The research relevance is due to the need for regional education systems to develop tools and assessment techniques that allow identifying professional deficits in Russian teachers. This article analyses the results of approbation diagnostics of professional competencies of teachers in the state information system “Electronic Education of the Republic of Tatarstan”. Diagnostic tools were developed by scientific and pedagogical workers of organisations of higher and additional professional education of the Republic in accordance with the provisions of the professional standard “Teacher”.Aim. The present research aims to analyse and summarise the results of the monitoring of professional deficits of teachers of the Republic of Tatarstan in the field of subject, methodological, psychological, pedagogical and regulatory competencies for further building individual development trajectories and targeted professional development programmes.Methodology and research methods. Teachers were tested in their personal accounts in the information system “Electronic Education of the Republic of Tatarstan”. Evaluation materials were divided into blocks that differ in content and purpose in accordance with the labour functions of the professional standard “Teacher”: legal, subject, methodological and psychological-pedagogical blocks. Each option consisted of 25 tasks. The maximum participant in the diagnostics could score 100 points. The interpretation of the level of professional deficits was carried out in accordance with the scale recommended by the Ministry of Education. The results of the diagnostic work were processed using visual and mathematical- statistical methods implemented within the tools and libraries of the Python programming language.Results and scientific novelty. The authors identified the target blocks corresponding to certain labour functions of a teacher. When performing the tasks from the identified target blocks, teachers experience the greatest and least professional difficulties. The authors identified control and measuring materials, which are associated to be the most difficult. The deficit level was analysed among subject teachers. The authors separately analysed the results of testing in the field of inclusiveness and individualisation of the educational process, the formation of digital competencies in the context of teachers of the humanities, natural sciences and primary school teachers. A correlation analysis of block diagnostic results was carried out. The influence of the territorial factor on the differentiation of test results was analysed. Difficulties, equally pronounced for both urban and rural teachers, in most cases are associated with the unwillingness of teachers to fully solve professional tasks in the formation of functional literacy of schoolchildren, the development of inclusive and personalised learning; with insufficient knowledge of the psychological and pedagogical features of the development of children of different ages to resolve conflict situations.Practical significance. The results obtained and the conclusions drawn can become the basis for the preparation of methodological recommendations in the field of organising the educational environment for advanced training courses, taking into account teachers’ urgent needs.

Modern capabilities of intelligent control systems are increasingly being used in areas previously considered the exclusive work of people — experts with relevant experience in a particular field. Machine learning capabilities in the field of electric power industry, obtaining forecasts based on the data of intelligent sensors of various purposes are not an exception. At present the Russian Government has adopted a program for the development of the manufacturing industry until the end of 2035: during this time manufacturing output should grow by 192 %. It is obvious that this program should also meet the requirements of the modern scientific concept of industrial revolution "Industry 4.0", when manufacturing enterprises and corporations begin to develop and apply subsystems and elements of "smart manufacturing", which help to build intelligent communications between individual tasks and operations during the entire life cycle of production, in accordance with the principles and methods of systems engineering. It is important to note that the issues of intelligent management in the subject-oriented area of electric power industry (in our case — energy saving), as the basis of any industrial production in modern conditions, require the development and implementation, first of all, of new solutions based on modern IT-technologies. It is known that energy intensity in Russia, according to the World Bank, is 3-4 times lower than in European countries. It is also known that in connection with the new provisions in the field of housing and communal services, aimed at improving economic efficiency in terms of electricity consumption, it becomes very important to ensure its accurate and operational accounting with the possibility of further forecasting of electricity consumption and the state of power grid facilities, which will allow specialized organizations and services, as well as the managing bodies in the shortest time to make a balanced specialized decisions This paper proposes the concept of intelligent control system to manage the process of condition monitoring based on data from intelligent sensors. The novelty of the concept is to consider a variant of solving the problem of integration of information systems associated with weakly structured subject-oriented information flows in the electric power industry enterprise by using methods of set theory and category theory.

BackgroundNatalizumab is effective in the treatment of multiple sclerosis (MS). In 2021, the European Medicines Agency approved the subcutaneous (SC) variant of natalizumab which can be used instead of intravenous administration. However, the course of drug levels varies between administration routes, and the Food and Drug Administration rejected the request for approval of natalizumab SC for reasons that were not disclosed. Our objective was to evaluate the course of natalizumab trough drug levels in patients who switched from natalizumab intravenous to SC on various treatment intervals.MethodsThe NEXT-MS trial (N=382) investigates personalised treatment of natalizumab, in which infusion intervals are prolonged based on individual natalizumab trough drug levels. In 2021, an amendment was approved allowing participants to switch from intravenous to SC administration with frequent measurements of natalizumab drug levels and antidrug antibodies (ADAs). Results were compared with linear mixed model analyses.ResultsUntil December 2022, 15 participants switched to SC natalizumab. Natalizumab drug levels with SC administration were on average 55% lower compared with intravenous administration (Exp (estimate) 0.45, 95% CI 0.39 to 0.53, pConclusionsNatalizumab trough drug levels can decrease after switching from natalizumab intravenous to SC administration. We advise to monitor trough drug levels in patients with low natalizumab drug levels during intravenous treatment, patients with higher body mass index or patients on extended treatment intervals who switch to SC administration of natalizumab.

