Healthcare workers have a high risk of burnout. This study aimed to investigate if the numbers of physical symptoms are associated with burnout among healthcare workers. We conducted a cross-sectional survey at a large university in Tokyo, Japan, in 2016. Participants were 1080: 525 faculties and 555 hospital workers. We investigated 16 physical symptoms perceived more than once per week and examined the association between the number of physical symptoms and Copenhagen Burnout Inventory (CBI); work-related (WBO), personal (PBO), and client-related (CBO) burnout. All CBI scores were higher among hospital workers than among faculties: WBO (43 vs. 29), PBO (50 vs. 33), CBO (33 vs. 29). Moreover, the higher the number of physical symptoms perceived, the higher the degree of burnout scores became (trend p-values < 0.001), except for CBO among faculties. Job strain (all except for CBO among hospital workers) and work-family conflict were associated with an increased risk of burnout. Being married (WBO and CBO among faculties), having a child (except for PBO and CBO among faculties), and job support (faculty and hospital workers with WBO and faculties with PBO) were associated with a decreased risk of burnout. Multiple physical symptoms might be useful for identifying high risk individuals for burnout.

Hospital workers have increased exposure risk of healthcare-associated infections due to the frontline nature of their work. Olfactory dysfunction is highly prevalent. The objectives for this investigation are to study the prevalence of long-lasting olfactory dysfunction associated with COVID-19 infection in hospital workers during the first pandemic wave, to identify clinical characteristics and associated symptomatology, and to analyze how many patients with COVID-19 infection had developed olfactory dysfunction during infection and maintained a reduced olfactory function for approximately 10 weeks after diagnosis. Between June and July of 2020, a cross-sectional study was carried out at the Hospital Central de la Cruz Roja San José and Santa Adela in Madrid, Spain. One hundred sixty-four participants were included, of which 110 were patient-facing healthcare staff and 54 were non-patient-facing healthcare staff. Participants were split into three groups, according to COVID-19 diagnosis and presence of COVID-19 related olfactory symptomatology. Participants were asked to complete a structured online questionnaire along with Sniffin’ Stick Olfactory Test measurements. In this study, 88 participants were confirmed for COVID-19 infection, 59 of those participants also reported olfactory symptomatology. The prevalence of COVID-19 infection was 11.35%, and the prevalence for olfactory dysfunction was 67.05%. Olfactory dysfunction associated with COVID-19 infection leads to long-lasting olfactory loss. Objective assessment with Sniffin’ Stick Olfactory Test points to odor identification as the most affected process. Lemon, liquorice, solvent, and rose are the odors that are worst recognized. Mint, banana, solvent, garlic, coffee, and pineapple, although they are identified, are perceived with less intensity. Background: Whether workplace social capital affects employees' mental health is debated. We examined the association between work-unit aggregated social capital and incident purchase of psychotropic medications among employees.Methods: We used data from the WHALE (Well-being in Hospital Employees) cohort study. The study population comprised 21,711 employees without recent psychotropic purchase-history nested within 2283 work units in the Capital Region of Denmark. Employees were invited to participate in a survey in March 2014 (86% response). We assessed workplace social capital by eight items (covering trust/justice and collaboration) and aggregated the mean of responses up to each work unit and categorized the scores into quartiles. Data on psychotropic purchases (antidepressants and anxiolytics/hypnotics/sedatives) were extracted via linkage to national registers. Using two-level mixed-effects survival models, we analyzed the association between work-unit social capital and psychotropic purchases during a one-year follow-up period adjusting for individual-level workplace social capital.Results: Low work-unit social capital was associated with higher purchases of overall psychotropic medications in a dose-response manner (low-versus-high: HR=1.32, 95% CI=1.05-1.65), but this effect attenuated after adjusting for individual-level workplace social capital (HR=1.14, 95% CI=0.88-1.46). Low work-unit social capital was associated with higher purchases of antidepressants (HR=1.78, 95% CI=1.16-2.73) even after adjusting for individual-level workplace social capital (HR=1.69, 95% CI=1.05-2.73).Limitations: Medical doctors/dentists were underrepresented in the data on workplace social capital.Conclusions: Low work-unit social capital may be associated with higher use of antidepressants among health-care employees. Interventions to improve social capital could potentially promote mental health at work in the healthcare setting.

