法医解剖後にアスベストの健康被害申請を行うために医学的検査を実施した事例を経験した．事例は，アスベスト作業歴のある60歳代の男性で，受診歴や健診歴はない．同居人は「数ヶ月前から咳や呼吸苦を生じて死亡した．」と話しているが，死因不明のために法医解剖に付された．左右の壁側胸膜に広範囲にわたる胸膜プラークと左右の肺に多発腫瘤を認め，死因を「肺癌」と診断した．後日，遺族から「石綿救済法」によるアスベストの健康被害申請のための医学的検査の依頼があり，石綿小体計測を行ったところ，乾燥肺重量1gあたり石綿小体を4,860本検出したものの認定に必要な5,000本は検出できなかった．石綿小体は病変部を含まない，肺実質の末梢側に蓄積されやすいとされている．解剖時に摘出・保存した肺組織は中枢側の病変部周囲であったため，計測結果がより低値になった可能性が考えられた．我が国ではアスベスト関連肺疾患発症者や法医解剖体数が増加している．法医解剖時に原発性肺癌を認めた場合には，アスベストの健康被害申請が行われる可能性を考慮して，肺の適切な部位の採取・保存を周知する必要があると考える． We had a forensic autopsy case that required additive pathological examination for the asbestos-related lung disease compensatory application afterwards. A man in his sixties with a history of occupational asbestos inhalation who had neither visited a hospital nor received a physical examination received forensic autopsy because of his death from unknown cause. An inmate said, “He developed cough and dyspnea, and died in the progression of the symptoms.” The autopsy revealed widespread pleural plaques on both sides of the parietal pleura and multiple tumors in both sides of the lungs. The cause of death was diagnosed as lung cancer. Additional pathological examination was asked by his family to certify that he had suffered from asbestos-related lung disease in order to apply to the Asbestos-related Damage Relief Law. The Japanese criteria of the compensation law of asbestos-related lung cancer is the detection of more than 5,000 asbestos bodies per gram of dry lung tissue, while his number of asbestos bodies was 4,860. Asbestos bodies were reported to be accumulated in the distal lung parenchyma with no pathological changes. The present lung samples were collected from proximal section around the tumor, which might have made the number of asbestos bodies less than the criteria. Both the number of patients suffering from asbestosrelated lung disease and the number of forensic autopsy cases have increased in Japan. Collecting lung samples from the appropriate lung section is essential and should be noted when the lung cancer is suspected at forensic autopsy in order to apply for asbestos-related lung disease compensation.

1 はじめに : 日本において、アスベスト被害が深刻な社会問題になっている。すでに澤田(2014b)にて示したとおり、アスベスト起因の労働災害は2012年現在で1万659人に上る。これまで筆者は、アスベスト被害の拡大を被害者の量的拡大と地理的拡大という二つの意味でとらえ、拡大の要因を企業間・産業間の生産の分業を基礎とした取引関係の構造分析によって明らかにしてきた。しかし、なぜそもそもアスベストが長期にわたり使用されてきたのであろうか。……, In 1994, the Japan Automobile Manufacturers' Association (JAMA) moved to replace parts containing asbestos by using substitutes, under its own initiative. Meanwhile, however, the construction industry continued to work with materials and parts containing asbestos until they were banned under domestic regulations in 2004. This essay focuses on the asbestos substitution process in the construction industry and building material manufacturers, as well as in the automobile industry and automobile parts manufacturers, conducting a factor analysis of the different responses from the two sectors. The results demonstrate that while the automobile industry and automobile parts manufacturers restricted the usage of asbestos abroad as part of the process of expansion into foreign markets and actively sought to find replacements domestically as well, the majority of other industries were primarily concerned with economic performance and remained in line with domestic regulations, failing to search for replacements.

1 はじめに : 本論文の課題は、日本における建築産業のアスベスト被害拡大の背景を、建築生産システムの特徴と労働安全衛生行政の特徴の分析を通じて考察することにある。……, This paper elucidates the background of asbestos disaster in the building industry, which has become increasingly severe, through an analysis of the features of the building production systems and its administration of occupational safety and health for the construction industry. One feature of the building production system can be found in the labor subcontracting structure and the fact that building is performed by the lowest level of workers. In addition, unlike the machinery industry, many of the workers who are responsible for the work at building sites have their own sets of electrical power tools, and they must use their own measures to protect themselves from asbestos dust. Furthermore, there exists a legal limitation in the guidance provided by the Labor Standards Inspection Office with regards to this feature of the building industry. Japan's building production system and the limitations of its administration of occupational safety and health serve as the backdrop to the expanding damage caused by asbestos in the building industry, and the design of an appropriate legislative system has been sought.