Candidiasis is an opportunistic infection caused by pathogens of the genus Candida. Due to the increase in antifungal resistance by Candida species, there is a need for alternatives in its treatment. Cnidoscolus aconitifolius is a medicinal plant with vast nutritional and antimicrobial properties. This study evaluated the in-vitro and in-vivo anti-candidal effect of Cnidoscolus aconitifolius (spinach tree) leaves extract. Clinical isolates of Candida albicans, Candida krusei, Candida tropicalis, Candida dublinensis, Candida glabrata, and Candida parapsilosis were used in this study and Cold maceration technique was employed in the plant extraction process, to obtain its ethanolic, methanolic, and aqueous extract. The agar well diffusion method and 2-fold dilution process -to obtain 250 mg/ml to 15.625mg/ml of the extracts -were used for the antimicrobial sensitivity test and three weeks old female Wistar rats infected with 20µl of standardized C. albicans were used for the in-vivo evaluation. Statistical analysis was done using Statistical package for Social Sciences (SPSS) version 22. In-vitro assay revealed ethanolic extract as the most potent extract with the highest inhibition zone diameter of 12.67±1.15c, 11.67±1.15c, and 12.33±0.58c at 250mg/ml against C. tropicalis, C. krusei2, and C. parapsilosis respectively. C. parapsilosis was the most sensitive to all the extracts. In-vivo, the disappearance of disease symptoms and progressive decrease in rats Candida burden- from 6.35×103 CFU/ml obtained after infection to 2.15×103CFU/ml after treatment revealed the ability of the plant to treat candidiasis. The study suggests that extract of Cnidoscolus aconitifolius leaves could act individually or synergistically as effective drug candidates in the treatment of candidiasis; thus if its individual antifungal constituents are purified, this plant could be the next major anti-candidal agent.

In most of the fields developed by the flooding method, there is an advanced flooding of borehole products associated with water breakthrough through the reservoir, backwater circulation, and also leakiness of the production column. In order to limit the extraction of "excess" water, water insulation technologies are widely used, in which hydrogels of various nature occupy a prominent place. For the rational use of water insulation technologies, it is necessary to know the rheology features and filtration characteristics of grouting materials. This article presents the results of experimental determination of a number of rheological characteristics of a composite insulating material based on sodium silicate, polyacrylamide (PAA), chromium acetate and rice husk dispersion. Rheological measurements were carried out in the oscillatory mode, which provide information about the elastic and viscous characteristics of the sample under study. The elastic modulus (accumulation modulus) characterizes the accumulated deformation energy in the system and reflects the property of the sample as a solid (elastic component). The viscous modulus (loss modulus) determines the energy dissipation and is responsible for the behavior of the sample as a liquid (viscous component). In each case, the range of linear viscoelastic behavior was determined as an interval of strain values in which the structure of the sample under study is not destroyed by deformation. Hydrodynamic modeling is increasingly used in the design of the RIW, which allows not only to reduce the time to justify the processing parameters, but also to increase its efficiency. For the convenience of subsequent hydrodynamic modeling, the results were processed using a two-component Burgers model to obtain the parameters of the elements of this model, which are a kind of «digital passport» of waterproofing materials.

The article is devoted to the study of the current state of Russia’s foreign language policy in the near abroad. The authors define Russia’s language policy abroad, identify its internal and external factors, analyze its effectiveness, and determine the specifics and peculiarities of its development in the post-Soviet space and its influence on the quality and effectiveness of integration processes in the Eurasian region. In the context of the analysis of Russian language policy in the near abroad, the authors consider the problems of preservation and promotion of the Russian language in the international space, the main among which are the following: the reduction of the geography of use of the Russian language and changes in its status in the near abroad and far abroad countries. In the concluding part of the article, the authors propose ways and solutions to strengthen the effectiveness of Russia’s language policy in both global and regional dimensions.

