Students’ identification with schools or universities predicts many positive aspects such as cooperation and completion of studies. Two hundred and eleven Italian students and 226 U.S. students wrote responses to open-ended questions about a time that they either felt connected (identified) or disconnected to (not identified) with their university. In Italy, students prompted to identify with the university mainly focused their descriptions on didactical aspects such as academic training experiences. Italian students prompted not to identify with the university mainly described their experiences with the university’s organizational and administrative aspects. U.S. students prompted to identify or not with the university mainly focused on the social aspects of their university experience such as their involvement with fraternities and sororities. In both nations, students rarely mentioned instrumental aspects of their university experience such as the evaluation of how much the university matters for professional training. These qualitative results help to fully understand the contents of the experience of university students. Understanding students’ identification can be vital in order to improve the quality of university teaching and to support professors in developing their roles., La identificación de los estudiantes con las escuelas o universidades se asocia con muchos resultados positivos, como el aumento de la cooperación y la finalización de los estudios. Este estudio es una investigación cualitativa de las experiencias de los estudiantes universitarios en relación con la identificación. n our role as medical educators and researchers, we support in the strongest manner possible the personal autonomy and self-determination of our students, patients, and colleagues. A fundamental value of the medical profession is for the patient to have personal autonomy in their healthcare decisions, including how they would like to be identified. It is, and will continue to be, an important priority to be taught and encouraged throughout healthcare education,1 and it begins in the anatomy laboratory with the proper and respectful treatment of body donors.2 Learning this value continues with respecting and accepting this same autonomy in fellow students, staff, faculty, and patients.

We developed an American Sign Language (ASL) learning platform in a Virtual Reality (VR) environment to facilitate immersive interaction and real-time feedback for ASL learners. We describe the first game to use an interactive teaching style in which users learn from a fluent signing avatar and the first implementation of ASL sign recognition using deep learning within the VR environment. Advanced motion-capture technology powers an expressive ASL teaching avatar within an immersive three-dimensional environment. The teacher demonstrates an ASL sign for an object, prompting the user to copy the sign. Upon the user's signing, a third-party plugin executes the sign recognition process alongside a deep learning model. Depending on the accuracy of a user's sign production, the avatar repeats the sign or introduces a new one. We gathered a 3D VR ASL dataset from fifteen diverse participants to power the sign recognition model. The proposed deep learning model's training, validation, and test accuracy are 90.12%, 89.37%, and 86.66%, respectively. The functional prototype can teach sign language vocabulary and be successfully adapted as an interactive ASL learning platform in VR., Comment: 36 pages, 9 figures

IntroductionInfants with SCID are treated with hematopoietic cell transplantation (HCT) or gene therapy (GT). Caregiver perceptions of pre-treatment counseling and understanding of durability of HCT/GT are poorly understood.MethodsA survey was designed and distributed to families of patients with SCID. Topics in the questionnaire included SCID genotype and treatment, family recollections of pre-treatment counseling and present clinical status.Results151 surveys were analyzed. 132 were treated with HCT, 19 with GT. From counseling received, 37% expected HCT/GT would lead to "cure"; 43% expected HCT/GT would last a lifetime. Of 136 living patients, 59% reported overall good health but 65% reported some persistent health challenges.ConclusionsFor some, interpretation of the word "cure" varied, leading to misunderstanding regarding need for continued medical evaluations and additional therapies. Clear communication regarding the importance of lifelong follow-up, no matter the treatment outcome, will help to optimize good health and quality of life.

