Background: We aim to assess whether time of day modified the treatment effect in the RACECAT trial (Direct Transfer to an Endovascular Center Compared to Transfer to the Closest Stroke Center in Acute Stroke Patients With Suspected Large Vessel Occlusion Trial), a cluster-randomized trial that did not demonstrate the benefit of direct transportation to a thrombectomy-capable center versus nearest local stroke center for patients with a suspected large vessel stroke triaged in nonurban Catalonia between March 2017 and June 2020. Methods: We performed a post hoc analysis of RACECAT to evaluate if the association between initial transport routing and functional outcome differed according to trial enrollment time: daytime (8:00 am –8:59 pm ) and nighttime (9:00 pm –7:59 am ). Primary outcome was disability at 90 days, as assessed by the shift analysis on the modified Rankin Scale score, in patients with ischemic stroke. Subgroup analyses according to stroke subtype were evaluated. Results: We included 949 patients with an ischemic stroke, of whom 258 patients(27%) were enrolled during nighttime. Among patients enrolled during nighttime, direct transport to a thrombectomy-capable center was associated with lower degrees of disability at 90 days (adjusted common odds ratio [acOR], 1.620 [95% CI, 1.020–2.551]); no significant difference between trial groups was present during daytime (acOR, 0.890 [95% CI, 0.680–1.163]; P interaction =0.014). Influence of nighttime on the treatment effect was only evident in patients with large vessel occlusion(daytime, acOR 0.766 [95% CI, 0.548–1.072]; nighttime, acOR, 1.785 [95% CI, 1.024–3.112] ; P interaction P interaction >0.1 for all comparisons). We observed longer delays in alteplase administration, interhospital transfers, and mechanical thrombectomy initiation during nighttime in patients allocated to local stroke centers. Conclusions: Among patients evaluated during nighttime for a suspected acute severe stroke in non-urban areas of Catalonia, direct transport to a thrombectomy-capable center was associated with lower degrees of disability at 90 days. This association was only evident in patients with confirmed large vessel occlusion on vascular imaging. Time delays in alteplase administration and interhospital transfers might mediate the observed differences in clinical outcome. Abstract: A swallowing disorder or dysphagia is defined as a disorder in the sequence of swallow-ing, during the oral or pharyngeal phase, which compromises the safety and/or efficiency of transit of the food bolus to the esophagus. The evaluation of neurodevelopment, nutrition, and preventive medicine actions are as important as the clinical evaluation of dysphagia, so they must be included and systematized in all pediatric evaluations; This evaluation can be divided into different parts: bedside swallowing evaluation, in-strumental swallowing evaluation, and additional studies. The management of swallowing disorders requires a multidisciplinary team approach, depending on the child's age, cognitive and physical abilities, and the specific swallowing and feeding disorder, ensuring adequate and safe nutrition and improving the patient's quality of life.

5. Baseline Characteristics and Longitudinal Outcomes of Traditional Serrated Adenomas: A Cohort Study

Traditional serrated adenomas (TSAs) may confer increased risk for colorectal cancer (CRC). Our objective with this study was to examine clinical characteristics and long-term outcomes associated with TSA diagnosis.We conducted a retrospective cohort study of U.S. Veterans ≥18 years of age with ≥1 TSA between 1999 and 2016. Baseline characteristics, colonoscopy findings, and diagnosis of incident and fatal CRC were abstracted. Advanced neoplasia was defined by CRC or adenoma with high-grade dysplasia, villous histology, or size ≥1 cm. Follow-up was through CRC diagnosis, death, or end of study (December 31, 2018).A total of 853 Veterans with a baseline TSA were identified; 74% were ≥60 years of age, 96% were men, 14% were Black, and 73% were non-Hispanic White. About 64% were current or former smokers. Over 2044 total person-years at follow-up, there were 11 incident CRC cases and 1 CRC death. Cumulative CRC incidence was 1.34% (95% confidence interval [CI], 0.67%-2.68%), and cumulative CRC death was 0.12% (95% CI, 0.00%-0.35%). Among the subset of 378 TSA patients with ≥1 surveillance colonoscopy, 65.1% had high-risk neoplasia on follow-up. CRC incidence among TSA patients was significantly higher than in a comparison cohort of patients with normal baseline colonoscopy at baseline (hazard ratio, 3.70; 95% CI, 1.63-8.41) and similar to a comparison cohort with baseline conventional advanced adenoma (hazard ratio, 0.86; 95% CI, 0.45-1.64).Individuals with TSA have substantial risk for CRC, based on their cumulative CRC incidence, as well as significant risk of developing other high-risk neoplasia at follow-up surveillance colonoscopy. These data underscore importance of current recommendations for close colonoscopy surveillance after TSA diagnosis.