The atmospheric circulation response to global warming is an important problem that is theoretically still not well understood. This is a particular issue since climate model simulations provide uncertain, and at times contradictory, projections of future climate. In particular, it is still unclear how a warmer and moister atmosphere will affect midlatitude eddies and their associated poleward transport of heat and moisture. Here we perform a trend analysis of three main components of the global circulation—the zonal-mean state, eddies, and the net energy input into the atmosphere—and examine how they relate in terms of a moist static energy budget for the JRA-55 reanalysis data. A particular emphasis is made on understanding the contribution of moisture to circulation trends. The observed trends are very different between the hemispheres. In the Southern Hemisphere there is an overall strengthening and during boreal summer, also a poleward shifting, of the jet stream, the eddies, and the meridional diabatic heating gradients. Correspondingly, we find an overall strengthening of the meridional gradients of the net atmospheric energy input. In the Northern Hemisphere, the trend patterns are more complex, with the dominant signal being a clear boreal winter Arctic amplification of positive trends in lower-tropospheric temperature and moisture, as well as a significant weakening of both bandpass and low-pass eddy heat and moisture fluxes. Consistently, surface latent and sensible heat fluxes, upward and downward longwave radiation, and longwave cloud radiative fluxes at high latitudes show significant trends. However, radiative fluxes and eddy fluxes are inconsistent, suggesting data assimilation procedures need to be improved. Significance Statement We use a long-term reanalysis dataset to get an overall view of the changes in the global circulation and its role in transporting moist static energy from the equator to the poles. We do this by examining the trends in its three main components—the zonal means, the eddies, and the net energy input into the atmosphere. We find that in the Southern Hemisphere, there is an overall strengthening of the eddies, their poleward energy fluxes, and correspondingly the meridional gradients of the net atmospheric energy input. In the Northern Hemisphere, though the pattern is more complex, there is an overall weakening of the eddies and poleward eddy fluxes, and of the meridional gradients of the net atmospheric energy input, consistent with Arctic warming.

Background: Antimicrobial stewardship programmes are interventions which have been developed to address irrational and inappropriate use particularly in health care centers. Antimicrobial stewardship programmes involve a set of activities which promote appropriate use of antimicrobials in terms of selection, dosing, route and duration of antimicrobial therapy. Methods: A Point-prevalence survey (PPS) was organized in line with Global point prevalence patient-based protocol for PPS and antimicrobial use was carried out in the paediatrics department of a tertiary healthcare institution in southern Nigeria. A total of 66 inpatients on admission for at least 24hours and still on admission at 8am on the day of the survey were included in the study. Results: The results show that 34 (51.50%) patients were on at least one antimicrobial drug and this included 19 (55.80%) males and 15 (44.20%) females. The highest proportion (n=15; 44%) of these patients were admitted in the children emergency ward while the least number (n=5; 14.7%) were found in children medical ward 1. The average age of patients on antimicrobials was 53 months. Thirty-four (51.50%) patients were on at least one antimicrobial drug and this included 19 (55.8%) males and 15 (44.2%) females. The three most prescribed antimicrobials were, ceftriaxone (23.88%), aminoglycoside gentamicin (20.90%), cefuroxime (16.42%) and Ceftazidime was the least common antibiotic prescribed (1.49%). Only 10.45 % were targeted therapy (based on microbiological results) while there was missing information for 10.45% of prescriptions. Conclusion: The findings of the study shows that it is important to initiate antimicrobial stewardship programmes within our hospital to optimize antimicrobial use to improve patient care in the hospital.

SUMMARY For long time the study of the Moho discontinuity (or Moho) has been a crucial topic in inferring the dynamics of the Earth's interior, and with profitable result it is mapped by seismic data, but due to the heterogeneous distribution of such data the quality varies over the world. Nevertheless, with the advent of satellite gravity missions, it is today possible to recover the Moho constituents (i.e. Moho depth; MD and Moho density contrast; MDC) via gravity observations based on isostatic models. Prior to using gravity observations for this application it must be stripped due to the gravitational contributions of known anomalous crustal density structures, mainly density variations of oceans, glacial ice sheets and sediment basins (i.e. stripping gravity corrections). In addition, the gravity signals related mainly with masses below the crust must also be removed. The main purpose of this study is to estimate the significance of removing also remaining non-isostatic effects (RNIEs) on gravity, that is, gravity effects that remain after the stripping corrections. This is carried out by using CRUST19 seismic crustal model and employing Vening Meinesz–Moritz (VMM) gravimetric-isostatic model in recovering the Moho constituents on a global scale to a resolution of 1° × 1°. To reach this goal, we present a new model, named MHUU22, formed by the SGGUGM2 gravitational field, Earth2014 topography, CRUST1.0 and CRUST19 seismic crustal models. Particularly, this study has its main emphasis on the RNIEs on gravity and Moho constituents to find out if we can modify the stripping gravity corrections by a specific correction of the RNIEs. The numerical results illustrate that the RMS differences between MHUU22 MD and the seismic model CRUST1.0 and least-squares combined model MOHV21 are reduced by 33 and 41 per cent by applying the NIEs, and the RMS differences between MHUU22 MDC and the seismic model CRUST1.0 and least-squares combined model MDC21 are reduced by 41 and 23 per cent when the above strategy for removing the RNIEs is applied. Hence, our study demonstrates that the specific correction for the RNIEs on gravity disturbance is significant, resulting in remarkable improvements in MHUU22, which more clearly visualize several crustal structures.

