The anonymized dataset described here is provided in both raw and processed formats to facilitate re-use and further analyses. Abstract

Rationale & Objective: It is unclear whether the effect of canagliflozin on adverse kidney and cardiovascular events in those with diabetic kidney disease varies by age and sex. We assessed the effects of canagliflozin among age group categories and between sexes in the Canagliflozin and Renal Endpoints in Diabetes with Established Nephropathy Clinical Evaluation (CREDENCE) study.Study Design: Secondary analysis of a randomized controlled trial.Setting & Participants: Participants in the CREDENCE trial.Intervention: Participants were randomly assigned to receive canagliflozin 100 mg/d or placebo.Outcomes: Primary composite outcome of kidney failure, doubling of serum creatinine concentration, or death due to kidney or cardiovascular disease. Prespecified secondary and safety outcomes were also analyzed. Outcomes were evaluated by age at baseline (Results: The mean age of the cohort was 63.0 ± 9.2 years, and 34% were female. Older age and female sex were independently associated with a lower risk of the composite of adverse kidney outcomes. There was no evidence that the effect of canagliflozin on the primary outcome (a composite of kidney failure, a doubling of serum creatinine concentration, or death from kidney or cardiovascular causes) differed between age groups (HRs, 0.67 [95% CI, 0.52-0.87], 0.63 [0.48-0.82], and 0.89 [0.61-1.29] for ages Limitations: This was a post hoc analysis with multiple comparisons.Conclusions: Canagliflozin consistently reduced the relative risk of kidney events in people with diabetic kidney disease in both sexes and across age subgroups. As a result of greater background risk, the absolute reduction in adverse kidney outcomes was greater in younger participants.Funding: This post hoc analysis of the CREDENCE trial was not funded. Abstract: In recent years, the green synthesis of different metal nanoparticles has become a substantial technique for the synthesis of different essential nanoparticles and their potential applications in technological, industrial along with biomedical fields. Among the several essential nanoparticles, copper nanoparticles (CuNPs) have attracted enormous attention for their wide range of applications like the production of gas sensors, solar cells, high-temperature superconductors as well as drug delivery materials and catalysis owing to its distinctive optical, electrical, dielectric, imaging and catalytic, etc. properties. Herein, in this review, our aim is to find out the recent progress of synthesis, as well as different optical and structural characterizations of green, synthesized CuNPs along with their broadspectrum biomedical applications, mainly antibacterial, antifungal, antiviral and anticancer as well as the future perspective of research trends in the green synthesis of CuNPs. CuNPs have been synthesized by different researchers using three methods, namely, physical, chemical, and biological. In this review, the eco-friendly, efficient and low cost different established biological/green synthesis methods of CuNPs using different plant extracts like leaves, flowers, fruits, seeds, latex, etc., as capping and reducing agents have been briefly discussed, along with reaction conditions together with their optical as well as structural analysis. Effects of different parameters on the green synthesis of CuNPs like the presence of phytochemicals and confirmation of phytochemicals, temperature, pH, etc., are elucidated. Studies of the antibacterial activity of biomolecules capped CuNPs by different researchers against both Gram-positive and Gram-negative bacterial strains along with minimum inhibitory concentration (MIC) values have been summarized. Furthermore, antifungal and antiviral effects of green synthesized CuNPs studied by different researchers are mentioned with minimum inhibitory concentration (MIC) values. The anticancer activity of green synthesized CuNPs against different cancer cells studied by different researchers is summarized with correlation sizes of CuNPs on anticancer activity. The review also focuses on in vivo applications of green synthesized CuNPs along with clinical trails. Furthermore, an emphasis is given to the effectiveness of CuNPs in combating COVID-19.

