anti-SARS-CoV-2 total antibodies (S1 and S2) and anti-SARS-CoV-2 (RBD - S1) neutralizing antibodies in individuals who received both doses of Coronavac vaccine, suggesting a lower efficiency of the humoral immune response among the elderly over 60 years of age, which could be associated with the senescence of the immune system. more...

Background In this study, the prevalence and persistence of anti-SARS-CoV-2 (severe acute respiratory syndrome-coronavirus) IgG was evaluated in volunteers 90 days after COVID-19 (coronavirus disease 2019) diagnosis by correlating response dynamics with clinical conditions, epidemiological characteristics, and disease severity. Methods The study recruited 200 volunteers aged 18 years or older of both sexes diagnosed with COVID-19. Of the 200 volunteers initially selected, the 135 individuals who underwent serological testing for anti-SARS-CoV-2 antibodies on the first visit to the laboratory, were invited to return, after 90 days, and provide a new blood sample for a second assessment of the presence of anti-SARS-CoV-2 IgG antibody. Disease severity and longevity of symptoms were evaluated for each individual and associated with the serological profile. Results Among the 135 individuals who underwent a previous serological test for anti-SARS-CoV-2 antibody, 125 showed reactivity to IgG (92.6%). Of the 125 individuals with detectable IgG in the first test, 87 (69.6%) showed persistence of this antibody after 90 days and 38 (30.4%) lost IgG reactivity in the second evaluation. The frequency of all reported symptoms was higher in individuals who maintained IgG persistence after 90 days of symptoms. Symptom manifestations lasted ≥21 days in the group with a persistent IgG response (39.6%) and ≤ 7 days in the group with a nonpersistent IgG response (50.0%). The length of hospital stay and supplemental oxygen use were higher in individuals with a persistent IgG response. Conclusions The results of the present study show a high frequency of loss of anti-SARS-CoV-2 IgG antibodies within 3 months after COVID-19 diagnosis in the Brazilian Amazon. more...

Descrever as variaveis neonatais, condicoes clinicas e nutricionais de recem-nascidos portadores de Atresia Esofagica submetidos a correcao cirurgica. Estudo documental, retrospectivo e quantitativo em 50 prontuarios de neonatos portadores de atresia de esofago, internados em um hospital de referencia. Na amostra: 96% das genitoras realizaram o pre-natal, com 64% nascidos de parto cesareo, sendo 62% do sexo feminino e 60% dos neonatos nasceram com peso normal. Sobre a classificacao da Atresia Esogagica, 84% tipo C. A media da amostra foi de 10 dias de vida a submissao da cirurgia de correcao e 20 dias de vida para introducao da dieta. A idade media no alcance da dieta plena foi de 30 dias, sendo em torno do 21o dia de pos-operatorio. A amamentacao foi informada em 44% da amostra. Predominou consonâncias entre as variareis do estudo comparadas com publicacoes nacionais e internacionais sobre o tema. Therapeutic and nutritional aspects of neonates submitted to esophageal atresia correction Abstract : To describe the neonatal variables, clinical and nutritional conditions of newborns with esophageal atresia who underwent surgical correction. This is a documentary, retrospective and quantitative study of 50 medical records of neonates with Esophageal Atresia, admitted in a reference hospital. In the sample: 96% of mothers received prenatal care, with 64% of infants born by cesarean delivery, 62% of whom were female and 60% of neonates were born with normal weight. Regarding the classification of Esophageal Atresia, 84% were classified as type C. The sample mean was being 10 days old until the moment that the newborn underwent correction surgery and 20 days old to introduce the diet. The average age to tolerate the full diet was 30 days, being around the 21st postoperative day. Breastfeeding was reported in 44% of the sample. Con respecto a la clasificacion de la Atresia de Esofago, 84% eran del tipo C. La media de la muestra fue de 10 dias hasta el momento de la realizacion de la cirugia de correccion y de 20 dias para introducir la dieta. La media de edad para alcanzar la dieta completa fue de 30 dias, alrededor del dia 21 de postoperatorio. La lactancia materna se informo en 44% de la muestra. Hubo una consonancia entre las variables de estudio en comparacion con las publicaciones nacionales e internacionales sobre el tema. A depressão afeta todas as faixas etárias e vem sendo cada vez mais observada em crianças e adolescentes. Os sintomas mais observados nessa população são a queda do rendimento escolar, tristeza e isolamento, que se intensificam prejudicando o desenvolvimento e contribuindo para a incapacidade humana. Dessa forma, o presente estudo tem como objetivo identificar os principais fatores para a ocorrência do transtorno depressivo infantil, buscando fatores influenciadores, tanto intrínsecos quanto extrínsecos a criança e ao adolescente. Para isso, foi realizada uma revisão integrativa, realizada a partir de dados disponíveis no PubMed, SciELO e Google Acadêmico, utilizando os termos MeSH do PubMed: “Depressive disorder”, “Depressive symptoms”, “Children”, “Adolescents” na qual foram selecionados 23 artigos. Existem diversos fatores de riscos que podem levar a criança e o adolescente a desenvolver o transtorno depressivo. A falta de afeto dos familiares, convivência com um dos pais depressivos, os estressores ambientais, como o abuso sexual e o físico e a perda de um ente querido são alguns desses fatores. Identificar a síndrome depressiva infantil é fundamental devido à necessidade de intervenções, emprego de técnicas de tratamentos e prevenção de problemas posteriores possibilitando assim, uma qualidade de vida maior para esse futuro adulto. more...

