Background: Beta thalassemia is one of the most common causes of inherited disorder in the world. In India, around 65000-70000 cases of thalassemia are present currently and every years around 9000-10000 new cases are added. Measurement of serum ferritin level gives an idea about when to start iron chelation therapy, which will reduce the concentration of iron in the serum and thereby prevents the multiple organ damages. Methods: A prospective hospital based study was conducted on 100 children having beta thalassemia major. These children were regularly transfused at our hospital and their serum ferritin level were measured by ELISA based assay. Detailed transfusion history and demographic profile were recorded and compared with serum ferritin level. Results were analyzed with SPSS software and Microsoft Excels. Results: In present study, mean serum ferritin level in 0-5 year age group was1262 µg/l, in 6-10 years age group was 1963.44µg/l and in 11-18 years age group was 2387.43µg/l. On applying ANOVA test, it was evident that there was a statistically significant correlation between increased serum ferritin level with increasing age of the patient as well as with increasing number of a total blood transfusion. Conclusions: High serum ferritin level in beta thalassemia is associated with increasing age of children as well as with increasing transfusion dependency. Serum ferritin measurement at regular interval helps in determining the ideal timing to start iron chelation therapy in transfusion dependant thalassemic patients.

Pneumocephalus, also known as pneumatocele or intracranial aerocele, is defined as the presence of air in the intracranial space. It can occur following trauma, cranial surgeries or spontaneously. It can be classified as acute if occurred in less than 72 hours or delayed if occurred after 72 hours. I am presenting a rare case of spontaneous onset as follows.[1]

The purpose of this research study is to examine the influence of social media on the Academic Performance of Students. Fifty Research questions and Four Research hypotheses guided the study. To achieve this, the Descriptive Survey Research design was adopted. The study focused on students of 9th and 10th standard of secondary school in Gandhidham city area and its surrounding Rural and Urban areas of Kutch district. The sample consists of all the 180 students. The simple random sampling technique was used to select a sample of 180 students. A five point Rating Scale Questionnaire type, titled: Social Media and Academic Performance of Students Questionnaire (SMAAPOS) were used to collect data from the participants. The descriptive statistics of frequency counts and percentage were used to analyze the demographic data while inferential statistics of Chi-square (χ²) test was used in testing the research hypotheses. Research findings showed that a great number of students are addicted to social media. To this end, the researcher recommended that social media should be used for educational purposes as well. Social Networking Sites should be expanded and new pages should be created to enhance academic activities and avoid setbacks in the students’ academic performance; and Students should be monitored by teachers and parents on how they use these sites This is to create a balance between social media and academic activities of students to avoid setbacks in the academic performance of the students., {"references":["A.C. Background: The implementation of electronic health information technologies is a key target for healthcare quality improvement. Among Swiss chiropractors, reliable data on the use of electronic heath information technologies and distribution of the health workforce was lacking. Objectives: To estimate the prevalence of electronic patient record (EPR) and encrypted email communication use among Swiss chiropractors and describe the geographic distribution of chiropractors in Switzerland. Methods: Population-based cross-sectional study of all active practising members of the Swiss Chiropractic Association (ChiroSuisse) between 3 December 2019 and 31 January 2020. We asked about clinician and practice characteristics, EPR use for clinical record keeping, use of encrypted email for patient communication, and information on EPR and encrypted email communication products used. Multivariable logistic regression analyses assessed the association between clinician and practice characteristics and (1) EPR use and (2) encrypted email use. Results: Among 286 eligible Swiss chiropractors (193 [68%] men; mean age, 51.4 [SD, 11.2] years), 217 (76%) completed the survey (140 [65%] men; mean age 50.7 [11.2] years). The density of chiropractors in Switzerland was 3.3 per 100,000 inhabitants. 47% of respondents (95% confidence interval [CI], 40% to 54%) reported using an EPR in their practice, while 60% (95% CI, 54% to 67%) endorsed using encrypted email technology. Chiropractors aged ≥60 (versus those ≤39) years were 74% less likely to use an EPR system (OR 0.26, 95% CI 0.08 to 0.77), while clinicians from practices with 4 or more chiropractors (versus those from solo practices) were over 5 times more likely to report EPR use (OR 5.6, 2.1 to 16.5). Findings for factors associated with encrypted email use were similar. Conclusions: As of January 2020, 286 duly licensed chiropractors were available to provide musculoskeletal healthcare in Switzerland — just under 50% of Swiss chiropractors used an EPR system in clinical practice, while 60% used encrypted email technology. Better implementation of EPR and electronic health information technologies in Swiss chiropractic practice is possible and desirable for the purpose of musculoskeletal healthcare quality improvement.