The geographic spatial and stratigraphic distributions of Uinta Basin E. callopyge and E. uintensis specimens were compared using ArcGIS and analysis of variance (ANOVA). The analysis revealed overlapping geographic distributions of these two species, and no significant differences in stratigraphic dispersal. This finding of extensive geospatial overlap between the two Uintan Echmatemys species highlights the need for accurate taxonomic identification, such as the gular scale morphology validated here. In addition, we sought to address a methodological question regarding the relative efficacy of data complexity in this context. Using epiplastra from three additional Eocene species of Echmatemys, we employed hierarchical analyses of increasing data complexity, from standard linear dimensions to 2D geometric morphometrics to 3D laser scans, to determine the degree to which data complexity contributes to taxonomic assessments within this genus. Uintan species E. callopyge and E. uintensis were found to differ significantly in epiplastral shape as captured by all three categories of data. Apps for depression have the potential to innovate mental health care and increase access to treatment. Yet, concerns abound with disparities between academic development of apps and those available direct-to-consumers through the app marketplace. Reviews have highlighted ethical shortcomings of these self-management tools, with a need for greater insight into how ethical issues may be experienced by users. We addressed these gaps by exploring user reviews of apps for depression to better understand user experiences and ethical issues. We conducted a thematic analysis of 2,217 user reviews sampled from 40 depression apps in Google Play and Apple App Store, totalling over 77,500 words. Users reported both positive and negative experiences, with ethical implications evident in areas of benefits, adverse events, access, usability and design, support, commercial models, autonomy, privacy, and transparency. We conclude by presenting an ethical framework for developing apps for depression and navigating their ethical tensions.

ObjectivesFew surveys have focused on physician moral distress, burnout, and professional fulfilment. We assessed physician wellness and coping during the COVID-19 pandemic.DesignCross-sectional survey using four validated instruments.SettingSixty-two sites in Canada and the United States.SubjectsAttending physicians (adult, pediatric; intensivist, nonintensivist) who worked in North American ICUs.InterventionNone.Measurements and main resultsWe analysed 431 questionnaires (43.3% response rate) from 25 states and eight provinces. Respondents were predominantly male (229 [55.6%]) and in practice for 11.8 ± 9.8 years. Compared with prepandemic, respondents reported significant intrapandemic increases in days worked/mo, ICU bed occupancy, and self-reported moral distress (240 [56.9%]) and burnout (259 [63.8%]). Of the 10 top-ranked items that incited moral distress, most pertained to regulatory/organizational ( n = 6) or local/institutional ( n = 2) issues or both ( n = 2). Average moral distress (95.6 ± 66.9), professional fulfilment (6.5 ± 2.1), and burnout scores (3.6 ± 2.0) were moderate with 227 physicians (54.6%) meeting burnout criteria. A significant dose-response existed between COVID-19 patient volume and moral distress scores. Physicians who worked more days/mo and more scheduled in-house nightshifts, especially combined with more unscheduled in-house nightshifts, experienced significantly more moral distress. One in five physicians used at least one maladaptive coping strategy. We identified four coping profiles (active/social, avoidant, mixed/ambivalent, infrequent) that were associated with significant differences across all wellness measures.ConclusionsDespite moderate intrapandemic moral distress and burnout, physicians experienced moderate professional fulfilment. Subjective wealth was assessed by individuals’ perceptions of their personal wealth (current/anticipated) and of their country´s wealth, whereas objective country-level wealth was assessed by GDP and HDI. Multilevel analyses showed that living in a country with a lower objective wealth and perceiving the country's relative wealth as low were associated with higher levels of perceived realistic threat. We also found that an anticipated decrease in personal wealth in the future was associated with higher threat perceptions only in low HDI countries. Our results suggest that perceived realistic threat is fostered by a perceived decline in the current wealth of the country, and country-level wealth may play a role in calibrating threat responses to anticipated personal wealth.

Human cadaveric donors are essential for research in the anatomical sciences. However, many research papers in the anatomical sciences often omit a statement regarding the ethical use of the donor cadavers or, as no current standardized versions exist, use language that is extremely varied. To rectify this issue, 22 editors-in-chief of anatomical journals, representing 17 different countries, developed standardized and simplified language that can be used by authors of studies that use human cadaveric tissues. The goal of these editor recommendations is to standardize the writing approach by which the ethical use of cadaveric donors is acknowledged in anatomical studies that use donor human cadavers. Using Parker Palmer’s The Courage to Teach, and in particular the notion of the undivided life, to guide reflections through the process of collaborative autoethnography, we reflect on our lived experiences with the Scholarship of Teaching and Learning (SoTL). The central question being: How does Palmer’s idea of the undivided life enable SoTL scholars to explore notions of identity and integrity that are intertwined with our academic practice? Ultimately, we found that Palmer’s insights provoked us to think deeply about our identities, and while perhaps we did not always see ourselves on the paths he illuminates, his work, and our collaborative ethnographic process, helped us to illuminate our own paths. More specifically, we share five themes arising from our collaborative autoethnography related to the importance of context and positionality, defining a SoTL scholar, the power to diminish, the importance of relationships and community, and collaborative autoethnography as method and process. Our stories highlight the need for us to see our community as complex, messy, and deeply human, and we remind readers of the need to think about the ethics of all methods and the power in our everyday practice to include or exclude.

