- Abstract
Background: Upfront primary tumor resection (PTR) has been associated with longer overall survival (OS) in patients with synchronous unresectable metastatic colorectal cancer (mCRC) in retrospective analyses. The aim of the CAIRO4 study was to investigate whether the addition of upfront PTR to systemic therapy resulted in a survival benefit in patients with synchronous mCRC without severe symptoms of their primary tumor. Patients and methods: This randomized phase III trial was conducted in 45 hospitals in The Netherlands and Denmark. Eligibility criteria included previously untreated mCRC, unresectable metastases, and no severe symptoms of the primary tumor. Patients were randomized (1 : 1) to upfront PTR followed by systemic therapy or systemic therapy without upfront PTR. Systemic therapy consisted of first-line fluoropyrimidine-based chemotherapy with bevacizumab in both arms. Primary endpoint was OS in the intention-to-treat population. The study was registered at ClinicalTrials.gov, NCT01606098. Results: Between August 2012 and February 2021, 206 patients were randomized. In the intention-to-treat analysis, 204 patients were included (n = 103 without upfront PTR, n = 101 with upfront PTR) of whom 116 were men (57%) with median age of 65 years (interquartile range 59-71 years). Median follow-up was 69.4 months. Median OS in the arm without upfront PTR was 18.3 months (95% confidence interval 16.0-22.2 months) compared with 20.1 months (95% confidence interval 17.0-25.1 months) in the upfront PTR arm (P = 0.32). The number of grade 3-4 events was 71 (72%) in the arm without upfront PTR and 61 (65%) in the upfront PTR arm (P = 0.33). Three deaths (3%) possibly related to treatment were reported in the arm without upfront PTR and four (4%) in the upfront PTR arm. - Abstract
Background: Many rare genetic neurodevelopmental disorders (RGNDs) are characterized by intellectual disability (ID), severe cognitive and behavioral impairments, potentially diagnosed as a comorbid autism spectrum disorder or attention-deficit hyperactivity disorder. Quality of life is often impaired due to irritability, aggression and self-injurious behavior, generally refractory to standard therapies. There are indications from previous (case) studies and patient reporting that cannabidiol (CBD) may be an effective treatment for severe behavioral manifestations in RGNDs. However, clear evidence is lacking and interventional research is challenging due to the rarity as well as the heterogeneity within and between disease groups and interindividual differences in treatment response. Our objective is to examine the effectiveness of CBD on severe behavioral manifestations in three RGNDs, including Tuberous Sclerosis Complex (TSC), mucopolysaccharidosis type III (MPS III), and Fragile X syndrome (FXS), using an innovative trial design. Methods: We aim to conduct placebo-controlled, double-blind, block-randomized, multiple crossover N-of-1 studies with oral CBD (twice daily) in 30 patients (aged ≥ 6 years) with confirmed TSC, MPS III or FXS and severe behavioral manifestations. The treatment is oral CBD up to a maximum of 25 mg/kg/day, twice daily. The primary outcome measure is the subscale irritability of the Aberrant Behavior Checklist. Secondary outcome measures include (personalized) patient-reported outcome measures with regard to behavioral and psychiatric outcomes, disease-specific outcome measures, parental stress, seizure frequency, and adverse effects of CBD. Questionnaires will be completed and study medication will be taken at the participants’ natural setting. Individual treatment effects will be determined based on summary statistics. - Abstract
Background:Adherence to antihypertensive drugs (AHDs) is crucial for controlling blood pressure (BP). We aimed to determine the effectiveness of measuring AHD concentrations using a dried blood spot (DBS) sampling method to identify nonadherence, combined with personalized feedback, in reducing resistant hypertension.Methods:We conducted a multicenter, randomized, controlled trial (RHYME-RCT, ICTRP NTR6914) in patients with established resistant hypertension. Patients were randomized to receive either an intervention with standard of care (SoC) or SoC alone. SoC consisted of BP measurement and DBS sampling at baseline, 3 months (t3), 6 months (t6), and 12 months (t12); AHD concentrations were measured but not reported in this arm. In the intervention arm, results on AHD concentrations were discussed during a personalized feedback conversation at baseline and t3. Study endpoints included the proportion of patients with RH and AHD adherence at t12.Results:Forty-nine patients were randomized to receive the intervention+SoC, and 51 were randomized to receive SoC alone. The proportion of adherent patients improved from 70.0 to 92.5% in the intervention+SoC arm (P = 0.008, n = 40) and remained the same in the SoC arm (71.4%, n = 42). The difference in adherence between the arms was statistically significant (P = 0.014). The prevalence of resistant hypertension decreased to 75.0% in the intervention+SoC arm (P < 0.001, n = 40) and 59.5% in the SoC arm (P < 0.001, n = 42) at t12; the difference between the arms was statistically nonsignificant (P = 0.14).Conclusion:Personalized feedback conversations based on DBS-derived AHD concentrations improved AHD adherence but did not reduce the prevalence of RH.