Abstract

Thalassemia is the world’s commonest monogenic disorder. Molecular biology and genetics have revealed more than 200 mutations of thalassemia syndrome. These children become symptomatic from progressive haemolytic anaemia. They have to take lifelong treatments with regular packed red blood cell transfusion. Our body has no effective means of iron excretion and this leads to progressive iron accumulation in various organs after prolonged blood transfusion. This results in dysfunction of liver, heart and endocrine system and increased mortality. Iron toxicity to pancreatic beta cell leads to impairment in glucose metabolism. : Our study aims to find out the incidence of impaired glucose tolerance (IGT) and diabetes mellitus (DM) in transfusion dependent thalassemia major patients. This is an observational descriptive study carried out from January 2017 to June 2018. The study included 48 samples. There post-prandial blood sugar level and glycated haemoglobin (HbA1c) was analysed. Any proved case of juvenile diabetes mellitus was excluded from the study. We found that the incidence of IGT and DM were 12.5% and 4.16% respectively in our study population. The blood sugar level was significantly high in study group (p=Thalassemia patients receiving regular blood transfusion are at increased risk of developing IGT and DM. Early initiation of chelation and judicious use of blood transfusion must be considered for these patients to prevent such complications.

Abstract

The challenges of new rural technological possibilities in the 21st century resonate with agriculture. These represent, in particularly the improving availability of ICT infrastructure and the digitisation of socioeconomic processes. This paper focuses on exploring issues related to the potential change in the business behaviour of agricultural holdings and farmers following digitization and the use of ICT. The empirical research was carried out in the territory of the Local Action Group (LAS) Železnohorský region, which is a typical inner rural periphery with a strong agricultural base. The paper presents both the results of an exploratory survey and also develops a specific empirical model according to the improved Cobb-Douglas production function with specifics of the agrarian sector (i.e., including the role of agricultural subsidies and also the application of advanced ICT). The results in the selected rural micro-region showed that the process of digitisation has been very marginal in the models of functioning of the relevant agricultural entities. The current prevailing “techno-optimist approach” in EU strategic planning needs to be overcome by more intensive and comprehensive support for the digitisation process in rural peripheral areas. Particularly, this involves a necessary combination of support for investment in ICT infrastructure and new technologies with more intensive support for digital literacy and knowledge empowerment, which will enable more intensive exploitation of the potential and possibilities of digital technologies and virtual environments.

18. Environmental noise annoyance trends In selected locations In Bratislava

Abstract

The rapid development in traffic density and the economic transformation since 1989 has brought new problems concerning road traffic noise in Slovakia. The aim is to follow the time trends of noise annoyance in monitored localities in Bratislava at time intervals of 10, 20, and 30 years. We used the validated methodology for subjective assessment of noise annoyance in young and healthy individuals, as well as a method of objectification by direct measurement of sound levels. Respondents (n=3,465) were university students, living in the exposed and control dormitories representing a homogenous sample. The sharp increase in traffic noise burden in the exposed area was found at the first 10-year interval (LAeq=67.5 dB). A slight decrease occurred after 2014, and in 2019 up to LAeq=63.9 dB. A sharp increase in road traffic noise annoyance was observed in the first 10-year interval (ORMH=2.56 (95 % CI=1.93-3.42) vs 6.01 (95% CI=4.97-7.95) with a slightly decreasing trend in 2019. An increase in noise annoyance from entertainment facilities was observed as well. Despite a slightly declining trend, road traffic noise annoyance is still an important issue and there is a need for preventive measures to reduce such exposure in residential areas.

Abstract

Introduction. The consequences of acquired hearing loss in adults condition their psychosocial functioning and imply changes in the quality of life related to disorders in social and emotional functioning. Hearing loss is considered a risk factor for reduced psychosocial functioning and self-esteem. Psychosocial well-being and preserved self-esteem imply having a positive self-image and realizing that a person is an important part of the social community in which he/she lives. The paper deals with self-esteem and psychosocial functioning of adults with acquired hearing loss. The aim of this research is to determine the level of self-esteem and the degree of expression of consequences in psychosocial functioning, that is, the connection between the level of self-esteem and the degree of expression of consequences in psychosocial functioning in adults with acquired hearing loss. Methods. The research was conducted on a sample of 60 adult subjects with acquired hearing loss and 60 subjects with normal hearing. The average age of the respondents with hearing loss was 54.1 (SD = 10.84), and the sample consisted mostly of male respondents (56.7%) with moderate hearing loss (25.4%). The Rosenberg Self-Esteem Scale and the questionnaire for the subjective assessment of the hearing condition - the HHIA questionnaire (Hearing Handicap Inventory for Adults) were used as instruments. Results. The results showed that the respondents expressed moderate or average self-esteem and that there was a connection between self-esteem and psychosocial functioning in adults with acquired hearing loss. Conclusion. Future research could be directed more towards examining self-esteem in more detail in more specific domains of psychosocial functioning of adults with acquired hearing loss.

