Biologicals have transformed the management of severe disease phenotypes in asthma, atopic dermatitis, and chronic spontaneous urticaria. As a result, the number of approved biologicals for the treatment of atopic diseases is continuously increasing. Although atopic diseases are among the most common diseases in the reproductive age, investigations, and information on half-life, pharmacokinetics defining the neonatal Fc receptors (FcRn) and most important safety of biologicals in pregnancy are lacking. Given the complex sequence of immunological events that regulate conception, fetal development, and the intrauterine and postnatal maturation of the immune system, this information is of utmost importance. We conducted a systematic review on biologicals in pregnancy for indications of atopic diseases. Evidence in this field is scare and mainly reserved to reports on the usage of omalizumab. This lack of evidence demands the establishment of a multidisciplinary approach for the management of pregnant women who receive biologicals and multicenter registries for long-term follow-up, drug trial designs suitable for women in the reproductive age, and better experimental models that represent the human situation. Due to the very long half-life of biologicals, pre-conception counseling, and health care provider education is crucial to offer the best care for mother and fetus. This position paper integrates available data on safety of biologicals during pregnancy in atopic diseases via a systematic review with a detailed review on immunological considerations how inhibition of different pathways may impact pregnancy.

Background: The clinical effects of smartphone-based interventions for bipolar disorder (BD) have yet to be established. Objectives: To examine the efficacy of smartphone-based interventions in BD and how the included studies reported user-engagement indicators. Methods: We conducted a systematic search on January 24, 2022, in PubMed, Scopus, Embase, APA PsycINFO, and Web of Science. We used random-effects meta-analysis to calculate the standardized difference (Hedges' g) in pre-post change scores between smartphone intervention and control conditions. The study was pre-registered with PROSPERO (CRD42021226668). Results: The literature search identified 6034 studies. Thirteen articles fulfilled the selection criteria. We included seven RCTs and performed meta-analyses comparing the pre-post change in depressive and (hypo)manic symptom severity, functioning, quality of life, and perceived stress between smartphone interventions and control conditions. There was significant heterogeneity among studies and no meta-analysis reached statistical significance. Results were also inconclusive regarding affective relapses and psychiatric readmissions. All studies reported positive user-engagement indicators. Conclusion: We did not find evidence to support that smartphone interventions may reduce the severity of depressive or manic symptoms in BD. The high heterogeneity of studies supports the need for expert consensus to establish ideally how studies should be designed and the use of more sensitive outcomes, such as affective relapses and psychiatric hospitalizations, as well as the quantification of mood instability. The ISBD Big Data Task Force provides preliminary recommendations to reduce the heterogeneity and achieve more valid evidence in the field.

Nonischemic-dilated cardiomyopathy (NIDCM) is an entity that gathers extremely heterogeneous diseases. This awareness, although leading to continuous improvement in survival, has increased the complexity of NIDCM patients' management. Even though the endorsed 'red-flags' approach helps clinicians in pursuing an accurate etiological definition in clinical practice, it is not clear when and how peripheral centers should interact with referral centers with specific expertise in challenging scenarios (e.g. postmyocarditis and genetically determined dilated cardiomyopathy) and with easier access to second-line diagnostic tools and therapies. This position paper will summarize each step in NIDCM management, highlighting the multiple interactions between peripheral and referral centers, from first-line diagnostic workup and therapy to advanced heart failure management and long-term follow-up.

OBJECTIVES: Excluding oligo-, di-, monosaccharides and polyols (FODMAPs) from the diet is increasingly being used to treat children with gastrointestinal complaints. The aim of this position paper is to review the available evidence on the safety and efficacy of its use in children and provide expert guidance regarding practical aspects in case its use is considered. METHODS: Members of the Gastroenterology Committee, the Nutrition Committee and the Allied Health Professionals Committee of the European Society for Pediatric Gastroenterology Hepatology and Nutrition (ESPGHAN) contributed to this position paper. Clinical questions regarding initiation, introduction, duration, weaning, monitoring, professional guidance, safety and risks of the diet are addressed. A systematic literature search was performed from 2005 to May 2021 using PubMed, MEDLINE and Cochrane Database of Systematic Reviews. In the absence of evidence, recommendations reflect the expert opinion of the authors. RESULTS: The systematic literature search revealed that the low-FODMAP diet has not been comprehensively studied in children. Indications and contraindications of the use of the diet in different pediatric gastroenterological conditions are discussed and practical recommendations are formulated. CONCLUSIONS: There is scarce evidence to support the use of a low-FODMAP diet in children with Irritable Bowel Syndrome and no evidence to recommend its use in other gastrointestinal diseases and complaints in children. Awareness of how and when to use the diet is crucial, as a restrictive diet may impact nutritional adequacy and/or promote distorted eating in vulnerable subjects. The present manuscript provides practical safety tips to be applied when the low-FODMAP diet is considered in children.