1 はじめに : アスベスト被害は全国で生じており、アスベストが原因とされる中皮腫死者数は2006年に1000人を突破し、2011年は1258人になった。このアスベスト被害の特徴を捉えるため、澤田(2011)は、アスベストのライフサイクルアセスメント(以下、LCAという)にしたがい、アスベスト鉱石の採掘、アスベスト材料・部品の製造、アスベスト材料・部品を用いた完成品の製造・建設、完成品の消費、完成品の廃棄の各段階に対応したアスベスト被害を分類した。澤田(2011)で明らかにしたことは、いま広がっているアスベスト被害の多くが、アスベストに関係する生産分業に起因していたことである。これまで労働災害・公害の発生は、経済学的には不変資本充用上の節約や私的費用の社会的費用化で捉えられてきた。これらの理論は労働災害・公害の個別企業による発生を捉えるうえでは今なお有効だろう。しかし、アスベスト被害は生産の分業を通じて広域に発生している。また、後述のように、アスベスト問題に関わる主体は多くの産業にまたがり、企業規模も大中小零細と多様であった。これらの特徴を経済の側面からどのように捉えることができるだろうか。……, This paper analyzes the trade in typical asbestos materials and components, such as friction components, asbestos building materials, and heat insulation materials, and describes characteristics of the trading relations in this industry with regard to company size. In the asbestos industry, most manufacturers of friction components are major companies that primarily have direct dealings with car manufacturers. Similarly, manufacturers of asbestos building materials have direct dealings with architectural firms including in-house construction firms, and have dealings through warehouse dealers. The major manufacturers of building materials have sales across the country, whereas, the minor ones have sales only in the immediate vicinity of their own factories. Heat insulation materials are provided by major manufacturers directly to industries that make finished products, whereas, most of the minor manufacturers are subcontractors. This paper approaches such trading relations to analyze the variety of manufacturers of asbestos materials and components, which previous papers did not study.

悪性胸膜中皮腫の約85%は胸水貯留で発症し、初期病変から胸水細胞診が陽性になることが多いとされている。今回我々は発症1ヶ月前に悪性胸膜中皮腫に特徴的とされる画像所見に乏しく、約1ヶ月の経過で大量の胸水貯留を来たし、細胞診から検索を進めて確定診断に至った症例を経験したので報告する。症例は70歳代の男性。1ヶ月前のCT所見は右肺S6の小結節程度であったが、2013年6月下旬に呼吸困難感を自覚し、胸部X線写真で右の片側胸水を認め入院となった。胸腔穿刺を行い、ほぼ血液様のものが引けた。胸水は滲出性であり、提出した細胞診でClassVであった。退院してから行ったPET検査では原発不明癌の右肺門リンパ節転移が疑われた。その後気管支鏡検査も行ったが確定診断には至らず、最終的にVATSで組織を採取し、免疫組織学的に確定診断に至った。今回我々は血胸で突然発症した悪性胸膜中皮腫の1例を経験した。悪性胸膜中皮腫の診断のためにはアスベスト曝露の有無を詳細に問診する必要があると思われた。

2006年10月～2013年2月の期間に、当院で経験した悪性中皮腫症例をレトロスペクティブに解析した。患者背景は年齢中央値が66.2歳、性別は男性10例、女性2例であった。病期はI期2例、II期0例、III期2例、IV期6例、不明2例であった。Performance status(PS)はO-1が11例と92%を占めた。病理組織型は上皮型7例、肉腫型1例、二相型2例、不明2例であった。また、6例(50%)で明らかなアスベストの曝露が確認された。治療については外科手術1例、化学療法10例、対症療法1例であった。初期治療で化学療法を施行した10例の詳細は、シスプラチン(CDDP)とべメトレキセド(PEM)の併用療法が8例、PEM単剤1例、他科で施行したドセタキセル(DTX)とゲムシタビン(GEM)の併用療法が1例であった。生存期間中央値(median survival time、MST)は全症例および化学療法群において、それぞれ17.0カ月、18.1カ月であった。化学療法10例のうち、CDDP+PEMで治療した2例で完全奏功(complete response、CR)が得られた。またCDDP+PEM群において、奏功率(response rate、RR)は37.5%、疾患制御率(disease control rate、DCR)は75.0%であった