A Esclerose Múltipla (EM) é uma doença crônica degenerativa que atinge o sistema nervoso central. O tratamento visa a diminuição do processo inflamatório e retardar o avanço da doença. Este trabalho analisa as opções terapêuticas existentes e atualmente disponíveis para o tratamento de pacientes com esclerose múltipla. Foi realizado um estudo epidemiológico, a partir de uma revisão integrativa dos dados disponíveis no PubMed, SciELO e Google Acadêmico, utilizando os descritores “esclerose múltipla” e “tratamento”. A terapia farmacológica para EM pode ser empregada para tratamento sintomático, de episódios agudos/surtos, ou pela utilização de medicamentos modificadores da doença. A variedade existente de medicamentos modificadores da doença permite flexibilidade na escolha da melhor opção de acordo com o perfil de cada paciente. Aliando-se aos tratamentos medicamentosos, terapias complementares são importantes para a neuroreabilitação e controle de sintomas, como a psicologia, fonoaudiologia, neuropsicologia, fisioterapia e terapia ocupacional. Outros pontos recentemente discutidos no tratamento da EM são a utilização complementar de vitamina D, o uso de compostos canabinoides, e uma perspectiva futura do transplante autólogo de células tronco. Considerando a prevalência e impacto da EM sobre a qualidade de vida dos pacientes a longo prazo, a importância do diagnóstico e início do tratamento precoce, bem como a busca por novas evidências para o melhor manejo terapêutico possível da EM são fundamentais para a qualidade de vida do paciente. more...

Objetivo: Relatar os desfechos observados no tratamento de pacientes com Transtorno Afetivo Bipolar durante a gestação. Metodologia: Trata-se de uma pesquisa descritiva do tipo revisão integrativa. Foram considerados os estudos publicados no período entre 2000 e 2022, obtidos de bases de dados científicas que abrangem fontes relevantes inerentes ao tema, utilizando como um dos principais critérios a seleção de artigos originais e atuais. Ao todo, foram analisados 27 artigos que abordaram o tema. Resultados: O uso de lítio deve ser feito com ressalvas, devido ao risco de defeitos neurais no feto, além do acompanhamento atento a litemia da gestante. Anticonvulsivantes também possuem risco de defeitos neurais, além da possibilidade de alteração no metabolismo de anticoncepcionais, com risco de gestação indesejada. Antipsicóticos atípicos não apresentam riscos ao feto, salvo a risperidona. Aqueles com maior perfil metabólico, como a quetiapina e olanzapina, possuem maior risco de desenvolvimento de diabetes gestacional. Conclusão: O controle do humor de pacientes com transtorno bipolar deve ser acompanhado de forma competente pelo profissional de saúde, que deve estar munido de todas as opções terapêuticas, para que possa apresentá-las as pacientes, obtendo assim uma estratégia compartilhada e com maiores chances de sucesso. more...

Background: Since the introduction of Antiretroviral Therapy (ART), the life expectancy and health quality for patients infected with Human Immunodeficiency Virus (HIV) have significant-ly improved. Nevertheless, as a result of not only the deleterious effects of the virus itself and pro-longed ART, but also the effects of aging, cardiovascular diseases have emerged as one of the most common causes of death among these patients. Objective: The purpose of this review is to explore the new insights on the spectrum of Cardiovascu-lar Disease (CVD) in HIV infection, with emphasis on the factors that contribute to the atherosclerot-ic process and its role in the development of acute coronary syndrome in the setting of infection. Methods: A literature search using PubMed, ScienceDirect and Web of Science was performed. Ar-ticles up to Mar, 2017, were selected for inclusion. The search was conducted using MeSH terms, with the following key terms: [human immunodeficiency virus AND (cardiovascular disease OR coronary heart disease) AND (antiretroviral therapy AND (cardiovascular disease OR coronary heart disease))]. Results: Clinical cardiovascular disease tends to appear approximately 10 years before in infected in-dividuals, when compared to the general population. The pathogenesis behind the cardiovascular, HIV-associated complications is complex and multifactorial, involving traditional CVD risk factors, as well as factors associated with the virus itself - immune activation and chronic inflammation – and the metabolic disorders related to ART regimens. Conclusion: Determining the cardiovascular risk among HIV-infected patients, as well as targeting and treating conditions that predispose to CVD, are now emerging concerns among physicians. more...