Aluminum alloys alloyed with rare earth and transition metal are promising materials for electric energy transportation due to their high properties of strength, thermal stability, and electrical conductivity. The features of strengthening, their mechanical properties and electrical conductivity of Al–0.2Y–0.2Sc–0.3Er alloy after cold rolling have been established. The alloy as a cast structure is presented by aluminum solid solution (Al) and dispersed eutectics with τ2 (Al75-76Er11-17Y7-14) phase upon complete dissolution of scandium in (Al), and a content of yttrium and erbium at the level of 0.2–0.3 % each. Cold rolling the ingot accelerates strengthening upon annealing at 270 and 300 °C, reducing the time of achieving peak hardness. The maximum strengthening due to precipitation of L12 dispersoid of Al3(Sc,Y,Er) phase with the average particle size up to 10 nm is achieved after 7 h of annealing at 300 °C after cold rolling. This shows the prevailing heterogeneous mechanism of nucleation due to defects accumulated during cold rolling which stimulates strengthening. The eutectic particles are located mainly along the boundaries, elongated in the rolling direction. Irrespective of the mode of sheet fabrication, the alloy demonstrates high thermal stability up to 400 °C. During annealing of the sheets to 450 °C, their non-recrystallized structure is retained. Ingot annealing at t = 300 °C in 7 h and cold rolling with subsequent annealing under the same conditions provide a high level of mechanical properties and electrical conductivity: σ0.2 = 194 MPa, σu = 210 MPa, δ = 12.1 % and IACS – 60,1 %. The alloy has demonstrated high yield stress up to 100 h of annealing at t = 300 °C.

Aim: comparative analysis of the expression profile of plasma microRNAs and target genes in patients with complicated pregnancy.Materials and Methods. A prospective observational comparative study in parallel groups was carried out. The study included 73 women divided into three groups: the main group – 42 patients with preeclampsia (PE), the comparison group – 12 pregnant women with fetal growth retardation (FGR), the control group – 19 clinically healthy women with uncomplicated pregnancy. An examination was performed, which included the analysis of clinical characteristics and the study of microRNA expression in blood plasma using the real-time polymerase chain reaction.Results. MicroRNA hsa-miR-210-5p and hsa-miR-1972 were not identified in any plasma sample. Analyzing plasma microRNAs in group of women with PE showed significant changes in the expression levels of hsa-miR-517a-3p (p = 0.025), hsa-miR-517c-3p (p = 0.036), hsa-miR-574-5p (p = 0.015), hsa-miR-517a-3p (p < 0.001) and an increase in miR-20a-5p (p = 0.046) compared to control group. No significant differences were found in the miRNA expression profile in group of women with FGR compared to control group. Assessing an influence of the studied microRNAs on regulatory signaling pathways allowed to establish that hsa-miR-miR-146a-5p, -181a-5p, -210-3p, -517a-3p, -517c-3p, -574-3p, -574-5p, -1304-5p are potential regulators of the reaction cascades involved in the PE pathogenesis.Conclusion. The changes revealed in the circulating blood plasma miRNA level indicate the presence of specific transcriptomic alterations during complicated course of pregnancy.

Introduction. The development of osteoporosis (OP) increases healthcare costs and often leads to disability of patients. In this regard, the search for ways to improve the effectiveness of OP treatment is very relevant. Today, there is a wide range of drugs for the effective correction of bone metabolism. However, low patient compliance in real clinical practice significantly reduces the effectiveness of therapy.Aim. To study the effect of patient compliance on the effectiveness of Zoledronic acid and Denosumab in OP in real clinical practice.Materials and methods. Study design: a retrospective analysis of outpatient records of 300 patients with OP, who were prescribed Zoledronic acid or Denosumab in 2019, with a prospective analysis of adherence to therapy for 3 years.Results. It was revealed that 12% of patients did not start pathogenetic therapy for OP (control group). 88% (264 patients) started pathogenetic therapy: of these, 22.33% (67 patients) preferred therapy with Denosumab; 65.67% (197 patients) – Zoledronic acid. After 1 year, therapy with Denosumab 19.4%, Zoledronic acid – 19.29% was discontinued. More than 1 month late with the next injection of the drug: Denosumab – 25.37%, Zoledronic acid – 16.24% of patients. Only 55.22% who received Denosumab and 64.47% who received Zoledronic acid fully complied with the recommendations. Most often, a violation of the schedule of drug administration was observed in patients over 75 years of age, alone, with impaired cognitive status. Discontinuation of therapy with Denosumab or violation of the schedule of its administration led to an increase in the level of bone resorption (C-telopeptide type I collagen (CTX-1)). During therapy with Zoledronic acid, there was no increase in CTX-1. In addition, the cost of course treatment with Zoledronic acid is 2–3 times less than with Denosumab.Conclusion. In real clinical practice, zoledronic acid has clinical and pharmacoeconomic advantages, especially in patients with expected low adherence to OP therapy.