There is evidence that democracies are under threat around the world while the quest for strong leaders is increasing. Although the causes of these developments are complex and multifaceted, here we focus on one factor: the extent to which citizens express materialist and post-materialist concerns. We explore whether objective higher levels of democracy are differentially associated with materialist and post-materialist concerns and, in turn, whether this is related to the wish for a strong leader. Testing this hypothesis across 27 countries (N = 5,741) demonstrated a direct negative effect of democracies’ development on the wish for a strong leader. Further, multi-level mediation analysis showed that the relation between the Democracy Index and the wish for a strong leader was mediated by materialist concerns. This pattern of results suggests that lower levels of democracy are associated with enhanced concerns about basic needs and this is linked to greater support for strong leaders., info:eu-repo/semantics/published

This position paper addresses the continued ethical challenges in mobile mental health and the need for transdisciplinary ethical principles and standards to facilitate the development of ethically designed mental health technologies. By comparing and synthesising ethical codes of conduct across disciplines in digital mental health – namely psychology, healthcare, human computer interaction, computer science, and engineering – we suggest transdisciplinary ethical principles and standards to facilitate the development of ethically designed mental health technologies. These preliminary findings form part of a larger research project which seeks to develop a transdisciplinary approach to the ethical design, marketing, and implementation of mental health technologies.

Background: There is great promise in the use of apps for the treatment of depression. Yet, there is uncertainty as to the alignment of publicly available depression apps with clinical guidance, their treatment fidelity and evidence–base, and their overall safety. Objectives: This study built on previous analyses and reviews to explore treatment and safety issues of publicly available apps for depression. Methods: We conducted a content analysis of apps for depression in the two main UK apps stores (Google Play and Apple iTunes). App store listings were analyzed for intervention content, treatment fidelity, and fit with the National Institute for Health and Care Excellence (NICE) guidelines for the treatment of depression in adults. Results: A total of 353 apps for depression were included in the review. App descriptions reported use of 20 treatment approaches and 37 treatment strategies. Many apps used transdiagnostic (155/353, 43.9%) and multitheoretical interventions to treat multiple disorders including depression. While many interventions appeared to be evidence–informed, there were issues with treatment fidelity, research evidence, and fit with clinical guidelines. None of the apps aligned with NICE guidelines for depression. Conclusions: App developers have adopted many evidence–informed treatments in their interventions, but more work is needed to improve the clinical validity, treatment fidelity, and safety of apps. We urge developers to consult relevant guidelines and standards, and to engage in reflective questioning on treatment and safety to address these issues and to improve treatment content and intervention design.

Translanguaging claims to advance social justice as a transformative pedagogy. This paper analyses a tension which developed over the life span of a European research project which aimed to improve the educational experience for Eastern European Roma pupils through teachers’ employment of a translanguaging pedagogy. Roma are ethnically and linguistically heterogeneous, but as a minority group face continued racism, whilst Roma pupils face educational exclusion. The voices of Roma parents, pupils and activists and academics alerted us to potential threats in utilising translanguaging as a political act for transformation in education. They revealed a central tension between recognition of linguistic pluralism for emancipation at school level (with possibilities for policy level changes at local or national levels) and unifying endeavours for collective action towards equality and human rights at a (trans)national level. To understand this tension we reframed it in light of the postmodernist positioning of translanguaging, and critiques of the de-politicizing tendencies of postmodernism. In proposing a way forward for research and pedagogy, we carve a path between pluralism and collectivism by placing translanguaging pedagogy and associated research into Nancy Fraser’s integrative model of recognition and redistribution for transformation.