BACKGROUND: Poor diet, alcohol use, and tobacco smoking have been identified as strong determinants of chronic diseases, such as cardiovascular disease, diabetes, and cancer. Smartphones have the potential to provide a real-time, pervasive, unobtrusive, and cost-effective way to measure these health behaviors and deliver instant feedback to users. Despite this, the validity of using smartphones to measure these behaviors is largely unknown. OBJECTIVE: The aim of our review is to identify existing smartphone-based approaches to measure these health behaviors and critically appraise the quality of their measurement properties. METHODS: We conducted a systematic search of the Ovid MEDLINE, Embase (Elsevier), Cochrane Library (Wiley), PsycINFO (EBSCOhost), CINAHL (EBSCOHost), Web of Science (Clarivate), SPORTDiscus (EBSCOhost), and IEEE Xplore Digital Library databases in March 2020. Articles that were written in English; reported measuring diet, alcohol use, or tobacco use via a smartphone; and reported on at least one measurement property (eg, validity, reliability, and responsiveness) were eligible. The methodological quality of the included studies was assessed using the Consensus-Based Standards for the Selection of Health Measurement Instruments Risk of Bias checklist. Outcomes were summarized in a narrative synthesis. This systematic review was registered with PROSPERO, identifier CRD42019122242. RESULTS: Of 12,261 records, 72 studies describing the measurement properties of smartphone-based approaches to measure diet (48/72, 67%), alcohol use (16/72, 22%), and tobacco use (8/72, 11%) were identified and included in this review. Across the health behaviors, 18 different measurement techniques were used in smartphones. The measurement properties most commonly examined were construct validity, measurement error, and criterion validity. - Abstract
Background: Double-layer compared to single-layer closure of the uterus after a caesarean section (CS) leads to a thicker myometrial layer at the site of the CS scar, also called residual myometrium thickness (RMT). It possibly decreases the development of a niche, which is an interruption of the myometrium at the site of the uterine scar. Thin RMT and a niche are associated with gynaecological symptoms, obstetric complications in a subsequent pregnancy and delivery and possibly with subfertility. Methods: Women undergoing a first CS regardless of the gestational age will be asked to participate in this multicentre, double blinded randomised controlled trial (RCT). They will be randomised to single-layer closure or double-layer closure of the uterine incision. Single-layer closure (control group) is performed with a continuous running, unlocked suture, with or without endometrial saving technique. Double-layer closure (intervention group) is performed with the first layer in a continuous unlocked suture including the endometrial layer and the second layer is also continuous unlocked and imbricates the first. The primary outcome is the reported number of days with postmenstrual spotting during one menstrual cycle nine months after CS. Secondary outcomes include surgical data, ultrasound evaluation at three months, menstrual pattern, dysmenorrhea, quality of life, and sexual function at nine months. Structured transvaginal ultrasound (TVUS) evaluation is performed to assess the uterine scar and if necessary saline infusion sonohysterography (SIS) or gel instillation sonohysterography (GIS) will be added to the examination. Women and ultrasound examiners will be blinded for allocation. Reproductive outcomes at three years follow-up including fertility, mode of delivery and complications in subsequent deliveries will be studied as well. - Abstract
Internet intervention research has been well established in the literature over the past two decades. The hundreds, if not thousands, of high quality research trials that have been conducted and published have convincingly demonstrated feasibility, efficacy and even effectiveness. The next decade will be a critical one for the field with numerous obstacles, barriers, disputes and debates to overcome. This talk will highlight many of the known challenges and controversies as well as key areas that will need to be part of future research. Furthermore, each of these areas overlap, resulting in additional topics of needed focus. Challenges include funding, recruitment, and conflict of interests to manage; controversies include best practices for iterative development, the relationship between usage and outcome, and public concerns of technology replacing jobs; issues that intersect with both challenges and controversies include how best to address the digital divide, the use of social networking, and how to determine the best control and comparison groups in ehealth research; future issues include a need to focus on deconstruction and dissemination trials, internationalization of interventions, and a greater emphasis on cost effectiveness; issues that traverse challenges and the future include a need for greater scientific underpinnings to interventions, improving the taxonomy and overall definitions used to define applications, and how best to maintain and sustain programs; controversies/future issues include the problem of so many programs continuing to be developed and made available with little to no scientific validity; and finally, issues that cross challenges, controversies and the future include the ongoing discussions of support, meta-analyses, and stepped care. Although not comprehensive, this list will provide a useful start for discussion and help guide research for the coming years.