Testing and treating asymptomatic populations has the potential to reduce the population’s parasite reservoir and reduce malaria transmission. Zanzibar’s electronic malaria case notification (MCN) platform collects detailed socio-demographic and epidemiological data from all confirmed malaria cases to inform programmatic decision-making. This Field Action Report describes the design and operationalization process of the platform, as well as other malaria surveillance resources that are enabling Zanzibar to progress toward malaria elimination.The MCN platform consists of an interactive short message service (SMS) system for case notification, a software application for Android mobile devices, a visual question set and workflow manager, a back-end database server, and a web browser-based application for data analytics, configuration, and management. Malaria case data were collected from August 2012 to December 2021 and reported via SMS from all public and private health facilities to a central database, and then to District Malaria Surveillance Officers’ mobile devices. Data included patient names, shehia and date of diagnosis, which enabled the officers to track patients, ideally within 24 hours of reporting. Patients’ household members were tested for malaria using conventional rapid diagnostic tests (RDTs). Treatment using artemisinin-based combination therapy was provided for persons testing positive.Between 2012 and 2021, a total of 48,899 index malaria cases were confirmed at health facilities, 22,152 (45.3%) of whom within 24 hours of reporting; 41,886 (85.7%) cases were fully investigated and followed up to household level. A total of 111,811 additional household members were tested with RDTs, of whom 10,602 (9.5%) were malaria positive.The MCN platform reports malaria case data in near real-time, enabling prompt testing and treatment of members in index case households. Along with routine testing and treatment and other preventive interventions, combined with comprehensive reactive-case detection efforts, the continued use of the MCN platform is likely to reduce malaria transmission and malaria morbidity even further, thereby enhancing malaria elimination in Zanzibar.

Background Evidence-based practice (EBP) is the integration of best research evidence with clinical expertise and patients’ values and preferences. Little is known about knowledge, attitudes, and application of EBP among chiropractic students and trainees. Our aims were to (1) examine the feasibility of implementing a new journal club format within a Swiss university chiropractic healthcare education setting, and (2) assess the associations between the new journal club implementation and EBP characteristics among chiropractic students.Methods A before-and-after study was conducted through a newly implemented journal club with 5th and 6th year chiropractic students and postgraduate trainees between 1 February 2021 and 31 July 2021. The journal club was developed based on the “community of practice” and “team-based learning” conceptual frameworks. EBP knowledge, attitudes, personal application, and future use, were assessed with a validated questionnaire. We summarised participant characteristics using descriptive statistics, estimated before-and-after EBP total and subscale scores (i.e., knowledge, attitudes, personal application, and future use), and conducted an exploratory subgroup analysis based on journal club attendance (Group A: 3–5 and Group B: ≤ 2 sessions attended).Results Among 32 eligible students and trainees, 29 participants (mean age 26 years; 79% women) were enrolled: 25 (78%) responded to the pre- and 29 (91%) to the post-assessment surveys. Most (80%) were chiropractic students and 20% were postgraduate trainees. Group A consisted of 12 (41%) and Group B of 17 (59%) participants, respectively. We found reasonable feasibility for the new journal club format and our findings were compatible with no difference in before-and-after EBP scores (median EBP total score before: 72.6 [IQR, 63.7–77.4], and after: 73.4 [IQR, 61.3–78.2]). Exploratory subgroup analyses based on journal club attendance were consistent with our overall findings.Conclusion Our study suggests that the newly implemented journal club and embedding chiropractic educational research within the journal club were feasible and acceptable. Small before-and-after differences in the EBP subscale scores for knowledge, attitudes, personal application, and future use were observed in chiropractic students and postgraduate trainees. The small study size and short timeframe during a single semester limit potential inferences.