Cardiomyopathies (CMPs) are myocardial diseases in which the heart muscle is structurally and functionally abnormal in the absence of coronary artery disease, hypertension, valvular disease and congenital heart disease sufficient to cause the observed myocardial abnormality. Thought for a long time to be rare diseases, it is now clear that most of the CMPs can be easily observed in clinical practice. However, there is a group of specific heart muscle diseases that are rare in nature whose clinical/echocardiographic phenotypes resemble those of the four classical morphological subgroups of hypertrophic, dilated, restrictive, arrhythmogenic CMPs. These rare CMPs, often but not solely diagnosed in infants and paediatric patients, should be more properly labelled as specific CMPs. Emerging consensus exists that these conditions require tailored investigation and management. Indeed, an appropriate understanding of these conditions is mandatory for early treatment and counselling. At present, however, the multisystemic and heterogeneous presentation of these entities is a challenge for clinicians, and time delay in diagnosis is a significant concern. The aim of this paper is to define practical recommendations for diagnosis and management of the rare CMPs in paediatric or adult age. A modified Delphi method was adopted to grade the recommendations proposed by each member of the writing committee.

[EN] The family album is understood as a book whose empty pages are prepared to house the photographs of the most important moments of the family that owns it. Its main characteristic is that these snapshots are intended to narrate the events that took place in the family nucleus and that would form part of its identity; so that they would later form part of the legacy that is usually passed on from generation to generation. Therefore, the photographic album would be the life story of some families, told and captured in images that narrate and portray the memory, the past events. But it seems that nowadays the concept of the family album has begun to be transformed by post-photography and digitalization which means that the image is losing its power and, in turn, causing a great lack of control over it. Due to the massification and excessive consumption of images, we could find ourselves in a situation in which photography would need to transform itself, in order to keep pace with digital advances and mediatisation. In other words, a paradigm shift from analogue to digital began on the medium that would end up transforming the album as well. The album, as a generator of stories, creates the visual and historical narrative that is told by the protagonists who appear in the images, and by those who narrate it with their cameras. But in all this, it is worth highlighting the role of women. From the interest that the female part of the families could have in bringing together their members and safeguarding special moments, she would be key in the creation, and responsible for the durability of this object. Thus, the motivation for this work is to carry out an investigation into the family album, both on a practical and technical level, in order to then organise an exhibition project with works by contemporary women artists, due to their prominence in relation to this object. In addition to describing the main formal transformations in the composition of family photography, studying the change in the relationship and use of the camera. Considering this hypothesis: digitalisation may have changed the way of production and collection of the family photographic album to scenarios of accessibility, in which women would have a relevant role in this process, which would also be reflected in their position as artists. The album, as a time capsule, or narrative element of family history, acquires meaning with the appearance of its protagonists in front of the camera. The conception of the classical family album may have changed over the years and this could be seen in the contemporary family album.

Plans and actions in an organization initially starts with decision making, which is done within the administration and management chambers. This encompasses laying structures of hierarchy; effective strategies and plans of what is to be done; how to achieve it including the required resources and procedures; and considering who is responsible in achieving the planned agenda within an organization. Administration and management do not mean being “a manager sitted in an office”, rather it can mean one’sself being responsible for whatever duty he/she is assigned to do. According to Fayol’s administrative principles; Luther Gulick and Lyndall Urwick’s edited papers in the science of administration; and Focal Organizational Variables, considered in this paper all contends that administration and management sections in any organization are assigned to responsibilities they must effectively perform for the efficiency and progress of their organizations. Effective management of an organization gives a high output even with limited resources and, or without wasting the available resources. An administrator has to be well aware of his/her position as almost every member looks on to what is practiced in the “administration section/office”. Poor administrators and managers are of a great setback to any organization’s efficient production and progress. Although basic statistics are covered during the Preventive Medicine posting in the majority of medical schools in India there is no attention paid to research. However, globally many leading universities encourage their undergraduates to do research. A classic example of this was in 1923 when Charles Best was an undergraduate at the University of Toronto, he together with Banting and Mcleod, discovered insulin. Banting shared the Nobel Prize that he was subsequently awarded with his student Best. Another Nobel laureate, Alan Hodgkin, won the Nobel Prize in 1972 for his work on nerve transmission that he had begun as an undergraduate in Cambridge, England.