1 はじめに : 一般に公害問題は、「資本主義の生産関係に付随して発生する社会的災害」であり、公害対策の節約を基礎とした「労働災害や職業病の地域への拡大」である。足尾鉱毒事件と熊本水俣病を取り扱った研究では、日本の公害を「地域ぐるみの人間と環境の収奪であり、人体被害はその結果としての地域社会の破壊の頂点」と規定している。しかしながら、クボタショックによって顕在化したアスベスト問題は、本論で論じるようにアスベストが生産の社会的分業構造に沿って広く全国へと普及したことにより、従来の公害・労働災害よりも被害が広域化している。アスベスト問題は、たとえば(株)クボタ(以下、クボタと略)旧神崎工場周辺住民への被害の場合には、足尾鉱毒事件や二つの水俣病、イタイイタイ病と類似した個別企業による公害として捉えることができる。……, This paper explores the reasons for the extensive spread of asbestos-related diseases across Japan. This phenomenon was caused by the use of asbestos in a number of industries, such as the asbestos, construction, and motor industries, in the second half of the twentieth century. These industries were interrelated on the basis of the social division of labor and the asbestos dust they emitted, which spread to the workplace of laborers. Since they were caused by inhaling asbestos dust, asbestos-related diseases appeared to be an industrial injury at first. At the same time, the dust spread extensively with the growth of the asbestos-related industries. Considering the interrelated structure of asbestosrelated industries, it can be concluded that asbestos-related diseases had spread due to the production activities of these industries.

A 70-year-old man with a history of doing wiring work for 30 years was admitted to our hospital because of lower abdominal pain and ascites. Four months before admission he underwent cholecystectomy due to cholelithiasis and cholecystitis.At that time there were no macroscopic abnormal findings in the peritoneum, surface of the gall bladder,liver,stomach or intestine.We suspected peritonitis carcinomatosa,but ascetic fluid was light yellow and clear, and cytology showed no malignant cells. Computed tomography (CT)showed no malignant findings in visceral organs including the liver, pancreas, spleen, kidney and lung, but prominent ascites and thickening of the peritoneum and omentum were observed.After admission the ascites increased significantly and pleural effusion appeared. The patient died due to respiratory insufficiency two weeks from the onset of disease. Autopsy findings revealed sarcomatoid mesothelioma of the peritoneum, omentum and mesentery.Asbestos bodies were found in the lung,and the concentration of hyaluronic acid in ascites was very high. In conclusion, in cases with rapidly progressive and refractory ascites, asbestos-induced mesothelioma should be included in the differential diagnosis., Article, 信州医学雑誌. 58(2): 69-74 (2010)

空気中に浮遊するアスベストの長期吸入による健康影響については国内外で広く研究されている. しかし, 水道水を介したアスベスト(石綿)の経口摂取による発がん性については, 不明な点が多い. 本調査では, 1.主に日本(特に新潟県)や海外のアスベスト・セメント水道管の布設状況を把握した上で, 2.水道水を介したアスベスト摂取とがん罹患との相関について国内外の諸論文を検証して, その因果関係の有無を評価した. その結果, 1.アスベスト・セメント管の布設割合は, 全国的にも, 新潟県でも近年, 低下傾向にあることがわかった. しかし依然として, 全国では18,710km (2003年度), 新潟県では937km (2004年度), と多く残存している. 論文検索の結果, 2.水道水を介したアスベスト摂取と消化器系がんの罹患との間に有意な関係が一部の論文で認められたが, 多くの論文では関係性に乏しいか, あるいは,積極的には肯定できないということであった. 今回調べた文献では, 呼吸によるアスベストの吸入, 調査期間中の転入や転出, 水道水中に含まれるその他の有害物質(砒素やトリハロメタンなど)等の多因子による相乗的な影響を加味していない. また, 呼吸器系で吸入したアスベストによる肺がんや悪性中皮腫の場合,発がんまでの潜伏期間が長いのに比して, 今回の論文での観察期間は短期であり, それらの関係性は不明である. 従って, 関係ありと断言することはできない.