Background: We aim to compare the outcome of patients from urban areas, where the referral center is able to perform thrombectomy, with patients from nonurban areas enrolled in the RACECAT trial (Direct Transfer to an Endovascular Center Compared to Transfer to the Closest Stroke Center in Acute Stroke Patients With Suspected Large Vessel Occlusion). Methods: Patients with suspected large vessel occlusion stroke, as evaluated by a Rapid Arterial Occlusion Evaluation score of ≥5, from urban catchment areas of thrombectomy-capable centers during RACECAT trial enrollment period were included in the Stroke Code Registry of Catalonia. Primary outcome was disability at 90 days, as assessed by the shift analysis on the modified Rankin Scale score, in patients with an ischemic stroke. Secondary outcomes included mortality at 90 days, rate of thrombolysis and thrombectomy, time from onset to thrombolysis, and thrombectomy initiation. Propensity score matching was used to assemble a cohort of patients with similar characteristics. Results: The analysis included 1369 patients from nonurban areas and 2502 patients from urban areas. We matched 920 patients with an ischemic stroke from urban areas and nonurban areas based on their propensity scores. Patients with ischemic stroke from nonurban areas had higher degrees of disability at 90 days (median [interquartle range] modified Rankin Scale score, 3 [2–5] versus 3 [1–5], common odds ratio, 1.25 [95% CI, 1.06–1.48]); the observed average effect was only significant in patients with large vessel stroke (common odds ratio, 1.36 [95% CI, 1.08–1.65]). Mortality rate was similar between groups(odds ratio, 1.02 [95% CI, 0.81–1.28]). Patients from nonurban areas had higher odds of receiving thrombolysis (odds ratio, 1.36 [95% CI, 1.16–1.67]), lower odds of receiving thrombectomy(odds ratio, 0.61 [95% CI, 0.51–0.75]), and longer time from stroke onset to thrombolysis (mean difference 38 minutes [95% CI, 25–52]) and thrombectomy(mean difference 66 minutes [95% CI, 37–95]). Conclusions: In Catalonia, Spain, patients with large vessel occlusion stroke triaged in nonurban areas had worse neurological outcomes than patients from urban areas, where the referral center was able to perform thrombectomy. Interventions aimed at improving organizational practices and the development of thrombectomy capabilities in centers located in remote areas should be pursued. La trombocitopenia inmune primaria es una enfermedad autoinmunitaria, en la que existe anticuerpos contra antígenos de superficie de las plaquetas, afectando la producción y generando la destrucción, lo que disminuye el número de las mismas (< 100.000/L), en ausencia de otros problemas que la justifiquen, y ocasionando alta susceptibilidad de sangrado en los pacientes según la severidad. Sus mecanismos fisiopatológicos de tipo inmunológico han permitido mejorar el manejo y tener varias opciones de tratamiento y evitar complicaciones que comprometan la vida de los pacientes. Asimismo, conocer las condiciones subyacentes de manera critica es fundamental para la toma de decisiones en las diferentes líneas de tratamiento, que están muy bien establecidas en la actualidad. Se Reporta un caso de un paciente masculino de 70 años con diagnóstico de trombocitopenia inmune primaria diagnosticada hace 5 años, con tratamiento actual con prednisona oral más azatioprina, con mala respuesta y abandono de este hace 1 año.

Hovenia dulcis is a non-native species with high capacity for invasion in subtropical forest fragments. In this study we used a greenhouse experiment simulating field abiotic and biotic conditions to investigate the germination and growth to determine the mechanisms associated with the high invasion potential of this species. We evaluated the effects of abiotic factors (light) and biotic factors (litter mass and litter type) on ecophysiological parameters: germination (GR), initial establishment (RR and ER) and shoot and root growth in a greenhouse experiment. Interactions between factors were compared using GLM, to select the best model and the factors that influence each parameter evaluated. GR, RR and ER were negative influenced by light intensities, when the low intensity presents more germination and establishment of individuals than intermediate intensity. Additionally, shoot height and root length were negatively influenced by the light intensity with a higher stem elongation in low light intensities. We found no interactions between the abiotic and biotic conditions tested. However, when evaluated individually, germination, establishment and growth were better in low light conditions regardless of the litter mass and litter type. These results challenge the classification of H. dulcis as an intolerant shade species.

