This article-third in a series of three-uses theoretical frameworks described in Part 1, and empirical markers reported in Part 2, to present evidence on how power dynamics shifted during the early years of a major English community empowerment initiative. We demonstrate how the capabilities disadvantaged communities require to exercise collective control over decisions/actions impacting on their lives and health (conceptualized as emancipatory power) and the exercise of power over these communities (conceptualized as limiting power) were shaped by the characteristics of participatory spaces created by and/or associated with this initiative. Two main types of participatory spaces were identified: governance and sense-making. Though all forms of emancipatory power emerged in all spaces, some were more evident in particular spaces. In governance spaces, the development and enactment of 'power to' emerged as residents made formal decisions on action, allocated resources and managed accountability. Capabilities for alliance building-power with-were more likely to emerge in these spaces, as was residents' resistance to the exercise of institutional power over them. In contrast, in sense-making spaces residents met informally and 'made sense' of local issues and their ability to influence these. These processes led to the development of power within capabilities and power to resist stigmatizing forms of productive power. The findings highlight the importance of designing community initiatives that: nurture diverse participatory spaces; attend to connectivity between spaces; and identify and act on existing power dynamics undermining capabilities for collective control in disadvantaged communities.

Objectives: The objectives were to synthesise available evidence and produce new insights relating to initiatives that aim to address loneliness among these populations, plus the logic, functioning and effects of such initiatives. Data sources: Electronic database searches (MEDLINE, Applied Social Sciences Index and Abstracts and Social Science Citation Index via Web of Science – no date restrictions were applied), grey literature searches, and citation and reference searching were conducted. Data were generated via nine workshops with three consultation panels involving 34 public contributors, and one practitioner workshop involving 50 participants. Review methods: Guided by ‘systems thinking’, a theory-driven synthesis was combined with an effectiveness review to integrate evidence on the nature and causes of loneliness, interventional types and programme theory, and intervention implementation and effectiveness. Results: The theory review indicated that common conceptualisations of ‘loneliness’ can be usefully extended to recognise four proximate determinants when focusing on migrant and ethnic minority populations: positive social ties and interactions, negative social ties and interactions, self-worth, and appraisal of existing ties. A total of 170 interventions were included. A typology of eight interventions was developed. Detailed logic models were developed for three common types of intervention: befriending, shared-identity social support groups and intercultural encounters. The models for the first two types were generally well supported by empirical data; the third was more tentative. Evaluation of intervention processes and outcomes was limited by study content and quality. Purpose – The purpose of this paper is to explore the emergence of “Roma health and wellbeing” as a focus of attention in European research and in policy and the possible detrimental consequences of action founded on a generic representation of “Roma health.” Design/methodology/approach – Based on discussions with and research conducted by scholars who work directly with Roma communities across European regions from a wide range of academic disciplines it suggests how future research might inform: a more nuanced understanding of the causes of poor health and wellbeing among diverse Roma populations and; actions that may have greater potential to improve the health and wellbeing among these populations. Findings – In summary, the authors promote three types of research: first critical analyses that unpick the implications of current and past representations of “Roma” and “Roma health.” Second, applied participatory research that meaningfully involves people from specific self-defined Roma populations to identify important issues for their health and wellbeing. Third, learning about processes that might impact on the health and wellbeing of Roma populations from research with other populations in similarly excluded situations. Originality/value – The authors provide a multidisciplinary perspective to inform research that does not perpetuate further alienation and prejudice, but promotes urgent action to redress the social and health injustices experienced by diverse Roma populations across Europe.

A research prioritisation exercise was conducted across the network. Methods: Qualitative workshops (15 groups) and a Delphi survey (3 rounds, 368 members) were used to consult expert opinion and achieve a consensus. A further 10 workshops were conducted after the Delphi survey to triangulate the data. Results: Round one, 253 participants (n=190 participants from two sets of workshops; n=63 survey responses) answered open questions around priorities for action. In round two of the survey, 144 participants used a 5 point Likert scale to rate 39 items generated via thematic analysis of round one data. Round three: 76 participants (half of the round two participants) re-rated responses alongside median responses to each item. Poverty/implications of austerity (4.87m, IQR 0) remained the priority issue in all rounds, with long-term unemployment (4.8m, IQR 0) and mental health (4.7m, IQR 1) second and third priorities. Conclusions: A strong consensus amongst the practitioners and academics was that reducing health inequalities in the North of England requires prioritising research that tackles structural determinants concerning poverty, the implications of austerity measures and unemployment.