Background Different hands-on interventions to protect women from severe perineal tears after birth have been widely implemented. Evidence to support the routine use of hands-on interventions to reduce severe tears is mainly based on aggregated data from observational studies. Aim To critically discuss the current evidence for the implementation of hands-on intervention as a routine practice to protect women from severe tears after birth. Discussion Observational studies have been used to justify the routine use of hands-on intervention to protect women from severe perineal tears despite randomized controlled trials and systematic reviews showing lack of benefit. There is strong evidence supporting the slow speed at the time of birth to prevent severe perineal tears. While hands-on intervention does reduce the speed of birth, it may have a negative effect on the birth process, on neonatal outcomes and women’s agency. Conclusion Evidence-based practice requires sufficient evaluation of interventions before being implemented in clinical practice as well as valuing the level of evidence when making clinical decisions. Evaluation of hands-on interventions to protect women from severe perineal tears must include not just one outcome of interest, but also an assessment of how the intervention interferes with the normal mechanism of birth, and how it affects neonatal outcomes and the autonomy of women.

Acute and chronic physical exercises may enhance the development of statin-related myopathy. In this context, the recent (2019) guidelines of the European Society of Cardiology (ESC) and the European Atherosclerosis Society (EAS) for the management of dyslipidemias recommend that, although individuals with dyslipidemia should be advised to engage in regular moderate physical exercise (for at least 30 min daily), physicians should be alerted with regard to myopathy and creatine kinase (CK) elevation in statin-treated sport athletes. However it is worth emphasizing that abovementioned guidelines, previous and recent ESC/EAS consensus papers on adverse effects of statin therapy as well as other previous attempts on this issue, including the ones from the International Lipid Expert Panel (ILEP), give only general recommendations on how to manage patients requiring statin therapy on regular exercises. Therefore, these guidelines in the form of the Position Paper are the first such an attempt to summary existing, often scarce knowledge, and to present this important issue in the form of step-by-step practical recommendations. It is critically important as we might observe more and more individuals on regular exercises/athletes requiring statin therapy due to their cardiovascular risk.

Background: Nowadays, safety management plans have an effective role in reducing the incidence of accidents. Therefore, this study aimed at investigating the role of safety management systems on accidents and the status of safety performance indicators in a combined cycle power plant in 2011. Materials and Methods: This descriptive-analytical research was carried out in two stages in all Yazd Combined Cycle Power Plant units on accident victims from 2004-2011, based on the documentation of the previous years and using census sampling. The Accident Frequency Rate (AFR), Accident Severity Rate (ASR), and Frequency Severity Rate (FSI) indices were derived from the OSHA standard using the formulas and data obtained from the completed questionnaires and interviews with the people. Results: The total number of accidents was 287 cases through the studied period (8 years). The results showed a significant relationship between age and work experience, as well as between age and marital status. The main causes of accidents were unsafe acts. The lowest level of personal protective equipment was used in 2004 (15.2%), whereas the highest rate was used in 2011 (30%). After the implementation of the Integrated Management Systems (IMSs), accident indicators showed a decreasing trend regarding the washing time. Also, the amount of AFR, ASR and FSI indices of the accidents in terms of work shift indicated a significant decrease in 2004 than after the implementation of the safety management systems. Conclusion: According to the results, due to the positive impact of safety programs (IMS), incident indicators had a downward trend and unsafe acts were decreased.

Epithelioid hemangioendothelioma (EHE) is an ultra-rare, translocated, vascular sarcoma. EHE clinical behavior is variable, ranging from that of a low-grade malignancy to that of a high-grade sarcoma and it is marked by a high propensity for systemic involvement. No active systemic agents are currently approved specifically for EHE, which is typically refractory to the antitumor drugs used in sarcomas. The degree of uncertainty in selecting the most appropriate therapy for EHE patients and the lack of guidelines on the clinical management of the disease make the adoption of new treatments inconsistent across the world, resulting in suboptimal outcomes for many EHE patients. To address the shortcoming, a global consensus meeting was organized in December 2020 under the umbrella of the European Society for Medical Oncology (ESMO) involving >80 experts from several disciplines from Europe, North America and Asia, together with a patient representative from the EHE Group, a global, disease-specific patient advocacy group, and Sarcoma Patient EuroNet (SPAEN). The meeting was aimed at defining, by consensus, evidence-based best practices for the optimal approach to primary and metastatic EHE. The consensus achieved during that meeting is the subject of the present publication., Highlights • This consensus paper provides key recommendations on the management of epithelioid hemangioendothelioma (EHE). • Recommendations followed a consensus meeting between experts and a representative of the EHE advocacy group and SPAEN. • Authorship includes a multidisciplinary group of experts from different institutions from Europe, North America and Asia.