Abstract

Titanium and its composites are widely used in implants of bones and teeth. Besides mechanical properties also surface characteristics are very important in these biomaterials. Very important are properties such as surface topography, roughness, chemistry, and surface energy, wettability, and Ti oxides or Ti nitride layers thickness. The concentrated solar power was used successfully to nitride Ti Grade 2 and powder metallurgical Ti prepared from hydrogenated dehydrogenated Ti powder. The nitriding experiments were performed under nitrogen atmosphere at different temperatures and time in SF40 (40kW horizontal solar furnace) at PSA, Spain. Concentrated solar energy has been shown to be an economical alternative to conventional gas nitriding techniques in electric furnaces, CVD, PVD, plasma nitriding, or laser treatments. It has been observed that the solar process represents a significant reduction of the heating time to several minutes (up to 5 minutes at temperature range 500-1000 °C), a clean and non-polluting high-temperature process. The formation of continuous and homogeneous surface layers of TiN, Ti2N and their mixture according to the nitriding temperature was investigated using X-ray diffraction and electron microscopy. Laser surface treatment is of great significance in modifying surface morphology and surface and near-surface region microstructures. Effects of lase treatment parameters on machined surface morphology, surface roughness and chemistry are analyzed in this study and discussed from the point of view of application in dental implantology. The current advances of our research group in application of laser-treated powder metallurgy prepared Ti-based materials are analyzed and discussed.

Although there is awareness of the impact of grief on survivors’ well-being, almost no research exists on the impact of death on sports team bereavement. The present study surveyed 40 members of athletic teams (coaches, staff, and athletes) from various levels to determine what happens in the aftermath of a team member’s death. Findings of the survey indicated that many of the respondents experienced acute grief responses affecting performance, which memorialization and community support was helpful whereas the news media was often not. In addition, a need for appropriate resources and a school bereavement policy specific to student athletes was seen. In line with the dual process model, the responses indicated use of both emotion focused and restoration focused coping. Implications of the findings suggested that addressing bereavement needs for athletes, and their coaches was tantamount to mitigating some of the complications associated with disenfranchised grief.

28. Etiology and classification of acute pancreatitis in children admitted to ICU using the Pediatric Sequential Organ Failure Assessment (pSOFA) score

Pediatric acute pancreatitis (AP) is rare but increasing. Severe AP is associated with higher morbidity and mortality. However, there are no universally accepted prognostic criteria for AP.This retrospective study included children with AP admitted to an intensive care unit (ICU) of our tertiary pediatric center between January 2009 and December 2018. The severity of organ dysfunction in AP was assessed according to the modified Atlanta criteria using the Pediatric Sequential Organ Failure Assessment (pSOFA) and Computed Tomography Severity Index (CTSI).Seventy acute episodes of AP were evaluated in 55 children with primary pancreatitis. In addition, secondary AP was diagnosed in 15 patients originally admitted to ICU for different indications. Mild AP [no organ dysfunction, normal computed tomography (CT) finding] was the most prevalent (64/85 episodes in 49 children), followed by moderate AP (15 children; pSOFA 2-9 points, CTSI 3-4 points on admission). Severe AP (pSOFA 4-17 points, CTSI 6-10 points) was diagnosed in 6 children with traumatic or secondary AP. The most frequent etiologies of primary AP episodes were idiopathic (39%) and biliary (31%). Children with idiopathic AP had frequent relapses and comorbidities. Hereditary AP was typically mild, but presented with high pancreatic enzyme levels and recurrence rates. Admission at ICU and an interval without enteral nutrition (EN) were relatively short in drug-induced AP and relatively long in secondary and traumatic AP. Endoscopic retrograde cholangiopancreatography (ERCP) was performed in 13 patients with biliary AP and in 4 patients with traumatic AP. No AP-related death was observed.pSOFA score accurately reflects the severity and prognosis of AP in children.