We assess the impact of climate change on the geographic distribution and productivity of Euterpe oleracea (Arecaceae), commonly called açaí. To construct the ecological niche model of E. oleracea, we used 95 points of occurrence, five bioclimatic variables in current and future climate scenarios and Maxent software. The Akaike Information Criteria (AIC) was used to rank the ability of the models (considering ecological, socioeconomic and spatial variables) to explain the variation in productivity of E. oleracea among 200 municipalities. The maps showed that regions with higher environmental suitability for E. oleracea were concentrated in northern and northeastern Brazil, which was similar to the spatial pattern of productivity data of E. oleracea. Future climate conditions tend to promote an increase in the geographical distribution of this species, even though the new regions are in the arch of Amazon deforestation. Only space and the environmental suitability (indicated by the ecological niche model) were important for explaining the productivity of E. oleracea. That is, municipalities that are more productive are located in more suitable environmental regions. A ataxia espinocerebelar tipo 3, também conhecida como doença de Machado-Joseph, é um distúrbio neurodegenerativo autossômico dominante, provocado por uma expansão da repetição CAG que codifica a glutamina no gene ATXN3. Afeta tanto homens como mulheres, com sintomas em geral tardios, surgindo por volta da segunda década de vida e piorando com o avançar da idade. Apesar de rara, é a forma mais comum de ataxia espinocerebelar e não há evidências de terapias capazes de promover a cura. Assim, o presente estudo reuniu informações para atualizar os conhecimentos sobre essa doença. Trata-se de uma revisão integrativa de literatura realizada nas bases de dados PUBMED, SciELO e BVS. A ataxia cerebelar progressiva é comum nesses pacientes. No espectro clínico inclui anormalidades oculomotoras, distúrbios do sono, síndrome piramidal, sinais extrapiramidais e neuropatia periférica. Esses sintomas neurológicos são expressos por efeito da perda neuronal em regiões específicas do cerebelo, gânglios da base, tronco encefálico e medula espinhal. Pode ocorrer ansiedade, depressão e alto nível de fadiga. Ainda não há um tratamento eficaz que seja capaz de modificar o curso natural dessa doença. Os estudos mais recentes têm como alvo terapêutico o silenciamento gênico e a interceptação dos efeitos tóxicos provocados pelas proteínas produzidas. Terapias sintomáticas e fisioterapia tem sido realizada para melhora da qualidade de vida. Conhecer as características clínicas da doença de Machado-Joseph, que é uma condição grave, incurável, altamente debilitante e capaz de gerar uma alta carga de sofrimento nos seus portadores é importante na atuação do neurologista. more...

Objetivo: Verificar a prevalência de infecções relacionadas à assistência à saúde em pacientes adultos atendidos em Unidades de Terapia Intensiva. Metodologia: Estudo descritivo, epidemiológico, retrospectivo com abordagem quantitativa sobre a ocorrência de IRAS na unidade de terapia intensiva adulta, em um hospital das Forças Armadas de Belém, Estado do Pará. Resultado: Foram selecionados 91 pacientes, sendo a maioria idosa, de ambos os gêneros e proveniente da internação clínica/cirúrgica. Quanto ao perfil das infecções, houve predomínio das relacionadas ao sistema respiratório com (73,6%), seguindo com o trato urinário com (17,6%) e corrente sanguínea com (3,3%). Dentre os microrganismos identificados existe o predomínio de Acinetobacter baumanni (28,6%) e Candida SP (28,6%). Os antimicrobianos mais utilizados foram: Ceftriaxona (35,5%), Tazocin (32,3%) e Vancomicina (29,0%). Conclusão: Considera-se ter alcançado seu objetivo quanto a prevalência de IRAS na UTI da instituição pesquisada, possibilitando a equipe de saúde elencar estratégicas e medidas de prevenção com maior especificidade e direcionamento voltado à realidade local. Recently, ecological niche models have been employed to investigate the potential geographical distribution of species. However, it is necessary to analyze the vast number of publications on this topic to understand the trends and biases of research using ecological niche models (ENMs). Therefore, this study aims to investigate trends in the scientific literature regarding studies on ENMs. For the quantitative analysis of the literature on ENMs, we performed a search in the Thomson ISI (Web of Science) database between 1991 and 2013. Dissertação de mestrado integrado em Arquitectura, Universidade Lusíada de Lisboa, 2017 Exame público realizado em 23 de Março de 2018 Fotografia e Arquitetura: 3 Fotógrafos para 3 Arquitetos, é uma dissertação que estuda três casos práticos de fotógrafos de renome internacional com três arquitetos, também eles consagrados internacionalmente, de modo a compreender a importância que a fotografia, enquanto instrumento, tem sobre a arquitetura e vice-versa. O estudo organizou-se a partir de uma contextualização e enquadramento à fotografia nos seus aspetos técnicos e estéticos. Segue-se uma leitura que pretende relacionar, em termos gerais, a fotografia e a arquitetura, confirmando a importância da luz para ambos os casos. Por fim, pretendeu-se, através dos casos de estudo, investigar esta relação de um modo mais particular. Photography and Architecture: 3 Photographers for 3 Architects, is a dissertation that studies three practical cases of photographers with world renown and compares them with three architects, whose quality is also globally admired. It is this dissertation’s aim to understand the importance that photography, as an instrument, has over architecture, and vice versa. The study was organized by contextualizing and framing photography in its technical and aesthetical aspects. It then follows a reading that pretends to establish a general relationship between photography and architecture, while confirming the importance of light for both cases. A longevidade humana é uma importante conquista histórica e social, mas também, uma preocupação pelas consequências multidimensionais que encerra, entre as quais as quedas, como terceira causa de incapacidade crónica em idosos, sendo responsáveis por uma elevada morbilidade, mortalidade e sofrimento admitindo a necessidade de intervenção baseada em evidência. O atual projeto realizado no Agrupamento de Centros de Saúde de Sintra, na Unidade de Cuidados na Comunidade Cacém Care e Unidade de Cuidados de Saúde Personalizados Olival, de outubro de 2016 a fevereiro de 2017, teve como objetivos identificar o risco de queda na pessoa idosa e implementar intervenções adequadas e exequíveis, com ênfase na prevenção de queda, promovendo a segurança e a independência do idoso no seu domicílio. Para o diagnóstico da situação recorreu-se a uma amostra de conveniência constituída por 31 idosos, com a finalidade de identificar os fatores de risco multidimensionais presentes. Foi utilizada a metodologia de Planeamento em Saúde e o quadro conceptual de Dorothea Orem. A avaliação multidimensional do risco de queda numa amostra de 31 idosos permitiu identificar os seguintes fatores de risco: o ter mais de 85 anos, comorbilidades, polimedicação, fatores de risco ambiental e comportamental, barreiras na comunicação entre os profissionais e os idosos, falta de informação dos idosos em relação à prevenção de quedas e a falta de informação dos profissionais sobre conhecimento e compreensão dos fatores de risco enquanto primeira intervenção para a prevenção de quedas na pessoa idosa. Este projeto permitiu a introdução do diagnóstico de risco de queda a todos os idosos que se deslocam à Unidade de Cuidados de Saúde Personalizados Olival e Unidade de Cuidados na Comunidade Cacém Care, a uniformização do registo em SClinico, a formação da equipa, o envolvimento de interpares assim como, intervenções de enfermagem no idoso que recorre a estas duas Unidades. More severe vertebral fractures have more personal impact. In the European Prospective Osteoporosis Study, more severe vertebral collapse was predictable from prior fracture characteristics. Subjects with bi-concave or crush fractures at baseline had a 2-fold increase in incident fracture size and thus increased risk of a disabling future fracture. Introduction: According to Euler's buckling theory, loss of horizontal trabeculae in vertebrae increases the risk of fracture and suggests that the extent of vertebral collapse will be increased in proportion. We tested the hypothesis that the characteristics of a baseline deformity would influence the size of a subsequent deformity. Methods: In 207 subjects participating in the European Prospective Osteoporosis Study who suffered an incident spine fracture in a previously normal vertebra, we estimated loss of volume (fracture size) from plane film images of all vertebral bodies that were classified as having a new fracture. The sum of the three vertebral heights (anterior, mid-body, and posterior) obtained at follow-up was subtracted from the sum of the same measures at baseline. Each of the summed height loss for vertebrae with a McCloskey-Kanis deformity on the second film was expressed as a percentage. Results and Conclusions: In univariate models, the numbers of baseline deformities and the clinical category of the most severe baseline deformity were each significantly associated with the size of the most severe incident fracture and with the cumulated sum of all vertebral height losses. In multivariate modeling, age and the clinical category of the baseline deformity (crush > bi-concave > uni-concave > wedge) were the strongest determinants of both more severe and cumulative height loss. Baseline biconcave and crush fractures were associated at follow-up with new fractures that were approximately twice as large as those seen with other types of deformity or who previously had undeformed spines. In conclusion, the characteristics of a baseline vertebral deformity determines statistically the magnitude of vertebral body volume lost when a subsequent fracture occurs. Because severity of fracture and number of fractures are determinants of impact, the results should improve prediction of the future personal impact of osteoporosis once a baseline prevalent deformity has been identified. more...