El dimensionamiento de los procesos industriales resulta importante dentro de la actividad productiva de una empresa de manufactura, pues permite el mejorar el proceso de coste beneficio y la calidad del producto. En los entornos modernos, la reconfiguración de las máquinas para diseños personalizados es posible, pero disminuye el volumen de producción y reduce la eficiencia del proceso como tal. Para el caso del presente artículo, analizaremos una propuesta de producción justo a tiempo, la misma que permite satisfacer la demanda que ha sido solicitada son generar cantidades en stock que tienen costes asociados a los procesos de bodega y almacenamiento. El artículo define y dimensiona claramente un proceso de producción de clúster, en donde se define la maquinaria, capacidades, y procedimientos necesarios en base a un proceso real, el mismo que actualmente está en desarrollo y permite obtener resultados cuantificables.

20. Audiovestibular manifestations in patients with rheumatologic diseases: a review

Rheumatologic disorders are chronic and systemic disorders with an estimated global prevalence of 3,200 per 100,000 inhabitants. Several clinical manifestations are known and may lead to physical disability and altered quality of life. The most common audio vestibular manifestations commonly reported are hearing loss, tinnitus, vertigo, and fullness, however, there are some more still unregistered. The aim of this review is to describe the most common audio vestibular features in three of the most common rheumatologic conditions such as rheumatoid arthritis (RA), systemic erythematous lupus and systemic sclerosis (SSc). A literature search was performed with articles published between 2000 to 2021. In this literature review an increased prevalence of hearing loss for RA of 40% and 70% for systemic erythematous lupus was observed. In SSc, due to its early onset and progression, there is no estimated prevalence, however, a notorious hearing impairment in early stages of the disease was described. Vertigo was also described in these disorders, affecting more than 60% of patients. Further studies are in need to describe the affection on early stages of all disorders as well as its progression.

Vestibular disorders may lead to physical disability and altered quality-of life. Even though, some specific diseases are associated with sleep, work, leisure, and emotional impairment, little is known about which vestibular disorders may lead to disturbances in daily life activities. We aim to extend the understanding of this issue and provide a comprehensive overview about how vestibular disorders are linked to altered quality of life (QoL). Currently available literature was analysed for understanding the affection of QoL in several focused daily-life activities. Vestibular disorders and QoL articles were reviewed; 431 articles were found on different databases and 50 were included in this review. We provide the latest evidence about how QoL is affected by vestibular disorders, their relationship, and upcoming challenges in this review. The deepest understanding of how QoL may be affected by vestibular disorders would provide better comprehension of this process for primary care physicians, otolaryngologists, neurologists, and neuro-otologists.

Introducción: La enfermedad granulomatosa crónica (EGC) es un error innato de la inmunidad, se caracteriza por una susceptibilidad a padecer infecciones bacterianas y fúngicas y a una falta de regulación inflamatoria sistémica. Las variantes patogénicas en el gen CYBB se trasmiten con un patrón de herencia ligada al X; mientras que las variantes patogénicas presentes en los genes EROS, NCF1, NCF2, NCF4 o CYBA se trasmiten con un patrón de herencia autosómico recesivo. Objetivos. Describir las características clínicas, inmunológicas y genéticas de dos pacientes con EGC e infección por BCG. Métodos: En neutrófilos de sangre periférica se midió la producción de H2O2 y la expresión de las subunidades de la NADPH oxidasa. La detección de las variantes patogénicas fue por secuenciación Sanger del gen NCF2. La información clínica fue extraída de los expedientes por los médicos tratantes. Resultados: Presentamos a dos lactantes masculinos de dos familias no relacionadas de la etnia maya, con EGC e infección por la vacuna de BCG. Se identificaron tres diferentes variantes patogénicas en el gen NCF2; por un lado, c.304 C>T (p.Arg102*) ya reportada, por otro lado, c.1369 A>T (p.Lys457*) y c.979 G>T (p.Gly327*) no reportadas. Conclusiones: En pacientes con infección micobacteriana por BCG debemos sospechar en un error innato de la inmunidad, como la EGC. El diagnóstico de EGC se realiza a través de la detección de una falta de producción de radicales libres en los neutrófilos. Los pacientes reportados tuvieron variantes patogénicas en el gen NCF2, dos de ellas no han sido reportadas previamente en la literatura.