Background Anxiety scales may help primary care physicians to detect specific anxiety disorders among the many emotionally distressed patients presenting in primary care. The anxiety scale of the Four-Dimensional Symptom Questionnaire (4DSQ) consists of an admixture of symptoms of specific anxiety disorders. The research questions were: (1) Is the anxiety scale unidimensional or multidimensional? (2) To what extent does the anxiety scale detect specific DSM-IV anxiety disorders? (3) Which cut-off points are suitable to rule out or to rule in (which) anxiety disorders? Methods We analyzed 5 primary care datasets with standardized psychiatric diagnoses and 4DSQ scores. Unidimensionality was assessed through confirmatory factor analysis (CFA). We examined mean scores and anxiety score distributions per disorder. Receiver operating characteristic (ROC) analysis was used to determine optimal cut-off points. Results Total n was 969. CFA supported unidimensionality. The anxiety scale performed slightly better in detecting patients with panic disorder, agoraphobia, social phobia, obsessive compulsive disorder (OCD) and post traumatic stress disorder (PTSD) than patients with generalized anxiety disorder (GAD) and specific phobia. ROC-analysis suggested that ≥4 was the optimal cut-off point to rule out and ≥10 the cut-off point to rule in anxiety disorders. Conclusions The 4DSQ anxiety scale measures a common trait of pathological anxiety that is characteristic of anxiety disorders, in particular panic disorder, agoraphobia, social phobia, OCD and PTSD. The anxiety score detects the latter anxiety disorders to a slightly greater extent than GAD and specific phobia, without being able to distinguish between the different anxiety disorder types. The cut-off points ≥4 and ≥10 can be used to separate distressed patients in three groups with a relatively low, moderate and high probability of having one or more anxiety disorders.

This essay arises from an ongoing discussion about the teaching of Literature which followed after a 'critical friend', Bella Illesca spent a series of consecutive lessons observing the action in Prue Gill's Year 12 Literature class. By examining, interpreting and exploring the events of the classroom as students discussed the short stories of contemporary Australian writer, Beverley Farmer, we were lead to articulate our aims with teachers, our puzzles and our concerns in ways that helped each of us think afresh about teaching.