目的 当科では腹膜中皮腫に対してPeritonectomyを用いた集学的治療(術中温熱化学療法(HIPEC),術後早期腹腔内化学療法(EPIC))を施行している.この治療結果を検討し,他施設と比較した.方法 1999年～2006年にPeritonectomyを施行した腹膜中皮腫11症例を対象に検討を加えた. 11例中, 3例に2回, 1例に3回,計16回のペリトネクトミーを, 9例に11回のHIPECを, 4例に6回のEPICを施行した.結果 男女比は7対4,平均年齢52.8±18.2,明らかなアスベスト暴露を4例で認めた. 11例中1例が周術期死亡, 3例が原病死, 7例が生存中である.統計学的検討では生存期間と2回目のPeritonectomy施行に10%の有意差を認めた(P=0.0734). Kaplan-Meier生存曲線からの1, 3, 5年生存率は72.7, 62.3, 62.3%であった.結論 我々と他施設の生存率に文献的考察を加えると, Peritonectomy集学的治療は腹膜中皮腫患者の予後の向上に寄与すると考えられる., AIMS : Recently, the issue of Asbestos-related disease has increased dramatically though out the world. Deaths from mesothelioma are often caused by asbestos exposure. The aim of the study was to present our unit's experience in the treatment of peritoneal mesothelioma between 1999 and 2006. In addition, the data was assessed and validated compared with other peritonectomy units. METHODS : Records of 11 patients with peritoneal mesothelioma were reviewed retrospectively. All patients were treated by cytoreductive surgery with peritonectomy procedures, 4 patients had second and 1 patient had third peritonectomy (in total 16 times peritonectomy). Hyperthermic Intraperitoneal Perioperative Chemotherapy (HIPEC) was performed 11 times across 9 patients. Early Postoperative Intraperitoneal Chemotherapy (EPIC) was performed 6 times across 4 patients. RESULTS : 4 female and 7 male patients with a mean age of 52.8±18.2 years old were studied. Asbestos exposure was recorded in 4 patients. Overall projected survival at 1, 3 and 5 years were 72.7, 62.3 and 62.3 per cent from Kaplan-Meier survival curve. The most significant positive predictive factor of survival was second peritonectomy (P=0.0734). CONCLUSION : Our unit's survival rate agreed with the experience from other units and supports peritonectomy with HIPEC and EPEC as the gold standard in the treatment of peritoneal mesothelioma worldwide.

1 はじめに : 本稿の課題は、アスベスト製品の一大産地であった大阪府の泉南地域を事例として、アスベスト粉塵対策の実態とその問題点を集塵装置に着目して明らかにすることである。……

平成17年8月、大学からの調査で、本校東館3階屋根裏に吹付けアスベストの使用が発見されました。11月に大気調査の結果が判明し、大学より東館3階閉鎖の指示が出されました。追加調査の結果、アスベスト汚染が東館・西館の校舎全体に及んでいることが判明し、混乱に拍車がかかりました。学びの場である教室・特別教室が次々に使用不可能になるという未曾有の学校危機に直面することとなりました。平成17年8月より平成18年4月までの本校でのアスベスト対策の記録を残します。

1996年6月から2008年10月までの間に市立室蘭総合病院呼吸器科にて病理診断を得たびまん性悪性胸膜中皮腫11例を対象に年齢、性別、アスベスト曝露歴、組織型、診断方法、診断までの期間、予後を検討した。結果：男性8名 女性3名、平均年齢66.6歳。初診時の主訴は発熱が多く、アスベスト曝露歴は8例に認めていた。診断法としては開胸生検3例・胸腔鏡下生検2例、経皮生検が5例、剖検症例が1例であった。組織型は上皮型が4例、肉腫型、二相型が2例、線維形成型が1例、そのほかは不明であった。当科受診時から診断までの期間が中央値87日（平均129.6日）を要していた。治療は手術・化学療法・放射線療法を行ったのが3例、化学療法が3例、支持療法のみ行った症例が5例という結果であった。結論：アスベスト曝露歴を詳細な問診にて聴取することでリスク群を評価し、アスベスト曝露歴のある原因不明胸水に対して積極的に胸膜生検を行うべきである。

Two kinds of pulp devitalizer, "Neo-Arsen Black (NAB)" and "Neo-Paraform Paste (NPP)", have been modified to change from asbestos to polyethylene fibers as their primary component because of health risks associated with the former. The effect of pulp devitalization with both "Modified Neo-Arsen Black (M-NAB)" and "Modified Neo-Paraform Paste (M-NPP)" were histopathologically evaluated, using 160 teeth of 10 dogs. The results were compared with the findings of the specimens made using two types of original NAB and NPP as controls. When evaluated, these histopathological data showed that both types of M-NAB and M-NPP have the same effect on the pulp as both original NAB and NPP, and are valuable as pulp devitalizers.

Polyethylene fibers, which could be substituted for the asbestos component in certain pulp devitalizers (Neo-Arsen Black and Neo-Paraform Paste), were embedded subcutaneously in rats, and the tissue reactions were studied histopathologically and electron microscopically. These results were compared with similar observations made using asbestos as a control. Whereas the latter materials were encapsulated, the polyethylene fibers were phagocytized by foreign body giant cells and decreased in quantity with the passage of time. Based upon these observations and a review of the literature on tissue reactions to these materials, it was determined that polyethylene fibers are biologically useful as a substitute for asbestos.