Worker-to-worker (Type III) violence is prevalent in health care settings and has potential adverse consequences for employees and organizations. Little research has examined perpetrator characteristics of this type of violence. The current study is a descriptive examination of the common demographic and work-related characteristics of perpetrators of Type III workplace violence among hospital workers. Analysis was based on documented incidents of Type III violence reported within a large hospital system from 2010 to 2012. Nurses were involved as either the perpetrator or target in the five most common perpetrator-target dyads. Incidence rate ratios revealed that patient care associates and nurses were significantly more likely to be perpetrators than other job titles. By examining characteristics of perpetrators and common worker dyads involved in Type III workplace violence, hospital stakeholders and unit supervisors have a starting point to develop strategies for reducing conflict between workers.

PurposeAccumulating evidence suggests an important role for psychosocial work factors in injury, but little is known about the interaction between psychosocial factors and previous injury experience on subsequent injury risk. We examined the relationships between psychosocial work factors and new or recurrent injury among hospital workers.MethodsWe studied 492 hospital workers including 116 cases with baseline injury and 376 injury-free referents at baseline over follow-up. Job strain, total support, effort-reward imbalance, overcommitment, and musculoskeletal injury at baseline were examined in logistic regression models as predictors of new or recurrent injury experienced during a 2-year follow-up period.ResultsThe overall cumulative incidence of injury over follow-up was 35.6 % (51.7 % for re-injury among baseline injury cases; 30.6 % for new injury among referents). Significantly increased risks with baseline job strain (OR 1.26; 95 % CI 1.02-1.55) and effort-reward imbalance (OR 1.42; 95 % CI 1.12-1.81) were observed for injury only among the referents. Overcommitment was associated with increased risk of injury only among the cases (OR 1.58; 95 % CI 1.05-2.39).ConclusionsThe effects of psychosocial work factors on new or recurrent injury risk appear to differ by previous injury experience, suggesting the need for differing preventive strategies in hospital workers.

Background With the increase in patient activity during the 2009 H1N1 pandemic, came an associated increase in occupational infections of healthcare workers (HCWs). Objectives The aim of this study was to examine factors associated with the transmission of pandemic (H1N1) 2009 among HCWs. Methods A 1:4 matched case-control study by hospital, ward, age, and gender was conducted in HCWs from hospitals in Beijing during February 2010. Cases were diagnosed with pandemic (H1N1) 2009, and controls had neither influenza-like illness nor diagnosis with pandemic (H1N1) 2009 during August 2009 to January 2010. Information during 7 days before symptom onset of case was collected, and controls were queried about the same period. Results A total of 51 cases identified via National Notifiable Infectious Disease Surveillance System participated in this study. Controls were matched to cases for a total of 255 individuals. About 19¡¤6% (10/51) of cases and 26¡¤0% (53/204) of controls recalled they had conducted a high-risk procedure on a patient with pandemic (H1N1) 2009. 72¡¤5% (37/51) of cases and 71¡¤6% (146/204) of controls stated they wore medical masks in ¡Ý80% of working time. Only 5¡¤9% (3/51) and 36¡¤3% (74/204) of cases and controls, respectively, reported receiving pandemic vaccination. Participants receiving pandemic vaccination had a significantly lower risk of infection during the pandemic (OR = 0¡¤150, 95% CI: 0¡¤047-0¡¤479, P = 0¡¤001). The estimated vaccine effectiveness was 85¡¤0%. Conclusions We showed a high vaccine effectiveness of the pandemic vaccine and that vaccination was the only factor significantly associated with risk of infection in HCWs.