The management of inflammatory bowel disease [IBD] is complex, and requires tight control of disease activity, close monitoring to avoid treatment side effects, health care professionals with expertise in IBD, and an interdisciplinary, holistic approach. Despite various efforts to standardise structures, processes, and outcomes,1–8 and due to the high variability at the local, national, and international levels, there are still no clear definitions or outcome measures available to establish quality of care standards for IBD patients which are applicable in all contexts and all countries. For this reason, the European Crohn’s and Colitis Organisation [ECCO] supported the construction of a list of criteria summarising current standards of care in IBD. The list comprises 111 quality standard points grouped into three main domains [structure n = 31, process n = 42, outcomes n = 38] and is based on scientific evidence, interdisciplinary expert consensus, and patient-oriented perspectives. The list of proposed criteria is intended to represent the position of ECCO regarding the optimum quality of care that should be available to patients. Since health care systems and regulations vary considerably between countries, this list may require adaptation at local and national levels. It is recognised that not all these criteria that have been identified as optimal will be available in every unit. However, ECCO will continue its efforts to develop and coordinate projects and initiatives that will help to guarantee optimal quality of care for all IBD patients.

Computational analysis that demonstrates biased representationsrelated to ethnicities has been widely covered in literature(see, for example, Garg et al. 2018, Tripodi et al. 2019, Lucy etal. 2020). Previous research in computational approaches has revealeddiachronic change of concepts in general (Tahmasebi et al.2018) and for the representation of antisemitism, in particular (Tripodiet al. 2019). However, literature that shows how local representationsof ethnic minorities change through contexts revealedin museum catalogues and metadata, how they are tagged, contextualisedand processed through museum databases, and consequentlywhat kind of knowledge is produced and/or retrieved, isrelatively scarce. In this paper we demonstrate how conceptual representationsrelated to Jews and Armenians fluctuate, dependingon where contexts related to these ethnic minorities are produced. The aim of this paper is to show how the analysis of museumcatalogues and online museum collections can reveal the differenceover geographic areas in dominating attitudes and multipleperspectives in the perception of minority cultures. We comparesearch results related to Jews and Armenians for the British Museumin London and the State Historical Museum in Moscow. The museums were chosen as they both represent an ‘imperial archive’(Barringer and Flynn 1998, p. 11, Khazanov 2000), theyhave collections that include multiple historical objects and theyare both located in capital cities for the two countries.Their onlinedigital collections produce sufficient results to analyse the representationof minorities and the perspectives of these two majormemory institutions on what is deemed an ethnic minority.