Aortic dissection is a type of acute aortic syndrome characterized by a high mortality rate in the first 48 hours. The problem of early diagnosis of acute aortic dissection is still relevant and due to a high risk of severe complications, as well as a non-specific clinical features. If left untreated, aortic dissection can be complicated by the formation of an aortic aneurysm and its rupture. Sometimes, the rupture of an aortic aneurysm is accompanied by the fistula formation. The article discusses the use of computed tomography (CT) scans for visualization of DeBakey type III aortic dissection complicated by aortic rupture and the formation of an aorto-esophageal fistula.A 60 y.o. male diagnosed with aortic dissection was urgently transferred from the city hospital for surgical treatment. As part of the preoperative preparation, the patient underwent computed tomography angiography (CT angiography), that demonstrated not only an acute DeBakey type III dissection with a thoracic aortic aneurysm of the descending aorta, but also its complication in the form of aortic rupture with the formation of aorto-esophageal fistula.The patient underwent a successful endovascular thoracic aortic stent grafting with unmarkable postoperative period. Follow-up CT angiography showed complete isolation of the false lumen and closure of the aorto-esophageal fistula. The patient showed clinical improvement and was discharged home on 14 day.

Introduction: The discharge of produced water into the aquatic environment presents a risk to the environment. This is a form of pollution and may release toxicants that are highly noxious to sensitive marine species even at low concentrations, which causes bio-degeneration/transformation and biodiversity loss. Research Gap: There are insufficient literatures on extensive monitoring of the physicochemical and microbial characteristics of produced water. Existing literatures concerning analysis of treated and untreated produced water are not comprehensive with respect to number of physicochemical variables analyzed. Aim: To ascertain if the physicochemical properties and microbial population of produced water were within acceptable regulatoy specifications. Place and Duration of Study: The study was conducted in the Federal University of Petroleum Resources, Effurun, Delta State, Nigeria from July, 2021 – April, 2022. Methodology: Electrical conductivity, dissolved oxygen, pH, temperature, turbidity, total petroleum hydrocarbons, nitrates, phosphates, sulphates and others were assessed using standard methods. Microbial counts were carried out for total heterotrophic bacteria and hydrocarbon utilizing bacteria using the pour plate method. Results: Values obtained for some key physicochemical parameters monitored in the produced water are as follow: pH (8.10±0.03 for untreated and 8.27±0.01 for treated); electrical conductivity (35700±11 µs/cm for untreated and 41600±17 µs/cm for treated); total dissolved solids (22848±14mg/l for untreated and 26629±9mg/l for treated); dissolved oxygen (2.05±0.01mg/l for untreated and 4.23±0.03mg/l for treated); biochemical oxygen demand (28.90±0.7mg/l for untreated and 18.4±0.1mg/l for treated); phosphate (2.05±0.01mg/l for untreated and 4.23±0.03mg/l for treated); nitrate (54.82±1.9mg/l for untreated and 50.21±0.9mg/l for treated); total hydrocarbon content (118.00±0.00mg/l for untreated and 34.00±0.00mg/l); total heterotrophic bacteria (4.1e+04 CFU/ml for untreated and 2.8e+04 CFU/ml for treated); and hydrocarbon utilizing bacteria (2.44e+03 CFU/ml for untreated and 1.58e+03 CFU/ml for treated). Statistical analysis of the produced water showed varying forms of correlation between physicochemical parameters of untreated and treated produced water. Conclusion: It is important to assess properties of produced water before disposal into aquatic environment as chronic impact associated with long-term exposures may pose potential ecological risks.

Cardiovascular syphilis has a variety of localizations and signs including an aortic aneurysm. Syphilitic aneurysms are most commonly localized in the ascending aorta, less in the aortic arch and descending aorta. Multiple aneurysms of syphilitic etiology are rare. A clinical case of a 56-year-old patient with multiple aneurysms of the aorta and brachiocephalic trunk of syphilitic etiology has been reported. Computed tomography showed multiple aneurysms of various localization, rupture of the ascending aortic aneurysm with partial thrombosis, its localization and the formation of an aortopulmonary anastomosis. The data obtained made it possible to carry out surgical treatment.