Abstract

The formation of the Ti substituted Mg2Ni alloys, a promising hydrogen storage material for various applications is studied in detail. Mg1.95Ti0.05Ni alloy and ribbons are successfully prepared by vacuum arc melting and melt spinning methods. The phases, microstructures, and thermal behavior of the alloys and ribbons are characterized by XRD, SEM, TEM, DTA/TG. Sievert-type apparatus is used to study hydrogen sorption properties. Apart from the dominant Mg2Ni phase, the formation of MgNi2, Mg, and Ni3Ti phases is seen in both Mg1·95Ti0·05Ni alloy and ribbons. During the initial three cycles, Mg1·95Ti0·05Ni ribbons showed 2 wt % hydrogen storage capacity. To explain the atomic-scale influence of Ti dopant in the studied alloys and hydrides, FP(L)APW + lo method based on Density Functional Theory (DFT) is applied to Mg2-xTixNi (x = 0.25 and 0.5) alloys and Mg2-xTixNiH4 (x = 0.25 and 0.5) hydrides. An increase in the Ti dopant on the Mg site leads to the hydrides destabilization. Bader's charge density topology analysis provides insight into the charge transfer and bonding between the constituent atoms.

For $3 \leq k \leq O(\sqrt{n})$, using techniques from universal compression, the optimal prediction risk in Kullback-Leibler divergence is shown to be $\Theta(\frac{k^2}{n}\log \frac{n}{k^2})$, in contrast to the optimal rate of $\Theta(\frac{\log \log n}{n})$ for $k=2$ previously shown in Falahatgar et al. (2016). These rates, slower than the parametric rate of $O(\frac{k^2}{n})$, can be attributed to the memory in the data, as the spectral gap of the Markov chain can be arbitrarily small. To quantify the memory effect, we study irreducible reversible chains with a prescribed spectral gap. In addition to characterizing the optimal prediction risk for two states, we show that, as long as the spectral gap is not excessively small, the prediction risk in the Markov model is $O(\frac{k^2}{n})$, which coincides with that of an iid model with the same number of parameters. Abstract

Zusammenfassung: Hintergrund: Intensivpatient_innen sind oft einer langen Immobilität ausgesetzt. Wenn sie aber frühzeitig mobilisiert werden, lassen sich positive Effekte auf ihr Outcome, wie z.B. eine Verbesserung der körperlichen Funktion, nachweisen. Einer der Gründe für die späte Mobilisation ist, dass zu wenig Hilfsmittel zur Verfügung stehen. Fragestellungen/Ziel: Dieser Beitrag gibt einen Überblick über den Einsatz von robotischen oder technischen Systemen als Hilfsmittel für die Frühmobilisation. Welche robotischen und technischen Hilfsmittel werden in Studien zur Frühmobilisation von erwachsenen Intensivpatient_innen durch Pflegefachpersonen oder Physiotherapeut_innen untersucht? Über welche Effekte von Frühmobilisation mittels robotischem und technischem System auf die Patientenoutcomes wird in den Studien berichtet? Methoden: Die Datenbanken Medline, Web of Science, CINAHL, Cochrane Library, Embase, IEEE Xplore, Scopus und WTI wurden zwischen Mai und Juli 2020 und im Januar 2022 systematisch durchsucht. Zusätzlich wurde im ersten Suchlauf eine Randsuche über GoogleScolar und ResearchGate durchgeführt. Ergebnisse: Es wurden 27 Veröffentlichungen eingeschlossen (9 RCTs, 7 Expertenmeinungen, 3 quantitative Querschnittstudien, 2 Fall-Kontroll-Studien, 2 Literaturreviews, 2 klinische Einzelfallstudien, 2 Interventionsstudien im Prä-Post-Design). Hier zeigte sich, dass als Hilfsmittel vor allem elektronische Bettfahrräder und Kipptische eingesetzt werden. Es war eine uneinheitliche Datenlage in Bezug auf verschiedene Patientenoutcomes nachweisbar. Schlussfolgerungen: Weitere Forschung zum Einsatz von technischen und robotischen Systemen zur Frühmobilisation ist vor allem in Bezug auf unterschiedliche Studienpopulationen notwendig. Frühmobilisationsrobotik ist noch nicht Teil der Regelversorgung.