24. Utilidad de la medición de la hormona paratiroidea sérica como predictor de hipocalcemia posterior a tiroidectomía

Introducción: Laprevalencia del cáncer de tiroides es del 4 al 7%, en Ecuador alcanza una tasa de 16/100.000 habitantes, en la mayoría de casos requiere resolución quirúrgica asociando una tasa de complicaciones del 2 al 15%, de estas la hipocalcemia representa al menos el 10.9%. El objetivo delpresente estudio fue evaluar la utilidad de la medición de la hormona paratiroidea sérica (PTH) como predictor de hipocalcemia posterior a tiroidectomía. Métodos: Se realizó un estudio transversal analítico de pacientes con tiroidectomía en el Hospital Metropolitano de Quito, en el periodo enero del 2017 a diciembre del 2019. Se analizaron variables demográficas, clínicas, quirúrgicas y complicaciones. Se comparó la relación de la PTH con la presencia de hipocalcemia clínica, serológica o ambas mediante RP, sensibilidad, especificidad, VPP, VPN, LR. Otras variables confusoras se analizaron mediante regresión logística multinomial comparando sus OR. Resultados: Se incluyeron 212 registros, 15.6% hombres y 84.4% mujeres, con promedio de 52.1 años. La indicación principal de tiroidectomía fue por Carcinoma papilar 123 (58.0%). Las complicaciones fueron el 22.7%. Un total de 48 pacientes desarrollaron hipocalcemia. Los niveles de PTH < 10 ng/l predicen la hipocalcemia con sensibilidad de 0.55, especificidad 0.78, VPP: 43%, VPN: 85%, RP fue 2.91 (IC 95% 1.06 - 8.01). Conclusiones: La cuantificación de PTH en 10 ng/l o menos, es útil para predecir hipocalcemia clínica en los pacientes sometidos a tiroidectomía total.

La información aquí obtenida será revisada para su posterior análisis. No hay grandes actualizaciones sobre esta patología, ya que su incidencia es muy baja, por ello la escasa información disponible en las diferentes bibliotecas medicas que existen en la actualidad, sin embargo, se conoce que esta patología se puede desencadenar o presentar por una secreción excesiva de andrógenos y desencadenar Tumores suprarrenales secretores de andrógenos e Hiperplasia suprarrenal, el tratamiento depende de la evolución de la enfermedad, el manejo va orientado a la administración de anticonceptivos y extirpación de tumores.

There is increasing evidence of the influence of the gut microbiota on kidney diseases and precursors such as hypertension, diabetes, cardiovascular diseases and its complications, such as stroke, heart failure, and myocardial infarction. This is no surprising considering that the most common risk factor for hypertension, such as age, sex medication, and diet, can also impact the gut microbiota. For example, sodium and fermentable fiber have been studied in relation to both hypertension and the gut microbe Inflammation and fibrosis are the important pathophysiologic processes in diabetic kidney disease (DKD), which is induced by epigenetics, especially histone posttranslational modification (HPTMs). Have been indicated to play different roles in the repair of DNA damage. Recent reports highlighted that butyrate, one of the short-chain fatty acids (SCFAs) primarily originated from the fermentation of dietary fiber in the gut, attenuates inflammation and fibrosis in the prevention and treatment of DKD, however the molecular mechanisms are still unclear. Histone lysine butyrylation (Kbu), a novel histone modification marker induced by butyrate, has been found to be involved in the regulation of pathophysiological processes. To reveal the mechanisms of butyrate-induced histone (Kbu), in the prevention and treatment of DKD, both DKD modelsin vivoand in vitro were treated with sodium butyrate (NaB). The results confirmed that exogenous NaB improved the disorder of glucose and lipid metabolism, prevented proteinuria and renal failure, and inhibited renal inflammation and fibrosis New treatment options in the form of prebiotics (dietary fiber), probiotics (lactobacillus spp) and postbiotics (the short-chain fatty acids acetate, propionate, and butyrate) have all been demonstrated to be beneficial in the kidney disease. We should include short-chain fatty acids such as butyric acid in our patients with kidney disease regardless of the etiology to maintain the intestinal microbiota and avoid dysbiosis that causes inflammation, proteinuria, endothelial dysfunction.

