Introduction: Chronic kidney disease constitutes a serious health problem all over the world and no therapy has evidenced to be effective to prevent it or to stop its progression, consequently, cytoprotective effect substances to the kidney such as erythropoietin, have been studied.Objective: To evaluate the effect of exogenous erythropoietin as renoprotective in animal models of kidney injury.Material and Methods: PRISMA method was used to conduct a systematic review of original articles written in English and Spanish and published during the last ten years, in which the cytoprotective effect of erythropoietin in animal models of kidney injury was evaluated. LILACS and Pubmed/Medline databases as well as Google Scholar search engine were used.Results: A total of 21 articles were included for synthesis and review. Most of the studies were based on animal models of acute kidney injury due to ischemia/reperfusion; only six research works were carried out in animal models of chronic kidney disease, three of them due to diabetic nephropathy. The use of adult Sprague-Dawley and Wistar male rats was predominant and so the high doses of erythropoietin. The schemes for treatment were diverse and they depended on the type of model studied. The effect in the structure and kidney function as well as biomarkers of apoptosis, cell proliferation, vascular restoration, oxidative stress, and inflammation were determined.Conclusions: Erythropoietin has renoprotective effects in animal models of kidney injury due to antiapoptotic, pro-proliferative, angiogenic, antioxidant, and anti-inflammatory mechanisms which could be considered as further strategies to prevent or decrease acute or chronic kidney disease., Introducción: la enfermedad renal crónica constituye un serio problema de salud al nivel mundial y ninguna terapia ha demostrado eficacia total para prevenirla o detener su progresión, por lo que se investigan sustancias con potencial citoprotector en el riñón, como la eritropoyetina.Objetivo: evaluar el efecto de la eritropoyetina exógena como renoprotector en modelos animales de daño renal.Material y métodos: con el empleo de la metodología PRISMA, se realizó una revisión sistemática de artículos originales, en español e inglés, publicados en los últimos diez años, que evaluaran el efecto citoprotector de la eritropoyetina en modelos animales de daño renal. Se utilizaron las bases de datos LILACS y Pubmed/Medline, y el motor de búsqueda de Google Académico.Resultados: se incluyeron 21 artículos para síntesis y revisión. La mayoría de los estudios se realizaron en modelos de daño renal agudo por isquemia/reperfusión; solo seis en modelos de daño renal crónico, tres de estos por nefropatía diabética. Predominó el empleo de ratas macho Sprague-Dawley y Wistar adultas, y las dosis altas de la eritropoyetina. Los esquemas de tratamiento fueron diversos y dependieron del tipo de modelo estudiado. Se determinó el efecto en la estructura y la función renal, así como en biomarcadores de apoptosis, proliferación celular, restauración vascular, estrés oxidativo e inflamación.Conclusiones: la eritropoyetina tiene efectos renoprotectores en modelos animales de daño renal, por mecanismos antiapoptóticos, pro-proliferativos, angiogénicos, antioxidantes y anti-inflamatorios, por lo que pudiera ser considerada en estrategias futuras para prevenir o reducir la enfermedad renal, tanto aguda como crónica.

Acute kidney injury (AKI) is a major health threat worldwide. The literature on herbal intervention in AKI was searched from English and Chinese databases and reports were critically analyzed in terms of preventing AKI, promoting repair and regeneration, enhancing extrarenal clearance of uremic toxins, and preventing progression to chronic kidney disease (CKD). Altogether, 16 herbal formulae and a few extracts derived from individual herbs were reported to prevent or mitigate AKI in animal models induced by renal ischemia/reperfusion, cisplastin, gentamicin, glycerol, adenine, sepsis or physical exhaustion. Four formulae and six individual herbs were reported to accelerate recovery and/ or to prevent CKD in established AKI animal models. Intrarectal herbal medicines, with or without simultaneous oral administration, were reported in six clinical trials and in an animal model to increase extrarenal clearance of uremic toxins. Additional 13 clinical trials reported oral or intravenous herbal interventions in AKI of different etiologies. Despite recurring problems, notably poor compliance with good practice guidelines for clinical trials and for authentication, naming and quality control of herbal materials, accumulating experimental data on the preventive effects of herbal medicines in AKI look encouraging and urge for better, definitive trials to guide clinical practice. Herbal enemas promoting extrarenal clearance of uremic toxins seem cost-effective, but better clinical evidence is certainly needed before any affirmative recommendation be made for AKI patients without access to dialysis. New frontiers, however, lie in those herbal remedies that promote repair/ regeneration and prevent chronicity after AKI. Recent experimental data suggest that this may be possible.