Contains fulltext : 71178.pdf (Publisher’s version ) (Closed access) OBJECTIVE: We recently showed, using a candidate gene approach in a case-control association study, that a 65-kb block encompassing tumor necrosis factor receptor-associated factor 1 (TRAF1) and C5 is strongly associated with rheumatoid arthritis (RA). Compared with case-control association studies, family-based studies have the added advantage of controlling potential differences in population structure and are not likely to be hampered by variation in population allele frequencies, as is seen for many genetic polymorphisms, including the TRAF1/C5 locus. The aim of this study was to confirm this association in populations of European origin by using a family-based approach. METHODS: A total of 1,356 western European white individuals from 452 "trio" families were genotyped for the rs10818488 polymorphism, using the TaqMan allelic discrimination assay. RESULTS: We observed evidence for association, demonstrating departure from Mendel's law, with an overtransmission of the rs10818488 A allele (A = 55%; P = 0.036). By taking into consideration parental phenotypes, we also observed an increased A allele frequency in affected versus unaffected parents (A = 64%; combined P = 0.015). Individuals carrying the A allele had a 1.2-fold increased risk of developing RA (allelic odds ratio 1.24, 95% confidence interval 1.04-1.50). CONCLUSION: Using a family-based study that is robust against population stratification, we provide evidence for the association of the TRAF1/C5 rs10818488 A allele and RA in populations of European descent, further substantiating our previous findings. CONSELHO NACIONAL DE DESENVOLVIMENTO CIENTÍFICO E TECNOLÓGICO O objetivo deste trabalho é argumentar em favor de uma nova interpretação para o papel dos diagramas nas demonstrações da geometria euclideana. À luz de trabalhos recentes acerca do tema, pretende-se promover, em particular, uma nova avaliação daquele que é considerado o primeiro sistema dedutivo rigoroso na história da matemática: a geometria de Euclides, sistematizada nos seus Elementos. Com efeito, a utilização dos diagramas como partes essenciais das demonstrações neste sistema fez com que, na modernidade, tal sistema fosse considerado um exemplo de sistema informal, no qual as demonstrações são meros esboços do que seriam verdadeiras demonstrações. Estas, de acordo com a concepção de demonstração que se tornou comum na modernidade, devem ser compostas exclusivamente de fórmulas, as quais podem ser derivadas umas das outras apenas com base em regras lógicas ou princípios explícitos de antemão. Uma vez que tal concepção tornou-se dominante, por conta de diversos fatores nem sempre interligados, os diagramas que faziam parte das demonstrações euclideanas passaram a ser vistos como uma das principais causas de uma alegada falta de rigor por parte das mesmas. Para devolver às demonstrações matemáticas o rigor que lhes é necessário, autores como Hilbert e Pasch propuseram reconstruções formais da obra de Euclides, nas quais as demonstrações prescindem totalmente dos diagramas. No presente trabalho pretende-se reconstruir a seqüência de eventos que levou ao declínio das representações diagramáticas em geometria, bem como mostrar que é possível uma interpretação da obra de Euclides que leve em conta a participação dos diagramas nas demonstrações, sem que com isso as demonstrações sejam deficientes em termos de rigor. Serão rebatidas as críticas dos que defendem a concepção de demonstração acima mencionada, e, assim, será requerida uma revisão de tal postura - visando tanto a adoção de uma concepção mais abrangente de demonstração, quanto uma interpretação da geometria euclideana que faça mais justiça ao seu sucesso. The main concern of this work is to argue for new interpretations regarding the role of the diagrams in Euclidean geometry. Taking into account recent works on the subject, the goal here is to present alternative ways to evaluate the system which is considered the first rigorous deductive system in the history of mathematics: Euclid`s Elements. In fact, the use of diagrams as parts of its demonstrations has been considered as a flaw of that formal system. According to the standard conception of demonstration in modern times, a demonstration must be a chain of formulae, each of them being either a principle (accepted without demonstration) or a formula that follows from some principle by logical inference. As this conception became influent, the diagrams in Euclidean geometry turned out to be seen as one of the main reasons for an alleged lack of rigor of its demonstrations. In face of this, authors like Pasch and Hilbert worked on a formalization of Euclidean geometry in modern fashion, i.e., suppressing the diagrams from its demonstrations. The present work aims at a reconstruction of the main events which led to the decline of diagrammatic representations in geometry. It will be shown that an alternative view is possible. This view takes into account the importance of diagrams for the demonstrations without denying their deductive rigor. It will be argued against the conception of demonstration mentioned above, and for a revision of such conception in order to achieve a broader and fairer conception of Euclidean geometry. more...