The process of formation and organization of the activities of fire brigades in the cities of the Yenisei province in the second half of the 19th — early 20th centuries is studied. The time frame for their creation, funding, as well as the level of training of firefighters, living conditions and service have been specified. It is shown that Krasnoyarsk organized fire fighting, focusing on Moscow and St. Petersburg, and county towns, in turn, followed the example and under the supervision of the provincial city. It is shown that due to the fact that cities arose here as administrative centers, and not as commercial and industrial centers, they were distinguished by such features as low population, the peasant way of life of the bulk of the townspeople, budget deficits, and underdevelopment of the urban economy. It is noted that city governments managed to create permanent fire brigades only by the end of the 19th century. It is alleged that the lack of funding determined their poor technical equipment, low pay and unfavorable working conditions for firefighters. It is emphasized that there was no professional training system in the region, specialists from European Russia traveled to Siberia extremely reluctantly. It is concluded that the First World War significantly exacerbated all existing problems.

An algorithm of the stage-by-stage synthesis of combined energy sources based on a high-voltage electrochemical explosion (HVECHE) was developed. The algorithm provides the necessary space-time force and energy impact on processing objects to perform specified technological operations. At the first stage, the synthesis of an energy source based on a high-voltage electric discharge in condensed media was done by means of solving inverse synthesis problems and, as a result, determining the required total energy with its subsequent optimal separation into electrical and chemical components. Experimental studies of the dependence functions of the force impact on the processing object on the energy characteristics of the HVECHE were carried out. Based on those studies, the criteria for the optimal proportion between the electrical and chemical components of the required total energy were determined. An example of a specific use of the developed algorithm for the synthesis of a combined energy source based on the HVECHE was given, which confirms the possibility to use this algorithm for engineering calculations.

Acute mesenteric ischemia (AMI) is a rare vascular disease characterized by a high mortality rate with untimely diagnosis and treatment. The overall incidence is low, estimated at 0.09–0.2 % of all hospital admissions to emergency departments. One of the causes of acute mesenteric ischemia is acute arterial thrombosis, which is most common in the elderly.The problem of early diagnosis of acute mesenteric thrombosis is still relevant due to a non-specific clinical features. Computed tomography angiography (CT angiography) of the abdominal aorta and its branches is characterized by high sensitivity (85–98 %) and specificity (91–100 %). Computed tomography angiography is the gold standard for the diagnosis of acute mesenteric ischemia.The article discusses the use of computed tomography angiography (CT angiography) for visualization of acute mesenteric thrombosis.

The adaptation of wildlife species to urban environments can drive changes in the ecology of ectoparasites and vector-borne disease. To better understand ectoparasite dynamics in an urban environment, we investigated the ectoparasite communities of 183 sylvatic and urban opossums and raccoons captured across four seasons at a rural research station and within the city of Gainesville, FL, and of 115 community cats from the Gainesville, FL area. Amblyomma americanum (L.) (Acari: Ixodidae), Dermacentor variabilis (Say), and Ixodes texanus Banks were collected from raccoons, A. americanum, D. variabilis, and Ixodes scapularis Say from opossums, and A. americanum from cats. Few ticks were collected from urban animals, although species richness of ectoparasites was similar between urban and sylvatic habitats. Ctenocephalides felis (Bouché) (Siphonaptera: Pulicidae) was collected from all sampled host species, but was particularly abundant on opossums. Additionally, Orchopeas howardi (Baker) (Siphonaptera: Ceratophyllidae) was collected from raccoons, and O. howardi and Xenopsylla cheopis (Rothschild) (Siphonaptera: Pulicidae) from opossums. Only raccoons were infested with raccoon lice, and only cats were infested with cat lice. Primarily opossums were infested with mites. Ectoparasite community composition varied by habitat, host species, and season; seasonal variation in ectoparasite communities differed between the sylvatic and urban habitats. While urban mesomammals did not appear to play an important role in supporting tick populations in an urban habitat, urban opossums appear to serve as an alternate host for large numbers of cat fleas, which may be an important consideration for treatment and control efforts against ectoparasites of companion animals.