The Swiss chiropractic practice-based research network (PBRN) is a nationwide project developed in collaboration with patients, clinicians, and academic stakeholders to advance musculoskeletal epidemiologic research. The aim of this study was to describe the clinician population recruited and representativeness of this PBRN to inform future collaboration. A population-based cross-sectional study was performed. PBRN clinician characteristics were described and factors related to motivation (operationalised as VAS score ≥ 70) to participate in a subsequent patient cohort pilot study were assessed. Among 326 eligible chiropractors, 152 enrolled in the PBRN (47% participation). The PBRN was representative of the larger Swiss chiropractic population with regards to age, language, and geographic distribution. Of those enrolled, 39% were motivated to participate in a nested patient cohort pilot study. Motivation was associated with age 40 years or older versus 39 years or younger (OR 2.3, 95% CI 1.0–5.2), and with a moderate clinic size (OR 2.4, 95% CI 1.1–5.7) or large clinic size (OR 2.8, 95% CI 1.0–7.8) versus solo practice. The Swiss chiropractic PBRN has enrolled almost half of all Swiss chiropractors and has potential to facilitate collaborative practice-based research to improve musculoskeletal health care quality.Trial registration: Swiss chiropractic PBRN (ClinicalTrials.gov identifier: NCT05046249); Swiss chiropractic cohort (Swiss ChiCo) pilot study (ClinicalTrials.gov identifier: NCT05116020).

Numerous traffic-related problems arise as a result of the exponential growth in the number of vehicles on the road. Vehicle detection is important in many smart transportation applications, including transportation planning, transportation management, traffic signal automation, and autonomous driving. Many researchers have spent a lot of time and effort on it over the last few decades, and they have achieved a lot. In this paper, we compared the performances of major deep learning models: Faster RCNN, YOLOv3, YOLOv4, YOLOv5, and SSD for vehicle detection with variable image size using two different vehicle detection datasets: Highway dataset and MIOTCD. The datasets that are most commonly used in this domain are also analyzed and reviewed. Additionally, we haveemphasized the opportunities and challenges in this domain for the future.

Current drugs for treating human cytomegalovirus (HCMV) infections are limited by resistance and treatment-associated toxicities. In developing mechanistically novel HCMV antivirals, we discovered an N-benzyl hydroxypyridone carboxamide antiviral hit (8a) inhibiting HCMV in submicromolar range. We describe herein the structure–activity relationship (SAR) for 8a, and the characterization of potent analogs for cytotoxicity/cytostatic property, the preliminary mechanism of action, and the absorption, distribution, metabolism and excretion (ADME) properties. The SAR revealed a few pharmacophore features conferring optimal antiviral profile, including the 5-OH, the N-1 benzyl, at least one –CH2− in the linker, and a di-halogen substituted phenyl ring in the amide moiety. In the end, we identified numerous analogs with sub-micromolar antiviral potency and good selectivity index. The preliminary mechanism of action characterization used a pUL89-C biochemical endonuclease assay, a virus entry assay, a time-of-addition assay, and a compound withdrawal assay. ADME profiling measuring aqueous solubility, plasma and liver microsomal stability, and parallel artificial membrane permeability assay (PAMPA) permeability demonstrated largely favorable drug-like properties. Together, these studies validate the N-benzyl hydroxypyridone carboxamide as a viable chemotype for potent and mechanistically distinct antivirals against HCMV.

An environment-friendly approach for regioselective acylation of 1-methyl-3-phenyl quinoxaline-2(1H)-ones was developed using water as a solvent. The protocol exhibits a wide substrate scope and employs commercially available, non-toxic acyl surrogates.

Musculoskeletal disorders (MSDs) contribute to a high burden of disability worldwide. The development of primary care practice-based research networks (PBRNs) may be considered one strategy to help improve MSD health care quality and reduce disability burden. The overarching aim of this scoping review is to systematically review and evaluate the landscape of MSD focused PBRNs.

The company or industry can increase system reliability and provide stress-free operation by adding redundant equivalent subsystems to the active unit. The warm standby system is accessible if the operational unit malfunctions, and the cold standby system can take over. This paper aims to analyze a system comprised of one operative unit, cold standby unit, and one warm standby unit. Cold standby is activated to become warm standby when the operative unit fails, and warm standby becomes operational immediately. A minor defect causes the warm standby unit to fail, whereas a major fault causes the operational unit to fail. Such systems are used by many businesses, sectors, and facilities to prevent operational and reputational losses. Cut-off values for the failure rate, activation rate, revenue cost, and cost per repairman visit have been calculated to determine when the system is profitable. Various system performance measures have been defined by using the Markov process and regeneration point method.