Background: Polymorphisms of the vitamin D receptor (VDR) gene have been implicated in the genetic regulation of bone mineral density (BMD). However, the clinical impact of these variants remains unclear. Objective: To evaluate the relation between VDR polymorphisms, BMD, and fractures. Design: Prospective multicenter large-scale association study. Setting: The Genetic Markers for Osteoporosis consortium, involving 9 European research teams. Participants: 26 242 participants (18 405 women). Measurements: Cdx2 promoter, FokI, BsmI, ApaI, and TaqI polymorphisms; BMD at the femoral neck and the lumbar spine by dual x-ray absorptiometry; and fractures. Results: Comparisons of BMD at the lumbar spine and femoral neck showed nonsignificant differences less than 0.011 g/cm2 for any genotype with or without adjustments. A total of 6067 participants reported a history of fracture, and 2088 had vertebral fractures. For all VDR alleles, odds ratios for fractures were very close to 1.00 (range, 0.98 to 1.02) and collectively the 95% CIs ranged from 0.94 (lowest) to 1.07 (highest). For vertebral fractures, we observed a 9% (95% CI, 0% to 18%; P = 0.039) risk reduction for the Cdx2 A-allele (13% risk reduction in a dominant model). Limitations: The authors analyzed only selected VDR polymorphisms. Heterogeneity was detected in some analyses and may reflect some differences in collection of fracture data across cohorts. Not all fractures were related to osteoporosis. Conclusions: The FokI, BsmI, ApaI, and TaqI VDR polymorphisms are not associated with BMD or with fractures, but the Cdx2 polymorphism may be associated with risk for vertebral fractures. more...

Vertebral fractures are associated with back pain and disability. There are, however, few prospective data looking at back pain and disability following identification of radiographic vertebral fracture. The aim of this analysis was to determine the impact of radiographically identified vertebral fracture on the subsequent occurrence of back pain and disability. Women aged 50 years and over were recruited from population registers in 18 European centers for participation in the European Prospective Osteoporosis Study. Participants completed an interviewer-administered questionnaire which included questions about back pain in the past year and various activities of daily living, and they had lateral spine radiographs performed. Participants in these centers were followed prospectively and had repeat spine radiographs performed a mean of 3.7 years later. In addition they completed a questionnaire with the same baseline questions concerning back pain and activities of daily living. The presence of prevalent and incident vertebral fracture was defined using established morphometric criteria. The data were analyzed using logistic regression with back pain or disability (present or absent) at follow-up as the outcome variable with adjustment made for the baseline value of the variable. The study included 2,260 women, mean age 62.2 years. The mean time between baseline and follow-up survey was 5.0 years. Two hundred and forty participants had prevalent fractures at the baseline survey, and 85 developed incident fractures during follow-up. After adjustment for age, center, and the baseline level of disability, compared with those without baseline prevalent fracture, those with a prevalent fracture (odds ratio [OR] = 1.4; 95% confidence interval [CI] 1.0 to 2.0) or an incident fracture (OR = 1.7; 95% CI, 0.9 to 3.2) were more likely to report disability at follow-up, though the confidence intervals embraced unity. Those with both a prevalent and incident fracture, however, were significantly more likely to report disability at follow-up (OR = 3.1; 95% CI, 1.4 to 7.0). After adjustment for age, center, and frequency of back pain at baseline, compared with those without baseline vertebral fracture, those with a prevalent fracture were no more likely to report back pain at follow-up (OR = 1.2; 95% CI, 0.8 to 1.7). There was a small increased risk among those with a preexisting fracture who had sustained an incident fracture during follow-up (OR = 1.6; 95% CI, 0.6 to 4.1) though the confidence intervals embraced unity. In conclusion, although there was no significant increase in the level of back pain an average of 5 years following identification of radiographic vertebral fracture, women who suffered a further fracture during follow-up experienced substantial levels of disability with impairment in key physical functions of independent living. more...