BACKGROUND: The use of molecular amplification as-says for TB diagnosis is limited by their costs and cartridge stocks. Pooling multiple samples to test them together is reported to have similar accuracy to individual testing and to save costs. METHODS: Two surveys of individuals with presumptive TB were conducted to assess the performance of pooled testing using Xpert® MTB/RIF (MTB/RIF) and Xpert® Ultra (Ultra). RESULTS: A total of 500 individuals were tested using MTB/RIF, with 72 (14.4%) being MTB-positive. The samples were tested in 125 pools, with 50 pools having 1 MTB-positive and 75 only MTB-negative samples: 46/50 (92%, 95% CI 80.8–97.8) MTB-positive pools tested MTB-positive and 71/75 (94.7%, 95% CI 86.9–98.5) MTB-negative pools tested MTB-negative in the pooled test (agreement: 93.6%, κ = 0.867). Five hundred additional samples were tested using Ultra, with 60 (12%) being MTB-positive. Samples were tested in 125 pools, with 42 having 1 MTB-positive and 83 only MTB-negative samples: 35/42 (83.6%, 95% CI 68.6–93.0) MTB-positive pools tested MTB-positive and 82/83 (98.8%, 95% CI 93.5–100.0) MTB-negative pools tested MTB-negative in the pooled test (agreement: 93.6%, κ = 0.851; P > 0.1 between individual and pooled testing). Pooled testing saved 35% (MTB/RIF) and 46% (Ultra) of cartridges. CONCLUSIONS: Pooled and individual testing has a high level of agreement and improves testing efficiency.

Background The timing at which venous thromboembolism (VTE) occurs after major surgery has major implications for the optimal duration of thromboprophylaxis. The aim of this study was to perform a systematic review and meta-analysis of the timing of postoperative VTE up to 4 weeks after surgery. Methods A systematic search of MEDLINE, Scopus, and CINAHL databases was performed between 1 January 2009 and 1 April 2022. Prospective studies that recruited patients who underwent a surgical procedure and reported at least 20 symptomatic, postoperative VTE events by time were included. Two reviewers independently selected studies according to the eligibility criteria, extracted data, and evaluated risk of bias. Data were analysed with a Poisson regression model, and the GRADE approach was used to rate the certainty of evidence. Results Some 6258 studies were evaluated, of which 22 (11 general, 5 urological, 4 mixed, and 2 orthopaedic postoperative surgical populations; total 1 864 875 patients and 24 927 VTE events) were eligible. Pooled evidence of moderate certainty showed that 47.1 per cent of the VTE events occurred during the first, 26.9 per cent during the second, 15.8 per cent during the third, and 10.1 per cent during the fourth week after surgery. The timing of VTE was consistent between individual studies. Conclusion Although nearly half of symptomatic VTE events in first 4 weeks occur during the first postoperative week, a substantial number of events occur several weeks after surgery. These data will inform clinicians and guideline developers about the duration of postoperative thromboprophylaxis.

Sleep is an important and complex physiological process that is necessary for the normal functioning of any organism. Sleep disorders diagnostics is an issue of interest in patients with acute ischemic stroke management. Currently these disorders are considered not only as a consequence of earlier stroke but as an acute cerebrovascular disease risk factor itself. Sleep disorders in patients with acute cerebrovascular disease can be presented in different types, including: insomnia, sleep apnea, central disorders of hypersomnolence, circadian rhythm sleep-wake disorders, parasomnias, sleep related movement disorders, unspecified sleep related disorders.There are very few publications on the subject of sleep disorders associated with acute ischemic stroke, despite the fact that imaging of each of these conditions separately occupies a significant place in radiology.Polysomnography is considered to be the gold standard in sleep disorders diagnostics. Contrary to that, the precise imaging of acute cerebrovascular accidents requires high-technology modalities, such as computed tomography (CT) and magnetic resonance imaging (MRI).The article represents a current literature review regarding radiologic methods in diagnostics of sleep disorders in patients with acute ischemic stroke anamnesis.

Objective: To compare the effects of atorvastatin monotherapy and the combination of atorvastatin with curcetin (a mixture of the bioflavonoids curcumin and quercetin) on lipid profile and inflammatory biomarkers in patients with unstable angina after COVID-19 (“Long COVID”).Material. An open simple comparative randomized study was conducted in 186 patients with unstable angina, including 77 (Group I) in whom angina destabilization occurred as a result of COVID-19 during 4-8 weeks prior to inclusion in the study, and 109 patients (Group II) in whom destabilization was not associated with infection.Results: In group I, the level of hsC-reactive protein [5,4 (2,06-7,4) g/l and IL-6 8,6 (5,4-10,3) pg/ml] was higher (P < 0,05) than in group II patients [3,8 (1,2-4,0) g/l and 6,9 (2,2-10,2) pg/ml], respectively. In subgroup I of patients after COVID-19, atorvastatin monotherapy (n = 43) did not have a significant effect after two months of treatment, while in subgroup II the combined use of atorvastatin with curcetin (n = 34) for 2 months reduced the level of hsCRP by 49,0% (P < 0,05) and Il-6 by 40,0% (P < 0,05).Conclusion. In patients with unstable angina after COVID-19, combination treatment with atorvastatin and curcetin reduced concentrations of inflammatory biomarkers compared with atorvastatin monotherapy.