Asphaltene Precipitation is a major issue in both upstream and downstream sectors of the Petroleum Industry. This problem could occur at different locations of the hydrocarbon production system i.e., in the reservoir, wellbore, flowlines network, separation and refining facilities, and during transportation process. Asphaltene precipitation begins due to certain factors which include variation in crude oil composition, changes in pressure and temperature, and electrokinetic effects. Asphaltene deposition may offer severe technical and economic challenges to operating Exploration and Production companies with respect to losses in hydrocarbon production, facilities damages, and costly preventive and treatment solutions. Therefore, asphaltene stability monitoring in crude oils is necessary for the prevention of aggravation of problem related to the asphaltene deposition. This study will discuss the performance of eleven different stability parameters or models already developed by researchers for the monitoring of asphaltene stability in crude oils. These stability parameters include Colloidal Instability Index, Stability Index, Colloidal Stability Index, Chamkalani’s stability classifier, Jamaluddin’s method, Modified Jamaluddin’s method, Stankiewicz plot, QQA plots and SCP plots. The advantage of implementing these stability models is that they utilize less input data as compared to other conventional modeling techniques. Moreover, these stability parameters also provide quick crude oils stability outcomes than expensive experimental methods like Heithaus parameter, Toluene equivalence, spot test, and oil compatibility model. This research study will also evaluate the accuracies of stability parameters by their implementation on different stability known crude oil samples present in the published literature. The drawbacks and limitations associated with these applied stability parameters will also be presented and discussed in detail. This research found that CSI performed best as compared to other SARA based stability predicting models. However, considering the limitation of CSI and other predictors, a new predictor, namely ANJIS (Abdus, Nimra, Javed, Imran & Shaine) Asphaltene stability predicting model is proposed. ANJIS when used on oil sample of different conditions show reasonable accuracy. The study helps Petroleum companies, both upstream and downstream sector, to determine the best possible SARA based parameter and its associated risk used for the screening of asphaltene stability in crude oils.

Swelling of shale potentially occurs when it is exposed to water-based drilling fluid. The migration of hydrogen ions (H+) in the nano-interlayered platelets of the shale rock is utterly responsible for the swelling behavior in the shale. Conventionally, swelling behavior of any shale formation can be experimentally determined by linear dynamic swell meter. However, it is extremely important to validate these experimental results; hence, this research study aims in conducting a comparative performance analysis for different kinetic models, namely Peleg’s model, first-order exponential association equation and pseudo-second-order kinetic model, and a newly developed scaling swelling model in estimating the experimental results of three different shale samples, namely Talhar, Ranikot and Murree, obtained from different regions of Pakistan. It was found that the performance of the scaling swelling model was the most accurate in predicting the experimental swelling results with accuracy greater than 95% in all the three samples. Peleg’s model is found to be the most inaccurate with $$p \mathrm{values}< \alpha (0.05)$$ p values < α ( 0.05 ) in all the three formations. The equilibrium state in all the three samples was unable to attain by the use of this model. This clearly shows that the transient states continue throughout the course of experimentation, thus demonstrating a higher water activity in the shale samples. Moreover, when comparison was made between the two remaining kinetic adsorption models, it was perceived that pseudo-second-order kinetic was far superior to first-order exponential association equation with $${\mathrm{mean}}_{\mathrm{model}}\simeq {\mathrm{mean}}_{\mathrm{experiment}}$$ mean model ≃ mean experiment and less dispersion in the dataset. Nevertheless, the performance of this model also suffers with the increase in clay content. Furthermore, all these analyses were further validated by different statistical error analysis that includes MAE, APRE% and ANOVA.