Everyone has lost their footing due to the pandemic. The academic community was unprepared, leaving both professors and students bewildered and without a sense of direction. The goal at the time was to overcome the calamity of educational discontinuity. Sustainable Development Goal (SDG) 4 – Education goal aims “to ensure inclusive and equitable quality education and promote lifelong learning opportunities for all” relative to this SDG and the surge of the pandemic, digital divide become prominent, altering the course of educational landscape in all levels across public and private institutions. In the Bicol region, education was delivered in many various forms and variability, predicting the future through strategic foresight could propel the region to a transformed future. This paper discusses in detail the status of education in the region and the possible scenarios using Jim Dator’s Future Scenario Archetypes, Causal Layered Analysis and through Scenario Planning Plus. Four plausible scenarios have surfaced in this paper: STUG-nation of Education, Unstable and Disarray of Educational elements, Flowing-restrained Education, and ORAGON Education. It requires multiple drivers as examined through the PESTEL that includes political, economic, social, technological, environmental, legal aspects. This study proved useful in crafting and anticipating the future scenarios of Bicol Region’s education. Preliminary as it may, this study highlighted important building blocks of future scenarios through foresight methods. Dator’s framework was proved to be very useful in developing the four scenarios, the use of other future study tools will further amplify plausible scenarios for the education of Bicol region and beyond.

34. Local wind in urban canyons of a residential area in Quito-Ecuador

This study evaluates the behavior of the wind from its speed in representative urban canyons of residential buildings in the city of Quito. The methodology was based on the selection of case studies from the area of Concepción located in the north of the city to build simplified models according to their aspect ratio height / width (H / W) in blocks of 250 m by 250 m. In each case includes multiple configurations of urban canyons, giving priority to the west-east and north-south orientations, which are simulated by the application of Computational Fluid Dynamics (CFD). The local wind is evaluated according to pedestrian height and their predominant direction obtained from the historical information obtained from the weather station in the area. The results show that the predominant relationship of the wind retains higher speeds when it flows canyons with a lower H/W ratio.

La belleza de un pequeño bosque de Polylepis, a 4.300 metros sobre el nivel del mar no solamente impacta por su hermosura, sino que además plantea la pregunta de ¿Por qué es tan reducida su superficie? Más allá de eso, nos interroga sobre si esa pequeña extensión es lo poco que queda de una vegetación anteriormente más abundante que está sufriendo la destrucción por la intervención humana. Surge naturalmente el deseo de defender tal “relicto”. Para ello este artículo trata de entender cuál es la dinámica de este pequeño bosque buscando identificar amenazas y posibles acciones para su defensa. Se explora la información que surge de la distribución de las plantas por clases diamétricas, la información de sensores remotos, y se discute en el contexto de otras informaciones sobre la región. Se concluye que el bosque no es un relicto sino un enclave bien asentado.

Metodología: Este estudio corresponde a un diseño descriptivo siendo una investigación cualitativa. La muestra estuvo conformada por 15 adultos mayores. Se utilizó la técnica de recolección de datos mediante una entrevista semiestructurada.Resultados: El rango de edad fue de 75 a 90 años ssiendo 6 mujeres y 9 hombres. Se obtuvieron cuatro categorías, modo de adaptación fisiológico: presenta enfermedades, modo de adaptación de autoconcepto: creencias de fe, modo de desempeño de roles: interacción con las personas que lo rodean y modo de adaptación de interdependencia: apoyo familiar.Conclusión: Se concluye que la mayoría de los adultos mayores presentan en el modo de adaptación de interdependencia un poco apoyo de parte de su círculo familiar.