BACKGROUND: In Spain, as in other countries, numerous studies on tobaccoism have been carried out. The purpose of this survey was to evaluate the prevalence of tobaccoism in a representative sample of hospital employees and its association with age, sex, profession level and attitudes. METHODS: Following a pilot study, a survey was carried out amongst a representative and random sample (n=360) of hospital personnel, who were asked about their attitudes towards smoking, classified according to age, sex and professional level. RESULTS: The total percentage of smokers was 36.4%, but no significant statistical differences were found between sex or age group. The professional levels with the highest percentage of smokers are ATS/DUE and clerks. 42.8% of non-smokers declared having smoked in the past. ATS/DUE, clerks and doctors showed the highest percentage. 40.5% of smokers declare that they were willing to give up smoking, whereas people over 50 were the most reluctant. The majority of cigarette smokers admitted that they smoke in the hospital. Most of them are intermediate smokers (10-20 cigarettes per day) and have been so for many years. The majority of former smokers gave up smoking in recent years. CONCLUSIONS: These results prove that a large number of hospital employees are smokers. However, many are willing to give up the habit. We should therefore encourage these people to give up smoking and help them by different means such as: anti-smoking advice, medical advice and health education. We propose to declare hospitals as A Non-Smoking Area as of the year 2000 which includes employees, patients and visitors, according to the WHO AHealth for All program., FUNDAMENTO: Sobre tabaquismo se han realizado muchos estudios tanto en España como en otros países. El objetivo de esta encuesta es valorar la prevalencia de tabaquismo en una muestra representativa de trabajadores del hospital y su asociación con la edad, sexo, estamento profesional y actitudes. MÉTODOS: Después de un estudio piloto, se realizó una encuesta en una muestra representativa y aleatoria (n=360) de trabajadores del hospital, a los que se les preguntó por sus actitudes al hábito tabáquico, clasificados por edad, sexo y estamentos. RESULTADOS: El porcentaje total de fumadores es de 36.4%, pero no se han encontrado diferencias estadísticamente significativas ni por sexo ni grupo de edad. Los estamentos con mayor porcentaje de fumadores son ATS/DUE y Administrativos. El 42.8% de los no fumadores declaran haberlo hecho antes, siendo los estamentos de ATS/DUE, Administrativos y Facultativos los que mostraron el mayor porcentaje. Un 40.5% de fumadores declaran estar dispuestos a dejar el hábito, mientras el grupo de personas mayores de 50 años se muestran más resistentes. La mayoría de fumadores de cigarrillos señalan que fuman en el hospital. La mayor parte de ellos son fumadores intermedios (10-20 cigarrillos/día) y lo llevan haciendo muchos años. La mayor parte de los exfumadores declaran haber dejado el hábito en los últimos años. CONCLUSIONES: Estos resultados demuestran que un gran porcentaje de trabajadores del hospital son fumadores. Sin embargo, existe un importante número que está dispuestos a dejarlo. Por ello, animamos a estas personas a que dejen de fumar y ayudarles por medio de diversas acciones tales como: consulta de tratamiento del tabaquismo, consejo médico y educación para la salud. Proponemos declarar al hospital como "HOSPITAL SIN TABACO" para el año 2000, lo cual incluye a trabajadores, pacientes y visitas, de acuerdo con la estrategia de la OMS "Salud para todos".

Current hand eczema was analyzed in 142 wet hospital workers from this cohort with respect to the etiologic importance of irritants, allergens and contact urticants. The density of the microflora and the effect on the microflora of topical treatment with a potent corticosteroid were studied in 20 patients with hand eczema. ’Wet’ hospital work was found to increase the odds of developing hand eczema only twice compared to 'dry' office work. Nursing children under four years old and the lack of a dish-washing machine significantly increased the risk of contracting hand eczema. Unfavourable combinations of these domestic factors increased the risk as much as wet work. A history of atopic dermatitis approximately tripled the odds both in wet as well as in dry work. Histories of earlier hand eczema (HHE), metal dermatitis (HMD) and of atopy were analyzed as risk factors for hand eczema in 1857 women in wet work. HHE increased the odds by a factor of 12.9 and created a subdivision of the population into high risk individuals and normal risk individuals. HHE was found in half of the subjects with atopic dermatitis, in one quarter of the subjects with atopic mucosal symptoms and in one fifth of the non-atopics. A HMD increased the odds by a factor of 1.8. This increase was seen as a high risk level in subjects with HHE and as a normal risk level in subjects with no HHE. A history of atopic disease as a complement to information about HHE and HMD increased the odds by another 1.3 times. The predicted probability of developing hand eczema ranged from 91 % in subjects with a combination of HHE, HMD and atopy to 24% in subjects with none of these risk factors.