O mercado de derivativos tem crescido de forma impressionante nos últimos vinte anos, seja em termos de volume ou em termos de instrumentos financeiros disponíveis. Embora tenha havido perdas que ocorreram com empresas bastante conhecidas do público e aliada as inúmeras advertências dos meios de comunicação sobre os riscos inerentes a esses instrumentos, ainda assim, o mercado de derivativos tem sido usado por um grande número de empresas, já que esses instrumentos permitem transferir os riscos tanto de mercado como o de crédito para uma terceira parte. Entre as causas que originaram as perdas acima mencionadas figuram as devidas à relativa falta de conhecimento com que esses instrumentos têm sido utilizados. Além do mais, esses instrumentos sempre foram considerados complexos e um tanto sofisticados e num primeiro momento não foram contabilizados, sendo portanto considerados como itens fora do balanço, e dessa forma, muitas empresas usavam-nos para fugir das exigências das legislações, mantendo assim os riscos dos ativos no próprio balanço, quando deveriam justamente evitar esse tipo procedimento. Nesse artigo, nosso objetivo se concentrou nas definições do que vem a ser um instrumento derivativo, procurando mostrar a evolução na forma de contabilizá-los, finalizando com a abordagem realizada pelo SFAS 133 na forma de mensurar os derivativos, mostrando que a forma de contabilizá-los a valores justos ("fair value") é uma evolução quando se compara com o "velho" método do custo histórico.The derivatives market has been increasing at an impressive pace over the last twenty years, both in terms of traded volume and financial instruments available. Although losses occurred for companies whose names were familiar to the public in general, along with the innumerous warnings given by the media about the risks involved in this kind of instruments, derivatives have been used by a wide range of companies, since they allow for the transfer of either market or credit risk to third parties. The aforesaid losses were due to the relative lack of knowledge with which these instruments have been used. Moreover, these instruments were always considered complex and rather sophisticated and, in the first instance, they were not accounted for, i.e., they were considered offbalance sheet items and, consequently, many companies used them to escape from legal demands, thus maintaining the assets' risks in their balance sheets, when they should really avoid this kind of procedures. In this article, our objective was concentrated on the definitions of a derivative instrument, seeking to show the evolution in its accounting treatment. International audience; Objectives: The study of polymorphisms of genes differentially expressed may lead to the identification of putative causal genetic variants in multifactorial diseases such as rheumatoid arthritis (RA). Based on preceding transcriptomic results, we genotyped 10 single nucleotide polymorphisms (SNPs) belonging to six genes (S100A8, RNASE2, PGLYRP1, RUNX3, IL2RB, and LY96) showing the highest fold change (\backslashtextgreater 1.9) when level of expression was compared between RA patients and controls. These SNPs were then analysed to evaluate their role in RA. Method: The relationship between gene expression and genotypes of SNPs was first investigated by Kruskal-Wallis and Mann-Whitney tests in RA patients and controls. The genetic association of these SNPs with RA were then analysed using family-based association tests in trio families. Results: We found that RNASE2 gene expression was related to rs2013109 genotypes in 14 RA patients (p = 0.030). The association study in a discovery sample of 200 French trio families revealed a significant association with RA for one SNP, PGLYRP1-rs2041992 (p = 0.019); this association was stronger in trios where RA patients carried the HLA-DRB1 shared epitope (SE) (p = 0.003). However, this association was not found in a replication sample of 240 European trio families (p = 0.6). Conclusions: Family-based association tests did not reveal an association between RA and any SNP of the candidate genes tested. However, RNASE2 gene expression was differentially expressed in RA patients considering a sequence polymorphism. In the European Prospective Osteoporosis Study (EPOS), a past spine fracture increased risk of an incident fracture 3.6–12-fold even after adjusting for BMD. We examined the possibility that biochemical marker levels were associated with this unexplained BMD-independent element of fracture risk. Each of 182 cases in EPOS of spine or non-spine fracture that occurred in 3.8 years of follow-up was matched by age, sex and study centre with two randomly assigned never-fractured controls and one case of past fracture. Analytes measured blind were: osteocalcin, bone-specific alkaline phosphatase, total alkaline phosphatase, serum creatinine, calcium, phosphate and albumin, together with the collagen cross-links degradation products serum CTS and urine CTX. Most subjects also had bone density measured by DXA. Cases who had recent fractures did not differ in marker levels from cases who had their last fracture more than 3 years previously. No statistically significant effect of recent fracture was found for any marker except osteocalcin, which was 17.6% lower in recent peripheral cases compared to unfractured controls (p more...