Impulsive sounds generated during seismic surveys have elicited behavioral responses in marine mammals and could cause hearing impairment or injury. Mitigating exposure to seismic sound often relies on real-time marine mammal detection. Detection performance is influenced by detection method, environmental conditions, and observer experience. We conducted a field comparison of real-time detections made by marine mammal observers (MMOs), a rotating infrared (IR) camera, and via passive acoustic monitoring (PAM). Data were collected from a 38 m research vessel offshore Atlantic Canada. Our results indicate that overall detection rates increase when complementary methods are used. MMOs and PAM are likely the most effective combination during high seas and precipitation. PAM and IR can be used in darkness. In good visibility, MMOs with IR or PAM should increase detections. Our results illustrate the importance of addressing false positive IR detections, matching system capabilities to sea conditions/species of interest, and employing experienced observers.

Mobile mental health presents many ethical challenges in the wild. These ethical issues and associated values were explored through a content analysis and ethical review of the depression app marketplace. App search and data collection was performed in Google Play Store (UK) and Apple iTunes (UK) between October to November 2018. Iterative data extraction and coding of ethical variables and values were conducted prior to synthetization of issues and themes. Search found 353 unique apps for depression. Analysis uncovered a range of ethical issues including: limited evidence of intervention validity, fidelity, and outcomes; insufficient safeguarding and duty of care; non-multisector development teams; lack of independent certification and regulation; lack of information and transparency for informed user choices; and concerns with privacy, confidentiality, and user permissions. These findings highlighted the presence and absence of ethical values in apps for depression, with most apps failing to reflect many key values. Our findings suggest a need for greater ethical value sensitive design in mobile mental health. This is challenging given the field’s multidisciplinarity and value conflicts. We encourage designers to adopt a responsible innovation approach to creating technologies that meet these ethical demands.

OBJECTIVE:The SI gene encodes the sucrase-isomaltase enzyme, a disaccharidase expressed in the intestinal brush border. Hypomorphic SI variants cause recessive congenital sucrase-isomaltase deficiency (CSID) and related gastrointestinal (GI) symptoms. Among children presenting with chronic, idiopathic loose stools, we assessed the prevalence of CSID-associated SI variants relative to the general population and the relative GI symptom burden associated with SI genotype within the study population. METHODS:A prospective study conducted at 18 centers enrolled 308 non-Hispanic white children ≤18 years old who were experiencing chronic, idiopathic, loose stools at least once per week for >4 weeks. Data on demographics, GI symptoms, and genotyping for 37 SI hypomorphic variants were collected. Race/ethnicity-matched SI data from the Exome Aggregation Consortium (ExAC) database was used as the general population reference. RESULTS:Compared with the general population, the cumulative prevalence of hypomorphic SI variants was significantly higher in the study population (4.5% vs. 1.3%, P < .01; OR = 3.5 [95% CI: 6.1, 2.0]). Within the study population, children with a hypomorphic SI variant had a more severe GI symptom burden than those without, including: more frequent episodes of loose stools (P < .01), higher overall stool frequency (P < .01), looser stool form (P = .01) and increased flatulence (P = .02). CONCLUSION:Non-Hispanic white children with chronic idiopathic loose stools have a higher prevalence of CSID-associated hypomorphic SI variants than the general population. The GI symptom burden was greater among the study subjects with a hypomorphic SI variant than those without hypomorphic SI variants.

This position paper addresses the continued ethical challenges in mobile mental health and the need for transdisciplinary ethical principles and standards to facilitate the development of ethically designed mental health technologies. By comparing and synthesising ethical codes of conduct across disciplines in digital mental health – namely psychology, healthcare, human computer interaction, computer science, and engineering – we suggest transdisciplinary ethical principles and standards to facilitate the development of ethically designed mental health technologies. These preliminary findings form part of a larger research project which seeks to develop a transdisciplinary approach to the ethical design, marketing, and implementation of mental health technologies.