Background Deciphering the functions of Y chromosome in mammals has been slow owing to the presence of repeats. Some of these repeats transcribe coding RNAs, the roles of which have been studied. Functions of the noncoding transcripts from Y chromosomal repeats however, remain unclear. While a majority of the genes expressed during spermatogenesis are autosomal, mice with different deletions of the long arm of the Y chromosome (Yq) were previously also shown to be characterized by subfertility, sterility and sperm abnormalities, suggesting the presence of effectors of spermatogenesis at this location. Here we report a set of novel noncoding RNAs from mouse Yq and explore their connection to some of the autosomal genes expressed in testis. Results We describe a set of novel mouse male-specific Y long arm (MSYq)-derived long noncoding (lnc) transcripts, named Pirmy and Pirmy-like RNAs. Pirmy shows a large number of splice variants in testis. We also identified Pirmy-like RNAs present in multiple copies at different loci on mouse Y chromosome. Further, we identified eight differentially expressed autosome-encoded sperm proteins in a mutant mouse strain, XYRIIIqdel (2/3 Yq-deleted). Pirmy and Pirmy-like RNAs have homology to 5′/3′UTRs of these deregulated autosomal genes. Several lines of experiments show that these short homologous stretches correspond to piRNAs. Thus, Pirmy and Pirmy-like RNAs act as templates for several piRNAs. In vitro functional assays reveal putative roles for these piRNAs in regulating autosomal genes. Conclusions Our study elucidates a set of autosomal genes that are potentially regulated by MSYq-derived piRNAs in mouse testis. Sperm phenotypes from the Yq-deleted mice seem to be similar to that reported in inter-specific male-sterile hybrids. Taken together, this study provides novel insights into possible role of MSYq-derived ncRNAs in male sterility and speciation.

Background Insecticide resistance is a serious threat to the continued effectiveness of insecticide-based malaria vector control measures, such as long-lasting insecticidal nets (LLINs) and indoor residual spraying (IRS). This paper describes trends and dynamics of insecticide resistance and its underlying mechanisms from annual resistance monitoring surveys on Anopheles gambiae sensu lato (s.l.) populations conducted across mainland Tanzania from 2004 to 2020. Methods The World Health Organization (WHO) standard protocols were used to assess susceptibility of the wild female An. gambiae s.l. mosquitoes to insecticides, with mosquitoes exposed to diagnostic concentrations of permethrin, deltamethrin, lambdacyhalothrin, bendiocarb, and pirimiphos-methyl. WHO test papers at 5× and 10× the diagnostic concentrations were used to assess the intensity of resistance to pyrethroids; synergist tests using piperonyl butoxide (PBO) were carried out in sites where mosquitoes were found to be resistant to pyrethroids. To estimate insecticide resistance trends from 2004 to 2020, percentage mortalities from each site and time point were aggregated and regression analysis of mortality versus the Julian dates of bioassays was performed. Results Percentage of sites with pyrethroid resistance increased from 0% in 2004 to more than 80% in the 2020, suggesting resistance has been spreading geographically. Results indicate a strong negative association (p = 0.0001) between pyrethroids susceptibility status and survey year. The regression model shows that by 2020 over 40% of An. gambiae mosquitoes survived exposure to pyrethroids at their respective diagnostic doses. A decreasing trend of An. gambiae susceptibility to bendiocarb was observed over time, but this was not statistically significant (p = 0.8413). Anopheles gambiae exhibited high level of susceptibility to the pirimiphos-methyl in sampled sites. Conclusions Anopheles gambiae Tanzania’s major malaria vector, is now resistant to pyrethroids across the country with resistance increasing in prevalence and intensity and has been spreading geographically. This calls for urgent action for efficient malaria vector control tools to sustain the gains obtained in malaria control. Strengthening insecticide resistance monitoring is important for its management through evidence generation for effective malaria vector control decision.