OBJECTIVE There is a paucity of information on pediatric traumatic brain injury (TBI) care in Asia and Latin America. In this study, the authors aimed to describe the clinical practices of emergency departments (EDs) participating in the Saline in Asia and Latin-America Neurotrauma in the Young (SALTY) study, by comparing designated trauma centers (DTCs) and nontrauma centers (NTCs) in their networks. METHODS The authors performed a site survey study on pediatric TBI management in the EDs in 14 countries. Two European centers joined other participating sites in Asia and Latin America. Questions were formulated after a critical review of current TBI guidelines and published surveys. The authors performed a descriptive analysis and stratified centers based on DTC status. RESULTS Of 24 responding centers (70.6%), 50.0% were DTCs, 70.8% had academic affiliations, and all centers were in urban settings. Patients were predominantly transferred to DTCs by centralized prehospital services compared to those sent to NTCs (83.3% vs 41.7%, p = 0.035). More NTCs received a majority of their patients directly from the trauma scene compared to DTCs (66.7% vs 25.0%, p = 0.041). Ten centers (41.7%) reported the use of a TBI management guideline, and 15 (62.5%) implemented CT protocols. Ten DTCs reported implementation of intervention strategies for suspected raised intracranial pressure (ICP) before conducting a CT scan, and 6 NTCs also followed this practice (83.3% vs 50.0%, p = 0.083). ED management for children with TBI was comparable between DTCs and NTCs in the following aspects: neuroimaging, airway management, ICP monitoring, fluid resuscitation, anticoagulant therapy, and serum glucose control. Hyperventilation therapy for raised ICP was used by 33.3% of sites. CONCLUSIONS This study evaluated pediatric TBI management and infrastructure among 24 centers. Limited differences in prehospital care and ED management for pediatric patients with TBI were observed between DTCs and NTCs. Both DTCs and NTCs showed variation in the implementation of current TBI management guidelines. There is an urgent need to investigate specific barriers to guideline implementation in these regions.

45. Abstract P6-11-03: Lipid-rich environment induces epigenetic reprogramming in non-transformed breast epithelial cells enhancing a mammary cell plasticity