Public bodies have a legal duty to make 'reasonable adjustments' to policies and practices to provide fair access and treatment for people with ID. This study aimed to identify adjustments to the Slimming World weight management programme to improve accessibility and assess acceptability and feasibility for this population. METHODS: This user-centred qualitative study was carried out with a steering group of people with ID (n = 4). Barriers and facilitators to using Slimming World were identified through interviews and focus groups with people with ID (n = 54), carers (n = 12) current members with ID (n = 8) and Slimming World group leaders (n = 11). Adjustments were made and their feasibility and acceptability were explored in a before-and-after mixed methods study where people with ID attended Slimming World for eight weeks. Participants (n = 9), carers (n = 7) and Slimming World group leaders (n = 4) were interviewed to explore their experiences of the adjustments. Participants were weighed at baseline then each week. RESULTS: Four key adjustments were identified and addressed by Slimming World who developed prototype Easy Read materials and a letter for carers. Six of the nine participants attended Slimming World for eight weeks and lost weight (1.4 kg to 6.6 kg, reduction in BMI between 0.5 and 1.7 kg/m2), indicating that the adjustments were feasible and acceptable. Two participants dropped out because they felt uncomfortable in a mainstream group and another left because they lacked control over food choice in their residential setting. CONCLUSIONS: This user-centred approach identified reasonable adjustments that were feasible to implement. There is a need for greater conceptual clarity in place-based initiatives that seek to give residents of disadvantaged neighbourhoods more control over action to address the social determinants of health inequalities at a local level. In this article, we address this issue as it relates to the concepts of participation and inclusion. We draw on qualitative data generated during the first phase of the Communities in Control Study, a longitudinal multisite independent evaluation of the impact of Big Local on the social determinants of health and health inequalities. Big Local is a resident-led area improvement initiative in England, funded by the UK Big Lottery Fund. Initiatives focused on community empowerment are increasingly prominent in public health policy and practice globally. Approaches emphasise the promotion of greater control over decisions and action among individuals, groups, and communities, particularly those living in disadvantaged circumstances. However, when it comes to participation and inclusion in taking action and making decisions, the field is characterised by conceptual confusion. This risks undermining the impact of these initiatives. While participation and inclusion are necessary conditions for empowerment and collective control, they are not necessarily sufficient. Sufficiency requires attention to the breadth of participation (i.e., to inclusion) and to the depth of participation (i.e., the extent to which it is experienced as empowering and ultimately enables the exercise of collective control over decisions and actions). In observing how different Big Local resident-led partnerships across England are tackling the day-to-day challenges of engaging with their communities, we reveal the potential for policy and practice of reframing, and therefore clarifying (to highlight the different roles they have) the concepts of participation and inclusion in terms of depth and breadth.

Findings reveal which parts of the social system are likely to be changed, by what mechanisms, and with what implications. They also raise some salient considerations for knowledge generation and methods development in public health evaluation, particularly for the evaluation of social initiatives where change does not necessarily happen in linear or predictable ways. We suggest future evaluations of such initiatives require the use of more flexible designs, encompassing qualitative approaches capable of capturing the complexity of relational systems processes, alongside more traditional quantitative methods.

Health care services are increasingly utilizing reminder systems to manage these negative effects. This study explores the effectiveness of reminder systems for promoting attendance, cancellations, and rescheduling of appointments across all health care settings and for particular patient groups and the contextual factors which indicate that reminders are being employed sub-optimally. We used three inter-related reviews of quantitative and qualitative evidence. Firstly, using pre-existing models and theories, we developed a conceptual framework to inform our understanding of the contexts and mechanisms which influence reminder effectiveness. Secondly, we performed a review following Centre for Reviews and Dissemination guidelines to investigate the effectiveness of different methods of reminding patients to attend health service appointments. Finally, to supplement the effectiveness information, we completed a review informed by realist principles to identify factors likely to influence non-attendance behaviors and the effectiveness of reminders. We found consistent evidence that all types of reminder systems are effective at improving appointment attendance across a range of health care settings and patient populations. Background Interventions to develop community-level empowerment and participation are advocated within public health strategies that emphasise the importance of control as a social determinant of health. Approaches to defining and measuring collective control, however, are limited. We developed and applied a contextually specific framework for identifying the emergence of collective control in the early stage of a UK area-based community empowerment initiative. Methods Existing conceptualisations of collective control were used as a basis for development of a logic model, and then framework, for identifying emergent markers of control at the level of intracapability, intercapability, and extra group action. The concepts of power within, power to, and power with were used as overarching categories under which markers of collective control capability and action were identified. The framework was applied to qualitative data generated by our team for this primary analysis from in-depth case studies carried out in ten intervention neighbourhoods in England. Data included contextual area information, 150 interviews, ethnographic observation, participatory group exercises, and the review of 30 local intervention plans. Data were coded in NVivo (version 10) with a predefined thematic framework. Narrative memos were developed and compared and contrasted within and across sites. Ethics approval for the study was awarded by Lancaster University on behalf of Liverpool and Lancaster Universities Collaboration for Public Health Research, and by each individual research institution. Findings Application of the framework showed that collective control manifested in different ways and to varying degrees across field sites. We observed the emergence of new forms of group organising and decision making; the development of collective efficacy and linkages with other organisations; and the beginnings of community action taking place. Approaches to defining and measuring collective control, however, are limited. We developed and applied a contextually specific framework for identifying the emergence of collective control in the early stage of a UK area-based community empowerment initiative. Methods Existing conceptualisations of collective control were used as a basis for development of a logic model, and then framework, for identifying emergent markers of control at the level of intracapability, intercapability, and extra group action. The concepts of power within, power to, and power with were used as overarching categories under which markers of collective control capability and action were identified. The framework was applied to qualitative data generated by our team for this primary analysis from in-depth case studies carried out in ten intervention neighbourhoods in England. Data included contextual area information, 150 interviews, ethnographic observation, participatory group exercises, and the review of 30 local intervention plans. Data were coded in NVivo (version 10) with a predefined thematic framework. Narrative memos were developed and compared and contrasted within and across sites. Ethics approval for the study was awarded by Lancaster University on behalf of Liverpool and Lancaster Universities Collaboration for Public Health Research, and by each individual research institution. Findings Application of the framework showed that collective control manifested in different ways and to varying degrees across field sites. We observed the emergence of new forms of group organising and decision making; the development of collective efficacy and linkages with other organisations; and the beginnings of community action taking place.