Obesity is a risk factor for heart failure with preserved ejection fraction (HFpEF). We hypothesized that assessment of left atrial (LA) strain may be useful to reveal precursors of HFpEF in obesity patients. Echocardiograms of obesity patients without known cardiovascular disease who underwent bariatric surgery, and echocardiograms of age- and gender matched controls were analyzed. The echocardiogram was repeated 1 year after bariatric surgery. In breeding programs, the identification of genetic variability and knowledge of genetic constitutions existing within segregating populations are of fundamental importance for the success and continuation of the program. The present study aimed to quantify the genetic diversity among progenies of a sour passion fruit population based on morphoagronomic variables of flowers and fruits using the Ward-MLM (modified location model) procedure, which was efficient in quantifying genetic diversity and allowed the identification and consistent formation of three groups within the population. Among the groups formed, group I presented the highest means for the variables related to flowers, group II presented the highest values for the variables related to yield, and group III presented the highest means for the variables related to fruit quality. The androgynous length, nectariferous chamber length, anther surface, nectareous chamber length, and stigma surface were the variables that most contributed to the divergence between the evaluated genotypes. Crosses between groups II and III genotypes are promising for the next cycle, contributing to increased production and improvement of the physical and chemical quality of fruits. RESUMO Nos programas de melhoramento, a identificação da variabilidade genética e o conhecimento das constituições genéticas existentes nas populações segregantes são de fundamental importância para o sucesso e continuação do programa. Este trabalho teve como objetivo quantificar a diversidade genética entre progênies de uma população de maracujazeiro azedo baseado em variáveis morfoagronômicas de flor e frutos utilizando o procedimento multivariado Ward-MLM (modified location model), que foi eficiente na quantificação da diversidade genética e permitiu a identificação e formação consistente de três grupos dentro da população. Dentre os grupos formados, o grupo I apresentou maiores médias para as variáveis relacionadas a flor, grupo II apresentou os maiores valores para as variáveis relacionadas a produtividade e o grupo III apresentou as maiores médias para as variáveis relacionadas a qualidade dos frutos. O comprimento do androginóforo, comprimento da câmara nectarífera, a superfície das anteras, o comprimento da câmara nectarífera e a superfície do estigma foram as variáveis que mais contribuíram para a divergência entre os genótipos avaliados. Cruzamentos entre os genótipos do grupo II e III são promissores para o próximo ciclo, contribuindo para o aumento da produção, melhoria da qualidade física e química dos frutos.

Purpose Formative assessments can help motivate students and ease students’ learning through feedback. There is a pressing need for improvement of clinical pharmacotherapy (CPT) education since junior doctors make many prescribing errors. The aim of this study was to determine whether a formative assessment with personalized narrative feedback helps medical students to increase their prescribing skills. Methods This retrospective cohort study was conducted among masters’ medical students at Erasmus Medical Centre, The Netherlands. Students made a formative and a summative skill-based prescription assessment, both during clerkships as part of their regular curriculum. Errors in both assessments were categorized by type and possible consequence and compared with each other. Results A total of 388 students made 1964 errors in the formative assessment and 1016 in the summative assessment. Most improvements after the formative assessment were seen for mentioning the weight of a child on the prescription (n = 242, 19%). Most new and repeated errors in the summative assessment were missing usage instructions (n = 82, 16% and n = 121, 41%). Conclusions This formative assessment with personalized and individual narrative feedback has helped students to increase the technical correctness of their prescriptions. However, errors repeated after the feedback were predominantly errors showing that only one formative assessment has not yet enhanced the clinical prescribing enough.

Objective: the development and verification of a scale for identifying patients at high risk of osteoarthritis (OA).Material and methods. The results of the analysis of a sample of patients aged 35–90 years from the database of the Institute of Trace Elements (n=3440), which included information on clinical and anamnestic, diagnostic parameters and pharmacotherapy of patients, including patients with OA (n=107), are presented. To analyze information about patients, modern methods of data analysis proposed within the topological theory of pattern recognition were used.Results. Based on the analysis of the sample, a 100-point scale of 27 points was developed which allows to identify patients at high risk of OA (sensitivity 88%, specificity 100%). For patients with high scores on the scale, a kind of “preemptive” prescription of symptomatic slowacting drugs for osteoarthritis (SySADOA) based on highly purified substances of chondroitin sulfate (CS) and glucosamine sulfate (GS) is promising. The evidence for CS/GS is extensive and has been reviewed previously.Conclusion. Early adoption of preventive measures (including the usage of SySADOA – CS/GS) in patients with a high score on the developed scale may reduce the risk of OA.