Background This short paper will look at the work of the Evolving Hands project which is undertaking three case studies ranging across document forms to demonstrate how TEI-based Handwritten Text Recognition (HTR) workflows can be iteratively incorporated into curation by under-resourced cultural institutions. The case studies include materials that range from: 19th-20th century handwritten letters from the UNESCO Gertrude Bell Archive, 18th century German, 20th century French correspondence, and a range of printed materials from the 19th century onward in English and French. A joint case study converts legacy complex printed material of the Records of Early English Drama project. By covering a wide variety of periods and document forms the project has a real opportunity to foster responsible and responsive support for cultural institutions. Smaller cultural institutions often do not have the resource for the investigation of different methods in what remains a swiftly changing field of content enhancement. The short paper will introduce the project and report on the work done so far, but focus more intently on the Newcastle University Case Study on the Gertrude Bell Archive (given the limited time) and the methodologies used. Newcastle University Case Study -- The Gertude Bell Archive This case study uses Newcastle Special Collection's UNESCO Gertrude Bell Archive (http://gertrudebell.ncl.ac.uk/), which document the activities of the explorer, archaeologist, and political agent who was instrumental in establishing the Kingdom of Iraq in 1921. (Barr 2012, Bell 1927, Richmond 1937, and Dodge 2003) Bell is the subject of plays, documentaries, feature films, and recently was nominated as a BBC 20th Century Icon. (c.f. Barry 2017, Marozzi 2015, and Sluglett 2007) A separate centenary project is digitizing and cataloguing her archive of diaries, letters, and photographs. Piggybacking on that, we have trained an HTR base model of Bell’s hand and applied it to carefully-selected content which has then been enriched in Transkribus to provide rich insights into political events in 1921. We are using the Transkribus platform’s tagging feature to enhance the initial HTR transcriptions from which we generate (and then further up-convert) TEI P5 XML. The outputs from this will be displayed on the new Gertrude Bell research website, eventually alongside digital images presented using a IIIF viewer. (c.f. Kudella 2019) Bucknell University Case Study -- Scholarly Production at Scale The Bucknell case study centres on processes used across multiple discrete projects by staff with a range of digital experience. These projects represent different models for testing the HTR to TEI conversion process. Their sources are drawn from Bucknell's Special Collections and research of faculty working with archives in the US, UK, Europe, and Asia. They include scribal hands, life papers, correspondence, and semi-legible typed government files from 1700-1990 and are in English, French, German, and Maithili. This case study is directly benefiting multiple projects at the university, and this workflow is optimised for sharing with smaller cultural institutions around the world. Joint Newcastle/Bucknell Case Study -- Transforming REED Print Collections Cummings (AHRC PI) and Jakacki (NEH PI) have collaborated on a case study converting collections produced by the Records of Early English Drama (http://reed.utoronto.ca) project that has published since 1979 edited documentary records of pre-1642 performance in premodern England, Scotland and Wales. However, the semantic information provided in the print collections, through the use of special symbols and formatting, is lost in OCR. Earlier tests using HTR by Jakacki and Cummings demonstrated that these distinctions can be preserved with HTR to TEI workflows. The project is documenting shared workflows for consistent upconversion into viable materials ready to enter the REED project's digital publication workflow. (c.f Chagué et al 2022) This has the potential to be of use for all the other REED legacy print volumes (well over 20,000 pages of rich scholarly material). Project Methodology The project methodology, being based on individual case studies, takes a similar approach across different forms of work but also has intentional differences. In each case Transkribus is being used for HTR, though other providers of HTR were tested. (c.f. Stokes et al 2021) The generated text output is converted to TEI either through the basic Transkribus conversion or through standalone XSLT stylesheets. (c.f. Nockels et al 2022) However, it is the differences in methodology which are more interesting. In the Gertrude Bell case study the Transkribus Web/Lite version has been used to provide as much tagging as possible through that interface, in the Bucknell versions the desktop version has been used but additional tagging provided in Oxygen. The REED case study uses Transkribus Web/Lite and more detailed up-conversion stylesheets to protect formatting-based structures. Overall the project is documenting these workflows in easy to understand how-to guides alongside other materials such as conversion scripts to make it easier for under-resourced cultural institutions to undertake such endeavours themselves.

The consensus achieved during that meeting is the subject of the present publication.

Managerial Leadership for Librarians: Thriving in the Public and Nonprofit World by G. Edward Evans and Holland Christie is an excellent primer for library students and professionals who are looking for a broad and comprehensive understanding of the role that leadership and management play in the public and nonprofit library world. The book itself has 20 chapters covering a number of different topics. This may seem overwhelming, but the authors have done a good job of organizing the text around four assumptions. First, the reader must understand the core concepts of managing an organization. Second, the reader must understand the core management functions and activities. From that, the third assumption is possible. The third assumption addresses the landscape in which the managerial leader operates and focuses on different elements of the environment, like governing boards, fundraising, and the like. Finally, the fourth assumption illustrates the importance of the managerial leaders continually seeking to improve their craft. The reason for this is that the better a managerial leader is, the better the organization in which they lead. Using these four assumptions as a guide, it is possible to provide an overview of the book and the underlying conceptual frameworks used for each section.

The strongest and best-documented risk factor for drug hypersensitivity (DH) is the history of a previous reaction. Accidental exposures to drugs may lead to severe or even fatal reactions in sensitized patients. Preventable prescription errors are common. They are often due to inadequate medical history or poor risk assessment of recurrence of drug reaction. Proper documentation is essential information for the doctor to make sound therapeutic decision. The European Network on Drug Allergy and Drug Allergy Interest Group of the European Academy of Allergy and Clinical Immunology have formed a task force and developed a drug allergy passport as well as general guidelines of drug allergy documentation. A drug allergy passport, a drug allergy alert card, a certificate, and a discharge letter after medical evaluation are adequate means to document DH in a patient. They are to be handed to the patient who is advised to carry the documentation at all times especially when away from home. A drug allergy passport should at least contain information on the culprit drug(s) including international nonproprietary name, clinical manifestations including severity, diagnostic measures, potential cross-reactivity, alternative drugs to prescribe, and where more detailed information can be obtained from the issuer. It should be given to patients only after full allergy workup. In the future, electronic prescription systems with alert functions will become more common and should include the same information as in paper-based documentation.

The concept of "deep mapping," has been increasingly reflected in recent years in the theory and practice of digital humanities. In this short paper we want to present the Polish experience in this matter, including the "Atlas of Holocaust Literature - Warsaw Ghetto," project prepared by our team in 2019.