OBJECTIVE—To describe the characteristics of a new set of European families with affected sib pairs (ASP) collected by the European Consortium on Rheumatoid Arthritis Families (ECRAF) to replicate the results of our first genome scan. Potential gradients for disease severity in Europe and concordance within families were studied. PATIENTS AND METHODS—From 1996 to 1998 European white families with at least two affected siblings were enrolled in the study. Demographic (sex, age at onset), clinical data (rheumatoid factor (RF), disease duration, erosive disease, extra-articular features (EF)), and HLA-DRB1 oligotyping were analysed. RESULTS—565 patients with rheumatoid arthritis (RA), belonging to 271 families including 319 affected sib pairs (ASP) were collected. Belgium, France, Italy, the Netherlands, Portugal, and Spain contributed 20, 96, 52, 24, 9, and 70 families, respectively. Sex (78% women), age at onset (mean 44 years), and RF positivity (79%) were similar among the countries. Objective To compare the safety, tolerability and efficacy of the new oral microemulsion formulation of cyclosporin A (CyA; Sandimmun Neoral) and the original CyA formulation (Sandimmun), in patients with severe active rheumatoid arthritis (RA), over a 12-month period. Methods In this double-blind, multicentre study, patients were randomized to treatment with Neoral or Sandimmun, starting with 2.5 mg/kg/day, with dose adjustments after 4 weeks. Primary efficacy criteria included patients' assessment of disease activity. Pharmacokinetic and safety assessments were performed at regular intervals. Results Compared with Sandimmun, Neoral showed a consistent trend towards greater clinical efficacy from week 12 onwards, including a significant difference in patients' assessment of disease activity at the study end-points. A significantly lower increase in dose from baseline was observed with Neoral at week 24. Pharmacokinetic assessments at week 24 showed increased absorption and decreased variability with Neoral. No differences in safety were found between treatment groups. Conclusion These observations indicate that Neoral is as safe and at least as effective as Sandimmun and have important implications for patient management given the increasing role for CyA in the treatment of severe, active RA. more...

Physalaemus biligonigerus, P. fuscomaculatus and P. sp. have 2n=2 chromosomes, consistent with the number of chromosomal pairs found in the genus.There were no substantial differences in the karyotypes of the three species, except for chromosome 3 which was submetacentric in P. fuscomaculatus and metacentric in P. biligonigerus and P. sp.All chromosomes were characterised by the presence of a C-band in the pericentromeric region. Some interstitial bands were also detected at the same position in all karyotypes, but the band located on the short arm of chromosome 3 was larger in P.biligonigerus and P. sp. than in P. fuscomaculatus.Also, only P. sp. had a large block of pericentromeric heterochro-matin in the short arm of chromosome 9 and another one in the telomeric band on the long arm. The NOR was located on the long arm of chromosome 9 in the three species. These findings confirmed that the three species are indeed closely related cytogenetically. more...

The aims of this study were to identify risk factors for hip fracture in men aged 50 years or more. We identified 730 men with hip fracture from 14 centers from Portugal, Spain, France, Italy, Greece and Turkey during the course of a prospective study of hip fracture incidence and 1132 age-stratified controls selected from the neighborhood or population registers. The questionnaire examined aspects of work, physical activity past and present, diseases and drugs, height, weight, indices of co-morbidity and consumption of tobacco, alcohol, calcium, coffee and tea. Significant risk factors identified by univariate analysis included low body mass index (BMI), low sunlight exposure, a low degree of recreational physical activity, low consumption of milk and cheese, and a poor mental score. Co-morbidity including sleep disturbances, loss of weight, impaired mental status and poor appetite were also significant risk factors. Previous stroke with hemiplegia, prior fragility fractures, senile dementia, alcoholism and gastrectomy were associated with significant risk, whereas osteoarthrosis, nephrolithiasis and myocardial infarction were associated with lower risks. Taking medications was not associated with a difference in risk apart from a protective effect with the use of analgesics independent of co-existing osteoarthrosis and an increased risk with the use of anti-epileptic agents. Of the potentially ‘reversible’ risk factors, BMI, leisure exercise, exposure to sunlight and consumption of tea and alcohol and tobacco remained independent risk factors after multivariate analysis, accounting for 54% of hip fractures. Excluding BMI, 46% of fractures could be explained on the basis of the risk factors sought. Of the remaining factors low exposure to sunlight and decreased physical activity accounted for the highest attributable risks (14% and 9% respectively). The use of risk factors to predict hip fractures had relatively low sensitivity and specificity (59.6% and 61.0% respectively). We conclude that lifestyle factors are associated with significant differences in the risk of hip fracture. Potentially remediable factors including a low degree of physical exercise and a low BMI account for a large component of the total risk. more...