Background Tanzania has made remarkable progress in reducing malaria burden and aims to transition from malaria control to sub-national elimination. In 2013, electronic weekly and monthly reporting platforms using the District Health Information System 2 (DHIS2) were introduced. Weekly reporting was implemented through the mobile phone-based Integrated Disease Surveillance and Response (eIDSR) platform and progressively scaled-up from 67 to 7471 (100%) public and private health facilities between 2013 and 2020. This study describes the roll-out and large-scale implementation of eIDSR and compares the consistency between weekly eIDSR and monthly DHIS2 malaria indicator data reporting, including an assessment of its usefulness for malaria outbreak detection and case-based surveillance (CBS) in low transmission areas. Methods The indicators included in the analysis were number of patients tested for malaria, number of confirmed malaria cases, and clinical cases (treated presumptively for malaria). The analysis described the time trends of reporting, testing, test positivity, and malaria cases between 2013 and 2021. For both weekly eIDSR and monthly DHIS2 data, comparisons of annual reporting completeness, malaria cases and annualized incidence were performed for 2020 and 2021; additionally, comparisons were stratified by malaria epidemiological strata (parasite prevalence: very low 30%). Results Weekly eIDSR reporting completeness steadily improved over time, with completeness being 90.2% in 2020 and 93.9% in 2021; conversely, monthly DHIS2 reporting completeness was 98.9% and 98.7% in 2020 and 2021, respectively. Weekly eIDSR reporting completeness and timeliness were highest in the very low epidemiological stratum. Annualized malaria incidence as reported by weekly eIDSR was 17.5% and 12.4% lower than reported by monthly DHIS2 in 2020 and 2021; for both 2020 and 2021, annualized incidence was similar across weekly and monthly data in the very low stratum. Conclusion The concurrence of annualized weekly eIDSR and monthly DHIS2 reporting completeness, malaria cases and incidence in very low strata suggests that eIDSR could be useful tool for early outbreak detection, and the eIDSR platform could reliably be expanded by adding more indicators and modules for CBS in the very low epidemiological stratum.

Motivation was associated with age 40 years or older versus 39 years or younger (odds ratio [OR] 2.3, 95% confidence interval [CI] 1.0 to 5.5), and with a moderate clinic size of 2 or 3 chiropractors (OR 2.5, 95%CI 1.1 to 5.9) or large clinic size of 4 or more chiropractors (OR 2.8, 95%CI 1.0 to 7.0) versus solo practice. The Swiss chiropractic PBRN has enrolled almost half of all Swiss chiropractors in over 100 clinical practices and has potential to facilitate collaborative practice-based research to improve chiropractic health care quality. Academic Laboratories plays a significant role during students and researchers academic life. In academic labs, students routinely performed different experiments and most often they are not aware of the experimental hazards and hence encounter accidents. Moreover, these research Labs possess complex nature because of their variety of operations, equipment and experiments, therefore, analyzing risks through conventional techniques become extremely difficult. In this research study, risks prioritization of an experiment related to asphaltene quantification in a crude oil has been carried out through four methods. Firstly, the conventional risk analysis technique Failure mode and effect analysis (FMEA) is applied. For the implementation of FMEA, 44 failure scenarios/modes that could occur and can impact health and safety of researcher or students are identified. Each failure mode is assessed in terms of severity, likelihood of occurrence and detection. FMEA, similar to other studies in literature, is not able to work effectively in prioritizing the risks. Therefore, in order to overcome this limitation of FMEA, FMEA integrated with Grey Relational Analysis and Fuzzy Analytic hierarchy process method is applied and improvement in risk ranking is observed. Finally for further analysis of risks, Fuzzy technique for order preference by similarity to ideal solution (TOPSIS) integrated with a Fuzzy AHP is implemented and considerable improvement in risk ranking is achieved. The proposed study will help researchers to take complete precautionary measures according to the risks involved in an experiment. Moreover, this proposed model can be used and applied for other experiments performed in Academic Labs.

This research study aims to conduct a comparative performance analysis of different scaling equations and non-scaling models used for modeling asphaltene precipitation. The experimental data used to carry out this study are taken from the published literature. Five scaling equations which include Rassamadana et al., Rassamdana and Sahimi, Hu and Gou, Ashoori et al., and log–log scaling equations were used and applied in two ways, i.e., on full dataset and partial datasets. Partial datasets are developed by splitting the full dataset in terms of Dilution ratio (R) between oil and precipitant. It was found that all scaling equations predict asphaltene weight percentage with reasonable accuracy (except Ashoori et al. scaling equation for full dataset) and their performance is further enhanced when applied on partial datasets. For the prediction of Critical dilution ratio (Rc) for different precipitants to detect asphaltene precipitation onset point, all scaling equations (except Ashoori et scaling equation when applied on partial datasets) are either unable to predict or produce results with significant error. Finally, results of scaling equations are compared with non-scaling model predictions which include PC-Saft, Flory–Huggins, and solid models. It was found that all scaling equations (except Ashoori et al. scaling equation for full dataset) either yield almost the same or improved results for asphaltene weight percentage when compared to best case (PC-Saft). However, for the prediction of Rc, Ashoori et al. scaling equation predicts more accurate results as compared to other non-scaling models.
