The article presents a clinical case of renal replacement therapy in the form of program hemodialysis in a patient with polycystic kidney disease and end-stage renal failure; treatment is carried out in the perioperative period of heart valve replacement., В статье представлен клинический случай проведения заместительной почечной терапии в виде программного гемодиализа у пациента с поликистозом почек и терминальной почечной недостаточностью, лечение которых осуществлялось в периоперационном периоде аллопротезирования клапанов сердца., У статті подано клінічний випадок проведення замісної ниркової терапії у вигляді програмного гемодіалізу в пацієнта з полікістозом нирок і термінальною нирковою недостатністю, лікування яких здійснювалося в періопераційному періоді алопротезування клапанів серця.

Проведено аналіз показників кальцієво-фосфорного обміну залежно від ступеня порушення функціонального стану нирок. Встановлено, що при хронічній хворобі нирок спостерігаються порушення мінерального обміну у вигляді гіпокальціємії, гіперфосфатемії та підвищення Са × Р продукту, що поглиблюються при прогресуванні хронічної ниркової недостатності. У більшості нефрологічних хворих концентрація паратгормону не відповідає цільовим значенням згідно з рекомендаціями NKF-K/DOQI (2003), причому на додіалізних стадіях хронічної хвороби нирок переважає вторинний гіперпаратиреоз, а у хворих на гемодіалізі зростає частка осіб із відносним гіпопаратиреозом., Проведен анализ показателей кальциево-фосфорного обмена в зависимости от степени нарушения функционального состояния почек.