Introduction. The identification of women specifically at risk for estrogen receptor negative breast cancer (ER- BC) and the targeted treatment of this disease are significantly unmet clinical needs. We analyzed the gene expression profiles of epithelial cells from the contralateral unaffected breasts (CUBs) of BC patients and identified a lipid metabolism gene signature enriched in the CUBs of women with ER- BC (PMID: 28263391). Subsequently, we observed that exposure of non-transformed breast epithelial cells to lipids results in significant changes in gene expression and histone posttranslational modifications; and increased flux through multiple metabolic reactions, including those of serine and methionine (PMID: 35508495). Interestingly, the upregulated genes are involved in neural development, axon guidance, neural crest pathways and stemness. Neural genes are highly expressed in Triple Negative Breast Cancers (TNBCs) especially in the C2 cluster. Given that mammary stem/progenitor cells have distinct metabolic properties compared to other mammary cell subsets, we hypothesized that upon lipid exposure, stem-like cells have a survival advantage, and that lipid induces epigenetic reprogramming into neural-like cells which may foster a malignant transformation. Methods. To interrogate potential mechanisms linking lipids and epigenetic reprogramming, we performed CUT&RUN for H3K4me3 and H3K27me3 in non-transformed, estrogen and progesterone receptor negative MCF-10A cells cells exposed to vehicle or octanoic acid (OA) for 24 hrs. Peaks were called using MACS2 and differential peaks identified with DiffBind. Differentially expressed genes affected by OA (PMID: 28263391) were compared with target genes from the CUT&RUN. To determine the contribution in OA exposed cells of serine and methionine metabolism to S-adenosyl methionine (SAM), 13C-glucose tracing was performed. The Aldefluour assay was used to identify stem-like (ALDH+) cells in lipid-exposed MCF-10A cells. To determine if lipid-exposed cells adopt a neural-like phenotype, MCF-10A cells were grown on Poly-D-Lysine/Laminin (PDL/LM) coated plates. Results. A total of 661 differential peaks were identified for H3K4me3 (FDR < 0.05) encompassing TNBC C2 markers (NRTN, CHRM3) and genes involved in neuron differentiation, axonogenesis (NGFR, NGF, FOXA2), neural crest migration (NTRK2, MMP2) and EMT (DLL4, MMP15). Approximately 73% of H3K4me3 OA-associated peaks encompass regulatory regions of genes that experienced a significant increase in gene expression with OA exposure (FDR < 0.01), including NGFR (log2 FC = 11.7), FOXA2 (log2 FC = 11.6), NGF (log2 FC = 8). Twelve H3K27me3 peaks were significantly enriched in vehicle (FDR < 0.05) and associated with increased gene expression in OA, among them were the stem cell markers LGR6 (log2 FC = 1.9) and PLAG1 (log2 FC = 2.8). 13C stable isotope tracing revealed that in presence of OA, glucose contributes to increased fractional abundance of the SAM M+1 isotopologue (adj p = 0.003) indicating that carbons derived from the serine synthesis pathway are used for re-methylation of homocysteine to methionine. Vehicle treated cells growing on PDL/LM plates assumed an epithelial phenotype while OA-treated cells adopted a neurite-like phenotype. Upon OA treatment the percentage of ALDH+ cells increased by a minimum of 10%. Conclusions. The increase of fractional abundance of SAM and the upregulation of neural genes regulated by H3K4me3 as well as the enrichment of ALDH+ cells and the development of a neural-like phenotype in a rich lipid environment, suggests that lipid exposure affects the production of SAM, which results in epigenetic fostered plasticity, leading to reprogramming/selecting cells with a multi-potential embryonic or stem-like state and/or differentiation to a neural/neural crest-like state. To that end, we analyzed the gene expression profiles of epithelial cells from the contralateral unaffected breasts (CUBs) of BC patients and identified a lipid metabolism gene signature, which was enriched in the CUBs of women with ER-negative BC (PMID: 28263391). Subsequent experiment revealed that exposure of non-transformed breast epithelial cells to lipid results in significant changes gene expression, chromatin accessibility and histone posttranslational modifications (PMID: 35508495). Several of the upregulated genes are hallmarks of the various fates of vagal neural crest: Neural, neurogenic and mesenchymal lineages. We hypothesize that lipid exposure imparts a survival advantage of stem-like cells, that lipid-induced epigenetic changes lead to a neural crest-like transcription signature and that these genes are not expressed normally in the breast. Methods: MCF10A cells were exposed to vehicle or octanoic acid (OA) for 24 hours. Gene expression was assayed by RNA-seq and OA responsive genes were identified (PMID: 35508495). Single-cell RNA sequencing (scRNA-seq) data from 14 human reduction mammoplasties (RM) was obtained from a publicly accessible data set (PMID: 34031589). The scRNA data was clustered and identified by unsupervised clustering (Seurat, v3.4.1) using cell-type markers curated using Supplementary table 2 from (PMID: 34031589). The bulk RNA-seq data from the OA treated cells was deconvoluted to cell-lineages using Bisque. The most significant upregulated VNC neural/neuronal/mesenchymal genes from the gene expression analysis were then plotted on the lineage clusters using FeaturePlot to determine if these markers are found in the normal breast epithelium, or other cell lineages. The plots were then filtered and re-clustered to look at basal-luminal cell types only. We utilized a second resource, a web-application for snATAC-seq data from various stages of mouse mammary development developed by the Wahl lab (https://wahl-lab-salk.shinyapps.io/Mammary_snATAC/), to query these same genes. Results: Deconvolution of the bulk RNA-sequencing data revealed a transition to a pericyte transcription program following exposure to OA. Nerve growth factor (NGF) was found to be expressed in pericytes while nerve growth factor receptor (NGFR) was found within the basal epithelial cell lineage. Genes overexpressed in the VNC neural cluster and overexpressed in the OA-exposed MCF10 cells, PPP1R1C (2.39x, adj p=1.6E-5), FOXD3 (6.7x, adj p=6.7E-10), DIO3 (5.9x, adj p=3.9E-6) and MOXD3 (4.2x, adj p=1.5E-23) all evidence little to no expression in the normal breast but are observed in murine fetal mammary stem cells. Schwann cell precursor (SCP) markers, CDH19 and ROPN1, significantly upregulated in OA treated cells: 5.4x and 1.6x, respectively, exhibited low expression in luminal progenitors but in the mouse were observed in the mammary stroma. CDH19, a gene exclusive to SCPs, is expressed in stroma following murine birth. PRRX1, a key regulator of the VNC mesenchymal cell fate cluster, is 9.3-fold (p adj=4.9E-49) overexpressed in the OA treated cells, expressed strongly in pericytes and stroma and to a lesser extent in basal epithelial cells of the normal human breast and stroma of the murine mammary gland. Conclusions: Treatment of non-transformed mammary epithelial cells with lipid, specifically OA, shows significant upregulation of multiple VNC genes associated with both neural and mesenchymal fate. scRNA-seq from RM patients reveals that many of these same markers are either found in non-epithelial cell clusters or are found with low expression in luminal mammary lineages (both progenitors and mature).