Dissertação de Mestrado em Turismo, Recursos Locais, Animação e Desenvolvimento. Este trabalho de investigação compreende seis capítulos que descrevem a investigação realizada. O primeiro capítulo desenvolve-se sobre o projecto de investigação, a perspectiva teórica e a metodologia; O segundo capítulo começa a debruçar-se sobre a geografia e a história geral de Cabo Verde, permitindo uma abordagem sobre algumas divergências de opinião entre os historiadores acerca do “descobrimento” de Cabo Verde e dos “descobridores”, tendo ficado substanciado o achamento de Cabo Verde, no ano de 1460, por navegadores portugueses. Alguns anos após este facto, iniciou-se o seu povoamento na ilha de Santiago. O percurso histórico leva-nos através dos séculos até chegarmos ao momento em que Cabo Verde se tornou independente, em 1975, depois da luta armada levada a cabo pelo PAIGC, tendo como dirigente Amílcar Cabral. O terceiro ponto deste trabalho descreve as diversas perspectivas sobre a antiga cidade da Ribeira Grande de Santiago, demonstrando o seu valor histórico e cultura, bem como o processo da socialização dos primeiros moradores e as suas primeiras actividades. Permitiu-se assim descobrir a antiga cidade como sendo um centro político, religioso e comercial, apontando para uma sociedade cristã e escravocráta, o que se contradiz nos seus próprios fundamentos. O capítulo seguinte debruça-se sobre o património histórico e cultural da localidade de Cidade Velha, correspondendo o quarto capítulo deste trabalho e aborda a descrição e o estado de conservação em que se encontram os monumentos, na medida que os patrimónios locais sofreram graves prejuízos devido os frequentes ataques dos corsários, o que obrigou à saída em massa da população da primeira capital de Cabo Verde para a vila da Praia (ainda na época) e, entre as saídas destacadas, também se encontravam grande parte dos responsáveis administrativos pelo que o resultado foi a negligência por parte da população dos monumentos históricos, que ficaram a mercê da população pouca escolarizada e pouca sensibilizada para a preservação do seu património histórico. O quinto capítulo deste trabalho procede com uma análise acerca do longo percurso do empenho pela patrimonialização da Cidade Velha e da sua denominação como Património da Humanidade até à atribuição do estatuto correspondente pela UNESCO. Ainda se fez o tratamento e exposição dos dados recolhidos realçando a opinião dos responsáveis, da população local e dos turistas acerca da patrimonialização da Cidade Velha. Este capítulo termina com um breve estudo sobre o museu e a sua função social e educacional. A última parte deste trabalho acentua o valor do turismo sustentável para a Cidade Velha, tendo em conta a escolha do turismo cultural como passo essencial ao alcance de um turismo de qualidade para aquela região. A escolha do ecomuseu como método de um desenvolvimento sustentável para que a localidade se tornasse um destino turístico onde a cultura e a história locais seriam promovidas, sem se prejudicarem os recursos locais existentes face aos objectivos específicos dos museus (educar, sensibilizar, estudar, conhecer e expor de uma forma cientifica a cultura local). A conclusão deste trabalho de investigação surge não somente como síntese de todo processo deste trabalho mas, acima de tudo, como um ponto de reflexão e encaminhamento, na medida que qualquer trabalho científico tem obrigação de demonstrar caminhos e permitir uma reflexão crítica a fim de reduzir os riscos dos investimentos. Os anos 90 foram, para os países em desenvolvimento, marcados pela busca de um caminho para a estabilização econômica, fazendo com que as empresas se adequassem à nova realidade. Com as empresas ligadas ao agronegócio não foi diferente. No Brasil, esse processo o foi mais intenso, principalmente após o advento do Plano Real (1994), quando as empresas tiveram de se adaptar a uma economia estabilizada, buscando novas fontes de receita e não aquelas advindas simplesmente do comércio dos produtos in natura a que elas estavam habituadas, mas sim agregando valor ao produto para se manter no mercado. Nesse contexto, aliado à abertura do mercado, o ambiente econômico ficou mais competitivo, obrigando as entidades produtivas e empresariais a aumentar sua exposição ao risco, reduzindo suas margens de lucro e aumentando sua eficiência como forma de se antecipar aos eventuais problemas ligados ao mercado ao qual ela estava inserida. Dentro dessa nova realidade, a gestão de risco tornou-se uma ferramenta imprescindível para as empresas, sejam elas ligadas á produção, comércio, indústria, instituições financeiras ou agronegócios. Nesse trabalho, objetivou conceituar os vários tipos de riscos inerentes ao setor agronegócio, bem como os instrumentos dos derivativos utilizados para mitigá-los ou até mesmo imunizá-los. Para ilustrar o uso dos derivativos, apresentaram-se algumas situações onde a utilização dessas ferramentas foi simples e pode proporcionar ao contratante as garantias de preços e retornos para seus produtos no mercado. ---------------------------------------------In the last decade, emerging market countries looked for their economic stabilization, forcing the companies to adapt themselves to the new reality. To agribusiness companies were not different. In being so, in Brazilian environment was more intense, mainly after “Plano Real” (1994), when the companies had to adjust themselves to a more stable economy, looking for new sources of revenues, not only those which came from in nature products, which they were used to, but adding value to product to keep in the market. In this context, along with the Brazilian market opening to the foreign banks, the environment became more competitive, forcing these entities to increase their risk exposure, reducing their spreads and increasing their efficiency in the way to anticipate to the problems linked to the market where they work. Considering this new reality, risk management becomes an essential tool to the companies, even though they are linked to commerce, industry, financial institutions or agribusiness. In this study, our objective was concentrated on defining the various types of risk inherent to agribusiness, making an approach to derivatives instruments used to temper or even eliminate them. The PTPN22 single nucleotide polymorphism (SNP) +1858 C/T (rs2476601) was found to be linked and associated with rheumatoid arthritis (RA).1 However, the frequency of the susceptibility +1858T allele range from 2% to 15% of south to north Europeans, and is not observed in an Asian population, suggesting the involvement of other disease-associated PTPN22 polymorphisms.2–4 A study performed in the type 1 diabetes (T1D) Japanese population led to the identification of the PTPN22 – 1123 G/C SNP (rs2488457), for which the –1123G allele was found to be associated with T1D.5 This allele in linkage disequilibrium with +1858T was also tested in a familial T1D UK Caucasian sample.