- Abstract
Results are presented on the production of K0S, hyperons and antihyperons in Pb-Pb and p-Be interactions at 40 GeV/c per nucleon. The enhancement pattern follows the same hierarchy as seen in the higher energy data - the enhancement increases with the strangeness content of the hyperons and with the centrality of collision. The centrality dependence of the Pb-Pb yields and enhancements is steeper at 40 than at 158 A GeV/c. The energy dependence of strangeness enhancements at mid-rapidity is discussed. © 2010 IOP Publishing Ltd.

Wireless networks equipped with the CSMA protocol are subject to collisions due to interference. For a given interference range we investigate the tradeoff between collisions (hidden nodes) and unused capacity (exposed nodes). We show that the sensing range that maximizes throughput critically depends on the activation rate of nodes. For infinite line networks, we prove the existence of a threshold: When the activation rate is below this threshold the optimal sensing range is small (to maximize spatial reuse). When the activation rate is above the threshold the optimal sensing range is just large enough to preclude all collisions. Simulations suggest that this threshold policy extends to more complex linear and non-linear topologies.

- Abstract
Multi-access networks may exhibit severe unfairness in throughput. Recent studies show that this unfairness is due to local differences in the neighborhood structure: Nodes with less neighbors receive better access. We study the unfairness in saturated linear networks, and adapt the multi-access CSMA protocol to remove the unfairness completely, by choosing the activation rates of nodes appropriately as a function of the number of neighbors. We then investigate the consequences of this choice of activation rates on the network-average saturated throughput, and we show that these rates perform well in a non-saturated setting.

- Abstract
The transverse mass spectra of K-S(0), Lambda, Xi and Omega particles produced in Pb-Pb collisions at 40AGeV/c have been studied for a sample of events corresponding to the most central 53% of the inelastic Pb-Pb cross section. We analyse the distributions in the framework of a parameterized model inspired by hydrodynamics. The dependence of the freeze-out parameters on particle species and event centrality is discussed and comparisons with results at higher energy are shown.

- Abstract
Results are presented on hyperon and antihyperon production in Pb-Pb, pPb and pBe collisions at 158 GeV/c per nucleon. Lambda, Xi and Omega yields have been measured at central rapidity and medium transverse momentum as functions of the centrality of the collision. Comparing the yields in Pb-Pb to those in pBe interactions, strangeness enhancement is observed. The enhancement increases with the centrality and with the strangeness content of the hyperons, reaching a factor of about 20 for the Omega in the central Pb-Pb collisions.

- Abstract
We present central-to-peripheral nuclear modification factors, R-CP, for the (PT) distributions of K-S(0), Lambda, Lambda, and negatively charged particles, measured at central rapidity in Pb-Pb collisions at top SPS energy. The data cover the 55% most central fraction of the inelastic cross section. The K-S(0) and Lambda R-CP((PT)) are similar in shape to those measured at root sNN = 200 GeV at RHIC, though they are larger in absolute value. We have compared our K-S(0) R-CP data to a theoretical calculation. The prediction overestimates the data at (PT) approximate to 3-4 GeV/c, unless sizeable parton energy loss is included in the calculation. (c) 2005 Elsevier B.V. All rights reserved.

- Abstract
The multiplicity of charged particles in the central rapidity region has been measured by the NA57 experiment in Pb-Pb collisions at the CERN SPS at two beam momenta: 158 A GeV/c and 40 A GeV/c. The value of dN(ch)/d eta at the maximum has been determined and studied as a function of centrality within the range covered by NA57 (about 55% most central part of the inelastic cross section). The central rapidity multiplicity varies roughly linearly with the number of wounded nucleons (participants) and increases approximately logarithmically with the centre of mass energy.

- Abstract
Enhancements of strange baryon and anti-baryon yields were first observed in Pb-Pb interactions at 160 A GeV / c by WA97. The aim of the NA57 experiment is to investigate how these enhancements behave as a function of energy, and over a broader centrality range than that which was available with WA97. This paper presents a comparison of the hyperon yields in Pb-Pb interactions at 40 A GeV / c and 160 A GeV / c. Preliminary signals for the p-Be data at 40 A GeV / c are also shown.