An automatic stress detection system that uses unobtrusive smart bands will contribute to human health and wellbeing by alleviating the effects of high stress levels. However, there are a number of challenges for detecting stress in unrestricted daily life which results in lower performances of such systems when compared to semi-restricted and laboratory environment studies. The addition of contextual information such as physical activity level, activity type and weather to the physiological signals can improve the classification accuracies of these systems. We developed an automatic stress detection system that employs smart bands for physiological data collection. In this study, we monitored the stress levels of 16 participants of an EU project training every day throughout the eight days long event by using our system. We collected 1440 hours of physiological data and 2780 self-report questions from the participants who are from diverse countries. The project midterm presentations (see Figure 3) in front of a jury at the end of the event were the source of significant real stress. Different types of contextual information, along with the physiological data, were recorded to determine the perceived stress levels of individuals. We further analyze the physiological signals in this event to infer long term perceived stress levels which we obtained from baseline PSS- 14 questionnaires. Session-based, daily and long-term perceived stress levels could be identified by using the proposed system successfully.

Stress is an inescapable element of the modern age. Instances of untreated stress may lead to a reduction in the individual's health, well-being and socio-economic situation. Stress management application development for wearable smart devices is a growing market. The use of wearable smart devices and biofeedback for individualized real-life stress reduction interventions has received less attention. By using our unobtrusive automatic stress detection system for use with consumer-grade smart bands, we first detected stress levels. When a high stress level is detected, our system suggests the most appropriate relaxation method by analyzing the physical activity-based contextual information. In more restricted contexts, physical activity is lower and mobile relaxation methods might be more appropriate, whereas in free contexts traditional methods might be useful. We further compared traditional and mobile relaxation methods by using our stress level detection system during an eight day EU project training event involving 15 early stage researchers (mean age 28; gender 9 Male, 6 Female). Participants' daily stress levels were monitored and a range of traditional and mobile stress management techniques was applied. On day eight, participants were exposed to a 'stressful' event by being required to give an oral presentation. Insights about the success of both traditional and mobile relaxation methods by using the physiological signals and collected self-reports were provided.

BackgroundCaregivers of children with primary immunodeficiency disorders (PIDs) experience significant psychological distress during their child's hematopoietic cell transplantation (HCT) process.ObjectivesThis study aims to understand caregiver challenges and identify areas for health care system-level improvements to enhance caregiver well-being.MethodsIn this mixed-methods study caregivers of children with PIDs were contacted in August to November 2017 through online and electronic mailing lists of rare disease consortiums and foundations. Caregivers were invited to participate in an online survey assessing sociodemographic variables, the child's medical characteristics, psychosocial support use, and the World Health Organization-5 Well-Being Index. Open-ended questions about health care system improvements were included. Descriptive statistics and linear multivariate regression analyses were conducted. A modified content analysis method was used to code responses and identify emergent themes.ResultsAmong the 80 caregiver respondents, caregivers had a median age of 34 years (range, 23-62 years) and were predominantly female, white, and married with male children given a diagnosis of severe combined immune deficiency. In the adjusted regression model lower caregiver well-being was significantly associated with lower household income and medical complications. Challenges during HCT include maintaining relationships with partners and the child's healthy sibling or siblings, managing self-care, and coping with feelings of uncertainty. Caregivers suggested several organizational-level solutions to enhance psychosocial support, including respite services, online connections to other PID caregivers, and bedside mental health services.ConclusionsCertain high-risk subpopulations of caregivers might need more targeted psychosocial support to reduce the long-term effect of the HCT experience on their well-being. ObjectiveCaregivers for patients undergoing hematopoietic cell transplantation (HCT) are susceptible to significant psychosocial distress. This cross-sectional study aimed to describe psychosocial support services offered and used by caregivers of pediatric primary immune deficiency (PID) during HCT at 35 hospitals across North America.MethodCaregivers of pediatric patients with PID were recruited by e-mail to participate in an anonymous 140-question survey instrument between April and May 2016 (N = 171).ResultOf those meeting inclusion criteria (53%), family counseling services were only offered to fewer than half of caregivers (42%). Of the survey participants not offered counseling services, the majority desired family counseling (70%) and sibling counseling (73%). That said, when offered counseling, utilization rates were low, with 22% of caregivers using family counseling and none using sibling counseling.Significance of resultsThese results indicate the need to offer and tailor counseling services for families throughout the HCT process. Further research should focus on reducing barriers to utilization of counseling services such as offering bedside counseling services, online modalities, and/or financial assistance.