48. The impact of taxi drivers’ characteristics on the propensity to do business: Case study from a sharing economy

Abstract

This paper aims to quantify the impact of selected demographic, financial, and economic factors on the propensity to do business in the taxi sector of the sharing economy. The sample comprised 375 taxi drivers from the Czech Republic and Slovak Republic. Data were collected using the query method via a questionnaire in April 2022. The structure of the respondents is divided into shared taxi service providers (N = 294) and traditional taxi service providers (N = 69). The study selected 14 factors: demographic (4), financial (7), and economic (3). The SEM approach was applied to evaluate the hypotheses. Shared taxi providers have a stronger propensity to do business than traditional taxi drivers. Demographic characteristics of a traditional taxi driver are the most significant factors with a strong influence on the propensity to do business (βS = 0.525 > βT = 0.425). On the other hand, the financial and economic characteristics of shared taxi drivers strongly influence the propensity to do business (βT = 0.565 > βS = 0.212). The characteristics of the enterprise are on the verge of significance in relation to the tendency to do business with shared taxi drivers, as opposed to traditional taxi drivers. For traditional taxi drivers, there is a strong influence of the characteristics of the enterprise on the propensity to do business (βT = 0.476 > βS = 0.026). This study contributes to understanding how participating in sharing economy may stimulate the propensity to do business.

49. Late Effects: Auf welche potentiellen Folgen einer Krebstherapie im Kindesalter müssen wir bei Erwachsenen achten?

Was ist neu? Spätfolgen einer Krebserkrankung In den letzten Jahrzehnten hat sich das Langzeitüberleben nach einer Krebserkrankung im Kindes-/Jugendalter stetig verbessert und liegt mittlerweile entitätenübergreifend bei ca. 80%. In zahlreichen Studien konnte gezeigt werden, dass über zwei Drittel dieser Langzeitüberlebenden Jahre bis Jahrzehnte nach Ende der Krebstherapie neue Erkrankungen, sogenannte Spätfolgen, entwickeln. Um diese rechtzeitig diagnostizieren und behandeln zu können, werden risikoadaptierte Vorsorgeuntersuchungen empfohlen, die aufgrund des breiten Spektrums an möglichen Folgeerkrankungen von interdisziplinären Nachsorge-Teams angeboten werden. Neue Nachsorge-Empfehlungen Aufgrund eines erhöhten Brustkrebsrisikos sollten Patientinnen nach thorakaler Radiatio in intensivierte Vorsorgeprogramme eingeschlossen werden. Dies schließt nun auch Patientinnen ab einer Strahlendosis von 10 Gray ein. Verschiedene Krebstherapien erhöhen das Risiko für eine niedrige Knochendichte. Daher sollten diese Patient*innen frühzeitig eine Knochendichte-Messung erhalten. Psychische Erkrankungen können auch Jahre bis Jahrzehnte nach einer Krebserkrankung auftreten und sollten in der Nachsorge regelhaft adressiert werden. Wie sollte die optimale Langzeitnachsorge aussehen? Im Mittelpunkt der Nachsorge steht ein Kernteam aus Internist*innen, pädiatrischen Onkolog*innen, psychosozialen Mitarbeiter*innen und Case Manager*innen. Alle ehemaligen Patient*innen sollten spätestens nach Abschluss der regulären onkologischen Nachsorge, im Gespräch mit den betreuenden Kinderonkolog*innen über Spätfolgen und Langzeitnachsorge aufgeklärt werden, über Bedürfnisse und Bedenken zum Thema Nachsorge sprechen können und einen individuellen Nachsorgeplan nach Risikostratifizierung erhalten.
